9. Successful Sperm Retrieval and Clinical Pregnancies Following Micro-TESE and ICSI Treatments in Patients with Nonobstructive Azoospermia Due to Various Etiologies.

Abstract

(1) Background: Nonobstructive azoospermia (NOA) etiologies affect the sperm retrieval rate (SRR) by microdissection testicular sperm extraction (micro-TESE) and the clinical outcomes following intracytoplasmic sperm injection (ICSI); (2) Methods: We investigated seven NOA etiologies. The SRR and clinical outcomes of 627 patients were analyzed between November 2017 and July 2022 in the Reproductive and Genetic Hospital of China International Trust and Investment Corporation-Xiangya (CITIC-Xiangya); (3) Results: The overall SRR was 39.4% (247/627). The SRR according to NOA etiologies were: Y chromosome azoospermia factor c microdeletions (26/46, 56.5%), Klinefelter syndrome (KS), 36/85, 42.4%), idiopathic (110/398, 27.6%), cryptorchidism (20/29, 69.0%), chromosome anomalies (7/13, 53.9%), orchitis (45/50, 90.0%), and cancer (3/6, 50.0%). The SRR were different for spermatogonia arrest (26/96, 27.1%), maturation arrest (76/177, 42.9%), and SCOS (30/80, 37.5%) according to histological examinations. The clinical pregnancy rate was similar among the NOA etiologies. The high-quality embryo rate differed between successful (54.7%) and unsuccessful (40.9%) pregnancies. Moreover, the successfully pregnant women (28.99 years) were younger than the unsuccessfully pregnant ones (30.92 years); (4) Conclusions: The SRR from patients with NOA was associated with the etiology and histological categories, while the clinical outcome was associated with the high-quality embryo rate and the female partner's age. In this study, we aimed to comprehensively delineate the testicular ERMS intra-tumoral heterogeneity and tumor microenvironment.Methods: Cell types and the corresponding marker genes were identified by single-nuclear RNA sequencing (snRNA-seq). Functional states of different clusters were evaluated by uniform manifold approximation and projection and differentially expressed genes. Kaplan–Meier curve analysis was constructed according to the gene expression profile to determine the correlation between candidate marker genes and the overall survival and disease-free survival of patients with osteosarcoma from TCGA.Results: A total of 8868 tumor cells and 10,147 normal cells were obtained from testicular ERMS tissues. The heterogeneous malignant subtype was composed of six subgroups (C1–C6) with differential proliferative and migratory potentials. Cell trajectory analysis revealed the C1 subgroup might be the starting cells of the tumor and transform into two different types of malignant cells, C2 and C5/6, during the development of RMS. The differentially expressed genes were closely related to cell adhesion and extracellular matrix signaling pathways. Furthermore, the interaction analysis between cell subgroups (macrophages and tumor cells, endothelial cells and tumor cells) demonstrated that collagen-related gene COL6A1 plays a key role from the initiation of ERMS to the entire process of malignant transformation.Conclusion: Our findings provide a new insight in the understanding of the initiation and progression of testicular ERMS and have potential value in the development of markers for the diagnosis and stratification of testicular ERMS.Keywords: embryonic rhabdomyosarcoma, intra-tumoral heterogeneity, biomarker, extracellular matrix, tumor microenvironment

Abstract

In lately December 2019, a novel coronavirus (SARS‐CoV‐2) outbreak occurred in Wuhan, PR China. It is a high contagious virus that has threatened human health worldwide. SARS‐CoV‐2 infection, termed COVID‐19, causes rapidly developing lung lesions that can lead to multiple organ failure in a short period. Whenever a novel virus emerges, reproductive risk assessments should be performed after infection. In this review, we show that male fertility might be damaged by coronavirus associated with (i) direct cytopathic effects derived from viral replication and viral dissemination in the testis; and (ii) indirect damage to male fertility derived from immunopathology. In this review, we briefly describe the impaired fertility of humans and animals infected with coronaviruses to deduce the impact of the new coronavirus on male fertility. Together with information related to other coronaviruses, we extrapolate this knowledge to the new coronavirus SARS‐CoV‐2, which may have a significant impact on our understanding of the pathophysiology of this new virus.

50. Analysis of the recruitment of human sperm bank in the post-epidemic period.

Abstract

Objective To explore the influence of COVID-19 epidemic on the recruitment of sperm donors, and to provide suggestion for sperm bank recruitment in the post-epidemic period. Methods The sperm donors for preliminary screening in Hunan Sperm Bank were statistically analyzed, and the changes in the recruitment and the qualified rate of sperm donors before and after the epidemic were analyzed, as well as the changes in the qualified rate of sperm donors of different populations in the post-epidemic period. Results The qualified rate, semen concentration and sperm motility of sperm donors decreased significantly after the epidemic, the qualified rate of sperm donors in age group 20~24, 25~29, 30 ~34 was significantly different before and after the epidemic. In the post-epidemic period, the qualified rate of sperm donors was not significantly affected by the donor's occupation, age and education background, but was significantly affected in different seasons. Conclusion The COVID-19 epidemic has a great impact on the recruitment of sperm donors. It is suggested that the donor recruitment of sperm bank in the post-epidemic period should be mainly comprised of college students, people aged 20-30, and people with college degree or above. In the season with suitable temperature, we should increase the publicity of sperm supply and pay attention to the mental health of sperm donors. [ABSTRACT FROM AUTHOR]
