Introduction: Left ventricular (LV) pseudoaneurysm is a rare but life-threatening complication following myocardial infarction (MI). Unlike true aneurysms, pseudoaneurysms lack myocardial elements in their structure, which increases the risk of rupture and sudden death. The diagnosis is challenging due to its often subtle and nonspecific symptoms. The high mortality rate associated with untreated pseudoaneurysms highlights the importance of awareness and prompt diagnosis. All patients after MI should receive specialized care and follow-up examinations to prevent unattended states of emergency. Purpose of research: This systematic review aims to discuss a rare complication of MI in a form of LV pseudoaneurysm. Due to its potential asymptomatic course, it is important to highlight the importance of a meticulous clinical and echocardiographic follow-up in all patients with a history of MI. Materials and methods: The study was conducted through search across PubMed and Google Scholar databases, using keywords such as "left ventricular pseudoaneurysm", "myocardial infarction", "post-infarction imagining", "surgical treatment of pseudoaneurysms". The analysis focused on scientific literature with validated insights into the pathophysiology, clinical presentation, diagnostic methods and treatment strategies of the MI complication in a form of left ventricular pseudoaneurysm. Results and conclusions: The potentially fatal consequences of pseudoaneurysm dictate proactive approach in the treatment of patients with a history of MI. Routine follow-up examinations, comprehensive cardiac imaging and a high clinical alertness are essential for early diagnosis. Pulmonary artery pseudoaneurysms are rare. We present a case of a 78-year old female who developed a pulmonary artery pseudoaneurysms causing hemoptysis after undergoing endovascular placement of a left atrial appendage occlusion device. The pulmonary arery pseudoaneurysm was treated with coil embolization.

Abstract Background Aortoiliac injury after intervertebral disc surgery is an uncommon but potentially fatal complication. This case study emphasizes how crucial it is to identify this uncommon complication, which can manifest itself in a variety of ways, including leg oedema, heart failure, and deep vein thrombosis. This case is unique in that it presents with an uncommon post-spinal surgery complication of fistulized pseudoaneurysm of the common iliac artery and vein. This combination calls for an immediate diagnosis and careful care. Case presentation A 36-year-old female patient presented with progressively worsening orthopnea, accompanied by chest pain and palpitations. Her medical history revealed gradually increasing right lower limb swelling since three months, following a discectomy procedure for the management of lower back pain associated with a herniated lumbar disc at the L5-S1 level. Doppler ultrasonography demonstrated an arterialized flow within the right common iliac vein. Subsequent computed tomography angiography identified a pseudoaneurysm of the right common iliac artery, which had formed a fistulous connection to the right common iliac vein. This uncommon complication was likely attributable to prior disc surgery. The patient received endovascular intervention, which was executed in two primary stages: first, embolization of the right internal iliac artery was performed, followed by the exclusion of the arteriovenous fistula through the deployment of a covered stent graft within the right common iliac artery. The intervention successfully resolved both the pseudoaneurysm and the arteriovenous fistula. Conclusions This case report underscores the importance of considering vascular complications in patients presenting with unusual symptoms following intervertebral disc surgery, even after a significant time lapse. While both open surgical and endovascular approaches can yield good results in treating such complications, the endovascular method, as demonstrated in this case, is associated with minimal morbidity. This approach offers a less invasive alternative that can effectively manage complex vascular injuries resulting from disc surgery. Clinicians should be aware of this rare but serious complication to ensure timely diagnosis and appropriate management, potentially improving patient outcomes in similar cases.

Abstract Background Extracranial internal carotid artery (ICA) pseudoaneurysm is a rare condition that can be caused either by penetrating or blunt trauma, including dog bites, which is an uncommon occurrence. Together with the possibility of no symptoms or nonspecific ones such as cervical pain, hematoma, swelling, or mass, considering ICA pseudoaneurysm following a dog attack is of paramount importance to avoid life-threatening complications. Case presentation We present a rare case of a 17-year-old male with a history of dog bites three months prior, who presented to the emergency department with left-sided neck pain, dizziness, and several episodes of blurred vision and diplopia. On physical examination, a palpable mass measuring approximately 20 × 30 millimeters was identified in the left neck region and multiple superficial lacerations were observed in this area. Laboratory tests yielded normal results. Doppler ultrasound revealed a pseudoaneurysm in the left internal carotid artery. Because the great saphenous veins were insufficient, the patient was successfully treated with synthetic graft patch arterioplasty, and no complications were seen in his one-year follow-up with computed tomography (CT) angiography. Conclusions This report emphasizes the significance of thorough initial evaluation and imaging in cases of dog attacks, even without apparent significant trauma, to rule out hidden arterial injuries.

Objective: The objective of this study is to demonstrate the sonographic and computed tomographic features of venous pseudoaneurysms and show the difference between true and false venous aneurysms and arterial pseudoaneurysms. Likewise, it was important to provide a historical context based on a literature review of vascular aneurysms and pseudoaneurysms, emphasizing venous pseudoaneurysms. Materials and Methods: Clinical cases are presented for context, and the vascular examinations were performed utilizing a General Electric (GE) Logiq Fortis or Logiq S8 scanners with a broad-band linear-array transducer (L2-9). Computed tomography (CT) with contrast was performed on a GE Revolution Maxima 64-slice CT scanner with 100 mL Omnipaque-350 as a contrast agent. The three-dimensional (3D) axial, sagittal, and coronal reconstructions were made with the Philips IntelliSpace software. Levels of evidence are provided based on a literature search of relevant publications using the National Library of Medicine, PubMed, and Google Books. Results: Different types of venous pseudoaneurysms, venous aneurysms, and arterial pseudoaneurysm diagnosed by duplex sonography and CT were presented. An extensive historical and literature review was conducted based on publications from the 18th to 21st centuries. Conclusion: Venous pseudoaneurysms are uncommon. Most of them are iatrogenic and are usually found as a complication of repetitive venipuncture in patients receiving hemodialysis with arteriovenous fistulas. This case report details the successful management of a patient who presented with a penetrating aortic ulcer and an extensive false aneurysm within the right pleura, employing an interdisciplinary approach involving cardiac surgeons, cardiologists, interventional cardiologists, and radiologists. The pivotal intervention involved the deployment of a covered and bare stent graft into the descending thoracic aorta to seal the aortic rupture. The patient’s clinical condition stabilized postoperatively, with no signs of recurrent hemorrhage. This case underscores the importance of rapid diagnosis, timely intervention, and the collaborative efforts of a specialized medical team in successfully managing such complex vascular injuries. Early recognition and referral to specialized centers are essential for improving patient outcomes in cases of penetrating aortic injuries with associated giant pseudoaneurysms.

Purpose: Pseudoaneurysm (PA) of the access artery is a common and potentially severe complication after percutaneous endovascular interventions. Most PA require interventional therapy, including thrombin injection, covered stent implantation, or surgical repair, which are costly and associated with other complications. Treating PA by external compression is uncomfortable and time consuming, with a low success rate. By injecting local tumescence anesthesia around the PA and close to its neck, the tumescence anesthesia and echo-guided compression technique (TACT) aims to improve efficacy and patient's comfort. Materials and Methods: We retrospectively reviewed all consecutive patients treated for femoral PA in our center from May 2009 to July 2019 and estimated efficacy and safety. Vascular specialists performed TACT according to a standardized protocol including ultrasound-guided injection of tumescence solution (45 ml of 0.9% saline, 15 ml of lidocaine 1%/epinephrine 5 μg/ml) in the soft tissue around the PA and its neck, local compression with the sonography probe until PA closure, and external compression by elastic belt for 3 hours. Results: Among a total of 125 patients with femoral artery PA, 82 (65.6%) were treated by TACT (including 3 patients with haemorrhagic shock), 14 (11.2%) by thrombin injection; 12 (9.6%) by endovascular interventions; 8 (6.4%) by open surgery; and 9 (7.2%) conservatively. In patients treated by TACT, PA neck was short (<1 cm) in 51.8% and mean cavity diameter 30 ± 23 mm. After injection of 58 ± 8 ml of tumescence solution, mean time of echo-guided compression was 12 ± 8 minutes. Pseudoaneurysm was successfully closed in 92.7% after the first attempt and in 96.3% after the second attempt. The intervention was well tolerated, without any adverse event. Conclusion: Tumescence anesthesia and echo-guided compression technique (TACT) is an effective and safe treatment for PA. Compared to other therapies, TACT is minimally invasive and widely available for minimal costs, even for large PA and in unstable patients. Clinical Impact: Tumescence anesthesia and echo-guided compression technique seems to be an efficient, safe, and a fair economical choice in comparison with alternative techniques. The implementation of this procedure is simple with low risk of complications. Splenic artery embolization plays an important role in the management of various medical and surgical conditions that are non-traumatic in etiology, in addition to its well-established and widely discussed role in managing splenic trauma. In nontraumatic emergencies of catastrophic bleeding originating from the spleen or splenic artery, splenic artery embolization can be effective in achieving hemostasis as a definitive management, temporary stabilizing measure, or preoperative optimization technique. In addition to emergency clinical conditions, splenic artery embolization can be performed electively as an alternative to splenectomy for managing patients with hypersplenism. Herein, we report 6 cases of splenic artery embolization performed at our center to highlight its various indications. This article aims to demonstrate the role of splenic artery embolization in different clinical scenarios and the considerations behind the techniques employed through illustrative cases.

Intimate sarcoma is a rare malignant tumor with a very poor prognosis. The incidence rate among all sarcomas is less than 1%. The primary localization of the tumor is most often the aorta and pulmonary artery. Other localizations account for about 10% of all observations. This tumor has an extremely nonspecific picture, is characterized by difficult diagnosis and poor prognosis. The main method of treatment is surgical – excision of the affected vessel and surrounding tissues with an interoperative histological examination, arterial reconstruction. About 140 cases of intimate sarcoma have been described in the literature. We present our own observation of intimate sarcoma. In the presented clinical case, the primary localization of the tumor was the common femoral artery. The patient was admitted with a clinical picture of a false femoral artery aneurysm, which was regarded as post-traumatic, without thrombosis or embolism of the distal bed and signs of limb ischemia. Repeated reconstructive surgical interventions were performed for a diagnosed false aneurysm. The intraoperative picture did not indicate any oncological process. After discharge, within six months there were two repeated requests for emergency indications in connection with erosive bleeding in the area of surgery. There were no signs of paraprosthetic infection. The oncological process was suspected with the appearance and growth of a voluminous formation of the inguinal region, which became the reason for performing a histological examination. The final diagnosis was made in the late stage of the disease, when radical surgical treatment was not feasible. The ongoing chemotherapeutic treatment did not have a significant effect, which led to an unfavorable outcome.

Pseudoaneurysms are rare but devastating complications of penetrating head traumas. They require rapid surgical or endovascular intervention due to their high risk of rupture; however, complex presentations may limit treatment options. Our objective is to report a case of severe vasospasm, flow diversion, and in-stent stenosis complicating the treatment of a middle cerebral artery pseudoaneurysm following a gunshot wound. A 33-year-old woman presented with multiple calvarial and bullet fragments within the right frontotemporal lobes and a large right frontotemporal intraparenchymal hemorrhage with significant cerebral edema. She underwent an emergent right hemicraniectomy for decompression, removal of bullet fragments, and evacuation of hemorrhage. Once stable enough for diagnostic cerebral angiography, she was found to have an M1 pseudoaneurysm with severe vasospasm that precluded endovascular treatment until the vasospasm resolved. The pseudoaneurysm was treated with flow diversion and in-stent stenosis was found at 4-month follow-up angiography that resolved by 8 months post-embolization. We report the successful flow diversion of an middle cerebral artery (MCA) pseudoaneurysm complicated by severe vasospasm and later in-stent stenosis. The presence of asymptomatic stenosis is believed to be reversible intimal hyperplasia and a normal aspect of endothelial healing. Traumatic aneurysms occurring in the digit are extremely rare. We report a case of a traumatic pseudoaneurysm arising from a terminal branch of the finger artery and presenting as a mixed lesion with pyogenic granuloma that was exposed to the outside of the body and treated surgically. We present the case of a 22-year-old male patient who developed “double” pseudoaneurysm of the superficial palmar arch of the left hand after trivial trauma with a kitchen knife. Following an unsuccessful embolization attempt, surgical excision of the pseudoaneurysm was performed during which the pseudoaneurysm was found to arise from the anterior wall of the palmar arch. Intraoperatively, a second pseudoaneurysm arising from the deeper surface of the superficial palmar arch was found and excised. This is probably the only reported case of double pseudoaneurysm of the palmar arch in literature. The possible mechanism of arterial injury, diagnosis, and management is discussed.

Aortic pseudoaneurysms (PA) vary in size and may remain asymptomatic. PAs may be caused by vascular injury, such as trauma or surgery, or other non-traumatic causes, such as Bechet disease, infection, or penetrating atherosclerotic ulcers. The diagnosis of PAs may have been delayed for decades. We present a case of a PA detected incidentally in a male patient who experienced traumatic bowel perforation due to blunt abdominal trauma 30 years before presentation. Computed tomography (CT) displayed a 9.2 cm mass in the pelvis, initially considered a neoplasm of small bowel origin. Further analysis of the CT images suggested a thrombosed PA at the aortic bifurcation, which was confirmed via surgical exploration. Graft interposition was performed using a Dacron 16-8 mm graft and the patient recovered without any complications. This case highlights the importance of a high index of suspicion for the diagnosis of a thrombosed aortic PA.

Background: Postsurgical thoracic aortic pseudoaneurysms (PTAPs) are a potentially lethal complication after cardiac or aortic surgery. Surgical management can pose a challenge with high in-hospital mortality rates. Transcatheter closure is a less-invasive alternative treatment option for selected patients, although current experience is limited. Aims: We aimed to evaluate procedural and imaging outcomes of our first 11 cases of transcatheter PTAP closure with the use of closure devices. Methods: Patients with a high operative risk who underwent transcatheter PTAP closure at our centre from 2019 to 2021 were retrospectively included. Suitability was evaluated on preprocedural computed tomography (CT) scans and three-dimensional (3D) reconstructions. All procedures were performed in the catheterisation laboratory. Intraprocedural aortography and postprocedural CT scans with 3D reconstructions were used to evaluate PTAP occlusion. Results: Eleven consecutive patients with a high operative risk and a history of cardiac/aortic surgery who underwent transcatheter PTAP closure were included. PTAPs were predominantly located at the proximal or distal anastomosis of a supracoronary ascending aortic vascular graft or Bentall prosthesis (82%). Implanted closure devices included Amplatzer Valvular Plug III (82%), Amplatzer septal occluder (9%) and Occlutech atrial septal defect occluder (9%). No periprocedural complications occurred. After device deployment, residual flow was absent on aortography in 64% and minimal residual flow was present in 36% of patients. Subtotal or total occlusion of the PTAP on follow-up CT ranged between 45% and 73%. Conclusions: Although subtotal or total occlusion of the PTAP was found at follow-up in only 45–73% of cases, transcatheter PTAP closure guided by preprocedural 3D reconstructions can offer a valuable minimally invasive primary treatment option for patients who otherwise would face a high-risk reoperation. Introduction: Splanchnic arterial aneurysms are a rare but potentially lethal disease with a mortality rate of more than 10% after rupture. Endovascular therapy is the first-line treatment for splanchnic aneurysms. However, appropriate management for splanchnic aneurysms after failed endovascular therapy remained inconclusive. Materials and methods: A retrospective review was performed for consecutive patients (from 2019 to 2022) who underwent salvage surgeries for splanchnic artery aneurysms following failed endovascular therapy. The authors defined failed endovascular therapy as the technical infeasibility to apply endovascular therapy, the incomplete exclusion of the aneurysm, or the incomplete resolution of preoperative aneurysm-associated complications. Salvage operations included aneurysmectomy with vascular reconstruction and partial aneurysmectomy with directly closing of bleeders from the intraluminal space of the aneurysms. Results: Seventy-three patients received endovascular therapies for splanchnic aneurysms, and 13 failed endovascular trials. The authors performed salvage surgeries for five patients and enrolled them in this study, including four false aneurysms of the celiac or superior mesenteric arteries and a true aneurysm of the common hepatic artery. The causes of failed endovascular therapy included coil migration, insufficient space for safely deploying the covered stent, a persistent mass effect from the postembolized aneurysm, or infeasibility for catheter cannulation. The mean hospital stay was nine days (mean ± SD, 8.8 ± 1.6 days), with no one suffering 90-day surgical morbidity and mortality, and all patients getting symptoms improvement. During the follow-up period (mean ± SD, 24 ± 10 months), one patient suffered a small residual asymptomatic celiac artery aneurysm (8 mm in diameter) and was treated conservatively due to underlying liver cirrhosis. Purpose: To study the association between median arcuate ligament compression (MALC) of celiac artery (CA) and splanchnic artery aneurysms/pseudoaneurysms (SAAPs) submitted to endovascular embolization. Methods: Single center retrospective study of embolized SAAPs between 2010 and 2021, to evaluate the prevalence of MALC, and compare demographic data and clinical outcomes between patients with and without MALC. As a secondary objective, patient characteristics and outcomes were compared between patients with different causes of CA stenosis. Results: MALC was found in 12.3% of 57 patients. SAAPs were more prevalent in the pancreaticoduodenal arcades (PDAs) in patients with MALC, compared to those without MALC (57.1% vs. 10%, P =.009). Patients with MALC had a greater proportion of aneurysms (71.4% vs. 24%, P =.020), as opposed to pseudoaneurysms. Rupture was the main indication for embolization in both groups (71.4% and 54% of patients with and without MALC, respectively). Embolization was successful in most cases (85.7% and 90%), with 5 immediate (28.6% and 6%) and 14 non-immediate (28.6% and 24%) post-procedure complications. Thirty and 90-day mortality rate were 0% in patients with MALC, and 14% and 24% in patients without MALC. Atherosclerosis was the only other cause of CA stenosis, in 3 cases. Conclusions: In patients with SAAPs submitted to endovascular embolization, the prevalence of CA compression by MAL is not uncommon. The most frequent location for aneurysms in patients with MALC is in the PDAs. False aneurysm of the brachiocephalic trunk is a very rare but highly lethal, life-threatening, and difficult-to-treat condition. In this report, we present a case of a patient who suffered from rapidly worsening dyspnea caused by infected false aneurysm of the brachiocephalic trunk compressing the trachea that was successfully treated by stent graft implantation. The main purpose of this article is to consider other, less common causes of dyspnea and to explain the pathogenesis of infected true/false aneurysm and its management. Due to the rarity, history-taking and physical examination should be thorough, and symptoms and signs should be analyzed deeply. Simplification should be avoided during diagnosis. In addition, we would like to highlight the option of stent grafts as an alternative to surgery in the management of patients suffering from infected aneurysm who are at high surgical risk.

Pseudoaneurysm and arteriovenous fistula can occur after iatrogenic trauma or penetrating injuries. Endovascular treatment is a minimally invasive method used to manage these complex vascular injuries. We have described the case of a 27-year-old male patient who had presented with progressively increasing pain and swelling of the left inguinal region after a gunshot injury 5 years earlier. The bell-bottom technique was used in a reversed fashion to exclude the pseudoaneurysm and treat the arteriovenous fistula, achieving symptom resolution without complications.

The use of non-penetrating pre-slaughter stunning (NPPSS) in ruminants remains a contentious issue in Indonesia since it is thought to interfere with the bleeding. This paper evaluated the bleeding time (BT), incidence of the false aneurysm (FA), and factors influencing these parameters in 460 NPPSS and 50 non-NPPSS slaughters in Indonesia. In this research, bodyweight, neck skin fold, sex, incisors condition, stunner strength, door closed-final stunning interval, number of shots, stunning success rate (SSR), stunning final-slaughter interval, broken skull, shooting placement, stunner-men and slaughtermen competencies, incision direction, incision location, BT, and FA were assessed for NPPSS slaughter. The same parameters were also assessed for non-NPPSS except the stunning parameter. The results showed that the BT of the non-NPPSS slaughters (187±47.37 seconds) was faster than that of the NPPSS slaughters, with the BT for the NPPSS dependent upon the level of unconsciousness. The longest BT was found in fully unconscious cattle shot once (304.32±69.76 seconds). The risk of FA incidence in non-NPPSS cattle was higher than in the NPPSS cattle. The FA incidence was 2.64 times higher in non-NPPSS cattle than in unconscious NPPSS cattle. The BT of non-NPPSS cattle was affected by FA, whereas the NPPSS cattle were affected by SSR, number of shots, broken skull, and FA incidence. The primary influence factor of FA in non-NPPSS cattle was incision location, whereas the NPPSS cattle were DCFS, incision location, and incision direction. Based on the results of this study, it can be concluded that the use of NPPSS prolongs BT and reduces the risk of FA in the slaughter of Australian Brahman cross cattle. [ABSTRACT FROM AUTHOR]

Pseudoaneurysm of the mitral‐aortic intervalvular fibrosa (P‐MAIVF) can be a rare but life‐threatening complication of infective endocarditis, cardiac surgery, or blunt chest trauma. Congenital heart diseases especially in young patients are reported as additionally predisposing factors. We present the case of an asymptomatic 52 years‐old male with bicuspid aortic valve and gastrointestinal polyposis syndrome in whom a P‐MAIVF was incidentally detected. The patient successfully underwent pseudoaneurysm surgical repair and aortic valve replacement and despite no evident causes were found we hypothesized addiotional underlying mechanism of P‐MAIVF. Background: Traumatic arteriovenous fistulas (AVFs) are rare entities, especially when referring to visceral arterioportal AVFs. Currently, there are no large epidemiological studies looking specifically at traumatic visceral AVFs. When traumatic AVFs have been discussed in the literature, it is in the form of case reports or case series and focused on peripheral AVFs. The following case is a presentation of a traumatic arterioportal AVF and its management with an endovascular approach., Case Presentation: A 16-year-old adolescent boy presented tachycardic and hypotensive with a single gunshot wound in his left abdomen. Multiple operations were required. His intensive care unit course was remarkable for persistent hypertension despite multiple antihypertensive medications. A computed tomography angiography (CTA) of the abdomen and pelvis showed a superior mesenteric arteriovenous fistula (AVF) with pseudoaneurysm. He underwent endovascular repair of this traumatic visceral AVF and pseudoaneurysm. This rare case illustrates the presentation, diagnosis, and treatment of a traumatic visceral arterioportal fistula with pseudoaneurysm., Conclusion: Traumatic AVF and visceral vascular injuries are rare. A CTA of the abdomen can be diagnostic, and we demonstrate that endovascular techniques can be diagnostic and therapeutic. The liver is the most frequently injured organ during abdominal trauma. Vascular injuries account for approximately 3% to 25% of all cases of liver trauma and are typically treated by transarterial embolization (TAE). We describe a case of American Association for the Surgery of Trauma grade V liver injury with a very large pseudoaneurysm and an arteriohepatic venous fistula (AHVF), which failed to respond to TAE. This case was successfully treated using a direct transhepatic glue injection without complications. AHVF is an exceedingly rare type of vascular injury in blunt trauma. Direct transhepatic glue injections could represent a reliable alternative to TAE for the treatment of visceral pseudoaneurysms, especially if TAE fails to eliminate the lesion.

Unlike other aneurysms, blood blister-like aneurysms(BBA) occur at non-branching sites, are commonly located at the dorsal wall of the supraclinoid internal carotid artery (ICA), and are suspected to arise from dissecting aneurysms. They are typically diagnosed after a bleed because of their small size and unusual site. Therefore, lesions exist after brain computed tomography or magnetic resonance imaging, but they are often not found. In the case of BBA, which have a high risk of recurrence, the treatment is difficult, and selecting the treatment method is also challenging. This paper describes the treatment of a BBA with internal trapping of the ICA that eventually recurred despite performing coil embolization twice.

Summary: We describe our use of balloon‐expandable stents in pulmonary arteries for a variety of indications. This is an endovascular technique that provides an alternative treatment option to treat some forms of complex pulmonary artery disease when standard surgical or other endovascular options are not possible. This case series describes the successful treatment of four patients with different pulmonary artery diseases. Indications for stent placement include multiple broad‐necked pulmonary artery pseudoaneurysms, symptomatic malignant pulmonary artery narrowing and post‐surgical iatrogenic pulmonary artery stenosis. No stent‐related complication had been identified from 3 months to 4 years of follow‐up. All surviving patients have experienced sustained symptomatic improvement. [ABSTRACT FROM AUTHOR]

Objective: The objective was to present the case of a late covered iliac stent late infection and report a comprehensive literature review on diagnosis and outcomes in this setting. Methods: A comprehensive review of the literature was performed through MedLine by two independent reviewers from 1990 to 2020 on reported cases of arterial stent late onset infection over arterial stents. The data about on the risk factors, clinical presentation, treatment and outcomes were collected. Results: Twenty-two studies were selected as pertinent for the analysis, totalling 24 patients including the indexed case. Infection occurred at a median of 22 months postoperatively (range 2–120 months) over a bare metal stent in 66.7% (n 16) of cases versus 33.3% (n 8) over a covered stent. Clinical presentation included local symptoms (local pain, oedema, petechiae or skin rash) in 21 (87.5%) cases and non-specific systemic symptoms (fever, sepsis, chills and leucocytosis) in 8 cases (33.3%). In 4 cases (16.7%), patients presented with haemorrhagic shock upon arterial rupture. The bacteria most frequently encountered were S. aureus (54.2% of cases). Several factors were supposed to be responsible for the infection including among which procedure-related (non-aseptic technique, lack of prophylactic antibiotics and repetitive punctures at the access site) or related to pre-existing patient's clinical conditions (immunosuppression, diabetes and concurrent infection) have been considered responsible for the infection. Treatment consisted in antibiotics alone (2 patients, 8.3%) or in association with surgical explant, both with or and without revascularization (n 21, 87.5%). In one case, an endovascular coiling was performed. Complications occurred in 29.2% (n 7) of cases and included the need for amputation, bowel resection, endocarditis, pulmonary failure or pneumonia. Overall, three patients (12.5%) died from a septic shock or multi-organ failure. Left ventricular pseudoaneurysm is a rare disease; it is defined as a ventricular rupture contained by epicardium, pericardial adhesions, or both. It most frequently occurs as a complication of acute myocardial infarction. Surgical treatment is recommended for pseudoaneurysms that are large or symptomatic and for those discovered less than 3 months after myocardial infarction. We report our experience with 2 patients who had left ventricular pseudoaneurysms discovered less than a week after inferior myocardial infarction. Both patients were middleaged men with right coronary occlusion in whom the diagnoses were established by echocardiography during the first week after infarction. Because both patients were clinically stable, we opted to defer surgery until scarring could facilitate correction; this decision was based on a review of the literature showing that in-hospital mortality is higher with early surgery. The patients were monitored closely in the intensive care unit and were prescribed β-blockers and vasodilators. Both patients underwent left ventricular patch reconstruction with exclusion of the pseudoaneurysm and posterior septum; both received moderate inotropic support and prophylactic intra-aortic balloon pump assistance. Their postoperative courses were uneventful. In 5 prior reports describing 45 patients (13 with acute pseudoaneurysm [≤2 wk after infarction] and 32 with nonacute pseudoaneurysm), in-hospital mortality was 61.5% for patients in the acute group and 15.6% for the nonacute group (P = .0066). We recommend that clinicians consider deferring surgery for patients with stable acute left ventricular pseudoaneurysm to reduce the risks associated with early repair. [ABSTRACT FROM AUTHOR]

Transcatheter arterial embolization is a safe and effective treatment for visceral artery aneurysms; nevertheless, some complications can occur. Coil migration to other organs after embolization is extremely rare, and only 16 cases have been reported previously. We report a rare case of coil migration to the duodenal lumen after embolization of a right colic artery pseudoaneurysm. To the best of our knowledge, this is the first case of coil migration after a right colic artery embolization. The patient exhibited no symptoms and was treated conservatively without any intervention. Some previous reports have demonstrated spontaneous coil passage and successful conservative management. Our case supports conservative treatment as the primary treatment for asymptomatic patients. Clinicians should assess the risks and benefits of coil removal in asymptomatic patients before performing any intervention.

Abstract Background The formation of a uterine artery pseudoaneurysm is rare and isolated cases have been reported in the existing literature following caesarean sections, curettages and cone biopsies. There has been no report of pseudoaneurysm formation following a loop electrosurgical excision procedure. Vaginal bleeding could potentially be life threatening if this diagnosis is not considered following cervical instrumentation or surgery. Management options range from haemostatic sutures, image-guided embolisation to surgical repair. We report the diagnosis and management of a case of uterine artery pseudoaneurysm after a loop electrosurgical excision procedure. Case presentation A 37-year-old woman was diagnosed with cervical intraepithelial neoplasia grade 3 (CIN3) and underwent a therapeutic loop electrosurgical excision procedure. One month after the procedure, the patient presented to the emergency department with repeated episodes of sudden-onset heavy vaginal bleeding associated with hypotension and syncope. A computed tomography angiogram was performed, which demonstrated a pseudoaneurysm of the right uterine artery. Following the diagnosis, image-guided embolisation was performed successfully. Post-embolisation angiograms showed successful embolisation of the pseudoaneurysm and the patient had no further episodes of bleeding. Conclusions Loop electrosurgical excision procedures are generally safe but rarely, can be complicated by the formation of uterine artery pseudoaneurysms. The depth of the loop electrosurgical excision procedure and vascular anatomy should be considered to prevent such complications. A computed tomography angiogram appears to be ideal for diagnosis. Image-guided embolisation is safe and effective as a therapeutic measure, with minimal morbidity.

True idiopathic radial artery aneurysm is a rare entity. There are only a few cases reported in the literature. Asymptomatic radial arterial aneurysm in young patients is even rarer. Here, we present a successfully managed case report of a left radial artery aneurysm in a young female who presented to us with a painless, pulsatile swelling at the wrist.

Pseudoaneurysm is a rare vascular complication of trauma, causing an incomplete tear of the vessel wall. We present a clinical case report arising from the distal branch of facial artery in an infant. Facial artery pseudoaneurysm is a rare complication of facial trauma and can easily be misdiagnosed especially in the paediatric age group. Prompt investigation and diagnosis with timely and apt intervention is the key to the successful management of facial artery pseudoaneurysm.

A feared complication of acute myocardial infarction is the formation of a cardiac pseudoaneurysm. We report a case of a gargantuan, arrhythmogenic left-ventricular pseudoaneurysm with contradictory morphological characteristics. The integrative use of high-resolution 3-dimensional magnetic resonance imaging and computed tomography proved essential for the diagnostic discrimination and successful therapeutic intervention. Purpose Liver injuries constitute an important cause of hospital admissions, pediatric morbidity, and sometimes mortality. This study was undertaken to assess the spectrum of pediatric liver injuries with special emphasis on the management of acute complications. Methods This retrospective study used data from the Department of Pediatric Surgery, Bangalore Medical College, Bengaluru, India, between March 2013 and March 2019. All children between 1–18 years were included in the study (n = 33). Children were assessed with relevance to age, sex, mode of injury, hemodynamic stability at admission, need for blood transfusions, surgery, radiological investigations, complications, total number of days of stay in hospital, and mortality. Results There were 27 (81.8%) males and 6 (18.18%) were females. The most common cause for injury was a road traffic accident (60.6%). The majority (nearly 65%) of pediatric liver injuries were Grade II and III. There were 4 children who had post traumatic complications including 2 pseudoaneurysms, 1 inferior vena cava thrombus, and 1 bile leak with a biloma formation which were managed appropriately. Conclusion Timely and appropriate radiological evaluation is required for assessments of complications. The majority of complications can be managed conservatively, and minimally invasive techniques like endoscopic retrograde cholangiopancreatography is helpful in the management of complications.

A case of endovascular treatment of a patient with a large false aneurysm of the splenic artery resulting from arrosion of its wall into the cavity of a previously existing pancreatic pseudocyst is described. In addition to the rather rare occurrence of this pathology, a feature of this clinical case was the patient’s sharp tortuosity of the access vessels, including the iliac arteries, abdominal aorta, and the splenic artery. Thus, both the endovascular prosthetics of the affected splenic artery and its embolization according to the traditional method using standard angiographic catheters and Gianturco coils turned out to be technically impossible because of the inability to reach the lesion site, especially with access through the common femoral artery that typical for such interventions, which was used by us when performing diagnostic selective arteriography. The second stage was the embolization of the splenic artery by access through the left axillary artery by means of conducting of guiding catheter into the celiac trunk. Through the lumen of this catheter, we introduced a microcatheter, through which detachable microcoils (usually used in interventional neuroradiology) were introduced into the splenic artery proximal and distal to the aneurysm cavity. As a result, the affected area of ​​the splenic artery with aneurysm was completely turned off from the bloodstream and thrombosed, that allowed to avoid extremely risky open surgical intervention and eliminated the risk of rupture of the aneurysm. Medication support included hypotensive therapy to reduce the risk of rupture of the aneurysm (before and after surgery), analgesics for the relief of post-embolization pain, and antibiotics for the prevention of infectious complications associated with pancreatitis and the possible development of spleen infarction. Monitoring the effectiveness of the intervention in the postoperative period was carried out using ultrasound dopplerography.

We present an endovascular approach for anatomic reconstruction of the iliac bifurcation in life-threatening arterioureteral fistula without sacrificing the pelvic arterial vascular supply. Five consecutive patients suffering from acute onset of significant gross hematuria caused by iliac-ureteral fistula resulting from previous oncologic surgery and radiation therapy were treated by transfemoral stent graft implantation in a double-barrel technique. Iliac-ureteral pseudoaneurysm coverage succeeded in an iliac neobifurcation with preservation of pelvic perfusion. Follow-up ranging from 9 to 37 months confirmed cessation of hematuria. One patient experienced stent graft thrombosis of the external iliac artery as a result of large cervical cancer invasion treated by crossover bypass. In all other patients, stent grafts were patent.

Abstract Charcot foot or neurogenic osteoarthropathy is a serious complication of diabetic peripheral neuropathy. The association between false aneurysm and Charcot foot is yet uncommon and has not been reported through the literature. We describe a case of false aneurysm in a Charcot foot in a 55‐year‐old female patient.
