Background: basic cardiopulmonary-cerebral resuscitation is present as its own course in the "D" and "E" study plans of the Medicine career; although in both the organization of the process has its particularities.Objective: to specify the differences in terms of structure, teaching process and academic results in the teaching of basic cardiopulmonary-cerebral resuscitation, between the "D" and "E" study plans of the Medicine career.Methods: a descriptive, prospective, cross-sectional study was carried out at Cienfuegos University of Medical Sciences from 2017 to 2019. Theoretical and empirical methods were used. To compare results, the student universe was used in both years, to evaluate satisfaction and quality a simple random sample. Descriptive statistical techniques were used. For both courses, a similar scenario and faculty were used.Results: the differences between both plans derive from the non-personal components of the teaching process; plan “D” has 20 hours and plan “E” 40, which expands its contents. All the topics and objectives of plan “D” were included in plan “E”. There was similarity in the general (80.4% and 75.2%) and quality (50.6% and 53.1%) school grades in both years. The fulfillment of the expectations, satisfaction and quality of the process was valued as very high and high (more than 98%) by the students in both courses.Conclusions: although there were differences in the teaching-learning process between both plans, there was no impact on student satisfaction, quality of the teaching process, or academic results. The “E” plan variant is more comprehensive for basic life support training., Fundamento: la reanimación cardiopulmocerebral básica está presente como curso propio en los planes de estudios “D” y “E” de la carrera de Medicina; aunque en ambos la organización del proceso tiene sus particularidades.Objetivo: precisar las diferencias en cuanto a estructura, proceso docente y resultados académicos en la enseñanza de la reanimación cardiopulmocerebral básica, entre los planes de estudios “D” y “E” de la carrera de Medicina.Métodos: se realizó un estudio descriptivo, prospectivo, transversal, en la Universidad de Ciencias Médicas de Cienfuegos en los años 2017 y 2019. Se utilizaron métodos teóricos y empíricos. Para comparar resultados se utilizó el universo estudiantil en ambos años, para evaluar satisfacción y calidad una muestra aleatoria simple. Se utilizaron técnicas de estadística descriptiva. Para ambos cursos se utilizó similar escenario y claustro de profesores.Resultados: las diferencias entre ambos planes derivan de los componentes no personales del proceso docente; el plan “D” tiene 20 horas y el “E” 40, con lo cual se amplían sus contenidos. Todos los temas y objetivos del plan “D” estuvieron incluidos en el “E”. Hubo similitud en las promociones general (80.4 % y 75.2 %) y de calidad (50.6 % y 53.1 %) en ambos años. El cumplimiento de las expectativas, satisfacción y calidad del proceso fue valorado de muy alto y alto (más 98 %) por los estudiantes en ambos cursos.Conclusiones: aunque existieron diferencias en el proceso enseñanza aprendizaje entre ambos planes, no hubo repercusión en la satisfacción de los estudiantes, calidad del proceso docente, ni los resultados académicos. La variante del plan “E” es más integral para la formación básica del apoyo vital.

- Abstract
In-hospital sudden death and cardiopulmonary cerebral resuscitation problems could have a common structured response. However, most hospitals lack a solu-tion-oriented strategy. This paper provides detailed update on the main alterna-tives to reduce lethality of in-hospital sudden death and presents a proposal from the “Hospital General de Cienfuegos”. The institutional approach for greater surviv-al and lesser disability when facing sudden death will be based on multidiscipli-nary initiatives primarily centered on quality and prevention in the different stages of cardiopulmonary cerebral resuscitation. A cardio-protected hospital comprises management, organizational, assistance, academic and logistical processes for every single link of the survival chain in a medical emergency event, mainly sudden cardiac arrest. The proposal of the “Hospital General de Cienfuegos”, a cardio-protected facility, brings together all elements of an integrated cardiopulmonary cerebral resuscitation system aimed at quality, safety and satisfaction of patients and caregivers., Los problemas de muerte súbita y reanimación cardiopulmocerebral intrahospita-larias pueden tener estructurada una respuesta común; sin embargo, en la mayo-ría de los hospitales no se dispone de una estrategia orientada a su solución. Este trabajo actualiza sobre las principales alternativas para reducir la letalidad por muerte súbita intrahospitalaria y presenta una propuesta del Hospital General de Cienfuegos. El abordaje institucional para mayor supervivencia y menor discapa-cidad ante la muerte súbita dependerá de iniciativas multidisciplinarias enfocadas a la calidad y prevención, en los diferentes eslabones, de la reanimación cardio-pulmocerebral. Un hospital cardioprotegido integra procesos gerenciales, organi- zativos, asistenciales, académicos y logísticos para todos los eslabones de la cade-na de supervivencia ante una emergencia médica, principalmente la parada car-díaca súbita. La propuesta del Hospital General de Cienfuegos como institución cardioprotegida agrupa todos los elementos de un sistema integrado de reanima-ción cardiopulmocerebral enfocado hacia la calidad, seguridad y satisfacción de pacientes y prestadores.

El síndrome de choque es un problema frecuente en las unidades de urgencias y emergencias con una elevada prevalencia y mortalidad debido a sus múltiples causas. Con el objetivo de caracterizar el diagnóstico y manejo de los pacientes con choque se desarrolló una revisión bibliográfica, citándose 20 artículos recuperados desde SciELO, Scopus, ClinicalKey y otras bases de datos. Los marcadores del shock grave constituyen una herramienta para la estratificación del riesgo, así como objetivos o metas para el restablecimiento precoz de una perfusión tisular adecuada. La monitorización clínica de la oxigenación tisular y de la función orgánica supone habitualmente la medición de criterios de valoración tradicionales, como la frecuencia cardíaca, constituyendo un pilar en el diagnóstico del choque. En general, la clave del tratamiento del shock es mantener la presión arterial media y asegurar una adecuada perfusión y transporte de oxígeno y otros nutrientes a los órganos vitales.

This paper describes psychocardiology studies conducted from 2002 through 2018 in the Cardiology Department of the Dr Gustavo Aldereguía Lima University General Hospital in Cienfuegos Province, Cuba. Given the strong association between cardiovascular diseases and lifestyle, negative emotions and personality traits, psychology and medicine are equally necessary components of prevention and patient care, indispensable to primary and secondary prevention and to rehabilitation. When its therapeutic principles are appropriately applied, psychology can have a positive impact on the course of the disease and on patients' adaptation to new habits and lifestyles. The psychocardiologist's job is fundamental in achieving conscious participation by patients in their rehabilitation. Theoretical and practical contributions include a gender-based approach, addressing patients' sexual needs, neuropsychological assessment of damage associated with cardiovascular diseases, the particularities of pediatric patient care, family involvement in rehabilitation, and services for families at risk for cardiovascular events due to genetic factors. Results of these studies are included in published methodology for intensive psychological treatment for patients and health care workers. [ABSTRACT FROM AUTHOR]

- Abstract
Foundation: active inquiry allows an early health status diagnosis of population groups. Objective: to describe the process of active research in the population to detect possible cases of COVID-19, carried out by students of medical sciences. Methods: prospective, descriptive, cross-sectional study, carried out at the Cienfuegos University of Medical Sciences, Cuba, between March 19 and April 18, 2020 by 2,280 students and 142 teachers. Techniques: participatory observation and daily reports of research activity. Results: between 795 and 2,186 students have progressively participated in daily research activities, which ranged from 91-92% of the students called. On average, 61 189 homes were visited daily and 114 491 inhabitants were surveyed (28.13% of the total population); Out of these, 11 were positive and 27 were suspected of the disease. COVID-19 has not been reported among students. Conclusion: massive community active research has quadrupled in its first month and has contributed to the early detection of acute respiratory infections related to COVID-19 in the Cienfuegos province. The research process has been safe for students and teachers, who were trained in aspects related to disease transmission and biosecurity measures. [ABSTRACT FROM AUTHOR]

- Abstract
Foundation: Having a triage system is essential for emergency systems, given the high demand for assistance and the need to offer quality medical care. Objective: to describe the system of triage developed at the Cienfuegos Province between 1988 and 2016. Methods: research in systems and health services, mixed in methods and techniques, which included the 24 emergency services of the territory. Indicators in the records of the health institutions were included and the results of other research conducted during the period, which also measured them. Results: atriage system was designed with three coding levels (red, yellow and green) according to vital risk. The red code was similar for the entire healthcare network. Main indicators: average time of arrival-triage 2.5 minutes; duration of the triage <5 min; arrival time-medical attention (red 0-7 minutes, yellow 7-14 minutes, green 28 minutes); percentage of patients attended but not classified 10.5%; agreement index between nurse-sorter and doctors for patients with green or yellow code 77.6%; percentage of income for patients with a red code, 98%; percentage of income for patients with a yellow code of 19.5%. In the period under review, the demand for medical care doubled (mainly due to the green code), and the mortality rate of the emergency department was reduced. Conclusion: The triage system in Cienfuegos is structured, is useful and has been effective mainly for the most serious adult patients. [ABSTRACT FROM AUTHOR]

- Abstract
Foundation: the National Project "Training and Research in life support and disasters" has among its objectives to reduce lethality due to emergencies and to contribute to the quality of medical care, but it requires an academic and organizational structure. Objective: to define teaching and research methodological elements for the project and for its six courses. Methods: qualitative cross research developed in Cienfuegos (July 10/11, 2018) as part of a national workshop called with this purpose with 83 professionals (74, 69% experts) from six provinces. Techniques as brain storming and focal group were used. For the general and specific recommendations about the courses, the group was divided in six sub-groups finally, these were written together to be exposed. Results: the general proposals are related with an updated, uniform, inter-sectorial, face-to-face academic program which is extended to community till post-graduate courses in health institutions and involves computer technologies. Proposals for the integration to the context in the medical practice include the design and adherence to behaving protocols and the integration to the re-organization of the emergency system. Conclusion: contents of teaching activities and academic and research strategies, so as their integration to the context of emergency management, may develop the training and training in life support in the project framework. For the impact in health indicators, it is necessary, in addition, inter-sectorial community work and the organizational improvement of the emergency system. [ABSTRACT FROM AUTHOR]

15. Publicaciones cubanas sobre reanimación cardiopulmocerebral: una aproximación bibliométrica. 1998 - 2018.

- Abstract
Introduction: multiple researches have been developed along the years about cardio-pulmonary (CPR); new evidences appear and scientific articles are published regularly which show its development. Objective: to characterize scientific publications about CPR in Cuba. Methods: descriptive bibliometric, retrospective study done from January 1998 to November 2018. All Cuban publications indexed to Scientific Electronic Library Online (Scielo) were reviewed. Editorials and letters to the editor were not included. It was designed a registration folder in Microsoft Office Excel 2010, in which the information about the articles was organized for their identification, counting and analysis of the bibliometric indicators. Results: from the 83 articles indexed in Scielo, only 24 had the requirements to be included, there was predominance of original articles with 58, 3%, followed by reviews. Regarding the number of authors per article the category 3 to 6 authors was the most frequent, with 5 articles. Conclusion: scientific production related to cardio-pulmonary-cerebral resuscitation has been unstable and poor. The periods of greater scientific production were 2003 and 2017. 18. Utilidad de la medición de la densidad mineral ósea en pacientes reumáticos

Introducción: La osteoporosis se considera uno de los grandes males silenciosos que afecta a la población actual, dentro de los pacientes que mayor predisposición presentan a sufrir esta enfermedad se encuentran los pacientes reumáticos los cuales presentan múltiples factores de riesgo para el desarrollo de la misma. Conocer la densidad mineral ósea de los mismos es vital para establecer la conducta terapéutica en el enfrentamiento de las afecciones reumáticas.Objetivo: Caracterizar las modificaciones de la densidad mineral ósea por densitometría en pacientes reumáticos.Metodología: Estudio observacional, descriptivo, longitudinal, en pacientes con diagnóstico de enfermedades reumáticas que asistieron al servicio de consulta externa de reumatología y medicina interna del centro especializado ambulatorio Héroes de Playa Girón del hospital general universitario Dr. Gustavo Aldereguía Lima de Cienfuegos en el período comprendido entre enero y mayo del 2013. Los pacientes cumplieron con los criterios diagnósticos para cada una de las enfermedades reumáticas y a los mismos se les realizó una densitometría ósea para definir el grado de afectación de su masa ósea y una encuesta con el objetivo de conocer variables socio demográficas de valor en esta investigación.Resultados: El 96.92 % de los pacientes fueron mujeres, predominando ostensiblemente el color blanco de la piel con un 80 % de la totalidad. La edad media fue de 58.68 ± 10.12, presentándose en las mujeres a mayor edad. Las afecciones más frecuente fueron la artritis reumatoide en un 52.3 % seguido de la osteoartrosis en un 23 %. Se encontró que la mayoría de los pacientes tuvieron osteoporosis a nivel de columna, sin embargo en el examen del fémur es más frecuente la osteopenia.Conclusiones: Los pacientes reumáticos presentan una disminución importante de la densidad mineral ósea asociado a la edad, a la edad de menopausia y al tratamiento esteroideo.

Introducción: Los pacientes reumáticos son susceptibles de rehabilitar tanto en fase aguda, subaguda o en remisión.Objetivo: Caracterizar a los pacientes atendidos en el servicio de rehabilitación del centro especializado ambulatorio con diagnóstico de enfermedades reumáticas.Método: Se realiza un estudio descriptivo, retrospectivo con 95 pacientes ingresados con diagnóstico de afecciones reumáticas en el centro especializado ambulatorio Héroes de Playa Girón del Hospital General Universitario Dr. Gustavo Aldereguía Lima de la provincia de Cienfuegos en el año 2011.Resultados: El 55.7 % de nuestros pacientes fueron mujeres, predominando ostensiblemente el color blanco de la piel con un 88.4% de la totalidad de los pacientes. La edad media general de la muestra fue de 59.28± 15.24, presentándose en los hombres a mayor edad. La patología más frecuente en el servicio de rehabilitación fue la coxartrosis para un 49.4 % del total de los casos estudiados siguiéndola en frecuencia la artritis reumatoide con un 29.4 %. La mayoría de nuestros pacientes fueron rehabilitados con kinesiología (86.3%) y MTN (71.5 %) para todos los diagnósticos y el agente físico más utilizado fue la magnetoterapia (23.1 %). La estadía media de estos pacientes de manera general fue de 15.8 ± 6.5 días.Conclusiones: Los pacientes con enfermedades reumáticas se benefician con las diferentes técnicas de rehabilitación lo que se traduce en mejoría de su percepción de calidad de vida.

Introduction: The possibility to locate structures involved in arrhythmias with the use of catheter electrodes gave an impetus to the development of techniques linked to the selective destruction of cardiac tissue. This is possible through radiofrequency catheter ablation. These procedures are the first choice treatment for most cardiac arrhythmias. The objective was to characterize clinically the pre and post therapeutic procedure for patients transferred to receive radiofrequency ablation. Method: A prospective and descriptive study in 26 patients with heart rhythm disorders who were referred for radiofrequency ablation and followed up for five years at the Cardiology Outpatient Department of the Dr. Gustavo Aldereguía Lima Hospital, in Cienfuegos. The variables studied were age, sex, type of clinical arrhythmia, arrhythmogenic substrate ablated, complications, initial success and recurrence of the treated arrhythmia. Results: The mean age of patients was 40.9 ± 17.6 years, mostly women (57.7%). Younger patients (mean age 38.6 ± 19.7 years) were treated for atrioventricular nodal reentrant tachycardia and older patients (mean age 53.4 ± 23 years) underwent a non-selective ablation of the atrioventricular node. Half of the patients had orthodromic tachycardia; therefore, the most commonly treated arrhythmogenic substrate was an accessory pathway (65.4%). There were two major complications (7.7%), none of them directly related to the application of radiofrequency energy. In two patients (9.1%), the previously treated clinical arrhythmia recurred during the first year; both of them had right accessory pathways. Conclusions: It was feasible to transfer patients for radiofrequency ablation because of agreements with two centers which conduct the procedure. Most of them had tachyarrhythmias involving accessory pathways. Only two major complications occurred; the initial success rate was adequate and the recurrence rate of the arrhythmia was small., Introducción: La posibilidad de localizar, con catéteres-electrodos, las estructuras que intervienen en las arritmias, propiciaron el ímpetu para el desarrollo de técnicas que tuvieran que ver con la destrucción selectiva del tejido cardíaco. Ello es posible mediante la ablación con catéter de radiofrecuencia. Estos procedimientos constituyen la opción terapéutica de primera elección para la mayoría de las arritmias cardíacas. El objetivo fue caracterizar clínicamente el pre y post procedimiento terapéutico a pacientes transferidos para ablación con radiofrecuencia. Método: Estudio descriptivo, prospectivo, en 26 pacientes con trastornos del ritmo cardíaco, enviados para ablación con radiofrecuencia, seguidos en consulta externa durante cinco años en el Servicio de Cardiología del Hospital "Dr. Gustavo Aldereguía Lima", Cienfuegos. Las variables estudiadas fueron: edad, sexo, tipo de arritmia clínica presentada, sustrato arritmogénico ablacionado, complicaciones, éxito inicial y recurrencias de la arritmia tratada. Resultados: La edad media de los pacientes fue de 40,9 ± 17,6 años, mayoritariamente mujeres (57,7 %). A los pacientes más jóvenes (edad media 38,6 ± 19,7 años) se les trató una taquicardia por reentrada intranodal y a los de mayor edad (media 53,4 ± 23 años) se les ablacionó de forma no selectiva el nodo aurículoventricular. La mitad de los pacientes presentaban una taquicardia ortodrómica de ahí que, el sustrato arritmogénico más tratado fue una vía accesoria (65,4 %). Ocurrieron dos complicaciones mayores (7,7 %), ninguna directamente relacionada con la aplicación de la energía de radiofrecuencia. En dos pacientes (9,1 %) la arritmia clínica previamente tratada recurrió durante el primer año, ambos tenían vías accesorias derechas. Conclusiones: Fue factible transferir pacientes para ablación con radiofrecuencia debido a convenios de trabajo con dos centros ejecutores. - Abstract
INTRODUCTION Cuba’s science policy is grounded in a scientific and technological innovation system applicable to all institutions. In hospitals, the system should infl uence medical care and administrative and teaching processes, as well as promotion of scientific activity as such. OBJECTIVE Describe results of the Scientific and Technological Innovation System at the Dr Gustavo Aldereguía Lima University Hospital, the main provincial hospital in Cienfuegos, Cuba. METHOD This was a key informant survey and document review concerning the hospital’s scientific activity during 2000–2014. A questionnaire was administered to 22 key informants to select key indicators and area. Data on the hospital’s scientific activity related to these indicators were retrieved from hospital, provincial public health and scientific publishing databases. A second group of 35 key informants confirmed linkages between scientific outputs and the innovation system’s main activities. RESULTS The following were reported over the study period: sustained development of scientific human resources (40% grade II specialists, 30% master’s degree holders and 11.4% accredited academic researchers among the hospital’s professional staff), high scientific output (annual average of 445 studies completed, 118 publications and 203 projects under way) and high visibility (national and international recognition) of the hospital’s achievements in science and innovation. Key informants considered that results related to development of scientific potential were influenced by the following activities of the Scientific and Technological Innovation System: promotion of grade II specialty training, researcher accreditation and awarding of master’s degrees and PhDs in the sciences, development of an extensive continuing education program (for researchers and their mentors), public recognition of professionals with good scientific results, promotion of research and other scientific activities, and the requirement that professional and technical staff participate in national events sponsored by scientific societies. CONCLUSIONS Implementation of a Scientific and Technological Innovation System can contribute to a hospital’s scientific capacity and productivity. [ABSTRACT FROM AUTHOR]
