Reproductive processes in fishes are regulated by the hypothalamic-pituitary-gonadal (HPG) axis, much like in tetrapods. Within this system, Gonadotropin-Releasing Hormone (GnRH) is released by the hypothalamus, binding to GnRH receptors in the pituitary gland and stimulating the secretion of gonadotropin hormones. The current study aimed to analyze the GnRH receptor in Oncorhynchus mykiss (rainbow trout) using a computational and structural biology approach. The GnRH receptor gene of O. mykiss comprises a nucleotide sequence of 1707 base pairs with an open reading frame of 1251 base pairs, which is responsible for encoding 416 amino acids. It was found that the GnRH receptor contains leucine (L) as the most abundant amino acid. The secondary structure revealed that alpha helices constitute the largest percentage (36 %) with 153 residues, followed by extended strands with 77 residues (17.51 %). The GnRH receptor contains 26 negatively charged and 37 positively charged amino acid residues. The highest hydrophilicity was observed for lysine (K) at position 310, with a value of −3.900, while the highest hydrophobicity was found for leucine (L) at position 290, with a value of 3.80. Molecular docking analysis showed that the most favorable binding energy was observed for Gestrinone (−7.8 kcal/mol). Gestrinone was found to form hydrogen bonds with MET160, LUE245, LUE62, TYR216, and GLN209 residues of GnRH. Moreover, molecular dynamics revealed that the complexes form robust and enduring connections, indicating their structural integrity throughout the simulation. The results of this study provide insights into the protein modeling, molecular docking, and virtual screening of antagonist ligands against the GnRH receptor. Additionally, they may significantly aid in the advancement and improvement of therapeutic strategies targeted at treating various fish reproductive dysfunctions.

The present study is aimed at designing bilayer-floating tablets to improve the drug concentration in the stomach for enhanced therapeutic efficacy. The tablets are comprised of an upper layer of levofloxacin (466.5 mg) and a lower layer of famotidine (133.5 mg). Five formulations (F1-F5) were developed by using hydroxypropyl methylcellulose grades (K4M, K15M, and K100M) along with Carbopol 934. In the case of the effervescent system (F1-F3), sodium bicarbonate was added to impart buoyancy to the tablets; while in the case of noneffervescent formulations (F4 & F5), guar gum and xanthan gum were incorporated to induce flotation and swelling and retard the release of a drug. The precompression characteristics of tablets depict the suitability of all formulation powder for direct compression. The ATR-FTIR analyses have shown that the components of both effervescent and noneffervescent tablets are compatible with each other. The total weight of each tablet was 600 mg, with a weight variation of about ≤10 mg. Both the layers were smooth and flat with a thickness ranging from 3.16±0.04 to 3.54±0.01 mm. Abstract Cancer is one of the leading causes of death worldwide. The increasing prevalence and resistance to chemotherapy is responsible for driving the search of novel molecules to combat this disease. In search of novel compounds with pro-apoptotic potential, pyrazolo-pyridine and pyrazolo-naphthyridine derivatives were investigated against cervical cancer (HeLa) and breast cancer (MCF-7) cells. The anti-proliferative activity was determined through the MTT assay. Potent compounds were then analyzed for their cytotoxic and apoptotic activity through a lactate dehydrogenase assay and fluorescence microscopy after propidium iodide and DAPI staining. Flow cytometry was used to determine cell cycle arrest in treated cells and pro-apoptotic effect was verified through measurement of mitochondrial membrane potential and activation of caspases. Compounds 5j and 5k were found to be most active against HeLa and MCF-7 cells, respectively. G0/G1 cell cycle arrest was observed in treated cancer cells. Morphological features of apoptosis were also confirmed, and an increased oxidative stress indicated the involvement of reactive oxygen species in apoptosis. The compound-DNA interaction studies demonstrated an intercalative mode of binding and the comet assay confirmed the DNA damaging effects. Finally, potent compounds demonstrated a decrease in mitochondrial membrane potential and increased levels of activated caspase-9 and -3/7 confirmed the induction of apoptosis in treated HeLa and MCF-7 cells. The present work concludes that the active compounds 5j and 5k may be used as lead candidates for the development of lead drug molecules against cervical and breast cancer.

PURPOSEOvarian cancer (OC) is ranked as the third most common gynecologic cancer in various Indian cancer registries. In India, OC is seen in the younger age group, with a median age < 55 years being reported by most of the studies. The majority of patients are diagnosed in advanced stage (70%-80%), where the long-term (10-year) survival rate is poor, estimated at 15%-30%. The aim of this study was to evaluate clinical epidemiology, treatment patterns, and survival outcomes in patients with epithelial OC.METHODSThis was a retrospective analysis of patients with epithelial OC who were treated at Sher-i-Kashmir Institute of Medical Sciences, Srinagar, over a period of 9 years, from January 2010 to December 2018.RESULTSOC constituted 2.94% of all cancers registered. Epithelial OC constituted 88.4% of all OCs, with a median age 50 years. More than two third of patients belonged to rural background and the majority (76.9%) of the patients were in stage III or IV at the time of diagnosis. The main presenting symptoms were abdominal distension/bloating (46.5%) and gastrointestinal disturbances (35.2%). The most common histologic types were serous (65.9%) followed by mucinous carcinoma (15%). Median overall survival for the whole study cohort was 30 months (95% CI, 28.0 to 31.9). Median overall survival for stage I, II, III, and IV was 72, 60, 30, and 20 months, respectively.CONCLUSIONMost of the patients presented in advanced stage of the disease and have poor outcome. Delay in diagnosis and improper management before registering in tertiary cancer center and lack of tertiary care facilities are the root causes of poor outcomes. The general population and primary care physicians need to be made aware of OC symptoms.

Results of neuroimaging datasets aggregated from multiple sites may be biased by site-specific profiles in participants’ demographic and clinical characteristics, as well as MRI acquisition protocols and scanning platforms. We compared the impact of four different harmonization methods on results obtained from analyses of cortical thickness data: (1) linear mixed-effects model (LME) that models site-specific random intercepts (LMEINT), (2) LME that models both site-specific random intercepts and age-related random slopes (LMEINT+SLP), (3) ComBat, and (4) ComBat with a generalized additive model (ComBat-GAM). Our test case for comparing harmonization methods was cortical thickness data aggregated from 29 sites, which included 1,340 cases with posttraumatic stress disorder (PTSD) (6.2–81.8 years old) and 2,057 trauma-exposed controls without PTSD (6.3–85.2 years old). We found that, compared to the other data harmonization methods, data processed with ComBat-GAM was more sensitive to the detection of significant case-control differences (Χ2(3) = 63.704, p < 0.001) as well as case-control differences in age-related cortical thinning (Χ2(3) = 12.082, p = 0.007). Both ComBat and ComBat-GAM outperformed LME methods in detecting sex differences (Χ2(3) = 9.114, p = 0.028) in regional cortical thickness. ComBat-GAM also led to stronger estimates of age-related declines in cortical thickness (corrected p-values < 0.001), stronger estimates of case-related cortical thickness reduction (corrected p-values < 0.001), weaker estimates of age-related declines in cortical thickness in cases than controls (corrected p-values < 0.001), stronger estimates of cortical thickness reduction in females than males (corrected p-values < 0.001), and stronger estimates of cortical thickness reduction in females relative to males in cases than controls (corrected p-values < 0.001). Our results support the use of ComBat-GAM to minimize confounds and increase statistical power when harmonizing data with non-linear effects, and the use of either ComBat or ComBat-GAM for harmonizing data with linear effects.

As part of its COVID emergency response, the Government of Pakistan’s Ministry of Planning, Development and Special Initiatives has promulgated its first ever Mental Health and Psychosocial Support (MHPSS) initiative. Supported by UNICEF, this initiative will be piloted in Pakistan’s federal capital in coordination with other government ministries. The core feature of this initiative is a web-based integrated system that provides MHPSS interventions at multiple levels, including psychosocial support to frontline responders. For this purpose, we developed a self-help tool, MyCare+, to help users assess and manage their own stress, and to consult a counsellor if needed. It is a comprehensive, evidence-driven, confidential application adapted to local needs and consolidates clinical data for further trend analysis. It is a practical, instructed self-guide for assessment and management of stress-related conditions in the field that is based on existing evidence, thus bridging a gap. Overall, the user feedback was positive for the English and Urdu versions of MyCare+, as they found the content relevant and helpful. More than 90% of users were able to follow the instructions and felt confident to use the tool. This article outlines a blueprint for developing this toolkit, which can be easily translated into regional languages and scaled up for supporting larger populations. Key implications for practice An evidence-driven, resource effective, potentially scalable solution is presented to support the frontline responders in the COVID-19 public health crisis in LMICs. A hybrid approach is followed that offers a self-help digital solution, supplemented by person to person contact with mental health professionals. The tool is designed to help conduct individual assessments and set personalised treatment goals to support frontline workers.

Metastasis from prostate cancer to sigmoid colon without direct extension is rarest of rare finding. Patients of ca prostate presenting with lower gastro-intestinal symptoms needs complete evaluation to rule out metastasis to colorectal region from prostatic adenocarcinoma. As the primary management of primary colorectal carcinoma is surgery, whereas in case of metastatic lesion from ca prostate requires hormonal and /or chemotherapy. Therefore, distinction is essential to tailor proper treatment. As many as 20% of patients of ca prostate are diagnosed to have distant metastases at the time of presentation . We are reporting a patient of ca prostate who presented with metastatic polyps in sigmoid colon without any intra-abdominal disease.

Abstract Background Involuntary admission or treatment for the management of mental illness is a relatively common practice worldwide. Enabling legislation exists in most developed and high-income countries. A few of these countries have attempted to align their legislation with the United Nations Convention on the Rights of Persons with Disabilities. This review examined legislation and associated issues from four diverse South Asian countries (Bangladesh, India, Pakistan and Sri Lanka) that all have a British colonial past and initially adopted the Lunacy Act of 1845. Method A questionnaire based on two previous studies and the World Health Organization checklist for mental health legislation was developed requesting information on the criteria and process for involuntary detention of patients with mental illness for assessment and treatment. The questionnaire was completed by psychiatrists (key informants) from each of the four countries. The questionnaire also sought participants’ comments or concerns regarding the legislation or related issues. Results The results showed that relevant legislation has evolved differently in each of the four countries. Each country has faced challenges when reforming or implementing their mental health laws. Barriers included legal safeguards, human rights protections, funding, resources, absence of a robust wider health system, political support and sub-optimal mental health literacy. Conclusion Clinicians in these countries face dilemmas that are less frequently encountered by their counterparts in relatively more advantaged countries. These dilemmas require attention when implementing and reforming mental health legislation in South Asia.

Small molecules with nitrogen-containing scaffolds have gained much attention due to their biological importance in the development of new anticancer agents. The present paper reports the synthesis of a library of new dihydropyridine and pyridine analogs with diverse pharmacophores. All compounds were tested against the human tissue nonspecific alkaline phosphatase (h-TNAP) enzyme. Most of the compounds showed excellent enzyme inhibition against h-TNAP, having IC50 values ranging from 0.49 ± 0.025 to 8.8 ± 0.53 µM, which is multi-fold higher than that of the standard inhibitor (levamisole = 22.65 ± 1.60 µM) of the h-TNAP enzyme. Furthermore, an MTT assay was carried out to evaluate cytotoxicity against the HeLa and MCF-7 cancer cell lines. Among the analogs, the most potent dihydropyridine-based compound 4d was selected to investigate pro-apoptotic behavior. The further analysis demonstrated that compound 4d played a significant role in inducing apoptosis through multiple mechanisms, including overproduction of reactive oxygen species, mitochondrial dysfunction, DNA damaging, and arrest of the cell cycle at the G1 phase by inhibiting CDK4/6. The apoptosis-inducing effect of compound 4d was studied through staining agents, microscopic, and flow cytometry techniques. Detailed structure–activity relationship (SAR) and molecular docking studies were carried out to identify the core structural features responsible for inhibiting the enzymatic activity of the h-TNAP enzyme. Moreover, fluorescence emission studies corroborated the binding interaction of compound 4d with DNA through a fluorescence titration experiment.

BACKGROUND: Assessment of parent/caretaker satisfaction with child and adolescent mental health services in a tertiary care setting METHODS: In this descriptive cross sectional study, 130 parents/caretakers of children aged ≤ 16 years were administered Parent/Caretaker Satisfaction Questionnaire which is a 20-item, 4-point Likert type scale, after their second visit/consultation with Child Psychiatric OPD via non-probability consecutive sampling directly as well as telephonically during six months period. Demographic variables were collected via Proforma. The data was analyzed using SPSS v 16.0. p- value 60), 24.6 %(n=32) were satisfied (satisfaction between 21-60) and only 2.3%(n=3) were unsatisfied (satisfaction ≤20). No significant association was found between demographic variables and level of satisfaction. CONCLUSION: Our study revealed a high level of parent/caretaker satisfaction with Child and Adolescent mental health services in a tertiary care setting. No significant association was found between socio-demographic variables and parent/caretakers’ satisfaction. Abstract The role of the cerebrovascular network and its acute response to TBI is poorly defined and emerging evidence suggests that cerebrovascular reactivity is altered. We explored how cortical vessels are physically altered following TBI using a newly developed technique, vessel painting. We tested our hypothesis that a focal moderate TBI results in global decrements to structural aspects of the vasculature. Rats (naïve, sham-operated, TBI) underwent a moderate controlled cortical impact. Animals underwent vessel painting perfusion to label the entire cortex at 1 day post TBI followed by whole brain axial and coronal images using a wide-field fluorescence microscope. Cortical vessel network characteristics were analyzed for classical angiographic features (junctions, lengths) wherein we observed significant global (both hemispheres) reductions in vessel junctions and vessel lengths of 33% and 22%, respectively. Biological complexity can be quantified using fractal geometric features where we observed that fractal measures were also reduced significantly by 33%, 16% and 13% for kurtosis, peak value frequency and skewness, respectively. Acutely after TBI there is a reduction in vascular network and vascular complexity that are exacerbated at the lesion site and provide structural evidence for the bilateral hemodynamic alterations that have been reported in patients after TBI.

Traumatic brain injury (TBI) is a significant public health concern, and can be especially disruptive in children, derailing on-going neuronal maturation in periods critical for cognitive development. There is considerable heterogeneity in post-injury outcomes, only partially explained by injury severity. Understanding the time course of recovery, and what factors may delay or promote recovery, will aid clinicians in decision-making and provide avenues for future mechanism-based therapeutics. We examined regional changes in brain volume in a pediatric/adolescent moderate-severe TBI (msTBI) cohort, assessed at two time points. Children were first assessed 2–5months post-injury, and again 12months later. We used tensor-based morphometry (TBM) to localize longitudinal volume expansion and reduction. We studied 21 msTBI patients (5 F, 8–18years old) and 26 well-matched healthy control children, also assessed twice over the same interval. In a prior paper, we identified a subgroup of msTBI patients, based on interhemispheric transfer time (IHTT), with significant structural disruption of the white matter (WM) at 2–5months post injury. We investigated how this subgroup (TBI-slow, N=11) differed in longitudinal regional volume changes from msTBI patients (TBI-normal, N=10) with normal WM structure and function. The TBI-slow group had longitudinal decreases in brain volume in several WM clusters, including the corpus callosum and hypothalamus, while the TBI-normal group showed increased volume in WM areas. Our results show prolonged atrophy of the WM over the first 18months post-injury in the TBI-slow group. The TBI-normal group shows a different pattern that could indicate a return to a healthy trajectory. Keywords: Tensor-based morphometry, Pediatric, Traumatic brain injury, Longitudinal
