Abstract

Abstract Background Since the implementation of universal antiretroviral therapy, kidney transplantation (K-Tx) has become a valuable option for treatment of end-stage kidney disease for people with HIV (PWH) with similar patient and graft survival as compared to HIV-uninfected patients. Little is known about the hazards and manifestations of infectious disease (ID) events occurring in kidney transplant recipients with HIV. Methods Using linked information collected in the Swiss HIV Cohort Study (SHCS) and the Swiss Transplant Cohort Study (STCS), we described in-depth demographical and clinical characteristics of PWH who received a K-Tx since 2008. Further, we performed recurrent time to event analyses to understand whether HIV was an independent risk factor for ID events. Results Overall, 24 PWH with 57 ID events were included in this study (100% match of SHCS to STCS). Of these, 17 (70.8%) patients had at least one ID event: 22 (38.6%) viral (HIV not counted), 18 (31.6%) bacterial, one (1.8%) fungal and 16 (28.1%) probable infections. Most ID events affected the respiratory tract (25, 37.3%) or the urinary tract (13, 19.4%). Pathogen types and infection sites were similar in PWH and a matched control group of HIV-uninfected patients. HIV was not an independent risk factor for ID events (adjusted hazard ratio 0.94, p = 0.9). Conclusion By linking data from two large national Swiss cohorts, we provided in-depth information on ID events in PWH receiving a K-Tx in Switzerland. HIV infection was not associated with an increased hazard for ID events after K-Tx.

Abstract

Background. Since 1998, the Swiss Organ Living-Donor Health Registry (SOL-DHR) has recorded peri- and postoperative complications of living kidney (LK) donors, as reported by all Swiss transplant centers and has collected follow-up data prospectively. Methods. We analyzed the early complications of 2379 consecutive individuals who donated a kidney between January 1998 and June 2022 and assessed their health-related quality of life (HRQoL) 1 y after donation. Results. In total, 447 early complications in 404/2379 LK donors (17.0%) were reported to the SOL-DHR. The frequency of donors with major complications (ie, Dindo-Clavien classification 3/4) was 2.4%. In total, 31 donors needed reoperation, and in 13/31 (42%), donors reoperation was necessary because of bleeding complications. Independent risk factors for major early complications were older donor age (P = 0.005) and type of surgical approach (ie, the laparoscopic retroperitoneal compared with laparoscopic transabdominal surgery; P = 0.01), but not sex. We observed a U-shaped association of body mass index, where very low/high body mass indexes had higher odds of major early complications, without reaching statistical significance. Although HRQoL was affected by kidney donation, 96.5% of donors indicated that they would donate their kidney again. Abstract

Valganciclovir (VGC) is administered as prophylaxis to kidney transplant recipients (KTR) CMV donor (D)+/recipient (R)− and CMV R+ after thymoglobulin-induction (R+/TG). Although VGC dose adjustments based on renal function are recommended, there is paucity of real-life data on VGC dosing and associations with clinical outcomes. This is a retrospective Swiss Transplant Cohort Study-embedded observational study, including all adult D+/R− and R+/TG KTR between 2010 and 2020, who received prophylaxis with VGC. The primary objective was to describe the proportion of inappropriately (under- or over-) dosed VGC week-entries. Secondary objectives included breakthrough clinically significant CMV infection (csCMVi) and potential associations between breakthrough-csCMVi and cytopenias with VGC dosing. Among 178 KTR, 131 (73.6%) patients had ≥2 week-entries for the longitudinal data of interest and were included in the outcome analysis, with 1,032 VGC dose week-entries. Overall, 460/1,032 (44.6%) were appropriately dosed, while 234/1,032 (22.7%) and 338/1,032 (32.8%) were under- and over-dosed, respectively. Nineteen (14.5%) patients had a breakthrough-csCMVi, without any associations identified with VCG dosing (p = 0.44). Unlike other cytopenias, a significant association between VGC overdosing and lymphopenia (OR 5.27, 95% CI 1.71–16.22, p = 0.004) was shown. VGC prophylaxis in KTR is frequently inappropriately dosed, albeit without meaningful clinical associations, neither in terms of efficacy nor safety.

Abstract

BackgroundEnterobacterales are often responsible for urinary tract infection (UTI) in kidney transplant recipients. Among these, Escherichia coli or Klebsiella species producing extended-spectrum beta-lactamase (ESBL) are emerging. However, there are only scarce data on frequency and impact of ESBL-UTI on transplant outcomes.MethodsWe investigated frequency and impact of first-year UTI events with ESBL Escherichia coli and/or Klebsiella species in a prospective multicenter cohort consisting of 1,482 kidney transplants performed between 2012 and 2017, focusing only on 389 kidney transplants having at least one UTI with Escherichia coli and/or Klebsiella species. The cohort had a median follow-up of four years.ResultsIn total, 139/825 (17%) first-year UTI events in 69/389 (18%) transplant recipients were caused by ESBL-producing strains. Both UTI phenotypes and proportion among all UTI events over time were not different compared with UTI caused by non-ESBL-producing strains. However, hospitalizations in UTI with ESBL-producing strains were more often observed (39% versus 26%, p = 0.04). Transplant recipients with first-year UTI events with an ESBL-producing strain had more frequently recurrent UTI (33% versus 18%, p = 0.02) but there was no significant difference in one-year kidney function as well as longer-term graft and patient survival between patients with and without ESBL-UTI.ConclusionFirst-year UTI events with ESBL-producing Escherichia coli and/or Klebsiella species are associated with a higher need for hospitalization but do neither impact allograft function nor allograft and patient survival.

Abstract

Abstract Background Many potential prognostic factors for predicting kidney transplantation outcomes have been identified. However, in Switzerland, no widely accepted prognostic model or risk score for transplantation outcomes is being routinely used in clinical practice yet. We aim to develop three prediction models for the prognosis of graft survival, quality of life, and graft function following transplantation in Switzerland. Methods The clinical kidney prediction models (KIDMO) are developed with data from a national multi-center cohort study (Swiss Transplant Cohort Study; STCS) and the Swiss Organ Allocation System (SOAS). The primary outcome is the kidney graft survival (with death of recipient as competing risk); the secondary outcomes are the quality of life (patient-reported health status) at 12 months and estimated glomerular filtration rate (eGFR) slope. Organ donor, transplantation, and recipient-related clinical information will be used as predictors at the time of organ allocation. We will use a Fine & Gray subdistribution model and linear mixed-effects models for the primary and the two secondary outcomes, respectively. Model optimism, calibration, discrimination, and heterogeneity between transplant centres will be assessed using bootstrapping, internal-external cross-validation, and methods from meta-analysis. Discussion Thorough evaluation of the existing risk scores for the kidney graft survival or patient-reported outcomes has been lacking in the Swiss transplant setting. In order to be useful in clinical practice, a prognostic score needs to be valid, reliable, clinically relevant, and preferably integrated into the decision-making process to improve long-term patient outcomes and support informed decisions for clinicians and their patients. The state-of-the-art methodology by taking into account competing risks and variable selection using expert knowledge is applied to data from a nationwide prospective multi-center cohort study. Ideally, healthcare providers together with patients can predetermine the risk they are willing to accept from a deceased-donor kidney, with graft survival, quality of life, and graft function estimates available for their consideration. Abstract

Pancreas transplantation for patients with type 1 diabetes is a therapeutic option when other treatments are not effective and physical complications occur. Psychological burden is prominent in patients, and non-adherence to treatment is often one manifestation of such difficulties. Time projection is an important factor affected by chronic disease. The prospect of transplantation has the potential to repair this disruption. It could re-establish a continuity in the patient’s self and history, by connecting the future to a life that was only about past and present. Taking care of oneself, adhering to treatment, being part of a long-term therapeutic project and going through transplantation are all processes that need a good ability to self-project in time. This is specifically a domain of psychotherapeutic interventions. In this article, the psychological implications of pancreas transplantation for patients and caregivers alike will be discussed, as well as the role of the psychiatrist in the transplantation process.

10. EXAM: Ex vivo allograft monitoring dashboard for the analysis of hypothermic machine perfusion data in deceased-donor kidney transplantation

Abstract

Background: Deceased-donor kidney allografts are exposed to injury during ex vivo transport due to the lack of blood oxygen supply. Hypothermic machine perfusion (HMP) is effective in reducing the risk of delayed graft function in kidney transplant recipients compared to standard cold storage. However, there is no software implementation available to read, process and analyse HMP data for state-of-the-art visualization and quality control in an analytics dashboard. Results: We implemented the tool EXAM (ex vivo allograft monitoring) which includes an interactive analytics dashboard. The backbone is a collection of functions in the R programming language to read, process and analyse HMP data from the LifePort kidney transporter (Organ Recovery Systems, USA). Time series for pressure, flow rate, organ resistance and temperature are visualized and relevant statistical indicators have been developed. We highlight a few showcases for data analysis and quality control. Conclusions: Allograft monitoring during transport and retrospective HMP data analysis are essential for quality control and novel research. EXAM is freely available software written in the R programming language that can be easily deployed by statisticians and data scientists to enable quality control at transplant organizations that use LifePort kidney transporters. The open development enables collaboration to further extend EXAM, e.g. integrating data from other kidney transporters or even form other organs into a unified state-of-the-art monitoring tool.

16. MO886: Outcomes in Incident Patients Treated With Incremental Haemodialysis as Compared With Thrice-Weekly Haemodialysis and Peritoneal Dialysis: The Geneva Experience

Abstract

BACKGROUND AND AIMS Preservation of residual kidney function (RKF) in maintenance haemodialysis (HD) patients is associated with better survival and quality of life. RKF may be better preserved with an incremental HD (frequency METHOD Incremental HD was implemented in incident HD patients with a urine output of >600 mL/day, a urea clearance (KRU) ≥2 mL/min and an interdialytic weight gain RESULTS From January 2013 to December 2020, 296 patients started chronic dialysis, with 162 on thrice-weekly HD, 63 on incremental HD and 71 on peritoneal dialysis (PD). Patients on incremental HD did not differ from those on thrice-weekly HD or PD in terms of age, gender and comorbidity score. Diuresis, eGFR and KRU at incremental HD initiation were 1842 ± 749 mL/day, 6.7 ± 3.1 mL/min and 4.0 ± 1.8 mL/min, respectively. Among patients on incremental HD, four could retrieve a sufficient RKF to become dialysis-independent and two were transplanted. Among the 57 remaining patients on incremental HD, median duration until transition to a thrice-weekly HD regimen or death was 10 (6–20) months. Within the first year of dialysis, median survival and hospital-free days were higher in patients starting with incremental HD as compared with thrice-weekly HD: 91% versus 77%, P = 0.02 and 348 (316–362) versus 338 (295–354) days; P = 0.03. CONCLUSION These preliminary results show that incremental HD can be implemented in incident HD patients as long as regular clinical and RKF assessments are found adequate. A median duration of 10 months before transition to thrice-weekly HD can be expected in this setting. Results of randomised clinical trials assessing long-term survival and quality of life in incremental HD are awaited prior to its large-scale implementation.

Abstract

Background Residual kidney function is considered better preserved with incremental haemodialysis (I-HD) or peritoneal dialysis (PD) as compared with conventional thrice-weekly HD (TW-HD) and is associated with improved survival. We aimed to describe outcomes of patients initiating dialysis with I-HD, TW-HD or PD. Methods We conducted a retrospective analysis of a prospectively assembled cohort in a single university centre including all adults initiating dialysis from January 2013 to December 2020. Primary and secondary endpoints were overall survival and hospitalization days at 1 year, respectively. Results We included 313 patients with 234 starting on HD (166 TW-HD and 68 I-HD) and 79 on PD. At the end of the study, 10 were still on I-HD while 45 transitioned to TW-HD after a mean duration of 9.8 ± 9.1 months. Patients who stayed on I-HD were less frequently diabetics (P = .007). Mean follow-up was 33.1 ± 30.8 months during which 124 (39.6%) patients died. Compared with patients on TW-HD, those on I-HD had improved survival (hazard ratio 0.49, 95% confidence interval 0.26-0.93, P = .029), while those on PD had similar survival. Initial kidney replacement therapy modality was not significantly associated with hospitalization days at 1 year. Conclusions I-HD is suitable for selected patients starting dialysis and can be maintained for a significant amount of time before transition to TW-HD, with diabetes being a risk factor. Although hospitalization days at 1 year are similar, initiation with I-HD is associated with improved survival as compared with TW-HD or PD. Results of randomized controlled trials are awaited prior to large-scale implementation of I-HD programmes.

Abstract

Major advances in the treatment of nondiabetic chronic nephropathy and ANCA associated-renal vasculitis were published within the past two years. A new formula for assessing GFR was developed that does not take ethnicity into account. For hemodialysis patients, hemodiafiltration does not diminish uremic neuropathy. In hemodialysis patients, DOACs induce less bleeding than K vitamin antagonists. Weaning of steroids should be more rapid in some transplant patients. COVID-19 vaccination is less effective in dialysis and transplant patients and will necessitate a third dose.De grandes avancées thérapeutiques ont été publiées récemment dans le chapitre de la néphropathie chronique non diabétique et des vascularites rénales. Une nouvelle formule d’estimation du débit de filtration glomérulaire estimé a été développée sans facteur de correction ethnique. En hémodialyse, l’hémodiafiltration ne diminue pas la neuropathie urémique et les anticoagulants oraux directs occasionnent moins de complications hémorragiques que les antivitamines K. Un sevrage plus rapide des corticostéroïdes chez certains greffés rénaux est possible. La vaccination contre le Covid a une efficacité moindre chez les dialysés et les transplantés rénaux, et nécessite une troisième dose.

20. Relevance of deceased donor proteinuria for kidney transplantation: A comprehensive national cohort study

Abstract

Proteinuria is frequent in patients with nephropathies and associated with progressive kidney disease and risk for end stage kidney disease. However, the relevance of deceased donor proteinuria on transplant outcome remains uncertain. In this nationwide cohort study, we evaluated the prevalence of proteinuria in deceased donor candidates and measured the impact on outcome after kidney transplantation.Data from the Swiss Organ Allocation System and the Swiss Transplant Cohort Study were analyzed, comprising 1725 donors and 1516 recipients transplanted between 2008 and 2019. We correlated urine findings with donor characteristics and quantified the impact of proteinuria on allograft function at 12 months and survival.Proteinuria influenced allocation decisions in 4.5% of nonimmunological organ declines and was the leading cause for decline in 0.2% of cases. 74.1%, 51.4%, and 35.3% of donor candidates had a baseline proteinuria above 15, 30, and 50 mg protein/mmol urine creatinine, respectively. Proteinuria above 30 mg/mmol was associated with female donor sex, mechanical resuscitation, acute kidney injury, and time delay between ICU entry and urine sampling. Donor proteinuria was not associated with patient or allograft survival, nor allograft function at 12 months.We report a high prevalence of proteinuria in donor candidates, without evidence of a deleterious impact of proteinuria on graft function and/or survival. Therefore, low-level proteinuria should not be considered a limiting contraindication for kidney allocation in deceased donor transplant.

Abstract

Kidney living donor is the best treatment of terminal kidney failure. Donors are remarkably altruistic. The first concern of the medical team is not to harm the donor and respect their will to give their kidney. The technological evolution towards mini-invasive approaches has largely contributed to a better post-operative recovery. The evolution of this trend has led us to use laparoscopic robot-assisted kidney harvesting as the optimal standard. This work describes our pathway to this option.Recevoir un rein par un donneur vivant est à ce jour le meilleur traitement de l’insuffisance rénale terminale. Les donneurs font un geste remarquablement altruiste. Le but primaire de l’équipe médicale est de pouvoir soigner un patient insuffisant rénal grâce au don d’organe sans nuire au donneur. Les avancées technologiques vers des approches mini-invasives ont contribué à l’amélioration de la prise en charge des donneurs en augmentant considérablement leur confort postopératoire et en réduisant drastiquement les durées moyennes d’hospitalisation. La procédure standard aux HUG à ce jour est la laparoscopie robot-assistée. Cet article retrace l’évolution mini-invasive du don de rein dans le service.

Abstract

Patients with chronic nephropathies were particularly affected during the COVID-19 pandemic. These patients suffer from multiple comorbidities and some specific treatments may put them at risk towards severe complications. Hemodialysis patients necessitating an in-center hemodialysis maintenance regimen were unable to be confined and despite strict prevention protocols suffered high infection and subsequently fatality rates. Renal transplant patients, because of their immunosuppressive drugs, also represent an at-risk population of acquiring the SARS-CoV-2.Au vu de leurs particularités, les patients avec insuffisance rénale chronique ont été particulièrement affectés par la pandémie de Covid-19. Ces patients fragiles ont souvent de multiples comorbidités associées et, dans certains cas, reçoivent des traitements qui les mettent à risque de contracter ce virus et de développer des formes sévères. Les hémodialysés en centre ne pouvant être confinés et, malgré des protocoles rigoureux de prévention, ont eu dans la région genevoise une prévalence élevée d’infection. Les transplantés rénaux, du fait de leur immunosuppression, constituent également une population à risque.

Abstract

Impact of gliflozines in the treatment of non-diabetic nephropathies and cardiac failure has lately been demonstrated. Tolvaptan has now been recognized in Switzerland as a treatment of hyponatremia. In hemodialysis, some progress has been made in the management of dysfunctional arterio-venous fistulas. A glimmer of hope in the treatment of uremic pruritus? Conservative management of a stable coronary heart disease is also advocated in patients with end-stage kidney disease. Therapy with immune cells may either minimize or remove the need for immunosuppression in renal transplant patients. A new predictive score combining several markers can predict long-term graft failure.L’efficacité des gliflozines est également reconnue dans le traitement de l’insuffisance cardiaque et des néphropathies non diabétiques. Le tolvaptan est maintenant reconnu en Suisse pour le traitement de l’hyponatrémie. Une stratégie de mise en dialyse plus attentiste dans l’insuffisance rénale aiguë est définitivement confirmée. En hémodialyse, quelques progrès sont obtenus dans la prise en charge des dysfonctions d’accès vasculaires et du prurit urémique. Un traitement conservateur d’emblée est préconisé pour une coronaropathie stable également chez les patients en insuffisance rénale terminale. En transplantation rénale, l’emploi d’une immunothérapie cellulaire permettrait de diminuer ou même d’arrêter l’immunosuppression. La perte du greffon peut être évaluée avec un nouveau score prédictif combinant plusieurs marqueurs.

27. [Access to the kidney transplantation waiting list for peritoneal dialysis patients in New-Caledonia and Wallis and Futuna]

Abstract

Since 2012, a deceased donor kidney transplant program exists for dialysis patients living in New-Caledonia in collaboration with Royal Prince Alfred Hospital in Sydney, Australia. This program has reduced the time spent out-of-territory for a renal transplantation and has reduced the economic burden of end stage renal disease in New-Caledonia. We have realised a photography of kidney transplants evaluation for patients in peritoneal dialysis in New-Caledonia and Wallis and Futuna. The first aim was to describe access to kidney transplants evaluation for dialysis patients. A second aim was to compare patients with a conformed kidney transplant evaluation and patients without transplant evaluation with no obvious reasons identified.All patients in peritoneal dialysis in New-Caledonia and Wallis and Futuna at the 2018, 31In total, 61 patients were included. The average age was 62 years old. The chronic kidney disease care average time was 6.7 years and the dialysis average time was 2.0 years. Among them, 11 (18 %) were registered on the waiting list, 26 (43 %) had at least one kidney transplant medical contraindication, 3 (5 %) had begun transplant exam since less than 6 months and 21 (34 %) had no transplant exam begun or transplant exam begun since more than 6 months without medical contraindication identified. Among those 21 patients, the three most common reasons were a faulty programming transplant exam (67 %; n = 14), a remote living place (48 %; n = 10) and an intercurrent health event (29 %; n = 6). Among patients living in Noumea and suburbs, 74 % had a conformed transplant evaluation against 44 % in patients living outside Noumea and suburbs (P = 0.058). Nearly one in two patients not on the waiting list had have no information about kidney graft or the information was not recorded in the medical record.This study showed two main factors of a non-conformed transplantation evaluation: living outside Noumea and suburbs and a non-efficient planning of pre-transplant assessment exams. There is also a lack of information to the patient. These risk factors for late registration and non-registration must be considered by the healthcare teams. This study will provide a point of reference to assess the impact of actions to improve access to renal transplantation deployed in New-Caledonia.

Abstract

The aim of this study was to estimate the incidence of COVID-19 disease in the French national population of dialysis patients, their course of illness and to identify the risk factors associated with mortality. Our study included all patients on dialysis recorded in the French REIN Registry in April 2020. Clinical characteristics at last follow-up and the evolution of COVID-19 illness severity over time were recorded for diagnosed cases (either suspicious clinical symptoms, characteristic signs on the chest scan or a positive reverse transcription polymerase chain reaction) for SARS-CoV-2. A total of 1,621 infected patients were reported on the REIN registry from March 16th, 2020 to May 4th, 2020. Of these, 344 died. The prevalence of COVID-19 patients varied from less than 1% to 10% between regions. The probability of being a case was higher in males, patients with diabetes, those in need of assistance for transfer or treated at a self-care unit. Dialysis at home was associated with a lower probability of being infected as was being a smoker, a former smoker, having an active malignancy, or peripheral vascular disease. Mortality in diagnosed cases (21%) was associated with the same causes as in the general population. Higher age, hypoalbuminemia and the presence of an ischemic heart disease were statistically independently associated with a higher risk of death. Being treated at a selfcare unit was associated with a lower risk. Thus, our study showed a relatively low frequency of COVID-19 among dialysis patients contrary to what might have been assumed., Graphical abstract

Abstract

Antiphospholipid antibody syndrome (APLS) is a rare autoimmune disease characterized by recurrent arterial and venous thromboembolic events, pregnancy related complications as well as the persistent detection of antiphospholipid antibodies at a 12 week interval. Renal complications tend to occur in 3% of APLS patients, with renal artery stenosis being the most common kidney related complication. Renal pathology may be subdivided into macro as well as microvascular thrombotic complications with stenosis, thrombosis and infarction representing the principle macrovascular events and APLS nephropathy representing the predominant microvascular complication. APLS related kidney disease may present with an array of heterogenous manifestations ranging from hematuria and non-nephrotic range proteinuria to hypertension or as part of a severe, life threatening and fulminant multiorgan failure disorder known as catastrophic antiphospholipid antibody syndrome (CAPS). Management of APLS related renal complications depends on the site of vascular injury, the thromboembolic risk profile based on the subtype, isotype and titer of the autoantibodies as well as the severity of the injury. Primary prophylaxis in these patients primarily revolves around the use of low dose aspirin, with prophylactic anticoagulation during events that increase thromboembolic like surgery and hospitalization. Anticoagulation is the cornerstone of treatment of APLS related kidney disease with INR targets varying depending on the associated venous or arterial thrombosis. Immunosuppression with the likes of rituximab, mTOR inhibitors, eculizumab and belimumab have been used with some success, but lack randomized control trial validation for their use. Pulsed corticosteroids with Plasmapheresis and intravenous immunoglobulins is the recommended treatment for CAPS.

Abstract

International audience; The treatment of active antibody-mediated rejection (ABMR) is still a matter of debate, the place of rituximab remaining controversial. The French multicenter double-blind RITUX-ERAH study included 38 patients with ABMR in the first year of renal transplantation. All patients received plasma exchanges, intravenous immunoglobulins, and corticosteroids and were randomly assigned rituximab or placebo infusion at day 5. Additional rituximab infusions were allowed. In the intention-to-treat analysis, 12-month graft survival and renal function were not different between the rituximab and placebo groups. Long-term data are needed to conclude. Evaluation of the 7-year outcomes of the RITUX-ERAH study patients according to the rituximab or placebo treatment received. Eleven patients received placebo and 27 at least one infusion of rituximab. Seven years after ABMR, death-censored kidney allograft survival and renal function were not different between the groups. The evolution of anti-HLA sensitization was similar. There was no statistically significant difference in the incidence of infectious or neoplastic complications, but to be noted, seven cancers developed in six patients treated with rituximab (mean period of 44~months post-ABMR). In this cohort, there was no benefit 7~years after ABMR of rituximab in addition to plasma exchanges, intravenous immunoglobulins, and steroids.

32. Aesthetic facial contour augmentation with microlipofilling

Abstract

Background We have found that the use of microfat grafts can improve the contour of the face. Objective We describe our technique for facial contour augmentation using microlipofilling and present the results of 32 cases treated between 1995 and 2002. Methods We conducted a preoperative evaluation, including photographic and radiologic studies, to determine for which patients the procedure was appropriate and to identify the need for additional procedures. Fat was harvested with the use of 10-mL syringes with 14-gauge needles. Only 5 mL of fat was harvested in each syringe. After the donated fat was washed and fragmented, it was infiltrated by means of a percutaneous approach with 18- or 20-gauge needles to reduce trauma and avoid scarring. Thin (2-mm-wide) rolls of fat grafts were placed in well-vascularized tissues to prevent absorption. The amount of fat infiltrated depended on the flatness of the area. Results All patients had satisfactory outcomes after follow-up ranging from 1 to 5 years. Complications were rare. Conclusions With appropriate placement, facial contour augmentation with microlipofilling is a safe, simple, inexpensive aesthetic procedure that provides consistent, long-term aesthetic improvement.

To the Editor: We would like to add a relevant piece of information to the elegant review by Cohen et al.1 on C4d as a biomarker of rejection in kidney transplantation by discussing the pathophysiology of C4d deposition on erythrocytes (EC4d) and its potential value in clinical transplantation. EC4d was described 40 years ago as the determinant for the Chido and Rodgers blood groups. EC4d was recently described as a marker of lupus activity2 and as being related to alloantibody response in heart3 and lung4 transplantation. We recently showed that EC4d is a noninvasive biomarker of antibody-mediated rejection in kidney transplantation.5 EC4d levels paralleled donor-specific anti-human leukocyte antibody occurrence, peritubular capillary C4d staining, and were more predictive of peritubular capillaritis, with better sensitivity, specificity, and positive predictive value compared with peritubular C4d. Interestingly, we observed that the major factor determining the amount of C4d deposition is immunoglobulin (Ig)G concentration. We found that C4d deposition on erythrocytes in vitro can be triggered by changing the physical conditions of the medium. We hypothesized that IgG aggregation, owing to high sucrose concentration, initiates C4d activation in vitro. C3d is another potential covalent footprint that could be found in tissues and on erythrocytes in the range of 1–3 log higher because of the C3 amplification loop.6 Surprisingly, C3d deposits are not detected or may be detected at a very lower level than expected on erythrocytes and in tissues. Another interesting issue is the higher prevalence of C4d positivity in ABO-incompatible renal allografts (almost invariably present) when compared with positive cross-match transplants and recipients with preformed donor-specific alloantibodies. The two immunoglobulin classes are actually different in the two groups. In ABO-incompatible transplants, natural IgM antibodies that activate the complement are involved, as against anti-human leukocyte IgG antibodies in positive cross-match transplants. IgM antibodies are more efficient in classical complement pathway activation when compared with IgG antibodies.7 IgM antibodies can also lead to C4d deposition by the lectin pathway,8 thereby explaining the excess C4d deposition in ABO-incompatible kidney transplantation.
