Abstract

Objectives: Health hazards such as adverse drug reactions and prolonged morbidity are fallouts of self-medication among young people in sub-Saharan Africa. The aim of this study was to assess the prevalence, practice, determinants, and adverse effects of self-medication among young people living in a suburban community of Ekiti, Nigeria. Methods: This survey was a descriptive, cross-sectional study of 602 young people aged 16–24 years., who were randomly selected in the community. A predesigned self-administered questionnaire was used for data collection. The study was conducted from 1st March to 31st May 2023. The independent variables measured include the sociodemographic characteristics, while the dependent variables are the practice of self-medication and factors that predisposes to self-medication. Abstract

The COVID-19 pandemic presents a serious public health challenge in all countries. However, repercussions of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections on future global health are still being investigated, including the pandemic's potential effect on the emergence and spread of global antimicrobial resistance (AMR). Critically ill COVID-19 patients may develop severe complications, which may predispose patients to infection with nosocomial bacterial and/or fungal pathogens, requiring the extensive use of antibiotics. However, antibiotics may also be inappropriately used in milder cases of COVID-19 infection. Further, concerns such as increased biocide use, antimicrobial stewardship/infection control, AMR awareness, the need for diagnostics (including rapid and point-of-care diagnostics) and the usefulness of vaccination could all be components shaping the influence of the COVID-19 pandemic. In this publication, the authors present a brief overview of the COVID-19 pandemic and associated issues that could influence the pandemic's effect on global AMR.

7. Evaluation of Anxiolytic Potential of Extract and Fractions of Vigna unguiculataspp dekindtianain Mice

Abstract

Anxiety disorder is mental disease that is characterised by feeling of worry or fear that interferes with daily activity. The folkloric use of Vigna unguiculataspp dekindtianahas been reported in the management of neurological diseases such as epilepsy and anxiety. Although orthodox drugs are available for this disorder, medicinal plants have been proven to have curative potentials for this disorder with minimal side effect, hence this study. The anxiolytic property of the methanol extract and fractions of Vigna unguiculataspp dekindtianawas evaluated using elevated plus maze (EPM) and hole-board models in mice. The extract and fractions were administered orally and once daily to mice of both sexes (20.6 ± 1.5 g) at 50, 100, and 200 mg/kg dose levels. The probable mechanism of anxiolytic-like activity was assessed using flumazenil, yohimbine and cyproheptadine antagonists as receptor probes. One-way ANOVA was applied for the analysis of data and significance level was determined at p< 0.05. The results showed that the extract, the ethylacetate fraction and the aqueous fraction (VUA) significantly increased the frequency of entry and the time spent on the open arm of the EPM at 50 and 100 mg/kg significantly, suggesting the anxiolytic-like activity. The open arm avoidance index further validated the anti-anxiety property of the extract and fractions. These effects were reversed by flumazenil, a GABAergic receptor antagonist (3 mg/kg), yohimbine (1 mg/kg), an antagonist of alpha2-adrenergic receptor, and cyproheptadine (0.5 mg/kg), a serotonergic receptor antagonist. There was also an increase in the frequency of head dips at 50 (except VUA) and 100 mg/kg, suggesting an anti-anxiety-like effect of the extract and fractions in the hole-board model compared to the control. The study concluded that the extract, aqueous and ethylacetate fractions of V. unguiculataspp dekindtianaexhibited an anti-anxiety effect in mice which was probably mediated through GABAergic, α2-adrenergic and serotonergic pathways.

Abstract

Highlights: Anxiety is an important morbidity among visually impaired older adults. There is potential for stakeholders to specifically channel their interventions towards particular IVI domains in order to reduce anxiety prevalence. Abstract: Visual impairment is a significant morbidity among adults globally. It has negative impacts on work force participation and productivity, as and well as higher depression and anxiety disorder rates. In the case of older adults, vision impairment can contribute to social isolation and a greater likelihood of early entry into nursing or care homes. This hospital-based, cross-sectional study was designed to determine anxiety disorder's prevalence among the study population and which Impact of Visual Impairment (IVI) domain is most correlated with anxiety disorder. The study was carried out at the Ophthalmology Clinic of the Ekiti State University Teaching Hospital, Ado Ekiti, Nigera. Questionnaires were used to collect information from 115 respondents. Information sought included sociodemographic information, anxiety status, and visual impairment's impact on daily functions. The Hospital Anxiety and Depression Scale (HADS) was used to assess anxiety disorder, while the IVI scale was used to assess the impact of visual impairment. The IVI scale is an instrument used for determining the impact of visual impairment under three domains: (reading and accessing information, mobility and independence, and emotional well-being). Anxiety disorder was detected in 10.4% of all the respondents, and the IVI's mobility and independence domain of IVI was most strongly correlated with anxiety disorder.

Abstract

In this review, the ethical issues peculiar to the COVID-19 pandemics and the role of healthcare workers, especially those in resource-limited settings are x-rayed. We contend that there is a pressing ethical issue that needs urgent clarification on the rights and responsibilities of healthcare workers, especially in the current context of COVID-19 pandemic preparedness and responsiveness. We searched MEDLINE, Web of Science, EMBASE, Google Scholar, PUBMED related articles, newspaper articles, and online news sources for relevant information. The various professional codes of conduct (World Medical Association, Medical and Dental Council of Nigeria) were also consulted. The ethical principles of equitable distribution of healthcare resources, confidentiality with associated stigmatization, issues relating to duty to care by the healthcare workers and those pertaining to conduct of clinical trials and access to approved therapies or vaccines were highlighted in this study. We agree with the submission that healthcare workers only have a moral duty to treat patients with COVID-19 if the necessary protective equipment and adequate compensation are not provided We argue that the duty of physicians and other healthcare workers to care for patients during pandemics such as COVID-19 is obligatory in the absence of required protective equipment and other forms of compensation. There is a need for the government and other stakeholders to put in place a National Pandemic /Epidemic Ethical Framework to address these identified ethical challenges.

Abstract

Globally, the inappropriate dispensing and use of antibiotics in animals has contributed to the development of bacterial antimicrobial resistance (AMR). In Zambia, there is insufficient infor-mation among community pharmacy professionals on antibiotic use (AMU) and AMR in food-producing animals. This study assessed community pharmacy professionals' knowledge, attitudes and practices regarding poultry antibiotic dispensing, use, and bacterial AMR the Lusaka district of Zambia. A cross-sectional study was conducted among 178 community pharmacy pro-fessionals between February and April 2022 using a semi-structured questionnaire. Data were analyzed using Stata version 17. Of the total participants (n=178), 51.1% (n=91) were pharmacists. The most dispensed antibiotic was oxytetracycline, a Watch antibiotic, mainly without prescrip-tions. Good knowledge of AMU and AMR was associated with work experience for more than one year (p=0.016), while good practices were associated with male gender (p=0.039) and work expe-rience of more than one year (p=0.011). The study found moderate knowledge, positive attitudes and moderate practices of pharmacy professionals on poultry AMU and AMR. There was high dispensing of poultry antibiotics without prescriptions, which calls for strict implementation of antimicrobial stewardship and surveillance programs in poultry production in Zambia to reduce AMR.

Abstract

Introduction: In many low and middle-income countries (LMIC), drug promotional literature (DPL) remains one of the main sources of drug information for physicians. Studies conducted in many LMICs showed poor compliance to the WHO guidelines for ethical drug promotion especially in the area of information about excipients, adverse drug reactions, drug-drug interactions and contra-indications. These inadequacies in the information provided may mislead the prescriber with potential adverse consequences among patients using the medicines. Nigeria has a big pharmaceutical sector which is poorly regulated and we hypothesize that such unethical drug promotional practices may exist. This study therefore set out to assess compliance to the WHO ethical drug promotion (using DPL) at the Ekiti State University Teaching Hospital (EKSUTH), Ado-Ekiti, South-West Nigeria. Methodology: This was a descriptive cross-sectional study conducted in several specialist clinics of EKSUTH, Ado-Ekiti. Printed DPLs (brochures and leaflets) were collected from these clinics, collated using a pre-designed data collection form and analyzed using the WHO ethical criteria for medicinal drug promotion. Results: Two hundred seventy-five DPLs were screened A total of out of which 234 DPLs were selected after screening to after removal of duplications. Only 5 (2.1%) DPLs met all the WHO criteria. DPLs promoting antibiotics, cardiovascular drugs, and vitamins/nutritional supplements were in the majority (22.2%, 17.1%, and 11.5% respectively. Most of the DPLs had the generic (223; 95.3%) and brand (234; 100%) names, active ingredients (209; 89.3%), excipients (149; 63.7%), and indications (232; 99.1%). Information about adverse drug reactions (76; 32.5%), contra-indications (73; 31.2%), and drug interactions (46; 19.7%) was less represented. Only 59 (25.2%) DPLs had references. Fixed-dose combination drugs made up 34.6% of drugs being promoted. Conclusion: The DPLs assessed in this study had low adherence to WHO ethical drug promotion criteria especially those related to adverse drug reaction, drug interactions, and contra-indications. Abstract

The increasing use of lower-cost multiple sourced medicines (generics) and biosimilars are essential to attain or retain universal healthcare in the face of continual pressure on available resources. Alongside this, reduce patient co-payments and enhance access to medicines in countries with high co-payments where affordability was an issue. Supply-side measures, including aggressive procurement practices and increased transparency in the manufacture and pricing of generics, can appreciably lower the price of generics. Such measures alongside demand-side measures can enhance savings versus originators as well as increase access to standard medicines. However, there needs to be trust in the generics to maximise savings. The same applies to biosimilars. Multiple demand-side measures can also appreciably enhance the preferential prescribing of multiple sourced medicines versus still patented medicines in a class or related class without compromising care. As a result, enhance utilisation at lower costs. Countries can learn from each other when planning or instigating reforms, and this will grow.

Abstract

The COVID-19 pandemic presents a serious public health challenge in all countries. However, repercussions of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections on future global health are still being investigated, including the pandemic's potential effect on the emergence and spread of global antimicrobial resistance (AMR). Critically ill COVID-19 patients may develop severe complications, which may predispose patients to infection with nosocomial bacterial and/or fungal pathogens, requiring the extensive use of antibiotics. However, antibiotics may also be inappropriately used in milder cases of COVID-19 infection. Further, concerns such as increased biocide use, antimicrobial stewardship/infection control, AMR awareness, the need for diagnostics (including rapid and point-of-care diagnostics) and the usefulness of vaccination could all be components shaping the influence of the COVID-19 pandemic. In this publication, the authors present a brief overview of the COVID-19 pandemic and associated issues that could influence the pandemic's effect on global AMR.
