Abstract

Purpose: To compare short‐term patient‐reported outcomes (PRO) of two contemporary matrix‐associated autologous chondrocyte implantation (M‐ACI) products for the treatment of large articular cartilage defects of the knee. Methods: A retrospective, registry‐based, matched‐pair analysis was performed, comparing PRO of patients undergoing isolated M‐ACI with either Spherox™, a spheroid‐based ACI (Sb‐ACI), or NOVOCART™ Inject, a hydrogel‐based ACI product (Hb‐ACI), for a focal full‐thickness cartilage defect of the knee ≥4 cm2. Matching parameters included age, sex, body mass index, defect size, defect localization, symptom duration and previous surgeries. The Knee Injury and Osteoarthritis Outcome Score (KOOS) and the International Knee Documentation Committee (IKDC) score were obtained up to the 24‐month follow‐up. The total KOOS response rate and percentage of patients attaining a substantial clinical benefit (SCB) in KOOS subscores were calculated. Results: A total of 45 patients per group were matched. The response rate after 24 months was not significantly different between the groups (Sb‐ACI 64.4% vs. Hb‐ACI 82.2%, p = 0.057). The number of patients with a SCB at 24 months was not significantly different in any KOOS subscore, despite significantly higher improvement of the total KOOS (14.8 ± 16.2 vs. 21.5 ± 15.4, p = 0.047) and KOOS pain in the Hb‐ACI group (12.2 ± 18.6 vs. 20.6 ± 19.1, p = 0.037). The IKDC score in the Hb‐ACI group was significantly higher at the 12‐ and 24‐month follow‐up (60.7 ± 20.2 vs. 70.9 ± 18.0, p = 0.013). Conclusion: The response rate and number of patients achieving an SCB were not significantly different between patients treated with Sb‐ACI or Hb‐ACI. Both procedures can achieve favourable 2‐year PRO. Hb‐ACI was associated with better PRO between 1 and 2 years postoperatively; however, the clinical relevance of this benefit is yet to be proven. Abstract

Die Gonarthrose ist gekennzeichnet durch eine fortschreitende Zerstörung des Gelenkknorpels, der kniegelenkführenden Bänder und des gelenkbildenden Knochens. Unterschieden werden primäre oder idiopathische von sekundären Gonarthrosen. Die konventionelle radiologische Bildgebung spielt in der Arthrosediagnostik eine große Rolle, da Aufnahmen unter Belastung möglich sind. Vor Therapiebeginn sollten bekannte Risikofaktoren für diese degenerative Gelenkerkrankung analysiert, überprüft und ggf. adressiert und modifiziert werden. Zudem ist die Indikation zu gelenkerhaltenden operativen Eingriffen frühzeitig zu überprüfen. Die nichtoperative Therapie gliedert sich in Basis- und spezifische Maßnahmen. Zu Ersteren gehören Gewichtsreduktion, Bewegungstherapie und gesundheitliche Aufklärung, zu Letzteren biomechanische Interventionen, Physiotherapie, physikalische Maßnahmen, die medikamentöse Behandlung und die Zelltherapie.

Abstract

Hintergrund Während die multiportale videoassistierte Thorakoskopie (mVATS) bereits eine große Rolle im thoraxchirurgischen Alltag einnimmt, ist der innovative und weniger invasive uniportale Zugangsweg bisher weniger weit verbreitet. Ziel der Studien war es auf Grundlage der größten deutschen Datenbank zu uniportalen VATS (uVATS) das Outcome der Patienten zu analysieren, um die Rolle der uVATS in der Behandlung von thoraxchirurgischen Patienten zu erörtern. Methoden Eine Datenbank über alle an der Charité Berlin per uVATS operierten Patienten zwischen 06/2012 und 09/2017 (n=642) wurde erstellt und hinsichtlich Notfalleingriffen, Pleuraempyemen und Pneumothoraxen ausgewertet. Die Analyse der Pleuraempyeme und Pneumothoraxe wurde bizentrisch in Zusammenarbeit mit der Universitätsklinik “Fondazione Policlinico A. Gemelly” in Rom durchgeführt. Ergebnisse Von den 642 per uVATS operierten Patienten waren 65.7% männlich und 34.3% weiblich bei einem durchschnittlichen Alter von 60.36 (16-91) Jahren. Darunter waren 12 uVATS Notfalleingriffe, die zum Stillen einer aktiven intrathorakalen Blutung durchgeführt wurden. Die durchschnittliche OP-Zeit betrug 106.25 (63-240) Minuten und die Thoraxdrainage konnte nach 6.75 (1-25) Tagen gezogen werden. Nach durchschnittlich 10.67 (4-26) Tagen konnten die Patienten wieder entlassen werden. Es konnte ein signifikanter Zusammenhang zwischen der ASA-Klassifikation und sowohl der Dauer des postoperativen Krankenhausaufenthaltes (r=0.617, p=0.032) als auch der Dauer des postoperativen Intensivstationsaufenthaltes (r=0.812, p=0.006) gezeigt werden. Ein Patient entwickelte eine postoperative Lungenentzündung und ein Patient einen Pneumothorax. Zwei Patienten starben, nicht Prozedur assoziiert, am 11. und 12. postoperativen Tag an Multiorganversagen. Es gab keine größeren Komplikationen. Unter 35 Patienten mit einem Pleuraempyem waren 30 parapneumotisch und 5 postoperativ. Alle Patienten erhielten präoperativ ein Breitspektrumantibiotikum und 23 wurden mit einer Thoraxdrainage versorgt. 42. Does Gender Influence Outcome in Cartilage Repair Surgery? An Analysis of 4,968 Consecutive Patients from the German Cartilage Registry (Knorpel Register DGOU)

Abstract

The goal was to examine gender differences of patient characteristics and outcome after cartilage repair based on a collective of nearly 5,000 patients. Patient characteristics, accompanying therapies, and outcome (Knee Injury and Osteoarthritis Outcome Score [KOOS], reoperations, patient satisfaction) of 4,986 patients of the German cartilage register DGOU were assessed by ttest for possible gender differences. Pvalues <0.05 were considered statistically significant. Women were older than men (38.07 ± 12.54 vs. 26.94 ± 12.394 years, P= 0.002), more often preoperated (0.30 ± 0.63 vs. 0.24 ± 0.55, P= 0.001), and had a longer symptom duration (25.22 ± 41.20 vs. 20.67 ± 35.32 months, P< 0.001). Men had greater mean leg axis malalignment than women (3.24° ± 3.26° vs. 2.67° ± 3.06°, P< 0.001), less favorable meniscal status (P= 0.001), worse defect stage (P= 0.006), and a more severely damaged corresponding articular surface (P= 0.042). At baseline (59.84 ± 17.49 vs. 52.10 ± 17.77, P< 0.001), after 6 months (72.83 ± 15.56 vs. 66.56 ± 17.66, P< 0.001), after 12 months (77.88 ± 15.95 vs. 73.07 ± 18.12, P< 0.001), and after 24 months (79.311 ± 15.94 vs. 74.39 ± 18.81, P< 0.001), men had better absolute KOOS values, but women had better relative KOOS increases 6 months (14.59 ± 17.31 vs. 12.49 ± 16.3, P= 0.005) as well as 12 months postoperatively (20.27 ± 18.6 vs. 17.34 ± 17.79, P= 0.001) compared with preoperatively, although 12 and 24 months postoperatively they were subjectively less satisfied with the outcome (P< 0.001) and had a higher reintervention rate at 24 months (0.17 ± 0.38 vs. 0.12 ± 0.33, P= 0.008). In summary, the present work shows specific gender differences in terms of patient characteristics, defect etiology, defect localization, concomitant therapy, and the choice of cartilage repair procedure. Unexpectedly, contrary to the established scientific opinion, it could be demonstrated that women show relatively better postoperative KOOS increases, despite a higher revision rate and higher subjective dissatisfaction.

43. Comparison of Clinical Outcome following Cartilage Repair for Patients with Underlying Varus Deformity with or without Additional High Tibial Osteotomy: A Propensity Score–Matched Study Based on the German Cartilage Registry (KnorpelRegister DGOU)

Abstract

Background Even though realignment procedures have gained popularity as concomitant techniques in cartilage repair approaches with underlying malalignment, the clinical efficacy has not been proven to full extent.Methods Out of 5474 patients from the German Cartilage Registry, 788 patients with focal cartilage defects on the medial femoral condyle having received either no accompanying surgery or high tibial osteotomy (HTO) were identified. After a 1:1 propensity score matching, outcome of 440 patients was evaluated using KOOS (Knee Injury and Osteoarthritis Outcome Score), VAS (visual analogue scale), and satisfaction during the 3-year follow-up.Results Patients having received a concomitant HTO had significantly higher postoperative KOOS values (12 months: 67.26 ± 15.69 vs.75.10 ± 16.12, P= 0.001; 24 months: 67.14 ± 23.85 vs. 77.11 ± 16.50, P= 0.010; 36 months: 74.40 ± 16.57 vs. 81.75 ± 14.22, P= 0.023) and lower pain levels (6 months: 3.43 ± 2.18 vs. 2.89 ± 2.15, P= 0.009; 12 months: 3.64 ± 2.20 vs. 2.17 ± 1.96, P< 0.001; 24 months: 4.20 ± 3.12 vs. 2.94 ± 2.45, P= 0.005; 36 months: 3.20 ± 2.18 vs. 2.02 ± 1.98, P= 0.003). One and 3 years postoperatively, concomitant HTO led to significantly higher satisfaction in patients. These advantages of accompanying HTO were also seen in the group of patients with a varus deformity of 5° or more, in which pain levels without concomitant HTO even increased during the 3-year follow-up.Conclusion The results of the present study underline the importance and safety of concomitant HTO in patients with cartilage defects and varus deformity. HTO should therefore be considered and recommended generously in patients with focal cartilage defects of the medial femoral condyle and varus deformity.

Abstract

Background: The role of triportal video-assisted thoracoscopic surgery (VATS) is widely recognized for the treatment of primary spontaneous pneumothorax (PSP). The aim of this study was to assess the effectiveness and the potential advantages of uniportal VATS (U-VATS) for the treatment of PSP compared with triportal VATS. Methods: A total of 104 triportal (n=39) and uniportal (n=65) VATS procedures where performed for the treatment of PSP in two University hospitals. The prospectively collected data of postoperative outcomes were retrospectively reviewed and a 1:1 propensity score matching analysis was performed to compare the two VATS approaches. Results: No major adverse events occurred after operation. Compared with triportal-VATS, Uniportal-VATS showed the same effectiveness in terms of risk of recurrence (null in both groups), post-operative complications (P=1.000) and operating time (66.04±16.92 vs. 74.57±21.38 min, P=0.141). However, there was a statistically significant difference in favor of uniportal-VATS in terms of necessity of further access [0 vs. 7 (30.4%), P=0.004], chest tube duration (4.39±1.41 vs. 6.32±0.94 days, P<<0.001), postoperative hospital stay (4.78±1.31 vs. 6.61±1.67 days, P<<0.001), visual analogue pain score (VAS) at 24 hours (3.45±1.41 vs. 6.44±2.45, P<<0.001), number of patients who had pain after chest drain removal [1 (4.3%) vs. 16 (69.6%), P<<0.001], VAS after drainage removal (0.11±0.47 vs. 2.74±2.25, P<<0.001), postoperative pain duration (2.50±1.20 vs. 14.82±37.41 days, P<<0.001), pain killers intake (0.75±1.06 vs. 7.53±3.96 days, P=0.001), chronic paresthesia (level scale: 0 to 2; 0 vs. 0.52±0.66, P<<0.001), chronic neuralgia (0 vs. 0.43±0.59, P<<0.001) and cosmetic results (level scale: 0 to 3; 2.91±0.28 vs. 2.00±0.77, P<<0.001). Abstract

Background: The efficacy of video-assisted thoracic surgery (VATS) in the treatment of pleural empyema has recently been proven. Till today, very few works evaluated the role of uniportal-VATS (U-VATS) approach in the treatment of pleural empyema even if it currently represents the most innovative and less invasive thoracoscopic approach. We report our experience with U-VATS in the treatment of pleural empyema. Methods: A retrospective bicentric analysis of 35 consecutive patients who underwent surgical treatment of stage II and stage III pleural empyema was performed, from January 2015 to May 2017. Results: The mean age of patients was 57.26±18.29 years and 54.3% of them were males. In 85.7% of the cases, empyema was related to a complicated parapneumonic effusion; in only 5 cases it was a post-surgical consequence. All patients were treated with broad-spectrum antibiotics and subsequent target therapy for 14.62±21.76 days prior to operation and 23 patients needed the placement of a chest tube. Twenty patients (57.1%) presented with stage III, 11 patients (31.4%) stage II and 4 patients (11.4%) stage I empyema. Complete debridement and decortication were obtained in all patients through U-VATS approach and no conversion or further access was needed for any reason. No major complication was recorded. Only 2 cases of trapped lung were not responsive to surgical treatment. At a mean follow-up of 247.42±306.29 days, 33 patients (94.3%) were alive with no recurrence, 2 patients died for causes unrelated to the operation. Conclusions: According to our experience, we consider U-VATS as an adequate procedure in the treatment of "stages II and III" empyemas when the necessary surgical expertise has been achieved. Indeed, U-VATS permits an easier performance and complete debridement and decortication, with a very low risk for conversion and excellent postoperative outcomes in terms of less pain, fast recovery and cosmetic results.

Abstract

Background: The field of orthopedics and trauma surgery is facing a serious shortage of new talent due to the increasing average age of active surgeons and a growing need for staff. The appeal of these specialties is declining among medical students., Solutions: This trend could be reversed by introducing practice-oriented curricula, mentoring programs, and early integration into professional societies. The quality of surgical education, especially bedside teaching, needs to be improved, as it is often compromised by a lack of time and insufficient pedagogical training of the educators. The practical year (PY) is crucial to attract students to these specialties, but data indicate that the quality of training in orthopedics and trauma surgery is inferior compared to other fields. Professional societies could enhance junior development through targeted programs. Moreover, the problem of recruitment is compounded by imbalanced gender ratios and working conditions that do not align with modern family models or efficient surgical training. There needs to be a rethinking of work scheduling and task distribution in surgery to make the profession more attractive and to ensure the quality of training., (© 2024.
