Indonesia and French Guiana share many geographic and medical concerns regarding access to care. The organisational needs in emergency management, particularly in the prehospital phase, are similar. Whereas emergency medicine (EM) is an academic and entire speciality in France, it is still under construction in Indonesia. In the framework of the French and Indonesian academic cooperation, the Medical school in Jakarta University and the French Guiana University in Cayenne signed a Memorandum of Understanding encompassing education programmes, joint research work and students' and health professionals' exchanges in EM. This partnership represented the first medical cooperation programme between the two countries. The first student class was launched in August 2022 and involved 50 Indonesian doctors who graduated in August 2023. The implementation and success of this Franco‐Indonesian cooperation were supported by political and academic partners from the two sides. Given the first student class's success, we aim to continue this programme, in line with Indonesia and World Health Organization's plan to establish a multi‐country training hub for health emergency operational readiness and Emergency Medical Teams. Overview of infectious and non-infectious diseases in French Guiana in 2022 Source of many myths, French Guiana represents an exceptional territory due to the richness of its biodiversity and the variety of its communities. The only European territory in Amazonia, surrounded by the Brazilian giant and the little-known Suriname, Ariane 6 rockets are launched from Kourou while 50% of the population lives below the poverty line. This paradoxical situation is a source of health problems specific to this territory, whether they be infectious diseases with unknown germs, intoxications or chronic pathologies. Some infectious diseases such as Q fever, toxoplasmosis, cryptococcosis or HIV infection are in common with temperate countries, but present specificities leading to sometimes different management and medical reasoning. In addition to these pathologies, many tropical diseases are present in an endemic and / or epidemic mode such as malaria, leishmaniasis, Chagas disease, histoplasmosis or dengue. Besides, Amazonian dermatology is extremely varied, ranging from rare but serious pathologies (Buruli ulcer, leprosy) to others which are frequent and benign such as agouti lice (mites of the family Trombiculidae) or papillonitis. Envenomations by wild fauna are not rare, and deserve an appropriate management of the incriminated taxon. Obstetrical, cardiovascular and metabolic cosmopolitan pathologies sometimes take on a particular dimension in French Guiana that must be taken into account in the management of patients. Finally, different types of intoxication are to be known by practitioners, especially due to heavy metals. European-level resources offer diagnostic and therapeutic possibilities that do not exist in the surrounding countries and regions, thus allowing the management of diseases that are not well known elsewhere. Thanks to these same European-level resources, research in Guyana occupies a key place within the Amazon region, despite a smaller population than in the surrounding countries. Thus, certain pathologies such as histoplasmosis of the immunocompromised patient, Amazonian toxoplasmosis or Q fever are hardly described in neighboring countries, probably due to under-diagnosis linked to more limited resources. French Guiana plays a leading role in the study of these diseases. The objective of this overview is to guide health care providers coming to or practicing in French Guiana in their daily practice, but also practitioners taking care of people returning from French Guiana. Seul territoire européen en Amazonie, entouré du géant brésilien et du méconnu Suriname, on y lance des fusées Ariane 6 depuis Kourou tandis que 50 % de la population vit en dessous du seuil de pauvreté. Cette situation paradoxale est source de problématiques de santé spécifiques à ce territoire, qu’il s’agisse de maladies infectieuses à germes méconnus, d’intoxications, ou de pathologies chroniques. Certaines maladies infectieuses telles que la fièvre Q, la toxoplasmose, la cryptococcose ou l’infection à VIH sont communes aux pays tempérés, mais présentent en Guyane des spécificités entraînant une prise en charge et un raisonnement médical parfois différents. Parallèlement à ces pathologies, de nombreuses maladies tropicales sont par ailleurs présentes sur un mode endémique et / ou épidémique telles que le paludisme, la leishmaniose, la maladie de Chagas, l’histoplasmose ou la dengue. De plus, la dermatologie amazonienne est extrêmement variée, allant de pathologies rares, mais graves (ulcère de Buruli, lèpre), à d’autres fréquentes et bénignes telles que les poux d’agouti (acariens de la famille des Trombiculidae) ou la papillonite. Les envenimations par la faune sauvage ne sont pas rares, et méritent une prise en charge appropriée au taxon incriminé. Les pathologies obstétricale, cardiovasculaire et métabolique cosmopolites prennent parfois en Guyane une dimension particulière à prendre en compte dans la prise en charge des patients. Enfin, différents types d’intoxication sont à connaître par les praticiens, notamment aux métaux lourds. Les ressources de niveau européen offrent des possibilités diagnostiques et thérapeutiques inexistantes dans les pays et régions des environs, permettant ainsi la prise en charge de maladies peu connues ailleurs. Du fait de ces mêmes ressources de niveau européen, la recherche en Guyane occupe une place clé au sein de la région amazonienne, malgré une population moins nombreuse que dans les pays alentour. Ainsi, certaines pathologies telles que l’histoplasmose du patient immunodéprimé, la toxoplasmose amazonienne ou la fièvre Q ne sont pratiquement pas décrites dans les pays voisins, probablement du fait d’un sous-diagnostic lié à des ressources plus limitées. La Guyane joue ainsi un rôle moteur dans l’étude de ces pathologies. Context: Road traumas represent 13.1 deaths per 105 inhabitants in Guadeloupe, with 41.2 deaths for 105 inhabitants among the 18-24 years old (vs. 5.3 and 11.4 respectively in Metropolitan France. Marie-Galante (MG), part of the Guadeloupe islands, 30km from Pointe-à-Pitre, with 10,867 inhabitants, is strongly impacted. The management of the victims involves organizational specificities, especially for the most serious patients, with the necessity of helicopter transfers to the University Hospital Center of Guadeloupe (CHUG) either directly from the accident site or secondarily after a management through the Sainte Marie Hospital Center (CHSM) of MG. Objectives: 1) Epidemiological description of road accident victims (RAV) in MG ; 2) Evaluation of initial medical care of the most serious victims. Materials and methods: Retrospective observational study from January 1st, 2016 to December 31st, 2018, including all RAV victims managed by the Medical Emergency and Resuscitation Service (SMUR) and/or by the CHSM Emergency Department (ED). The contextual and clinical victims data were collected and the patients were distributed according to the following severity Vittel Criteria (VC): - Unstable: at least one criteria: PAS 90mmHg after volume expansion, GS 9-13, anatomic lesions according to VC ; - Potentially serious: without vital distress criteria with velocity criteria or aggravating factors according to VC ; - Stable: without any of the previous criteria. Results: During the 3 years, 499 RAV victims required a rescue or medical management in MG: 33% involving light vehicles (14% non-belted), 43% involving 2-motorized-wheels (27% un-helmeted) and 12% involving bicycles. The population is young (29 [21-49]) and mostly male (77%). Of these, 423 were analyzed: 6 (1.4%) were Unstable, 11 (2.6%) Critical, 142 (34%) Potentially Severe, and 264 (62%) Stable. CHSM ED managed 95% of the victims, 7 were directly transferred to the CHUG by helicopter, 52 secondarily after a CHSM first support. At the end, 100% of Unstable patients and 90.9% of Critics were admitted to the CHUG. The medical records of patients transferred to the CHUG revealed significantly shorter delays in primary vs secondary transferred, respectively 148 [136 - 174] min versus 262 [203 - 488] min (p, Contexte : La traumatologie routière représente 13,1 décès pour 105hab.en Guadeloupe, avec 41,2 décès pour 105 hab. parmi les 18-24 ans (contre 5,3 et 11,4 respectivement en France Métropolitaine). La prise en charge des victimes implique des spécificités organisationnelles avec, notamment pour les patients les plus graves, un transfert héliporté (TH) vers le Centre Hospitalier Universitaire de Guadeloupe (CHUG) soit directement des lieux de l’accident soit secondairement après un passage par le Centre Hospitalier Sainte Marie (CHSM) de MG. Objectifs de l’étude : 1) Description épidémiologique des victimes d’accident de la voie publique (AVP) à MG ; 2) Évaluation de la prise en charge (PEC) médicale initiale des victimes les plus graves. Matériels et méthodes : Étude observationnelle rétrospective du 1er janvier 2016 au 31 décembre 2018 incluant toutes les victimes d’AVP prises en charge par le Service Médical d’Urgence et de Réanimation (SMUR) et/ou par le Service d’Accueil des Urgences (SAU) du CHSM. Les données contextuelles et cliniques des victimes ont été recueillies et les patients ont été répartis selon les Critères de Vittel (CV) de gravité à leur PEC : - Instables : au moins un critère : PAS 90mmHg après expansion volémique, SG 9-13, lésions anatomiques selon CV ; - Potentiellement graves : sans critère de détresse vitale avec critères de cinétique ou facteurs aggravants selon CV ; - Stables : sans aucun des critères précédents.Résultats : Sur la période de 3 ans, 499 victimes d’AVP ont nécessité une PEC par les secours ou médicale à MG :33% impliquant des Véhicules Légers (14% non ceinturés), 43% des 2 roues motorisés (27% non casqués) et 12% des vélos. La population est jeune (29[21-49]ans) et masculine(77%). Parmi ces patients, 423 ont été analysés: 6(1,4%) étaient Instables, 11(2,6%) Critiques, 142(34%) Potentiellement Graves et264(62%) Stables. 17. An Observational Study of Sexual Assaults in French Guiana During 2019-2020.

- Abstract
Introduction: Sexual violence is a major public health issue, including in French Guiana. The feeling of insecurity is significant in this part of France. Sexual violence is an important reason for consultation in forensic and emergency medicine. The challenge is to provide care within the first 72 hours, particularly in medicolegal terms and for infectious disease management. The objectives of our study were, firstly, to establish the epidemiology of sexual assaults at Cayenne General Hospital (CGH), and secondly, to evaluate the management of these victims., Materials and Methods: From January 1st, 2019 to December 31st, 2020, we conducted a single-center retrospective descriptive study including patients who were consulted for sexual assaults in the Forensic medicine and the Emergency departments of CGH., Results: Over this period, 400 sexual assault victims were consulted. Most of them, were women (87%) with a median age of 13 years-old [8; 17.5]. The aggressor was mostly male (99%) frequently known by the victim (87%) and from her family (39%). Suspected assaults represented 19% of consultations. The most frequent assault on women was penile-vaginal penetration (82%) and penile-anal penetration (77%) on men. The delay of consultation was superior to 72 hours in 60% of the cases. A psychological follow-up was recommended for 62% of these victims., Conclusion: This work allowed to identify a young and female population at risk, most often assaulted by a male known to her. Most of the patients consulted more than 72 hours after the assault.
