Background: Several factors contribute to post-anesthetic hepatic dysfunction, including a decrease in oxygen supply to the liver, direct physical compression of the liver, viral hepatitis, blood transfusions, preexisting hepatic dysfunction, and the use of hepatotoxic drugs. Diagnosing volatile anesthetic drug-induced liver injury (VA-DILI) involves excluding these causes. Case: The patient underwent total mastectomy under anesthesia using sevoflurane. He had diabetes, and no abnormal results were found on preoperative laboratory examinations, and surgery was uneventful. Abnormal laboratory findings were observed after surgery, including an aspartate aminotransferase level of 1,417 IU/L, an alanine aminotransferase level of 2,176 IU/L, and a total bilirubin level of 3.8 mg/dl. He presented with symptoms of mild icteric sclera, fatigue, and pruritus. After ruling out other causes of liver injury, we concluded that these results indicated VA-DILI. Asphalt mixture is a complex heterogeneous material. Its mechanical behaviour strongly depends on its mastic characteristics. Thus, an analysis of mastic can predict the behaviour of its corresponding asphalt mix. This study focuses on numerical simulation of fatigue behaviour of bituminous mastic by using a heterogeneous multiscale approach. The fatigue equation, obtained from experimental tests was applied to the numerical models. Then, the fatigue damage was computed and presented through visual illustrations as well as the local damage distribution. A new numerical method was developed to determine the local damage failure in samples. Thereafter the local damage at the mesoscale scale can be related to global damage at the macroscopic scale. The results showed that the numerical values are close to the experimental data. Finally, the developed multiscale heterogeneous approach can be applied to the larger scale of given bituminous composites such as mortar and HMA models. [ABSTRACT FROM AUTHOR]

Fatigue Avoidance Scheduling Tool (FAST) is a computerized fatigue prevention tool, which was developed based on a Bio-mathematical model called SAPTE (Sleep, Activity, Performance and Task Effectiveness). Similarly, actigraphy technology is used as a tool for sleep evaluation. This study was an attempt to assess the employability of FAST (both independently and integrated with actigraphy) for detecting operational fatigue by determining the 'Measures for diagnostic accuracy'. 103 pre-flight medical checks from 51 aircrew were evaluated to detect fatigue both clinically and by using objective measures (FAST and actigraphy integrated FAST). The 'measures of diagnostic accuracy' namely sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), likelihood ratio positive (LR +ve), likelihood ratio negative (LR -ve) and Youden's Index were determined and compared. The diagnostic measures for FAST were: sensitivity (68.4%), specificity (92.8%), PPV (68.4%), NPV (92.8%), LR+ve (9.5), LR-ve (0.34) and Youden's Index (0.61). For actigraphy integrated FAST these values were: sensitivity (84.2%), specificity (96.4%), PPV (94.2%), NPV (94.4%), LR+ve (23.5), LR-ve (0.16) and Youden's Index (0.80). The increase in sensitivity (from 68.4% to 84.2%), specificity (from 92.8% to 96.4%), PPV (from 68.4% to 94.2%), NPV (92.8% to 94.4%), LR+ve (9.5 to 23.5) and Youden's Index (0.61 to 0.80) indicated that actigraphy integrated FAST is a better screening tool in comparison to the independently employed FAST. The purpose of this literature review was to investigate the current methods to monitor neuromuscular fatigue and assess whether any of these methods were of reliable and valid use among sitting-based para-athletes. Sitting-based para-athletes refers to any athlete who uses a wheelchair for performance or competes in a sitting or reclined position that requires the use of the upper extremities. A literature search yielded a paucity of available research with just 7 articles being found eligible for inclusion. These articles highlighted that isokinetic dynamometry, athlete self-report measures and biomedical markers were all reliable measures of neuromuscular and/or muscular fatigue within this population group. However these methods are not useful as field based tests for strength and conditioning coaches as they require additional equipment, resources and training to operate. Furthermore, a review of methods to monitor neuromuscular fatigue in able-bodied athletes was included to assess whether any methods have the potential to be adapted to sitting-based para-athletes. These articles highlighted that upper-extremity sprinting ability, a plyometric push-up, isometric shoulder strength testing and a cycle sprint (using the upper extremities) could have the potential to monitor upper-extremity muscular fatigue in sitting-based para-athletes. In my practical experience as a strength and conditioning coach, isometric strength testing of the shoulder joint, using a VALD Forceframe can be used to measure shoulder strength in sitting-based para-athletes, however this method has not been studied in the scientific literature to date. Therefore, further research is required to assess the reliability and validity of the VALD Forceframe in sitting-based para-athletes before this method is used within high-performance settings. Purpose: The hyperbolic distance–time relationship can be used to profile running performance and establish critical speed (CS) and D′ (the curvature constant of the speed–time relationship). Typically, to establish these parameters, multiple (3+) performance trials are required, which can be highly fatiguing and limit the usability of such protocols in a single training session. This study aimed to compare CS and D′ calculated from a 2-trial (2-point model) and a 3-trial (3-point model) method. Methods: A total of 14 male distance runners completed 3 fixed-distance (3600, 2400, and 1200 m) time trials on a 400-m outdoor running track, separated by 30-min recoveries. Participants completed the protocol 9 times across a 12-mo period, with approximately 42 d between tests. CS and D′ were calculated using all 3 distances (3-point model) and also using the 3600- and 1200-m distances only (2-point model). Results: Mean (SD) CS for both 3-point and 2-point models was 4.94 (0.32) m·s−1, whereas the values for D′ were 123.3 (57.70) and 127.4 (57.34) m for the 3-point and 2-point models, respectively. Overall bias for both CS and D′ between 3-point and 2-point model was classed as trivial. Conclusion: A 2-point time-trial model can be used to calculate CS and D′ as proficiently as a 3-point model, making it a less fatiguing, inexpensive, and applicable method for coaches, practitioners, and athletes for monitoring running performance in 1 training session. Purpose: Although the acute effects of cold-water immersion (CWI) have been widely investigated, research analyzing the effects of CWI over a chronic period in highly trained athletes is scarce. The aim of this study was to investigate the effects of CWI during an intense 3-wk preseason phase in elite rugby athletes. Methods: A total of 23 elite male rugby union athletes were randomized to either CWI (10 min at 10°C, n = 10) or a passive recovery control (CON, n = 13) during 3 wk of high-volume training. Athletes were exposed to either CWI or CON after each training day (12 d in total). Running loads, conditioning, and gym sessions were kept the same between groups. Measures of countermovement jump, perceived muscle soreness, and wellness were obtained twice a week, and saliva samples for determining cortisol and interleukin-6 were collected once per week. Results: Although no significant differences were observed between CWI and CON for any measure, CWI resulted in lower fatigue markers throughout the study as demonstrated by the moderate effects on muscle soreness (d = 0.58–0.91) and interleukin-6 (d = −0.83) and the small effects (d = 0.23–0.38) on countermovement jump in comparison with CON. Conclusions: CWI may provide some beneficial effect by reducing fatigue and soreness during an intense 3-wk training phase in elite rugby athletes. [ABSTRACT FROM AUTHOR]

Background: The transportation and utilities industries include establishments engaged in the movement of passengers and freight, or the provision of public power, water, and other services. Along with the warehousing industry, they make up the US National Occupational Research Agenda's Transportation, Warehousing and Utilities (TWU) industry sector. In 2018 the sector composed 5% of the US workforce, with approximately 8 million workers. TWU workers experienced 19% of all fatalities among U.S. workers in 2018 and 7% of total occupational injuries and illnesses. Methods: Around‐the‐clock operations, heavy workloads, long and irregular shifts, complicated schedules, and time pressures characterize work across the US TWU sector. However, there are considerable differences in worker priorities and concerns between TWU industries. Major areas of concern within the sector include disparities in work schedules; required training for employee fatigue awareness and prevention; physical and mental job demands; and safety culture. Results: Strategies for fatigue mitigation are critical to reduce the prevalence of injuries, safety‐critical events, and crashes in TWU workers. Further research on the incidence and characterization of fatigue among TWU workers will guide the development of effective mitigation strategies. The influence of work scheduling on missed sleep opportunities and disrupted circadian rhythms should be determined. Evaluation of fatigue mitigation strategies can lead to the adoption of the most effective ones for each TWU industry. Monitoring players’ fatigue is essential to maintaining the best performance of players during sports games. The level of fatigue can be measured by the external workload, the aggregated amount of physical activity or internal workload, which is an individual’s psycho-physiological response to that activity. There have been a growing number of studies focusing on the relationship between external and internal workloads for efficient fatigue monitoring. However, they utilize aggregated features to represent the external workload, losing raw data details such as sequential information. This study proposes a deep learning algorithm to predict Rate of Perceived Exertion (RPE) from players’ movement data instead of aggregated features. Electronic Performance and Tracking Systems (EPTS) powered by GPS sensors collected players’ movement data and the RPE from training and match sessions during a Korean professional soccer team season. We preprocessed the raw GPS data to obtain linear and angular components of velocity, acceleration, and jerk. Our proposed model, named FatigueNet, effectively predicted the RPE with mean absolute error (MAE) = 0.8494 ± 0.0557 and root mean square error (RMSE) = 1.2166 ± 0.0737 using the preprocessed movement features. To interpret the predictions of the FatigueNet, we also performed regression activation mapping to localize the discriminative time intervals that contributed more to the prediction results. Our experimental results imply the possibility of automated and objective fatigue monitoring systems based on deep learning instead of arduous manual data collection from players.

This paper presents the proposed measures to prevent traffic accidents from compressed natural gas (CNG) buses. The fire-gutted bus with a CNG system was investigated in detail of component and system defects as a case study, although many single bus accidents were recorded from and caused by errant bus drivers in Thailand. Thus, several measures were proposed to encourage safer road users from bus drivers in Thailand, such as drink-not-drive, GPS-monitored speed, and the readiness of rest areas, including driver fatigue prevention. While there are many existing vehicle standards for buses from United Nations (UN) regulation and Thailand, several buses have still erupted in flames. Therefore, the proposed measures from the investigation of the fire-gutted bus and the existing vehicle regulations can be used to enhance and accelerate actions in the approval of CNG bus and periodical technical inspection for roadworthiness. Through data information from the accident investigation and proposed technical inspection, the prevention of bus accidents with CNG systems can be continuously improved and enhanced under various traffic situations. Tough bioadhesion has important implications in engineering and medicine but remains challenging to form and control. We report an ultrasound (US)-mediated strategy to achieve tough bioadhesion with controllability and fatigue resistance. Without chemical reaction, the US can amplify the adhesion energy and interfacial fatigue threshold between hydrogels and porcine skin by up to 100 and 10 times. Combined experiments and theoretical modeling suggest that the key mechanism is US-induced cavitation, which propels and immobilizes anchoring primers into tissues with mitigated barrier effects. Our strategy achieves spatial patterning of tough bioadhesion, on-demand detachment, and transdermal drug delivery. Natural plants from plateaus have been the richest source of secondary metabolites extensively used in traditional and modern health care systems. They were submitted to years of natural selection, co-evolved within that habitat, and show significant anti-fatigue-related pharmacological effects. However, currently, no review on high-altitude plants with anti-fatigue related properties has been published yet. This study summarized several Chinese traditional high-altitude plants, including Rhodiola rosea L., Crocus sativus L., Lepidium meyenii W., Hippophaerhamnoides L., which are widely used in the Qinghai–Tibet Plateau and surrounding mountains, as well as herbal markets in the plains. Based on phytopharmacology studies, deeper questions can be further revealed regarding how these plants regulate fatigue and related mental or physical disease conditions. Many active derivatives in high-altitude medical plants show therapeutic potential for the management of fatigue and related disorders. Therefore, high-altitude plants significantly relieve central or peripheral fatigue by acting as neuroprotective agents, energy supplements, metabolism regulators, antioxidant, and inflammatory response inhibitors. Their applications on the highland or flatland and prospects in natural medicine are further forecast, which may open treatments to reduce or prevent fatigue-related disorders in populations with sub-optimal health. Objectives Several studies have shown not only a high prevalence of fatigue but also a reduction in health-related quality of life (HRQoL) in patients with rheumatic diseases. Owing to insufficient research in this area, we aimed to assess the prevalence of fatigue and its contribution to impairment of HRQoL in patients with Takayasu arteritis (TAK). Methods This single-centre case–control study included 53 TAK patients who were matched by age, BMI and sex with 100 healthy individuals. Aside from the patients' general data, the following information was collected: disease activity, level of activities of daily living (HAQ), physical activity levels and chronic fatigue. Results The TAK patients and healthy individuals were comparable in terms of current age, BMI and sex distribution. The median disease duration of TAK was 13.0 (7.0–20.0) years, and 11 (20.8%) patients had active disease. Compared with healthy individuals, patients with TAK had a higher prevalence of fatigue and lower HAQ score, physical activity level and intensity, and physical and psychosocial domains of the modified fatigue impact scale (P  <   0.01). Moreover, TAK patients had increased fatigue rates compared with the healthy individuals (fatigue severity scale: odds ratio = 2.6; 95% CI = 1.2, 5.4; modified fatigue impact scale: odds ratio = 2.6; 95% CI = 1.2, 5.5). Fatigue was positively correlated with worsening HAQ, CRP levels, daily prednisone dose and disease activity, and negatively correlated with disease duration. Conclusion TAK patients have a higher prevalence of fatigue, which affects different aspects of the disease, including physical function. The study aimed to evaluate the impact of ischemia, used only before particular sets of a lower limb resistance exercise on power output. Ten healthy resistance-trained males (age = 26 ± 6 years; body mass = 90 ± 9 kg; training experience = 9 ± 7 years) performed two experimental sessions (with ischemia; control without ischemia) following a randomized crossover design. During the ischemic condition, the cuffs were inflated to 60% of arterial occlusion pressure. The cuffs were applied before each set for 4.5 min and released 30 s before the start of the set as the reperfusion (4.5 min ischemia + 0.5 min reperfusion). In the control condition, ischemia was not applied. During the experimental sessions, the subjects performed the Keiser machine squat exercise protocol which consisted of 5 sets of two repetitions, at a load of 60% of one-repetition maximum (1RM), with 5 min rest intervals between sets. The repetitions were performed with maximal velocity. The two-way repeated-measures ANOVA showed a statistically significant interaction effect for power output (p < 0.01; η2 = 0.26). There was also a statistically significant main effect of condition for power output (p = 0.02; η2 = 0.40). The post hoc analysis for interaction did not show significant differences between conditions in particular sets. The post hoc analysis for the main effect of the condition revealed that power output was significantly lower in the control group compared to the group where ischemic was used (p = 0.02). The t-test comparisons for particular sets showed a significant lower power output in set 3 (p = 0.03); set 4 (p < 0.01) and set 5 (p < 0.01) for the control condition when compared to the ischemic condition. The results indicate that ischemia applied before each set and released 30 s prior to the start of the squat exercise did not increase power output performance. However, we observed a significantly lower decline in power for the ischemic condition (4.5 min ischemia + 0.5 min reperfusion) in sets 3–5 compared to the control condition. Thus repeated ischemia with reperfusion used between sets can be an effective form of performance enhancement by preventing or at least diminishing fatigue during resistance exercise. Fatigue is one of the most common and disabling symptoms of multiple sclerosis. A recent randomized controlled trial comparing a fatigue self-management program and a general multiple sclerosis education program found that both programs improved fatigue in participants with multiple sclerosis. Participants were randomized to a self-management program (fatigue: take control, n = 109) or a multiple sclerosis education program (multiple sclerosis: take control, n = 109). This secondary analysis of that trial used multilevel moderation analysis to examine moderators of treatment-related effects on fatigue (Modified Fatigue Impact Scale) from baseline through the 6-mo follow-up. The following potential treatment moderators were examined: age, sex, cohabitation/marital status, and baseline levels of self-efficacy, depression symptoms, and sleep quality. Cohabitation status (living with or without a spouse/partner) interacted with intervention group and time to predict fatigue impact (P = 0.04). Fatigue: take control participants who lived with a spouse/partner showed a marginal effect in greater rate of improvement in fatigue compared with those who lived alone (P = 0.08). However, rates of improvement in fatigue in multiple sclerosis: take control participants were similar in those living with or without a spouse/partner. These findings suggest that living with a spouse or partner may facilitate benefit from self-management interventions for multiple sclerosis-related fatigue. In people in the chronic phase of Guillain-Barré syndrome (GBS), how much more does a supervised, individualised exercise program improve functional independence with activities of daily living than a home-based exercise program? How do the two exercise programs compare regarding their effects on muscle strength, fatigue, pain and quality of life? Randomised controlled trial with concealed allocation, intention-to-treat analysis and blinding of outcome assessors. Sixteen adults with stable residual disability ≥ 6 months after the onset of GBS. Participants in the experimental group were allocated to 60-minute sessions of physiotherapist-supervised strengthening, endurance and breathing exercises, gait training and pain management, two to three sessions/week for 12 weeks. The control group was prescribed a home program of 30-minute sessions of maintenance exercises and education in self-management, two to three sessions/week for 12 weeks. Functional independence in activities of daily living on the 100-point Barthel Index (primary outcome), muscle strength on the 60-point Medical Research Council scale, fatigue on the 0-to-63 Fatigue Severity Scale, a visual analogue scale of pain severity, and quality of life, measured at baseline and months 6 and 12. At month 6, the median between-group difference was 5 (95% CI 0 to 20) for functional independence, 8 (95% CI 4 to 18) for strength, –13 (95% CI –28 to –1) for fatigue, and 12 (95% CI 3 to 13) for the environment domain of quality of life. Estimated effects at month 12 had a similar magnitude, but most of the CIs had greater uncertainty. Supervised, individualised exercise reduced fatigue and improved strength and quality of life more than unsupervised home exercise in people with chronic Guillain-Barré syndrome. CTRI/2016/08/007150. Background: The coronavirus disease of 2019 (COVID-19) epidemic has resulted in a growing population of individuals recovering from SARS-CoV. Although the evidence is limited, observational data suggest that patients who suffer from COVID may experience symptoms long after recovery from acute illness, referred as "long COVID," post-COVID syndrome or "post-COVID conditions".The recovery from COVID-19 disease is very different but many features appear to be similar to recovery from other viral illnesses. Our study aims to investigate and validate the effectiveness of respiratory rehabilitative therapeutic program in patients with chronic COVID. Objective: To analyse the effects of a Pulmonary Tele-rehabilitation Program in Covid-19 Outpatients who received Pulmonary Rehab (Breathing and Therapeutic Exercises) compared to those who only received conventional care. Method: A Randomized Controlled Trial study has been undertaken.30 Post COVID Patients meeting inclusion criteria were randomly allocated into two groups. Experimental Group received therapeutic protocol and Controlled Group received conventional care. Duration for the study was 6 weeks. The Significant and relevant tests for this study are Modified Borg Dyspnoea Rating Scale and Fatigue Severity Scale. Result: The statistical analysis was carried out using't- test' to determine the effectiveness of breathing and therapeutic exercises. Results showed that the patients in experimental group recovered more rapidly and demonstrated independence in their ADL without any further complications.Females were more fatigued than males, therefore benefitted more from the therapeutic program. Conclusion: This study demonstrated improvement in patients who received the therapeutic program. No telerehab patients required additional oxygen supplementation or any medical care. Predictive simulation of human motion could provide insight into optimal techniques. In repetitive or long-duration tasks, these simulations must predict fatigue-induced adaptation. However, most studies minimize cost function terms related to actuator activations, assuming it minimizes fatigue. An additional modeling layer is needed to consider the previous use of muscles to reveal adaptive strategies to the decreased force production capability. Here, we propose interfacing Xia's three-compartment fatigue dynamics model with rigid-body dynamics. A stabilization invariant was added to Xia's model. We simulated the maximum repetition of dumbbell biceps curls as an optimal control problem (OCP) using direct multiple shooting. We explored three cost functions (minimizing torque, fatigue, or both) and two OCP formulations (full-horizon and sliding-horizon approaches). We adapted Xia's model by adding a stabilization invariant coefficients S = 10 5 for direct multiple shooting. Sliding-horizon OCPs achieved 20 to 21 repetitions. The kinematic strategy slowly deviated from a plausible dumbbell lifting task to a swinging strategy as fatigue onset increasingly compromised the humerus to remain vertical. In full-horizon OCPs, the latter kinematic strategy was used over the whole motion, resulting in 32 repetitions. We showed that sliding-horizon OCPs revealed a reactive strategy to fatigue when only torque was included in the cost function, whereas an anticipatory strategy was revealed when the fatigue term was included in the cost function. BACKGROUND Global nations have enforced strict health protocols because of the COVID-19's high transmission, infectivity, and mortality. As shown by increased online learning and video conferencing, the employment and education sectors are shifting to home-based activities. Video conferencing as a communication medium has subtly led to zoom fatigue. This study aimed to analyze the risk factors of zoom fatigue for early prevention and treatment. METHODS This cross-sectional study was conducted on 335 Indonesian university students selected by purposive sampling in July 2021. Data were collected using a demographic questionnaire including online courses duration during the COVID-19 pandemic; Pittsburgh sleep quality index; depression, anxiety and stress scale-21; and zoom & exhaustion fatigue (ZEF) scale through Google Form (Google LLC, USA) distributed via social media and student forums. Association and correlation tests were used, and the model was developed using linear regression. RESULTS The respondents were aged 21.3 (1.8) years with 12.8 (5.1) months of online courses during the COVID-19 pandemic and a ZEF scale of 2.8 (0.9). Students with higher ZEF had irregular physical exercise, poorer sleep quality, longer video conferencing sessions, longer months of courses during the COVID-19 pandemic, and higher mental illness (i.e., stress, anxiety, and depression). Smoking negatively correlated with fatigue (r = -0.12). The model for ZEF showed good predictability for zoom fatigue (p<0.001, R² = 0.57). CONCLUSIONS Daily exposure to video conferencing in educational settings throughout the pandemic has drastically increased zoom fatigue. OBJECTIVE: To understand hospital nurses' current fatigue risk management (FRM), identify design goals and principles, and obtain feedback on FRM design concepts. BACKGROUND: FRM systems can address fatigue and associated risks, yet they are not widely implemented in hospital nursing. This may be due to a lack of contextually appropriate FRM tools. METHODS : A user-centered design approach was used, including interviews with 21 hospital nursing stakeholders. FINDINGS: Nurses described integrated fatigue monitoring and management activities to pursue balance between work demands and capacity to meet those demands as individual nurses, within the unit, across the hospital, and over time. Seven principles were identified and applied to 2 initial design concepts for tools to support FRM. Participants' feedback on designs was positive. Aims: The aim of this study was to explore the protective role of social support and spirituality in the prevention of depression, anxiety and fatigue severity. Design: In a cross‐sectional study, 305 women of breast cancer survivors were recruited in the study. Methods: The participants answered a self‐reported the hospital anxiety and depression scale, fatigue severity scale (FSS), social support and spiritual health questionnaires. The data were analysed using a multiple linear regression model to estimate the adjusted regression coefficients. Results: A greater proportion of patients had moderate depression (62.9%) and moderate anxiety (67.3%). The higher level of social support was significantly associated with a lesser degree of depression (β=−0.05, p =.001) and anxiety (β=−0.04, p =.001), but there had been no association with the severity of fatigue score, while the association of spiritual health with depression and anxiety was not independently significant. To explore the subjective perceptions of women from rural and urban communities in Bangalore, India, regarding their perspectives about addictive substance use (ASU), and the interventions/support they need to quit use. Based on the data collected, the goal of this study was to develop an intervention that would address the women's needs. Mixed methods were used as follows: (a) door-to-door survey of women (N = 2044) regarding their use of addictive substances, and (b) focus group discussions with the women to explore their perceptions. Interviews were transcribed verbatim, themes and sub-themes identified. More than one-third (32.7%) reported past 3-month ASU, with majority reporting smokeless tobacco products (82.6%). Alcohol use was reported by 17.4%. Four major themes (with sub-themes) were identified from the focus group discussions: (1) Patterns of use (oral tobacco products-kaddipudi, thambaku; alcohol), (2) Reasons for use (to relieve stress and forget problems, tobacco use as a way of life, for postnatal benefits, due to easy availability, to reduce fatigue and pain, indigestion, hunger), (3) Low risk perception related to ASU ("We know how to use tobacco safely", "No real harm from tobacco"), and (4) Issues related to quitting ASU and help-seeking (unwillingness to quit, methods of intervention- "videos, or tablets/injections to help us quit ASU"). The findings highlight challenges involved in the development of effective interventions for women. Occupational fatigue is a serious problem since it may cause several issues, including deteriorating human performance. Some major accidents in the oil and gas industries were associated with the lack of human performance due to occupational fatigue. This study aimed to analyze the impact of occupational fatigue on human performance among oil and gas workers in Indonesia. This study used a cross-sectional design using a self-administered validated questionnaire to gather information on demographic (gender and age), work characteristics (work rotation, work duration, shift work model, day/night shift, job position), sleep debt, sleep quality, occupational (acute and chronic) fatigue, and at-risk behavior as the indicator of human performance. In this study, a total of 1,650 workers from different fields (production, drilling, well service, construction, and administration/office) participated. This study showed that occupational fatigue (both chronic and acute) has potentially decreased level of human performance. This study investigated the effect of crank length on biomechanical parameters and muscle activity during standing cycling. Ten participants performed submaximal cycling trials on a stand-up bicycle using four crank lengths. Joint angles, moments, powers, and works of the lower limbs were calculated from motion data and pedal reaction forces. Electromyographic (EMG) data were recorded from gluteus maximus (GM), vastus medialis, rectus femoris, biceps femoris (BF), gastrocnemius medialis, soleus, and tibialis anterior, and used to obtain the integrated EMG. Statistical parametric mapping was employed to analyse the biomechanical parameters throughout the pedalling cycle. Knee and hip flexion angles and hip power increased at the initiation (0–20%) of pedalling with increasing crank length, while the BF and GM muscle activities increased during propulsion (20–40%). Additionally, increasing the crank length resulted in increased knee power absorption during upstroke phase (70–100%). Peak knee extension moment increased with decreasing crank length during propulsion, but the moment at a short crank length during propulsion was comparable to fast walking. Consequently, longer crank lengths require increased propulsion power by the lower limb muscles during standing cycling compared to shorter crank lengths. Objectives: Due to accelerating wildland fire activity, there is mounting urgency to understand, prevent, and mitigate the occupational health impacts associated with wildland fire suppression. The objectives of this review of academic and grey literature were to: Identify the impact of occupational exposure to wildland fires on physical, mental, and emotional health; and Examine the characteristics and effectiveness of prevention, mitigation, or management strategies studied to reduce negative health outcomes associated with occupational exposure to wildland fire. Methods: Following established scoping review methods, academic literature as well as government and industry reports were identified by searching seven academic databases and through a targeted grey literature search. 4679 articles were screened using pre-determined eligibility criteria. Data on study characteristics, health outcomes assessed, prevention or mitigation strategies studied, and main findings were extracted from each included document. The results of this scoping review are presented using descriptive tables and a narrative summary to organize key findings. Results: The final sample was comprised of 100 articles: 76 research articles and 24 grey literature reports. Grey literature focused on acute injuries and fatalities. Health outcomes reported in academic studies focused on respiratory health (n = 14), mental health (n = 16), and inflammation and oxidative stress (n = 12). The identified studies evaluated short-term outcomes measuring changes across a single shift or wildland fire season. Most research was conducted with wildland firefighters and excluded personnel such as aviation crews, contract crews, and incident management teams. Five articles reported direct study of mitigation strategies, focusing on the potential usage of masks, advanced hygiene protocols to reduce exposure, fluid intake to manage hydration and core temperature, and glutamine supplementation to reduce fatigue. Conclusions: While broad in scope, the evidence base linking wildland fire exposure to any one health outcome is limited. The lack of long-term evidence on changes in health status or morbidity is a clear evidence gap and there is a need to prioritize research on the mental and physical health impact of occupational exposure to wildland fire. Although hemodialysis treatment is one of the main treatment methods in chronic kidney failure, it also causes many health problems. A variety of complementary and integrative therapies are used to relieve or minimize hemodialysis symptoms. Fatigue (60%-97%) is also one of these problems. Therefore, we have conducted a systematic review to determine the effect of complementary and integrative therapies on fatigue in hemodialysis patients. Relevant studies published from 2005 to 2019, in PubMed, EBSCOhost, ScienceDirect, and Cochran Library databases have been reviewed. In the study, initially, 21 039 pieces of data were obtained, and then 14 randomized controlled studies meeting the inclusion criteria were reviewed. The methodological qualities of the articles included in the study were determined by using the quality evaluation checklist in Jadad randomized controlled studies. In the literature, acupressure, aromatherapy, reflexology, massage, and yoga practice were found to be applied to hemodialysis patients and be effective in reducing pain and anxiety as well as improving sleep quality. This article reports the findings of a study designed to investigate the effectiveness of the Roy Adaptation Model, as it relates to improvements in nursing care outcomes for patients undergoing coronary bypass graft surgery. Results revealed that the implementation of a training program based on this model enhanced staff education and led to decreases in the level of fatigue and improved the quality of life for this group of patients. Fatigue is a symptom of tiredness that is related to an exertion of body metabolism. This complaint is the most challenging problem experienced by food industry workers in Indonesia. Therefore, a specific intervention preventing the long-term effect of fatigue is needed in this population. The purpose of this quasi-experimental study was to examine the initial use of chromopressure to reduce fatigue among food industry women workers. Chromopressure is a combination of chromotherapy and acupressure. Chromotherapy is a visible spectrum or colors of electromagnetic radiation to cure diseases. This is the first known study to examine the effect of chromopressure on fatigue. Three food industries were selected in the study. A total of 59 participants were recruited by using purposive and quota sampling. The intervention group (n = 30) received chromopressure for 30 minutes and usual care, but the control group (n = 29) only received usual care. Instruments used to collect the data were UNIMMA Work Fatigue Instrument, Nordic Body Map, and blood cortisol measurement. Descriptive statistics and independent t-test were applied for data analysis. The significance level of 0.05 was considered for hypothesis testing. The findings confirmed that chromopressure combined with usual care is more effective in reducing fatigue than the usual care. Furthermore, the participants felt comfortable when they received the intervention during the study. This therapy may help the nurses working in the community in terms of fatigue intervention, however further testing of the chromopressure instrument is required. Fatigue driving is one of the main causes of traffic accidents. In recent years, considerable attention has been paid to fatigue detection systems, which is an important solution for preventing fatigue driving. In order to prevent and reduce fatigue driving, a driver fatigue detection system based on computer vision is proposed. In this system, an improved face detection method is used to detect the driver's face from the image obtained by a charge coupled device (CCD) camera. Then, the feature points of the eyes and mouth are located by an ensemble of regression trees. Next, fatigue characteristic parameters are calculated by the improved percentage of eyelid closure over the pupil over time algorithm. Finally, the state of drivers is evaluated by using a fuzzy neural network. The system can effectively monitor and remind the state of drivers so as to significantly avoid or decrease the occurrence of traffic accidents. The experimental results show that the system is of wonderful real-time performance and accurate recognition rate, so it meets the requirements of practicality in driver fatigue detection greatly. Few studies have directly compared the effects of different exercise therapies on reducing fatigue in patients with multiple sclerosis. Thus, we conducted a Frequentist network meta-analysis to analyze and compare the effectiveness of different types of exercise on reducing multiple sclerosis-related fatigue. Relevant randomized controlled trials were searched in PubMed, Web of Science and Cochrane Library databases from the date of their inception up to April 1, 2021. In total, 27 articles involving 1470 participants and 10 types of interventions met the inclusion criteria. The results indicated that aquatic exercise ranked as the most effective among these interventions, and aerobic exercise had small-to-moderate effect sizes. Most of the interventions were shown to be better than the control group, except for climbing. Climbing was the only intervention that ranked worse than the controls. Purpose: Children with hearing loss (CHL) are considered at risk for listening-related fatigue and its negative consequences. We collected data via focus groups and interviews from three stakeholder groups--CHL, their parents, and teachers/school professionals--in order to define the construct of listening-related fatigue from the perspective of CHL and key stakeholders. This is an important first step in our long-term goal to construct and validate a measure (i.e., scale) of listening-related fatigue for the pediatric population, with a focus on CHL. This article provides an overview of the data gathering process, analysis of qualitative reports, and the development of a theoretical framework for understanding the experience of listening-related fatigue in CHL. Method: We conducted focus groups and interviews in school-age children exhibiting bilateral, moderate-to-profound hearing loss (n = 43), their parents (n = 17), and school professionals who work with CHL (n = 28). The discussions were audio-recorded, transcribed, and coded using a hierarchical coding system. Qualitative analysis was conducted using an iterative inductive--deductive approach. Results: We identified primary themes from the focus group/interview discussions to develop a theoretical framework of listening-related fatigue in CHL. The framework demonstrates the complex interaction among situational determinants that impact fatigue, symptoms or manifestations of fatigue, and the utilization of, and barriers to, coping strategies to reduce listening-related fatigue. Conclusions: Participant discussion suggests that listening-related fatigue is a significant problem for many, but not all, CHL. Aim: This study aimed to explore whether 30‐min rest breaks were as effective at lowering acute fatigue among 12‐h shift hospital nursing staff who cared for patients with COVID‐19 as among those who did not. Design: The study was cross‐sectional in design. Methods: Data from the SAFE‐CARE study collected online between May and June 2020 were used. A subsample (N = 338) comprised of nursing staff who reported working 12‐h shifts, and providing direct patient care in hospitals was used in this study. Data on socio‐demographics, work and rest breaks, and subjective measures of fatigue, psychological distress, sleep and health were used. Hierarchical multiple linear regression followed by stratified analyses was conducted to explore the relationships between rest breaks and acute fatigue among nursing staff groups with and without COVID‐19 patient care. Results: The sample, on average, had high acute fatigue. Around 72% reported providing care to patients with COVID‐19, and 71% reported taking rest breaks 'sometimes', 'often' or 'always'. In the group that cared for patients with COVID‐19, there was no significant relationship between rest breaks and acute fatigue (p =.507). In the group that cared for patients hospitalized for other reasons, rest breaks were associated with lower acute fatigue (p =.010). Conclusion: Our findings showed both the importance and inadequacy of rest breaks in reducing acute fatigue. The process of within‐work recovery is complex, and routine rest breaks should be facilitated by nursing management on hospital units during and after the COVID‐19 pandemic. Impact: Rest breaks may present an effective strategy in lowering fatigue. Although rest breaks were not associated with less fatigue among staff caring for patients with COVID‐19, other co‐workers experienced some fatigue recovery. For frontline nursing staff, routine rest breaks are encouraged, and a systematic evaluation pertaining the sufficiency of rest breaks during high work demands in future research is needed. • We introduce eight years' worth of New Zealand forestry incident data. • We analyse forestry incidents from the perspective of worker-failure and fatigue. • 70% of incidents in this data are attributed, at least in part, to worker-failures. • 78% of worker-failure based causes show indications of fatigue. Reports of incidents in dangerous work environments can be analysed to identify common hazards, in turn aiding in the prevention of future accidents. Whilst studies exist that do this, most focus on causes that involve physical risks. In this paper we propose an alternative approach, and illustrate causes of forestry incidents from the perspective of worker-failure and fatigue. This paper outlines the analysis of eight years' worth of New Zealand forestry incident data, with a focus on the cause of, and time that, incidents occur. This has resulted in two main findings. First, 70% of incidents can be attributed, at least in part, to worker-failures. Second, 78% of worker-failure based causes show indications of fatigue. This indicates that a significant number of forestry incidents are caused by worker-fatigue. Finally, this dataset showed inconsistencies in data quality, similar to those that exist in other datasets. This did not affect our analysis. However, these types of errors have the potential to affect the data quality in the national reporting system. The results from this study will be used in a larger project on detecting fatigue in forestry workers for injury and incident prevention. Purpose: Fatigue is one of the most common and distressing symptoms experienced by cancer survivors. Understanding fatigue trajectories from pre- to post-diagnosis could inform fatigue prevention and management strategies. Methods: We used the Surveillance, Epidemiology and End Results Medicare Health Outcomes Survey (SEER-MHOS) linked data resource to characterize fatigue trajectories and their predictors 1214 older adult survivors of breast, colorectal, or prostate cancer. Fatigue was measured prior to the cancer diagnosis (T0) and at two timepoints after diagnosis (T1: mean = 20 months and T2: mean = 39 months post-diagnosis). Latent growth curve modeling and mixed effects models for repeated measurements were used to investigate fatigue experiences before and after a cancer diagnosis. Results: Overall, mean fatigue T-scores declined (T0 = 50, T1 = 46, and T2 = 45) indicating worsening fatigue over time. Four latent trajectory subgroups were identified: severe fatigue worsening over time (8.2% of sample), severe fatigue persisting over time (14.4%), no fatigue pre-diagnosis and mild fatigue post-diagnosis (44.4%), and not fatigued (33%). Age, cancer stage, comorbidities, and depressed mood predicted membership in the two trajectory groups experiencing severe fatigue that persisted or that worsened post-diagnosis. Older age, advanced cancer stage at diagnosis, and depressed mood were significantly associated with worsening fatigue from T1 to T2 (all p < 0.0001). Conclusions: Evaluating cancer patients for depressive symptoms and considering prior fatigue levels, age, comorbid conditions, and cancer stage may help providers anticipate fatigue trajectories and implement pre-emptive strategies to lessen fatigue impact. Objectives: Ongoing calls to implement fatigue risk management in residency education assume a shared understanding of physician fatigue as a workplace hazard, yet we lack empirical evidence that all health care team members maintain this assumption. Thus, this study seeks to explore how health care team members understand the role of physician fatigue in an effort to inform the implementation of fatigue risk management in residency training and medical practice. Methods: This study uses constructivist grounded theory to explore perceptions of workplace fatigue and its impact on clinical practice. We conducted individual semistructured interviews with physicians, nurses and senior residents across four hospitals in 8 different specialties for a total of 40 participants. Constant comparative analysis guided data analysis and led to the final grounded theory. Results: While participants outlined multiple problematic manifestations of physician fatigue on clinical performance, they were reluctant to acknowledge any negative impact of fatigue on patient care. We refer to these contradictions as the fatigue paradox. Four key themes sustain the fatigue paradox: the indefatigable physician, blind spots, faith in safety nets and the minimisation of fatigue- related events. Conclusions: This study suggests that health care team members do not universally feel that physician fatigue is problematic for patient care, despite providing multiple examples to the contrary. This paradoxical understanding of fatigue likely exists because the system relies on fatigued physicians, particularly trainees, and provides few mechanisms to critically examine fatigue. Successful implementation of fatigue risk management in residency training may prove elusive if clinical supervisors are skeptical of the potentially negative impact of workplace fatigue. Aim: The aim of this paper is to investigate the effectiveness of physical exercise in managing fatigue during radiotherapy for prostate cancer patients. It explores the impact of various physical exercise regimes and their role in the prevention and management of fatigue to help inform best practice. Method: A literature search was conducted on OVID Medline database with a follow-up search on google scholar to include relevant references found during the initial search. Relevant systematic reviews and randomised controlled trials (RCTs) arising from this search were reviewed. Findings: There is evidence to support the notion that physical exercise in all its forms is an effective and safe intervention for fatigue management for prostate cancer patients undergoing radiotherapy. Although widely studied, there is limited evidence of fatigue management strategies being clearly implemented into current radiotherapy practice for patients with prostate cancer. This information is essential to enable therapeutic radiographers to educate prostate cancer patients regarding effective exercise strategies and ensure that fatigue is managed optimally. Conclusion: Further research is required into the optimum physical exercise prescription to reduce radiation-induced fatigue, and standardised best practice guidelines should be developed nationally. A future move toward patient education into physical exercise and wellbeing should be a central component of the therapeutic radiographer role with specialist advice offered by review radiographers, empowering patients to become more physically active during treatment. Therapeutic radiographers have a unique opportunity to educate and promote physical exercise through a holistic wellbeing approach that aims to mitigate fatigue and improve quality of life. People with hearing loss experience fatigue, and it is unknown whether this is alleviated by treatment with hearing aids. The objective of this study was to address this issue and to investigate the possible concomitant effect of hearing-aid fitting on activity levels. An intervention group (n = 53) who were due to be fitted with their first-ever hearing aid(s) and a control group (n = 53) who had hearing loss but no change in hearing aid status–completed a battery of self-report outcome measures four times: once before fitting, and at 2 weeks, 3 months, and 6 months post fitting. Self-report outcome measures at each assessment captured fatigue, listening effort, hearing handicap, auditory lifestyle, social participation restrictions, and work, social and physical activity levels. Hearing-aid fitting led to a significant reduction in listening-related fatigue, but not general fatigue, in the intervention group compared to the control group. Additionally, social activity level increased and social participation restriction decreased significantly after hearing aid fitting in the intervention group compared to the control group. No significant interaction was found between working status and change in listening-related fatigue score. This study is the first to make a longitudinal measurement of fatigue before and after first-ever hearing aid fitting and to identify an increase in social activity level after hearing aid fitting. BACKGROUND: : Randomized clinical trials (RCT) suggest a multidisciplinary approach to pain rehabilitation is superior to other active treatments in improving pain intensity, function, disability, and pain interference for patients with chronic pain, with small effect size (ds = 0.20–0.36) but its effectiveness remains unknown in real-world practice. OBJECTIVE: The current study examined the effectiveness of a multidisciplinary program to a cognitive and behavioral therapy (pain-CBT) in real-world patients with chronic back pain. METHODS: Twenty-eight patients ( M 푎푔푒 = 57.6, 82.1% Female) completed a multidisciplinary program that included pain psychology and physical therapy. Eighteen patients ( M 푎푔푒 = 58.9, 77.8% Female) completed a CBT-alone program. Using a learning healthcare system, the Pain Catastrophizing Scale, 0–10 Numerical Pain Rating Scale, and Patient-Reported Outcomes Measurement Information System ® measures were administered before and after the programs. RESULTS: We found significant improvement in mobility and pain behavior only after a multidisciplinary program (p 's < 0.031; d = 0.69 and 0.55). We also found significant improvement in pain interference, fatigue, depression, anxiety, social role satisfaction, and pain catastrophizing after pain-CBT or multidisciplinary programs (p 's < 0.037; d s = 0.29–0.73). Pain ratings were not significantly changed by either program (p 's > 0.207). Background: Patients with haematological cancer had considerable symptom burden, in which fatigue was the most prevalent. Almost 70% of haematological cancer patients reported fatigue. Methods: We conducted a parallel-group, non-blinded, randomised control trial at the haemato-oncology unit of University Malaya Medical Centre, from 1st October 2019 to 31st May 2020. Patients included were ≥ 18 years, had histopathological diagnosis of haematological cancer, and fatigue score of ≥4 based on the fatigue subscale of Edmonton Symptom Assessment System (ESAS). Patients allocated to the intervention group received standard care plus a guided 30-min mindful breathing session, while those in control group received standard care. The study outcomes include fatigue severity according to the fatigue subscale of ESAS, visual analogue scale of 0 – 10, and Functional Assessment of Chronic Illness Therapy Fatigue Scale Version 4, at minute 0 and minute 30. Results: Of 197 patients screened, 80 were eligible and they were equally randomised into 30-min mindful breathing versus standard care. Lymphoma (58.9%) was the commonest haematological malignancy, followed by multiple myeloma (13.8%), acute leukaemia (11.3%), myeloproliferative neoplasm (6.3%), chronic leukaemia (5.0%) and myelodysplastic syndrome (5.0%). There was no difference in the demographic and clinical characteristics between the 2 groups. At minute 0, both arms of patients had similar ESAS-fatigue score (median, 5) and FACIT-fatigue score (mean ± SD, 24.7 ± 10.6 for intervention group versus 24.7 ± 9.7 for control group). At minute 30, intervention group had lower ESAS-fatigue score (median, 3 versus 5) and FACIT-fatigue score (mean ± SD, 17.1 ± 10.5 versus 24.8 ± 11.3) compared to control group. Both the ESAS-fatigue score reduction (median, − 2 versus 0, p = 0.002) and FACIT-fatigue score reduction (mean ± SD, − 6.7 versus + 0.8; p < 0.001) for the intervention group were statistically significant. The calculated effect size Cohen's d was 1.4 for between-group comparison of differences in total FACIT-fatigue score. Conclusions: Our results provide evidence that a single session of 30-min mindful breathing was effective in reducing fatigue in haematological cancer patients. BACKGROUND: Diamond industry employees often experience musculoskeletal pain, fatigue, and stress, contributing to a low quality of life (QoL). Yoga is an ancient discipline of mind-body practice Yoga has numerous health benefits. OBJECTIVE: This study investigated the efficacy of workplace yoga in improving stress, musculoskeletal pain, fatigue, and QoL among employees of the diamond industry. METHODS: One-hundred sixty-six employees of the diamond industry between the ages of 20 and 60 participated in the study. Participants were assigned to either yoga (n = 84) or waitlist (n = 82) groups. The yoga group received one hour of yoga, four days a week for three consecutive months. Participants in the waitlist group followed their daily routines. Musculoskeletal pain, fatigue, stress, and QoL were assessed at baseline and after three months. RESULTS: One-hundred fifty-five employees completed the study. The yoga group showed significant (p < 0.05) improvement in pain, perceived stress, fatigue, and QoL domains after three months compared to baseline. The waitlist group showed no significant improvement in any of the measures. In the post scores comparison between the groups revealed a significant difference between the groups. CONCLUSION: Workplace yoga found to be a useful, cost-effective and feasible intervention in improving stress, musculoskeletal pain, fatigues and quality of life among employees of the diamond industry.
