1. Biosynthetic scaffolds for partial meniscal loss: A systematic review from animal models to clinical practice

Abstract

Acute or degenerative meniscus tears are the most common knee lesions. Meniscectomy provides symptomatic relief and functional recovery only in the short- to mid-term follow-up but significantly increases the risk of osteoarthritis. For this reason, preserving the meniscus is key, although it remains a challenge. Allograft transplants present many disadvantages, so during the last 20 years preclinical and clinical research focused on developing and investigating meniscal scaffolds. The aim of this systematic review was to collect and evaluate all the available evidence on biosynthetic scaffolds for meniscus regeneration both in vivo and in clinical studies. Three databases were searched: 46 in vivo preclinical studies and 30 clinical ones were found. Sixteen natural, 15 synthetic, and 15 hybrid scaffolds were studied in vivo. Among them, only 2 were translated into clinic: the Collagen Meniscus Implant, used in 11 studies, and the polyurethane-based scaffold Actifit®, applied in 19 studies. Although positive outcomes were described in the short- to mid-term, the number of concurrent procedures and the lack of randomized trials are the major limitations of the available clinical literature. Few in vivo studies also combined the use of cells or growth factors, but these augmentation strategies have not been applied in the clinical practice yet. Current solutions offer a significant but incomplete clinical improvement, and the regeneration potential is still unsatisfactory. Building upon the overall positive results of these “old” technologies to address partial meniscal loss, further innovation is urgently needed in this field to provide patients better joint sparing treatment options.

Abstract

Toxoplasmosis is a common disease of warm-blooded animals including man that can be trasmitted by meat containing tissue cysts. Recent findings lead to the conclusion that human toxoplasmosis has health sequelae also in the general public, in addition to congenital infections or immunosuppressive conditions. A risk based approach is therefore necessary to reduce the possibility of Toxoplasma gondii cysts in food. Due to the difficulty of revealing the infection in meats at slaughterhouse level, a reduction of the infection in the pre-harvest phase can be recommended. Few data are available on the real prevalence of the infection in Italian pig herds. A preliminary investigation on the seroprevalence in pigs reared and slaughtered in Umbria is reported. Blood samples of 288 animals were collected in three different slughterhouses and were representative of about 17% of the pigs reared in the Umbria Region. A seroprevalence of 14.58% was detected.

Abstract

Donkeys have been used as working animals for transport and farm activities worldwide. Recently, in European countries, there has been an increasing interest in donkeys due to their use as pets, onotherapy or milk production. During 2014-2016, a countrywide survey was conducted to determine prevalence and risk factors of principal helminth infections in 1775 donkeys in 77 Italian farms. A questionnaire on management and parasite control practices was filled out for each farm. Faecal samples were examined using a modified McMaster technique, a centrifugation/flotation method and a sedimentation technique. Pooled coprocultures were performed for differentiation of strongylid eggs. Strongyles were the most common parasites detected (84.9%), followed by Dictyocaulus arnfieldi (6.9%), Oxyuris equi (5.8%), Parascaris spp. (3.6%), Anoplocephala spp. (1.0%), Strongyloides westeri (0.3%). Coprocultures revealed an omnipresence of cyathostomins (100%), followed by Strongylus vulgaris (31.0%), Poteriostomum spp. (25.0%), Triodontophorus spp. (9.0%), Strongylus edentatus (7.0%), Strongylus equinus (5.0%). Logistic regression analysis identified breed, co-pasture with horses, living area, herd size and number of treatments as significantly associated with strongyles. Sex, age, living area and herd size were significantly associated with Parascaris spp. Dictyocaulus arnfieldi was significantly associated with sex, grass, co-pasture with horses, living area and herd size. Strongylus vulgaris was significantly associated with living area and herd size. The mean number of anthelmintic treatments/year was 1.4; most of the donkeys (71.8%) were dewormed using an ivermectin drug. It is important to design parasite programs to specifically address both D. arnfieldi and S. vulgaris in donkeys, and this is especially important if donkeys co-graze with horses.

8. Correlation between coronal and Apical arch form and degree of Divergence: a volumetric computed Tomography analysis

Abstract

Purpose To determine the existence of any correlation between the degree of divergence and apical and coronal arch form using cone-beam computed tomography (CBCT). Materials and methods A total of 176 (88 coronal and 88 apical) CBCT scans pertaining to a sample of 44 subjects (16 males and 28 females), of which 26 were Class I, 14 Class II and 4 Class III, were analysed. A lateral projection of the skull was obtained from each VCT and cephalometric tracing was performed (according to Ricketts) so as to divide the sample into subgroups based on the degree of divergence (11 short-faced, 18 norm-faced and 15 long-faced subjects). Dahlberg's index values were calculated and Student's t-test for paired data was applied. Results On the whole, the hyperbolic cosine curve was found to be the most representative of the arch forms considered. Conclusions A correlation between degree of divergence and the arch form of the apical and, especially, coronal portions of both the upper and lower jaws was revealed.

9. Nanostructured Coating Used to Reduce Friction

Abstract

This paper shows the experimental results obtained using a biomometic nano-coating applied to the slippers of an axial piston pump, to reduce the friction losses in order to improve the pump overall efficiency. Axial piston pumps and motors are widely used in heavy-duty applications and play a fundamental role in hydrostatic and power split drives. The mechanical power losses in hydraulic piston pumps come from the friction between parts in relative motion. The application of special surface treatments have been exploited in pioneering works in the past, trying different surface finishing or adding ceramic or heterogeneous metallic layers. The potential of structured coatings at nanoscale, with superhydrophobic and oleophobic characteristics, has never been exploited until now.

Abstract

The Balanced Budget Act of 1997, coupled with the effects of the Prospective Payment System implemented in October 2000, has created a situation that seriously compromises the financial viability of existing home health agencies. To survive the maelstrom, agencies must act rapidly to stem the financial hemorrhage and prevent closure. Attention to patient volumes, allowable reimbursement, timely submission of requests for payment, and education across the continuum will provide the best opportunity for survival in the short term. Regionalizing services available through home care will provide the best cost structure with which to spread traditional costs of doing business. Swift action to accomplish these goals today will promote stability across the home care agency arena.

Abstract

Modern health care has evolved into technological wizardry that defies the traditional concept of caring by the bedside nurse. Industry constraints have created an environment where caring for the patient and employee has become easily discarded in favor of the rigors of technological care. With an aging and diminished work force, nursing leaders must be prepared to create a caring environment where nurses are empowered to deliver the humanitarian as well as technological aspects of caring. From a concept of work redesign that mirrors traditional nursing paradigms, nurses can enrich not only their professional work environment but also deliver the high touch aspect of care in their daily work.

I Italia gli scarich di acque reflue depurate in uscita dagli impianti di depurazione sono immessi nei corpi idrici superfiociali e nel Piano di Tutela delle Acque gli scarichi sono annoverati tra le pressioni cui è sottoposta la risorsa idrica Per questo lo studio di qualità dei fiumi italiani può emergere anche da un monitoraggio qualuitativo degli scarichi con lo scopo di valutare gli eventuali impatti sui corsi d'acqua che li ricevono, ancvhe in riferimento agli obiettivi di qualità ambientale che le Regioni devono raggiungere e mantenere per i corpi idrici significativi, perchè siano mantenuti processi naturali di autodepurazione e supportatte comunità vegetali e animali ampie e ben diversificate. Nell'ambito di un progetto sperimentale è stato condotto un monitoraggioo biennale che ha consentito una prima valutazione della qualità (contenuto in metalli pesanti, metalli e microelementi) di un tratto di due corpi idrici superficiali, classificati come significativi nel Piano di Tutela delle Acque della Regione Emilia-Romagna, che ricevono lo scarico di acque reflue depurate di diversa natura.

Be prepared dear readers—the winds of change are preparing to blow at gale force. You’ve probably heard the term Accountable Care Organization (ACO), but you might not yet realize the impact that an ACO is likely to have on your total operation. In fact, the drive to ACOs is likely to deliver a dramatic change to the way home health agencies operate at all levels of the organization. Certainly the details about ACO structure and requirements are continuing to emerge at the Federal level. Yet we do have some early indication of the key features and likely downstream impacts expected with the drive to ACOs. Let’s be clear. Much of the downstream impacts written in this column represent “educated conjecture” mixed with a tincture of speculation, all based on current knowledge about the ACO requirements. Despite the fact that there are continuing refinements to the ACO program, you should expect that the final impact to home health care agencies to align pretty closely with what is written here. Regardless of the size and market penetration of your individual agency you should expect the way you operate to change fairly dramatically. In many cases, the change will not occur for everyone on a set date—the changes will most likely come as the number organizations moving to accountable care deepens its penetration across the North American health care market. The drive to accountable care is designed to bend the health care cost curve. According to the Congressional Budget Office (CBO) 96% of Medicare spending goes to seniors with four our more chronic illnesses. The most frequently treated conditions among Medicare beneficiaries include hyperlipidemia, COPD/asthma, diabetes mellitus, hypertension, mental disorders, cancer, trauma-related disorders, and heart conditions (Thorpe, Ogden, & Galactionova, 2010). Look at that list again. Think about what constitutes the majority of your visits. What percentage of your agency revenue is driven by these conditions? What would happen if that revenue was suddenly and dramatically reduced or eliminated? Are you prepared for such dramatic changes? These are just a few of the questions we must be prepared to answer. As the various components of Patient Protection and Affordable Care Act progresses (PPACA) the bar will continue to be set higher across the three major areas of coverage expansion, financing, and delivery system reform. At the core of the PPACA is the paradox of giving more care to more people while at the same time paying less for that care. How this paradox is achieved represents the million dollar question. Unfortunately, more questions than answers have emerged. Does the federal government raise taxes or cut benefits? Another possible scenario is to reallocate spending to health care from other federal programs or to simply cut reimbursement to providers. What is not in question is that every provider will need to deliver care differently amid an incentive-based reimbursement system. The focus will be a shift further away from fee for service to capitation and sharedsavings models. The first likely impact for home health agencies will occur in October 2012 when pay-for-performance payment schemes begin. The core focus of pay-for-performance will be readmissions. This program targets those patients readmitted within 30 days of discharge. In contrast to current payment rules, patients readmitted within 30 days will receive no additional payment for additional services. But the program doesn’t stop here. There is an overall payment reduction penalty to those hospitals, which experience “excessive readmissions.” The penalty will be determined by an adjustment factor calculated as the percentage of revenue paid for excessive readmissions divided by total revenue. Overall, it is expected that pay-for-performance will reduce Medicare payments by US$7.1 billion between 2013 and 2019 (Senate Finance Committee, 2009).

Abstract

To verify the possibility of phytoplasma elimination from tissue cultures without employing meristem-tip culture or antibiotics, micropropagated periwinkle shoots infected with an aster yellows phytoplasma (Hydrangea virescence phytoplasma: HyV) were obtained. Preliminary tests carried out on shoots immersed in sterile water containing decreasing concentrations of PAP-II, for different periods of time showed an increased number of necrotic shoots after 48 hours exposure. Similar tests showed that when PAP-II was added to the medium by percolation no phytotoxicity was detectable. Batches of 1-3 cm long shoots grown in a solid medium were treated with serial dilutions of purified PAP-II, After periods of growth varying from 15 to 150 days, shoots ranging from 0.1 to 0.6 g were used for DNA extraction with a silica gel system. Nested-PCR employing general and group 16SrI specific phytoplasma primers [R16F2n/R2 followed by R16(I)F1/R1] was performed to verify phytoplasma elimination in a total of about 200 infected shoots. The percentage of phytoplasma-free samples ranged from 40 to 50 % in dilutions ranging from 1:10 to 1:1,000 and for periods of time between 50 and 150 days, Asymptomatic shoots deriving from HyV infected material were found to be positive in nested-PCR as well as the symptomatic ones. Periwinkle infected shoots maintained in micropropagation on the same medium as control without PAP-II showed phytoplasma presence when tested in the same nested-PCR system.

Abstract

Greater use of computer technology has permitted rapid access to many forms of data. Hospitals have traditionally been slower to accept this technology for patient medical records. With the rapid approach of the new millennium, hospitals are being forced into re-evaluating many processes, including the volumes of data collected on paper. Electronic medical records are one approach to reducing storage and streamlining care across the health care continuum. This article examines the ethical impact of computerized medical records, including access to data, ownership of data, confidentiality, and medical record brokering.

Abstract

È stato messo a punto un sistema di monitoraggio delle acque della rete idrografica minore in aree significative della pianura padana per il controllo del loro stato qualitativo e delle eventuali fonti di inquinamento derivante dalla inunissione di acque reflue. Il progetto si è svolto in tre anni di attività sperimentale ed ha previsto l'utilizzo di sistemi informativi geografici (GIS) per le elaborazioni cartografiche. A livello di macrobacino (pianura padana) è stata ricostruita la rete idrica ed irrigua ed è stato organizzato un geodatabase dei depuratori censiti nel catasto degli scarichi regionali. A livello di medio e microbacino (Bacino del Fiume Reno) è stato costruito il Modello Digitale di Elevazione (DEM) ed è stato applicato il modello di vulnerabilità all'infiltrazione a tutta la zona di pianura emiliano-romagnola. I campionamenti di acqua sono stati eseguiti, con frequenza mensile, per 22 mesi in otto stazioni opportunamente scelte: nel Canale Emiliano Romagnolo (CER), canale artificiale di distribuzione delle acque ad uso irriguo derivate dal Po; nel Canale dei Molini; nei depuratori di Castenaso e di Imola all 'uscita scarico e a valle e a monte del Torrente ldice e del Fiume Santemo, recettori dei due scarichi. I campioni sono stati analizzati in spettrometria in emissione al plasma (lCP-OES) per la determinazione di metalli pesanti e microelementi (32 parametri). Sono stati confrontati i diversi campioni idrici e le acque reflue depurate sono state valutate sulla base della rispondenza ai limiti previsti dalla legge e della loro idoneità all 'irrigazione.
