Abstract

Introduction In 2019 there were 1,760 patients in Denmark’s hospitals who experienced cardiac arrest (IHCA patients = In Hospital Cardiac Arrest patients). Of these patients about 70% survived. There is only limited knowledge about the mental and cognitive state of cardiac arrest survivors. However, it seems, that cardiac arrest survivors, perform mentally and cognitively worse compared to the background population. The mental and cognitive difficulties can lead to reduced quality of life for both those affected and their relatives. Objectives Because the above-mentioned area has limited knowledge, further studies are needed to shed more light into the problem. Methods To find out if the patients can be included in the study, the patient journals will be studied. After that there will be performed an interview-survey-based study, in which IHCA patients’ possible symptoms of depression, anxiety, PTSD and suicide risk, the patients’ quality of life and any cognitive disorder, shortly after and three months after cardiac arrest, will be examined. The study will also, if possible, focus on the patients’ relatives and on the eventual difficulties they may experience in the aftermath of a relative surviving a cardiac arrest. The above-mentioned will be done using already existing relevant psychiatric and neuropsychological examination tools. In relation to the patients’ relatives, however, a separate survey tool, that has been developed, will be used. Results It is an ongoing study. Results are expected in 2023. Conclusions In the long run the study hopefully can contribute to establishing relevant help, counseling and rehabilitation for the patients and relatives affected. Abstract

Background Recent guidelines recommend centralisation of post-resuscitation care in out-of-hospital cardiac arrest (OHCA) patients in dedicated Cardiac Arrest Centres (CACs) aiming to improve outcome. Purpose To describe the process of implementation of the centralisation policy and impact on 30-day mortality rate. Methods This nationwide study included all adult OHCA patients (>18 years) based on the Danish Cardiac Arrest Registry. Centralisation was introduced in 2009, 2010, and 2012 in four regions (A, B, C, D), and one region has not yet implemented the policy (control). Parallel trends assumption was made and a difference-in-difference model with repeated cross-sections was used to estimate the average treatment effect on the treated patients. The 30-day mortality was compared between patients living in regions that introduced centralisation versus control region without centralisation. Results A total of 20,566 patients with OHCA were identified between 2007 and 2020. The majority were between 50 to 75 years (53.7%) and 67.8% male. Bystander cardiopulmonary resuscitation was performed in 55.3%. Shockable rhythm was present in 34.6% of the patients. The policy of centralisation of OHCA in CACs has been gradually and not yet completely implemented (Figure 1A). Patients with shockable rhythms (72%), and younger patients < 50 years (64%) were more frequently transported to CACs. The 30-day mortality was lower among patients transported to CACs (Figure 1B). Centralisation significantly improved survival by 6% (95% CI: 2%-10%) percentage points compared to the control region after controlling for all known covariates (sex, age, Charlson Comobidity Index, shockable rhythm, and bystander cardiopulmonary resuscitation). Conclusions In this nationwide study, centralisation of post-resuscitation care of OHCA in dedicated CACs has been gradually implemented over time. Although, OHCA patients with favorable prognostic factors were more frequently transported to CACs, centralisation increased survival by 6 absolute percentage points.

Abstract

Back pain is a common and debilitating disorder with largely unknown underlying biology. Here we report a genome-wide association study of back pain using diagnoses assigned in clinical practice; dorsalgia (119,100 cases, 909,847 controls) and intervertebral disc disorder (IDD) (58,854 cases, 922,958 controls). We identify 41 variants at 33 loci. The most significant association (ORIDD = 0.92, P = 1.6 × 10−39; ORdorsalgia = 0.92, P = 7.2 × 10−15) is with a 3’UTR variant (rs1871452-T) in CHST3, encoding a sulfotransferase enzyme expressed in intervertebral discs. The largest effects on IDD are conferred by rare (MAF = 0.07 − 0.32%) loss-of-function (LoF) variants in SLC13A1, encoding a sodium-sulfate co-transporter (LoF burden OR = 1.44, P = 3.1 × 10−11); variants that also associate with reduced serum sulfate. Genes implicated by this study are involved in cartilage and bone biology, as well as neurological and inflammatory processes. © 2022, The Author(s).

Abstract

BackgroundLimited data is available regarding long-term effectiveness of SARS-CoV-2 vaccines in patients with immune-mediated inflammatory diseases (IMIDs) on immunosuppressive therapy. Whether the persistence of vaccine-induced humoral immunity against SARS-CoV-2 differs between this patient population and the general public is currently unknown.ObjectivesTo compare the persistence of anti-Spike antibodies following two SARS-CoV-2 vaccine doses between IMID patients using immunosuppressive medication and healthy controls and identify predictors of antibody decline.MethodsWe included patients with inflammatory joint- and bowel diseases on immunosuppressive medication and healthy controls enrolled in the prospective observational Nor-vaC study. Serum samples were collected at two time points following two dose SARS-CoV-2 vaccination (first assessment within 6–48 days and second within 49–123 days). Sera were analysed for antibodies binding the receptor-binding domain (RBD) of the SARS-CoV-2 Spike protein. Anti-RBD ResultsA total of 1097 patients (400 rheumatoid arthritis, 189 psoriatic arthritis, 189 spondyloarthritis, 129 ulcerative colitis, 190 Crohn´s disease) (median age 54 years [IQR 43–64]; 56% women) and 133 controls (median age 45 years [IQR 35–56]; 83% women) provided blood samples within the defined intervals (median 19 days [IQR 15–24] and 97 days [86–105] after second vaccine dose). Abstract

Die Lebertransplantation (LTPL) ist fur Patienten mit terminaler Leberinsuffizienz oder einem hepatozellularen Karzinom (HCC) die einzige kurative Option. Die Organverteilung erfolgt uber Eurotransplant in Leiden, Niederlande. Fur die Allokation ist der „Model of End-stage Liver Disease (MELD) Score“ wesentlich, der die Schwere der Lebererkrankung abbildet. Das heterogene Patientenkollektiv sowie hepatisch bedingte Komorbiditaten und ihre Dynamik stellen Herausforderungen dar. Dem Anasthesisten kommt die wichtige Aufgabe einer Einschatzung der Gesamtprognose zu; hierbei sind kardiale, pulmonale, renale und neurologische Komorbiditaten zu berucksichtigen. Operativ unterteilt sich die LTPL in mehrere Phasen. Fur den Anasthesisten stellen neben dem Volumen‑, Warme- und Gerinnungsmanagement haufig die kardiozirkulatorische Stabilisierung sowie der Ausgleich des Saure-Basen-Haushaltes wesentliche Aufgaben dar.

Abstract

BackgroundPatients with immune-mediated inflammatory diseases (IMIDs) on immunosuppressive therapy have an inadequate serologic response following two-dose SARS-CoV-2 vaccination, and a standard vaccination strategy of three doses for this patient group is currently under implementation in several countries. However, the serological response and safety of this strategy has not been evaluated.ObjectivesTo assess serological response and safety of a three-dose vaccination strategy in IMID patients on immunosuppressive therapy as compared to standard two-dose vaccination of healthy controls.MethodsThe prospective observational Nor-vaC study (NCT04798625) enrolled adult patients on immunosuppressive therapy for inflammatory joint- and bowel diseases. Healthy controls were health care workers from participating hospitals. All participants received standard vaccines according to the national vaccination program with three doses in patients and two doses in controls. The third dose was offered to IMID patients >4 weeks after the second dose. Analyses of antibodies binding the receptor-binding domain of the SARS-CoV-2 Spike protein were performed prior to, and 2-4 weeks after the second and third vaccine doses. Levels were compared across groups by Mann-Whitney U tests and multivariate linear regression was used to identify predictors of response.ResultsOverall, 961 patients (315 rheumatoid arthritis, 156 spondyloarthritis, 171 psoriatic arthritis, 132 ulcerative colitis and182 Crohn’s disease) (median age 54 years [IQR 43-64]; 56 % women) and 227 controls (median age 44 years [IQR 32-55]; 83 % women) were included in the present analyses. TNFi monotherapy was used by 399 patients, 229 used TNFi in combination with other immunomodulators, 189 methotrexate monotherapy, 39 vedolizumab, 32 JAKi and 73 patients used other drugs. Patients on rituximab were not included. Patients were vaccinated with Pfizer BNT162b2 (54% patients, 14% controls), Moderna mRNA-1273 (16% patients, 40% controls) or a combination of vaccines (30% patients, 46% controls). Patients received the third vaccine dose a median of 120 (IQR 102-143) days after the second dose. After two doses, median anti-Spike antibody levels were significantly lower in patients (861 BAU/ml (IQR 418-4275) than controls (6318 BAU/ml (IQR 2468-9857)), pAdverse events were reported by 438 (48%) of patients after the third dose as compared to 471 (54%) after the second dose and 193 (78 %) of controls. Disease flares were reported by 42 (5%) and 69 (8%) patients after the second and third dose, respectively.ConclusionThis study suggests that a third vaccine dose for immunosuppressed patients closes the gap in serological response between patients and the healthy population. Antibody levels following the three-dose regimen in IMID patients were comparable to healthy controls vaccinated twice, and no new safety issues emerged. Abstract

The Heart Failure Association (HFA) of the European Society of Cardiology (ESC) has recently issued a position paper on the role of sodium–glucose co-transporter 2 (SGLT2) inhibitors in heart failure (HF). The present document provides an update of the position paper, based of new clinical trial evidence. Accordingly, the following recommendations are given:. • Canagliflozin, dapagliflozin empagliflozin, or ertugliflozin are recommended for the prevention of HF hospitalization in patients with type 2 diabetes mellitus and established cardiovascular disease or at high cardiovascular risk. • Dapagliflozin or empagliflozin are recommended to reduce the combined risk of HF hospitalization and cardiovascular death in symptomatic patients with HF and reduced ejection fraction already receiving guideline-directed medical therapy regardless of the presence of type 2 diabetes mellitus. © 2020 European Society of Cardiology

Abstract

Heart failure (HF) is common and associated with a poor prognosis, despite advances in treatment. Over the last decade cardiovascular outcome trials with sodium–glucose co-transporter 2 (SGLT2) inhibitors in patients with type 2 diabetes mellitus have demonstrated beneficial effects for three SGLT2 inhibitors (empagliflozin, canagliflozin and dapagliflozin) in reducing hospitalisations for HF. More recently, dapagliflozin reduced the risk of worsening HF or death from cardiovascular causes in patients with chronic HF with reduced left ventricular ejection fraction, with or without type 2 diabetes mellitus. A number of additional trials in HF patients with reduced and/or preserved left ventricular ejection fraction are ongoing and/or about to be reported. The present position paper summarises recent clinical trial evidence and discusses the role of SGLT2 inhibitors in the treatment of HF, pending the results of ongoing trials in different populations of patients with HF. © 2020 European Society of Cardiology

Abstract

Aims: In hospitalized patients with a clinical diagnosis of acute heart failure (HF) with preserved ejection fraction (HFpEF), the aims of this study were (i) to assess the proportion meeting the 2016 European Society of Cardiology (ESC) HFpEF criteria and (ii) to compare patients with restrictive/pseudonormal mitral inflow pattern (MIP) vs. patients with MIP other than restrictive/pseudonormal. Methods and results: We included hospitalized participants of the ESC-Heart Failure Association (HFA) EURObservational Research Programme (EORP) HF Long-Term Registry who had echocardiogram with ejection fraction (EF) ≥ 50% during index hospitalization. As no data on e', E/e' and left ventricular (LV) mass index were gathered in the registry, the 2016 ESC HFpEF definition was modified as follows: elevated B-type natriuretic peptide (BNP) (≥100 pg/mL for acute HF) and/or N-terminal pro-BNP (≥300 pg/mL) and at least one of the echocardiographic criteria: (i) presence of LV hypertrophy (yes/no), (ii) left atrial volume index (LAVI) of '34 mL/m2), or (iii) restrictive/pseudonormal MIP. Next, all patients were divided into four groups: (i) patients with restrictive/pseudonormal MIP on echocardiography [i.e. with presumably elevated left atrial (LA) pressure], (ii) patients with MIP other than restrictive/pseudonormal (i.e. with presumably normal LA pressure), (iii) atrial fibrillation (AF) group, and (iv) ‘grey area’ (no consistent description of MIP despite no report of AF). Of 6365 hospitalized patients, 1848 (29%) had EF ≥ 50%. Natriuretic peptides were assessed in 28%, LV hypertrophy in 92%, LAVI in 13%, and MIP in 67%. The 2016 ESC HFpEF criteria could be assessed in 27% of the 1848 patients and, if assessed, were met in 52%. Of the 1848 patients, 19% had restrictive/pseudonormal MIP, 43% had MIP other than restrictive/pseudonormal, 18% had AF and 20% were grey area. There were no differences in long-term all-cause or cardiovascular mortality, or all-cause hospitalizations or HF rehospitalizations between the four groups. Despite fewer non-cardiac comorbidities reported at baseline, patients with MIP other than restrictive/pseudonormal (i.e. with presumably normal LA pressure) had more non-cardiovascular (14.0 vs. 6.7 per 100 patient-years, P ' 0.001) and cardiovascular non-HF (13.2 vs. 8.0 per 100 patient-years, P = 0.016) hospitalizations in long-term follow-up than patients with restrictive/pseudonormal MIP. Conclusions: Acute HFpEF diagnosis could be assessed (based on the 2016 ESC criteria) in only a quarter of patients and confirmed in half of these. When assessed, only one in three patients had restrictive/pseudonormal MIP suggestive of elevated LA pressure. Patients with MIP other than restrictive/pseudonormal (suggestive of normal LA pressure) could have been misdiagnosed with acute HFpEF or had echocardiography performed after normalization of LA pressure. They were more often hospitalized for non-HF reasons during follow-up. Symptoms suggestive of acute HFpEF may in some patients represent non-HF comorbidities. © 2020 The Authors. 38. Dairy cattle welfare – the relative effect of legislation, industry standards and labelled niche production in five European countries

Abstract

The only common European Union (EU) legislation set up specifically to ensure the welfare of dairy cattle is for calves. As a consequence, there is wide diversity in how dairy cattle welfare is ensured in EU countries. A few countries have legal requirements for dairy cattle welfare, while in others, it is left to industry standards or niche production requirements, typically linked to various premium labels. In this paper, we compared animal welfare provisions in dairy cattle production across five countries with different combinations of legislative and other approaches: Denmark, Germany, the Netherlands, Sweden, and the United Kingdom. Firstly, we aimed to map the diversity of animal welfare initiatives. Secondly, we used the Benchmark method of expert valuations and weightings of the relative importance of individual welfare provisions. We found that Denmark and Sweden have the highest level of dairy cattle welfare provisions as measured by the Benchmark method, partly due to high legislative welfare requirements, followed by the United Kingdom, which has an extensive industry standard with very high uptake. Germany and the Netherlands, on the other hand, have lower levels of documented welfare provisions, and correspondingly a Benchmark score closer to a baseline defined by legal requirements at EU level. We also found differences in what elements of animal welfare were focussed on. Some initiatives emphasised fulfilling the social needs of cattle, while others focused more on space and freedom to move. Also, the countries with the highest Benchmark score had a relatively high level of production of organic and other specialty dairy products. We found the effect of national legislation or ambitious industry standards on dairy cattle welfare to be much larger than previous studies have found in either pigs or poultry. At a time when the EU is considering stepping up its efforts to improve animal welfare in terms of common minimum standards, the results of this study could have important policy implications. The diversity in the level of dairy cattle welfare standards found across countries may speak in favour of having shared minimum standards, both at EU level and globally. However, even among countries with a similar Benchmark score, we found a difference in the kinds of welfare provisions at work, which may make full harmonisation of standards more challenging.

Abstract

© 2017 Elsevier Ltd.The placenta and tumors share important characteristics, including a requirement to establish effective angiogenesis. In the case of the placenta, optimal angiogenesis is required to sustain the blood flow required to maintain a successful pregnancy, whereas in tumors establishing new blood supplies is considered a key step in supporting metastases. Therefore the development of novel angiogenesis inhibitors has been an area of active research in oncology. A subset of the molecular processes regulating angiogenesis are well understood in the context of both early placentation and tumorigenesis. In this review we focus on the well-established role of androgen regulation of angiogenesis in cancer and relate these mechanisms to placental angiogenesis. The physiological actions of androgens are mediated by the androgen receptor (AR), a ligand dependent transcription factor. Androgens and the AR are essential for normal male embryonic development, puberty and lifelong health. Defects in androgen signalling are associated with a diverse range of clinical disorders in men and women including disorders of sex development (DSD), polycystic ovary syndrome in women and many cancers. We summarize the diverse molecular mechanisms of androgen regulation of angiogenesis and infer the potential significance of these pathways to normal and pathogenic placental function. Finally, we offer potential research applications of androgen-targeting molecules developed to treat cancer as investigative tools to help further delineate the role of androgen signalling in placental function and maternal and offspring health in animal models.

Abstract

We address the problem of fusing array operations based on criteria such as shape compatibility, data reuse, and minimizing for data reuse, the fusion problem has been formulated as a static weighted graph partitioning problem (known as the Weighted Loop Fusion problem). We show that this scheme cannot accurately track data reuse between multiple independent loops, since it overestimates total data reuse of certain cases. Our formulation in terms of partitions allows use of realistic cost functions that can track resource usage accurately. We give correctness proofs, and prove that WSP can maximize data reuse in programs exactly, in contrast to prior work.For the exact optimal solution, which is NP-hard to find, we present a branch-and-bound algorithm together with a polynomial-time preconditioner that reduces the problem size significantly in practice. We further present a polynomial-time greedy approximation that is fast enough to use for JIT-compilation and gives near-optimal results in practice.All algorithms have been implemented in the automatic parallelization platform Bohrium, run on a set of benchmarks, and compared to existing methods from the literature.

Abstract

The concentration of creatinine and urea in plasma and clearances of inulin, endogenous creatinine, urea, and PAH have been measured in pigs on which were performed unilateral nephrectomy (Group A), renal autotransplantation with uretero-vesical anastomosis (Group B), and renal autotransplantation with uretero-ureteral anastomosis (Group C). In Groups B and C contralateral nephrectomy was carried out simultaneously. The concentration of creatinine and urea of the pigs in Group A was normal throughout the experimental period. In Groups B and C the concentration rose rapidly immediately after the operation to become normal in the course of 7 days. In Group B a secondary rise was observed between the 7th and 42nd day, in all probability due to ureteral stricture. On the 94th day the clearances of inulin and PAH, calculated per 10 kg body weight, were 17 and 57 ml/min for Group A, 15 and 50 ml/min for Group B, and 18 and 67 ml/min for Group C. The maximal tubular excretion (Tm) of PAH was 19, 19 and 25 mg/min/10 kg body weight for Groups A, B, and C, respectively. These values are 70–100% of the values in normal pigs, indicating that compensatory hypertrophy has occurred in all cases.

Abstract

In this paper, we explore the mapping of the NumCIL C# vector library where operations are offloaded to the Bohrium runtime system and evaluate the performance gains. By using a feature-rich language, such as C#, we argue that productivity can be increased. The use of the Bohrium runtime system allows all vector operations written in C# to be executed efficiently on multi-core systems.

Abstract

Trade in e-commerce is well known to have benefited individuals, firms and countries in the developed world. However, according to organizations including the WTO, OECD and UNCTAD, as well as the USA and EU, trade in e-commerce is also a means of improving the overall economic growth and performance of less developed nations. In fact, the WTO considers e-commerce to be the key force in integrating LDCs into the multilateral trading system. This paper sets out to problematize this view, not by arguing that e-commerce cannot make an important contribution to development, but by querying the assumptions that underpin current thinking. We start by repairing these defects: the absence of any role for the state; the underestimation of polarization in society and its effect on economic growth; and the failure to distinguish between types of e-commerce. Employing the participation and enhancement classification of e-commerce adoption, we consider whether developing states are ready for the participation phase of e-commerce, if developing countries have the kinds of industries that might act as demand-pull sectors for e-commerce growth, and whether social, political and institutional arrangements are in place to encourage and sustain e-commerce.

Abstract

The LOFT mission concept is one of four candidates selected by ESA for the M3 launch opportunity as Medium Size missions of the Cosmic Vision programme. The launch window is currently planned for between 2022 and 2024. LOFT is designed to exploit the diagnostics of rapid X-ray flux and spectral variability that directly probe the motion of matter down to distances very close to black holes and neutron stars, as well as the physical state of ultra-dense matter. These primary science goals will be addressed by a payload composed of a Large Area Detector (LAD) and a Wide Field Monitor (WFM). Abstract

We investigate the effect of a localized magnetic field on the propagation of sound in an infinite fluid described by the magnetohydrodynamic equations (MHD). An externally imposed magnetic field will scatter an acoustic wave, and the scattered wave is related to the spatial structure of the magnetic field. Measuring it is thus a non-intrusive probe for the magnetic field. Simple examples likely to be encountered in practice are worked out, and estimates are given that suggest the practical feasability of this diagnostic tool in current MHD experiments.
