Abstract

Abstract Background Long-term sequelae are frequent and often disabling after epidermal necrolysis (Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN)). However, consensus on the modalities of management of these sequelae is lacking. Objectives We conducted an international multicentric DELPHI exercise to establish a multidisciplinary expert consensus to standardize recommendations regarding management of SJS/TEN sequelae. Methods Participants were sent a survey via the online tool “Survey Monkey” consisting of 54 statements organized into 8 topics: general recommendations, professionals involved, skin, oral mucosa and teeth, eyes, genital area, mental health, and allergy workup. Participants evaluated the level of appropriateness of each statement on a scale of 1 (extremely inappropriate) to 9 (extremely appropriate). Results were analyzed according to the RAND/UCLA Appropriateness Method. Results Fifty-two healthcare professionals participated. 3. Laser therapy for genital lichen sclerosus: A systematic review of the current evidence base

Abstract

Abstract Background Lichen sclerosus (LS) is a chronic, inflammatory dermatosis. Initial treatment with superpotent topical corticosteroids is the accepted and evidence‐based first‐line therapy. For those who do not respond after exclusion of other potentiating factors, the best second‐line therapy is unclear. Laser therapy is an emerging treatment for genital LS and despite uncertain efficacy its use is gaining popularity in the private sector. Objectives We aimed to review the effectiveness of laser therapy for genital LS in men, women and children. Methods We conducted a systematic review of all primary studies reporting the use of laser in genital LS. Ovid MEDLINE, PubMed, Ovid Embase, Cochrane CENTRAL, Web of Science, CINAHL and PsycINFO were searched from inception to February 2021. The quality of the studies was assessed using the revised Cochrane risk‐of‐bias tool for randomized trials, ROBINS‐I tool for non‐randomized trials and Joanna Briggs Institute checklist for case studies. Results A total of 24 studies, involving 616 adults, met inclusion criteria. These were six randomized controlled trials (RCTs), one non‐randomized trial, nine single arm trials and eight case series. Where assessed, most studies suggest that laser therapy in patients with LS may improve symptoms, clinical signs, quality of life and sexual function. However, results were highly heterogeneous and methodological quality was very low, therefore meta‐analysis was not possible. Conclusions There is poor evidence to support the use of laser therapy for genital LS at present. Effectiveness of laser needs to be robustly investigated in well‐conducted RCTs.

Abstract

An eight-cavity, 1.3 GHz, SLAC linac coherent light source II High Energy cryomodule was assembled and tested at Fermilab to verify performance before the start of production. Its cavities were processed with a novel nitrogen doping treatment to improve gradient performance. The cryomodule was tested with a modified protocol to process sporadic quenches, which were observed in Linac Coherent Light Source II production cryomodules and are attributed to multipacting. Dedicated vertical test experiments support the attribution to multipacting. The verification cryomodule achieved an acceleration voltage of 200 MV in continuous wave mode, corresponding to an average accelerating gradient of 24.1 MV/m, significantly exceeding the specification of 173 MV. The average Q_{0} (3.0×10^{10}) also exceeded its specification (2.7×10^{10}). After processing, no field emission was observed up to the maximum gradient of each cavity. This paper reviews the cryomodule performance and discusses operational issues and mitigations implemented during the several month program.

Abstract

The aim of this case report is to describe the use of physio-therapy interventions in a patient diagnosed with cervicogenic headache(CGH) and temporomandibular dysfunction (TMD). This case study involveda 26 year old male. The subject’s cervical and temporomandibular joint(TMJ) active range of movement; passive accessory movements; headacheintensity, periodicity and response to analgesics; TMJ clicking and discomfort;Numerical Pain Rating Scale (NPRS) scores, were assessed on initial assessment and on discharge 14 weeks later. The physiotherapy interventionincluded cervical and TMJ mobilisation (Maitland approach); trigger pointtherapy and massage; strengthening of the deep cervical neck flexors and scapulothoracic muscles; and posture correction. The subject improved on all outcome measures and was discharged after nine physiotherapy sessions, withfull cervical and TMJ range of movement, improved muscle strength and decreased headache intensity and frequency.This case describes a positive outcome for a patient diagnosed with cervical headaches and temporomandibular dysfunction in which physiotherapy interventions aimed at correcting joint dysfunction, myofascial pain and decreased muscle strength were used effectively.

6. The association between trunk muscle endurance and lumbo-pelvic stability in adolescent low back pain: A cross sectional study

Abstract

Decreased trunk muscle endurance has been identified as a risk factor for adolescent LBP, and poor lumbo-pelvic stability has been found to be associated with LBP in the adult population. The aim of the study was to investigate the association between adolescent LBP, trunk muscle endurance and poor lumbo-pelvic stability. Design: A cross sectional study. Participants: 80 adolescents in grade 8 to grade 11, aged 12 to 17 years, at three high schools in Gauteng, who agreed to participate in the study. Method: Data was collected by means of a validated questionnaire and physical tests. The active straight leg raise test was used to record the lumbo-pelvic stabilising muscles. The Sorensen, Shirado and side-bridge tests were used to record trunk extensor, flexor and side flexor muscle endurance, respectively. Results: The results revealed a lifetime prevalence of LBP of 82.50%, one year prevalence of 78.80% and point prevalence of 23.80%. Adolescents with LBP demonstrated decreased trunk extensor muscle endurance but increased trunk flexor muscle endurance (p=0.044), compared to non-LBP adolescents. Poor lumbo-pelvic stability was not associated with adolescent LBP, but was associated with decreased extensor trunk muscle endurance (p=0.031). Conclusion: There was an association between trunk flexor muscle endurance and adolescent LBP, and between decreased trunk extensor muscle endurance and poor lumbo-pelvic stability. No association was found between LBP and poor lumbo-pelvic stability.

9. Lunar Terrain Vehicle (LTV) Remote Teleoperation Studies Under Four Lunar Communication Latencies

Abstract

Remotely operating a lunar rover from Earth while subject to an Earth-Moon time delay of multiple seconds could result in a dangerous state where the roving vehicle is either damaged or lost, thereby potentially compromising an entire mission or series of missions. Providing the right capabilities to the remote operator to manage inherent communication latencies will be important for remote driving to be successful. NASA conducted two studies to investigate the average speed and number of kilometers per day that an operator on Earth could teleoperate a notional Artemis unpressurized rover with minimal remote operator capabilities under 0- and 4-second communication delays (April 2023 study) and 6- and 8-second delays (August 2023 study). A primary goal of these studies was to understand if an Artemis Lunar Terrain Vehicle (LTV) could cover 6 kilometers (km) in 24 hours when operated remotely. During the April 2023 evaluation, eight test operators used an in-house simulation of the lunar surface South Pole to teleoperate a NASA government reference LTV. Each operator received approximately 30 minutes of remote driving familiarization/training prior to their test run. Operators viewed the surrounding terrain via a single, rover mast-mounted, high-resolution camera with pan/tilt/zoom capabilities; continuous communication was provided throughout all testing. In the August 2023 evaluation, remote operators received approximately 3 hours of familiarization training in each latency, and the simulation environment provided remote operators with an operator-selected rate limiter to enable finer sensitivity in the hand controller and a predictive circle function to better assist operators with predicting the path the vehicle could take. All test operators were able to successfully navigate and drive through six different types of terrain and five planned traverse scenarios using natural lighting under all communication delays. Results for average speeds for each communication delay, computed by averaging the data from all test conditions for that latency and all operators, are shown in the table below. The average speed data was then used to derive the total time needed to cover 6 km, 8 km, and 20 km (distances relevant to LTV-SYS071 and -029 requirements). Remote operators drove slower and used the brake more frequently when subject to a communication latency as opposed to no communication latency. Subjective workload assessments revealed that while operating in a latency the overall workload significantly increased when compared to a 0-s delay with mental demand, frustration, and performance being the primary contributing factors. Driving strategies in the 0-s delay did not vary significantly among subjects; however, in the 4-s delay condition, three different driving strategies were identified. In the 6-s and 8-s latency conditions the operator’s use of the cruise control to maintain speed was more apparent. Additionally, over the course of the August study, the operator took advantage of the predictive circle indicator on the navigation display and over 95% of the operator’s navigation used the mast camera 180-degree panning function for ground truthing in terms of boulders and craters. Operators started to define more specific parameters in driving strategies for general operations. This consisted of setting the vehicle into a low-speed cruise mode of approximately 11.5 kph and noticing driving performance of the vehicle seemed to be much harder at slower speeds 0.4–0.8 kph; however, the vehicle was more responsive at speeds of 2.9–3.6 kph. Regardless of communication delay, operators used both the horizontal translation rails and the vehicle fenders as guides to predict a path for the vehicle through heavily concentrated terrain features. Test operators acknowledged that the teleoperations training for this study was substantially less than what an actual LTV remote operator will ultimately receive. They estimated a minimum of 20 to 100 hours spread across multiple days and weeks (e.g., strategies included immersion training over a 3-day period, to a short 8-week starter program) would be needed to get an operator ~ 60% proficient (i.e., able to complete a subset of remote driving tasks), to a yearlong program for full proficiency in remote driving tasks under all terrain types and natural lighting conditions. Remotely operating a vehicle on another planetary body while subject to communication latency is a complex task. Speed, distance covered, time spent driving, time spent navigating, brake usage and rock contacts are all affected by operator workload, driving strategies, workstation ergonomics and training. These studies provided a “first-look” answer to a potential system requirement (namely if a remote operator could cover a given distance in a given amount of time); however, considerable general knowledge was gained to begin to understand what it will take to make a successful lunar rover teleoperator.

37. Effect of adolescent pregnancy on maternal and foetal health

Abstract

Background: Adolescence is a time when structural, functional, and psychosocial developments occur. Pregnancy during teenage can adversely affect the health of both the mother and the foetus as the adolescent female concerned is yet to attain her full growth potential. Adolescent pregnancy is a global phenomenon with serious health, social and economic consequences. Methods: It was a single centric, prospective, observational study. 211 Patients who attended the inpatient or outpatient department of obstetrics and gynaecology in an urban tertiary care hospital and followed up till outcome. Results: As per the study conducted, almost 83% of teenage mothers conceive by 19 years of age. Owing to the increasing awareness regarding maternal and foetal wellbeing, majority of the teenage mothers were booked. Teenage mothers and their babies are prone to intrapartum and postpartum complications as well as stillbirths. The most common comorbidity associated in teenage mothers was pregnancy induced hypertension (PIH) spectrum disorders followed by anaemia. The rate of neonatal intensive care unit (NICU) admission for babies of teenage mothers was 10%. Conclusions: Adolescent mothers and their babies are at a risk of complications than other mothers in the twenties. The need of the hour are comprehensive measures and convergence among various departments to address all the needs of adolescents. Robust measures and policies to end teenage marriages and consequent pregnancies are exactly what the developing nations need at present.

38. A prospective observational study of post-partum intrauterine contraceptive device acceptance in a tertiary care centre

Abstract

Background: Provision of PPIUCD is being rapidly scaled up in India with facilities in at least 19 states offering the method in 2013. According to National Family Health Survey (NFHS)-3, the prevalence of modern method of contraceptive use is 48.5% and all methods 56% in India. PPIUCD placement remains a viable option for patients who wish to use a long-acting reversible contraceptive (LARC) method and to have it placed at the time of their delivery. Hence, we planned this study with an aim to evaluate acceptability PPIUCD at tertiary care centre. Methods: It was an observational and prospective study of acceptance of PPIUCD as a method of contraception in patients who delivered within the study period in our institute Grant Medical Hospital and College, Mumbai. Data analysis was done with statistical software SPSS V 25.0. Results: A total of 2014 patients were enrolled in the study. The mean age was noted to be 25.87 years. Frequency of ANC visits among the patients was 4 to 6 (39.87%). Commonest obstetric history finding was previous live birth history in 59.38% cases. Pregnancy outcome was vaginal delivery noted in 63.01% of the females. 879 (43.65%) cases accepted PPIUCD. For those who did not accept PPIUCD, commonest cause was tubal ligation in 34.19%, followed by fear of pain in 18.94%, partner’s refusal in 10.31%. Conclusions: The acceptance rate in study for PPIUCD was 43.65% which was higher than most of the published evidence. The common reasons for not accepting PPIUCD were tubal ligation.
