1. Safety and Efficacy of Recombinant Factor VIIa (NovoSeven) Use during ECMO Support in Patients after Cardiac Surgery

Abstract

Background: Acute postoperative bleeding in cardiac surgical patients is a major cause of morbidity and mortality. Substitution of coagulatory factors may not always provide optimal hemostasis and off label, use of recombinant FVIIa has been proposed. When on ECMO, extra care must be taken during coagulatory substitution as clotting of the system may cause cardiovascular complications and possible ECMO failure, leading to death. In this paper, we examined the safety and efficacy of rFVIIa during ECMO support in postoperative cardio-surgical patients. Methods: We retrospectively examined all patients receiving rFVIIa postoperatively from December 2005 and January 2020. Clinical characteristics, demographics, bleeding, thrombotic complications, mortality, and rFVIIa administration were analyzed. Results: A total of 74 patients received rFVIIa postoperatively due to uncontrollable bleeding after cardiac surgery on our ICU. Of these patients, 23 patients were on ECMO treatment. Twelve patients received rFVIIa during, but not prior to the initiation of ECMO therapy. Six patients (50%) were male; mean age was 46 years (30-72 years). Eleven patients (91.7%) were on venoarterial ECMO, one patient was on central ECMO (8.3%). Dose of administered rFVIIa was corrected for body weight; mean dosage was 82μg/kg. We saw a significant reduction in need for red packed cells, fresh frozen plasma and thrombocyte transfusion after rFVIIa administration. There was no impact on the functionality of the ECMO system, especially regarding the oxygenator after rFVIIa administration. One patient suffered a stroke due thromboembolism (8.3%). One patient developed late thromboembolism in the leg (8.3%), and two cases of pulmonary embolism (16.7%) were recorded. Overall survival was 25% and there was no significant difference in survival between ECMO and non-ECMO patients. Weaning from ECMO could be achieved successfully in 41.7% of our patients. Conclusion: Recombinant Factor VIIa is an effective agent in reducing blood loss during ongoing ECMO therapy in patients with refractory bleeding. Although no direct relation between rFVIIa application and thromboembolic events could be established, its use should be done with the utmost care and in selected patients. However, rFVIIa therapy did not impact ECMO function in our cohort.

5. Sizing Up Shylock's Name Again

Abstract

Abstract Whence the name of the Hebrew moneylender Shylock? Although Gross has a chapter title relating to this, the name sounds basically more British than anything else. Indeed, the dramatist's father was himself a lender of money and often considered a Catholic recusant, so Shylock's name connects with that of another such recusant, Shacklock, a name with curious associations with Sherlock. Suggestions of the German scheu and Scottish loch with Shy-lock are also possible. The name may also have connections with John Florio.

Abstract

Abstract Is Prufrock's name a mere will-o'-the-wisp? Several lengthy studies of its meaning, etymologically and onomastically (or connotatively), have raised a number of issues concerning what Eliot had in mind, consciously or unconsciously. These have, to some extent, overlapped, and so a study bringing them together is in order. Each segment of the full name deserves careful study again, including the ambiguous initial, the resonant middle name, and the Germanic surname.

7. Caliban's Name and the 'Brave New World'

Abstract

Abstract One of Shakespeare's most puzzling made-up names, that of Caliban in The Tempest, is traceable to the influence of the log of Columbus' first voyage to America, or from a transcription thereof by Las Casas, deriving from the linguistic confusion of Caribbean people (“Caribs”) with Canibs, hence with cannibals. The allusion, however, is probably not to actual man-eaters, the question of whether the natives were anthropophagi (physically or mythically) notwithstanding. Although Montaigne's influential essay did indeed cite the latter, Shakespeare could have improvised here a bit upon Florio's translation.

Abstract Several names in Dickens' novel The Mystery of Edwin Drood have been the source of disagreement among scholars. Among these are Drood itself, which may relate to rood ‘cross’; Neville Landless, both parts suggesting disenfranchisement; and the pet names for Rosa Bud, Rosebud and Pussy, both of which probably should not carry the sexual connotations some have ascribed to them.

Abstract

Abstract Although etymologically Cordelia's name relates to cor de ilia ‘ with, or from, the heart’ and the name in King Lear suggests an anagram of ideal heart, such sentiment is at variance with her failure to communicate with the king and her self-willed behavior. Thus, it may be helpful to see in her name another anagram, cord and a lie, suggesting that her response to Lear, despite her good intentions, is a breaking of her bond to him.

11. The Weakness of T. S. Eliot's J. Alfred: More of Verdenal in 'Prufrock'

Abstract

Abstract Recently published letters from Jean Verdenal to T. S. Eliot corroborate my earlier suggestion that Prufrock may be modelled on Verdenal. Prufrock's weakness in normal sexual identification reflects the poet's close friendship with Verdenal, who has also been suggested as the “Floret” of another poem. Onomastically, the connection may go beyond the secretive J. of the first name to the name Alfred itself, which is almost an anagram of Verdenal.

C | Abstract: In 1997 I was asked to organize humanities outreach activities at the University of California, Irvine. The result was the formation of Humanities Out There (HOT). In our workshops, faculty members and graduate students supervise teams of undergraduates in order to take the methods and materials of the university into the larger community.I believe that programs like these will become increasingly important in the next century, as economic, cultural, and educational divisions deepen in the wake of the demise of affirmative action and as the humanities fight to define their missions in a world driven by technology and its discontents. In this brave new world, what I call the new outreach may have a role to play in responding to social crises as they are visited on the life of the university. The new outreach will be driven by intellectual content, not public relations.It will take its orientation from the faculty rather than administrators. It will engage all the research disciplines rather than remain the purview of education departments. It will be integrated into the professional lives of its participants rather than rely on the spirit of volunteerism alone.

Abstract

Nano-structured surfaces were generated by laser interference lithography and femtosecond-laser direct writing of photo resists that subsequently were metallized by electroless plating or sputter deposition of silver. Laser lithography was performed with a 405 nm coherent diode laser in AZ9260, using two-beam interference with double illumination by 90° rotating of the substrate, leading to 2D periodic surface patterns with smallest features of the order of 200 nm. With fs-laser direct writing using a Ti-sapphire oscillator of 800 nm and 15 fs pulse length, feature sizes down to 100 nm were realized in SU8, even with aspect ratios much larger than 1. Metallization with electroless plating delivered either grainy silver coatings with a grain size around 100 nm or needle-like silver coatings with a needle length around 100 nm and a width of around 10 nm. The metallized substrates were exposed to aqueous solutions of Rhodamine 6G (Rh6G) of different concentrations and the corresponding Raman signals were recorded with a Raman micro-probe spectrometer. The nano-structured surfaces lead to formation of Raman bands attributable to Rh6G. In case of the grainy silver coatings, surfaces without nano-structures did not show Raman activity, indicating that grating-coupled surface plasmons play the dominant role for Raman enhancement. In case of substrates coated with the needle-shaped silver crystallites, Raman activity was also seen in regions without laser-generated nano-structures, indicating that localized particle plasmons play the dominant role for Raman enhancement. A comparison with Raman spectra measured with conventional Raman spectrometer showed that the enhancement factor achieved by the laser-generated nano-structures themself, is of the order of 6×10 4 . Raman intensity as a function of Rh6G concentration revealed a regular behaviour, as expected from a Langmuir isotherm.
