1. Enhancing Civil Engineering Education through Affordable AR Tools for Visualizing BIM Models

Abstract

Traditionally, civil engineering infrastructures have been represented through two-dimensional (2D) drawings, which can lead to interpretation errors and jeopardize engineers' understanding of complex projects. However, with the development of building information modeling (BIM) methodology, civil engineering projects can now be virtually modeled in three-dimensional (3D), offering a more accurate representation. Augmented reality (AR) tools can be linked to BIM models for educational purposes, allowing for a more immersive and interactive learning experience. While previous studies have focused on improving the visualization of mechanical and civil engineering drawings, this study presents the development of a low-cost AR tool for visualizing BIM models and its application in civil engineering courses. To achieve this, the main state-of-the-art AR software was compared for their characteristics and functionalities, and the software, Unity, was selected to develop an AR visualization tool. The application of this tool was then presented, along with an analysis of its benefits when introduced into two civil engineering courses at the University of Castilla La Mancha (UCLM) in Spain: Descriptive geometry, and Design work of a structure. Overall, the results of the study suggest that the integration of AR tools into civil engineering education can enhance students' learning experience and improve their understanding of complex projects.

3. A new path towards ultra-high efficient laser power converters: Silicon carbide-based multijunction devices

Abstract

Current high power laser transmission technology faces two major limitations to improve the efficiency of the photovoltaic receivers: the intrinsic entropic losses associated to low bandgap materials (such as GaAs) and the series resistance losses that degrade the device performance at high power densities. The use of high bandgap materials and new architectures for laser power converters (LPC) have been pointed out as alternatives to overcome these limitations. In this work, three silicon carbide polytypes (3C, 4H and 6H) are proposed as base materials for the standard horizontal laser power converter (hLPC) architecture and the Vertical Epitaxial Hetero-Structure Architecture (VEHSA). 3C SiC based hLPCs outperform the power converters based on the other two polytypes, achieving a maximum efficiency of 84.6% at Image 1, but suffer from series resistance losses, that deteriorate their efficiency, at higher laser power densities. This issue is solved with 3C SiC 4 cells VEHSAs that demonstrated increasing efficiency with the input power, reaching a maximum of 87.4% at Image 2. The VEHSA reduced number of cells minimize the risks of efficiency losses due to current mismatch between cells. These results support the feasibility of a new generation of LPCs capable of efficiently convert ultra-high laser power densities.

Abstract

Abstract Energy is key in achieving sustainable societies. There have been great efforts towards improving energy access worldwide. Despite the advances in energy access, energy poverty remains a major problem in many parts of the world, particularly in rural communities. Modern energy, in particular electricity, can help rural communities develop through improving education and health. During the last two decades, there have been improvements on bioenergy technological innovations, e.g. electricity generation from bioenergy from residual biomass from several agricultural crops in biorefineries. Most research has focussed on large biorefineries, with limited research on small-scale gasifiers’ location and contribution to energy poverty. This paper is aimed at assessing technological options to generate electricity in situ from biomass to reduce energy poverty of rural communities. This is done using four analysis methods: (1) crops availability data; (2) poverty and marginalisation data; (3) electricity provision/distribution; and, (4) GIS/Geographical latitude and longitude to locate municipalities in Mexico. The results shows that the generating potential for electricity using residual biomass with gasifiers could improve the welfare of almost 10 million people communities using residual biomass from crops harvested in such communities. This research provides location solutions on the best places to locate small-scale biorefineries. The research also provides systemic analysis to reduce energy poverty through in situ electricity generation using cheap accessible small plant technologies and biomass as raw material, as well as their location. Generating electricity in a decentralised way through agricultural residual biomass can help lift rural communities from poverty and improve their well-being, and, thus, make societies more sustainable.

Abstract

Indoor agricultural offers efficient alternatives for intensive food production through automation technologies and controlled environments. Light plays a crucial role in plant development; however, photons captured by the crop are often wasted in empty spaces, resulting in low light efficiency and high energy costs. This research aims to simulate eight structural designs for an indoor lettuce crop, exploring different planting systems and light and culture bed combinations (static and mobile) to identify the most effective mechanism for light efficiency during crop growth. The simulations were carried out with spreadsheets based on applying formulas of yield in dry biomass per photosynthetic photons, lighting costs, harvest, and production. The results indicate that Circular Moving Light and Mobile Culture Bed with Quincunx Planting (CML-QM) and Circular Moving Light and Mobile Culture Bed with Linear Planting (CML-LPM) exhibit higher photon capture percentages (85% and 80%, respectively) and lower electricity consumption compared to static designs. The simulation results demonstrate the potential for significant improvements in photon capture and cost savings through optimized system designs. This investigation provides valuable insights for designing more efficient systems and reducing electricity consumption to enhance the capture of photosynthetic photons in indoor lettuce cultivation.

10. Current status of genetic urinary biomarkers for surveillance of non-muscle invasive bladder cancer: a systematic review

Abstract

Abstract Background Genetic biomarkers are a promising and growing field in the management of bladder cancer in all stages. The aim of this paper is to understand the role of genetic urinary biomarkers in the follow up of patients with non muscle invasive bladder cancer where there is increasing evidence that they can play a role in avoiding invasive techniques. Methods Following PRISMA criteria, we have performed a systematic review. The search yielded 164 unique articles, of which 21 articles were included involving a total of 7261 patients. Sixteen of the articles were DNA based biomarkers, analyzing different methylations, microsatellite aberrations and gene mutations. Five articles studied the role of RNA based biomarkers, based on measuring levels of different combinations of mRNA. QUADAS2 critical evaluation of each paper has been reported. Results There are not randomized control trials comparing any biomarker with the gold standard follow-up, and the level of evidence is 2B in almost all the studies. Negative predictive value varies between 55 and 98.5%, being superior in RNA based biomarkers. Conclusions Although cystoscopy and cytology are the gold standard for non muscle invasive bladder cancer surveillance, genetic urinary biomarkers are a promising tool to avoid invasive explorations to the patients with a safe profile of similar sensitivity and negative predictive value. The accuracy that genetic biomarkers can offer should be taken into account to modify the paradigm of surveillance in non muscle invasive bladder cancer patients, especially in high-risk ones where many invasive explorations are recommended and biomarkers experiment better results.

Abstract

Due to the need to develop more agroforestry systems, the Moringa oleifera, Olea Europea, Glycine max, Brassica napus, Helianthus annuus, and Jatropha curcas are identified as unconventional species for their expansion under these systems in Colombia. With the Colombian Environmental Information System (SIAC) database, zoning was carried out according to the agroclimatic species requirements and optimal coverage for their production. As a result, a total area of 212,977.2 km2 was identified, mainly including the departments of Casanare, Arauca, Vichada, Guajira, Córdoba, Meta, Magdalena, Cesar, Tolima, and Cundinamarca. The species and associations species with the most options for productive expansion are Moringa (75,758 km2), Moringa, Jatropha, and Sunflower (42,515.1 km2), Moringa and Jatropha (37,180.4 km2), Jatropha (20,840 km2), Jatropha and Sunflower (17,692.1 km2), Olive (7332.1 km2), and Soybean (3586.3 km2). Of the potential agroforestry areas to their establishment, 36% correspond to herbaceous and/or shrubby vegetation, 34% to grasses, and 22% to heterogeneous agricultural areas. This research is the first step to representing the agronomic versatility of these promising species and their potential contribution to the diversification of the agri-food and agroforestry sectors.

Abstract

Abstract Background Natural killer (NK) cells are important anti-tumor cells of our innate immune system. Their anti-cancer activity is mediated through interaction of a wide array of activating and inhibitory receptors with their ligands on tumor cells. After activation, NK cells also secrete a variety of pro-inflammatory molecules that contribute to the final immune response by modulating other innate and adaptive immune cells. In this regard, external proteins from NK cell secretome and the mechanisms by which they mediate these responses are poorly defined. Methods TRANS-stable-isotope labeling of amino acids in cell culture (TRANS-SILAC) combined with proteomic was undertaken to identify early materials transferred between cord blood-derived NK cells (CB-NK) and multiple myeloma (MM) cells. Further in vitro and in vivo studies with knock-down of histones and CD138, overexpression of histones and addition of exogenous histones were undertaken to confirm TRANS-SILAC results and to determine functional roles of this material transferred. Results We describe a novel mechanism by which histones are actively released by NK cells early after contact with MM cells. We show that extracellular histones bind to the heparan sulfate proteoglycan CD138 on the surface of MM cells to promote the creation of immune-tumor cell clusters bringing immune and MM cells into close proximity, and thus facilitating not only NK but also T lymphocyte anti-MM activity. Conclusion This study demonstrates a novel immunoregulatory role of NK cells against MM cells mediated by histones, and an additional role of NK cells modulating T lymphocytes activity that will open up new avenues to design future immunotherapy clinical strategies.

Abstract

Acinetobacter baumannii is an emergent bacterial pathogen that provokes many types of infections in hospitals around the world. The genome of this organism consists of a chromosome and plasmids. These plasmids vary over a wide size range and many of them have been linked to the acquisition of antibiotic-resistance genes. Our bioinformatic analyses indicate that A. baumannii plasmids belong to a small number of plasmid lineages. The general structure of these lineages seems to be very stable and consists not only of genes involved in plasmid maintenance functions but of gene sets encoding poorly characterized proteins, not obviously linked to survival in the hospital setting, and opening the possibility that they improve the parasitic properties of plasmids. An analysis of genes involved in replication, suggests that members of the same plasmid lineage are part of the same plasmid incompatibility group. The same analysis showed the necessity of classifying the Rep proteins in ten new groups, under the scheme proposed by Bertini et al. (2010). Also, we show that some plasmid lineages have the potential capacity to replicate in many bacterial genera including those embracing human pathogen species, while others seem to replicate only within the limits of the Acinetobacter genus. Moreover, some plasmid lineages are widely distributed along the A. baumannii phylogenetic tree. Despite this, a number of them lack genes involved in conjugation or mobilization functions. Interestingly, only 34.6% of the plasmids analyzed here possess antibiotic resistance genes and most of them belong to fourteen plasmid lineages of the twenty one described here. Gene flux between plasmid lineages appears primarily limited to transposable elements, which sometimes carry antibiotic resistance genes. In most plasmid lineages transposable elements and antibiotic resistance genes are secondary acquisitions. Finally, broad host-range plasmids appear to have played a crucial role.

Abstract

We aimed to propose a reliable and accurate model using non-destructive measurements of leaf length (L) and/or width (W) for estimating leaf area (LA) of Surinam cherry (Eugenia uniflora L.). For model construction, 560 leaves were randomly sampled from different levels of the tree canopies and encompassed the full spectrum of measurable leaf sizes. Power models better fit E. uniflora leaf area than linear models; but, among of then, the best fit were made when product of the L and W (LW) were used. To validate these models, independent data set of 156 leaves were used. Thus, we developed a single power model (Yi = β0 xβ1) [LA = 0.685 (LW)0.989; standard errors: β0 = 0.014, β1 = 0.005; R2 a = 0.997] with high precision and accuracy, random dispersal pattern of residuals and unbiased. A simpler linear model [LA = 0.094 + (LW * 0.655); standard errors: β0 = 0.025, β 1 = 0.001; R2 a = 0.998] also described here to estimate leaf area of E. uniflora, which are as good as the first. The simplicity of the latter model may be relevant in field studies, as it does not demand high precision or expensive instruments.

Abstract

The use of statistical models and machine-learning techniques along satellite-derived aerosol optical depth (AOD) is a promising method to estimate ground-level particulate matter with an aerodynamic diameter of ≤2.5 μm (PM2.5), mainly in urban areas with low air quality monitor density. Nevertheless, the relationship between AOD and ground-level PM2.5 varies spatiotemporally and differences related to spatial domains, temporal schemes, and seasonal variations must be assessed. Here, an ensemble multiple linear regression (EMLR) model and an ensemble neural network (ENN) model were developed to estimate PM2.5 levels in the Monterrey Metropolitan Area (MMA), the second largest urban center in Mexico. Four AOD-SDSs (Scientific Datasets) from MODIS Collection 6 were tested using three spatial domains and two temporal schemes. The best model performance was obtained using AOD at 0.55 µm from MODIS-Aqua at a spatial resolution of 3 km, along meteorological parameters and daily scheme. EMLR yielded a correlation coefficient (R) of ~0.57 and a root mean square error (RMSE) of ~7.00 μg m−3. ENN performed better than EMLR, with an R of ~0.78 and RMSE of ~5.43 μg m−3. Satellite-derived AOD in combination with meteorology data allowed for the estimation of PM2.5 distributions in an urban area with low air quality monitor density.

21. Germination behavior of Jatropha curcas L. after different imbibition times

Abstract

Jatropha curcas is an important specie for production of biofuel. The specie can survive and produce fruits and seeds even in drought condition. For an adequate establishment in the field is necessary that seeds have a good quality in vigor and viability. In this study, we evaluated the seed water relation with different imbibition times, in deionized water, from 0 to 24 hours. Imbibed seeds were sown in polyethylene trays with 1,200 g of river sand. The germination was recorded every day for 25 days. Seeds with at least 10 mm radicle on the soil surface was considered as germinated. To determinate seed water content (SWC), 10 seeds were weighed in fresh (SFW), turgid (STW) and dry weight (SDW) at 105°C for 24 hours. After 24 hours of imbibition our results show a decrease in the germination rate from 85% to 47%, and an increase of the mean germination time from 4.8 to 7.1 days. The initial moisture of the seed used in this experiment was about 8% and after 24 hours of imbibition, the SWC was around 60%. The initial low moisture in the seeds produce imbibition damage because the tissue hydration takes place in a not controlled way so that the reconstruction of internal structures of the cells and organelles were affected. According to the PCA analysis the seed germination had a negative correlation with the imbibition time (r = -0.72, p < 0.05) and with the electrical conductive (r = -0.88, p < 0.05), variables related to the seed vigor. This study suggests that electrical conductivity may be useful in J. curcas for vigor test and their seeds do not need previously water imbibition to improve germination from seeds with initial moisture less than 8%.

Abstract

Objective: To describe the distribution of the central corneal thickness (CCT) measurements on a healthy Hispanic sample population and its correlation with age, mean simulated keratometry (SimK), and mean refractive spherical equivalent (MRSE). Methods: Retrospective analysis on the records of healthy patients from the Ophthalmology and Visual Sciences Institute, Tecnologico de Monterrey, January 2015 to August 2015. CCT data, age, gender, corneal curvature, and spherical equivalent was obtained. A descriptive analysis and correlation by the Spearman method was performed. The sample was divided by age subgroups: than 40 years old and correlation analysis with CCT values was determined. Results: A total of 93 (186 eyes) patients were included. Mean age: 32.54 ± 12.04 years. 43% were women. Mean CCT: 545.69 ± 36.88 μm, mean SimK: 43.56 ± 1.90 D and MRSE: −2.54 ± 3.15 D. No correlation was registered between CCT and the variables when analyzed with the Anderson–Darling (p = 0.006), Shapiro–Wilk (p = 0.043), and Kolmogorov–Smirnov (p = 0.01). CCT showed a bimodal distribution with higher density at 540 μm. Age groups 40 years showed significant difference in CCT (p = 0.016), a positive correlation with CCT was observed in the group

Abstract

In this work, we study injectivity issues caused by the use of the Peaceman equation in the numerical simulation of chemical enhanced oil recovery (EOR) processes aimed at reducing fluid mobility, such as foam injection, on coarse grids. Employing analytical solutions, we demonstrate that the Peaceman equation, commonly applied to mathematical modeling of the injectivity in commercial simulators, leads to errors of more than two orders of magnitude in the injection well pressure drop when the foam flow effects near the well are considered for assuming homogeneous mobility in a coarse-well block. To circumvent this issue, we investigate numerical treatments focused on the grid of well blocks through local grid partitioning strategies (radial and Cartesian) to improve the injection well bottom-hole pressure (BHP) estimation. This methodology does not change the input data nor the injectivity model characteristics of the commercial simulator. It does not significantly affect the computational cost of the simulation, since the grid treatment occurs only in the blocks containing the wells. Thus, the radial and Cartesian grid partitioning for the well block are compared using the STARS simulator. Our results show the clear capability of the methodology to reduce the well BHP overestimation, mitigating the errors caused by the Peaceman equation. Indeed, in some simulated scenarios, the BHP overestimation was reduced a hundredfold after applying the partitioning technique. Furthermore, we discuss the choice of simulation parameters leading to more accurate and numerically stable results.

Abstract

Background: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92]; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events. La Organización Mundial de la Salud (OMS) refiere que las etapas básicas de evaluación de la carga de morbilidad ambiental por metilmercurio son las siguientes: determinar la distribución de las concentraciones de mercurio en el cabello de las mujeres en edad fértil que consumen pescado contaminado con metilmercurio; calcular los descensos del coeficiente intelectual en los lactantes basándose en la distribución de las concentraciones de mercurio en el cabello de mujeres en edad fértil, y estimar la tasa de incidencia de retraso mental leve inducido por el metilmercurio y la consiguiente carga de morbilidad expresada en años de vida ajustados por discapacidad (AVAD). Esto debe realizarse para cada población. El objetivo del estudio fue desarrollar una etapa de evaluación gestacional básica del metilmercurio a través del cabello del recién nacido que permita identificar a infantes en zonas de riesgo ambiental, y posterior evaluación neuroconductual. Material y métodos: En una comunidad de pescadores de subsistencia se tomaron muestras de cabello, aproximadamente 50 a 100 hebras de la zona del cuero cabelludo de la región occipital, al nacimiento o al mes de edad. Resultados: Participaron 15 recién nacidos; en 11 (73.33%) se detectó metilmercurio. En 6 (54.5%) de ellos la concentración osciló entre 100 y 699 ppm, y en los otros cuatro osciló entre 1,000 y más de 4,000 ppm. Conclusiones: El cabello del recién nacido es una fuente más de información para obtener datos de concentraciones de mercurio en cabello, y de interés para estudiar la carga de morbilidad ambiental por metilmercurio.

Abstract

Background Intracoronary pressure wire is useful to guide revascularization in patients with coronary artery disease. Aims To evaluate changes in diagnosis (coronary artery disease extent), treatment strategy and clinical results after intracoronary pressure wire study in real-life patients with intermediate coronary artery stenosis. Methods Observational, prospective and multicenter registry of patients in whom pressure wire was performed. The extent of coronary artery disease and the treatment strategy based on clinical and angiographic criteria were recorded before and after intracoronary pressure wire guidance. 12-month incidence of MACE (cardiovascular death, non-fatal myocardial infarction or new revascularization of the target lesion) was assessed. Results 1414 patients with 1781 lesions were included. Complications related to the procedure were reported in 42 patients (3.0%). The extent of coronary artery disease changed in 771 patients (54.5%). Figure 1 shows extent of coronary artery disease based on coronary angiographic analysis and following intracoronary pressure wire study. Figure 2A shows the extent of coronary artery disease based on coronary angiographic analysis and its reclassification following intracoronary pressure wire study. Abstract

Determining how growth rates and body size vary spatially and among reef fish species is important to understanding functional traits and demographic trade-offs. Variability in reef fish growth trajectories may be influenced by intrinsic (e.g., biological, phylogenetic) and extrinsic factors (e.g., environmental), as well as their interaction via ecological processes. To assess interspecific variation in these traits, we estimated age and growth for a guild of butterflyfishes sampled from reefs spanning ~ 10˚ degrees of latitude in the Red Sea to the adjacent Gulf of Aden. This study region was chosen because it spans environmental gradients known to influence fish life history traits and allowed for comparisons between regional versus more widespread butterflyfish species. Across the 10 study species, we found significant interspecific differences in growth. This finding contrasted with almost no intraspecific differences between populations across the study region. Moreover, we found that maximum body size was significantly correlated with the phylogenetic placement of the butterflyfish species. These patterns suggest that intrinsic factors and a high degree of ecological specialization may elicit spatially conservative demographic profiles, even when faced with considerable environmental variation across a region.

Abstract

Genome sequencing has been useful to gain an understanding of bacterial evolution. It has been used for studying the phylogeography and/or the impact of mutation and recombination on bacterial populations. However, it has rarely been used to study gene turnover at microevolutionary scales. Here, we sequenced Mexican strains of the human pathogen Acinetobacter baumannii sampled from the same locale over a 3 year period to obtain insights into the microevolutionary dynamics of gene content variability. We found that the Mexican A. baumannii population was recently founded and has been emerging due to a rapid clonal expansion. Furthermore, we noticed that on average the Mexican strains differed from each other by over 300 genes and, notably, this gene content variation has accrued more frequently and faster than the accumulation of mutations. Moreover, due to its rapid pace, gene content variation reflects the phylogeny only at very short periods of time. Additionally, we found that the external branches of the phylogeny had almost 100 more genes than the internal branches. All in all, these results show that rapid gene turnover has been of paramount importance in producing genetic variation within this population and demonstrate the utility of genome sequencing to study alternative forms of genetic variation.

Abstract

The emblematic French Way of Saint James (Camino de Santiago Francés) crosses towns, cities, and Spanish regions to the Cathedral of Santiago de Compostela (Galicia, Spain), However, where is The French Way of Saint James going with respect to the urban sustainability of its host cities? As each city is unique and urban sustainability favors the revitalization and transition of urban areas, to know where to go, it is first necessary to establish a diagnosis that makes the different urban situations visible. In this article, the behavior of urban sustainability is analyzed in the six host cities of The French Way of Saint James in the Autonomous Community of Castilla y León, a region characterized by its link with the rural environment and its current depopulation problems. The data and indicators used are officially provided by the Spanish Urban Agenda, which, through the normalization of its values, are able to territorialize the SDGs at the local level and reflect the realities of the cities of Burgos, Astorga, Cacabelos, León, Ponferrada, and Valverde de la Virgen. The results make it possible to diagnose and compare these host cities, identifying weaknesses, skills, and opportunities that favor the promotion of action plans, local or joint (favored by The French Way of Saint James), in the multiple aspects of sustainability. In addition, they show that Valverde de la Virgen is the city with the best performance in terms of urban sustainability.

Abstract

A series of surgical interventions - relaxing corneal incisions, intraocular lens, and intrastromal rings - were used to correct a case of extreme ametropia in a thin cornea after a penetrating keratoplasty in an 18-year-old patient who presented with a -10.25 -8.50 × 120 preoperative refraction and 20/200 best-corrected visual acuity (BCVA). After a series of surgical procedures, the patient's BCVA in his left eye improved to 20/30 with +0.50 -1.00 × 170, the slit lamp examination showed no significant findings, and the patient's visual complaints disappeared. At the 1-year follow-up, the BCVA was 20/25, without visual complaints. The process of individualizing the surgical procedure in the present case was employed in an outcome-based approach, that is, the next surgical procedure was defined after the surgery and postoperative evaluation. The patient did not present complications during the follow-up period of 2.5 years.

Introduction: Keratoconus is an idiopathic and progressive disease, where the cornea develops an irregular and conical shape, being the most common form of dystrophy or corneal ectasia, developing between the age of 12 and 20. In Mexico, the epidemiological information about the pathology is scarce. Purpose: To explore the epidemiology of keratoconus in Mexico among adolescents, and to compare the prevalence with international literature reports. This study identified associated pathologies and examined the management of patients. Methods: A retrospective study was conducted in an ophthalmology clinic; 500 charts were randomly selected from patients between 10 and 20 years of age in order to acquire information about the identification of the patient; the patient's gender, birthday, and age; three principal diagnoses at the first visit; as well as refraction and visual acuity in both eyes. After this, statistical analysis of the information was done. Results: The prevalence rate of keratoconus was 1.8%, affecting 66% of females and 33.3% of males. The mean age of presentation was 16.1 years. The most frequently associated refractive error was compound myopic astigmatism (44.4%); 88.8% presented with bilateralism. The majority of patients were being managed conservatively. Conclusion: Through this study, we found that our statistics matched those of internationally published reports concerning the early age of onset of the disease and its corresponding bilateralism. However, contrary to the international reports, it was evident that this condition was more prevalent among the females in our study sample, and no other associated pathologies were found.

La colonización de hábitat disponible es un proceso fundamental de la ecología de poblaciones y comunidades de organismos marinos. La colonización sobre sustratos artificiales por invertebrados y su variación espacial fue estudiada por un año en el arrecife coralino de La Azufrada, Isla Gorgona (Colombia). Cinco juegos de baldosas (terracota y grés) fueron dispuestos en tres zonas arrecifales (trasarrecife, planicie y talud). Las baldosas fueron recuperadas y examinadas después de 12 meses posteriores a su instalación. La comunidad desarrollada estuvo compuesta por 24 taxa siendo notable la ausencia de corales. No se encontraron diferencias en la riqueza de especies entre las zonas arrecifales, los materiales utilizados y las superficies de las baldosas. Las baldosas fueron dominadas por cirripedios (Balanus trigonus) con una densidad de 26 787.8 (±47 301.0) ind/m² (media±1D.E). La densidad de esta especie fue significativamente más alta en el talud y menor en las superficies superiores. La dominancia de B. trigonus durante el experimento sugiere un posible efecto sinérgico entre sus características reproductivas y su habilidad competitiva por el espacio evitando que otros organismos, como los corales, recluten exitosamente.

41. Short-term assessment of the sediment deposition rate and water conditions during a rainy season on La Azufrada coral reef, Gorgona Island, Colombia

Abstract

One of the major stresses on corals is the settlement of suspended sediment on their surfaces. This leads to the blocking of light, the covering of the coral mucus surface and an increased risk of disease. For this reason sediment deposition on a reef is considered a highly important variable in coral reef studies. With the use of sediment traps and oceanographic sensors, the sediment deposition rate and water conditions during a rainy season (April-May 2009) on a Tropical Eastern Pacific coral reef (La Azufrada) at Gorgona Island in Colombia were investigated. To quantify sediment deposition, sediment traps were established in nine stations along the coral reef (three stations per reef zone: backreef, flat and slope). To minimize disturbance by aquatic organisms in the sediment traps these were filled with hypersaline borax-buffered 10% formaline solution before their deployment; animals found in the filter contents were fixed and stored in a 4% formalin solution, frozen and identified in the laboratory. To determine the water conditions, discrete samples of water from 1 m and 10 m depths were collected using a Niskin bottle. Oceanographic variables (temperature, salinity and dissolved oxygen) as well as turbidity, chlorophyll and nutrient concentration (nitrite, nitrate and phosphorus) were measured in the samples from both depths. Vertical records of temperature and salinity were carried out with a Seabird-19 CTD nearest to La Azufrada and water transparency was measured using a Secchi disk. We found a mean trap collection rate of 23.30±4.34gm-2d-1 and did not detect a significant difference in the trap collection rate among reef zones. The mean temperature and salinity in the coral reef depth zone (0-10m layer) were 26.98±0.19°C and 32.60±0.52, respectively. Fourteen taxonomic groups of invertebrates were detected inside the sediment traps with bivalves and copepods being the most abundant and frequen. We investigate and classify possible analytical solutions for a simplified version of the foam bubble population model, by varying injection conditions and kinetic foam generation parameter. We prove that the behavior of the analytical solutions changes at the transition between two regions, similar to rarefaction-shock solutions for the Buckley-Leverett equation. In one region (region I), the solutions are in the form of traveling waves, in rather good agreement with CT scanned foam experiments and numerical simulations reported in Simjoo and Zitha (2015). In region II, however, corresponds to solutions as a sequence of waves: one spreading wave and one traveling wave. These corresponding flow profiles are different from those found so far in the experiments.

Abstract

Foam is used in enhanced oil recovery to improve the sweep efficiency by controlling the gas mobility. A common way to describe the foam displacement is by using population balance models, which consider the foam texture as part of the gas phase. Numerical simulation of such equations presents serious difficulties connected to the high non-linearity in the fractional flow. The linear kinetic model is studied mathematically for large initial reservoir water saturation and all possibilities of injection saturation. It was observed that the model contains some structural instabilities, i.e., a small variation in some parameters leads to qualitatively different solutions. One of these solutions presented localized decay in relative gas mobility, indicating that this behavior is due to the equations’ mathematical properties.

CD8+ T-cells directed against common viruses, such as influenza and Epstein-Barr virus, have been detected inside of human atherosclerotic lesions. These virus specific CD8+ T-cells have been hypothesized to contribute to the development of atherosclerosis, however, their mechanism of action in the atherosclerotic lesion remains unclear. This study aimed to characterize the behavior of virus specific CD8+ T-cells in the atherosclerotic lesion. The presence of virus specific CD8+ T-cells in atherosclerotic lesions was assessed by performing T-cell receptor (TCR)-β sequencing on human endarterectomy samples and patient matched blood samples (N=10). These TCRs were subsequently compared to known virus specific TCR sequences. Virus specific CD8+ T-cells seemed to accumulate in the atherosclerotic lesion (mean=3.5%), compared to patient matched blood samples (mean=2.0%) (p=0.053). Moreover, these virus specific CD8+ T-cells produced significantly more IFN-γ (p=0.0009) and Granzyme B (p=0.0038) in response to external stimuli, suggesting these T-cells may play a pro-inflammatory role in the pathogenesis of atherosclerosis. To investigate if virus specific CD8+ T-cells can be stimulated in situ, the immunopeptidome of 51 pooled human plaques was determined and matched with various viral peptidomes. Only one peptide presented in the atherosclerotic lesions matched a viral peptidome, rendering almost all virus specific CD8+ T-cells in the lesion antigen non-specific. In conclusion, virus specific CD8+ T-cells are enriched in atherosclerotic lesions and have an activated phenotype, compared to other plaque residing CD8+ T-cells. The absence of virus specific antigen presentation in the atherosclerotic lesion suggests that if these CD8+ T-cells contribute to inflammation, they do so in an antigen independent manner.

Introduction Coronary artery disease in diabetic patients has more aggressivity and tends to be a more complex disease characterized by small, diffuse, calcified, multivessel involvement that it presents higher in-stent restenosis rates after percutaneous coronary intervention (PCI). Besides, left main coronary artery (LMCA) disease has poor prognosis and it usually associates more severe lesions in other coronary arteries. Currently, the PCI in LMCA disease in these patients is controversial. Purpose The main objective of this study was to evaluate the efficacy and safety of PCI in LMCA disease in diabetic patients at a 14-year follow-up. Methods We prospectively included 228 consecutive patients (71 ± 11 years, 70.2% male) with LMCA disease treated with PCI between June 2006 and June 2020. We evaluated the occurrence of major adverse cardiovascular events (MACE) defined as cardiac death, nonfatal myocardial infarction, target lesion revascularization (TLR) and stent thrombosis after a 14-year clinical follow-up (median 66 months). Results 53.1% of patients had stable coronary disease and 46.9% acute coronary syndrome (42.1% Non-STEMI and 4.8% STEMI). 40.3% of patients presented moderate-severe left ventricular systolic dysfunction. 63.6% had LMCA and ≥ 2 vessels disease and 43% had a SYNTAX score ≥ 33. Mean Logistic Euroscore was 7.2%. The most frequent bifurcation technique employed in LMCA was "provisional stenting" in 65.4% and zotarolimus-eluting stent was used in 75.8% of cases. Angiographic success rate was 99.6% and complication rate intra-procedure was 3.1% without intraoperative death. During follow-up, MACE rate at 14 years was 19.8% (14.4% of cardiac death, 6.6% of nonfatal myocardial infarction, 5.7% of TLR and thrombosis rate 0.4%). Higher MACE rate was significantly associated to left ventricular systolic dysfunction (p=0.04) and chronic kidney insufficiency (p=0.02). 22.2% of patients had angiographic follow-up. Conclusions PCI on LMCA in diabetic patients is safe and effective, with low complication rate and a low rate of long-term follow-up events. Left ventricular systolic dysfunction and chronic kidney insufficiency showed a significantly worse outcome.

Introduction The progressive aging of the population leads to higher prevalence of ischemic heart disease with an increased left main coronary artery (LMCA) disease. Elderly patients (>75 years) represent an unfavorable subgroup because of their fragility, the frequent presence of multiple comorbidities associated and their especially high rate of surgical risk. Purpose The main objective of this study was to evaluate the efficacy and safety of percutaneous coronary interventions (PCI) in LMCA disease at 14 years follow-up in the elderly. Methods We prospectively included 217 consecutive patients (80.3 ± 2.4 years, 68.7% male) with LMCA disease treated with PCI between June 2006 and June 2020. We evaluated the occurrence of major adverse cardiovascular events (MACE) defined as cardiac death, non-fatal myocardial infarction, target lesion revascularization (TLR) and stent thrombosis after 14 years clinical follow-up (median 56 months). Results 48.4% of patients had stable coronary disease and 51.6% acute coronary syndrome (42.4% Non-STEMI and 9.2% STEMI). 43.3% were diabetic patients and 45.9% presented moderate-severe left ventricular systolic dysfunction. Mean logistic EuroSCORE was 10.4% and 47.5% of lesions with Syntax score ≥ 32. LMCA bifurcation was affected in 78.8% of cases and the most frequently bifurcation technique employed was "provisional stenting" in 64.1% of cases. Second-generation drug eluting stent was used in 97.6% of cases. The intraprocedural complication rate was 4.5% with one death. During follow-up, MACE rate at 14 years was 19.9% (14.7% cardiac death, 3.2% non-fatal myocardial infarction, 4.1% TLR and thrombosis rate 0.5%). There were significant differences in the occurrence of MACE in patients with moderate-severe ventricular systolic dysfunction (p=0.01 OR 3.92; IC 95% de 1.26-8.57). Conclusions Left main PCI in elderly patients with high surgical risk provide very favourable results with a low rate of immediate complications in the procedure and a low rate of cardiac adverse events at a very long-term follow-up. However, patients with ventricular dysfunction have poor prognosis.

Introduction Diabetic patients present more aggressive coronary artery disease and worse outcomes after percutaneous coronary intervention (PCI), especially in complex coronary bifurcation lesions. Currently, the benefit of "double stenting" techniques in the treatment of complex bifurcations in diabetics with coronary artery disease, is controversial. Purpose The aim of this study was to compare the outcomes of PCI with "mini-crush" two-stent technique in diabetic and non-diabetic patients, in terms of MACE: cardiovascular mortality, non-fatal myocardial infarction, target lesion revascularization (TLR) and stent thrombosis after long follow-up (median 32 months). Methods Observational study in which were included 152 consecutive patients (mean age 69.5 years, 38.3% diabetics) with 152 complex bifurcated coronary severe lesions that underwent PCI from April 2014 to April 2020. Results No significant differences were observed in the baseline characteristics, clinical presentation and antiplatelet pretreatment between both groups. Clopidogrel was the P2Y12-receptor inhibitor most commonly used. There were no differences in mean complexity according to the Syntax score (29.7 ± 11.3 in diabetics vs 30.4 ± 14.4 in non-diabetics, p=0.293). Left main bifurcation was the most frequently coronary artery affected in both groups and circumflex was the most common side vessel. The characteristics of the procedure was similar in both groups: no differences observed in pre-dilation rate either in final kissing balloon post-dilation rate. Similar length and diameter of the stents implanted in both groups. Angiographic follow-up clinically guided was 15.8% in diabetics and 16.9% in non-diabetics. We found no significant differences in the incidence of MACE or in any of its components separately. TLR rate was similar in diabetics and non-diabetics patients. Conclusions Comparable to non-diabetic patients, PCI in complex coronary bifurcation lesions with "mini-crush" two-stent technique in diabetic patients shows favorable results with low event rates and need for revascularization at long-term follow-up.

Introduction Drug eluting stents (DES) has improved the results of percutaneous coronary intervention (PCI) in terms of safety and effectiveness in left main coronary artery (LMCA) disease. Zotarolimus is a second-generation drug with excellent clinical outcomes but nowadays their results are unclear in this subgroup of high-risk coronary lesions. Purpose The main objective of this study was to evaluate the efficacy and safety of zotarolimus eluting stent (ZES) in LMCA disease at 14-year follow-up. Methods We prospectively included 432 consecutive patients (69 ± 12 years, 75.5% male) with LMCA disease treated with PCI and ZES between June 2006 and June 2020. We evaluated the presence of major adverse cardiovascular events (MACE) defined as cardiac death, nonfatal myocardial infarction, target lesion revascularization (TLR) and stent thrombosis, after 14-year clinical follow-up (median 61 months). Results 52.8% of patients had stable coronary disease and 47.2% acute coronary syndrome (38.2% Non-STEMI and 9% STEMI). 40.7% were diabetic patients and 40.8% presented moderate-severe left ventricular systolic dysfunction. 5.8% were protected LMCA and 81.7% bifurcated lesions. An intra-aortic balloon pump was needed in 4.9% of the cases. Mean logistic EuroSCORE was 6.7% and Syntax score was ≥ 23 in 73.1% of patients. The most frequently bifurcation technique employed in LMCA was "provisional stenting" in 67.1% of cases with an angiographic success of 99.3%. Complication rate in the procedure was 4.6% with one intraoperative death. During follow-up, MACE rate at 14 years was 17.4% (12.5% cardiac death, 3.5% nonfatal myocardial infarction, 4.2% TLR and thrombosis rate 0.5%). We observed significant differences in the occurrence of MACE in diabetic patients (p=0.002), chronic kidney insufficiency (p Conclusions PCI treatment with ZES in LMCA disease is safe and effective with a low rate of immediate complications and a low rate of mayor cardiac events at very long-term follow-up. Introduction Left main coronary ostial lesions are notable for being found more commonly among middle-aged women with fewer conventional coronary artery risk factors and for lower long-term patency of internal thoracic grafts. Isolated ostial left main coronary artery (LMCA) disease is involved in the 18% of lesions and have a management approach do well either with percutaneous coronary intervention (PCI) or cardiac surgery. Continuous improvement in the devices and the emergence of better drug-eluting stents (DES) has improved the results of PCI in these high-risk lesions. Purpose The main objective of this study was to evaluate the efficacy and safety of PCI in ostial LMCA disease at a 14-year follow-up. Methods We prospectively included 109 consecutive patients (72± 12 years, 72.5% male) with ostial LMCA disease treated with PCI between June 2006 and June 2020. We evaluated the presence of major adverse cardiovascular events (MACE) defined as cardiac death, non-fatal myocardial infarction, target lesion revascularization (TLR) and stent thrombosis after a 14-year clinical follow-up (median 63.6 months). Results 55.9% had stable coronary disease and 44.1% had acute coronary syndrome (35.9% Non-STEMI and 8.2% STEMI). 45% were diabetic patients and 36.6% presented moderate-severe left ventricular systolic dysfunction. The mean logistic EuroSCORE was 8.1 % and SYNTAX score was ≥ 23 in 72.5% of the patients. We implanted second-generation DES in the 83.5% of patients (76% zotarolimus-eluting stent) and post-dilatation was performed in the 68.8%. The angiographic success rate was 99%. Complication rate in the procedure was 5.5% without intra-procedure deaths. During follow-up, MACE rate at 14 years was 18.3% (13.7% of cardiac death, 4.6% of nonfatal myocardial infarction, 7.3% of TLR and thrombosis rate of 0.9%). 24.2% of patients had an angiographic follow-up. Conclusions In spite of high surgical risk, complex coronary anatomy and older aged patients, PCI in ostial LMCA disease is safe and effective with a low rate of major cardiovascular events at long-term follow-up.

Introduction Contradictory results have been seen regarding the benefit of double stenting techniques over provisional stenting in the treatment of complex bifurcation coronary lesions. There are few results for the "mini-crush" technique when used in acute coronary syndrome. Purpose To determine the influence of the clinical context on the results of the "mini-crush" technique, in terms of incidence of all-cause mortality and major cardiac events (MACE): combined event of cardiovascular mortality, AMI due to target lesion, and target lesion revascularization (TLR). Methods Observational study with 152 patients (mean age 69.5 ± 12.5 years, 73.7% male) from our hospital, presenting 152 complex severe bifurcation coronary lesions according to the Medina classification (1-0-1; 0-1-1; 1-1-1). Patients were divided into 2 groups: PCI (Percutaneous Coronary Intervention) due to acute coronary syndrome, or PCI scheduled in stable condition, using "mini-crush" technique from March 2014 to December 2020. Median follow-up of 32 months. Results The main results are shown in Table 1. 50% of the procedures were performed in patients with acute coronary syndrome. Regarding the baseline characteristics of the patients, no differences were found between the groups in the distribution of cardiovascular risk factors and LVEF (Left Ventricular Ejection Fraction) at the time of PCI. Complexity of coronary anatomy was similar in both groups according to the SYNTAX I scale. There was a high rate of pretreatment with a second antiplatelet agent in both groups (clopidogrel was the most frequent). In the acute coronary syndrome group, the main bifurcation vessel most frequently affected was the left anterior descending artery (diagonal artery the side branch). In stable patients, the left main, and the circumflex artery the side branch. There were no differences in the rate of predilatation or ending with "Kissing Balloon", neither in the stents used (zotarolimus stent was the most commonly used in both groups in the main vessel and side branch, with no differences in diameter and length). At the end of follow-up, new angiography had been performed for clinical indication in 13.2% of patients. The incidence of MACE was similar in both groups (13.2% in the acute coronary syndrome vs 11.8% in the group of stable patients, p=0.806). There was also no difference in all-cause mortality (13.2% vs 17.1%, p=0.698). Conclusions "Mini-crush" technique for the treatment of complex bifurcation coronary lesions showed similar results whether it was performed in the context of acute coronary syndrome or scheduled in stable condition, with no increase in all-cause mortality.
