1. Wrist shortening arthrodesis with volar plate in a dorsal position for spastic wrist contracture

Abstract: Total wrist arthrodesis in severe wrist flexion deformities (greater than 60°) due to spasticity represents a valid therapeutic option. It aims to improve the hand's appearance, hygiene, function and to prevent the deformity from getting worse. The objective of this study is to evaluate the clinical and anatomical results of wrist shortening arthrodesis using a classic volar plate in the dorsal position in functional surgery for central spastic hands. We conducted a single-center analysis of a series of patients who underwent this shortening arthrodesis. The review at a minimum 1-year follow-up included a clinical evaluation (House score, INOM score, patient satisfaction and complications), and anatomical evaluation (arthrodesis position, bone healing and carpometacarpal arthropathy). Twenty-eight patients with a mean age of 40.6 years (18-74) were included at a mean follow-up of 30.6 months (12-75). The fusion rate was 100%. No carpometacarpal arthropathy was noted. The mean position of the fused wrist was 11° extension and 15° ulnar tilt. There were two complications (7%): one postoperative hematoma and one case of discomfort due to impingement that required plate removal. The House score was significantly improved postoperatively (2.4 (0-5) versus 1.8 (0-4), p < 0.001), as was the INOM score (45 (12-64) versus 63 (36-84), p < 0.001). The patient satisfaction rate was 93%. The use of a simple and common material (volar plate in dorsal position) during this challenging surgery (spastic wrist contracture), provides good anatomical results and high patient satisfaction. 3. What can be expected from tendon transfers in the upper limb in central nervous system disorders?

Abstract: In addition to motor deficits, central nervous system disorders combine major alterations in the motor pattern with spasticity and over time, contractures. Their varied clinical presentation makes their assessment and the therapeutic strategy more complex. For these reasons, tendon transfers in this population will have more limited indications and above all, will have to be integrated into a complex surgical program combining other procedures such as tendon lengthening, selective neurotomies and joint stabilization. The surgical strategy is far from being obvious. When faced with clinical presentations having very different objectives – functional or comfort only – it is difficult at first sight to build a standardized surgical program. We therefore propose a method to evaluate these patients, thanks to a score (INOM) that integrates prognostic factors and parameters to be corrected surgically. Three components guide this program: a prognostic factor (proximal motor control of the shoulder and elbow), correction of abnormal limb postures and restoration of active elbow, wrist and finger extension. The surgical strategy can be constructed from the INOM score which establishes the priorities for care. Nerve blocks and botulinum toxin injections are essential tools for this analysis. They help distinguish between spasticity and contracture, and can unmask certain antagonistic muscles. A tendon transfer in this population will be just as effective by the function it restores as by the action it suppresses in a malpositioned limb. For each joint, we describe the indications for tendon transfers and their relative role among the techniques with which they must be combined.

Abstract: Objective To assess the benefit of isokinetic strengthening of the upper limb (UL) in patients with chronic stroke as compared to passive mobilization. Design Randomized blinded assessor controlled trial. Setting Physical Medicine and Rehabilitation departments of 2 university hospitals. Participants Patients (N=20) with incomplete hemiplegia (16 men; mean age, 64y; median time since stroke, 32mo). Interventions A 6-week comprehensive rehabilitation program, 3d/wk, 3 sessions/d. In addition, a 45-minute session per day was performed using an isokinetic dynamometer, with either isokinetic strengthening of elbow and wrist flexors/extensors (isokinetic strengthening group) or passive joint mobilization (control group). Main Outcome Measures The primary endpoint was the increase in Upper Limb Fugl-Meyer Assessment (UL-FMA) score at day 45 (t1). Secondary endpoints were increases in UL-FMA scores, Box and Block Test scores, muscle strength, spasticity, and Barthel Index at t1, t2 (3mo), and t3 (6mo). Results Recruitment was stopped early because of excessive fatigue in the isokinetic strengthening group. The increase in UL-FMA score at t1 was 3.5±4.4 in the isokinetic strengthening group versus 6.0±4.5 in the control group ( P =.2). Gains in distal UL-FMA scores were larger (3.1±2.8) in the control group versus 0.6±2.5 in the isokinetic strengthening group ( P =.05). No significant group difference was observed in secondary endpoints. Mixed models confirmed those results. Regarding the whole sample, gains from baseline were significant for the UL-FMA at t1 (+4.8; P P =.013) and t2. Conclusions In a comprehensive rehabilitation program, isokinetic strengthening did not show superiority to passive mobilization for UL rehabilitation. Findings also suggest a sustained benefit in impairments and function of late UL rehabilitation programs for patients with stroke.

Abstract: Motor nerve blocks with anesthetic drug for local anesthesia are commonly used in physical and rehabilitation medicine (PRM), especially in the field of spasticity. Guidelines in this context are currently lacking.Eighteen experts selected on the basis of their recognized experience by the scientific committees of the French PRM (SOFMER) and Anesthesia and Intensive care (SFAR) societies were invited to work and propose guidelines for the use of loco-regional anesthetic drug for motor nerve blocks in PRM setting. Eight issues were addressed: which neural blocks for which indications; drugs and contraindications; medical survey and attitude in case of adverse event; injection and guidance material; patient preparation and pain relief; efficacy assessment; patient information; education of PRM physiatrists. The Medline, Cochrane and Embase databases for the period 1999 to 2018 were consulted and 355 papers analyzed. The drafts were commented then approved by the whole group using electronic vote, before final approval by scientific committee of each society.No scientific evidence emerged from the literature. Thus, these guidelines are mainly based on the opinion of the expert panel. Guidelines for each issue are reported with the main points of arguments. The main question deals with the recommendation about doses for each drug: for lidocaine - up to 2mg/kg - "check contraindications, emergency truck available, no need of previous anesthetic consultation nor presence of anesthetic physician"; for ropivacaine - up to 1.5mg/kg, with a maximum of 100mg - the same but after intravenous line. Beyond these doses, SFAR guidelines have to be applied with the need of anesthetic physician.These are the first organizational guidelines devoted to increase the security of motor nerve block use in PRM settings.

Abstract: Objective To study the results and complications of percutaneous needle tenotomy for superficial retracted tendons in patients with brain damage. Design Prospective observational study. Setting University hospital. Participants Patients with severe brain damage (N=38; mean age, 60.7y; age range, 24–93y; 21 women) requiring surgical management of contractures and eligible for percutaneous needle tenotomy were enrolled between February 2015 and February 2016. Interventions The percutaneous needle tenotomy gesture was performed by a physical medicine and rehabilitation physician trained by an orthopedic surgeon, under local or locoregional anesthesia. Treated tendons varied among patients. Main Outcome Measures All patients were evaluated at 1, 3, and 6 months to assess surgical outcomes (joint range of motion [ROM], pain, and functional improvement) while screening for complications. Results Improvements in ROM (37/38) and contractures-related pain (12/12) were satisfactory. Functional results were satisfactory (Goal Attainment Scale score ≥0) for most patients (37/38): nursing (n=12), putting shoes on (n=8), getting in bed or sitting on a chair (n=6), verticalization (n=7), transfers and gait (n=8), and grip (n=2). Five patients had complications related to the surgical gesture: cast-related complications (n=2), hand hematoma (n=2), and cutaneous necrosis of the Achilles tendon in a patient with previous obliterative arteriopathy of the lower limbs (n=1). Conclusions Percutaneous needle tenotomy yields good results in the management of selected superficial muscle and tendon contractures. The complications rate is very low, and this treatment can be an alternative to conventional surgery in frail patients with neurologic diseases.

Abstract: Background. Bimanual coordinated movements may be impaired after stroke, so an assessment of causes is necessary to optimize rehabilitation strategies. Objective. We assessed the role of afference-based sources of coordination, including phase entrainment and error correction based on visual and somatosensory feedback. Methods. In all, 10 persons with unilateral chronic stroke and 8 age-matched controls participated in a kinesthetic tracking protocol, in which the hemiparetic upper limb was passively driven by the machine. The task consisted of matching the trajectory of the driven limb as accurately as possible with the freely moving limb in 2 conditions: eyes closed and eyes open. We analyzed the continuous relative phase (CRP), the mean absolute difference between positions (ADP) between the positions of the 2 limbs, and the jerk of the matching limb. Results. Coordination instability (CRP standard deviation) and mean ADP were significantly higher for patients with eyes closed, compared with patients with eyes open, controls with eyes closed, and controls with eyes open. Moreover, the jerk was higher for the nonparetic limb of patients than for the control group. Thus, the nonparetic limb did not produce optimally smooth movements even as the motor-driven paretic limb did. Conclusion. Besides deficits caused by interhemispheric competition and motor execution of the paretic limb, somatosensory feedback is a limiting factor in bimanual coordination after stroke. The findings have clinical implications pertaining to the design and individualization of efficient bimanual movement therapy.

Abstract: International audience; INTRODUCTION: In hemiplegic patients with a spastic clenched fist deformity, one of the goals of surgery is to address the hygiene, nursing and appearance problems. Transfer of the flexor digitorum superficialis (FDS) to the flexor digitorum profundus (FDP), initially described by Braun and colleagues, opens the non-functional hand in these patients. The primary objective of our study was to confirm the effectiveness of this technique for correcting these deformities. The secondary objectives were to demonstrate potential functional gains and to identify potential complications. MATERIAL AND METHODS: A Braun procedure was performed in 26 patients (9 women, 17 men, ranging in age from 36 to 79 years). The overall appearance of the hand was graded using the Keenan classification system. Complications related to the surgery were documented. The hand's function was evaluated with the House score. RESULTS: The average follow-up was 47~months. Preoperatively, all patients had a class V hand: severe clenched-fist deformity with zero pulp-to-palm distance. Postoperatively, 10 patients had a type I hand (open hand, with less than 20° spontaneous extension deficit of the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints) and 12 patients had a type II hand (20° to 40° spontaneous extension deficit of the MCP and PIP joints). The mean House score for all patients went from 0 to 0.88, and seven patients had functional improvements. Four patients had a forearm supination posture, 10 had intrinsic deformity with spontaneous MCP flexion and 6 had a swan-neck deformity. CONCLUSION: Superficialis-to-profundus tendon transfer (STP) provides satisfactory outcomes in terms of hand opening, with some patients also experiencing improved hand function. However, the complications cannot be ignored. LEVEL OF EVIDENCE: IV-retrospective or historical series.

10. Le score INO d’évaluation du membre supérieur chez le patient cérébrolésé – validation et applications cliniques

Abstract: Le polymorphisme clinique du membre superieur du cerebrolese explique en grande partie l’absence de classification pertinente. Notre objectif est de presenter et de valider un score d’evaluation du membre superieur du cerebrolese, offrant la possibilite de construire une strategie therapeutique et de l’evaluer. Ce score doit s’appuyer sur des parametres potentiellement modifiables lors de l’evaluation par blocs anesthesiques preoperatoires, apres des injections de toxine botulinique ou un programme chirurgical. L’evaluation doit etre rapide, realisable sans instrument et de facon indirecte a partir de videos standardisees. Le score compte six items modifiables bases sur la motricite et les attitudes vicieuses, il integre la fonction proximale de l’epaule et du coude, la prono-supination, l’extension du poignet, des doigts longs et du pouce. Trente patients âges en moyenne de 45 ans (16–79 ans) ont ete filmes lors de la realisation du test. Il s’agissait de 19 hommes et 11 femmes, presentant des sequelles d’un accident vasculaire cerebral dans 16 cas, d’une paralysie cerebrale dans 10 cas et d’un traumatisme crânien dans 4 cas. Onze de ces patients ont eu une evaluation pre- et postoperatoire. Afin de determiner les reproductibilites intra- et inter-individuelles de la grille, trois evaluateurs independants ont analyse en aveugle les videos des patients, et ce a 2 reprises puis les coefficients de correlation intra-classe ont ete calcules pour chaque item. Le score est exprime en pourcentage. Par ailleurs, pour chaque patient le score INO a ete compare a celui de House evaluant la qualite de prehension. La reproductibilite intra- et inter-individuelle de la grille d’evaluation INO etait bonne a excellente. La duree moyenne de realisation du test etait 1 minute (20 s a 4 min). Le score INO moyen etait de 47,5 % ± 28,1 (5–100 %) avec une repartition homogene des valeurs. Une forte correlation est observee entre les scores INO et celui de House. Une difference significative du score INO a ete observee entre les evaluations pre- et postoperatoires. Le score INO est un outil d’evaluation simple et rapide, utilisable a partir de videos avec une bonne fiabilite. Basee sur des deficits elementaires et des attitudes vicieuses, il guide la strategie therapeutique. Sa sensibilite aux effets des differentes therapeutiques et une forte correlation avec le score de House semblent pouvoir en faire un outil predictif des resultats de la chirurgie.

13. The percutaneous needle tenotomy in the treatment of tendon contractures in brain damaged patients: Pilot study

Abstract: Objective Assessment of the feasibility and efficacy of percutaneous needle tenotomy in patients with neuro-orthopedic disorders secondary to impairment of the central nervous system. Material and method Fourteen patients were followed in the Physical medicine and rehabilitation Department from September 2014 to March 2015. The average age was 58.7 years (29–86 y). The origin of neuro-orthopedic disorder was stroke (n = 7), parkinsonism (n = 2), head trauma (n = 2), cerebral palsy (n = 1), Little's disease (n = 1) and Alzheimer's disease (n = 1). The indication of percutaneous needle tenotomy was selected during a medical-surgical consultation. Twenty-one goals were identified: hygiene (n = 6), pain (n = 4), sitting (n = 3), standing (n = 4), transfers (n = 1), walking (n = 2), grasping (n = 1). A total of 31 sites were covered: the finger flexors (n = 9), the semitendinosus, biceps femoris and tensor fascia lata (n = 7), the Achilles tendon (n = 5), the biceps and brachioradialis (N = 3), the wrist flexors (n = 2), flexor digitorum longus (7 claw toes) and hip adductors (n = 1). In 10 patients the treatment involved several sites. Tenotomy was performed with a 18G needle (1,2 × 40 mm), under local or regional anesthesia. A plaster cast was associated with hamstring tenotomy if the objective was functional. A cast was always associated with Achilles tendon, elbow flexor and wrist flexor tenotomy. The Goal Attainment Scale (GAS) was used to assess the effectiveness of treatment. Results The targets were achieved in all cases (GAS ≥ 0). No side effects were noted. Discussion Percutaneous needle tenotomy is a technique which can be used as treatment of some neuro-orthopedic disorders in brain-damaged patients. Percutaneous tenotomy of the Achilles’ tendon has already been described by Minkowitz. Our study shows the feasibility and effectiveness of needle tenotomy, sometimes multi-site, performed in this type of patient.

14. Neuro-orthopedic deformities managment: A multidisciplinary approach

Abstract: Introduction Dystonic and dyskinetic syndromes produce musculotendinous retractions, subluxations and hip or shoulder dislocations. Most of the time, deep brain stimulation and pallidotomy cannot be used in secondary syndromes as they are in primitive forms. Drug treatments have frequent side effects. Very often these treatments are inadequate to prevent musculoskeletal deformities, which have an impact on pain, nursing, positioning and quality of life. Observation We followed a 45-year-old man with a spastic tetraparesia and a generalised dystonic and dyskinetic syndrome residual of a cerebral anoxia due to a heart arrest. His initial treatment was oral baclofen, anticholinergics and secondary intratechal baclofen. His symptoms increased progressively: pain, communication and nursing difficulties, mispositioning, elbow crease mycosis, as a consequence of musculoskeletal deformities. The patient had triple flexion of his upper limbs, antero inferior irreducible left shoulder dislocation, bilateral flessum of the knees. Multidisciplinary management has been provided, initially surgical then medical. Surgery included rotator cuff desinsertion of the left shoulder, needle percutaneous tenotomies of left elbow flexors, biceps femoris and semitendinus and plaster cast immobilisation. The medical treatment included twice-daily bolus of intratechal baclofen with dose-increase, botulinum toxin injection (left semimembranosus, right shoulder retractor muscles), phenolyc neurolysis (bilateral ansa pectoralis, left musculocutaneous nerve), physiotherapy and occupational therapy. The pain relief allowed a progressive morphinic doses decrease. Both armchair and bed positioning have been improved. Dyskinetic movements disappeared and verbal communication became easier. Discussion Neuro-orthopedic deformities management in patients with dystonia and dyskinesia may allow a decrease of dyskinetic movements and improve their comfort. It consists of multidisciplinary medicosurgical approach with tenotomies in association with oral or intratechal baclofen, botulinum toxin, chemical neurolysis and rehabilitation.

Abstract: Introduction The hand of the brain damaged patient combines motor deficit, abnormal muscle tone causing spasticity, and sensory defect. The purpose of surgical program could be functional by restoring grasp or non-functional by resolving hygienic problems, and improving cosmetic appearance due to the clenched fist deformity. The superficialis-to-profundus tendon transfer, first described by Braun and al., allows the opening of non-functional hands. The aim of our study was to confirm the efficacy of this technique to correct vicious attitudes, to highlight a possible functional gain and finally to identify any complications. Materials and methods Patients (9 women and 17 men, aged 36–79 years) were evaluated by a team of rehabilitation physicians and orthopeadic surgeons. Palliative transfer of the flexor digitorum superficialis tendons to the flexor digitorum profundus tendons has been achieved. Pictures were taken both preoperatively and during follow-up consultations. The hands were classified according to their appearance: the position of the wrist and fingers (Keenan and al.). Surgery complications were sought. Results Mean follow-up was 47 months. Preoperatively all hands were classified type 5 (closed hand: no distance between palm and pulp). Postoperatively 38% of patients have type 1 hand (wrist in neutral position, metacarpophalangeal joints [MCP] and proximal interphalangeal [PIP] at 20° of flexion) and 46% type 2 hand (fingers are more flexed [MCP and PIP at 40° of flexion]), with good cosmetic appearance. None of them have hygienic problems. Mean House score increased from 0 to 0.9, with 7 patients improved. Four patients had an attitude of supination of the forearm, 10 had an intrinsic-plus deformity and 6 had swan neck deformity of the finger. Secondary procedures had to be made. Conclusion This transfer gives satisfactory results on the opening of the hand but with significant complications. Intrinsic spasticity may be unmasked. Accessories pronator muscles release, while the biceps is spastic, can lead to an attitude of supination of the forearm, which causes functional and esthetic problems.

21. Percutaneous needle tenotomy in the treatment of neuro-orthopedic complications of upper limb related to central nervous system impairment: Open study on 12 patients

Abstract: Objective The musculotendinous retractions are common complications of central neurological damage with consequences for comfort and function. Treatment of uncomfortable retractions is often surgical, mainly based on tendon gestures. The objective of this study was to evaluate the efficiency of percutaneous needle tenotomy in these indications. Material/patients and methods The indication was accepted at a medical-surgical consultation. Tenotomy was performed using an 18 Gauge needle. The patients were older than 18 years, had a history of central neurological disease, and had a musculotendinous retraction regarding a superficial tendon of upper limb. The primary endpoint was the Goal Attainment Scale (GAS). Secondary outcomes were pain, the pulp-palm distance and range of motion gain. The evaluation was conducted in pre-tenotomy, immediately post-tenotomy, 3 months and 6 months. Results Twelve patients (8 women) were included in our study. The mean age was 62.91 years (28–87). Neuro-orthopedic disorders were related to a stroke ( n = 4), head trauma ( n = 2), to Parkinsonian syndrome ( n = 3) at a cerebral palsy ( n = 3). The objectives identified were nursing ( n = 12), pain ( n = 11), the skin condition ( n = 5), gripping ( n = 2), posture ( n = 1) and dressing ( n = 1). The targets were achieved in all cases. The average score GAS per patient ranged from 0 to 2. GAS score was unchanged at 3 months and 6 months. Discussion–conclusion Our experience confirms the locations already described for percutaneous needle tenotomy of the flexor of the fingers. We also report the effectiveness of percutaneous tenotomy in the wrist and elbow flexors without vascular and nerve injury elbow.

Abstract: Objective The sensori-motor impairment of upper limb (UL) affects more than 50% of patients after stroke. The objective of this work was to study the efficiency of isokinetic muscle strengthening (IMS) in the chronic phase of stroke in this population. Material/Patients and methods The patients underwent 6 weeks of outpatient rehabilitation, 3 days per week, combining physiotherapy (twice/day) and occupational therapy every day. The program was completed by 30 minutes elbow and wrist concentric slow speed IMS of flexor and extensor muscles, in the study group versus 30 minutes passive mobilisation of the joints carried out by the same isokinetic dynamometer in the control group. Inclusion criteria Age > 18 years, post stroke period > 6 months, muscle strength > 2/5 manual testing, spasticity Principal judgement criteria The UL Fugl Meyer (FM) score gain at the end of the program (6 weeks). Secondary criteria FM at 3 and 6 months, Box and Block test, Barthel Index and measures of muscle strength at 6 weeks, 3 and 6 months. Results Twenty patients were included: 16 men, 13 left hemiplegia, 16 ischemic strokes, average age 63. The gain in the FM score at 6 weeks was comparable between the two groups: 3.5 point (±4.4) versus +6 (±4.5) in the control group (P = 0.224). We have not observed shoulder pain or increase spasticity. No significant differences between the two groups has been demonstrated on secondary endpoints. All patients improved their FM score (4.7; P Discussion - Conclusion Combined with an intensive multidisciplinary rehabilitation program, IMS of UL is not more efficient than passive mobilisation of the wrist and elbow late after stroke. Our study also suggests the value of an intensive, out-patient rehabilitation treatment program in chronic phase after stroke, in cases of mild to moderate motor deficit of UL, remains beneficial over 6 months post-program.

Abstract: Background According to treatment guidelines in spasticity management, botulinum toxin injections should be offered as a treatment option for focal spasticity. In severe combined upper and lower limb or in multifocal spasticity, doses required to improve goal achievement and fulfil patients’ needs can result in total doses exceeding those currently approved, which are in France 360 U for onabotulinumtoxin A 400 U for incobotulinumtoxin A and 1500 U for abobotulinumtoxin A. Therefore, the challenges of treating patients with multifocal spasticity with botulinum toxin type A increase with the number of disabling patterns. Physicians may need to prioritize treating patterns with a high impact on overall goal achievement. Methods TOWER is a prospective, open-label, non-randomized, single-arm, multicenter, dose-titration phase III study with the aim to investigate the safety and efficacy of incobotulinumtoxinA in the treatment of spasticity in patients who are deemed to require total body doses of up to 800 U. The study employs a dose-titration approach over three injection cycles with a fixed total body dose of 400 U of incobotulinumtoxinA for injection cycle 1, 600 U for cycle 2 and 800 U for cycle 3 with a maximum dose per limb of 400 U in cycle 1 and 600 U in cycle 2 and 3. Results One hundred and fifty-five patients aged 18–80 years with spasticity of the upper and lower limb of the same body side due to cerebral causes and having to deemed by the investigator to require total body doses of incobotulinumtoxinA 800 U during the course of the trial, participate at 30 study sites in Canada, France, Germany, Italy, Norway, Portugal, Spain, and the USA. Results of the cycle with 800 U show that the number of patterns treated is higher with this cycle than at the cycle with 600 U. The same results are found between the cycle with 600 U and 400 U. Conclusion Many physicians would like to be allowed greater dosing flexibility to improve treatment satisfaction for a subset of patients with post-stroke spasticity. Results from TOWER study show that the treated number of patterns is higher with this cycle than the cycle with 600 U. The same results are found between the cycle with 600 U and 400 U.

Abstract: Innovative technologies for sensorimotor rehabilitation after stroke have dramatically increased these past 20 years. Based on a review of the literature on “Medline” and “Web of Science” between 1990 and 2013, we offer an overview of available tools and their current level of validation. Neuromuscular electric stimulation and/or functional electric stimulation are widely used and highly suspected of being effective in upper or lower limb stroke rehabilitation. Robotic rehabilitation has yielded various results in the literature. It seems to have some effect on functional capacities when used for the upper limb. Its effectiveness in gait training is more controversial. Virtual reality is widely used in the rehabilitation of cognitive and motor impairments, as well as posture, with admitted benefits. Non-invasive brain stimulation (rTMS and TDCS) are promising in this indication but clinical evidence of their effectiveness is still lacking. In the same manner, these past five years, neurofeedback techniques based on brain signal recordings have emerged with a special focus on their therapeutic relevance in rehabilitation. Technological devices applied to rehabilitation are revolutionizing our clinical practices. Most of them are based on advances in neurosciences allowing us to better understand the phenomenon of brain plasticity, which underlies the effectiveness of rehabilitation. The acceptation and “real use” of those devices is still an issue since most of them are not easily available in current practice.

Abstract: Objective Isokinetic strengthening is a rehabilitation technique rarely used in stroke patients. However, the potential benefits of force and endurance training in this population are strongly suspected. Method This literature review synthesizes the results of clinical trials on this topic. The research was conducted on PubMed, using “Stroke”, “rehabilitation”, “isokinetic”, “upper limb” and “training” as keywords. Results Seventeen studies focusing on the use of isokinetics in assessment or rehabilitation (six studies) following stroke were reviewed. For the lower limb, muscle strength and walking ability improved after isokinetic rehabilitation programs. For the upper limb, the only two studies found in the literature suggest improvement in the strength of the trained muscles, of grip force, of the Fugl-Meyer motor score and of global functional capacities. This review does not reveal any consensus on the protocols to be implemented: type of muscle contraction, velocities…. Conclusion While isokinetic strengthening has not proven its efficiency in rehabilitation of the upper limb following stroke, its interest with regard to rehabilitation of the lower limbs has been recognized. Randomized controlled trials in this field are necessary to confirm its efficiency, especially concerning upper arm rehabilitation.

It covers the 3 pillars of the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA): (A) Prevention and health promotion, (B) Care and cure, (C) and (D) Active and independent living of elderly people. All sub-activities (poly-pharmacy, falls prevention initiative, prevention of frailty, chronic respiratory diseases, chronic diseases with multimorbidities, chronic infectious diseases, active and independent living and disability) have been included in MACVIA-LR which has a strong political commitment and involves all stakeholders (public, private, patients, policy makers) including CARSAT-LR and the Eurobiomed cluster. It is a Reference Site of the EIP on AHA. The framework of MACVIA-LR has the vision that the prevention and management of chronic diseases is essential for the promotion of AHA and for the reduction of handicap. The main objectives of MACVIA-LR are: (i) to develop innovative solutions for a network of Living labs in order to reduce avoidable hospitalisations and loss of autonomy while improving quality of life, (ii) to disseminate the innovation. The three years of MACVIA-LR activities are reported in this paper.

Abstract: especially those who are seen soon after their stroke. Therefore, we aimed to create simplified more practical cards for inpatients. The presentation of the rehabilitation work by individual cards allow for delivery of only few exercises for each patient, which makes the program more acceptable and allows for better concentration on the prescribed program. Methods.– We have created 52 illustration cards in three categories: stretching, training and functional movements, addressing both upper and lower limbs using color-coding for easy identification, sketch indicating the location of the targeted muscle to be worked on, and related details always following the same format. The language was chosen to be understandable by all. The photos were selected to limit confusing elements. Different difficulty levels are available for the same muscle in order to select the appropriate card for the patient. Results.– Upon patient arrival in the department (traditional hospitalization or day hospital), the doctor prescribes GSC using the manual or the cards depending on the patient’s cognitive abilities. Individual rehabilitation sessions allow for the therapist to review and change each exercise and to instruct the patient on how to complete the logbook. After discharge, the patient regularly attends small group self-rehabilitation workshops to review the program and receive encouragement, advice and corrective instructions. Discussion.– The cards have been in use since February 2012 and are still being tested. We expect to improve homogeneity between the cards and the manual so that therapists and patients can easily switch from one tool to the other. Examples of cards and our experience with the first patients who have been using them will be presented.

48. Percutaneous needle tenotomies: indications, procedures, efficacy and safety. A systematic review.

Abstract: Background: Percutaneous needle tenotomies constitute a promising approach that enables direct access to tendons through minimally invasive interventions. They can be performed rapidly without need for large incisions or general anaesthesia. However, the reported procedures are heterogeneous and currently conducted without guidelines., Objectives: We aimed to determine the indications for percutaneous needle tenotomies described in the current literature. Our secondary aim was to identify the different procedures reported, as well as their efficacy and their safety., Methods: A systematic review following PRISMA guidelines was conducted to identify original articles that mentioned percutaneous needle tenotomy in humans and reported its application, description, effectiveness or adverse events. Non-percutaneous tendinous surgical procedures and ineligible designs were excluded. The Downs and Black checklist was used to assess the risk of bias., Results: A total of 540 studies were identified from the MEDLINE, Embase, Cochrane Library, and PEDro databases. Fourteen clinical studies met the inclusion criteria and were found to have an acceptable quality (674 individuals, 1664 tenotomies). Our results indicated a wide variety of indications for percutaneous needle tenotomies in children and in adults. We highlighted 24 tendons as eligible targets in the upper and lower limbs. Tenotomies were performed with either 16- or 18-Ga needles, lasted from 1 to 30 min, and were performed using various procedures. Their efficacy was mainly assessed through clinical outcomes highlighting tendon discontinuity on palpation after the procedure. Passive range-of-motion gains after tenotomy were reported for both upper and lower limbs with an estimated 5 % complication rate., Conclusion: This is the first review to systematically synthesize all the available evidence on the indications, procedures, efficacy and safety of percutaneous tenotomies exclusively performed with needles. 49. Arthroscopic shoulder suspensioplasty in painful hemiplegic shoulder subluxation-a case series.

Abstract: Background: Inferior glenohumeral subluxation (GHS) can cause disabling pain in hemiplegics. Conservative treatments have not been proven to be effective or maintained over time. A few studies have shown the benefits of surgical treatment. The objective of our study was to evaluate the medium-term clinical and radiological results of arthroscopic glenohumeral suspensioplasty surgery by biceps tenodesis in the setting of painful GHS in hemiplegics., Methods: We conducted a retrospective study of patients who underwent arthroscopic glenohumeral suspensioplasty. The assessment, at a minimum of 1 year, included a clinical evaluation (pectoralis major spasticity, pain, range of motion, satisfaction) and a radiographic evaluation (Dursun classification, height of subacromial space)., Results: Five patients with a mean age of 51 years (36-72 years) were included at a mean follow-up of 40 months (12-70). Satisfaction was good in 80% of patients. Pain decreased in all patients, but not significantly. Four patients (80%) would repeat the procedure if it were necessary. In all patients, a reduction in GHS over time was observed, with a reduction in subacromial height, except in 1 patient who suffered a tenodesis rupture during a fall., Conclusion: Our results suggest that arthroscopic glenohumeral suspensioplasty by biceps tenodesis may be a therapeutic option in hemiplegic patients with painful GHS., (© 2023 The Authors.)

Abstract: Objective: To establish international recommendations for the management of spastic equinovarus foot deformity., Design: Delphi method., Setting: International study., Participants: A total of 24 international experts (N=24) in neuro-orthopedic deformities, from different specialties (Physical and Rehabilitation Medicine physicians, neurologists, geriatricians, orthopedic surgeons, neurosurgeons, plastic surgeons)., Interventions: Experts answered 3 rounds of questions related to important aspects of diagnosis, assessment, and treatment of spastic equinovarus foot deformity., Main Outcome Measures: A consensus was established when at least 80% of experts agreed on a statement RESULTS: A total of 52 items reached consensus. Experts recommend assessing effect of the deformity on functional activities before treatment. Before treatment, it is crucial to differentiate spastic muscle overactivity from soft tissue contractures, identify which muscles are involved in the deformity, and evaluate the activity of antagonist muscles. Motor nerve blocks, 2-dimensional video analysis, and radiologic examinations are often required to complement a clinical examination. The treatment of equinovarus foot depends on the correctability of the deformity and the patient's ability to stand or walk. The preoperative assessment should include an interdisciplinary consultation that must finalize a formal agreement between physicians and the patient, which will define personalized attainable goals before surgery., Conclusion: The establishment of guidelines on managing equinovarus foot will help physicians and surgeons, specialists, and nonspecialists to diagnoses and assess the deformity and direct patients to a network of experts to optimize patient functional recovery and improve their autonomy., (Copyright © 2022.
