Vandaag de dag is er in de neuropsychologie veel kennis verworven over de structuur van aandacht als cognitief fenomeen. Recent psychometrisch onderzoek met de Everyday Life Attention Scale (ELAS) wijst uit dat de structuur van aandacht in het dagelijks leven situatiespecifiek is. De verschillende neuropsychologische componenten van aandacht, zoals intensiteit en selectiviteit, smelten in het dagelijks leven samen tot een situatiespecifieke ervaring. De ELAS beschrijft negen alledaagse situaties (boek lezen, televisie kijken, activiteit binnenshuis, lezing bijwonen, gesprek voeren, opdracht uitvoeren, maaltijd bereiden, opruimen en autorijden) die door de deelnemer gescoord worden op de verschillende aandachtsfuncties. De kwaliteit van aandacht van de deelnemer kan vervolgens voor iedere (voor de deelnemer relevante) situatie worden vastgesteld aan de hand een grote Nederlandse normgroep (n = 1.874; leeftijd 18–76 jaar). Personen met aandachtstekortstoornis met hyperactiviteit (ADHD) hebben een grotere kans om minder goede (levens)beslissingen te nemen en om risicovolle activiteiten te ondernemen dan personen zonder ADHD. Mogelijk komt dit doordat de kenmerken van ADHD van invloed zijn op het besluitvormingsproces. Hoewel beslissingsproblematiek reeds uitgebreid is onderzocht bij kinderen en adolescenten met ADHD, is er nog relatief weinig bekend over de besluitvorming van volwassenen met ADHD. Om die reden was het doel van dit literatuuronderzoek de aard en omvang van eventuele tekorten in het besluitvormingsproces van volwassenen met ADHD vast te stellen. Hiertoe is de bestaande literatuur, waarin de prestatie van volwassenen met ADHD op beslissingstaken werd vergeleken met de prestatie van een gezonde controlegroep, systematisch doorzocht, waartoe de databases PsycINFO, MEDLINE en PubMed zijn geraadpleegd. In totaal werden er 31 studies geïncludeerd. In de meerderheid van de studies (i.e. 55 %) weken de prestaties van volwassenen met ADHD af op een of meer van de gebruikte beslissingstaken in vergelijking met de controlegroep(en). Dit literatuuronderzoek levert daarmee voorzichtig bewijs voor het bestaan van verschillen in het besluitvormingsproces tussen gezonde individuen en volwassenen met ADHD. De grote inconsistentie in de bevindingen wordt deels verklaard door de verscheidenheid aan domeinen van besluitvorming die werden onderzocht, de comorbide stoornissen van de participanten en het medicatiegebruik in de ADHD-groepen. Het literatuuronderzoek besluit met een bespreking van de implicaties die de bevindingen hebben voor theorieën over de onderliggende mechanismen van ADHD.

Ein 48-jahriger hochtrainierter und leistungsfahiger Patient ohne klassische myokardiale Risikofaktoren wurde nach rezidivierenden pektanginosen Beschwerden mit leichtem Troponin-T-Anstieg (cTnT) zur Abklarung aufgenommen. Eine vorangehende kardiologische Untersuchung einschlieslich Szintigraphie und Koronarangiographie hatte keinen pathologischen Befund ergeben. Anamnestisch waren eine rheumatoide Arthritis mit persistierender Raynaud-Symptomatik sowie Hamoglobin- und Hamatokritwerte im oberen Normbereich bekannt. Wahrend einer Langzeitbelastung auf dem Ergometer traten nach 1,55 h plotzlich Angina pectoris und massive ST-Hebungen in II, III und aVF auf. Die anschliesend durchgefuhrte Koronarangiographie zeigte jedoch ein unauffalliges Koronarsystem. Bei einem Anstieg des cTnT auf maximal 0,979 ng/ml wurde von einem Infarktgeschehen infolge eines Vasospasmus bei sonst freien Koronararterien ausgegangen und eine antivasospastische Therapie eingeleitet. 6 Monate spater traten erneut bei geringerer Belastung Thoraxschmerzen auf. Bei der Aufnahme ST-Absenkung in V4 und V5 mit praterminal negativen T bei normalen Herzenzymen inklusive cTnT. In der daraufhin durchgefuhrten Mehrschicht-CT-Koronardarstellung zeigte sich eine koronare Eingefaserkrankung mit filiformer Stenose des proximalen RIVA, die nachfolgend kathetertechnisch dilatiert und mit einem Stent versorgt wurde. Im Rahmen der Abklarung der erhohten Hamatokrit- und Hamoglobinwerte wurde eine Polycythaemia vera bei Vorliegen einer JAK2-V617-Mutation gesichert. Der weitere Verlauf war unter der Medikation mit Acetylsalicylsaure, Clopidogrel, Candesartan, Atorvastatin und intermittierenden Aderlassen komplikationslos. Auch wenn eine relative Gewichtung schwierig ist, durfte das Zusammentreffen einer Hyperkoagulabilitat bei Polycythaemia vera, einer erhohten spastischen koronaren Reagibilitat, moglicherweise verstarkt durch physischen Belastungsstress, sowie einer belastungsinduzierten Hamokonzentration die Ursache fur rezidivierende thrombotische koronare Ereignisse bzw. den subtotalen Verschluss des RIVA gewesen sein. Umfangreiches Ausdauertraining und fehlende „klassische“ myokardiale Risikofaktoren schliesen das bei dieser Konstellation offensichtlich nicht aus.

Background Long-term synaptic plasticity is a ubiquitous form of neuronal plasticity that regulates the strength of synaptic transmission in many brain areas. However, most data on long-term potentiation and long-term depression rely on research in animal brain slices. The role of synaptic plasticity in physiology and pathology of the functioning human brain remains obscure. Considering recent studies that provided evidence for a dysfunction of brain plasticity as the neurobiological basis of affective disorders, we assessed neural transmission and its plastic modulation in the visual pathway in healthy control subjects and patients with major depression. Methods Recordings of visually evoked potentials (VEPs) in humans. Results Prolonged visual presentation of checkerboard reversals resulted in a sustained amplitude modulation of early components of subsequent VEPs. After a 10-min block of visual stimulation (two checkerboard reversals per second), the C1 component was reduced, whereas P1 and N1 were both significantly increased for >30 min. Chronic application of the selective serotonin reuptake inhibitor sertraline in healthy control subjects augmented these effects, whereas the polarity of the modulation was inverted in patients with severe major depression. Moreover, early VEP amplitudes were decreased in depressed patients when compared with matched control subjects and increased in normal control subjects after chronic intake of an antidepressant. Conclusions Our results demonstrate that stimulus-induced response plasticity of VEPs can be induced in the human brain and is sharing properties with hebbian forms of synaptic plasticity. Major depression and antidepressant treatment of healthy control subjects differentially modulate amplitude and plasticity of evoked potentials. This study provides direct evidence in humans for a central role of synaptic plasticity in the pathophysiology of depression.

Das von der amerikanischen Schriftstellerin Getrude Stein stammende Zitat “Rose is a Rose is a Rose is a Rose.”, das sinngemäß mit „Dinge sind wie sie sind“ übersetzt werden kann, steht in der vorliegenden Arbeit als Frage formuliert für eine Diskussion um die Neuaushandlung der Rolle von Unternehmen in der Gesellschaft. Entgegen der von vielen öffentlichen und veröffentlichten Meinungen produzierten Rolle von Unternehmen als Akteure, die lediglich ökonomische Verantwortung als Systemimperativ ihres Handelns zeigen, handeln zahlreiche Unternehmen über das traditionelle ökonomische Verständnis des Unternehmens hinaus. Anhand einer Fallstudie in der Schnittblumenindustrie Ecuadors zeigt die vorliegende Arbeit, wie Spielregeln unternehmerischen Handelns und die Verantwortungsbereiche von Farmen in der Gemeinde Tabacundo des Kantons Pedro Moncayo neu interpretiert werden. Der ecuadorianische Blumenverband Expoflores spricht landesweit von ca. 700 produzierenden Schnittblumenbetrieben. Der Schwerpunkt der Industrie befindet sich mit ca. 80 Farmen nördlich von Quito im Kanton Pedro Moncayo auf insgesamt ca. 970 Hektar Land. 30% der 33.000 im Kanton lebenden Personen sind direkt in der Schnittblumenindustrie beschäftigt. Einige der Farmen kommunizieren soziales und ökologisches Engagement mit grünen Labels. Mit Hilfe des Konzeptes der Corporate Social Responsibility als theoretischen Zuschnitt wird untersucht, wie sich labelzertifizierte Farmen in sozialer Hinsicht im Forschungsgebiet Tabacundo engagieren und wie sich dieses Verhalten auf ausgewählte Indikatoren einer sozial nachhaltigen Regionalentwicklung auswirkt. Implementiert wird soziales Verantwortungsdenken in unternehmensinterne Prozesse mit Hilfe einer breiten Auswahl an Instrumenten, von denen insbesondere die Arbeitnehmer in den Farmen profitieren. Die Befriedigung der Bedürfnisse der Farmbelegschaften zeigt sich in einer Akkumulation von Sachkapital, Humankapital und Sozialkapital in den Farmen, das sich aufgrund der verstreuten Lage der Wohnorte der profitierenden Farmmitarbeiter fragmentarisch in Tabacundo und den umliegenden Gemeinden und Kantonen verteilt. Bezüglich der Fristigkeit beschriebenen sozialen Engagements von Blumenfarmen zeigt sich, dass die zeitliche Dauer der Engagements von den Motivationen der Farmführungen abhängig ist. Rein altruistische Beweggründe führen gegenüber ökonomisch motivierten CSR-Aktivitäten zu deutlich kurzfristigeren CSR-Engagements.

Objective Doppler-derived flow velocity measured by transesophageal echocardiography (TEE) may overestimate pulmonary vein stenosis. We hypothesized that combining peak velocity with a stenotic flow pattern improves diagnosis compared with magnetic resonance imaging (MRI). Methods TEE and MRI were performed in 44 patients 19 ± 11 months after radiofrequency catheter ablation. Pulmonary vein stenosis was defined by a peak velocity of 110 cm/s or more plus a stenotic flow pattern (turbulence and reduced flow variation) on TEE and a lumen reduction of more than 50% on MRI. Results In all, 175 pulmonary veins were studied. MRI showed 7 cases of pulmonary vein stenosis that were correctly identified by TEE. In addition, TEE criteria for pulmonary vein stenosis were met in 4 pulmonary veins that did not show obstruction on MRI. In all, 5 pulmonary veins with normal appearance on MRI had peak velocity of 110 cm/s or more with normal flow pattern. Conclusions TEE Doppler measurements can be reliably used to detect or exclude significant pulmonary vein stenosis if the diagnosis is restricted to a combination of elevated peak velocity (≥ 110 cm/s) with turbulence and little flow variation.

MRT (magnetic resonance imaging) is a new method of producing pictures of internal organs; the main advantage compared to radiography is the ability to subtly differentiate soft tissues. 14 children suffering from juvenile chronic arthritis of different subgroups are described. MRT pictures of knee joints show clearly the structure and form of the articular cartilage, ligaments as well as the nature, density and thickness of the inflammatory tissue. MRT permits a very early diagnosis of a progression in the inflammatory articular process without accompanying ionising radiation, an advantage in childhood.

Here, the case of a 48-year-old highly trained patient without classic myocardial risk factors is described who reported on frequent and recurrent angina pectoris. In a previous examination, the test for cardiac troponin T (cTnT) was slightly positive, however, cardiac examination including myocardial perfusion scintigraphy and coronary catheterization was without pathologic findings. Worth mentioning in the past medical history was a rheumatoid arthritis with persistent Raynaud's symptoms and hemoglobin as well as hematocrit levels in the upper normal range.The patient reported that symptoms would occur most likely during long-term endurance exercise. Therefore, a bicycle ergometry with 180 W and open ending was performed. After 1.55 h, the patient complained of severe angina pectoris. The ECG showed massive ST segment elevations in II, III, and aVF (Figure 2). However, coronary catheterization showed no major stenosis or occlusion. Nevertheless, cTnT increased to 0.979 ng/ml. (Therefore, the reason for the symptomatic ST elevation was believed to be a myocardial tissue damage due to coronary vasospasm and the patient received an antivasospastic medication (amlodipine, atorvastatin, and acetylsalicylic acid [ASS]). After 6 months, the patient had a relapse during moderate physical activity. The ECG showed an ST segment depression in V(4) and V(5) while markers for myocardial tissue damage including cTnT were negative. A coronary CT angiography was performed that revealed a subtotal stenosis of the proximal LAD (Fig ure 3), which was successfully treated by angioplasty with subsequent stenting. In the course of further examinations, a polycythemia vera (JAK2-V617 mutation) was diagnosed as the cause for the high hemoglobin and hematocrit levels. Since then, the patient was without further events under a medication consisting of ASS, atorvastatin, candesartan, and intermittent phlebotomy.Although a relative weighting is difficult, it can be assumed that the combined effects of a polycythemia- associated hypercoagulability, an increased reagibility of the coronary arteries (aggravated by physical stress) as well as a hemoconcentration following prolonged exercise, could account for symptomatic recurrent minor thrombotic coronary events as well as the subtotal occlusion of the LAD. Obviously, this could not be prevented by a healthy lifestyle, regular physical activity, and the absence of classic coronary risk factors.

Die kardiale Magnetresonanztomographie (CMR) ist ein nichtinvasives bildgebendes Verfahren ohne ionisierende Strahlen, das nicht nur die Morphologie mit hohem Weichteilkontrast abbildet, sondern auch zahlreiche funktionelle Informationen liefert. Das Phanomen der Magnetresonanz wurde erstmals 1946 von Bloch u. Purcell beschrieben (Bloch et al. 1946; Purcell et al. 1946) und zunachst fur die Strukturanalyse von Molekulen verwandt. Die Bildgebung mit der Magnetresonanz wurde 1973 von Lauterbur eingefuhrt (Lauterbur 1989), die klinische Anwendung zur Herzbildgebung begann in den 80er-Jahren. Heute steht fur die Magnetresonanz ein breites Spektrum von Pulssequenzen zur Verfugung, um Herzmorphologie, chemische Zusammensetzung des Myokards, Ausdehnung eines Infarktes, Pumpfunktion, Blutfluss, Klappenfunktion und zahlreiche weitere relevante Grosen zu untersuchen. Im nachstehenden Kapitel werden derzeit verfugbaren Methoden, deren klinische Anwendung, sowie zukunftige Entwicklungen beschrieben.

Aims Pulmonary vein ablation offers the potential to cure patients with atrial fibrillation. In this study, we investigated the incidence of pulmonary vein stenosis after radiofrequency catheter ablation of refractory atrial fibrillation by systematic long-term follow-up. Methods and results Forty-seven patients with refractory and highly symptomatic atrial fibrillation underwent radiofrequency catheter ablation of arrhythmogenic triggers inside the pulmonary veins and/or ostial pulmonary vein isolation with conventional mapping and ablation technology. These patients had follow-up examinations at 2 years with transoesophageal doppler-echo and/or angio magnetic resonance imaging for the evaluation of the pulmonary veins. Seventy-seven percent of the patients were free from atrial fibrillation, 51% were without antiarrhythmic drugs, and 26% were on previously ineffective antiarrhythmic drug therapy. However, 13 of the 47 patients showed significant pulmonary vein stenosis or occlusion. Only three of these 13 patients complained of dyspnoea. Distal ablations inside the pulmonary vein were associated with a 5.6-fold higher risk of stenosis than ostial ablations. Conclusions At 2-year follow-up, the risk of significant pulmonary vein stenosis/ occlusion after radiofrequency catheter ablation of refractory atrial fibrillation with conventional mapping and ablation technology was 28%. Distal ablations inside smaller pulmonary veins should be avoided because of the higher risk of stenosis than ablation at the ostium.
