Following the first point-prevalence study in Dutch nursing homes conducted each November from 2007 to 2009, we conducted a follow-up point-prevalence study of healthcare-associated infections (HCAIs) each November from 2010 to 2017. Similar methods and criteria were used. Resident characteristics were recorded, data collection was performed by the attending elderly care physicians via an online survey, as well as via a specifically designed App from 2012. As of the same year, information on incontinence was added. Between 2010 until 2017 on average 1786 residents per year were included, ranging from 1571 to 2185. HCAI prevalence with respect to age (mean: 83 years) and sex (31% men and 69% women) were similar over all the years. The overall mean prevalence rate in the first four years was 6.7% versus 2.2% in the last six years. Urinary tract infection was the most prevalent HCAI (1.5%). Most HCAIs occurred among residents of rehabilitation units. The prevalence of HCAI varied by nursing home (0.0-37.0%). The average use of antibiotics was stable over the years (6.0%) irrespective of HCAI rate. Use of incontinence materials was on average 73.5% with 64.3% of residents being reported as incontinent. Those implementing improvement of infection control and surveillance within a new setting do need to continue for multiple years before seeing the success of their endeavour.

Background The phase III CLinical Evaluation Of Pertuzumab And TRAstuzumab (CLEOPATRA) trial established the combination of pertuzumab, trastuzumab and docetaxel as standard first-line therapy for human epidermal growth factor receptor 2 (HER2)-positive locally recurrent/metastatic breast cancer (LR/mBC). The multicentre single-arm PERtUzumab global SafEty (PERUSE) study assessed the safety and efficacy of pertuzumab and trastuzumab combined with investigator-selected taxane in this setting. Patients and methods Eligible patients with inoperable HER2-positive LR/mBC and no prior systemic therapy for LR/mBC (except endocrine therapy) received docetaxel, paclitaxel or nab-paclitaxel with trastuzumab and pertuzumab until disease progression or unacceptable toxicity. The primary endpoint was safety. Secondary endpoints included progression-free survival (PFS) and overall survival (OS). Prespecified subgroup analyses included subgroups according to taxane, hormone receptor (HR) status and prior trastuzumab. Exploratory univariable analyses identified potential prognostic factors; those that remained significant in multivariable analysis were used to analyse PFS and OS in subgroups with all, some or none of these factors. Results Of 1436 treated patients, 588 (41%) initially received paclitaxel and 918 (64%) had HR-positive disease. The most common grade ≥3 adverse events were neutropenia (10%, mainly with docetaxel) and diarrhoea (8%). At the final analysis (median follow-up: 5.7 years), median PFS was 20.7 [95% confidence interval (CI) 18.9-23.1] months overall and was similar irrespective of HR status or taxane. Median OS was 65.3 (95% CI 60.9-70.9) months overall. OS was similar regardless of taxane backbone but was more favourable in patients with HR-positive than HR-negative LR/mBC. In exploratory analyses, trastuzumab-pretreated patients with visceral disease had the shortest median PFS (13.1 months) and OS (46.3 months). Conclusions Mature results from PERUSE show a safety and efficacy profile consistent with results from CLEOPATRA and median OS exceeding 5 years. Results suggest that paclitaxel is a valid alternative to docetaxel as backbone chemotherapy. Exploratory analyses suggest risk factors that could guide future trial design.

6. An Iterative Least Squares Method for Proton CT Image Reconstruction

Clinically useful proton Computed Tomography images will rely on algorithms to find the three-dimensional proton stopping power distribution that optimally fits the measured proton data. We present a least squares iterative method with many features to put proton imaging into a more quantitative framework. These include the definition of a unique solution that optimally fits the protons, the definition of an iteration vector that takes into account proton measurement uncertainties, the definition of an optimal step size for each iteration individually, the ability to simultaneously optimize the step sizes of many iterations, the ability to divide the proton data into arbitrary numbers of blocks for parallel processing and use of graphical processing units, and the definition of stopping criteria to determine when to stop iterating. We find that it is possible, for any object being imaged, to provide assurance that the image is quantifiably close to an optimal solution, and the optimization of step sizes reduces the total number of iterations required for convergence. Purpose The diagnosis and treatment of cancer negatively affect patients’ physical, functional and psychological wellbeing. Patients’ needs for care cannot be addressed unless they are recognized by healthcare providers (HCPs). The use of quality of life (QoL) assessments with feedback to HCPs might facilitate the identification and discussion of QoL-topics. Methods 113 patients with stage I–IIIB breast cancer treated with chemotherapy were included in this randomized controlled trial. Patients were randomly allocated to receive either usual care, or usual care with an intervention consisting of a QoL-monitor assessing QoL, distress and care needs before every chemotherapy cycle visit. Patients completed questionnaires regarding QoL, illness perceptions, self-efficacy, and satisfaction with communication. From the 2nd visit onwards, patients in the intervention arm and their HCPs received a copy of the QoL overview and results were shown in patients’ medical files. Audio-recordings and patients’ self-reports were used to investigate effects on communication, patient management and patient-wellbeing. A composite score for communication was calculated by summing the number of QoL-topics discussed during each consultation. Results Use of the QoL-monitor resulted in a higher communication score (0.7 topics increase per visit, p = 0.04), especially regarding the disease-specific and psychosocial issues (p n = 60) had higher scores on satisfaction with communication (p Conclusions Use of a QoL-monitor during chemotherapy in patients with early breast cancer might result in a more frequent discussion of QoL-topics, associated with high levels of patients’ satisfaction.

Background The phase 3 CAIRO3 study showed that capecitabine plus bevacizumab (CAP-B) maintenance treatment after six cycles capecitabine, oxaliplatin, and bevacizumab (CAPOX-B) in metastatic colorectal cancer (mCRC) patients is effective, without compromising quality of life. In this post hoc analysis with updated follow-up and data regarding sidedness, we defined subgroups according to RAS/BRAF mutation status and mismatch repair (MMR) status, and investigated their influence on treatment efficacy. Patients and methods A total of 558 patients with previously untreated mCRC and stable disease or better after six cycles CAPOX-B induction treatment were randomised to either CAP-B maintenance treatment (n = 279) or observation (n = 279). Upon first progression, patients were to receive CAPOX-B reintroduction until second progression (PFS2, primary end point). We centrally assessed RAS/BRAF mutation status and MMR status, or used local results if central assessment was not possible. Intention-to-treat stratified Cox models adjusted for baseline covariables were used to examine whether treatment efficacy was modified by RAS/BRAF mutation status. Results RAS, BRAF mutations, and MMR deficiency were detected in 240/420 (58%), 36/381 (9%), and 4/279 (1%) patients, respectively. At a median follow-up of 87 months (IQR 69–97), all mutational subgroups showed significant improvement from maintenance treatment for the primary end point PFS2 [RAS/BRAF wild-type: hazard ratio (HR) 0.57 (95% CI 0.39–0.84); RAS-mutant: HR 0.74 (0.55–0.98); V600EBRAF-mutant: HR 0.28 (0.12–0.64)] and secondary end points, except for the RAS-mutant subgroup regarding overall survival. Adjustment for sidedness instead of primary tumour location yielded comparable results. Although right-sided tumours were associated with inferior prognosis, both patients with right- and left-sided tumours showed significant benefit from maintenance treatment. Conclusions CAP-B maintenance treatment after six cycles CAPOX-B is effective in first-line treatment of mCRC across all mutational subgroups. The benefit of maintenance treatment was most pronounced in patients with RAS/BRAF wild-type and V600EBRAF-mutant tumours. ClinicalTrials.gov number NCT00442637.

Rationale - Asthma is one of the most common chronic diseases in childhood, occurring in up to 10% of all children. Exercise induced bronchoconstriction (EIB) is indicative of uncontrolled asthma and can be assessed by means of an exercise challenge test (ECT). These tests however draw heavily on healthcare resources and require demanding repetitive forced breathing manoeuvres of children. In this study the electric activity of the respiratory muscles was measured as a tool to assess EIB. Methods - Children suspected of exercise induced respiratory symptoms performed an ECT wearing a portable EMG amplifier (Dipha-16, Demcon Macawi respiratory systems, Enschede, the Netherlands). EIB was defined as a fall in FEV1 of greater than 13%. Electrodes were placed bilaterally at the diaphragm and accessory breathing (intercostal-, sternocleidomastoid- and trapezoid) muscles. A single reference electrode was placed at the sternum. Children were asked to sit still for 30 seconds after each spirometry measurement to obtain EMG measurements. Data was pre-processed and analyzed in Matlab. Results - 20 Out of 43 children were diagnosed with EIB. Peak amplitude measured at the diaphragm increased significantly more in children with EIB than in children without EIB; 4.85uV (1.82 - 7.84), compared to 0.20uV (-0.10 - 0.54); (P < 0.001) at the point of maximal bronchoconstriction. Increases in EMG peak heights at the diaphragm can accurately distinguish between EIB and non-EIB (Sensitivity 95%, Specificity 91%, AUC 0.973). Increase in activity at the diaphragm is related to the decrease in pulmonary function (Pearson's R = 0:77; R2 = 0.58; P < 0.001). Accessory breathing muscles were often not measurable at baseline, therefore accurate assessment of the changes in activity in response to exercise could not be attained. Conclusion - These results imply that EMG measurements of the diaphragm can be used to accurately distinguish between EIB and non-EIB in children. Larger increases in peak amplitude suggest an increased work of breathing as is expected in children with EIB. Moreover, we found a relation between the decrease in pulmonary function and the increase of EMG peak activity. This technique provides opportunities to non-obtrusively measure bronchoconstriction when spirometry is not feasible or available and may be applied in the clinical setting, such as emergency medicine or in hospital- or home monitoring

Contains fulltext : 48454.pdf (Publisher’s version ) (Closed access) Interleukin-6 (IL-6) is a pleiotropic cytokine, signaling intracellularly via its unique membrane-bound receptor IL-6R and gp130. In peripheral nerve injury models, IL-6 and IL-6R are increased at the injured nerve and the respective dorsal root ganglion. IL-6 is increased at the ipsilateral dorsal and ventral horn of the spinal cord. IL-6 is known to affect neuronal survival, differentiation and regeneration. It is involved in synaptic plasticity and hyperexitability and induces the synthesis or release of other substances with known neuroprotective or neuromodulatory effects. In this study, intrathecal administration of recombinant rat IL-6 to rats with a chronic constriction injury of the sciatic nerve, induced a logarithmic dose-dependent increase in cold allodynia with a threshold of 10 pg IL-6 and a maximal effect at 100 ng IL-6. Intrathecal administration of saline or denaturated IL-6 was without effect. In rats with a chronic constriction injury, systemic administered IL-6 did not induce a hyperalgesic effect, illustrating that IL-6 acts at the level of the dorsal root ganglion or the spinal cord. Intraplantar injection of 100 ng IL-6 in the operated hind paw resulted in an increased cold allodynia. This study demonstrates that the sensitivity to exogenous intrathecal or peripheral IL-6 increases in rats with a mononeuropathy.

The stability of interleukin 6 (IL-6), its soluble receptor (sIL-6R), IL-10 and CC16 or uteroglobin (an endogenous cytokine inhibitor) in human serum was examined using an accelerated stability testing protocol according to the Arrhenius equation. Further, the effect of time delay between blood sampling and sample processing, clotting temperature and repeated freeze-thaw cycles on serum levels of these proteins were determined. Paired serum samples were stored at 4 degrees C, 20 degrees C, 30 degrees C and 40 degrees C for 1 to 21 days. We found that IL-6 and CC16 concentrations did not change at 4 degrees C, 20 degrees C and 30 degrees C. Interleukin-6 concentrations significantly declined after 11 days at 40 degrees C. The concentrations of sIL-6R and IL-10 did not change at 4 degrees C but significantly decreased at 20 degrees C (after 21 and 14 days respectively), 30 degrees C and 40 degrees C (after 1 day at both temperatures for both cytokines). Arrhenius-plots indicated that sIL-6R and IL-10 are stable for at least several years at -20 degrees C and -70 degrees C, respectively. Since their relative stability, no Arrhenius-plot could be calculated for IL-6 and CC16. The concentrations of the proteins examined were not significantly altered by repeated freeze-thaw cycles, nor by extended clotting times at 4 degrees C or 20 degrees C. We conclude that serum samples for the determination of IL-6, sIL-6R and CC16 can be stored at -20 degrees C for several years, but for IL-10 determinations, storage at -70 degrees C is recommended.

Background Prospective data on chemotherapy for elderly patients with metastatic breast cancer (MBC) remain scarce. We compared the efficacy and safety of first-line chemotherapy with pegylated liposomal doxorubicin (PLD) versus capecitabine in MBC patients aged ≥65 years in a multicentre, phase III trial. Patients and methods Patients were randomized to six cycles of PLD (45 mg/m2 every 4 weeks) or eight cycles of capecitabine (1000 mg/m2 twice daily, day 1–14 every 3 weeks). Results The study enrolled 78 of the planned 154 patients and was closed prematurely due to slow accrual and supply problems of PLD. Many included patients were aged ≥75 years (54%) and vulnerable (≥1 geriatric condition: 71%). The median dose intensity was 85% for PLD and 84% for capecitabine, respectively. In both arms, the majority of patients completed at least 12 weeks of treatment (PLD 73%; capecitabine 74%). After a median follow-up of 39 months, 77 patients had progressed and 62 patients had died of MBC. Median progression-free survival was 5.6 versus 7.7 months (P = 0.11) for PLD and capecitabine, respectively. Median overall survival was 13.8 months for PLD and 16.8 months for capecitabine (P = 0.59). Both treatments were feasible, grade 3 toxicities consisting of fatigue (both arms: 13%), hand–foot syndrome (PLD: 10%; capecitabine: 16%), stomatitis (PLD: 10%; capecitabine: 3%), exanthema (PLD: 5%) and diarrhoea (PLD: 3%; capecitabine: 5%). Only 1 of 10 patients aged ≥80 years completed chemotherapy, while 3 and 6 patients discontinued treatment due to toxicity or progressive disease, respectively. Conclusion Both PLD and capecitabine demonstrated comparable efficacy and acceptable tolerance as first-line single-agent chemotherapy in elderly patients with MBC, even in vulnerable patients or patients aged ≥75 years. We have measured the ratio of prompt production rates of the charmonium states {chi}{sub c1} and {chi}{sub c2} in 110 pb{sup -1} of p{ovr p} collisions at sqrt[s] = 1.8 TeV. The photon from their decay into J/{psi}{gamma} is reconstructed through conversion into e{sup +}e{sup -} pairs. The energy resolution this technique provides makes the resolution of the two states possible. We find the ratio of production cross sections {sigma}{sub {chi}{sub c2}}/{sigma}{sub {chi}{sub c1}} = 0.96{+-}0.27(stat){+-}0.11(syst) for events with p{sub {tau}}(J/{psi})>4.0 GeV/c, |{eta}(J/{psi})| 1.0 GeV/c.

An angular analysis of B{sup 0}{yields}J/{psi}K*{sup 0} and B{sup 0}{sub s}{yields}J/{psi}{phi} has been used to determine the decay amplitudes with parity-even longitudinal (A{sub 0}) and transverse (A{sub {parallel}}) polarization and parity-odd transverse (A{sub {perpendicular}}) polarization. The measurements are based on 190 B{sup 0} and 40 B{sup 0}{sub s} candidates obtained from 89 pb{sup -1} of p{ovr p} collisions at the Fermilab Tevatron. The longitudinal decay amplitude dominates with |A{sub 0}|{sup 2} = 0.59{+-}0.06{+-}0.01 for B{sup 0} and |A{sub 0}|{sup 2} = 0.61{+-}0.14{+-}0.02 for B{sup 0}{sub s} decays. The parity-odd amplitude is found to be small with |A{sub {perpendicular}}|{sup 2} = 0.13{sub -0.09}{sup +0.12}{+-}0.06 for B{sup 0} and |A{sub {perpendicular}}|{sup 2} = 0.23{+-}0.19{+-}0.04 for B{sup 0}{sub s} decays.

We report the results of a search for second and third generation leptoquarks using 88 pb{sup -1} of data recorded by the Collider Detector at Fermilab. Color triplet technipions, which play the role of scalar leptoquarks, are investigated due to their potential production in decays of strongly coupled color octet technirhos. Events with a signature of two heavy flavor jets and missing energy may indicate the decay of a second (third) generation leptoquark to a charm (bottom) quark and a neutrino. As the data are found to be consistent with standard model expectations, mass limits are determined.

We have searched for direct production of scalar top quarks at the Collider Detector at Fermilab in 88 pb{sup -1} of pp collisions at {radical}(s)=1.8 TeV . We assume the scalar top quark decays into either a bottom quark and a chargino or a bottom quark, a lepton, and a scalar neutrino. The event signature for both decay scenarios is a lepton, missing transverse energy, and at least two b -quark jets. For a chargino mass of 90 GeV/c{sup 2} and scalar neutrino masses of at least 40 GeV/c{sup 2} , we find no evidence for scalar top production and present upper limits on the production cross section in both decay scenarios. We report results from a study of events with a leading antiproton of beam momentum fraction 0.905 7 GeV. Using the dijet events, we evaluate the diffractive structure function of the antiproton and compare it with expectations based on results obtained in diffractive deep inelastic scattering experiments at the DESY ep collider HERA.

We report on a search for the flavor-changing neutral current decays B{sup +} {yields} {mu}{sup +}{mu}{sup -}K{sup +} and B{sup 0} {yields} {mu}{sup +}{mu}{sup -}K{sup *0} using 88 pb{sup -1} of data from p{ovr p} collisions at sqrt[s] = 1.8 TeV, collected with the Collider Detector at Fermilab. Finding no evidence for these decays, we set upper limits on the branching fractions B(B{sup +} {yields} {mu}{sup +}{mu}{sup -}K{sup +})

If the technicolor {omega}{sub T} particle exists, a likely decay mode is {omega}{sub T}{r_arrow}{gamma}{pi}{sub T} , followed by {pi}{sub T}{r_arrow}b{bar b} , yielding the signature {gamma}b{bar b} . We have searched 85 pb{sup {minus}1} of data collected by the CDF experiment at the Fermilab Tevatron for events with a photon and two jets, where one of the jets must contain a secondary vertex implying the presence of a b quark. We find no excess of events above standard model expectations. We express the result as an exclusion region in the M{sub {omega}{sub T}}-M{sub {pi}{sub T}} mass plane. {copyright} {ital 1999} {ital The American Physical Society }

We present a search for like-sign dielectron plus multijet events using $107{\mathrm{pb}}^{\ensuremath{-}1}$ of data in $p\overline{p}$ collisions at $\sqrt{s}\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}1.8\mathrm{TeV}$ collected in 1992--1995 by the CDF experiment. Finding no events that pass our selection, we set $\ensuremath{\sigma}\ifmmode\times\else\texttimes\fi{}\mathrm{BR}$ limits on two supersymmetric processes that can produce this experimental signature: gluino-gluino or squark-antisquark production with $R$-parity violating decays of the charm squark or lightest neutralino via a nonzero ${\ensuremath{\lambda}}_{121}^{\ensuremath{'}}$ coupling. We compare our results to the next-to-leading order calculations for gluino and squark production cross sections and set lower limits on $M(\stackrel{\ifmmode \tilde{}\else \~{}\fi{}}{g})$, $M({\stackrel{\ifmmode \tilde{}\else \~{}\fi{}}{t}}_{1})$, and $M(\stackrel{\ifmmode \tilde{}\else \~{}\fi{}}{q})$.

We have used $87{\mathrm{pb}}^{\ensuremath{-}1}$ of data collected with the collider detector at Fermilab to search for new particles decaying to $b\overline{b}$. We present model-independent upper limits on the cross section for narrow resonances which exclude the color-octet technirho in the mass interval $350lMl440\mathrm{GeV}{/c}^{2}$. In addition, we exclude top-gluons, predicted in models of top-color-assisted technicolor, of width $\ensuremath{\Gamma}\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}0.3M$ in the mass range $280lMl670\mathrm{GeV}{/c}^{2}$, of width $\ensuremath{\Gamma}\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}0.5M$ in the mass range $340lMl640\mathrm{GeV}{/c}^{2}$, and of width $\ensuremath{\Gamma}\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}0.7M$ in the mass range $375lMl560\mathrm{GeV}{/c}^{2}$.

Interleukin-6 (IL-6) plays a major role in hematopoiesis, immune functioning, and the acute phase response. In umbilical cord blood, this cytokine was thought to be a marker of neonatal defense to stress and infection, however, neonatal IL-6 production is immature. We speculated that a maternal influence exists on neonatal IL-6, at least during uncomplicated deliveries. Of the 81 healthy parturients included in this study, 51 delivered vaginally, 20 with and 31 without epidural analgesia, and 30 underwent elective cesarean section, 20 with epidural and 10 with general anesthesia. Maternal blood was sampled on hospital admission and just after delivery. Neonatal blood was collected from the umbilical cord. A significant positive correlation was found between neonatal cord blood interleukin-6 levels and maternal serum IL-6 levels on admission (r = 0.57, p

We present a measurement of the differential cross section d{sigma}/d{Sigma}E{sub T}{sup jet} for the production of multijet events in p{ovr p} collisions where the sum is over all jets with transverse energy E{sub T}{sup jet}>E{sub T}{sup min}. The measured cross section for events with {Sigma}E{sub T}{sup jet}>320 GeV is compared to O({alpha}{sub s}{sup 3}) perturbative QCD predictions and QCD parton shower Monte Carlo predictions. The agreement between the O({alpha}{sub s}{sup 3}) predicted and observed event rates is reasonable for E{sub T}{sup min} = 100 GeV, but poorer for E{sub T}{sup min} = 20 GeV.

We search for the flavor-changing neutral current decays of the top quark $t\ensuremath{\rightarrow}q\ensuremath{\gamma}$ and $t\ensuremath{\rightarrow}\mathrm{qZ}$ (here $q$ represents the $c$ and $u$ quarks) in $\overline{p}p$ collisions at $\sqrt{s}\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}1.8\phantom{\rule{0ex}{0ex}}\mathrm{TeV}$. We use a dataset $(\ensuremath{\int}L\mathrm{dt}\ensuremath{\sim}110{\mathrm{pb}}^{\ensuremath{-}1})$ collected during the 1992--1995 run of the Collider Detector at Fermilab. We set 95% confidence level limits on the branching fractions $B(t\ensuremath{\rightarrow}q\ensuremath{\gamma})l3.2%$ and $B(t\ensuremath{\rightarrow}\mathrm{qZ})l33%$, consistent with the standard model.

Background: Interleukin-6 is a pleiotropic cytokine with a wide range of physiological activities. It plays an important role in the immuno-neuro-humoral axis during stress and surgery. Methods: Serum interleukin-6 in parturients was measured on hospital admission, immediately after birth and 12 and 24 hours later. All parturients had uncomplicated pregnancies, and delivered vaginally without (n=31) or with (n=20) epidural analgesia, or underwent Caesarean section under epidural (n=20) or general (n=10) anaesthesia. Results: Serum interleukin-6 assayed immediately following Caesarean section was low, but peaked 12 hours later, irrespective of the anaesthetic technique or other foetomaternal characteristics. Patients who delivered vaginally showed the highest interleukin-6 levels immediately after delivery. These were positively correlated with serum interleukin-6 on admission and duration of labour. Serum interleukin-6 was significantly higher in parturients who had epidural analgesia, and was significantly lower in those receiving intravaginal prostaglandins compared to those without prostaglandins. Conclusion: The interleukin-6 response after Caesarean section can be explained by a generalized acute phase response to surgery, with no anaesthetic, maternal or neonatal interference. The rapid increase in peripartum serum interleukin-6 levels after vaginal delivery reflects, in part, cervical ripening or labour, their physiological triggers and psychological or physical stress. Regional anaesthesia, duration of labour and exogenous prostaglandin administration can modulate the peripartum interleukin-6 response and subsequently the physiological effects of this cytokine.

The lifetime of the ${B}_{s}^{0}$ meson is measured using the exclusive decay mode ${B}_{s}^{0}\ensuremath{\rightarrow}J/\ensuremath{\psi}\ensuremath{\varphi}$, where $J/\ensuremath{\psi}\ensuremath{\rightarrow}{\ensuremath{\mu}}^{+}{\ensuremath{\mu}}^{\ensuremath{-}}$ and $\ensuremath{\varphi}\ensuremath{\rightarrow}{K}^{+}{K}^{\ensuremath{-}}$. The data sample consists of $110{\mathrm{pb}}^{\ensuremath{-}1}$ of $p\overline{p}$ collisions at $\sqrt{s}\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}1.8\phantom{\rule{0ex}{0ex}}\mathrm{TeV}$, collected by the CDF detector at the Fermilab Tevatron collider during 1992--1995. We find $58\ifmmode\pm\else\textpm\fi{}12$ signal events, and the ${B}_{s}^{0}$ meson lifetime is determined to be ${\ensuremath{\tau}}_{{B}_{s}^{0}}{\phantom{\rule{0ex}{0ex}}=\phantom{\rule{0ex}{0ex}}1.34}_{\ensuremath{-}0.19}^{+0.23}(\mathrm{stat})\ifmmode\pm\else\textpm\fi{}0.05(\mathrm{syst})$ ps. This result is consistent with previous measurements of the ${B}_{s}^{0}$, ${B}^{+}$, and ${B}_{d}^{0}$ meson lifetimes and with theoretical predictions.

Despite its widespread occurrence and devastating effects, this complication is still not fully understood, and there is no treatment available to prevent its development.Methods: In this study, immunocytochemistry for activating transcription factor 3, a marker for cell injury, was used to investigate the stress response in dorsal root ganglion neurons in both in vitro and ex vivo models of diabetic neuropathy.Results: Our findings showed increased activating transcription factor 3 expression in hyperglycemic culture conditions and in dorsal root ganglion neurons isolated from diabetic rats. Glial cell line-derived neurotrophic factor, a substance with known neuroprotective properties, was able to reduce diabetes mellitus-induced neuronal stress in vitro, while gabapentin and carbamazepine, currently used to treat neuropathic pain, showed only limited effects.Conclusion: Growth factors may have a therapeutic benefit as neurotrophic agents in the treatment of diabetic peripheral neuropathy, but gabapentin and carbamazepine have no direct protective effect on sensory neurons. The lead-liquid argon sampling calorimeter of the SLD detector is one of the largest detectors employing cryogenic liquids now in operation. This paper details the design and performance considerations, the mechanical and cryogenic systems, the absorber design and tower segmentation, the data acquisition electronics, and the control systems of the detector. The initial operational performance of the device is discussed. Detailed resolution studies will be presented in a later paper.

A sensitive and specific radioimmunoassay was used to measure interleukin 8 (IL-8) in bronchoalveolar lavage fluids from control subjects, patients with the adult respiratory distress syndrome (ARDS) and patients undergoing coronary bypass surgery, a risk factor for developing ARDS. Concentrations of IL-8, albumin, total protein and numbers of neutrophils were higher in both patient groups than in controls. Levels of IL-8 were significantly correlated with the influx of neutrophils, plasma protein extravasation and with the PaO2/FiO2 ratio. These data suggest that IL-8 may mediate the recruitment of neutrophils from the vascular compartment into the alveolar space and may therefore be an important determinant in neutrophil-mediated lung injury. Since increased levels of IL-8 were also found in BAL fluid from patients at risk in whom ARDS did not develop, other factors are likely to be involved and IL-8, as well as other markers of inflammation, are of little prognostic use.

Symbiotic algae in coral species distributed over a large depth range are confronted with major differences in light conditions. We studied the genetic variation of Symbiodinium in the coral genus Madracis over depth (5-40 m) and at two different colony surface positions. Using polymerase chain reaction-denaturing gradient gel electrophoresis ITS2 nuclear ribosomal DNA analyses, we consistently identified three symbiont genotypes with distributions that reveal patterns of host specificity and depth-based zonation. ITS2 type B7 Symbiodinium is the generalist type, occurring in all zooxanthellate Madracis corals and at all depths. Type B13 is restricted to the shallow water specialist Madracis mirabilis. Type B15 is typical of deep reef environments and replaces B7 in the depth generalist Madracis pharensis. Contrasting with variation over depth, we found strong functional within-colony uniformity in symbiont diversity. Relating symbiont distributions to measured physical factors (irradiance, light spectral distribution, temperature), suggests depth-based ecological function and host specificity for Symbiodinium ITS2 types, even among closely related coral species.

Twelve days before the formalin testing, male Sprague Dawley rats underwent adrenalectomy or sham-adrenalectomy, and non-operated rats were used as reference. The number of flinches and the duration of licking or biting behavior were measured during the early and late phase. In reference and sham-operated rats, injection of formalin 5% resulted in a marked pain behavior in the early and late phase with significant increases in ACTH and corticosterone plasma levels. In adrenalectomized rats, pain behavior was decreased during both phases. Naloxone, administered before the late phase, did not alter pain behavior in sham or reference rats, whereas in adrenalectomized rats pain reactivity returned to those levels observed in reference rats. Beta-endorphin plasma levels above the detection limit were more frequently found in adrenalectomized rats. Thyrotropin and prolactin levels were not different between studied groups. We speculate that the observed reduced pain behavior in adrenalectomized rats after formalin, is the result of an increased production of pro-opiomelanocortin, the pro-drug of both adrenocorticotrophic hormone and beta-endorphin.

Item does not contain fulltext Surgery is followed by an acute-phase response, including hypothalamo-pituitary-adrenal (HPA)-axis activation and fever. Considering its physiological properties and its behaviour in plasma after stress and surgery, the pro-inflammatory cytokine interleukin (IL)-6 is a putative candidate in eliciting these stress-related symptoms. However, evidence for this hypothesis is lacking. Rats subjected to individual psychological stress for 1h were injected intraperitoneally with saline or 3.33 microg per 100g rat neutralizing antibodies against rat IL-6. Thereafter, the single-housed rats were anaesthetized for 25 min, with or without undergoing a laparotomy. Intermittently, oesophageal temperatures were measured at defined time points. A parallel group of rats undergoing the same study protocol were decapitated, at time points when body temperatures differed, to obtain blood for measurement of plasma adrenocorticotropic hormone and corticosterone. Individual housing resulted in hyperthermia. Antibodies against IL-6 accelerated normalization of body temperature after individualizing stress, limited postoperative hyperthermia after laparotomy, but accentuated hyperthermia after anaesthesia alone. Antibody administration was not able to significantly influence the plasma hormone levels during any experiment. The present study indicates that IL-6 is a thermoregulatory factor during psychological, anaesthesiological and surgical stress, but the cytokine does not participate in HPA-axis activation until 6h after anaesthesia or surgery. A dose-finding study with antibodies against IL-6 ought to further identify the degree of contribution of IL-6 to perioperative thermoregulation.

UNLABELLED We evaluated the efficacy of intraarticular sufentanil in the prevention of postoperative pain after day-case arthroscopic procedures. Sixty patients were randomly assigned to receive either intraarticular sufentanil, 5 or 10 microg, and saline IV, or intraarticular saline and sufentanil 5 microg IV (control). All study medication was administered in a double-blinded fashion. Postoperatively and the day after surgery, pain levels at rest and during movement (i.e., active flexion of the knee), measured by a visual analog scale, were significantly lower in the Sufentanil groups compared with the Control group. Moreover, intraarticular sufentanil significantly reduced the postoperative consumption of analgesics. The time until discharge from the postanesthesia care unit (assessed by the Aldrete score) was significantly shorter in the patients receiving sufentanil intraarticularly. There were no significant differences between the two Sufentanil groups either in the intensity of postoperative pain or in discharge times from the postanesthesia care unit. We conclude that intraarticular sufentanil in arthroscopic knee procedures is a simple, effective, safe and well-tolerated analgesic technique for outpatients undergoing arthroscopic procedures. Increasing the dose sufentanil from 5 to 10 microg intraarticularly offered no additional advantage. Intraarticular sufentanil (5-10 microg) administration improves postoperative management after day-case diagnostic arthroscopic knee procedures. We report the observations of the decays B{sup +}{yields}{psi}(2S)K{sup +} and B{sup 0}{yields}{psi}(2S)K*(892){sup 0} in p{ovr p} collisions at a center-of-mass energy of 1.8 TeV using a 110 pb{sup -1} data sample recorded by the Collider Detector at Fermilab. We also reconstruct the decays B{sup +}{yields}J/{psi}K{sup +} and B{sup 0}{yields}J/{psi}K*(892){sup 0} and measure the six ratios of branching fractions of these four decays. The relative branching-fraction results are shown to be consistent with phenomenological factorization calculations of hadronic B-meson decays. We use the world-average branching fraction B(B{sup +}{yields}J/{psi}K{sup +}) to derive B(B{sup +}{yields}{psi}(2S)K{sup +})=(0.56{+-}0.08{+-}0.10)x10{sup -3}, B(B{sup 0}{yields}{psi}(2S)K*(892){sup 0})=(0.92{+-}0.20{+-}0.16)x10{sup -3}, and B(B{sup 0}{yields}J/{psi}K*(892){sup 0})=(1.78{+-}0.14{+-}0.29)x10{sup -3}, where the first and second uncertainties are statistical and systematic, respectively.

We search for new long-lived particles which decay to Z{sup 0} bosons by looking for Z{sup 0}{yields}e{sup +}e{sup -} decays with displaced vertices. We find no evidence for parent particles of the Z{sup 0} with long lifetimes in 90 pb{sup -1} of data from the CDF experiment at Fermilab. We set a cross section limit as a function of the lifetime of the parent particle for both a generic Z{sup 0} parent and a fourth-generation, charge -1/3 quark that decays into Z{sup 0}b.

Postoperative epidural analgesia using mixtures of bupivacaine and opioids has become common practice following abdominal surgery. Side effects such as hypotension, motor block, respiratory depression, pruritus, and urinary retention are well known. Pressure sores occurring within the first 24 hours are extremely rare.Three parturients scheduled for cesarean delivery received for postoperative pain relief with a patient-controlled epidural analgesia, consisting of 0.11% bupivacaine, 2 micrograms/mL sufentanil, 3 micrograms/mL clonidine, and 1.25 micrograms/mL epinephrine.Within the first postoperative day pressure sores were observed at one heel. In one patient the coccygeal area was also affected. After discontinuation of the patient-controlled analgesia and local treatment, including application of heel pads, the pressure sores healed uneventfully.Pressure sores following postoperative epidural analgesia may occur even in young patients. Although bupivacaine may induce a motor block, its combination with other drugs in the analgesic mixture or other contributing factors may explain the occurrence of pressure sores. Prophylaxis and increased alertness should eliminate this complication.

Summary Two patients are described in whom fulminant haemoptysis occurred during extracorporeal circulation. The use of high frequency jet ventilation resulted in a dramatic decrease of blood loss from the tracheal tube, avoiding the need for more aggressive management.

In these processes, cytokines might play an important role.

Summary - Ptaquiloside has been discovered for the first time in milk samples 01 a bovine fed a dietary complement (6 kg fresh weighVday) of freshly-eut bracken fronds. This was equivalent to ca 216 mg/day pure ptaquiloside. The compound was found in a quantity of no less than 0.11 mg/I in milk. This was determined by the appearance of pterosin 6 - the decomposition product of ptaquiloside - in prepurified milk sampi es treated with milg 1I1kllii (pH 8.0 and 11.5). The lack of base treatment led to the absence of pterosin B. A mi!kj}rilpurlflcatlon protocol based on meth,anol-acetonitrile precipitations in succession, hexllne and dlchloromethane extractions, alkaline treatment, reextraction and silica gel microcolumn chromatography followed by reverse-phase HPLC was developed for the detection 01 ptaquiloside.

One hundred medical charts of patients of a psychogeriatric nursing home with 'possible' (senile) dementia of the Alzheimer type were reviewed. Eighty-seven patients had died and 13 were alive at the time of analysis. The aim of the study was to obtain epidemiological information and to investigate (co)morbidity and mortality of these patients. All the data of this group of nursing home patients appeared to have a wide range. The process of dementia started at a mean age of about 75 years and patients remained in their homes for more than five years before admission. The average time spent in the nursing home was 3.0 years. These patients had a shorter life expectancy, counted from the time of admission, compared with the Dutch population. At admission a large percentage of patients suffered from circulatory system disease, musculoskeletal and connective tissue disease and/or sensory deprivation. During the stay the most frequent diseases were urinary and respiratory tract infections, constipation and chronic ulcers of the skin. Symptoms specific of Alzheimer's disease also occurred: feeding problems, disorders of gait and neurological symptoms such as paratonia. A large percentage of patients showed adverse effects of drugs or suffered from the consequences of trauma. Patients died of cachexia or bronchopneumonia. It is unclear whether the population studied is representative of the total population of Alzheimer patients.
