International audience; The results of a rigorous study of the two first pure rotational transitions of CO perturbed by Ar are presented. The experimental part is based on the use of three different spectrometers covering together the pressure range from 0.02 up to 1500 torr. The measurement results of collisional line shape parameters are supported by fully ab initio calculations, which are in remarkable agreement with retrieved data. A sub-percent uncertainty of line intensity measurements is achieved and the first firm evidence that the resonance spectrum of CO is observed on the continual pedestal is given. We analyze the results of our ab initio calculations on the basis of early analytical theories and demonstrate a good general applicability of the latter to the CO-Ar collisional system.

3. Clinical evaluation of General Electric new Swiftscan solution in bone scintigraphy on NaI-camera: A head to head comparison with Siemens Symbia.

PurposeThe General Electric (GE) Swiftscan solution combines a new Low Energy High Resolution and Sensitivity collimator (LEHRS) with image processing (Clarity 2D) and tomographic step and shoot continuous mode. The aim of this study was to compare clinical and physical performances of this new technology in bone scintigraphy.MethodsPhysical phantom measurements were performed using GE LEHRS, GE Low Energy High Resolution (LEHR) and Siemens LEHR collimators. These measurements were associated with a prospective clinical study. Sixty-seven patients referred for bone scintigraphy were enrolled from February to July 2018. Each patient underwent two acquisitions consecutively on GE and Siemens gamma camera, using respectively Swiftscan solution and LEHR collimator.ResultsOn planar acquisitions, maximum sensitivity was 100 cts/MBq for Siemens LEHR. GE SwiftScan LEHRS and GE LEHR maximum sensitivity were respectively 9% and 22% lower. Using Clarity 2D, GE Swiftscan LEHRS spatial resolution was the best with 9.2 mm versus 10.1 mm and 10.6 mm for GE LEHR and Siemens LEHR collimators. In tomographic mode, the sensitivity of GE Swiftscan solution was superior to both LEHR systems (16% and 25% respectively for Siemens and GE). There was no significant difference in spatial resolution. In clinical use, signal was higher on Siemens system and noise was lower on GE Swiftscan solution. Contrast-to-noise ratios were not significantly different between the two systems. There was a significant image quality improvement with GE SwiftScan in planar images and in whole body scan. No significant difference in image quality was observed on SPECT images.ConclusionNew GE SwiftScan collimator design improved sensitivity compared to "classical" GE LEHR collimator without compromising resolution. GE SwiftScan solution enhances planar image quality with a better Clarity 2D resolution recovery and noise treatment. In SPECT mode, GE SwiftScan solution improves volumetric sensitivity without significant impact on image quality, and could lead to time or dose reduction.

Information about molecular collisions is encoded in the shapes of collision-perturbed molecular resonances. This connection between molecular interactions and line shapes is most clearly seen in simple systems, such as the molecular hydrogen perturbed by a noble gas atom. We study the H2–Ar system by means of highly accurate absorption spectroscopy and ab initio calculations. On the one hand, we use the cavity-ring-down-spectroscopy technique to record the shapes of the S(1) 3-0 line of molecular hydrogen perturbed by argon. On the other hand, we simulate the shapes of this line using ab initio quantum-scattering calculations performed on our accurate H2–Ar potential energy surface (PES). In order to validate the PES and the methodology of quantum-scattering calculations separately from the model of velocity-changing collisions, we measured the spectra in experimental conditions in which the influence of the latter is relatively minor. In these conditions, our theoretical collision-perturbed line shapes reproduce the raw experimental spectra at the percent level. However, the collisional shift, δ0, differs from the experimental value by 20%. Compared to other line-shape parameters, collisional shift displays much higher sensitivity to various technical aspects of the computational methodology. We identify the contributors to this large error and find the inaccuracies of the PES to be the dominant factor. With regard to the quantum scattering methodology, we demonstrate that treating the centrifugal distortion in a simple, approximate manner is sufficient to obtain the percent-level accuracy of collisional spectra.

International audience; ObjectifsLa voie robot-assistée a récemment été décrite pour l’implantation du sphincter artificiel urinaire (SAU) chez la femme pour tenter de diminuer la morbidité périopératoire. L’objectif de cette étude était de rapporter les résultats périopératoires et fonctionnelles de la première série multicentrique internationale de sphincter artificiel urinaire robot-assistée.MéthodesToutes les patientes ayant eu une implantation robot-assistée d’un SAU en utilisant une technique standardisée (voie antérieure) entre 2013 et 2020 dans quinze institutions européennes ont été incluses dans une étude rétrospective. L’indication du SAU était une incontinence urinaire d’effort par insuffisance sphinctérienne définie comme la combinaison d’une faible pression de clôture urétrale et un urètre peu mobile/figé. Le principal critère d’évaluation était le résultat fonctionnel classé comme sec (aucune protection), amélioré ou inchangé.RésultatsCent vingt-cinq patientes ont été incluses. Les caractéristiques des patientes sont résumées dans le Tableau 1. Il y a eu 20 complications peropératoires (16 %) : 12 plaies du col vésical et huit plaie vaginales. Dans un cas de plaies multiples du col vésical, l’intervention a été interrompue mais elle a été poursuivie dans tous les autres cas. Vingt-deux patientes ont eu une complication postopératoire (17,6 %) mais seulement quatre étaient Clavien ≥ 3 (3,2 %) : trois explantations précoces (2 pour érosion vaginale et une pour infection) et une réintervention pour fistule vésicale. Après un suivi moyen de 13 mois, il y a eu 7 explantations (4,1 %) : les 3 précoces rapportées ci-dessus, une pour rétention chronique d’urine et trois pour érosions tardives (2 vaginales et une urétrale). Au dernier suivi, 101 patientes étaient sèches (80,8 %), 11 étaient améliorées (8,8 %) et 13 étaient inchangées (10,4 %).ConclusionCette série est la première série multicentrique internationale évaluant les Résultats de l’implantation du SAU chez la femme par voie robot-assistée selon une technique standardisée. Cette étude confirme les résultats périopératoires et fonctionnels prometteurs rapportés dans les précédentes séries.

We present an avatar and task-oriented dialog agent for monitoring user discomfort during a virtual reality (VR) cognitive exercise and providing personalized information and advice on its relief. The goal of this approach is to provide instantaneous assistance to users for a more comfortable VR experience, thereby enabling them to spend more time on cognitive tasks. We developed an avatar in a VR environment with which users may communicate verbally, and a dialog agent in a machine-learning based conversational AI platform. We performed a technical evaluation of the natural language understanding (NLU) component by comparing 2 models (BERT and StarSpace) using a train-test split, showing a significant benefit of BERT with smaller data sets. We validated the turn prediction using a train-test split and using randomly generated conversations. Both validations showed acceptable conversation-level accuracy. We undertook a usability study at two sites, showing effectiveness at both and good acceptability at one of the two. The framework outlined can be used to develop other virtual agents for cognitive self-care. Suggested improvements include validating the avatar with integrated BERT and reducing reliance on data augmentation, offline voice interaction modules, improved UX design, clinically validating the effect of the dialog agent on user discomfort and on cognitive performance, and increasing the ubiquity of the avatar within the VR cognitive care environment., 2020 IEEE 33rd International Symposium on Computer-Based Medical Systems (CBMS), July 28-30, 2020, Rochester, MN, USA

Among the widely available information technologies for remote health care, emergency care, and more specifically self-care in urgent situations (through automated, protocol-based intervention), has not been considered to date. Urgent care is particularly difficult due to the individual transient and dynamic patient context; the design-for-all approach currently being implemented does not consider the varying manifestation of physical states and cognitive disabilities. Therefore, the objective was to design and integrate into a remote health care platform, an intelligent user interface scheme that adapts in real time to the clinically urgent patient condition. The current study presents a mobile implementation, targeting diabetic hypoglycemia treatment. Continuous physiological data from medical-grade wearable sensors, and results from customized cognitive exercises evaluating the corresponding common emergency symptoms are used to define the patient state. This state feeds the adaptation rules and determines the most appropriate interaction interface delivering the intervention instructions. The designed adaptations prioritize the incorporation of multimodal interfaces (including voice and facial recognition and interactions), with the goal of augmenting patient-technology information exchange, thus providing more efficient and effective treatment delivery. This application is being validated with clinical partners in endocrinology.

- Abstract
La recherche biomédicale est définie par le Code de la santé publique comme « une recherche organisée et pratiquée sur l’être humain en vue du développement des connaissances biologiques et médicales ». Plusieurs étapes historiques depuis la fin du XXe siècle ont permis d’établir les grands principes de l’éthique dans l’expérimentation humaine avec des textes internationaux (déclaration d’Helsinki), européens (Directives et règlements européens) et nationaux. La réglementation en vigueur résulte de plusieurs textes de loi portant sur la protection de la personne humaine dans le cadre de la recherche médicale et l’utilisation des données personnelles et des produits issus de la personne. La recherche clinique du Service de santé des armées a pour but d’améliorer la santé de la communauté de Défense. L’article décrit les étapes incontournables de la conduite d’une recherche biomédicale sur la personne humaine à destination des praticiens du Service de santé des armées, en tenant compte des bonnes pratiques cliniques et des spécificités du Service.

Resume But Evaluer l’accessibilite previsible au post-internat pour les internes d’urologie terminant leur internat entre novembre 2013 et 2016. Materiel De septembre a novembre 2013, les representants des internes d’urologie inscrits en DESC dans chaque region ont ete contactes pour participer a l’etude. Un questionnaire leur a ete soumis afin de recenser localement le nombre d’internes prevoyant de terminer leur internat entre novembre 2013 et 2016, ainsi que le nombre de postes de chef de clinique-assistant (CCA) et d’assistant specialiste (AS) d’urologie permanents et provisoires dans la region, et leur occupation prevue durant la meme periode. Resultats En novembre 2013, notre etude a recense 334 urologues en formation (197 internes inscrits en DESC, 81 CCA, 56 AS). Cinquante-cinq internes ont termine leur internat en novembre 2013, alors que 67, 50, 77 internes ont prevu de terminer leur formation respectivement de novembre 2014 a 2016. L’accessibilite previsible au post-internat d’urologie a ete evaluee a 96,4 %, 82,1 %, 90,0 %, 74,0 % respectivement pour les internes terminant leur formation de novembre 2013 a 2016. Le deficit previsible de postes de post-internat a ete calcule a –2, –12, –5, –20 postes respectivement de novembre 2013 a 2016. Conclusion Le nombre previsible de postes de post-internat d’urologie reste insuffisant en France pour la periode 2013–2016. En raison d’un flux non stabilise d’internes prevoyant de terminer leur internat de novembre 2014 a 2016, leur accessibilite immediate a un post-internat a ete mesuree a 82,1 %, 90,0 %, 74,0 % respectivement sur la meme periode. Resume Introduction L’objectif de ce travail de l’AFUF etait d’etablir un etat des lieux des remunerations des gardes et astreintes des internes, assistants et chefs de clinique d’urologie en France. Materiel et methodes Les donnees ont ete recueillies de fevrier a mai 2013 par l’intermediaire d’un questionnaire envoye a tous les membres de l’AFUF (327 membres). Les remunerations etaient donnees en valeurs brutes. Resultats Quarante-trois internes ont participe a l’etude, 16 assistants et 16 chefs de clinique, permettant d’obtenir une representativite nationale de 62 % (54 hopitaux sur les 92 centres pratiquant l’urologie en France). La majorite des repondants etait d’astreinte operationnelle ou de securite. Vingt hopitaux (38 %) avaient une activite de prelevements multi-organes (PMO). Les remunerations medianes des internes etaient de 59,51 € par astreinte deplacee (demi-garde) et de 119,02 € par garde. Les assistants et chefs de clinique etaient remuneres de maniere forfaitisee pour 37,5 % d’entre eux (140 € pour les assistants [avec des variabilites de 40 a 195 €] et 130 € pour les chefs de clinique [42,5–180]) ou selon un tarif qui dependait du nombre ou du temps de deplacement pour les autres (62,5 %) : les deux premiers deplacements ou un deplacement e ou au-dela de 3 h et 365 et 473 € respectivement au-dela de 8 h. Les PMO etaient egalement remuneres de maniere forfaitisee (60 %) ou au tarif horaire (40 %). Au-dela d’un seuil de 2–3 heures, la remuneration horaire etait plus interessante que la remuneration forfaitisee. Conclusion Des disparites de remuneration des astreintes et gardes d’urologie ont ete mises en evidence. Les plus grandes variabilites concernaient la remuneration des astreintes forfaitisees au-dela d’un certain seuil d’heures deplacees et les astreintes de PMO des assistants et chefs de clinique. Ces differences meritent d’etre reconsiderees en vue d’une harmonisation nationale. Niveau de preuve 3.

Les resultats sont presentes sous forme de mediane (ecart interquartile). Resultats Soixante reponses sur 115 etudiants ayant fait un Master 2 (52 %) ont ete analysees. L’âge median etait de 29 ans (28–30) lors de la realisation du Master 2 et la proportion masculine de 75 %. La bourse de l’AFU etait obtenue par 57,4 % des postulants, 56,4 % pour les diverses bourses et 47,6 % pour la bourse annee recherche. Le montant median brut annuel etait de 29 870 € (22 710–30 195) sans difference significative entre les subdivisions. Les complements financiers provenaient essentiellement de gardes (26,2 %), d’astreintes (28,6 %) et de remplacements (25 %). L’annee de Master 2 etait le plus souvent realisee entre la quatrieme et la cinquieme annee d’internat (53 %), pour une duree d’un an (96,7 %) et en France (86,6 %). La cancerologie urologique (60 %) etait la thematique la plus etudiee. Le Master 2 etait realise dans un projet de carriere hospitalo-universitaire (31,7 %) ou dans l’attente d’un post-internat (20,8 %). Environ un quart se voyait proposer un poste de CCA avant le Master 2. Cette annee etait source de publication pour 76,7 %. La these de Sciences interessait 63,3 % des repondants. Conclusion Cette etude confirmait l’interet des jeunes urologues francais pour une annee de Master 2. Le financement grâce aux bourses facilitait le deroulement de cette annee dans l’optique d’une carriere universitaire pour nombre d’entre eux. Niveau de preuve Niveau 3.

19. Évaluation de l’utilisation de la simulation dans la formation des jeunes urologues français : une enquête de l’association française des urologues en formation (AFUF)

Resume Objectifs Les objectifs de cette etude etaient d’evaluer la place de la simulation dans la formation initiale des jeunes urologues francais. Materiels et methodes Un questionnaire electronique a ete diffuse a l’ensemble des internes et chefs de clinique, membres de l’AFUF entre fevrier et mai 2013. Les resultats sont presentes sous forme de mediane (ecart interquartile). Resultats Les reponses de 125 urologues en formation ont ete analysees (taux de reponse 38 %). Il s’agissait d’internes dans 90 cas (72 %), et de chefs de clinique ou assistants dans 35 cas (28 %). L’âge median etait de 29 ans (27–30), la proportion masculine de 77 %. Tous les CHU etaient representes. Quatre-vingt-dix (72 %) d’entre eux avaient acces a un pelvi-trainer et 66 (53 %) a des modeles animaux ou cadaveriques, mais ne l’utilisaient jamais ou moins d’une fois par mois dans 83 et 97 % des cas, respectivement. Soixante-douze (58 %) avaient utilise au moins une fois un simulateur utilisant la realite virtuelle et 38 (30 %) y avaient acces regulierement, mais sans encadrement dans 64 % des cas. Les facteurs limitant l’entrainement sur simulateur etaient l’absence de simulateur a disposition (70 %), le manque de temps (58 %), d’incitation (34 %) et d’encadrement (20 %). Si ces conditions etaient reunies, 86 % des jeunes urologues seraient prets a consacrer plus d’une heure par semaine a l’entrainement sur simulateur. Conclusion Cette etude a revele parmi l’echantillon interroge une sous-utilisation de la simulation dans la formation pratique. Cela etait explique par un manque d’accessibilite, mais surtout d’utilisation des moyens a disposition. Niveau preuve 3

Introduction L’eosinophilie est un motif frequent de consultation. Les etiologies sont dominees par les causes allergiques et medicamenteuses, neoplasiques et parasitaires. Lorsqu’elle est importante et durable, elle peut etre symptomatique mais parfois les signes sont quasi inexistants. Cela ne dispense pas d’explorations afin de prevenir la survenue de signes systemiques. Observation Un patient de 38 ans aux antecedents de colique nephretique consulte au service d’accueil des urgences pour douleur spontanee de l’hallux gauche d’horaire inflammatoire. La biologie de routine montre une eosinophilie a 11 G/L sans anomalie des autres lignees. Il n’y a pas d’alteration de l’etat general. Il a une toux chronique non productive invalidante. L’examen clinique est sans anomalie particuliere. Il est convoque pour des explorations. Dans l’intervalle, il a des douleurs du mollet gauche, sans phlebite objectivee. Le patient n’a jamais quitte la France. La NFS montre : hemoglobine 16 g/dL, plaquettes a 181 G/l, leucocytes 18,15 G/L, polynucleaires neutrophiles 2,05 G/L, polynucleaires eosinophiles 11 G/L, lymphocytes 3,65 G/L. Il n’a pas de syndrome inflammatoire. Les serologies toxocarose, trichinellose sont negatives. Le sediment urinaire ne montre pas de proteinurie ni d’hematurie, pas d’eosinophilie. Les ANCA, de meme que les anticorps antinucleaires et le facteur rhumatoide sont negatifs, le complement est normal. Les IgE totales sont negatives. La recherche du transcrit FIP1L1 PDGFRA (4q) par RT PCT revient positive. Sous IMATINIB, il est note une remission clinique avec disparition de la toux, de la douleur de l’hallux et des douleurs du mollet gauche ainsi qu’une diminution de l’eosinophilie et la negativation de la recherche du transcrit. Conclusion L’hypereosinophilie essentielle FIP1L1/PDGFRA correspond a une deletion 4q12 responsable de la fusion entre les genes FIP1L1 et PDGFRA. La proteine chimerique resultant de cette fusion a une activite tyrosine kinase constitutive, ce qui explique les remissions cliniques et cytologiques des hypereosinophilies essentielles traitees par un inhibiteur specifique de tyrosine kinase, l’IMATINIB. Dans ce cas, les manifestations cliniques retrouvees sont dominees par des atteintes musculosquelettiques (douleur de hallux d’horaire inflammatoire, douleur du mollet sans phlebite objectivee), que l’on peut attribuer a l’eosinophilie, puisque regressives a 3 mois de l’initiation du traitement. Dans la majorite des etudes sur l’eosinophilie, cette atteinte n’est pas mentionnee. Elle est decrite neanmoins, dans la maladie de Shulman. Dans cette derniere, des myalgies spontanees ou provoquees sont rapportees a la phase initiale chez pres de deux patients sur trois et jusqu’a 86 % des patients au moment du diagnostic. Ce cas nous amene a rechercher les atteintes musculosquelettiques dans les syndromes hypereosinophilique, non uniquement dans la maladie de Shulman.

BACKGROUND:A virtual reality (VR) neurosurgical simulator with haptic feedback may provide the best model for training and perfecting surgical techniques for transsphenoidal approaches to the sella turcica and cranial base. Currently there are 2 commercially available simulators: NeuroTouch (Cranio and Endo) developed by the National Research Council of Canada in collaboration with surgeons at teaching hospitals in Canada, and the Immersive Touch. Work in progress on other simulators at additional institutions is currently unpublished.OBJECTIVE:This article describes a newly developed application of the NeuroTouch simulator that facilitates the performance and assessment of technical skills for endoscopic endonasal transsphenoidal surgical procedures as well as plans for collecting metrics during its early use.METHODS:The main components of the NeuroTouch-Endo VR neurosurgical simulator are a stereovision system, bimanual haptic tool manipulators, and high-end computers. The software engine continues to evolve, allowing additional surgical tasks to be performed in the VR environment. Device utility for efficient practice and performance metrics continue to be developed by its originators in collaboration with neurosurgeons at several teaching hospitals in the United States. Training tasks are being developed for teaching 1- and 2-nostril endonasal transsphenoidal approaches. Practice sessions benefit from anatomic labeling of normal structures along the surgical approach and inclusion (for avoidance) of critical structures, such as the internal carotid arteries and optic nerves.CONCLUSION:The simulation software for NeuroTouch-Endo VR simulation of transsphenoidal surgery provides an opportunity for beta testing, validation, and evaluation of performance metrics for use in neurosurgical residency training.

A nonlinear extended finite element (XFEM) modeling framework under a stochastic cohesive zone is presented for realistic prediction of delamination in polyphenylene sulfide (PPS)/glass composites in mode I of fracture. The cohesive zone model adopts damage evolution of the material based on a bilinear traction-separation law, the critical energy release rate and the J-integral method to formulate the delamination interface under stochastic fracture properties. To demonstrate the application of the approach, numerical predictions are compared to experimental data using Double Cantilever Beam (DCB) tests. In particular, it is shown how the XFEM model can be used to capture test non-repeatability due to uncertain fracture properties, which is often the case during the characterization of composites using standard fracture tests. © 2013 Springer Science+Business Media Dordrecht.

23. Impact of laparoscopic sacrocolpopexy on symptoms, health-related quality of life and sexuality: a medium-term analysis

Objective To evaluate the impact of laparoscopic sacrocolpopexy on symptoms, health-related quality of life (HRQL) and sexuality among women with symptomatic urogenital prolapse (UGP). Patients and Methods A prospective analysis was carried out including 148 women with symptomatic UGP. Baseline characteristics, medical and obstetric history were recorded. The Pelvic Organ Prolapse Quantification (POP-Q) classification was used to stage the UGP. Validated tools were used to evaluate symptoms (Pelvic Floor Distress Inventory, PFDI-20) and HRQL (Pelvic Floor Impact Questionnaire, PFIQ-7). Sexual function was evaluated using the Pelvic organ prolapse urinary Incontinence Sexual Questionnaire (PISQ-12). Measurements were recorded at the preoperative examination, then at 3 and 12 months after surgery. We compared the follow-up results with preoperative data. Results The anatomical results at 3 months showed a significant correction (P < 0.05) relative to the preoperative values, on the three pelvic floor parameters measured, with a clinical relapse rate of 6.3%. This improvement remained significant after 12 months (P < 0.05). There was no difference between the results obtained at 3 months and those at 12 months. At 3 months compared with the preoperative data, there was a significant improvement in PFDI-20 total mean score (32.24 vs 94.31, P < 0.05). At 12 months, the improvement remained significant (38.06 vs 94.31, P < 0.05) for all scores compared with the preoperative scores. Again, there was no difference between results at 3 months, and those at 12 months. The results showed a significant improvement in the PFIQ-7 score at 3 (16.61 vs 64.04, P < 0.05) and 12 months (18.21 vs 64.04, P < 0.05). There was no significant difference between the scores at 3 months and those at 12months. The total PISQ-12 score was linked significantly to urinary symptoms (P < 0.05), pelvic symptoms (P < 0.05) but not with ano-rectal ones. At 3 months, the total mean PISQ-12 score had improved significantly compared with the preoperative score (35.42 vs 32.07, P < 0.05). At this time, only two items of the PISQ-12 questionnaire were significantly increased: ‘existence of negative emotions during sexual activity’ (P < 0.05) and ‘the avoidance of sexual activity because of prolapse’ (P < 0.05). The total mean score remained significantly improved at 12 months (36.56 vs 32.07, P < 0.05) and there was no statistical difference compared with the results at 3 months. Conclusions Laparoscopic sacrocolpopexy resulted in the early improvement (primarily during the first 3 months) of all symptoms, HRQL and sexual function. This improvement was persistent in the medium term. Symptoms linked with UGP had different effects on sexuality fields. Anatomical improvement was not related to an improvement in all sexual fields.

Resume But Le but de notre travail a ete de determiner precisement le lien existant entre les differentes caracteristiques d’un prolapsus uro-genital (PUG) et la sexualite, a l’aide d’outils valides. Patientes Etude prospective (148 patientes) avec evaluation du stade du prolapsus (classification Pelvic Organ Prolapse Quantification [POPQ]), questionnaire Pelvic Floor Distress Inventory-20 (PFDI) pour la severite de la symptomatologie et pelvic organ prolapse urinaire incontinence sexuel Questionnaire-12 (PISQ) pour la fonction sexuelle. Resultats Aucune relation n’a ete demontree entre le degre clinique du prolapsus et la fonction sexuelle globale ( p = 0,24). Les symptomes urinaires et pelviens etaient associes de maniere significative a une baisse de la qualite de vie sexuelle ( p = 0,04 et p = 0,02). L’importance des symptomes ano-rectaux etait statistiquement associes a une diminution de la satisfaction sexuelle ( p ≤ 0,05) ainsi qu’a l’existence d’une ejaculation prematuree chez le partenaire ( p ≤ 0,05). Il existait un lien significatif entre la severite des symptomes urinaires et l’existence de dyspareunies ( p ≤ 0,01), un evitement des activites sexuelles ( p ≤ 0,05), l’apparition d’emotions negatives ( p ≤ 0,01) ainsi qu’avec une diminution de l’excitation sexuelle ( p ≤ 0,05). La severite des symptomes pelviens etaient correles a un evitement des activites sexuelles ( p ≤ 0,01), a l’existence de dyspareunies ( p ≤ 0,05) ainsi qu’a l’alteration qualitative de l’orgasme ( p ≤ 0,05). Conclusion La severite clinique d’un prolapsus n’etait pas associee a une alteration de la sexualite. En revanche, la severite des symptomes urinaires, pelviens et ano-rectaux coexistant avec le prolapsus etaient associee a une alteration de la sexualite du couple.

Objectifs La chirurgie robotique par son environnement de travail complexe necessite un ensemble de competences non techniques (CNT) pour la bonne gestion du travail d’equipe : communication et organisation de l’environnement sont indispensables. Nous avons voulu etudier la relation entre la maitrise des CNT et la survenue d’evenements porteurs de risques (EPR) (evenement non attendu non directement lie au patient) pendant une intervention afin d’etablir un guide de bonnes pratiques et des objectifs de formation. Methodes Il s’agit d’une etude observationnelle multicentrique (5 centres) de septembre 2016 a juin 2017. Trois types de procedures robotiques etaient filmees et observees par un expert en CNT, issu de l’aviation militaire. Il etablissait un score CNT cree selon des scores valides qui evalue 2 dimensions : environnement (ergonomie, organisation, perturbateurs) et communication (conscience de la situation, prise de decision, cooperation, leadership) entre le chirurgien et l’equipe. Chaque item etait cote de 1 a 5. Les equipes realisaient une auto-evaluation sur leur travail avec le meme score. Les criteres de jugements etaient le nombre d’EPR (exemple : erreur de comprehension, conflits avec les bras du robot, erreur de materiel) et la duree de l’intervention. Resultats Les scores de CNT pouvaient varier de 7 (note la moins bonne) a 35. Les resultats descriptifs de l’ensemble des interventions seront presentes pour confirmer notre hypothese : – type d’intervention : nombre d’EPR par intervention et duree d’intervention ; – evaluation de 2 dimensions : l’environnement et la communication, leur relation, ainsi que le score composite CNT ; – les relations entre les criteres de jugements et le score de CNT ainsi que leurs 2 dimensions respectives ; – les concordances entre le score CNT et l’auto-evaluation par l’equipe ( Tableau 1 ). Conclusion L’environnement et la communication sont etroitement lies et permettent quand les deux sont bien utilisees d’ameliorer la qualite et la performance d’une intervention. Une maitrise des CNT permet de diminuer la survenue d’EPR et a un impact significatif sur la gestion des risques. Une formation specifique pourrait etre integree a l’apprentissage de la chirurgie robotique.

27. À propos d’un cas d’hypercalcémie maligne en TEP/TDM au 18F-FDG

Introduction Le carcinome parathyroidien represente moins d’1 % des cas d’hyperparathyroidie primaire. La morbidite est essentiellement liee aux complications de l’hypercalcemie chronique ou aigue. Dans de rares cas, une origine genetique liee a une mutation du gene HRPT2 est retrouvee. Materiels et methodes Nous presentons le dossier d’une patiente de 40 ans, orientee pour une hypercalcemie maligne. Elle presente pour antecedent majeur un carcinome parathyroidien droit sur mutation HRPT2, ayant beneficie d’un traitement chirurgical en 2005, mais ayant recidive a deux reprises au cours des 10 dernieres annees, necessitant un complement de traitement par chirurgie et radiotherapie cervicale. Le bilan biologique retrouve une calcemie corrigee a 5,31 mmol/L, une PTH a 3173 pg/mL (norme Resultats La TEP/TDM ne retrouve pas d’aspect de recidive cervicale, mais montre un hypermetabolisme diffus du pancreas, de multiples calcifications viscerales, notamment du pancreas, un uterus fibromateux, une masse latero-uterine droite hypometabolique, des lacunes osseuses hypermetaboliques des cotes, des femurs, des tibias, de l’hemi-bassin droit et de la mandibule, evoquant des tumeurs brunes, ainsi qu’un rehaussement diffus des corticales des os longs. La calcemie corrigee est stabilisee aux alentours de 3 mmol/L grâce a un traitement medicamenteux par Cinacalcet. Conclusion Cette observation rapporte en TEP/TDM au 18F-FDG des complications de l’hyperparathyroidie chronique, rares pour certaines, comme les tumeurs brunes et la pancreatite aigue. Elle decrit egalement les anomalies retrouvees dans le syndrome « hyperparathyroidie–tumeur de la mâchoire », lie a la mutation HRPT2, qui associe chez un meme individu un adenome ou un carcinome parathyroidien, des tumeurs de la mâchoire, des tumeurs uterines et des lesions kystiques ou tumorales renales.

The aim of this study was to produce intrinsically and uniformly doubly (15)N-(13)C-labeled proteins. These proteins can be used as intrinsic tracers of dietary amino acids, both α-amino groups and carbon skeletons, during postprandial metabolic utilization. Two (Rhodes) laying hens were fed for 16 days with a standard poultry diet supplemented with 0, 0.2% or 0.4% of a mixture of 20 doubly (15)N-(13)C-labeled AAs. A third hen was given a non-enriched diet, as the control. The eggs laid were collected over 24 days, from 3 days before to 4 days after supplementation. The (15)N and (13)C enrichments in proteins from white and yolk were measured by EA-IRMS and GC-C-IRMS for enrichment in individual amino acids. After 10 days of supplementation, the (15)N enrichment reached an isotopic plateau at 1500 to 3000 ‰, depending on the supplementation level, in both white and yolk while the (13)C enrichment was 220 to 650 ‰ in white and was 100 to 250 ‰ in yolk. The (15)N enrichment was similar among the amino acids, except for the aromatic ones in which the enrichment was lower. The δ(13)C values were variable among amino acids in both white and yolk, ranging from 77 ‰ for tyrosine to 555 ‰ for proline with the 0.2 % supplementation level. In conclusion, the incorporation of 0.2 % labeled amino acids in the hen diet allowed us to achieve sufficient enrichment for metabolic studies. However, due to the non-homogeneity of the (13)C labeling, adequate (13)C enrichment of individual amino acids must be considered depending on the investigated metabolic pathway.

Resume L’objectif de ce travail est d’exposer la prise en charge diagnostique et therapeutique de la sclerose de l’anastomose uretrovesicale apres prostatectomie totale pour un cancer de la prostate localise. C’est dans la proximite anatomique entre le sphincter strie de l’uretre et la zone de sclerose que reside l’enjeu de cette complication qui peut alterer significativement la qualite de vie malgre un controle carcinologique de la maladie. La sclerose de l’anastomose peut se manifester par des troubles mictionnels : dysurie, urgenturie, mauvaise vidange vesicale. Elle peut etre a l’origine d’une incontinence urinaire par sclerose du sphincter strie. Une debitmetrie, une fibroscopie et une cystographie retrograde permettent le diagnostic. La gestion therapeutique doit etre gradee en fonction de la severite et des objectifs en termes de qualite de vie du patient. L’uretrotomie interne a la lame froide entretenue par des calibrages reguliers est le traitement habituellement retenu en premiere intention. Ce traitement peut etre repete en cas de recidive. La dilatation pneumatique represente une alternative mais le taux de recidive est eleve. Le laser holmium-YAG donne de bons resultats sur la sclerose en seconde intention avec un faible taux de recidive mais aux depend d’un taux d’incontinence postoperatoire eleve en cas d’incisions multiples. En cas de multiples recidives, a un stade ultime, l’implantation d’un sphincter urinaire artificiel apres la mise en place d’un stent transanastomotique ou la realisation d’une derivation urinaire, offrent une meilleure qualite de vie que le catheterisme permanent.

Failure criteria of antiangiogenic treatment that must make envisage a change of molecule are still difficult to define. Concerning the evaluation of the response, if the RECIST criteria seem to be limited, several other tools of evaluation (clinical, radiological or biological tools) can be interesting. It is the case of contrast-enhanced ultra-sonography, but a precise definition of functional parameters should be defined and a standardization of the technique is necessary. Side effects do not translate necessarily a treatment failure. They must be estimated by taking into account the frequency of some of symptoms. Asthenia is noticed in more than 50% of the patients ; it is however necessary to exclude another aetiology, in particular iatrogenic hypothyroidism.

Resume Introduction et objectifs La preparation de l’examen classant national (ECN) est une preoccupation majeure des etudiants hospitaliers (EH). Il existe de nombreux supports pedagogiques pour preparer le module d’urologie : polycopies de la faculte et ouvrages d’urologie communement appeles « poly d’uro ». Le but de cette etude a ete de determiner les criteres de selection des EH pour choisir leur support pedagogique en urologie pour preparer l’ECN. Materiel et methode Un questionnaire a ete distribue a 200 EH (DCEM3 et 4) preparant l’ECN. Ce questionnaire comprenait trois parties : (1) les informations sur l’etudiant (faculte d’origine, assiduite aux cours, participation a des conferences d’internat) ; (2) le degre de satisfaction de l’etudiant vis-a-vis des supports pedagogiques mis a sa disposition ; (3) les criteres de choix (18 items) du polycopie utilise. Resultats Parmi les 200 etudiants interroges, seuls 32 % travaillaient sur les supports pedagogiques fournis par l’universite et 22 % affirmaient que leur faculte ne leur fournissait pas de support en urologie. Concernant ce support, 59 % pensaient qu’il etait « moyennement » a « pas du tout adapte » au programme de l’ECN et 70 % pensaient qu’ils etaient « moyennement » a « pas du tout » prepares a l’ECN. On notait que 83,5 % des etudiants pensaient que les conferences d’internat etaient « utiles » a « tres utiles » pour preparer l’ECN en urologie et 85 % d’entres eux pensaient qu’un polycopie etait « utile » a « tres utile » pour preparer l’ECN. Le polycopie du College d’urologie etait le support le plus utilise : 38 % des etudiants interroges. Les quatre criteres de choix les plus importants etaient « l’avis de leur entourage », « le style de presentation et de redaction » et « les conseils de leurs aines internes ». « L’auteur » et « le prix » etaient des considerations qui n’arrivaient qu’en onzieme et douzieme position. Conclusion Dans notre etude, les etudiants semblent juger insuffisante et inadaptee l’offre de la faculte pour les preparer a l’epreuve de l’ECN en urologie. En revanche, le polycopie du College d’urologie leur apparait comme un outil de travail satisfaisant en termes de presentation et de redaction.

The fact that external beam radiotherapy and brachytherapy are now considered to be curative techniques has led to major review of the modalities of follow-up after radiotherapy for prostate cancer. The problem concerns both the diagnosis of recurrence, rapidly announced by elevation of prostatic-specific antigen (PSA), usually at a subclinical stage, and the validity of criteria of biochemical recurrence to allow comparison of various study. Physicians involved in follow-up should be aware of the potential of bounce in PSA follow-up after external beam radiotherapy or brachytherapy. The PSA bounce phenomemon was defined by a rise of PSA values (+0.1 -0.8 ng/ml) with a subsequent fall. Biochemical failure after external beam radiotherapy or brachytherapy (with or without hormonotherapy) was defined by Phoenix criteria by a rise of 2 ng/ml above an initial PSA nadir. This definition was more correlated to PSA bounce phenomenon.

In France, the prevalence of urinary incontinence is about 44%. Obesity, defined by a Body Mass Index (BMI) above 30kg/m(2), is well established as a risk factor of stress urinary incontinence. Odds ratio (OR) varies between 1.7 and 2.4. Urge or mixed incontinence also occurs in obesity. Urinary incontinence epidemiology is not well-known in obese women. Weight loss, obtained by a weight reduction diet program or bariatric surgery, improves urinary symptoms of stress, urge or mixed incontinence. Functional outcome of urge incontinence surgery is not influenced by obesity. Typically, functional outcome and morbidity of tension-free vaginal tape are not influenced by BMI variations.

Les objectifs de ce reseau sont la mise en place d’une coordination nationale de prise en charge des tumeurs rares du rein de l’adulte. Cette unite s’est constituee autour de deux reunions de concertation pluridisciplinaire (RCP) regionales, l’une sur l’Ile-de-France et l’autre sur le Grand-Sud de facon a recenser, enregistrer et evaluer les traitements proposes a ces patients. L’objectif est de rediger un referentiel pour les tumeurs rares renales elaborees de facon multidisciplinaire afin de definir les modalites de diagnostic, de traitement et de surveillance. La constitution d’une tumorotheque et d’une serotheque, s’inscrivant dans la demande de l’institut du cancer, est mise en place afin d’identifier des marqueurs diagnostiques, pronostiques et predictifs de la reponse therapeutique aux anti-angiogeniques actuellement en cours d’evaluation. L’objectif final est d’obtenir des donnees epidemiologiques, cliniques et histologiques permettant de recenser les cas sur le plan national, de realiser un suivi de l’histoire naturelle, d’evaluer l’efficacite des traitements medicaux (anti-angiogeniques, inhibiteurs de mTOR [mammalian target of rapamycin]...) et chirurgicaux innovants et d’etre le support de publications de series homogenes sur le plan international.

MR breast screening in patients with genetic mutation Purpose. To compare screening breast MRI with conventional screening techniques in high-risk patients with genetic mutation. Materials and methods. Prospective study of 85 patients (mean age of 43 years) with genetic mutation and screening 2 years (mean of 2.7 years, 231 screening examinations). BI-RADS lesions 3-5 were biopsied; isolated BI-RADS 3 lesions on MRI were followed. Results from both techniques were compared. The number of short interval follow-up examinations and biopsy results were reviewed. Results. Eight cancers were diagnosed (3 in situ, 5 invasive carcinomas including an interval cancer, mean size of 14 mm). The sensitivity values for mammography, US and MRI were 12.5%, 50% and 95%, and specificity values were 98.7%, 97.3% and 94.8% respectively. Nineteen short interval follow-up MRI examinations were performed (19%): 14% of patients at initial screening and 5% and 6% at the the second and third screenings. Thirty-two biopsies were performed in 17 patients, including 18 after MRI (PPV of cytology: 30%, and biopsy: 58%). Conclusion. Our results confirm the value of MRI for screening of high-risk patients with genetic mutation.

Resume Objectif Dresser un panorama des differentes formes cliniques du cancer de l’uretre chez la femme a partir de trois cas cliniques et faire une mise au point sur les recommandations de prise en charge correspondantes. Methode Nous rapportons ici trois cas de cancer de l’uretre feminin. Le cas 1 est un carcinome epidermoide apparue chez une patiente de 56 ans sans antecedent. Le cas 2 etait une tumeur urotheliale developpee dans un diverticule uretral chez une patiente de 60 ans, tabagique. Le cas 3 etait une patiente de 69 ans qui avait un carcinome urothelial invasif. Resultats Le cas 1 a subi une uretrectomie segmentaire, il n’y a pas eu de traitement complementaire, l’evolution a ete favorable. Le cas 2 a subi une pelvectomie anterieure, il n’y a pas eu de traitement complementaire. La patiente a recidive sous forme d’une carcinose peritoneale a deux ans et est decedee. Le cas 3 a ete traite initialement par une pelvectomie anterieure puis par une association radio-chimiotherapie apres recidive locale. L’evolution a ete favorable. Conclusion Les presentations cliniques et les formes histologiques du cancer de l’uretre chez la femme sont nombreuses. Les lesions distales localisees peuvent etre traitees par une resection circonferentielle simple. Le traitement des autres lesions associe pelvectomie anterieure et radio-chimiotherapie a base de sels de platine ou M–VAC. Les facteurs pronostiques les plus importants de ces tumeurs sont leur taille, leur type histologique, leur localisation et l’existence d’une extension ganglionnaire pelvienne.

We report on the epitaxial growth and spectroscopic study of highly doped Y2SiO5:Yb3+ (YSO:Yb) thin films on YSO substrates. The realization of Ge, La and Gd-codoped high quality thin films, with thickness up to 100 μm, is demonstrated. YSO:Yb layers have their fluorescence and absorption spectra similar to the bulk ones, with a globally higher crystalline quality. The Yb3+ lifetime evolution exhibits a particularly slow decrease with Yb doping, proof of a low extrinsic quenching centers concentration. The refractive index increase with respect to dopants concentration is measured, and a phenomenological law is proposed. Such highly doped YSO:Yb layers could be an interesting alternative for active integrated optics or laser devices.

Yb-doped Y2SiO5 thin films Raman active phonons and crystal-field (CF) excitations have been studied by Raman spectroscopy and infrared transmission, and compared to the single crystals results. The thin films, grown on Y2SiO5 undoped substrate, have been co-doped with Ge, Gd or La in order to adjust the lattice mismatch. Raman active phonon bandwidths indicate that thin films present less strains and defects than single crystals. Interesting thin films properties, such as Yb3+ site occupancies and Yb3+–Yb3+ pair interaction types, may be adjusted by the co-dopants. Infrared absorption shows notably that La3+ is a good co-doping ion to enhance selectively one of the two Yb sites and pair satellite contributions. Ge and Gd co-dopants may also be used to form independently Yb3+–Yb3+ ion pairs with either ferromagnetic or antiferromagnetic dominant interactions.

Particle migration in a concentrated viscous suspension subjected to a non-homogeneous shear field was computed using a 3D extension of the diffusion model of Phillips et al. [Phillips, R. J., Armstrong, R. C., Brown, R. A., Graham, A. L., & Abott, J. R. (1992). A constitutive equation for concentrated suspensions that accounts for shear-induced particle migration. Physics of Fluids A , 4 , 30–40]. The numerical results were compared to experimental data from the literature for simple flows and to our own data in the case of two helical ribbon based mixing systems. It is shown that this type of diffusion model, which was developed to predict the behavior of concentrated suspensions dominated by particle–particle interactions, can predict migration trends but definitely requires additional improvements.

International audience; The objective of the present work was to relate the physical evolution quantified by image analysis to the chemical transformation of beet pulp particles during enzymatic degradation. Beet pulps were degraded into a torus reactor equipped for visualisation. Pectinolytic and cellulolytic enzymes were used separately or in combination. Two global image analysis techniques were tested to characterise the size distribution of overlapping particles. Granulometric curves were extracted by mathematical morphology and a regularisation dimension was assessed by fractal analysis. Both techniques proved efficient to follow particle size evolution during degradation. When using cellulolytic enzymes alone, no chemical or physical evolution was observed. When using pectinolytic enzymes, a chemical modification occurred without any physical evolution. Particles physically disappeared when both enzymes were used. The chemical and physical evolutions of particles during degradation were interpreted taking into account the current model of molecular arrangement of primary cell walls.

Au cours du stockage, la datte est sujette a plusieurs evolutions qui affectent ses caracteristiques organoleptiques. La caracterisation des parametres inducteurs de ces evolutions doit permettre de maitriser les mecanismes biochimiques et enzymatiques afin de pouvoir offrir les conditions adequates pour assurer la stabilite de la datte au cours du stockage preservant sa valeur nutritive et sa valeur marchande. Ce travail a permis l'extraction et le fractionnement des substances pectiques, au cours de la maturation, de la datte qui contient environ 2 % de pectines. La degradation enzymatique de ces pectines par les polygalacturonases, responsables avec les pectinemethylesterases du ramollissement, est illustree par l'activite polygalacturonase qui augmente au cours de la maturation pour atteindre une activite maximale au dernier stade de maturation (0,53 Ul/g Matiere Fraiche). La polygalacturonase de la datte serait une exo-polygalacturonase qui presente une activite optimale a une temperature de 40 °C et a un pH egal a 5.

The objective of this study is to present a new numerical strategy using soft-computing techniques to determine the optimal die gap programming of extrusion blow molding processes. In this study, the design objective is to target a uniform part thickness after parison inflation by manipulating the parison die gap openings over time. To model the whole process, that is, the parison extrusion, the mould clamping and the parison inflation, commercial finite element software (BlowSim) from the National Research Council (NRC) of Canada is used. However, the use of such software is time-consuming and one important issue in a design environment is to minimize the number of simulations to get the optimal operating conditions. To do so, we proposed a new strategy called fuzzy neural–Taguchi network with genetic algorithm (FUNTGA) that establishes a back propagation network using a Taguchi’s experimental array to predict the relationship between design variables and responses. Genetic algorithm (GA) is then applied to search for the optimum design of die gap parison programming. As the number of training samples is greatly reduced due to the use of orthogonal arrays, the prediction accuracy of the neural network model is closely related to the distance between sampling points and the evolved designs. The extrapolation distance concept is proposed and introduced to GA using fuzzy rules to modify the fitness function and thus improving search efficiency. The comparison of the results with commercial optimization software from NRC demonstrates the effectiveness of the proposed approach.

In the pig, dietary protein value depends on the availability of amino acids (AA), usually estimated by way of ileal digestibility measurement. However, AA availability for body protein deposition is influenced by endogenous digestive losses specifically induced by feed components. The purpose of the experiment was to assess the impact of some feed manufacturing processes on AA ileal endogenous losses and their resulting availability for growth. Four technological treatments were applied to a wheat–sunflower meal mix and compared in vivo according to a 4×4 Latin square design. These treatments were two grinding conditions, leading to different particle sizes (1000 and 500 μm), and two pelleting processes, “regular” and “high-flow extrusion” (HFE), operated on the finest particle size only. Ileal specific endogenous losses and the coefficient of ileal real digestibility (CIRD) of AA were measured by way of the isotopic dilution method using 15 N -leucine labelled pigs fitted with ileorectal anastomoses. In this work, a gradient-based numerical optimization scheme is proposed to determine the optimal process operating conditions to produce a blow molded part by with a given performance. Finite element simulations are used to relate the part performance to the processing conditions. A performance optimization is first performed to find the minimum part thickness distribution that minimizes the part weight while satisfying mechanical performance constraints such as maximum part deflection or maximum stress for an applied load. Then a process optimization finds the optimal operating conditions, e.g. the die gap opening profile, that minimize the part weight while respecting the minimum thickness distribution dictated by the performance optimization. The results show that the optimization scheme minimizes the part weight with minimal constraint violation. The addition of a constraint associated with process stability is proposed.
