2. A specific controlled ovarian stimulation (COS) protocol for fertility preservation in women with breast cancer undergoing neoadjuvant chemotherapy

Aim of the study : The authors present a novel and specific controlled ovarian stimulation protocol for fertility preservation in women with estrogen-positive receptor breast cancer undergoing neoadjuvant chemotherapy. The protocol foresees random start ovarian stimulation and the use of letrozole associated to tamoxifen. Material and methods : Forty breast cancer patients were included in the study. COS was performed either with recombinant FSH or hMG. Concomitantly with COS, letrozole in a dose of 5 mg and tamoxifen in a dose of 20 mg were given orally on a daily basis. The trigger was performed with 0.2 mg of triptorelin, in the presence of follicles ≥ 19 mm. Oocyte retrieval was scheduled 35–36 hours after triptorelin injection. Our main outcome measures were the number of oocytes collected and number of oocytes vitrified, the length of ovarian stimulation, total dose of gonadotropins administered, and levels of estradiol on the day of the trigger. Results : The mean age of patients was 30.43 ±4.25 years. Nineteen women commenced COS in the luteal phase, eleven in the early follicular phase and ten in the late follicular phase. The mean number of collected oocytes was 11.78 ±9.12 and the mean number of vitrified oocytes was 9.72 ±7.36. The mean duration of COS was 10.03 ±1.33 days. The mean estradiol concentrations on the triggering day was 623.10 ±441.27, and the mean dose of gonadotropins administered was 2540 ±713.10. Conclusions : The authors suggest that the protocol is efficient and may be a safe option for oocyte vitrification in these patients.

3. Lifestyle improvement reduces the consumption of ultra-processed foods in adults with metabolic syndrome

The high consumption of ultra-processed products is a concern because it is positively associated with the incidence of chronic non-communicable diseases, as metabolic syndrome (MetS). The aim is to evaluate the effects of three different interventions to modify lifestyle on the consumption of ultra-processed foods in adults with MetS.This was a randomized clinical trial, in which the participants were divided into three groups: Standard Intervention (SI), Group Intervention (GI) and Individual Intervention (II). The interventions were carried out over a three-month period and the data was collected in a 24-h food record, taken at the beginning and end of the intervention. The food they ate was classified into four groups according to the degree of processing (unprocessed or minimally processed foods, processed culinary ingredients, processed foods, and ultra-processed foods) in accordance with the NOVA food classification. Seventy adults took part in the study with a mean age of 51.2 ± 6.6 years old; most of whom were female (55.7%). The amount of ultra-processed food consumed by the three groups (SI, GI and II) was significantly reduced (46%, 34%, and 33%, respectively). The amount of processed food consumed only reduced in the II group. The Total Energy Value (TEV) consumed by the SI and II groups decreased.The interventions that were intended to alter lifestyles were able to reduce the amount of ultra-processed food consumed, which can have an impact on the prevention and treatment of MetS.registered in the Brazilian Registry of Clinical Trials, ReBEC, under number: RBR-9wz5fc.

OBJECTIVE To evaluate, by a literature review, the Timed "Up & Go" (TUG) test use and its main methodological aspects in children and adolescents. DATA SOURCES The searches were performed in the following databases: PubMed, CINAHL, Web of Science, SciELO and Cochrane Library, from April to July 2012. Studies published from 1990 to 2012 using the terms in Portuguese and English "Timed "Up & Go", "test", "balance", "child", and "adolescent" were selected. The results were divided into categories: general characteristics of the studies, population, test implementation METHODS, interpretation of results and associations with other measurements. DATA SYNTHESIS 27 studies were analyzed in this review and most of them used the TUG test along with other outcome measures to assess functional mobility or balance. Three studies evaluated the TUG test in significant samples of children and adolescents with typical development, and the most studied specific diagnoses were cerebral palsy and traumatic brain injury. The absence of methodological standardization was noted, but one study proposed adaptations to the pediatric population. In children and adolescents with specific clinical diagnoses, the coefficient of within-session reliability was found to be high in most studies, as well as the intra and inter-examiner reliability, which characterizes the good reproducibility of the test. CONCLUSIONS The TUG test was shown to be a good tool to assess functional mobility in the pediatric population, presenting a good reproducibility and correlation with other assessment tools.

OBJETIVO: comparar as informações obtidas pela ultra-sonografia pélvica e transvaginal (USG), histerossalpingografia (HSG), histeroscopia diagnóstica (HSC), ressonância nuclear magnética pélvica (RNMP) e hísterossonografia tridimensional (HSSNG 3D) para melhorar a acurácia e simplificar a investigação do fator uterino cervical e corporal em infertilidade conjugal. MÉTODOS: no período de janeiro a julho de 2003, cinquenta mulheres com queixa de infertilidade de, no mínimo, dois anos foram submetidas a USG, HSG, HSC, RNMP e HSSNG 3D como exames de rastreamento para o diagnóstico do fator uterino. Foram analisados o canal endocervical, endométrio, miométrio e presença de malformações uterinas. Os resultados de cada exame foram analisados e comparados. RESULTADOS: das 50 mulheres incluídas, 12 (24%) apresentaram alteração em pelo menos um dos exames realizados. Quando se comparou a HSSNG 3D com a USG, a HSSNG 3D forneceu informações adicionais em 7 casos (58,3%); comparada com a HSG, forneceu informações adicionais em 7 casos (58,3%); com a HSC, em 4 casos (32,1%), e comparada à RNPM, em 6 casos (50%). Somente em dois casos a HSG detectou alterações do canal endocervical que não foram visualizadas na HSSNG 3D; em todos os outros casos a HSSNG 3D forneceu o mesmo diagnóstico ou deu informações adicionais em relação aos outros exames. A análise pelo teste kappa mostrou que existe concordância entre os diagnósticos obtidos com o uso da HSSNG 3D e a USG, HSG e RNMP. No entanto, quando se associaram os resultados da HSG e da HSSNG 3D observamos que todas as condições clínicas poderiam ter sido diagnosticadas, de forma precisa, somente com esses exames. CONCLUSÃO: a associação da HSG com a HSSNG 3D pode ser suficiente para o diagnóstico do fator uterino cervical e corporal em infertilidade, reduzindo o número de exames realizados por cada paciente, os custos totais finais, a ansiedade e o retardo para o início do tratamento.PURPOSE: to compare the information obtained with pelvic and transvaginal ultrasonography (USG), hystero-salpingography (HSG), diagnostic hysteroscopy (HSC), pelvic nuclear magnetic resonance imaging (PNMR), three-dimensional hysterosonography (3D HSSNG), to optimize and simplify the investigation about cervical and corporeal uterine factors in conjugal infertility. METHODS: in the period between January and July 2003, fifty women reporting infertility for at least two years were submitted to USG, HSG, HSC, PNMR, and 3D HSSNG as tracking examinations for uterine factor diagnosis. The endocervical canal, as well as the endometrium, myometrium, and the presence of uterine malformations were investigated. The results of each examination were analyzed and compared. RESULTS: of the 50 women studied, 12 (24%) presented alteration in at least one of the examinations. When 3D HSSNG was compared to USG, 3D HSSNG provided additional information in 7 cases (58.3%); when compared to HSG, it provided additional information in 7 cases (58.3%); when compared to HSC, it provided additional information in 4 cases (32.1%), and when compared to PNMR, it provided additional information in 6 cases (50%). There were only two cases in which HSG detected alterations of the endocervical canal that were not visualized using 3D HSSNG. In the other cases 3D HSSNG imparted the same diagnosis; furthermore, it provided additional information in comparison to the other examinations. Drop-weight experiments studied the damages due to repeated low-velocity impacts of hybrid composite laminates. The laminate made by carbon woven fabric and glass woven fabric impregnated by vinyl ester resin was subjected at single impact, at 5 repeated impacts and 10 repeated impacts, for an energetic level of U=20J. The final damage after the single and repeated impact events was analysed by no-destructive methods, Pulse Thermography and Holographic Interferometry to evaluate the influence of the multi-hit events on the damaged area's evolution.

Eur J Clin Invest 2010; 40 (9): 803–811 Abstract Background Adrenal incidentalomas (AI) have been associated with and an increased prevalence of metabolic and bone complications. The N363S and BclI polymorphisms of the glucocorticoid receptor (GR) have been associated with an increased sensitivity to glucocorticoid (GC). This observational study aims to evaluate whether BclI and N363S polymorphisms play a role in the development of complications in AI. Materials and methods We enrolled 100 patients with AI (66 F; 34M). The presence of diabetes, arterial hypertension (AH), dyslipidaemia, osteoporosis and vertebral fracture (Fx), waist circumference and the Body Mass Index (BMI) were assessed. DNA samples were genotyped. Patients with wild-type BclI, wild-type N363S and heterozygous BclI polymorphism were classified as carriers of haplotype 1 (H1; n = 86), patients with homozygous BclI and heterozygous N363S polymorphism of GR of haplotype 2 (H2; n = 14). Results We found no clinical or biochemical differences between haplotype 1 and 2 groups, but a higher prevalence of the simultaneous presence of Fx plus AH in H2 patients (H2 n = 7, H1 n = 16, P = 0·01). Logistic regression analysis showed that the presence of Fx and of AH and the combination of the presence of Fx plus AH were associated with the H2 genotype regardless of the degree of cortisol secretion, age, BMI and BMD (OR 4·88, 95%CI 1·47–18·40, P = 0·05; OR 8·25, 95%CI 0·98–69·52, P = 0·05; OR 7·25, 95%CI 1·57–35·78, P = 0·011; respectively). Conclusions In AI patients, the presence of the haplotype 2 of BclI and N363S is associated with the presence of AH, Fx and with the combination of Fx and AH.

Background: The role of adrenal scintigraphy (AS) in the diagnosis of subclinical hypercortisolism (SH) in adrenal incidentaloma (AI) is debated. Aim: To evaluate the possible role of AS in identifying SH in AI patients. Subjects and methods: In the retrospective phase (2000–2004), 102 AI patients [71 females (F)/31 males (M)] referred to our center were reevaluated for SH. In the prospective phase (2005–2006), 42 patients (32F/10M) with suspected SH were evaluated performing AS and biochemical assessment of cortisol secretion. We report data of the prospective phase of the study. In these patients AS was performed at baseline; the difference between the uptake of the affected and the controlateral gland [mean Δ uptake (MΔu)] was calculated. Cortisol secretion was evaluated in 3 different occasions. Patients were considered affected with SH if they presented at least twice 2 of the following criteria: urinary free cortisol >193 nmol/l, cortisol after 1 mg dexamethasone suppression test >83 nmol/l, ACTH levels

As the population ages, osteometabolic diseases and osteoporotic fractures emerge, resulting in substantial healthcare resource utilization and impaired quality of life. Many types of mechanical stimulation have the potential of being recognized by bone cells after a mechanical sign is transformed into a biological one (a process called mechanotransduction). The therapeutic ultrasound (TU) is one of several resources capable of promoting bone cell mechanical stimulation. Therefore, the main purpose of present study was to evaluate the effect of TU on the proliferation of pre-osteoblasts using in vitro bioassays.We used MC3T3-E1 pre-osteoblast lineage cells kept in Alpha medium. Cells were treated using pulsed mode therapeutic ultrasound, with frequency of 1 MHz, intensity of 0.2 W/cm(2) (SATA), duty cycle of 20%, for 30 minutes. Nifedipine and rapamycin were used to further investigate the role of L-type Ca(2+) channels and mTOR pathway. Intracellular calcium, TGF-β1, magnesium, and the mRNA levels of osteopontin, osteonectin, NF-κB1, p38α were evaluated.The results show that TU stimulates the growth of MC3T3-E1 cells and decreases the supernatant calcium and magnesium content. Also, it increases intracellular calcium, activates NF-κB1 and mTOR complex via p38α. Moreover, TU promoted a decrease in the TGF-β1 synthesis, which is a cell growth inhibitor.Therapeutic ultrasound, with frequency of 1 MHz, intensity of 0.2 W/cm(2) (SATA) and pulsed mode, for 30 minutes, was able to increase the proliferation of preosteoblast-like bone cells. This effect was mediated by a calcium influx, with a consequent activation of the mTOR pathway, through increased NF-κB1 and p38α.

The aim of this study is to evaluate the response to an inflammatory stimulus in mice exposed to LPS-induced neonatal stress at different ages and sexes. Balb/c mice were submitted to intraperitoneal injections on postnatal days 3 and 10 with lipopolysaccharide (nLPS) or saline solution (nSal). At 21 or 60 days, either saline solution was injected or an inflammatory stimulus was induced by the injection of 1% carrageenan. Inflammatory cytokines, reactive oxygen species, and neutrophil extracellular traps (NETs) production were measured in peritoneal fluid. LPS-induced neonatal stress can reduce inflammatory cytokines in males and females. An increase in NETs production was observed when 60 day nLPS animals were compared to 21 day mice in both sexes. The ROS production was not affected by neonatal stress. The results shown here indicate that LPS-induced neonatal stress can alter cytokine production in response to inflammatory stimuli at different ages, in a sex-dependent effect. © 2016 Wiley Periodicals, Inc. Background: The molecular epidemiology of rubella virus (RV) based on the analysis of the viral E1 gene sequences indicated the existence of two genotypes that differ from each other by 8 to 10% in their nucleotide sequences: genotype I is present in Europe, North America and Asia; and genotype II is present only in Asia. Objectives: The purpose of the study was to identify the RV genotypes circulating in Brazil. Study design: In this study, we analysed 86 clinical samples collected between 1996 and 1999 during a rubella outbreak and from sporadic cases of rubella in Rio de Janeiro State. For the molecular characterisation of RV strains we have used PCR/nested amplification and direct sequencing of a 513-nucleotide region of the E1 gene. Results: The E1 gene sequences of 14 RVs were obtained and were assigned to two lineages, both within genotype I. The percentage divergence of nucleotide sequence ranged from 3.4 to 5.1% between these two lineages. These results were in agreement with the pattern of variation observed among the sequences obtained from other lineages of RV. Conclusions: This work demonstrated that two new lineages of RV circulated simultaneously between the years 1996 and 1999 in the state of Rio de Janeiro. These results provided new approaches for monitoring the progress of vaccination efforts in Brazil.

Aims: The persistent circulation of hepatitis A virus (HAV) in the Mediterranean area suggests the need for monitoring its presence in the environment. A reverse transcriptase-polymerase chain reaction (RT-PCR) was used to detect the presence of HAV in several consecutive raw sewage and final effluent samples, collected over an 8-month period from an activated sludge treatment plant in southern Italy. Methods and Results: Two distinct purification protocols, either based on antigen-capture with monoclonal antibody (AC) or RNA extraction, were compared. The possible influence of the antibody used in the AC phase was evaluated in preliminary experiments on HAV-spiked samples, using two different monoclonal antibodies. Hepatitis A virus RNA was detected in all but one sewage environmental sample examined. The contemporary presence of enteroviruses, reoviruses and phages was observed, while HAV growth in cell culture was hampered. Conclusions: The RT-PCR technique was confirmed to be a valuable tool for the rapid monitoring of HAV in sewage samples. In addition, this study demonstrated that application of different sample purification methods can result in different levels of sensitivity of the assay and that, in the antigen-capture method, the choice of antibody can have a crucial role. Significance and Impact of the Study: This work underlines the need for technical uniformity in environmental studies from different laboratories for a correct and useful comparison of the results.

14. WASTEWATER DISINFECTION BY UV AT TRANI MUNICIPAL PLANT

The pilot investigation on the use of UV as an alternative disinfectant to NaOCI was started in 1992 at Trani (South Italy) municipal wastewater treatment plant (335 m3/h). The results collected after six months continuous operation enabled us to compare UV and NaOCl disinfection effectiveness on the basis of secondary effluent characteristics, quantify photoreactivation effects, evidence possible DBP formation and assess costs.

The aim of the study was to examine the effects of beta-glucan treatment in women with recurrent vulvar candidiasis or previously treated with diathermocoagulation (DTC) for vulvar lesions caused by human papillomavirus (HPV) infection.From January to March 2008, 23 women with a history of recurrent candidiasis and 209 women who underwent to DTC for HPV-correlated vulvar lesions were recruited at the Department of Obstetrics and Gynaeco-logy of San Camillo-Forlanini Hospital of Rome. The two groups were treated with two cycles of a daily topical application of beta-glucan for 15 consecutive days with a suspension of 20 days. The effects of beta-glucan were analyzed at the 1st and the 3rd month from the start of the therapy.A total of 209 women, 21 with recurrent candidiasis and 188 who underwent to DTC for HPV-correlated lesions, completed the study. After the first month of treatment we not found any evidence of disease in both groups; otherwise we observed, a month after the term of the therapy, 5% and 3% of recurrence of candidiasis or lesions secondary to HPV infection, respectively, even if with low grade.Our results demonstrate the efficacy of beta-glucan treatment for recurrent candidiasis, HPV-correlated lesions and in the reparative process of epidermis.

Background: BioEnterics Intragastric Balloon (BIB®) is a non-invasive, temporary and relatively safe procedure shown to be effective in the short-term treatment of obesity. Nowadays, BIB® does not show convincing evidence of significant long-term weight loss, as compared with conventional management, and data regarding changes in metabolic and nutritional parameters are lacking. Methods: Forty obese patients [11 males, 29 females, age 36.65±10.6 yr, body mass index (BMI) 44.9±8.9 kg/m2] were evaluated before and 3 and 6 months after BIB® placement by assessment of anthropometric and biochemical parameters as well as nutritional habits. Results: Patients showed a significant reduction in weight (−13.2±6.5%), BMI (−13.2%), waist circumference (−6.5 cm), and percentage of fat mass (−19.5%), but not fat-free mass. A significant improvement in insulin sensitivity but not in lipid pattern was seen. After BIB® insertion, a significant reduction in caloric intake was paralleled by a redistribution of nutrients; in particular, increased lipid (12.8%) and decreased carbohydrate (−11.7%) percentage, but not absolute intake was observed. Conclusion: These data show that BIB® improves anthropometric parameters, with reduction of fat mass and preservation of fat-free mass, as well as insulin resistance, but not other metabolic features. The observed change in dietary habits, with a relative increase in lipid intake, once BIB® is removed, might favor body weight regain and impact negatively on body weight composition and the other traits of the metabolic syndrome.

In this article, we discuss the case of a 42-year-old female diabetic patient who developed severe lactic acidosis, coma, shock, rhabdomyolysis, and renal and hepatic failure during metformin therapy after alcohol abuse. The APACHE II score of this patient was 38, which is associated with an estimated mortality rate >88%. Conventional treatment of acidosis with IV infusion of sodium bicarbonate (400 mmol) was ineffective in restoring a normal acid/base balance. Lactic acidosis was successfully treated with bicarbonate-buffered continuous veno-venous hemodiafiltration. The patient also had a complete recovery from all complications.

Background Macroprolactinaemia may represent a relevant cause of misdiagnosis, unnecessary investigation and inappropriate treatment. The aim of this study was to investigate the clinical and neuroradiological characteristics of patients with and without macroprolactinaemia and to evaluate the impact of macroprolactin determination on the diagnostic work-up of hyperprolactinaemic patients. Materials and methods Retrospective analysis in 135 consecutive hyperprolactinaemic patients (111 women and 24 men; mean age 37 ± 11·6 years) whose archived sera were subsequently tested for macroprolactin. Recoveries ≤ 40% after polyethylene glycol precipitation were indicative of macroprolactinaemia. Results Macroprolactin, entirely explaining biochemical hyperprolactinaemia, was found in 42·2% of patients, a third of whom presented with signs and symptoms of hyperprolactinaemia. Determination of macroprolactin changed the initial diagnosis in a consistent proportion of patients. In particular, idiopathic hyperprolactinaemia, initially diagnosed in 41 patients, was then excluded in 28 of them. Diagnosis of prolactin-secreting pituitary microadenoma shifted to non-secreting pituitary microadenoma in 10 of 49 patients, while in all patients with prolactin-secreting pituitary macroadenoma or hyperprolactinaemia due to stalk deafferentation the presence of macroprolactin was excluded and the initial diagnosis confirmed. Finally, macroprolactin was present in the majority of patients with magnetic resonance imaging (MRI) scans suggestive for primary empty sella (4 of 5 women) or pituitary hyperplasia (12 of 17 women, 3 of 3 men). Collectively, about half of subjects with macroprolactinaemia showed variable MRI abnormalities. Conclusions The presence of macroprolactin was a relevant cause of misdiagnosis in patients with hyperprolactinaemia. However, due to the unexpected high frequency of pituitary abnormalities observed in the present series, we suggest that the diagnostic algorithm of hyperprolactinaemic states should include both polyethylene glycol precipitation test and MRI imaging.

Background: With the evolution of laparoscopic surgery comes the need for specific instruments that apply traction to parenchymal tissue, like the spleen, without exposing the organ to the associated high risk of bleeding. To meet this need, we designed and developed a suction-cup grasper that allows easy grasping and manipulation of the spleen. Some of the difficulties usually encountered during laparoscopic splenectomy may be overcome by using this device. Materials: The instrument consists of a cone-shaped, silicone rubber suction cup designed with an antislip internal surface. The cup is connected to a support arm with a flexible distal end that can be rotated. Traction is exerted with a commonly available suction system. The device is inserted through a 12-mm-diameter guide sheath. Results: The two interventions performed with the atraumatic device were completed with laparoscopic technique. No complications arose during or after the operations. The average operating time was 110 min. The patients were discharged after 4 and 5 days postoperative, respectively. Conclusions: As a device specifically designed for grasping parenchymal organs, the atraumatic suction grasper affords the operator a faster and safer technique in laparoscopic splenectomy.

Cervical pregnancy is a rare kind of intrauterine ectopic pregnancy. Diagnosis and treatment of cervical pregnancy have enormously changed in the last 15 years. Before 1980, diagnosis was made when dilation and curettage for presumed incomplete abortion resulted in sudden and uncontrollable hemorrhage. Hysterectomy was practiced in order to save the patient's life. Today, cervical pregnancy is diagnosed by ultrasound (US) during the Ist trimester of pregnancy, so that the patient's fertility can be preserved. Therefore any physician should consider the possibility of a cervical pregnancy in a woman with abdominal pain and vaginal bleeding during the first trimester of pregnancy. In this study we reviewed the literature on the epidemiology, etiology, diagnosis and treatments of cervical pregnancy.

Context: The effects of chronic therapy with long-acting somatostatin analogs (SSTa) on metabolic and cardiovascular parameters have been partially elucidated in acromegalic patients. Objective: The objective of this study was to compare the long-term effects of SSTa treatment and successful surgery on GH/IGF-I secretion and cardiovascular risk parameters in acromegaly. Design, Patients, and Intervention: This was a retrospective study of 36 acromegalic patients treated with SSTa and evaluated after a median of 66 months and of 33 sex-, age-, and body mass index-matched cured patients evaluated after a similar period of remission, all from the Institute of Endocrine Sciences (Milan, Italy). Main Outcome Measures: The main outcome measures were fasting and post-oral load glucose homeostasis, hemoglobin A1c, insulin sensitivity and secretion by several indexes, lipid profile, and blood pressure. Results: Fasting and areas under the glucose response curve rose in patients controlled (n 29) and not controlled (n 7) by SSTa, becoming higher than those in cured subjects. A 1% hemoglobin A1c increase was observed in all nondiabetic SSTa patients, but not in cured subjects. Basal insulin secretion and resistance, evaluated by homeostasis model assessment, decreased in all SSTa patients, whereas oral glucose tolerance test-derived insulin secretion and resistance, evaluated by insulinogenic index and oral glucose tolerance test-derived insulin secretion, improved only in SSTa-treated controlled patients. Triglycerides did not change during SSTa, whereas high-density lipoprotein cholesterol increased in SSTa-treated controlledpatients.Atthelastvisit,thecontemporarypresenceofatleast three cardiovascular risk factors was more frequent in patients treated with SSTa than in cured subjects. Conclusions: SSTa therapy induces long-lasting disease control and improvement of insulin sensitivity and high-density lipoprotein cholesterol levels in responsive patients. The progressive glucose homeostasis alterations, observed independently from the degree of cure, suggest the need for glucose homeostasis and peripheral vascular complications monitoring during chronic SSTa treatment. Giardia and Cryptosporidium are important agents of water-borne parasitic diseases. In this work we have examined the recovery efficiency of two methods for concentrating Giardia cysts and Cryptosporidium oocysts from water: a membrane filtration method and a crossflow filtration method. Results demonstrated a higher recovery efficiency for crossflow filtration method in comparison to the membrane filtration method. In addition, Giardia cysts and Cryptosporidium oocysts concentration was evaluated in wastewater samples submitted to chemical flocculation or chemical flocculation followed by slow sand filtration. Results showed that slow sand filtration was capable of reducing the number of Giardia cysts, but not of Cryptosporidium oocysts in wastewater.

The authors report their experience in the management of Ogilvie's syndrome, a rare form of large bowel acute pseudo-obstruction. The study includes fifteen cases of the disease. There were ten males and five female, with a mean age of sixty-two years (range 45-92). Three patients were only treated with medical and conservative measures, one with unsuccessful colonoscopy and twelve (two with colonic perforation) underwent laparotomy. The surgical procedures performed were tube cecostomy (40%), colonic resection with primary anastomosis (27%) and exploratory laparotomy with decompression (13%). The overall morbidity and mortality rate were respectively 0 and 13%. Our epidemiological, clinical and therapeutic results are similar to those reported in the international literature. The pathophysiology of the syndrome is still unknown. It can be "idiopathic" or can complicate other diseases or surgical procedures (urological and gynaecological procedures mostly). Plain abdominal roentgenogram is the most useful diagnostic test, but colonoscopy may be an alternative diagnostic (and therapeutic) weapon. Conservative treatment is the method of choice but when the cecal diameter is more than 12 cm. (impending perforation), when the colon is perforated or when medical measures are unsuccessful, surgical procedure is compulsory. The age of the patient, cecal size, delay in colonic decompression are the most important prognostic factors. Even with a proper management, the prognosis is severe and the mortality rate is high (3-50%).

Even if it is a rare disease, acute cecal diverticulitis is a diagnostic and therapeutic challenge for the surgeon. The authors present their experience synthetically and compare their results with the literature. Their experience is not very different from the results reported by many other authors. Five cases underwent radical treatment (right colectomy) and 6 cases were treated with a more conservative operation (diverticulectomy and/or invagination). Only one patient was treated medically. Acute diverticulitis of the cecum requires the solution of three difficult questions: 1) preoperative diagnosis; 2) intraoperative diagnosis; 3) correct therapeutic procedure.

The study included 20 patients affected by cholelithiasis and choledocholithiasis. Al patients underwent cholecystectomy; after choledochotomy, choledochal stones were removed using a Fogarty's biliary catheter (no. 5) and, subsequently, the papilla was calibrated by inserting the end balloon of the catheter, which and been filled with 0.3 cc of water, beyond the level of the papilla itself. The catheter was then withdrawn. No T-tubes were left in the common bile duct in any patient, no post-operative complications were observed and all patients were dismissed within seven days.

Two cases of pseudo-obstruction of the colon (Ogilvie's syndrome) are presented. Increasing abdominal distension and the pain suggested surgical treatment for both patients. The operation showed in the first case a marked dilatation of the transverse colon and sigmoid, in the second the dilatation of the ascending and transverse colon, without any distal obstruction. Enterotomy was performed only in the first case, as the diameter of the colon was more then 10 cm. The authors, by review of the literature and their own experience, propose a critical revision of the syndrome, discussing the physiopathology of the rising disease.

A case of pancreatitis that preceded and complicated, in the postoperative phase, one of ileal volvulus imprisoned in the Douglas and operated two days after admittance is reported.
