Background: Hysterectomy is the surgical removal of the uterus. It is the most frequently performed major gynaecological surgical procedure, with millions of procedures performed annually throughout the world. Hysterectomy can be performed for benign and malignant indications. Misoprostol, a synthetic analogue of prostaglandin E1, has been extensively evaluated as a uterotonic agent in obstetrics mainly for prevention and management of postpartum haemorrhage and reduction of bleeding during caesarean delivery. The purpose of this research is to evaluate the impact and efficacy of pre-operative sublingual misoprostol given in women having hysterectomy in terms of blood loss. Results: This randomized single blind controlled trial was carried on 70 women undergoing total abdominal hysterectomy. They were randomly allocated in two equal groups: misoprostol group: patients received two tablets of Misoprostol (=200 μg) 30 minutes before operation and a control group (placebo group): patients received two tablets of Placebo 30 minutes before operation. Haemoglobin and Haematocrit reductions were significantly lower among misoprostol group than among placebo group. Blood loss was significantly lower among misoprostol group than among placebo group. The most common indication for hysterectomy was a fibroid, dysfunction uterine bleeding and, adenomyosis. The fibroid was the most significant for the effect of misoprostol in decreasing blood loss, haemoglobin and haematocrit during abdominal hysterectomy more than adenomyosis and dysfunction uterine bleeding. Conclusions: A single preoperative dose of 400 μg of misoprostol administered 30 minutes before abdominal hysterectomy resulted in a significant reduction of blood loss.

Background/Aim: Papillary thyroid cancer (PTC) is the most common endocrine malignancy with a rising incidence. There is a need for a non-invasive preoperative test to enable better patient counselling. The aim of this systematic review was to investigate the potential role of circulating microRNAs (miRNAs) in the diagnosis and prognosis of PTC. Materials and Methods: A systematic literature search was performed using MEDLINE, Cochrane, and Scopus databases (last search date was December 1, 2021). Studies investigating the expression of miRNAs in the serum or plasma of patients with PTC were deemed eligible for inclusion. Results: Among the 1,533 screened studies, 39 studies met the inclusion criteria. In total, 108 miRNAs candidates were identified in the serum, plasma, or exosomes of patients suffering from PTC. Furthermore, association of circulating miRNAs with thyroid cancer-specific clinicopathological features, such as tumor size (13 miRNAs), location (3 miRNAs), extrathyroidal extension (9 miRNAs), pre- vs. postoperative period (31 miRNAs), lymph node metastasis (17 miRNAs), TNM stage (9 miRNAs), BRAF V600E mutation (6 miRNAs), serum thyroglobulin levels (2 miRNAs), (131)I avid metastases (13 miRNAs), and tumor recurrence (2 miRNAs) was also depicted in this study. Conclusion: MiRNAs provide a potentially promising role in the diagnosis and prognosis of PTC. There is a correlation between miRNA expression profiles and specific clinicopathological features of PTC. However, to enable their use in clinical practice, further clinical studies are required to validate the predictive value and utility of miRNAs as biomarkers.

In the present study, Thirty-six Class patients II (A condition in which the upper jaw is larger than the lower jaw) were randomly selected and assigned to one of two groups based on their maturation stage: the pre-pubertal group (18 patients, mean age 9.15 ± 1.5 years) and post-pubertal group (18 patients, mean age 16.3 ± 1.0 years). All patients were treated with a metallic splint-supported Herbst IV appliance(An appliance that acts like artificial joint working between the upper and power jaws that keeps the lower jaw in a forward position, thus improving the Class II condition ). Pretreatment (T1) and post-Herbst IV treatment (T2) scans were obtained for both groups. Dental and skeletal measurements were made on the scans and statistically analyzed using paired and independent t-tests. The study hypothesis was that; the dentoskeletal changes in Class II malocclusion treatment using Herbst appliance in the Pre-pubertal is more than the Post-pubertal growth stage due to the remaining growth potential for the pre-pubertal patients. The comparison between the two groups revealed statistically significant differences in horizontal skeletal parameters in the lower jaw only, while other readings were similar

Purpose Disruption of calcium homeostasis is the most common complication after total thyroidectomy in adults. We explored the incidence and risk factors of hypocalcaemia and hypoparathyroidism after total thyroidectomy in children (≤ 18 years of age). Methods One hundred six children underwent total thyroidectomy. Patient, operative and outcome data were collected and analyzed. Results The indication for surgery was Graves' disease in 52 children (49.1%), Multiple Endocrine Neoplasia type-2 in 36 (33.9%), multinodular goiter in 3 (2.8%) and follicular/papillary thyroid carcinoma in 15 (14.2%). Neck dissection was performed in 23 children (18.9%). In 14 children (13.2%), autotransplantation was performed; in 31 (29.2%), ≥ 1 glands were found in the specimen. Hypocalcaemia within 24 h of thyroidectomy was observed in 63 children (59.4%) and 52 (49.3%) were discharged on supplements. Hypoparathyroidism at 6 months persisted in 23 children (21.7%). The ratios of all forms of calcium-related-morbidity were larger among children with less than four parathyroid glands remaining in situ: hypocalcaemia within 24 h of thyroidectomy (54.0% versus 47.5%; p = 0.01), hypoparathyroidism on discharge (64.4% versus 37.7%; p = 0.004) and long-term hypoparathyroidism (31.1% versus 14.8%; p = 0.04). Conclusion The incidence of postoperative hypocalcaemia and hypoparathyroidism among children undergoing total thyroidectomy is considerable. The inability to preserve the parathyroid glands in situ during surgery seems an important factor. For optimal outcomes, the parathyroid glands should be preserved in situ. The Nugrus-Hafafit and El Shalul areas are regarded as key regions for many enigmatic problems and debatable issues concerning the tectonic setting of the Najd-related shear zones in the Eastern Desert of Egypt. The Neoproterozoic basement succession in the studied areas is well-developed and represented by four rock groups arranged from older to younger as infrastructure unite (amphibolites, migmatized hornblende-biotite gneiss, psammitic gneiss and chlorite-biotite schist), suprastructure units including ophiolitic melange (metaultramafics and related derivatives) and island arc assemblages (metagabbros and metavolcanic-metasedimentary association), syn-tectonic granitoids (gneissic to undeformed granodiorite-tonalite), late-tectonic to post-tectonic/post-collision granites (mylonitized muscovite granite, biotite granite and perthitic alkali granite), and post-granitic dykes (pegmatites and quartz veins). The tectonic structures in the studied regions indicate at least three successive phases of deformation (D1, D2, and D3). D1 represents NE-SW (to ENE-WSW) shortening associated with major thrusting of the supracrustal rocks over the infracrustal rocks in Nugrus-Hafafit area and NW-SE shortening accompanied the orogen-parallel extension/extrusion in El Shalul area. D2 is characterized by NW-SE (to NNW-SSE) transpression along the Najd-related shear zones. During D3, the NW-SE (to NNW-SSE) shortening produced dextral shearing along the NE-SW Mubarak-Barramiya Shear Belt (MBSB). Field relations and observations indicate that the NW Najd-related Nugrus and El Shalul shear zones and the NE-oriented shears are not a conjugate pairs. Furthermore, the tectonic-magmatic-metamorphic evolution of the Pan-African belt in the Eastern Desert of Egypt has been progressively changed and/or developed from south to north, in terms of tectonic transportation/thrust direction and deformational style.

The usage of deep leaning came out of its advantage in extracting features well. Ant colony can be run continuously and adapt to changes in real time so it can deal well with the dynamic parameters. Results obtained through the study are promising.

The success of minimally invasive parathyroidectomy (MIP) relies on accurate localization of the abnormal parathyroid glands. Concordant findings on ultrasound (US) and 99mTc-scintigraphy (sestamibi) are currently considered the ‘gold standard’. Computed tomography (CT) has also recently been used in preoperative planning. We sought to assess the accuracy of CT for localization of abnormal parathyroid glands in such patients. An audit of 75 patients with primary hyperparathyroidism (PHPT) who underwent neck US and CT between 2017 and 2019 at our center as their first-line imaging. All 75 patients underwent US and CT and 54 (72.0%) also had sestamibi. CT alone identified a potential target in all patients, of which the location was correct in 63 (84.0%). The overall combined sensitivity of US and CT was 88% (95% CI 78–94) and was higher than the combined sensitivity of US and sestamibi (65% [95% CI 53–76]; p 0.99). The combination of US and CT was able to correctly identify the location of the abnormal parathyroid in 88% of patients and, in comparison with US and sestamibi, had better diagnostic accuracy, especially for smaller and ectopic adenomas. This finding suggests that US and CT could be considered as a first-line imaging modality in patients with PHPT considered for MIP.

The brain is a particularly rare site of metastasis from papillary microcarcinoma, with only few cases described in the literature. We present a case of 59-year-old man who presented with seizures and dysphasia due to left frontal lobe cystic mass, which was excised and turned out on histopathology to be of thyroid origin. Total thyroidectomy was performed and histology showed multifocal papillary microcarcinoma with the largest focus of 3mm with no other adverse features. The patient had ablative radioactive iodine postoperatively, with subsequent exit scan showing no uptake in the brain. Follow-up brain magnetic resonance imaging showed continuous regression of the surgical cavity. Although rare, such cases should be aggressively treated and followed up over the long term, because of reported associated high mortality.

A total of 300 random eggs, representing 60 samples, (30 from baladi hens, and 30 from poultry farms) were collected from different markets, poultry farms, groceries and supermarkets located in Assuit governorate, Egypt. Each egg sample composed of 5 eggs and these samples were examined for the incidence of Helicobacter species using Helicobacter pylori special peptone agar (HPSPA). The results revealed that the incidence of Helicobacter species from examined baladi hen's egg shells was 10 (33.33%), poultry farms hen's egg shells was 9 (30%) and 1(3.33%) from examined baladi hen's egg content samples. H. pylori was isolated in a percentage of 5 (16.67%) from examined baladi hen's egg shells while from poultry farms hen's egg shells was 4 (13.33%). In addition, otherHelicobacter species were isolated from the examined samples as H. cinaedi in a percentage of (6.67%) from examined baladi hen's egg shells and (10%) from examined poultry farms hen's egg shells. H. felis was isolated in a percentage of (6.67%) from both examined baladi and poultry farms hen's egg shells samples and H. pullorum was isolated in a percentage of (3.33%) from examined baladi hen's egg shells. Also, it wasisolated from examined baladi hen's egg contents in a percentage of (3.33%). Identification of ureC gene for H. pylori isolated from both baladi hen's egg shells and poultry farms hen's egg shells by using polymerase chain reaction (PCR) revealed that 8 from 9 samples were positive for H. pylori by using (PCR) in a percentage of (88.89%).

Background:Posterior fossa tumors are the most common solid tumors found in the pediatric population. Approximately, 70–90% of patients with posterior fossa tumors present with hydrocephalus. This presents an interesting problem to pediatric neurosurgeons for which there remains no consensus: What is the best management strategy for hydrocephalus at the time of the initial presentation of the patient? Objective: This multicenter prospective cohort study compared endoscopic third ventriculostomy (ETV) and ventriculoperitoneal shunt (VPS) in the treatment of pediatric patients with secondary obstructive hydrocephalus due to posterior fossa tumors. Patients and Methods: Sixty four patients with posterior fossa tumors (24 medulloblastoma, 12 ependymoma, 27 astrocytoma and 1 schwannoma) associated with secondary hydrocephalus were divided into two equal groups according to the treatment procedure done to the secondary hydrocephalus either ETV or VP shunts, with a follow-up period of 6 months, were prospectively studied in this work. Results: Both procedures (ETV and VP shunt) proved to be effective clinically and radiographically. Endoscopic third ventriculostomy (ETV) proved to be superior due to shorter duration of surgery (17 min versus 58 min), no mortality (0% versus 6.25%), and lower incidence of procedure failure (15.6% versus 21.9%) with highest success rate of 100% was for astrocytomas followed by 80% for ependymomas and 63.6% for medulloblastomas. Conclusion: Both procedures (ETV and VP shunt) proved to be effective clinically and radiographically in the treatment of secondary obstructive hydrocephalus in pediatric patients with posterior fossa tumors. For ETV, the safety, simplicity, a more physiological of internal shunting, a significantly shorter surgery duration, absence of mortality, lower significant complications rate, a lower rate of procedure failure, the significant advantages of not becoming shunt dependent and avoidance of hardware complications (malfunction, obstruction, infection) and making safe handling and resection of the posterior fossa tumors feasible, all the previous factors made the ETV to be recommended as the first choice in the treatment of pediatric patients with obstructive hydrocephalus due to posterior fossa tumors.

Intraoperative monitoring of parathyroid hormone (IOPTH) is a reliable method of predicting the cure of primary hyperparathyroidism (PHPT). The aim of this study is to assess whether common clinical variables (CCV) frequently encountered in patients with PHPT may affect the magnitude of PTH drop or the likelihood of patients meeting the intraoperative cure criterion. Patients who were surgically cured from PHPT caused by single gland disease (SGD) and had full IOPTH protocol (4 measurements) were stratified according to age, gland weight, renal function, vitamin D status and severity of hypercalcemia. The percentage of IOPTH drop and the frequency of patients who had true positive IOPTH test results were compared among groups. 762 patients had surgery for PHPT, of whom 746 were (98%) cured. Of these 746 patients, 511 who had SGD and a full IOPTH protocol were included in this study. The median IOPTH drop was significantly higher among younger patients, those with severe hypercalcaemia at 5, 10, 15 min after gland excision, giant glands (at 5-min only), patients with vitamin D deficiency (at 10, 15 min), and those with normal renal function (at 15 min only). The likelihood of the patients meeting the intraoperative cure criterion was not significantly affected among the groups except in patients with mild hypercalcaemia, who were significantly less likely to have 50% IOPTH drop than those with severe hypercalcaemia at all time points. The frequency of mildly hypercalcaemic patients who met cure criterion was significantly improved by extending measurement to 15 min. IOPTH monitoring has the ability to mitigate the variability of IOPTH kinetics associated with most clinical variables. Mildly hypercalcemic patients in particular may benefit from waiting for 15-min measurement before any surgical decision is made.
