This study sought to investigate the effect of subcontracting on the Performance of Manufacturing Multinational Firms (MNFs) in Nigeria. The study employed a survey research design, through the administration of structured questionnaire to a sample of 397 management staff of the three selected firms. The validity of the instrument was tested using content validity index (CVI) through the assessment of five academics and five top staff of MNFs service firms and it was rated on a two-point scale (relevant and not relevant) which gave a value of .8819. A pilot study was conducted through test re-test method to ascertain the extent of consistency of the research instrument; the result gave a Cronbach alpha of 0.82 which indicated that the instrument is reliable. The findings revealed that, performance elements (cost reduction, production enhancement, market expansion and risk sharing) have both individual and combine effect on subcontracting, with tvalue of 7.02 (0.0000), 9.88 (0.0000), 5.86 and 5.42 (0.0000) respectively. While the F-value of (63.29*0.000), (96.96*0.000), (34.52*0.000) and (29.49*0.000) respectively, suggests that the model is reliable for decision and policymaking. It further indicated that subcontracting has a positive and significant effect on MNFs performance It is therefore recommended that, MNFs should continuously improve and adopt subcontracting policy towards sustainable performance and the government through the ministry of trade, commerce and industry should encourage both poly and geo centric orientations in subcontracting practices of MNFs. Background. Knowledge and attitudes concerning eye disorders are important socio-demographic parameters critical for eye care uptake and outcomes, especially in underserved populations. Objectives. To assess knowledge and attitudes concerning common eye disorders in a rural south-eastern Nigerian population. Methods. The study was a population-based survey of knowledge and attitudes concerning common eye diseases conducted in Abagana community in September 2011. Results. The participants comprised 263 males and 238 females. Their profile of good knowledge was eye infections: 38.9%; refractive error: 28.9%; cataracts: 18.0%; diabetic retinopathy: 8.0%; glaucoma: 5.8%; and overall good knowledge: 4.0%. Prevention and treatment of eye disorders were rated as first priorities over other major disabilities by 91.4% participants. Older age and formal education were the significant predictors of good knowledge. Conclusion. In Abagana, there is poor overall and disorder-specific knowledge of eye disorders. Socio-demographic factors are important determinants of knowledge. Community-based preventive eye health education is underscored. [ABSTRACT FROM AUTHOR]

Objective: To investigate the associations between time to presentation, and sociodemographic and clinical characteristics of new adult ophthalmic outpatients at the University of Nigeria Teaching Hospital (UNTH), Enugu, between March and August 2010. Study design: Hospital-based case-control study. Methods: Consecutive new ophthalmic outpatients at UNTH were categorized into controls (early presenters, i.e. <3 months after onset of current eye disease) and cases (late presenters, i.e. >3 months after onset of current eye disease). Relevant data were obtained from the participants' case notes and interviews. Descriptive statistics yielded frequency distributions; bivariate and multivariate comparisons were used to test the significance of associations. P < 0.05 was considered to indicate significance. Results: Eight hundred and twenty-four subjects [454 males and 370 females, mean age 39.2 (standard deviation 1.2) years, range 19-82 years] participated in this study. There were 370 early presenters and 454 late presenters. Multivariate analysis found that late presentation was significantly associated with age >50 years [odds ratio (OR) 1.34, 95% confidence interval (Cl) 1.28-2.22; P < 0.01], female gender (OR 1.63, 95% Cl 1.52-2.11; P < 0.01), residence >20 km from UNTH (OR 0.65, 95% Cl 0.38-0.89; P = 0.0212), individual-level deprivation (OR 0.69, 95% Cl 0.55-0.92; P = 0.0324) and subnormal visual acuity at presentation (OR 1.32, 95% Cl 1.05-1.76; P = 0.0353). Conclusions: At UNTH, most new adult ophthalmic outpatients present >3 months after onset of their current eye disease. Measures to overcome age-, gender- and distancerelated causes of late presentation, grassroots economic empowerment of the feeder population and public education about eye health are required. An equation for sludge dewatering using FMTLxLyLz dimensional analysis approach has been presented. A pilot plant made of 12mm thick steel plate was used in designing a drying bed, 1.20m long, 0.75m wide, 0.80m deep with a total area of 0.90m2 and was used in dewatering sewage sludge from the University of Nigeria, Nsukka, waste water treatment plant. Parameters affecting filtration processes were used to derive an equation using dimensional analysis approach. The derived equation based on the concept of specific resistance, was used to investigate the performance of the sand drying bed as a sludge dewatering option. On verification using the experimental data obtained from the sewage plant, the theoretical prediction of the derived equation gave a close relationship to the practical values with a correlation of 0.994. The specific resistance from the experimental data was found to increase as the hydrostatic pressure increases numerically. Also, when ferric chloride was used to verify the effect of chemical conditioner on the sludge, it was observed that the specific resistance decreases with increasing dosage of chemical with the following results: Dosage increase of 10g, 20g, 30g, 40g and 50g of ferric chloride gave specific resistance decrease of 1.2271 × 108 m/kg, 0.4998 × 108 m/kg, 0.29803 × 108 m/kg, 0.18124 × 108 m/kg, 0.075466 × 108 m/kg respectively. Equally, when the effect of dilution was investigated, the specific resistance showed a decrease in value with increasing initial solid content. Palmar fasciitis and polyarthritis syndrome (PFPAS) is an exceedingly rare rheumatologic condition characterized by fibrotic changes in the palmar fascia with joint pains. It is known to be associated with gynecological malignancy, especially ovarian adenocarcinoma, gastric cancer, pancreatic, prostate, breast, and lung cancer. We present a unique case of a 75-year-old Caucasian female with PFPAS preceding the diagnosis of ovarian cancer by eight months. Our case highlights the importance of considering PFPAS as a potential paraneoplastic syndrome. Background Arthritis is a prevalent, chronic condition with significant implications for morbidity and healthcare utilization. Understanding trends in arthritis prevalence and associated chronic health indicators is vital for informing public health interventions and healthcare policies. Objective This retrospective study aimed to analyze trends in arthritis prevalence and associated chronic health indicators among adults using data from the Behavioral Risk Factor Surveillance System (BRFSS) database. Methods This retrospective study utilized data from the BRFSS database covering 2019 to 2022. Participants included United States adults aged 18 years and older who completed BRFSS surveys during the specified period. Primary variables included arthritis prevalence and its correlation with chronic health indicators and demographics. Data collection involved standardized telephone questionnaires administered annually, with rigorous attention to data quality and consistency. Prevalence estimates were calculated using weighted proportions, and statistical analysis utilized analysis of variance (ANOVA). Results The study revealed relatively stable arthritis prevalence over the study period, with notable demographic variations. Arthritis prevalence remained stable (2019: 43.3%, 2021: 42.5%). Females consistently had higher rates than males (2019: 45%, 2021: 44.9%). Activity limitation, joint pain, and work limitation were more prevalent in arthritis patients. White, non-Hispanic individuals had higher rates than other groups. Physical inactivity increased from 2019 (29.4%) to 2022 (72.4%), particularly in males. Counseling for physical activity was lower in males. Targeted interventions are needed to address these disparities and improve arthritis management. Conclusion This study provides insights into trends in arthritis prevalence and associated chronic health indicators among United States adults. The findings underscore the importance of considering demographic factors in arthritis prevention and management strategies. Introduction: Using software for self-management interventions can improve health outcomes for individuals with low back pain, but there is a dearth of research to confirm its effectiveness. Additionally, no known research has evaluated the effective elements of software-based interventions for low back pain self-management components. This study aimed to synthesize the effectiveness of software-based interventions to promote self-management health outcomes among individuals with low back pain., Design: A systematic review and meta-analysis was conducted., Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Statement, relevant studies up to July 2022 were searched via four electronic databases: PubMed, Embase, Cumulative Index to Nursing and Allied Health Literature, and Web of Science., Results: 4908 adults with low back pain who participated in 23 studies were included. Software-based interventions were effective in reducing fear avoidance (mean difference [MD] = -0.95, 95% CI: -1.45 to -0.44), pain catastrophizing (MD = -1.31, 95% CI: -1.84 to -0.78), disability (MD = -8.21, 95% CI: -13.02 to -3.39), and pain intensity (MD = -0.86, 95% CI: -1.17 to -0.55). Specifically, interventions that included an exercise component were more effective in reducing pain and disability. Additionally, cognitive behavioral therapy (CBT) intervention significantly reduced fear avoidance and pain catastrophizing but had no noticeable impact on disability and pain compared to standard treatment. The certainty of the evidence in this review varied from very low to high across outcomes. The heterogeneity of the study results was significant, suggesting that future studies in this area could optimize the design, time points, measures, and outcomes to strengthen the evidence., Conclusions: Low back pain self-management interventions delivered through software-based programs effectively reduce pain intensity, disability, fear avoidance, and pain catastrophizing., Clinical Relevance: Low back pain is among the most common reasons for seeking healthcare visits. Purpose: The purpose of this meta-analysis is to study the impact of varicocele repair in the largest cohort of infertile males with clinical varicocele by including all available studies, with no language restrictions, comparing intra-person conventional semen parameters before and after the repair of varicoceles., Materials and Methods: The meta-analysis was performed according to PRISMA-P and MOOSE guidelines. A systematic search was performed in Scopus, PubMed, Cochrane, and Embase databases. Eligible studies were selected according to the PICOS model (Population: infertile male patients with clinical varicocele; Intervention: varicocele repair; Comparison: intra-person before-after varicocele repair; Outcome: conventional semen parameters; Study type: randomized controlled trials [RCTs], observational and case-control studies)., Results: Out of 1,632 screened abstracts, 351 articles (23 RCTs, 292 observational, and 36 case-control studies) were included in the quantitative analysis. Introduction: Yoga has been shown to reduce pain and improve function in populations with chronic low back pain (cLBP), yet the underlying molecular mechanisms remain elusive. This study examined the feasibility and acceptability of a yoga research protocol, including recruitment, retention, and data collection, and investigated the preliminary effects of yoga on psychological and neurophysiological functions, including gene expression and DNA methylation profiles, in participants with cLBP., Methods: A one-arm trial was conducted with 11 participants with cLBP who enrolled in a 12-week yoga intervention. Data on subjective pain characteristics, quantitative sensory testing, and blood for analysis of differentially expressed genes and CpG methylation was collected prior to the start of the intervention and at study completion., Results: Based on pre-determined feasibility and acceptability criteria, the yoga intervention was found to be feasible and highly acceptable to participants. There was a reduction in pain severity, interference, and mechanical pain sensitivity post-yoga and an increase in emotion regulation and self-efficacy. No adverse reactions were reported. Differential expression analysis demonstrated that the yoga intervention induced increased expression of antisense genes, some of which serve as antisense to known pain genes. In addition, there were 33 differentially hypomethylated positions after yoga (log2 fold change ≥ 1), with enrichment of genes involved in NIK/NF-kB signaling, a major pathway that modulates immune function and inflammation., Discussion/conclusions: The study supports the feasibility and acceptability of the proposed protocol to test a specific mechanism of action for yoga in individuals with cLBP. Bias in healthcare negatively impacts disparities in care, treatment, and outcomes, especially among minority populations. A scoping review of the literature was performed to provide a deeper understanding of how bias influences musculoskeletal pain and potential effects of bias-targeted interventions on reducing pain disparities, as well as identify gaps and make suggestions for further research in this area. Publications from peer-reviewed journals were searched using the databases PubMed/MEDLINE, PsycINFO, CINAHL, and Scopus, with 18 studies identified. The literature review revealed that clinician-based bias and discrimination worsen pain and disability by reducing access to treatment and increasing patient pain-related injustice, catastrophizing, depression, and perceived stress. In contrast, clinician education and perspective-taking, patient decision tools, and community outreach interventions can help reduce bias and disparities in musculoskeletal pain outcomes. Increasing the diversity of the healthcare workforce should also be a priority. Chronic low back pain is one of the most common, costly, and debilitating pain conditions worldwide. Increased mechanistic understanding of the transition from acute to chronic low back and identification of predictive biomarkers could enhance the clinical assessment performed by healthcare providers and enable the development of targeted treatment to prevent and/or better manage chronic low back pain. This study protocol was designed to identify the neurological and transcriptomic biomarkers predictive of chronic low back pain at low back pain onset. This is a prospective descriptive longitudinal inception cohort study that will follow 340 individuals with acute low back pain and 40 healthy controls over 2 years. To analyze the neurophysiological and transcriptomic biomarkers of low back pain, the protocol includes psychological and pain-related survey data that will be collected beginning within 6 weeks of low back pain onset (baseline, 6, 12, 24, 52 weeks, and 2 years) and remotely at five additional time points (8, 10, 16, 20 weeks, and 18 months). Quantitative sensory testing and collection of blood samples for RNA sequencing will occur during the six in-person visits. Introduction: mental health of students deteriorate when they begin studies in a medical school. The study aimed to determine the prevalence of depression and associated factors among medical students in a university in Nigeria., Methods: a cross-sectional study design was used. All matriculated medical students of Ebonyi State University Abakaliki, Nigeria were included in the study. Information was obtained using a self-administered structured questionnaire. Beck´s depression inventory was used to determine the prevalence of depression. Chi square test was used to ascertain association between variables. Level of statistical significance was determined by p value of <0.05., Results: the mean age of the students was 23.2 ± 3.3 years and majority, 60.2% were males. Prevalence of depression was 17.4%. Third-year class (major examination class) had the highest proportion of students who were depressed, 24.2% while final year class had the least, 8.8%. Coping mechanisms for low moods included sleeping, 50.4% and having interactions with colleagues, 46.9%. Factors associated with depression included being <25 years, (p=0.008), being in the pre-clinical school, (p=0.023) and being afraid some students may not graduate from medical school (p=0.030)., Conclusion: burden of depression was high among the students and most pronounced among third year students. There is need for proper orientation of newly admitted medical students on the medical curriculum by authorities of the university. Efforts should be made to decrease undue anxiety among students especially during examination. Introduction . There is insufficient literature on the prevalence of and risk factors for work-related musculoskeletal disorders (MSDs) among teachers in Nigeria. This study aimed to investigate the prevalence of work-related MSDs and their associations with occupational and socio-demographic factors of Nigerian-based school teachers. Materials and methods . Socio-demographic and occupational characteristics, prevalence and pattern of work-related MSDs of 352 school teachers (age 24-60 years) were investigated using a standard Nordic questionnaire. Data were summarized with descriptive statistics. Univariate and multivariate analyses were used to assess predictors of work-related MSDs. Results . The results showed that 70.2% of the teachers had work-related MSDs predominantly in the shoulder (62.3%) and neck (57.9%) regions. Most of the studied socio-demographic features and anthropometric and occupational characteristics were significantly associated ( p  < 0.05) with MSDs in at least one body region. Conclusions . There was a prevalence of work-related MSDs among the teachers. Advanced educational qualification, elevated teaching boards, teaching experience over 10 years and age range of 35-50 years were the major predictors of MSDs.

Background: Effective acute pain management strategies are important for young adults in order to reduce risk for transition to chronic pain., Aim: To describe pain and pain self-management strategies used by African American young adults., Design & Setting: A national online cross-sectional survey design was used., Participant/subjects: Ninety-four African Americans Qualtrics panelists ages 18-25 who reported previous experience with acute pain responded. Methods: Respondents completed the Brief Pain Inventory Short Form to describe their pain intensity, pain interference with function, pain self-management, and percent of relief obtained from their self-management., Results: African American young adults reported pain primarily in the back (n = 22, 23.4%) and head (n = 19, 20.2%), with moderate pain intensity M = 4.5 (standard deviation [SD] = 1.79) and pain interference with function M = 4.6 (SD = 2.36). African American young adults described their worst pain in the last 24 hours as M = 5.7 (SD = 2.01), least pain as M = 3.4 (SD = 2.41), and average pain as M = 5.1 (SD = 2.09). They reported 61.3% pain relief from self-treatment. A total of 45 (47.9%) reported no pain self-management strategies., Conclusions: African American young adults report moderate levels of pain intensity and pain interference with function. A significant number report no pain self-management strategies. Focused pain assessment and education about efficacious pain self-management strategies, both pharmacological and complementary, could assist young African Americans to reduce their pain and risk of chronic pain in the future., (Copyright © 2020 American Society for Pain Management Nursing. Background/objective: The study's objective was to determine the efficacy of guided internet-assisted intervention (GIAI) on depression reduction among educational technology students of Nigerian universities., Methods: The design of the study was a 10-weeks group-randomized trial (GRT) which involved a pre-test, post-test, and follow-up assessment. A total of 192 educational technology students who were identified as having depression formed the sample of the study. Beck's Depression Inventory-II (BDI-II) was the measure used for data collection in the study. Data collected were analyzed using ANOVA with repeated measures., Results: The initial assessment results showed that the participants in both treatment and usual-care control groups had depression. After 10-weeks participation in GIAI, the assessment results showed a significant reduction in depression among students in the treatment group when compared to those in the usual-care control group. The follow-up assessment indicated a further significant reduction in the depression among participants in the treatment group when compared to those in the usual-care control group., Conclusion: The authors concluded that GIAI was significantly effective in reducing depression among university students in the treatment group compared to those in the usual-care control group. Therefore, educational technologists, counselors, psychologists, health workers, and other social workers should adopt educational intervention using GIAI in helping university students undergo depression reduction.

Background: There is stigma attached to the identification of residents carrying antimicrobial resistant organisms (ARO) in long term care homes, yet there is a need to collect data about their prevalence for public health surveillance and intervention purposes., Objective: We conducted a point prevalence study to assess ARO rates in long term care homes in Ontario using a secure data collection system., Methods: All long term care homes in the province were asked to provide colonization or infection counts for methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant enterococci (VRE), and extended-spectrum beta-lactamase (ESBL) as recorded in their electronic medical records, and the number of current residents. Data was collected online during the October-November 2011 period using a Paillier cryptosystem that allows computation on encrypted data., Results: A provably secure data collection system was implemented. Overall, 82% of the homes in the province responded. MRSA was the most frequent ARO identified at 3 cases per 100 residents, followed by ESBL at 0.83 per 100 residents, and VRE at 0.56 per 100 residents. The microbiological findings and their distribution were consistent with available provincial laboratory data reporting test results for AROs in hospitals., Conclusions: We describe an ARO point prevalence study which demonstrated the feasibility of collecting data from long term care homes securely across the province and providing strong privacy and confidentiality assurances, while obtaining high response rates.

Background: There are many benefits to open datasets. However, privacy concerns have hampered the widespread creation of open health data. There is a dearth of documented methods and case studies for the creation of public-use health data. We describe a new methodology for creating a longitudinal public health dataset in the context of the Heritage Health Prize (HHP). The HHP is a global data mining competition to predict, by using claims data, the number of days patients will be hospitalized in a subsequent year. The winner will be the team or individual with the most accurate model past a threshold accuracy, and will receive a US $3 million cash prize. HHP began on April 4, 2011, and ends on April 3, 2013., Objective: To de-identify the claims data used in the HHP competition and ensure that it meets the requirements in the US Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule., Methods: We defined a threshold risk consistent with the HIPAA Privacy Rule Safe Harbor standard for disclosing the competition dataset. Three plausible re-identification attacks that can be executed on these data were identified. For each attack the re-identification probability was evaluated. If it was deemed too high then a new de-identification algorithm was applied to reduce the risk to an acceptable level. We performed an actual evaluation of re-identification risk using simulated attacks and matching experiments to confirm the results of the de-identification and to test sensitivity to assumptions. The main metric used to evaluate re-identification risk was the probability that a record in the HHP data can be re-identified given an attempted attack., Results: An evaluation of the de-identified dataset estimated that the probability of re-identifying an individual was .0084, below the .05 probability threshold specified for the competition. The risk was robust to violations of our initial assumptions., Conclusions: It was possible to ensure that the probability of re-identification for a large longitudinal dataset was acceptably low when it was released for a global user community in support of an analytics competition. This is an example of, and methodology for, achieving open data principles for longitudinal health data.

Background: Ptergium is a frequently encountered clinical entity in ophthalmic practice. Although commoner in the tropics compared with the temperate region, all Ophthalmologists worldwide should be conversant with the clinical features and outcome of various approaches to surgical management. A review of the efficacy of the currently available surgical procedures for pterygium excision using postoperative recurrences as the main outcome measure is presented., Method: Manual and electronic (PUBMED, MEDLINE, AJOL AND HINARI) search for medical literature relevant to clinical features and surgical treatment of pterygium was conducted. Previous reviews, large case series reports, randomized controlled trials and one meta-analysis were selected and critically reviewed., Results: Postoperative recurrence is a dominant complication of pterygium excision irrespective of the adopted surgical procedure. Pterygium excision with adjunctive therapy and ocular surface reconstruction is the current procedure of choice in view of its comparatively higher efficacy in preventing recurrence., Conclusion: Recurrence is the commonest postoperative complication of pterygium excision. There is need for further research into the aetiopathogenesis and risk factors for recurrence of human pterygia.

Introduction: Among the refractive errors, myopia is a leading cause of visual impairment throughout the world and its prevalence is on the increase. Pathological myopia is a genetically determined refractive error and a growing body of evidence shows that visual experiences early in life may affect ocular growth and eventual refractive error. In addition to the human cost of visual debility, there is a profound economic cost to the society. The current review highlights recent advances in the management of myopia., Methods: Information was obtained by searching Medline for citations of articles in English using the Keyword "Myopia" in addition to current literature review., Result: Myopia can be classified into two groups, low to moderate degree of myopia (refered to as simple myopia-0.5 to -6.0 dioptres) and high or pathological myopia (greater than 6.0 dioptres). Simple myopia can be corrected with spectacles or contact lenses whereas high myopia may be complicated by potentially blinding conditions such as vitreous and macular degeneration and retinal detachment., Conclusion: Recent advances in the management of myopia have made it possible to restore vision to a reasonable degree.

Arrested development of Haemonchus, Cooperia and Trichostrongylus spp. was studied in (1) 14 naturally infected and eight experimentally infected West African Dwarf (WAD) goats reared in the derived savanna zone of eastern Nigeria and (2) 55 naturally infected slaughter goats obtained from the savanna and sahel regions of northern Nigeria. Six of the WAD goats carried natural infections of H. contortus and T. colubriformis and eight other (tracer) goats acquired their infections from a grass paddock artificially contaminated with H. placei, C. pectinata and C. punctata, during May to October. Another three WAD goats were artificially infected with mixed cultures of L3 of the latter three nematodes, while five goats were inoculated with 1500-2000 L3 of H. contortus harvested from cultures incubated at 25-30 degrees C for 8 days either in the dark or under normal laboratory conditions. Approximately 41% (9/22) of the infected WAD goats harboured arrested larvae of Haemonchus and/or Cooperia. No arrested larvae of Trichostrongylus were found in the six animals that were infected with this nematode. The level of inhibition varied from 0.4 to 20% and only three animals showed greater than 10% inhibition. This very low level of inhibition occurred in naturally and experimentally acquired infections, irrespective of the time of year. In the case of Haemonchus, the species and strain of the parasite and infection with L3 cultured in the dark also appeared not to influence the level of inhibition. By contrast, 65.5% (36/55) and 5.5% (3/55) of the northern savanna and sahel goats harboured arrested larvae of H. contortus and T. colubriformis, respectively. The mean percentage inhibition of the former was low (2-25%) during most of the rainy season (June-August) and high (75-90%) during the late rains and the dry season (October-April). The lowest and highest mean percentage inhibitions occurred during July and November, respectively.
