Rupture of the pyelocalyceal cavities or fornix is a rare and potential urological emergency. It is most often secondary to obstructive uropathy. It can lead to extravasation of urine at the perirenal or retro-peritoneal level, which can have serious consequences. The clinic reported the case of a 37-year-old patient who presented to the emergency department with hyperalgesic (right) renal colic. Intense right lumbar tenderness was found on clinical examination, renal function was preserved. The radiological examination, CT scan mainly, revealed a significant ureteralhydronephrosis by urinary meatus calculus measuring 3 mm with a density of 271 HU and a perirenal effusion and extravasation of the PDC at a late time. The emergency therapeutic management consisted of a bypass with a double J stent. The evolution was marked by a clinico-biological improvement.

Urinoma is defined as the extravasation of urine from any part of the urinary collecting system, which causes lipolysis of the surrounding fat, resulting in an encapsulated collection. The most common cause of urinoma is the direct obstruction of the urinary system. The other etiologies include trauma and postinstrumentation/surgery. Parenchymal cause for spontaneous urinoma is exceedingly rare. We present a case of a 30-year-old gentleman who presented with lower abdominal pain and was treated with a Diclofenac injection. The pain got better temporarily, but he presented again with right loin pain after 3 days. His computed tomography scan images revealed bilateral perinephric urinoma. As there was significant deterioration of renal function, he underwent a renal biopsy. The histopathology was reported as acute interstitial nephritis (AIN). Drug-induced AIN is very rarely presented with acute loin or abdominal pain due to spontaneous perinephric urinoma, as it was seen in our case.

Introduction and Background: Spontaneous rupture of the urinary collecting system with extravasation of urine is a rare complication of obstructive stone disease. Most of the cases are caused by obstructive ureteral stones. We herein present a case of a spontaneous caliceal rupture with a large perirenal urinoma formation that was silent on presentation and managed with endoscopic stenting and percutaneous catheter drainage. Case Presentation: A 56-year-old man presented with complaints of vague right flank discomfort. A noncontrast CT scan revealed a 9.4 mm right mid ureteral obstructive calculus with a 14 cm collection in the perirenal space communicating with the lower calix of the right kidney. Retrograde insertion of 6F Double-J stent was done endoscopically and a pigtail catheter was placed in the right perinephric collection. Initially the catheter drained 100 mL clear urine and decreased progressively. A repeat ultrasonography revealed no collection and the catheter was removed after 10 days. The patient underwent clearance of stones after 8 weeks. On table, retrograde pyelogram showed no leak. The patient is doing well 2 weeks postoperatively. Conclusions: Obstructive ureteral stone presenting with spontaneous forniceal rupture and large perinephric collection in a silent manner. Although endoscopic management alone offers excellent results in small ruptures, diversion of the collecting system with drainage of the collection remains the mainstay of treatment in large urinomas to prevent complications. Definitive management of the cause of obstruction is paramount and should be done after complete healing of the rupture.

Extravasation of urine following rupture of the renal fornix is a rare complication mostly caused by obstruction secondary to distal ureteric stones. We report the case of a rupture of FORNIX secondary to a bladder tumor.

Background: Urinary fistula is a common complication of kidney transplantation. It may cause severe complications like graft loss and patient death.Case presentation: Here we present a recipient who developed graft ureteral necrosis with leakage of urine into the superficial perineal space but not into the peri-graft or the retroperitoneal space, as an unusual clinical feature. This may be due to dense adhesions in the peri-graft region and dam-like effect of adhesive spermatic cord and inferior epigastric vessels traversing across the graft ureter. Urine leaking out with high pressure extravasates through the potential gaps in the incision of aponeurosis at the lateral margin of the rectus abdominis and into the superficial perineal space. The proximal ureter of the graft was end-to-end anastomosed to the right ureter. Both the recipient and graft recovered well.Conclusions: Comparing the creatinine levels in the leaking fluid and serum is an effective way to confirm urinary fistula. Early aggressive management is a rational option for treatment of urinary fistula.

A 64-year-old female presented to the ED with severe abdominal pain. Initially it was suspected to be spontaneous aortic rupture or dissection. Contrasted CT imaging studies did not identify aneurysm and dissection, but did identify a concerning, confounding, and curious collection of fluid in the upper right quadrant. Angiography imaging was obtained and identified the origin as the collecting duct of the right kidney. The patient was admitted to the medical service. She was then evaluated by the urology service and they identified this presentation as a urinoma with extravasation of urine, in the absence of trauma or identifiable obstruction.

We report three cases of iatrogenic ureteral injury associated with total laparoscopic hysterectomy. Case 1 was a 50-year-old woman. She underwent total laparoscopic hysterectomy (TLH) for myoma uteri. Postoperatively, a blood test revealed renal dysfunction, and ultrasonography (US) revealed left hydronephrosis. She was referred to our department on the next day of TLH. Computed tomography (CT) revealed left hydronephrosis. Retrograde pyelography (RP) was tried, but the ureteral catheter could not be inserted into the left ureteral orifice. An operation was performed on the second day after THL. The left ureter was ligated near the ureterovesical junction, and so uretero-ureterostomy was conducted. Case 2 was a 38-year-old woman. She underwent TLH for myoma uteri. Postoperatively, she complained of abdominal fullness and diarrhea. A blood test revealed renal dysfunction and US revealed left hydronephrosis on the ninth day after THL. She was referred to our department next day. CT revealed left hydronephrosis and ascitis. RP revealed extravasation of contrast medium from the left ureter. She was diagnosed with left ureteral injury accompanied by extravasation of urine into the intraperitoneal space. Uretero-cystoneostomy was performed. Case 3 was a 45-year-old woman. She underwent TLH for myoma uteri. Postoperatively, a blood test revealed renal dysfunction, and US revealed left hydronephrosis. She was referred to our department on the fifth day after TLH. CT revealed left hdronephrosis and ureteral obstruction of the left lower ureter. An operation was performed under laparoscopy, cystoscopy, and fluoroscopy on that day and the left lower ureter was kinked by threads. Cutting of the threads was performed under laparoscopy. After thread cutting, the kink of the ureter was improved and ureteral stent could be indwelled between the renal pelvis and bladder. After 1 month, the ureteral stent was removed and left hydronephrosis disappeared.

We present a case of a 45-year-old female who experienced rupture of a right calyceal diverticulum caused by ureteroscopy. Fifteen hours after the operation, she had severe right flank pain and a high fever (38.9°C). Computed tomography revealed perinephric extravasation of urine and bleeding inside the diverticulum. We diagnosed rupture of a calyceal diverticulum; therefore, we continued antibiotic administration and pain relief medication. She became afebrile on postoperative day 4 and was discharged from the hospital on postoperative day 7. Owing to renal cortex thinning in the diverticula, ureteroscopy is associated with a risk of rupture of calyceal diverticula.

Background Renal colic is a frequently encountered diagnosis among emergency department patients. Point-of-care (POC) ultrasound has taken on an increasingly important role in the diagnostic strategy of this disease, as it has been shown to be an effective modality for diagnosis and for guiding management. The primary pathology that the emergency physician will evaluate for in cases of suspected renal colic is hydronephrosis, which is a dilatation of the renal pelvis and calyces resulting from a distal obstruction such as a ureteral stone. However, other significant findings can be seen, one of which is the extravasation of urine around the kidney secondary to a forniceal, or calyceal, rupture. Case Series We present three cases of unanticipated perinephric fluid collections identified initially on POC ultrasound in cases of suspected simple renal colic. Concomitant hydronephrosis was also seen in each of these cases. Why Should an Emergency Physician Be Aware of This? While the utilization of POC ultrasound in cases of suspected renal colic increases, we should be prepared to encounter and identify perinephric fluid collections in our evaluations. Although the ideal management of these cases is not completely defined from the current literature, we benefit from knowing how to identify these on POC ultrasound, understanding the underlying pathophysiology, and appreciating the possibility of complications that may arise.

Percutaneous nephrolithotomy (PCNL) despite its minimally invasive approach has an high complication rate, with the most common complications being extravasation of urine and perioperative bleeding requiring transfusion. While most of these complications are minor, many serious and life-threatening complications do occur. One such complication is the development of hemothorax or hydrothorax which usually develops in the early postoperative period with blood or urine passing from the surgical site through a newly established pleural-peritoneal fistula. Here we describe an unusual presentation and clinical management of delayed onset acute urohemothorax and hemodynamic collapse several days following PCNL.

A posterior urethral valve (PUV) may lead to extravasation of urine, resulting in prenatal ascites and/or perirenal urinoma. Extravasation has been presumed to act as a pop-off mechanism, preserving renal function, but previous reports addressing this presumption have been inconclusive.The present study compares renal function in patients with PUV with and without extravasation.Sixty boys with a confirmed diagnosis of PUV as neonates (gestational age [GA]44 weeks) throughout 2001-2016 were included. Clinical data were collected from medical records. Renal function was assessed by nadir plasma creatinine, creatinine at the last follow-up, and glomerular filtration rate (GFR) at the last follow-up. The GFR was estimated using the Schwartz formula. Renal function was classified according to the kidney disease: improving global outcomes (KDIGO) guidelines' grades of chronic kidney disease (CKD). Glomerular filtration rate90 ml/min/1.73mTwelve patients (20%) had ascites and/or urinoma, and 48 (80%) did not. GA and birth weight were not different in patients with and without extravasation. PUV was suspected from prenatal ultrasound findings in 66.7% of the patients in both groups. Median nadir creatinine was 21 (range, 11-33) μmol/L in boys with ascites/urinoma, and all values were within the age-adjusted reference values. Nadir creatinine was 23 (14-199) μmol/L in boys without extravasation, and it was above the normal range in 14 boys. The incidence of elevated nadir creatinine was significantly different in the two groups (p 0.025). One of the 12 patients with extravasation developed chronic renal failure (CKD 3). In the group of 48 patients without extravasation, 20 (42%) had chronic renal failure grade 2-5, and among these, 5 patients have had a renal transplant (CKD grade 5). The prevalence of CKD grade 2-5 was statistically different in the two groups (p = 0.03). These findings are presented in the summary figure.Extravasation of urine was found in 12 of 60 (20%) boys with PUV. These patients had significantly lower prevalence of CKD at the last follow-up than patients without extravasation. This finding is important in prenatal counseling. It also indicates that prenatal decompression of the bladder and upper tract is beneficial in patients with PUV, which is relevant to the discussion of prenatal intervention in these fetuses.

We report a rare case of extravasation of urine, which may be associated with bilateral complete ureteral duplication, vesicoureteral reflux (VUR), and benign prostatic hyperplasia (BPH). A 71-year-old male presented with a complaint of right abdominal pain. An extravasation of urine was noted, and was improved by indwelling urethral catheterization. Transurethral resection of the prostate and the endoscopic subureteral injection of dextanomer/hyaluronic acid were performed for the treatment of BPH and VUR, respectively. The post-surgery recovery was successful.

Spontaneous rupture of the ureter is an uncommon condition defined by non-traumatic extravasation of urine. Causative factors leading to rupture include urinary calculi, malignancies, instrumentation or trauma. Ureteral rupture can lead to numerous complications, including abscess formation, urinomas, and urosepsis. Minimal literature is available regarding spontaneous ureteral perforation, especially at the ureteropelvic junction. We present a rare case of spontaneous perforation of the ureter at the ureteropelvic junction caused by an undiagnosed non-compliant bladder. This case illustrates a previously undescribed cause of spontaneous ureteral perforation and the importance of prompt identification and treatment.

Background: Osteophytes are bony outgrowths commonly found on lumbar vertebrae. They rarely produce complications with the most common complication being nerve entrapment, but rarer complications including aorta or inferior vena cava rupture, superior mesenteric artery syndrome, compression of the iliopsoas muscle, and cerebrospinal fluid leaks have been described. Rare cases affecting the ureter resulting in ureteral colic or extravasation of urine have been described. Case Presentation: We describe a case in which a lumbar osteophyte bridging the L4 and L5 disks was encircling the ureter and minor trauma caused a ureteral injury, resulting in urine extravasation into the L4 and L5 disks space and the retroperitoneum. Owing to the comorbidities of this patient, this case was treated conservatively with stenting and the patient has suffered no further complications. Conclusion: This is a rare complication of a lumbar osteophyte but should be considered as a potential cause of ureter injury. Treatment should be individualized by patient preference and comorbidities, as some patients would elect to pursue more aggressive therapy whereas others would incline for conservative measures.

Spontaneous extravasation of urine is becoming more frequently recognised. When associated with rupture of the renal pelvis it is more likely to present to a general surgeon as an "acute abdomen" than to a urological surgeon, and unless suspected early will not be diagnosed until after laparotomy has been performed. The case presented demonstrates many of the problems in the diagnosis of this condition, and the management is discussed.

Spontaneous perforation of the renal pelvis and extravasation of urine into the perinephric space is an extremely rare condition in childhood. It is mostly related to underlying congenital urinary tract abnormalities and caused by urolithiasis or infection in the setting of urinary obstruction. Here, we report a case of an 18-month-old female patient with rupture of the renal pelvis by an extruded kidney stone and its management.

SummaryUse of a bilateral caudal superficial epigastric skin flap and perineal urethrostomy for reconstruction of a wound which occurred secondary to rupture of the distal urethra and extravasation of urine is reported. A 10-month-old male neutered Domestic Shorthaired cat was presented with a history of trauma, signs of pain of the hind quarters and anuria. Progression of the clinical signs, including anuria, lethargy, anorexia, and azotemia, prompted referral and investigation 108 hours following the initial injury. Retrograde urethrography indicated a rupture of the distal pelvic urethra with extravasation of urine subcutaneously. Development of extensive skin necrosis of the perineum and rump progressed leaving a large wound defect which was managed with wound debridement and dressing. The wound was closed using staged caudal superficial epigastric skin flaps and perineal urethrostomy as part of the reconstruction.

Backgroud: Primary ureteral tumors are not common. The most common presentations of upper urinary tract urothelial carcinoma are gross or microscopic hematuria. Objective: Report a rare case of acute ureteric colic with perirenal urinoma, secondary to primary ureteric carcinoma. Methods: Case report and review of literature. Result: Perirenal extravasation and ureteric obstruction by tumor was diagnosed by contrast-enhanced computed tomography. The patient was successfully treated with laparoscopic radical nephroureterectomy after the obstruction was relieved by ureteric stent. Adjuvant radiation was done at the renal fossa for local control. There was no evidence of disease recurrence at twelve month after surgery. Conclusion: The renal caliceal fornix rupture is usually associated with acute ureteric obstruction by stone. Rarely, it occurs by primary ureteric tumor. Urinoma is peripelvic extravasation of urine seen as the squeal of urinary trauma or stones. Little is known about maternal urinoma during pregnancy. A 27-year-old primigravida presented with gradual worsening right flank pain at 21(5/7) weeks of gestation. She denied any past medicosurgical history. Initial work-up revealed cost-vertebral angle tenderness and mild hydronephrosis by ultrasonography. Her symptom worsened and follow-up ultrasonography was performed 48 h later, which showed worsened hydronephrosis with large perinephric fluid collection. A Double-J stent was placed under cystoscopy. After the placement, the patient's symptoms improved quickly. The stent was removed three weeks later. The patient delivered vaginally at 39 weeks of gestation without complication. Maternal urinoma during pregnancy developed in the right side of the kidney in five of six reported cases (83.3%), and all were seen in the second half of pregnancy. All showed flank pain as the initial presentation. Ultrasonography for diagnosis was used in half of the patients. Fifty percent underwent Double-J stent before delivery and 16.7% after delivery. All cases delivered at term and 33.3% underwent cesarean delivery. Maternal urinoma is an important differential diagnosis for flank pain during pregnancy. Double-J stent placement was the main management. Close monitoring of the symptom with serial ultrasonography may be the key for diagnosis.

Spontaneous rupture of renal pelvis with urine extravasation is a rare condition and usually associated with obstructing ureteric calculus. It poses diagnostic and therapeutic dilemmas, while a stepwise approach for the confirmation of diagnosis, treatment and follow up is needed. We present a case of a 75-year old male patient who had a renal pelvis rupture with perirenal extravasation of urine due to a 4 mm stone located at the right ureterovesical junction. Diagnosis was confirmed by computed tomography, while the patient was treated successfully with the placement of a percutaneous nephrostomy. A week later a CT- nephrostomography showed the healing of renal pelvis with no extravasation and no evidence of the obstructing stone.

Non-traumatic ureteral rupture has been reported more frequently, resulting from increased intraluminal pressures from distal urinary tract obstruction. We report the case of a 77-year-old man presenting with chronic urinary retention secondary to massive prostatic enlargement through acute kidney injury. Ultrasound scan detected a shallow left perinephric fluid collection with a possible bladder mass, demonstrated on flexible cystoscopy to be a massive median lobe of prostate. Computed tomography confirmed extravasation of urine from the left proximal ureter. In the absence of specific symptoms, the patient had successful conservative management with antibiotics and urinary catheterization for his acute episode, although declined further surgical intervention.

INTRODUCTION: The traditional delayed treatment of iatrogenic complete ureteral obstruction is open surgery. An easy endourological technique, transluminal re-canalization of the ureter by guide-wire puncture under fluoro-endoscopic control, which has been performed on 4 patients, is described. SURGICAL TECHNIQUE: With the guidance of C-arm fluoroscopy, by moving the C-arm to different planes, the tip of the ureteroscope is directed to the correct plane to meet the obliterated proximal end of the ureter and under direct vision, transluminal puncture is performed using the stiff end of a 0.035-inch guide wire. Once the stiff end of the guide-wire is in the lumen of the proximal ureter, an ureteral catheter is introduced over the guide wire, the guide wire is then removed and reinserted through the ureteral catheter with its soft end leading and a double J catheter is inserted. Ureteral stricture, if later encountered, is treated with balloon dilatation. RESULTS: Continuity of the ureter was restored in all 4 patients. The double J stents were removed 6 weeks later and a retrograde pyelography revealed resolution of the hydronephrosis without extravasation of urine. CONCLUSION: Although a very satisfactory result was achieved in our cases, more cases are needed to show if it can be an alternative to conventional surgical repair. However, we believe that this minimally invasive technique can be used for short obliterated ureteral segments and neither delays nor does it preclude further management using open surgery.

Perirenal extravasation of urine is a rare event in congenital uropathies, mostly observed perinatally in the case of infravesical obstruction. We describe a male infant with a primary non-refluxing obstructive left megaureter who developed an acute perirenal extravasation and ascites at the age of 4 weeks. After temporary upper urinary diversion, ureterocystoneostomy was successfully performed. Intraoperatively, the distal ureter showed an extremely narrow lumen over a distance of 3 cm. In the current literature, postnatal management of primary megaureters is preferably conservative. Our patient demonstrates an extremely rare complication demanding early surgical intervention.

Spontaneous rupture of the collecting system with extravasation of urine and urinoma formation is usually associated with urinary tract obstruction by a ureteral calculus. Tumor growth is an extremely rare cause of urinary extravasation. Here we report a case of bilateral obstructive uropathy with a huge spontaneous left retroperitoneal urinoma caused by advanced infiltrative transitional cell carcinoma of the urinary bladder. The point of leakage was located in the left renal pelvis. The urinary leakage ceased after percutaneous nephrostomy drainage, and the patient subsequently underwent radical cystoprostatectomy. Histopathology revealed a high-grade urothelial carcinoma of the urinary bladder with pelvic lymph node metastasis. The patient refused any adjuvant treatment and expired 6 months after the operation from disseminated metastasis from bladder cancer.

Pleural effusion caused by post-traumatic extravasation of urine from the abdominal cavity to the pleural cavity (urothorax) is an uncommon complication following traumatic injury. To the authors' knowledge, this is the first report of a case of traumatic urothorax in a dog presented with pleural and abdominal urine effusion. Combined urothorax and uroabdomen should be included in the differential-diagnosis list for dogs with recent trauma and a bicavitary effusion. The diagnosis can be confirmed by elevated creatinine concentrations in both effusates, compared to its serum concentration.

An 82-year-old man was admitted because of recurrent swelling, local erythema, itch, desquamation, and ulcer in both legs for 2 years. He reported having no systemic disease. Physical examination revealed multiple healed wounds with eschar on his lower legs. With a provisional diagnosis of frequent recurrent cellulitis and to rule out occult venous deficiency, he had venography with 55 ml contrast medium injection, followed by computed tomography (CT) with a catheter placed in the iliac vein and 115 ml contrast medium administration to demonstrate the relationships of veins and arteries. An incidental finding of bilateral peripelvic extravasation of urine from the kidneys was noted (Figure 1). The distended urinary bladder and enlarged prostate were demonstrated by CT. On the right side the contrast medium leaked from the renal calix and on the left side from the renal pelvis (Figure 2). The CT image showed no stones in bilateral ureters and no mass lesions in the abdomen. The enlarged prostate causing urinary outlet obstruction and rapid increasing fluid volume induced bilateral peripelvic extravasation of urine. The patient felt mild flank pain in the meantime. A Foley catheter was inserted immediately and more than 1000 ml urine was collected. No fever or leukocytosis in the urine was noted over the following days. The patient was treated with observation and supportive treatment. No specific symptoms and no more flank pain were noted, and he was discharged uneventfully. The patient was then transferred to the urology clinic and received transurethral resection of prostate 2 months later.

STUDY DESIGN: A case report of xanthogranulomatous funiculitis and epididymo-orchitis. Xanthogranulomatous inflammation is an uncommon, non-neoplastic process characterised by destruction of tissue, which is replaced by a striking cellular infiltrate of lipid-laden macrophages. CASE REPORT: A 21-year male sustained complete tetraplegia at C-6 level, after a fall in 1998. The neuropathic bladder was managed with an indwelling urethral catheter. He had many unsuccessful trials of micturition. Sixteen months after the cervical injury, he noticed swelling of the left side of the scrotum following removal of a blocked catheter. He was prescribed antibacterial therapy. Four weeks later, physical examination revealed a hard and irregular swelling encompassing the testis, epididymis and spermatic cord. The clinical diagnosis was epididymo-orchitis progressing to pyocele. Through a scrotal incision, the swollen testis, epididymis and diseased segment of the spermatic cord were removed en masse. Histopathology showed extensive areas of necrosis, with xanthogranulomatous inflammation in the spermatic cord and to a lesser extent in the testis/epididymis. CONCLUSION: Repeated episodes of high-pressure urinary reflux along the vas deferens during dyssynergic voiding, and subsequent interstitial extravasation of urine together with chronic, low-grade, suppurative infection possibly led to development of xanthogranulomatous inflammation in the testis and the epididymis. Since tissue destruction is a feature of xanthogranulomatous inflammation, the definite and curative treatment is either complete (or, where applicable, partial) excision of the affected organ in most of the cases.

Iatrogenic ureteral injuries are among the most serious complications in gynecologic surgery. With the increasing popularity of laparoscopic gynecologic surgery, the incidence of ureteral injuries is on the rise. We report 2 cases of post laparoscopic-assisted vaginal hysterectomy (post LAVH) ureterovaginal fistulas, which were managed successfully with retrograde stenting using ureteroscopy. Three middle-aged women who underwent LAVH for symptomatic myomas of the uterus presented with ureterovaginal fistulas in the late postoperative period. Excretory urography revealed ureterovaginal fistulas involving the distal ureter. Retrograde stenting was possible in 2 patients, using a 7.5F rigid ureteroscope. Both patients became continent 2 days after surgery. Urography at 6 weeks revealed normal renal function without obstruction or extravasation of urine, and the stents were removed. Stenting failed in the third patient; the patient underwent a ureteric reimplantation successfully. Post LAVH ureterovaginal fistulas are amenable to ureteroscopic retrograde double-J stenting, which enables spontaneous recovery of the injured ureter. An attempt of ureteroscopic stenting should be considered in all patients with post LAVH ureterovaginal fistulas before subjecting them to other modalities.

We present six cases which illustrate the spectrum of clinical features, macroscopic findings and light microscopic findings of urine extravasation from the upper urinary tract. The early lesions are characterized macroscopically by an oedematous, glistening or gelatinous appearance to the renal perihilar and peripelvic fat. Light microscopically there is lipolysis with associated foamy macrophages, multinucleate giant cells and lymphocytes. Immunohistochemical staining for Tamm-Horsfall protein is strongly positive in the extracellular space and in the foamy macrophages confirming urine extravazation. Later lesions are characterized by cicatrization of fibrous tissue around the renal pelvis and hydronephrosis. Microscopically there is relatively bland fibrosis with occasional lymphocytes and histiocytic cells. The late lesions are also characterized by extracellular deposits of weakly eosinophilic, granular or hyaline material, the so called 'urinary precipitates'. These deposits stain strongly with diastase PAS and weakly positive for Tamm-Horsfall protein. The staining of these urinary precipitates is analogous to renal tubular hyaline casts, thus supporting the theory that they are derived from uroproteins. We consider that these deposits are pathognomic of past urine extravasation.

In eight patients with sudden onset of flank pain, urography showed extravasation of urine. The cause of peripelvic leakage was verified ureteral stone in four cases, stricture of the ureteropelvic junction in one, and unknown in three cases. Treatment was symptomatic in four cases, drainage in two and surgery in two cases. Seven patients recovered uneventfully, but one required nephrectomy. The prognosis in spontaneous urinary extravasation is usually good without drainage. Open surgery is seldom indicated.

Background. Rupture of the urinary collecting system with peripelvic extravasation of urine is an uncommon pathologic condition usually associated with ureteral obstruction from calculi. Case. We report a patient with calyceal rupture and peripelvic extravasation of urine secondary to distal ureteral obstruction by recurrent ovarian carcinoma. Diagnosis was established with computed tomography and renal scans. Placement of an indwelling ureteral stent via a nephrostomy resolved the urinoma. Conclusion. Gynecologic oncologists should be aware that calyceal rupture is a potential complication of gynecologic malignancy. Causes of perirenal urinary extravasation and approaches to diagnosis and management are reviewed.

Spontaneous peripelvic extravasation of urine is relatively uncommon. We experienced 11 cases of spontaneous peripelvic extravasation. Urinary obstruction was caused by calculi in 9 cases, invasion by sigmoid carcinoma in 1 case and ureteral tumour (transitional cell carcinoma) in 1. Six patients presented with signs of peritonitis and 4 out of 9 with calculus did not develop microhaematuria. It is important to differentiate peripelvic extravasation from other acute abdominal conditions. We must pay special attention to patients with signs of peritonitis and normal urinalysis.

Augmentation of the urinary bladder is a popular surgical procedure for increasing the capacity and lowering the intraluminal pressure in patients whose bladder is small, noncompliant, or has high pressure. Among 250 augmentations performed at our hospital, nine patients have had 16 episodes of postoperative extravasation of urine from their augmented bladder. Thirteen episodes were studied by fluoroscopically monitored contrast cystography, which was diagnostic in 12. One patient's perforation was noted during an unrelated operation and the other two died of peritonitis before cystography could be performed. Four episodes occurred early, within 3 weeks of surgery. All were at a surgical anastomosis and presumably represented incomplete healing. Each patient with an early episode of extravasation had a perforation later. Nine perforations occurred from 1.0 to 6.5 months after the augmentation. Of these, three occurred at an anastomosis and four occurred in the augmenting segment. The locations of two were never fully documented. Three perforations occurred late, from 2.5 to 6.0 years after surgery, and all were in the augmenting segment. Three of the nine patients died of complications associated with perforation. Perforation of the bladder is not rare and is a potentially fatal complication of augmentation. Whenever a patient who has undergone augmentation has signs or symptoms suggesting perforation, cystography can play a crucial role in diagnosis and should be performed without delay.

Background and Purpose: Spontaneous perforation of the upper ureter is a rare condition that poses diagnostic and therapeutic problems. We report on five cases from three institutions and discuss the literature. Patients and Methods: Five patients presented with renal colic and the imaging modalities used to assess them showed extravasation of urine. Results: The cause of spontaneous perforation of the ureter was a ureteral stone in one case and was unknown in four cases. In all cases, a Double-J ureteral stent was inserted under fluoroscopy. Urinoma was percutaneously drained in only one patient. Repeat imaging showed normal renal function and morphology in all patients. Conclusion: Spontaneous perforation of the ureter should be suspected after renal colic. Endourologic treatment offers excellent results, even for the management of acute complications.

A urinoma is an encapsulated extravasation of urine. In this case history report, the radiographic and ultrasonographic features of a urinoma in a dog are described. The dog presented with a palpable mass in the right flank which developed following ovariohysterectomy. Urinoma was considered when ultrasonographically, a sharply marginated, anechoic mass immediately caudal to the right kidney, ipsilateral hydroureter and hydronephrosis were detected. Extravasation of contrast medium was not seen during excretory urography. A fluid-filled retroperitoneal mass adjacent to a transected ureter was found at exploratory laparotomy. The diagnosis of urinoma (para-ureteral pseudocyst) and hydronephrosis was confirmed histopathologically.

Rupture of the urinary collecting system associated with peripelvic extravasation of the urine is an unusual condition and commonly associated with obstructing calculus. We report a patient, recently given chemotherapy due to lymphoma, with acute abdomen symptoms. He had a renal pelvis rupture with perirenal extravasation of urine, an uncommon condition due to a stone in the ureter. Diagnosis was suspected on serial ultrasonography, and confirmed by computed tomograhy. Diagnosis, follow-up, and therapeutic approach are discussed.

We report an unusual cause of the pleural effusion due to extravasation of urine from the retroperitoneal space into the thoracic cavity. In our case, the urinoma occurred owing to obstructing urinary tract lesion due to opaque stone. Although rare, urinothorax should be considered when pleural effusion occurs in patients with urinary tract obstruction accompanied by retroperitoneal urinoma.

A 76-year-old man presented with acute urinary retention due to a huge benign prostatic hyperplasia. Although 1.5 litres of turbid urine was initially drained through a urethral catheter, the suprapubic mass persisted and subsequently increased in size. He developed acute retention due to blood clot. Even after the catheter was changed, he remained in retention necessitating emergency prostatectomy. Rupture of the bladder with extravasation of urine and necrotising fasciitis was found at surgery. The prostate gland was very large. It was also located so low and posteriorly that the index finger was unable to enucleate the adenoma. The recti had to be divided transversely to improve access. A metal urethral dilator was used to complete the enucleation. The specimen weighed 246 gm and on histology showed BPH. BACKGROUND A urinoma is a cyst formed by the extravasation of urine from any constituent of the urinary tract; that is, via the kidney, ureter, urinary bladder, or the urethra. It may vary in its site and size according to its etiology, the point of the extravasation, and its duration and time of diagnosis. It commonly is associated with obstruction of the lower urinary tract by an impacted urinary calculus. METHOD Case reports. FINDINGS Two cases of fatal intra-abdominal urinomas in patients with spinal cord injury (SCI). CONCLUSION Complications of SCI place these patients at risk for the development of urinoma. Risk is highest among individuals with recurrent urinary tract infection, stone disease, and obstructive uropathy. Providers need to be alert to this potentially curable condition that may be obscured by the paucity of intra-abdominal findings due to the nature of the spinal cord syndrome.

Perirenal extravasation of urine with an intact renal parenchyma and with dye visualized in the ureter was seen on an excretory urogram (IVP) and computed tomographic (CT) scan in two patients (bilateral in one) with multiple injuries following blunt trauma. Expectant management with no invasive procedures resulted in disappearance of the extravasation within 3 to 5 days. Traumatic rupture of a calyceal fornix is thought to be the cause of the extravasation similar to the perirenal extravasation seen with renal colic from a ureteral calculus.

The so-called "inflammatory" aneurysm constitutes around 5% of all abdominal aortic aneurysms. 1-7 Surrounding structures may become encased in fibrous tissue and in 25% of cases the ureter becomes obstructed, causing hydronephrosis leading to loin pain and in some cases renal failure. 2-4'8-1° We believe that rupture of the ureter with extravasation of urine has not been reported in such a case.

The macroscopic and microscopic features of a uriniferous perirenal pseudocyst in a castrated male 12-year-old American Domestic Short Hair cat are characterized. The development in humans and animals of a perirenal pseudocyst due to the accumulation of urine, lymph, or blood is discussed. In the presented case, microscopic evidence suggests that the pseudocyst developed following formation of a split in the kidney capsule. Emphasis is placed on the development of uriniferous pseudocysts and three proposed criteria for their development: trauma, abnormal openings that allow extravasation of urine, or urethral obstruction.

This is a case report of polytrauma in a 4-year-old boy who sustained a rare aorto-iliac dissection injury along with pelvic fracture and rupture of posterior urethra. This child was initially treated for the rupture of posterior urethra and extravasation of urine with suprapubic cystostomy and drainage. Lower limb ischemia developed later. Exploration showed aorto-iliac dissection injury for which aorto-iliac replacement grafting was carried out. Subsequently delayed repair of posterior urethral rupture by transpubic urethroplasty was carried out.

Background: The kidney is the most frequently injured abdominal organ in children and controversy surrounds some aspects of management. This study looks at the experience of our institution and reviews the literature towards developing an optimal strategy for managing this common childhood injury. Methods: One hundred and forty-two cases of paediatric renal trauma are reported from a catchment population of approximately 240 000 children < 14 years of age over a 12 year period. Injuries were classified into four groups: groups 1 and 2 were regarded as minor injuries (85%) and group 3 and 4 injuries were those with extravasation of urine or pedicle injury (15%). Results: The male:female ratio was 2:I with an average age of 8.5 years. Major renal injuries frequently required large amounts of resuscitation fluid. Associated injuries were present in 41% of all cases with an average of two injuries each. With one exception in each case, the development of complications and the need for early surgery were confined to major injuries. There were no long-term complications. The renal loss rate was 2.1%. Conclusions: Renal injuries can be usefully classified into major or minor by determining whether extravasation of urine or pedicle injury is present. Minor injuries should be managed conservatively. Major injuries causing ongoing haemorrhage require urgent surgery. Other major injuries should be imaged regularly and patients with more severe urinomas benefit from early elective surgery at 2–5 days. In cases where extravasation of urine has not shown clear evidence of settling by 5 days' elective surgery probably results in less morbidity and fewer complications than protracted conservative treatment.

A 71-year-old man presented complaining of severe left flank pain. A computed tomography scan of the abdomen disclosed a left peripelvic extravasation of urine and a 4.0-cm abdominal aortic aneurysm with a significant amount of perianeurysmal thickening and prominent left hydroureter. The patient was diagnosed as having an inflammatory aneurysm of the abdominal aorta (IAAA) with peripelvic extravasations of urine. We report the results of a patient with IAAA with ureteral obstruction successfully treated with steroid therapy and a ureteral stent.

Vesicourethral anastomotic strictures are an important complication of radical prostatectomy for prostatic cancer. Their formation has been attributed to extravasation of urine at the anastomosis and to the surgical technique used to construct the anastomosis. Our study examines whether the formation of a vesicourethral anastomotic stricture correlates with (a) contrast extravasation seen on postoperative voiding cystourethrograms and (b) the surgical technique used to construct the vesicourethral anastomosis. We also describe the postoperative appearances of the anastomosis.One hundred and forty-two patients who underwent radical retropubic prostatectomy at our institution between June 1, 1987, and December 31, 1991, were included in the study. Of these, 101 had a direct end-to-end vesicourethral anastomosis, and 41 had their anastomosis constructed with traction sutures (Vest procedure). Voiding cystourethrograms were obtained 3 weeks after the prostatectomy. The appearance of the anastomosis, the presence of extravasation of contrast material at the anastomotic site, and the relationship of the subsequent formation of an anastomotic stricture to extravasation were evaluated. The influence of the surgical technique used to construct the vesicourethral anastomosis on the development of anastomotic strictures was analyzed.Contrast extravasation at the anastomotic site was seen in 14 (14%) of 101 patients who had a direct procedure and in three (7%) of 41 patients who had a Vest procedure. No relationship was found between contrast extravasation and subsequent formation of a stricture. Anastomotic strictures occurred in 16 (16%) of 101 patients who had a direct anastomosis and in 12 (29%) of 41 patients who had a Vest procedure. The surgical technique used to construct the vesicourethral anastomosis influenced the appearance of the vesicourethral anastomosis on cystourethrograms.Contrast extravasation at the anastomotic site is not infrequently seen on voiding cystourethrograms obtained after radical retropubic prostatectomy and resolves with continued drainage via a Foley catheter. As long as catheters are left in place until anastomotic healing is complete, extravasation of contrast material (implying urine extravasation at the anastomotic site) does not influence the subsequent formation of anastomotic strictures. Anastomoses that heal more slowly are no more likely to develop strictures than normally healing ones. Construction of the vesicourethral anastomosis by using the Vest procedure is a significant risk factor for stricture formation.

We report a case of secondary squamous cell carcinoma of the appendix causing spontaneous extravasation of urine and urinoma formation. On the basis of clinical findings, we interpret this rare lesion as metastatic from a cervical carcinoma surgically resected 8 years earlier. Ureteral continuity was restored by retrograde catheterization and the urinoma was drained percutaneously. The final diagnosis was made at laparotomy.
