Competition between mosquito species during the larval phase is a well-established mechanism structuring container mosquito communities, with invasive species often outperforming natives. We assessed the competitive outcome between 2 species that occur on the island of Puerto Rico, the historic invasive Aedes aegypti (L.) and the endemic Aedes mediovittatus (Coquillett) (Diptera: Culicidae). Trials were conducted under intra- and interspecific densities crossed with 2 amounts of 4 different leaf types serving as detrital resources for developing larvae; leaf types were representative of either urban or rural locations. Response variables included survival, male and female mass and development time, and a composite index of population growth (λ'). We also measured tannin levels in detrital environments, a secondary metabolite that can negatively influence mosquito development and survival. The co-occurrence of Ae. aegypti and Ae. mediovittatus generally did not affect survival or population growth of Ae. mediovittatus, however the converse was not true. Specifically, Ae. aegypti suffered lower survival and lower population growth in the presence of Ae. mediovittatus compared to when it was alone. Tannin concentrations did not correspond to competitive outcomes, as the highest tannin levels occurred under the detrital conditions where mosquitoes had highest growth and survival, with no evidence of differences between urban or rural leaf types. This is the first study to quantify the competitive outcomes between these species, both of which are part of the dengue transmission cycle on the island, and our findings suggest that Ae. mediovittatus is capable of outcompeting Ae. aegypti under some resource environments., (© The Author(s) 2024. Older adults are at higher risk for social isolation because of widowhood, loss of friends, retirement, physical limitations, geographic relocation, and caregiving demands. Behavioral interventions aimed at increasing social contact may help to maintain cognition and prevent cognitive decline. The purpose of this pilot study was to examine a novel intervention for social isolation with an intergenerational book club that had weekly in-person and virtual meetings of college students and older adults. We wanted to know whether the study was feasible and if our methods would be likely to generate meaningful results should it be expanded to a larger number of participants. We predicted that wellbeing and cognition would improve following participation in the book club. Results found that while measures of quality of life and affect were not statistically different before and after participation in a book club, scores on a measure of cognition (the Montreal Cognitive Assessment) were statistically significant between groups (intervention and control) showing greater improvement among book club participants., Competing Interests: The author(s) declared no potential conflicts of interest with respect to the research, authorship, and/or publication of this article., (© The Author(s) 2023.)

Purpose: This tutorial provides an overview of two behavioral approaches, physical activity and social engagement, to prevent or slow cognitive decline in older adults and to increase awareness in the field of speech-language pathology of the important role that speech-language pathologists (SLPs) play in an interprofessional team working with this population., Method: A review of exercise science, neuroscience, and social science literature was used to synthesize evidence and to outline the impact of physical activity and social engagement on cognition. The following topics were explored: How do exercise and social engagement support cognition? What are modifiable risk factors of dementia? What is the impact of inactivity and isolation on cognition? What is the potential role of the SLP on an interprofessional team focusing on preventive measures for cognitive decline? What is the impact of physical exercise and social engagement on nursing home residents?, Results: Research increasingly points to the critical importance of physical activity and social engagement to prevent cognitive decline in normal aging and to slow cognitive decline associated with mild cognitive impairment and dementia. Research suggests that physical activity maintains or improves memory, attention, executive function, visuospatial function, speed of processing, and general cognitive function. Social engagement has been found to maintain and improve general cognitive function., Conclusions: Behavioral interventions are an effective strategy to prevent or slow cognitive decline in the older adult population. SLPs have a role to play on an interprofessional team that works to prevent cognitive decline. By considering factors that play a role in the prevention of cognitive decline, such as physical activity and social engagement, the quality of life and overall health of older adults can be improved. Areas of improvement include memory, attention, executive function, visuospatial function, speed of processing, and general cognitive function.

Background: Current antibody-mediated rejection (AMR) therapies (intravenous immunoglobulin, apheresis, rituximab, polyclonal antibodies) do not target the primary antibody producing B cells, that is, the plasma cell. We report the preliminary results from the first clinical experience with plasma cell targeted therapy with bortezomib. Bortezomib is approved by the US Food and Drug Administration for the treatment of plasma cell tumors (multiple myeloma)., Methods: Kidney transplant patients with mixed acute cellular rejection (ACR) and AMR episodes (by Banff '97 criteria, update 2005) were treated with bortezomib (1.3 mg/m(2) per dose x 4) at standard labeled doses. Patients were monitored by serial donor specific anti-HLA antibody (DSA) determinations [Luminex/Labscreen beads] and quantified by conversion to fluorescence intensity to molecules of equivalent soluble fluorescence (MESF)., Results: Five patients were treated with bortezomib. Each patient also had coexisting ACR. In each case, bortezomib treatment led to prompt ACR and AMR rejection reversal. DSA levels decreased significantly in all patients (except 1 patient who had short follow-up). Observed toxicities from bortezomib included a transient grade III thrombocytopenia (1 patient) and mild-to-moderate nausea, vomiting, and/or diarrhea (3/5 patients). Opportunistic infections were not observed., Conclusions: Bortezomib therapy provides effective reduction in DSA levels with long-term suppression. These preliminary results indicate that proteasome inhibition provides an effective means for reducing HLA antibody levels in transplant recipients.

Elimination of corticosteroid-related morbidity has been a goal of transplant clinicians from the earliest days of renal transplantation more than 50 years ago. Over the past decade, this goal has begun to be realized. Herein, we describe our efforts to eliminate corticosteroid therapy from maintenance immunosuppression-efforts that have spanned 15 years and have included design and conduct of five multicenter trials and over ten single center trials with over 650 patients at the University of Cincinnati. These efforts have led to a near complete elimination of corticosteroid-related morbidity, and, importantly, a more precise definition of the risk/benefit assessments of corticosteroid withdrawal in individual patient populations, which has allowed individualization and tailoring of corticosteroid-free immunosuppression.

Objectives: Occupational therapy students often perceive their curriculum to be stressful. Anecdotal reports have described occupational therapy students' stress and coping strategies during their academic education; this study quantifies students' perceptions., Method: A questionnaire was used to survey 1,095 occupational therapy students representing the major geographical regions of the United States about perceived stress and coping strategies. The data were analyzed to determine patterns and frequencies of subjects' stress and coping strategies., Results: At least 86% of the subjects reported their top stressors to be examinations, amount of class work, lack of free time, long hours of study, and grades. Sixty-two percent of subjects said that the stress they experienced strengthened their commitment to their professional education., Conclusions: Academic issues and time pressures were more stressful than nonschool issues for occupational therapy students. Subjects' strategies for coping with these stressors usually included perseverance and rarely involved drugs, sex, or alcohol.

Objective: To determine what child restraints would accommodate infants with Pierre Robin sequence who often require special attention in motor vehicle travel since microagnathia usually requires a prone position to keep the infant's airway open., Research Design: Dynamic testing and clinical trial., Setting: An Indiana children's hospital providing primary and tertiary care., Patients: Four patients with Pierre Robin sequence are described to illustrate use of the modified infant car seat and the appropriateness of the car bed restraints for meeting requirements for prone positioning during travel., Selection Procedures: Convenience sample., Intervention: Selected restraints were loaned to families through a clinical setting until the patient was able to use a conventional child restraint., Measurements and Results: Three child restraint systems were determined to accommodate the prone position necessary to keep the airway open for children with Pierre Robin sequence. Dynamic crash testing demonstrated the crashworthiness of an infant car seat modified to allow for prone positioning. Through a clinical trial, two car bed restraints were also found to provide safe prone positioning of infants., Conclusions: To enable safe transportation for infants with Pierre Robin sequence, health care providers can direct parents to appropriate resources for travel and can monitor the airway and oxygenation of the infant with Pierre Robin sequence before hospital discharge.

The in vivo incorporation of [35-S]sulfate, [3-H]fucose and [3-H]leucine into sciatic nerve myelin was investigatedmpolyacrylamide gel electrophoresis of thr proteins indicated that the 35-S-labeling of proteins occurred almost exclusively in the major myelin protein; A smaller myelin glycoprotein migrating just ahead of the major one was labeled with [3-H]fucose but did not incorporate 35-S to a detectable extent. There was little or no 35-S associated with basic proteins on polyacrylamide gels when the proteins were extracted with chloroform/methanol; Fucose-labeled myelin glycoproteins were converted to glycopeptides by pronase digestion; The glycopeptides gave a single peak on tsephadex G-50 in which the 3-H and 35-S coincided. The association of 35-S with glycopeptides was not caused by binding of sulfatide or free inorganic sulfate. This study shows that the major myelin protein in the sciatic nerve of the rat is glycosylated and sulfated.

Myelin was purified from rat brain and sciatic nerve after in vivo labeling with [3H]fucose and [14C]glucosamine to provide a radioactive marker for glycoproteins. The glycoproteins in the isolated myelin were digested exhaustively with pronase, and glycopeptides were isolated from the digest by gel filtration on Bio-Gel P-10. The glycopeptides from brain myelin separated into large and small molecular weight fractions, whereas the glycopeptides of sciatic nerve myelin eluted as a single symmetrical peak. The large and small glycopeptide fractions from central myelin and the single glycopeptide fraction from peripheral myelin were analyzed for carbohydrate by colorimetric and gas liquid chromatographic techniques. The glycopeptides from brain myelin contained 2.4 micrograms of neutral sugar and 0.59 micrograms of sialic acid per mg total myelin protein, whereas sciatic nerve myelin glycopeptides contained 10 micrograms of neutral sugar and 3.8 micrograms of sialic acid per mg total protein. Similarly, the gas-liquid chromatographic analyses showed that the glycopeptides from peripheral myelin contained 4- to 7-fold more of each individual per mg total myelin protein than those from central myelin. Most of the sialic acid and galactose in the glycopeptides from central myelin were in the large molecular weight fraction, and the small molecular weight glycopeptides contained primarily mannose and N-acetylglucosamine. The considerably higher content of glycoprotein-carbohydrate in peripheral myelin supports the results of gel electrophoretic studies, which indicate that the major protein in peripheral myelin is glycosylated while the glycoproteins in purified central myelin are quantitatively minor components.
