Abstract

Objective: Opioid use disorder (OUD) affects approximately 5.6 million people in the United States annually, yet rates of the use of effective medication for OUD (MOUD) treatment are low. We conducted an observational cohort study from August 2017 through May 2021, the MOUD Study, to better understand treatment engagement and factors that may influence treatment experiences and outcomes. In this article, we describe the study design, data collected, and treatment outcomes. Methods: We recruited adult patients receiving OUD treatment at US outpatient facilities for the MOUD Study. We collected patient-level data at 5 time points (baseline to 18 months) via self-administered questionnaires and health record data. We collected facility-level data via questionnaires administered to facility directors at 2 time points. Across 16 states, 62 OUD treatment facilities participated, and 1974 patients enrolled in the study. We summarized descriptive data on the characteristics of patients and OUD treatment facilities and selected treatment outcomes. Results: Approximately half of the 62 facilities were private, nonprofit organizations; 62% focused primarily on substance use treatment; and 20% also offered mental health services. Most participants were receiving methadone (61%) or buprenorphine (32%) and were predominately non-Hispanic White (68%), aged 25-44 years (62%), and female (54%). Compared with patient-reported estimates at baseline, 18-month estimates suggested that rates of abstinence increased (55% to 77%), and rates of opioid-related overdoses (7% to 2%), emergency department visits (9% to 4%), and arrests (15% to 7%) decreased. Conclusions: Our results demonstrated the benefits of treatment retention not only on abstinence from opioid use but also on other quality-of-life metrics, with data collected during an extended period. The MOUD Study produced rich, multilevel data that can lay the foundation for an evidence base to inform OUD treatment and support improvement of care and patient outcomes. [ABSTRACT FROM AUTHOR]

18. Preliminary Family Outcomes of a Field Experiment on Intensive In-Home Interventions for Children in Psychiatric Crisis.

Abstract

This paper describes the Home-Based Crisis Intervention (HBCI) program in New York and the characteristics and outcomes of the families involved in a study that investigated the effectiveness of the program. The HBCI program provides short-term intensive in-home services to families with children who are at-risk of out-of-home placements. The intervention focuses on family strengths and needs using a multifaceted approach including skills building, counseling, and the provision of concrete services. The program effectiveness study included 221 children (ages 5-17) experiencing a psychiatric crisis that required immediate intensive intervention, hospitalization, or placement in another restrictive setting. Results indicate 63 percent of the families were Hispanic and 26 percent were African American. Eighty percent of the families had annual incomes of less than $20,000. Only 20 percent of the primary caretakers were employed. Seventy-two percent of the caretakers were single parents, and 55 percent of the caregivers had completed high school. The average age of enrolled children was about 11.5 years. Disruptive behavior and adjustment disorder diagnoses were the most common. After treatment, significant gains were found in cohesion and adaptability from intake to discharge. (CR)

Counselors work with families to prevent psychiatric hospitalization or out-of-home placement. The intervention focuses on family strengths and needs by using a multifaceted approach that includes skills building, counseling, and the provision of concrete services. The program effectiveness study included 221 children (ages 5-17) who were experiencing a psychiatric crisis requiring immediate intensive intervention, hospitalization, or placement in another restrictive setting. Results indicate that despite additional resources and expansion of services in the HBCI program, hospitalization rates did not change significantly at the two participating hospitals. Rather than preventing hospitalization, in-home services may represent another community-based option for children whose safety and clinical needs are not as acute as children judged to require hospitalization. Children enrolled in in-home services gained in self-esteem and these gains were maintaining over the follow-up period. 20. Addressing Children's Exposure to Violence in a Short-Term Crisis Intervention Program.

Abstract

This paper describes the Home-Based Crisis Intervention (HBCI) program in New York and the effects that exposure to violence had on 36 participating children (ages 5-18), the training counselors received on violence issues, and strategies used as a result of efforts to address the effect of violence. The HBCI program provides short-term intensive in-home services to families with children who are at-risk of out-of-home placements. The intervention focuses on family strengths and needs, using a multifaceted approach that includes skills building, counseling, and the provision of concrete services. Of the 36 children, 87 percent reported witnessing someone being beaten up, 26 percent knew someone who was robbed, 23 percent knew someone who had been stabbed, and 31 percent knew someone who was shot at. Twenty-nine percent felt unsafe at school and 19 percent felt unsafe at home. Interventions included conducting a safety analysis of the home, implementing plans to reduce risks of dangers, promotion of future-oriented activities, and promotion of family activities. A case study is presented demonstrating how violence was interwoven in the life of one child. (CR)

28. Examining the Impact of Policy on Collaboration in Systems of Care

Abstract

Since Jane Knitzer's 1982 report on "Unclaimed Children", many states and localities have been developing systems of care to more effectively serve children with serious emotional disturbance. We report on the methods and outcomes of a two-phase study designed to determine the relationship between the mechanisms that have been used to establish systems of care and their resulting levels of collaboration. A national survey of state mental health authorities revealed that, allowing for multiple approaches, 82% of respondents with systems of care used legislation, 15% used inducements, 41% used capacity building and 44% used other means of system change (e.g., blended funding). Cluster analysis was used to select 10 sites for visits. Multiple methods of collecting data at these sites were used including the administration of a scale on collaboration. Data were obtained from 302 respondents. Difference in beliefs about collaboration and collaborative behaviors were found, indicating that level of collaboration may be an important variable to consider in studies of outcomes of systems of care for children and families.

29. Developing a Parent-Professional Team Leadership Model in Group Work: Work with Families with Children Experiencing Behavioral and Emotional Problems

Abstract

Building on the respective strengths of parent-led and professional-led groups, a parent-professional team leadership model for group interventions was developed and evaluated for families of youths with emotional and behavioral problems. The model was developed based on feedback from 26 parents in focus group sessions and recommendations from mental health professionals in staff meetings. Evaluations of an implementation of the model in a support, empowerment, and education group intervention (S.E.E. group) have demonstrated the usefulness of this approach in work with families of children with behavioral and emotional problems. This article discusses the challenges of instituting the model in an S.E.E. group. It explores how parents and professionals build the team leadership model and the strengths of this approach in working with parents of youths with serious emotional disturbances.

30. Support, Empowerment, and Education: A Study of Multiple Family Group Psychoeducation

Abstract

In this article we describe the results of a randomized trial (N = 94) that developed and evaluated a multiple family group psychoeducation intervention (MFGPI) for parents/primary caregivers of children with serious emotional disturbance (SED) enrolled in community-based child and youth intensive case management programs. Through group problem-solving and interactive knowledge development activities, the MFGPI addressed the key components of social support, parent education, and parental empowerment. The structured group sessions used a professional/parent leadership model. Parents/primary caregivers were randomly assigned to one of two treatment conditions (intensive case management plus adjunctive MFGPI or "treatment as usual" intensive case management). Parent problem-solving skills, parental coping skills, perceived social support resources, and child behavior were measured at three points in time (baseline, 9 months, and 18 months). No statistically significant difference occurred by group condition for the key variables of social support versus problem-solving/coping measures; parents in both intervention conditions had significantly more individuals to help them out over time. Youth behavior in both conditions improved significantly over time as measured by the overall child behavior scores. Analysis of the time by intervention interaction suggested that parents in the group condition also reported significant improvements on child externalizing and internalizing behavior scores and several subscale behavior scores.

Abstract

In December 2019, a cluster of cases of pneumonia emerged in Wuhan City in central China's Hubei Province. Genetic sequencing of isolates obtained from patients with pneumonia identified a novel coronavirus (2019-nCoV) as the etiology (1). As of February 4, 2020, approximately 20,000 confirmed cases had been identified in China and an additional 159 confirmed cases in 23 other countries, including 11 in the United States (2,3). On January 17, CDC and the U.S. Department of Homeland Security's Customs and Border Protection began health screenings at U.S. airports to identify ill travelers returning from Wuhan City (4). CDC activated its Emergency Operations Center on January 21 and formalized a process for inquiries regarding persons suspected of having 2019-nCoV infection (2). As of January 31, 2020, CDC had responded to clinical inquiries from public health officials and health care providers to assist in evaluating approximately 650 persons thought to be at risk for 2019-nCoV infection. Guided by CDC criteria for the evaluation of persons under investigation (PUIs) (5), 210 symptomatic persons were tested for 2019-nCoV; among these persons, 148 (70%) had travel-related risk only, 42 (20%) had close contact with an ill laboratory-confirmed 2019-nCoV patient or PUI, and 18 (9%) had both travel- and contact-related risks. Eleven of these persons had laboratory-confirmed 2019-nCoV infection. Recognizing persons at risk for 2019-nCoV is critical to identifying cases and preventing further transmission. Health care providers should remain vigilant and adhere to recommended infection prevention and control practices when evaluating patients for possible 2019-nCoV infection (6). Providers should consult with their local and state health departments when assessing not only ill travelers from 2019-nCoV-affected countries but also ill persons who have been in close contact with patients with laboratory-confirmed 2019-nCoV infection in the United States.

Abstract

CDC, the Food and Drug Administration (FDA), state and local health departments, and public health and clinical partners are investigating a multistate outbreak of lung injury associated with the use of electronic cigarette (e-cigarette), or vaping, products. In late August, CDC released recommendations for health care providers regarding e-cigarette, or vaping, product use associated lung injury (EVALI) based on limited data from the first reported cases (1,2). This report summarizes national surveillance data describing clinical features of more recently reported cases and interim recommendations based on these data for U.S. health care providers caring for patients with suspected or known EVALI. It provides interim guidance for 1) initial clinical evaluation; 2) suggested criteria for hospital admission and treatment; 3) patient follow-up; 4) special considerations for groups at high risk; and 5) clinical and public health recommendations. Health care providers evaluating patients suspected to have EVALI should ask about the use of e-cigarette, or vaping, products in a nonjudgmental and thorough manner. Abstract

To inform prevention strategies, we assessed the extent of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmission and settings in which transmission occurred in a Georgia public school district.During 1 December 2020-22 January 2021, SARS-CoV-2-infected index cases and their close contacts in schools were identified by school and public health officials. For in-school contacts, we assessed symptoms and offered SARS-CoV-2 reverse-transcription polymerase chain reaction (RT-PCR) testing; performed epidemiologic investigations and whole-genome sequencing to identify in-school transmission; and calculated secondary attack rate (SAR) by school setting (eg, sports, elementary school classroom), index case role (ie, staff, student), and index case symptomatic status.We identified 86 index cases and 1119 contacts, 688 (61.5%) of whom received testing. Fifty-nine of 679 (8.7%) contacts tested positive; 15 of 86 (17.4%) index cases resulted in ≥2 positive contacts. Among 55 persons testing positive with available symptom data, 31 (56.4%) were asymptomatic. Highest SARs were in indoor, high-contact sports settings (23.8% [95% confidence interval {CI}, 12.7%-33.3%]), staff meetings/lunches (18.2% [95% CI, 4.5%-31.8%]), and elementary school classrooms (9.5% [95% CI, 6.5%-12.5%]). The SAR was higher for staff (13.1% [95% CI, 9.0%-17.2%]) vs student index cases (5.8% [95% CI, 3.6%-8.0%]) and for symptomatic (10.9% [95% CI, 8.1%-13.9%]) vs asymptomatic index cases (3.0% [95% CI, 1.0%-5.5%]).Indoor sports may pose a risk to the safe operation of in-person learning. Preventing infection in staff members, through measures that include coronavirus disease 2019 vaccination, is critical to reducing in-school transmission. Because many positive contacts were asymptomatic, contact tracing should be paired with testing, regardless of symptoms.
