Background The current management of lung cancer patients has reached a high level of complexity. Indeed, besides the traditional clinical variables (e.g., age, sex, TNM stage), new omics data have recently been introduced in clinical practice, thereby making more complex the decision-making process. With the advent of Artificial intelligence (AI) techniques, various omics datasets may be used to create more accurate predictive models paving the way for a better care in lung cancer patients. Methods The LANTERN study is a multi-center observational clinical trial involving a multidisciplinary consortium of five institutions from different European countries. The aim of this trial is to develop accurate several predictive models for lung cancer patients, through the creation of Digital Human Avatars (DHA), defined as digital representations of patients using various omics-based variables and integrating well-established clinical factors with genomic data, quantitative imaging data etc. A total of 600 lung cancer patients will be prospectively enrolled by the recruiting centers and multi-omics data will be collected. Data will then be modelled and parameterized in an experimental context of cutting-edge big data analysis. All data variables will be recorded according to a shared common ontology based on variable-specific domains in order to enhance their direct actionability. An exploratory analysis will then initiate the biomarker identification process. The second phase of the project will focus on creating multiple multivariate models trained though advanced machine learning (ML) and AI techniques for the specific areas of interest. Finally, the developed models will be validated in order to test their robustness, transferability and generalizability, leading to the development of the DHA. - Abstract
Simple Summary Thymic epithelial tumours are rare and insidious malignancies. Histologically, they can be divided into different WHO subtypes and relapse risk classes. Pre-treatment biopsy is not always feasible or accurate in distinguishing WHO classes. (18)FDG PET/CT scan has been reported to play a remarkable role in the prediction of histology in these tumours (the so-called "non-invasive biopsy"). The present narrative review would like to summarise current evidence on this topic and discuss potential applications. Background: The usefulness of (18)FDG PET/CT scan in the evaluation of thymic epithelial tumours (TETs) has been reported by several authors, but data are still limited and its application in clinical practice is far from being defined. Methods: We performed a narrative review of pertinent literature in order to clarify the role of (18)FDG PET/CT in the prediction of TET histology and to discuss clinical implications and future perspectives. Results: There is only little evidence that (18)FDG PET/CT scan may distinguish thymic hyperplasia from thymic epithelial tumours. On the other hand, it seems to discriminate well thymomas from carcinomas and, even more, to predict the grade of malignancy (WHO classes). To this end, SUVmax and other PET variables (i.e., the ratio between SUVmax and tumour dimensions) have been adopted, with good results. Finally, however promising, the future of PET/CT and theranostics in TETs is far from being defined; more robust analysis of imaging texture on thymic neoplasms, as well as new exploratory studies with "stromal PET tracers," are ongoing. Conclusions: PET may play a role in predicting histology in TETs and help physicians in the management of these insidious malignancies.

- Abstract
In the last decade, the emergence of effective systemic therapies (ESTs) in the form of both targeted and immuno-based therapies has revolutionized the treatment of patients with advanced stage III and stage IV melanoma. Even though lungs represent the most frequent site of melanoma metastases, only limited data are available on the role of surgery in isolated pulmonary metastases from malignant melanoma (PmMM) in the era of ESTs. The aim of this study is to describe the outcomes of patients who underwent metastasectomy of PmMM in the era of ESTs, in order to identify prognostic factors affecting survival and to provide a framework for more informed patient selection of treatmeant with lung surgery in the future. Clinical data of 183 patients who underwent metastasectomy of PmMM between June 2008 and June 2021 were collected among four Italian Thoracic Centers. The main clinical, surgical and oncological variables reviewed were: sex, comorbidities, previous oncological history, melanoma histotypes and primary site, date of primary cancer surgical treatment, melanoma growth phase, Breslow thickness, mutation pattern disease, stage at diagnosis, metastatic sites, DFI (Disease Free Interval), characteristics of lung metastases (number, side, dimension, type of resection), adjuvant therapy after lung metastasectomy, site of recurrence, disease-free survival (DFS) and cancer-specific survival (CSS; defined as the time interval between the first melanoma resection or lung metastasectomy and death from cancer). All patients underwent surgical resection of the primary melanoma before lung metastasectomy. Twenty-six (14.2%) patients already had a synchronous lung metastasis at the time of primary melanoma diagnosis. A wedge resection was performed in 95.6% of cases to radically remove the pulmonary localizations, while an anatomical resection was necessary in the remaining cases. - Abstract
Alveolo-pleural fistula remains a serious post-operative complication in lung cancer patients after surgery, which is associated with prolonged hospital stay and higher healthcare costs. The aim of this study is to evaluate the efficacy of a polyglycol acid (PGA)-sheet known as Neoveil in preventing post-operative air-leak in cases of detected intra-operative air-leak after lung resection. Between 11/2021 and 7/2022, a total of 329 non-small cell lung cancer (NSCLC) patients were surgically treated in two institutions. Major lung resections were performed in 251 cases. Among them, 44 patients with significant intra-operative air-leak at surgery were treated by reinforcing staple lines with Neoveil (study group). On the other hand, a historical group (selected by propensity score matched analysis) consisting of 44 lung cancer patients with significant intra-operative air leak treated by methods other than the application of sealant patches were considered as the control group. The presence of prolonged air-leak (primary endpoint), pleural drainage duration, hospital stay, and post-operative complication rates were evaluated. The results showed that prolonged air-leak (>5 days after surgery) was not observed in study group, while this event occurred in four patients (9.1%) in the control group. Additionally, a substantial reduction (despite not statistically significant) in the chest tube removal was noted in the study group with respect to the control group (3.5 vs. 4.5, p = 0.189). In addition, a significant decrease in hospital stay (4 vs. 6 days, p = 0.045) and a reduction in post-operative complications (2 vs. 10, p = 0.015) were observed in the study group when compared with the control group. Therefore, in cases associated with intra-operative air-leak after major lung resection, Neoveil was considered a safer and more effective aerostatic tool and represents a viable option during surgical procedures.

- Abstract
Colorectal cancer (CRC) is one of the most common causes of death from cancer. Lung seeding occurs in approximately 10% of patients surgically treated for primary CRC with radical intent: the lung is the most common site of metastases after the liver. While surgical treatment of liver metastases is widely accepted to affect long-term outcomes, more controversial and not standardized is the therapy for CRC patients developing lung metastases. Experience suggests the potential curative role of pulmonary metastasectomy, especially in oligometastatic disease. However, the optimal strategy of care and the definition of prognostic factors after treatment still need to be defined. This review focused on the uncommon scenario of single pulmonary metastases from CRC. We explored pertinent literature and provide an overview of the epidemiology, clinical characteristics and imaging of single pulmonary metastases from CRC. Additionally, we identified the best available evidence for overall management. In particular, we analyzed the role and results of locoregional approaches (surgery, radiotherapy or ablative procedures) and their integration with systemic therapy.

Methods: We performed a pictorial review of pertinent literature to describe different roles and applications of this imaging tool to manage TET patients. Finally, we summarized future prospects and potential innovative applications of PET in these neoplasms. Results:18 F FDG PET/CT scan may be of help to distinguish thymic hyperplasia from thymic epithelial tumours but evidences are almost weak. On the contrary, this imaging tool seems to be very performant to predict the grade of malignancy, to a lesser extent pathological response after induction therapy, Masaoka Koga stage of disease and long-term prognosis. Several other radiotracers have some application in TETs but results are limited and almost controversial. Finally, the future of PET/CT and theranostics in TETs is still to be defined but more detailed analysis of metabolic data (such as texture analysis applied on thymic neoplasms), along with promising preclinical and clinical results from new “stromal PET tracers”, leave us an increasingly optimistic outlook. Conclusions: PET plays different roles in the management of thymic epithelial tumours, and its applications may be of help for physicians in different clinical settings.

- Abstract
In this paper we aimed to address the role of pulmonary metastasectomy (PM) in patients affected by Lung Metastases (LM) from Renal Cell Carcinoma (RCC) and to analyse prognostic factors affecting overall survival (OS), disease-free interval (DFI) between primary RCC and first LM, and disease-free survival (DFS) after PM and before lung recurrence. Medical records of 210 patients who underwent PM from RCC in 4 Italian Thoracic Centres, from January 2000 to September 2019, were collected and analysed. All patients underwent RCC resection before lung surgery. The main RCC histology was clear cells (188, 89.5%). The 5-and 10-year OS from the first lung operation were 60% and 34%, respectively. LM synchronous with RCC (p = 0.01) and (Karnofsky Performance Status Scale) KPSS < 80% (p < 0.001) negatively influenced OS. Five-and 10-year DFI were 54% and 28%, respectively. The main factors negatively influencing DFI were: male gender (p = 0.039), KPSS < 80% (p = 0.009) and lactate dehydrogenase > 1.5 times 140 U/L (p = 0.001). Five-and 10-year disease-free survival were 54% and 28%, respectively; multiple LM (p = 0.036), KPSS < 80% (p = 0.001) and histology of RCC other than clear cells negatively influenced disease-free survival. Conclusions: patients with KPSS > 80%, single metachronous LM with a long DFI from RCC diagnosis, and clear cell histology, benefit from pulmonary metastasectomy.

Objective: Evaluate the accuracy of Cone Beam Computed Tomography (CBCT) for the study of the mental region of human cadaveric heads. Methods: 9 cryopreserved human cadaveric heads were selected for the anatomic study of the mental region. Through CBCT exam and direct observation of the head post-dissection, it was evaluated for each mental foramen: number, shape, location, horizontal and vertical diameter, distance to the superior and inferior border of the mandible and surrounding cortical bone thickness. Data analysis was performed using descriptive and inferential statistics for p<0,05. Results: No significant differences were found between the imagological and direct evaluation of the number, shape and position of the mental foramen (K=1; p<0.05). Submillimetric differences (<1 mm) were found between the CBCT linear measurements and the gold standard. There was a high correlation (p=0,904; p<0.05) for all the linear measurements, except for the vertical diameter (p=0.813; p<0.05) and the distance from the mental foramen to the inferior border of the mandible (p=0.700; p=0.188). The presence of tooth affected the accuracy of the CBCT when performing linear measurements. Conclusion: CBCT is a reliable and accurate method for taking linear measurements on the mental region in the presence of soft tissue, and so a valid tool for clinical diagnosis. - Abstract
Background: Outcomes for locally advanced NSCLC with pathological complete response (pCR), i.e., pT0N0 after induction chemoradiotherapy (IT), have been seldom investigated. Herein, long-term results, in this highly selected group of patients, have been evaluated with the aim to identify prognostic predictive factors. Methods: Patients affected by locally advanced NSCLC (cT1-T4/N0-2/M0) who underwent IT, possibly following surgery, from January 1992 to December 2019, were considered for this retrospective analysis. Survival rates and prognostic factors have been studied with Kaplan-Meier analysis, log-rank and Cox regression analysis. Results: Three-hundred and forty-three consecutive patients underwent IT in the considered period. Out of them, 279 were addressed to surgery; among them, pCR has been observed in 62 patients (18% of the total and 22% of the operated patients). In the pCR-group, clinical staging was IIb in 3 (5%) patients, IIIa in 28 (45%) patients and IIIb in 31 (50%). Surgery consisted of (bi)lobectomy in the majority of cases (80.7%), followed by pneumonectomy (19.3%). Adjuvant therapy was administered in 33 (53.2%) patients. Five-year overall survival and disease-free survival have been respectively 56.18% and 48.84%. The relative risk of death, observed with the Cox regression analysis, was 4.4 times higher (95% confidence interval (CI): 1.632–11.695, p = 0.03) for patients with N2 multi-station disease, 2.6 times higher (95% CI: 1.066–6.407, p = 0.036) for patients treated with pneumonectomy and 3 times higher (95% CI: 1.302–6.809, p = 0.01) for patients who did not receive adjuvant therapy. Conclusions: Rewarding long-term results could be expected in locally advanced NSCLC patients with pCR after IT followed by surgery. Baseline N2 single-station disease and adjuvant therapy after surgery seem to be associated with better prognosis, while pneumonectomy is associated with poorer outcomes.

Objectives: To evaluate the concentration of kidney injury molecule‐1 and activity of urinary gamma‐glutamyl transferase in cats with urethral obstruction and healthy cats. Materials and Methods: Blood and urine samples were collected from a group of 15 healthy cats (control group) and a group of 20 cats with urethral obstruction at presentation, and 24 hours and 7 days after unblocking the obstruction. The serum creatinine, urinary creatinine and urinary gamma‐glutamyl transferase were measured by spectrophotometry and kidney injury molecule‐1 by the sandwich enzyme‐linked immunosorbent assay. Results: On presentation, cats with obstruction had serum creatinine concentration and urinary gamma‐glutamyl transferase index higher than healthy cats (mean difference 544 μmol/L, 95% confidence intervals 222 to 865 μmol/L, and 0.0022 U/μmol‐uCre, 0.00043 to 0.0039 U/μmol‐uCre, respectively), urine creatinine concentration lower (mean difference 25,624 µmol/L, 17,329 to 33,919 µmol/L), and no significant difference in the kidney injury molecule‐1/urinary creatinine ratio (mean difference 13 pg/μmol‐uCre, −33 to 59 pg/μmol‐uCre). In the group of cats with urinary obstruction, over time serum creatinine decreased, urine creatinine increased, urinary gamma‐glutamyl transferase index did not change significantly, and kidney injury molecule‐1/urinary creatinine ratio increased. Clinical Significance: Cats with post‐renal obstruction and potential intrinsic renal damage had higher urinary gamma‐glutamyl transferase index than healthy cats at the time of presentation and showed increase in kidney injury molecule‐1/urinary creatinine ratio over time. This work describes what are the regulations in the field of safety and prevention in the workplace, with particular regard to the legal medical aspects of the doctor in charge of Occupational Medicine in correlation with ergophthalmological examinations. The main subjects of this treatment are the employees videoterminalists. The first part of this work aims to analyze the compliance, as well as laws and legislative decrees governing the obligations and duties of the employer to its employees, regarding the compliance of workplaces and equipment with the instrument that are routinely used by employees videoterminalists. The discussion continues by focusing on the specific aspects of the ergophthalmological examination. It also describes the different phases of the eye examination, starting from the identification of the employee and then to the collection of anamnestic data, which are an integratedpart of the medical-legal act and are also a useful tool for the final diagnosis. The discussion of the various phases of the ergophthalmological examination concludes with a description of indirect ophthalmoscopy with its crucial points, which are important points in any ophthalmological examination. Given the considerable diffusion and the great amount of technological innovations in the field of display screens, we conclude the discussion with a historical overview of the display screens that have been habitually used by employees in their workplace, describing the mechanism that underlie their operation through some hints of Physics. Bredemeyera floribunda roots are popularly used to treat snakebites in the semiarid region of Northeast Brazil, and previous studies indicate the anti-ophidian actions of triterpenoid saponins found in its roots. To assess B. floribunda root extract (BFRE) activity against the effects of Bothrops jararacussu venom (BjuV), antiphospholipasic, antiproteolytic, antihemorrhagic, antinecrotic, and anti-edematogenic activities were investigated in mice. Phytochemical analysis revealed the presence of saponins, flavonoids, and sugars, with rutin and saccharose being the major constituents of BFRE. Acute toxicity was determined and BFRE was nontoxic to mice. Phospholipase A2 and proteolytic activities induced by BjuV were inhibited in vitro by BFRE at all concentrations tested herein. BFRE (150 mg/kg) inhibited paw edema induced by BjuV (50 mg/animal), reducing total edema calculated by area under the curve, but carrageenan-induced paw edema was unchanged. Hemorrhagic and necrotizing actions of BjuV (50 mg/animal) were considerably decreased by BFRE treatment. Thus, BFRE blocked the toxic actions of B. jararacussu venom despite having no anti-inflammatory activity, which points to a direct inhibition of venom’s toxins, as demonstrated in the in vitro assays. The larger amounts of rutin found in BFRE may play a role in this inhibition, since 30,40-OH flavonoids are known inhibitors of phospholipases A2.

In the Philippines, English is the medium of instruction from primary to tertiary levels of Education. It is the Filipinos’ second language of instruction and communication. As such, there is a common notion that students at the tertiary level of education are already very good and fluent in English. However, the figure showed that students had difficulty in expressing themselves more so when they reach the first step towards teaching. It is in this context that this research has been conceived whether or not the freshmen can speak fluently. This study used descriptive method to effectively present and illustrate studies that were connected to the research. The researchers employed the random sampling technique in selecting the respondents. Thirty students were chosen randomly as respondents from three sections of freshmen English majors of the College of Teacher Education of Bestlink College of the Philippines. The questionnaire and focus group discussion was engaged to obtain information from respondents. Freshmen English majors had encountered challenges in the first stage of tertiary education in terms of different areas as in grammar, vocabulary, macro skills, and that there is less challenge in linguistics. Students likewise undergo impediments such as the prevalence of mother-tongue used for speaking the native language rather than English. Other impediments also count such as peer impact, socio-economic status, psychological factor, teacher’s influence, and self-interest. The confirmations from the respondents about their slow learning progress proved the existence of challenges that needed to be addressed to improve the teaching-learning process in reaching the proficiency of the language. It is viewed that the all-out cooperation of the community, parents, teachers, and learners have to be set out to have the desired outcome of their English language education.

O presente trabalho teve como objetivo avaliar o potencial fisiológico das sementes de soja durante a maturação e secagem artificial. O ensaio foi conduzido utilizando sementes da cultivar BRS Valiosa RR, produzidas na safra 2007/08 e colhidas em diferentes pontos no campo de produção. As sementes foram colhidas com diferentes teores de água (60%, 55%, 50%, 40%, 35%, 30% e 25%) e posteriormente separadas em duas amostras, sendo parte diretamente submetida a testes fisiológicos de germinação e vigor e, outra parte levada a um secador com circulação de ar forçada de pequena escala. Após a secagem, estas também foram avaliadas quanto ao potencial fisiológico. Utilizou-se delineamento experimental inteiramente casualizado com quatro repetições, em esquema fatorial 7 x 2, sendo sete épocas de colheitas e dois tratamentos de secagem. Concluiu-se que as sementes colhidas com teor de água 30 e 40% são as com maior potencial fisiológico, sendo a tolerância à dessecação evidente a partir de 40% de teor de água; as sementes com teor de água acima de 40% quando submetidas à secagem, têm seu potencial fisiológico comprometido.

In the present paper, the corrosion behavior of 1020 carbon steel in commercial gasoline-ethanol blends was investigated. The composition of each gasoline-ethanol blend was evaluated by infrared spectroscopy, and the ethanol content was determined by the ABNT 13992 reference method. Electrochemical Impedance Spectroscopy (EIS) and polarization methods were employed to evaluate corrosion resistance and penetration rates. Statistical analyses revealed that the gasoline's solution resistance governs the corrosion process, the RON (Research Octane Number) and MON (Motor Octane Number) numbers as well as the olefin content being more related to the corrosion rates. A castanha-do-brasil (Bertholletia excelsa Bonpl.) é um produto florestal não madeireiro de destaque, por consolidar aspectos econômicos, sociais e ecológicos. No entanto, práticas de manejo ainda precisam ser estabelecidas para que a extração desse produto seja realizada de forma segura, a fim de que o extrativista tenha um produto de qualidade e que seja sustentável para a espécie. Este estudo foi realizado com o objetivo de avaliar a dinâmica da dispersão primária de frutos de Bertholletia excelsa em um castanhal nativo no estado do Acre, buscando obter informações que subsidiem uma definição segura sobre a melhor época e o intervalo de coleta dos frutos. Durante o período de dispersão, 20 árvores foram monitoradas, sendo seus frutos contados e marcados de acordo com o tempo de queda. A queda dos frutos ocorreu em picos, sendo que, após 50 dias do início da dispersão, praticamente todos os frutos já haviam caído. Os resultados deste estudo indicam que a coleta deve começar a partir da oitava semana após o início da dispersão, e apenas um retorno às árvores deve ser realizado para a coleta dos frutos remanescentes, proporcionando ganhos em termos de qualidade e maior eficiência na produtividade.

Renal fibrosis is characterized by glomerulosclerosis and tubulointerstitial fibrosis and its pathogenesis is associated with the activity of mesenchymal cells (fibroblasts), being essentially characterized by a process of excessive accumulation resulting from the deposition of extracellular matrix components. The aim of this study was to characterize the morphological presentation of chronic and fibrotic lesions in the glomerular, tubular, interstitial, and vascular compartments in feline CKD, as well as the possible participation of myofibroblasts in renal fibrotic processes in this species. Cat kidneys were collected and processed according to the conventional techniques for light microscopy, circular polarization, immunohistochemistry, and electron microscopy. Fibrotic alterations were present in all compartments analyzed. The main findings in the glomerular compartment were different degrees of glomerular sclerosis, synechia formation, Bowman's capsule calcification, in addition to glomerular basement membrane thickening and pericapsular fibrosis. The tubulointerstitial compartment had intense tubular degeneration and the immunostaining in tubular cells for mesenchymal cell markers demonstrated the possibility of mesenchymal epithelial transition and consequent involvement of myofibroblasts in the development of interstitial tubule damage. Infiltration of inflammatory cells, added to vessel thickening and fibrosis, demonstrated the severity and role of inflammation in the development and perpetuation of damage. Thus, we may conclude that fibrotic lesions play a relevant role in feline CKD and the mechanism of perpetuation of these lesions need further elucidation regarding the origin and participation of myofibroblasts and consequent mesenchymal epithelial transition in this species. 1 RESUMO Existe um conjunto de tecnologias de baixo custo de construcao e manutencao utilizadas no semiarido,que possibilitam o aproveitamento da agua da chuva, viabilizando o seu uso para o abastecimento humano, animal e agricola, garantindo a seguranca alimentar das familias. As barragens subterrâneas destacam-se como uma destas tecnologias alternativas. Trata-se de barragens para captacao e armazenamento de agua no semiarido brasileiro, difundidas com denominacao geral de “barragens subterrâneas”. Existem, no Brasil, quatro metodos construtivos que armazenam agua em diferentes zonas dos solos.Por isso, possuem diferente capacidade de acumulacao hidrica posterior a sua efetivacao. Tais diferencas precisam ser melhores compreendidas e estudadas, pois tem causado confusao terminologica quanto a sua denominacao. A existencia de variacoes nas metodologias de construcao das barragens subterrâneas demonstra a grande capacidade de adaptacao as variacoes ambientais locais e as demandas dos beneficiarios.Diante disso, o objetivo deste artigo e descrever a evolucao historica dos termos utilizados para denominar a tecnologia de armazenamento de agua em sub-superficie no semiarido brasileiro. Propoe ainda, uma nova sistematizacao e caracterizacao das variacoes dos metodos de construcao a partir da divisao das barragens subterrâneas em dois grupos: submersas e submersiveis. Os modelos ASA Brasil e EMBRAPA (submersivel) sao indicados para atender a demanda familiar em situacoes restritivas do potencial de oferta de agua e o modelo Costa & Melo (submersa) e mais adequado, quando demanda uma maior oferta hidrica. Nao ha uma metodologia ideal de construcao de barragens subterrâneas, mas a que melhor se adequa a realidade ambiental do local onde se pretende construi-la e, tambem da realidade social da familia beneficiada. UNDERGROUND DAM IN BRAZILIAN SEMIARID REGION: HISTORICAL ANALYSIS AND METHODOLOGY OF CONSTRUCTION 2 ABSTRACT A number of low cost building and maintenance technologies employed in semi-arid regions make possible the use of rain water for human and animal consumption as well as in agriculture. Underground dams stand out as one of those alternative technologies. Equally referred to as “underground barrages”, is one of the most important among such alternative technologies and they are used to collect and store water in the Brazilian semi-arid. In Brazil there are four distinct building techniques of underground dams each storing water in a different underground layer and therefore with different water accumulation capacities after its execution. Such differences need be further studied and better understood because of the confusion regarding its actual terminology. Variations in underground dam construction techniques demonstrate its great adaptability to different environments and consumer needs. Thus, the aim of this research was to describe the historical development of the semantic usage of the terms connected to the underground water storage technology in the Brazilian semi-arid. It also proposes a new systematization and characterization of construction methods variations emerging the division of underground dams in the two groups: submerse and submersible. The ASA Brasil and EMBRAPA (submersible) models are indicated for family supply in situations of low potential water supply whereas the Costa & Melo (submerse) one is more appropriated when a greater water supply is required. There’s no ideal methodology for construct underground dams, but the one that best fits to the environmental condition of the place where it intends to be build and also social reality of the beneficiary family. The thermodynamics of water electrolysis cells is presented, followed by a review of current and future technology of commercial cells. The irreversibilities involved are analyzed and the resulting equations assembled into a computer simulation model of electrolysis cell efficiency. The model is tested by comparing predictions based on the model to actual commercial cell performance, and a parametric investigation of operating conditions is performed. Finally, the simulation model is applied to a study of electrolysis cell dynamics through consideration of an ideal pulsed electrolyzer.
