Abstract Extreme hydrological and thermal regimes characterize the Mediterranean zone and can influence the phenology of greenhouse gas (GHG) emissions in reservoirs. Our study examined the seasonal changes in GHG emissions of a shallow, eutrophic, hardwater reservoir in Spain. We observed distinctive seasonal patterns for each gas. CH4 emissions substantially increased during stratification, influenced predominantly by the increase in water temperature, net ecosystem production, and the decline in reservoir mean depth. N2O emissions mirrored CH4's seasonal trend, significantly correlating to water temperature, wind speed, and gross primary production. Conversely, CO2 emissions decreased during stratification and displayed a quadratic, rather than a linear relationship with water temperature—an unexpected deviation from CH4 and N2O emission patterns—likely associated with photosynthetic uptake of bicarbonate and formation of intracellular calcite that might be exported to sediments. This investigation highlights the imperative of integrating these idiosyncratic patterns into GHG emissions models, enhancing their predictive power.

Abstract Background Obesity is a complex chronic disease associated with several adverse health outcomes that increase mortality risk. Physical activity (PA) is recommended for the prevention and treatment of obesity and is related to a decreased risk of cardiovascular disease, cancer and all-cause mortality. This systematic review and meta-analysis estimates the effect of PA levels on mortality (cardiovascular, cancer and all-cause mortality) in adults with obesity. Methods A systematic search was conducted in MEDLINE, Embase, Web of Science and SPORTDiscus from inception to June 2024. Prospective cohort studies that explored the association between PA and mortality in adults with obesity (according to their body mass index, ≥ 30 kg/m2) aged ≥ 18 years were included. Our main outcomes were all-cause mortality, and cardiovascular, and cancer mortality reported in primary studies by hazard ratios or relative risk, which were pooled for the meta-analysis when at least two studies reported the effect estimate for the same outcome. The PRISMA recommendations and the MOOSE guidelines were followed. The reported mortality risk estimates comparing insufficiently active versus active (moderate to very active) adults with obesity were pooled using the DerSimonian and Laird random-effects model. Results A total of 9 prospective cohort studies involving 199,425 adults with obesity (age range: 35–85 years) were included, of which 59,873 were insufficiently active and 84,328 were active. Active individuals had a 21% lower risk of all-cause mortality (HR: 0.79, 95%CI: 0.74 to 0.84; I2 = 38.2%), and a 24% lower risk of cardiovascular mortality (HR: 0.76, 95%CI: 0.66 to 0.87; I2 = 0.0%) than insufficiently active individuals. The HR for cancer mortality was 0.91 (95%CI: 0.80 to 1.02; I2 = 0.0%), and although this was mostly consistent with a benefit, it was based on only two studies. Conclusion Our data support that moderate to high levels of PA are associated with a 21% lower risk of all-cause and 24% cardiovascular disease mortality in adults with obesity. Although data from the only two published studies seem to indicate a protective effect of PA on cancer risk, the estimates are not statistically significant. Abstract Reservoirs act as carbon sinks when sedimentation of particulate organic carbon (POC) exceeds CO2 and CH4 emissions. Here, we study the poorly explored process where phytoplankton‐derived acidic polysaccharides (APs) aggregate into particulate organic matter, promoting carbon export to sediments. This source of POC in sediments can mineralize to CO2 and CH4 over various timescales. Our research, centered on a Mediterranean reservoir, elucidates phenological trends of APs and POC sedimentation and identifies their predominant drivers. Our findings present synchronic sedimentation patterns of POC and APs but identify a 2‐week delay between POC sedimentation and CH4 emissions. Despite its eutrophic status, our data demonstrate this reservoir's role as a carbon sink, sequestering 4.33 g C m−2 yr−1. This highlights the need to consider various time scales when quantifying carbon budgets in reservoirs.

Lupus nephritis (LN) is a serious manifestation of systemic lupus erythematosus that can lead to end-stage renal disease. Many clinical and prognostic data on which our therapeutic decisions are based come from international cohorts, which have important ethnic and prognostic differences. To identify clinical and prognostic data from patients with LN in Spain, we undertook a bibliographic search of NL-related papers by Spanish authors and published in national and international journals between 2005 and 2022. According to the selected references, renal biopsy is not only essential for LN diagnosis but its repetition can be useful for the follow-up. Regarding LN treatment, standard strategy consists of an induction phase and a maintenance phase. However, as new drugs have been released, a new paradigm of treatment in a single, continuing and personalized phase has been proposed. Resumen: La nefritis lúpica (NL) es una manifestación grave del lupus eritematoso sistémico que puede llevar a una enfermedad renal terminal. Introducción: El ECOE es una herramienta de evaluación de competencias clínicas prácticas en Fisioterapia, hasta ahora no empleada para examinar las habilidades de los especialistas en Terapia Manual Ortopédica. Objetivo: El objetivo de este trabajo es evaluar la competencia a través de las calificaciones, la satisfacción y el desempeño percibido de los estudiantes del Máster de Terapia Manual Ortopédica en el Tratamiento del Dolor de la Universidad Europea de Canarias. Métodos: Se realizó un estudio descriptivo en una muestra de estudiantes de posgrado (n=21, M=10, V=11, 24,61 años) entre el 1 de febrero y el 31 de julio de 2023 en el Hospital Simulado de la Universidad Europea de Canarias (Tenerife, España). Resultados: Las calificaciones obtenidas por los participantes fueron mayoritariamente altas (media = 8,54 /10, DT= 0.612), aunque en Anatomía e interpretación de pruebas complementarias registraron los resultados más bajos. El 95,1% (n=20) expresó una alta satisfacción con el ECOE. Respecto a la interpretación de pruebas complementarias, un 19,1% (n=4) consideró su desempeño como deficiente y un 4,8% (n=1) muy deficiente. Conclusión: En síntesis, a pesar de las calificaciones más bajas en ciertas áreas, los resultados de la evaluación de competencias clínicas prácticas a través del ECOE son positivos. Además, se observa un elevado grado de satisfacción y percepción positiva del desempeño por parte del especialista.

Abstract Background The concept of early vascular aging (EVA) represents a potentially beneficial model for future research into the pathophysiological mechanisms underlying the early manifestations of cardiovascular disease. For this reason, the aims of this study were to verify by confirmatory factor analysis the concept of EVA on a single factor based on vascular, clinical and biochemical parameters in a healthy adult population and to develop a statistical model to estimate the EVA index from variables collected in a dataset to classify patients into different cardiovascular risk groups: healthy vascular aging (HVA) and EVA. Methods The EVasCu study, a cross-sectional study, was based on data obtained from 390 healthy adults. To examine the construct validity of a single-factor model to measure accelerated vascular aging, different models including vascular, clinical and biochemical parameters were examined. In addition, unsupervised clustering techniques (using both K-means and hierarchical methods) were used to identify groups of patients sharing similar characteristics in terms of the analysed variables to classify patients into different cardiovascular risk groups: HVA and EVA. Results Our data show that a single-factor model including pulse pressure, glycated hemoglobin A1c, pulse wave velocity and advanced glycation end products shows the best construct validity for the EVA index. The optimal value of the risk groups to separate patients is K = 2 (HVA and EVA). Conclusions The EVA index proved to be an adequate model to classify patients into different cardiovascular risk groups, which could be valuable in guiding future preventive and therapeutic interventions.

Abstract Charcoal and micro-layers of soot trapped in speleothems from the inner galleries of Nerja Cave were analysed through an interdisciplinary study. The absolute dating of the prehistoric subterranean activity of the cave and the identification of different phases of visits to the deep parts are presented and discussed. The charcoal analysis includes anthracological analysis and SEM–EDX. The soot analysis includes optical microscopy, Raman spectroscopy and TEM–EDX, and the microcounting of soot microlayers. The 14C dating of 53 charcoals identified 12 phases of prehistoric visits to the cave between 41,218 and 3299 cal. BP, putting back the origin of human occupation of this emblematic cave by 10,000 years. The interdisciplinary analysis of the soot microlayers allowed us to perform a high-precision zoom on the last three visitation phases identified by Bayesian analysis (8003–2998 cal. BP.), demonstrating that these phases contain at least 64 distinct incursions, with an average of one visit every 35 years for the Neolithic period. Spatial analysis showed that not all areas of the cave were used in the same periods, highlighting the repetition of visits to certain specific sectors of the Lower Galleries of the cave. Lastly, the anthracological data indicate a cross-cultural and unique use of Pinus tp. sylvestris-nigra wood for lighting activities over an extended period between the Gravettian and Upper Magdalenian.

Previous evidence associates insulin resistance with arterial stiffness in various pathologies, yet limited reports exist in healthy adults. Therefore, this study aims to estimate the association between insulin resistance and arterial stiffness in healthy adults. The cross-sectional EVasCu study enrolled 390 participants (42.05 ± 13.15 years). ANCOVAs, unadjusted (model 1) and adjusted (model 2), explored the association between arterial stiffness markers (aortic Pulse Wave Velocity [aPWV], Augmentation Index [AIx@75] and Cardio-Ankle Vascular Index [CAVI]), and insulin resistance markers (Homeostasis Model Assessment of Insulin Resistance [HOMA-IR], Quantitative Insulin Sensitivity Check Index [QUICKI] and Triglycerides-Glucose [TyG]). In model 1, all insulin resistance markers were associated with aPWV, HOMA-IR and QUICKI were associated with AIx@75, and the TyG index was associated with CAVI. In model 2, HOMA-IR and QUICKI increased aPWV by 0.179 and 0.156 m/s (p = 0.001 and p = 0.011), and AIx@75 by 4.17 and 5.39% (p = 0.009 and p = 0.003). The EVasCu study offers valuable insights into the relationship between insulin resistance and arterial stiffness in healthy adults, providing a deeper understanding of metabolic and cardiovascular health. By examining this influence, we embark on an intriguing exploration of how these factors interplay in the human body.

Time-restricted eating (TRE) has emerged as a dietary strategy that restricts food consumption to a specific time window and is commonly applied to facilitate weight loss. The benefits of TRE on adipose tissue have been evidenced in human trials and animal models; however, its impact on bone tissue remains unclear. To systematically synthesize and examine the evidence on the impact of TRE on bone health (bone mineral content (BMC), bone mineral density (BMD), and bone turnover factors), PubMed, Scopus, Cochrane CENTRAL, and Web of Science databases were systematically explored from inception to 1 October 2023 searching for randomized controlled trials (RCTs) aimed at determining the effects of TRE on bone health in adults (≥18 years). The Cochrane Handbook and the PRISMA recommendations were followed. A total of seven RCTs involving 313 participants (19 to 68 years) were included, with an average length of 10.5 weeks (range: 4 to 24 weeks). Despite the significant weight loss reported in five out of seven studies when compared to the control, our meta-analysis showed no significant difference in BMD (g/cm2) between groups (MD = −0.009, 95% CI: −0.026 to 0.009, p = 0.328; I2 = 0%). BMC and bone turnover markers between TRE interventions and control conditions were not meta-analyzed because of scarcity of studies (less than five). Despite its short-term benefits on cardiometabolic health, TRE did not show detrimental effects on bone health outcomes compared to those in the control group. Nevertheless, caution should be taken when interpreting our results due to the scarcity of RCTs adequately powered to assess changes in bone outcomes.

Enterocins are bacteriocins synthesized by Enterococcus strains that show an interesting antimicrobial effectiveness against foodborne pathogens such as Listeria monocytogenes. The objectives of this study were to identify and analyze the expression of enterocin genes of Enterococcus isolated from breast-fed infants and evaluate their ability to inhibit three human isolates of virulent L. monocytogenes, as well as some probiotic bacteria. The susceptibility of the strains of L. monocytogenes to fifteen antibiotics was tested, detecting their resistance to cefoxitin (constitutively resistant), oxacillin, and clindamycin. The production of enterocins A, B, and P was observed in Enterococcus faecium isolates, while enterocin AS-48 was detected in an Enterococcus faecalis isolate. AS-48 showed antilisterial activity by itself, while the joint action of enterocins A and B or B and P was necessary for inhibiting L. monocytogenes, demonstrating the synergistic effect of those combinations. The presence of multiple enterocin genes does not assure the inhibition of L. monocytogenes strains. However, the expression of multiple enterocin genes showed a good correlation with the inhibition capacity of these strains. Furthermore, the potential beneficial strains of lactobacilli and bifidobacteria examined were not inhibited by any of the enterocins produced individually or in combination, with the exception of Bifidobacterium longum BB536, which was inhibited by enterocin AS-48 and the joint production of enterocins A and B or B and P. The enterocins studied here could be candidates for developing alternative treatments against antibiotic-resistant bacterial infections. Moreover, these selected enterocin-producing E. faecium strains isolated from breast-fed infants could be used as probiotic strains due to their antilisterial effect, as well as the absence of virulence factors.

Soy beverages can be a source of bioactive isoflavones, with potential human health benefits. In this work, the suitability of three Lacticaseibacillus and three Bifidobacterium probiotic strains as functional starters for soy beverage fermentation were evaluated, alongside with the effect of refrigerated storage on the viability of the strains and the isoflavone composition of the fermented beverages. The three bifidobacteria strains suffered a decrease in their viability during refrigeration and only Bifidobacterium breve INIA P734 produced high concentrations of bioactive isoflavones. Meanwhile, L. rhamnosus GG and L. rhamnosus INIA P344 produced high levels of aglycones and, with L. paracasei INIA P272, maintained their viability during the refrigeration period, constituting promising starters to obtain functional soy beverages that could gather the benefits of the bioactive isoflavone aglycones and the probiotic strains. Moreover, the three lactobacilli caused an increase in the antioxidant capacity of the fermented beverages, which was maintained over the refrigerated storage.

Resumen: Objetivo: Evaluar la capacidad de respuesta del sistema de salud percibida por pacientes crónicos de atención primaria, así como los factores asociados a esa percepción. Método: Estudio transversal basado en entrevistas a pacientes mayores de 18 años diagnosticados de al menos una enfermedad crónica que visitaron los centros de atención primaria durante junio y julio de 2015 en una zona básica de Salud de La Rioja. El resultado fue el índice de respuesta y los factores potencialmente relacionados incluyeron características sociodemográficas y de salud. Entrevistadores capacitados realizaron el cuestionario breve en español de la encuesta multipaís de la Organización Mundial de la Salud con 403 participantes. Se realizaron estadística descriptiva y regresión logística bivariable y multivariable. Resultados: El 87,10% de los participantes consideró buena la respuesta general. Los dominios que obtuvieron las puntuaciones más altas fueron confidencialidad (99,3%), dignidad (98,3%) y comunicación (97,3%). Los peor evaluados fueron atención rápida (38,6%) y calidad de los servicios básicos (31,8%). La clase social baja fue el factor más importante asociado a la capacidad de respuesta, concretamente con la autonomía y la atención rápida. Sexo, educación y ocupación estuvieron relacionados con comunicación, y los pacientes con peor salud percibida valoraron peor la respuesta general. Los dominios considerados más importantes fueron dignidad (33,5%) y atención rápida (30,5%). Conclusiones: Los dominios mejor evaluados fueron los relacionados con el respeto a las personas. La atención rápida tiene una baja valoración, pero una gran importancia, y por lo tanto requiere una acción prioritaria. Abstract: Objective: To evaluate the health systems’ response capacity according to the perception of chronic patients, and the factors related to that perception. Method: Source of data: patients diagnosed with at least one chronic disease who visited primary care centers during June and July 2015 in a basic health area of La Rioja. Design: cross-sectional descriptive study based on interviews to over 18s who visited primary care centers. The dependent variable was the health systems’ response capacity and independent variables were sociodemographic and health related. In order to collect data, trained interviewers conducted a short questionnaire in Spanish from the World Health Organization Multi-country Survey Study with 403 subjects. Descriptive statistics, bivariate and multivariate logistic regression were performed. Results: The overall health systems’ response capacity was considered good by 87.10%. The domains that scored highest were: confidentiality (99.3%), dignity (98.3%) and communication (97.3%). Those evaluated worst were: rapid service (38,6%) and quality of basic services (31.8%). Low social class was the most important factor associated with the responsiveness, mainly with autonomy and rapid service. Sex, educational level, and occupation were related to communication domain, and patients with worse perceived health rated the general response worse. The domains considered most important were dignity (33.5%) and rapid service (30.5%). Conclusions: The domains best evaluated were those related to respect for people. Rapid service has a low health systems’ response capacity, but a high importance, and therefore requires priority action.

This study investigated gender assignment strategies in mixed noun phrases containing a Spanish determiner and an English noun among Spanish-English bilinguals (n = 38) in New Mexico (U.S.A.). Previous research has reported different gender assignment strategies based on a preference for a default determiner, the gender of the translation equivalent, or shape-based cues from the other language. The present study consisted of (i) a language background questionnaire, (ii) a two-alternative forced-choice judgment task, and (iii) two director-matcher tasks: a forced-switch task and a spontaneous card game. The results of the judgment task indicate that participants preferred the gender of the translation equivalent, i.e., la window ‘the.FEM window’ following the gender of the Spanish noun la ventana. Results from the production tasks also show that participants produced both gender congruent and incongruent mixed NPs, with Late English bilinguals producing more congruent mixed NPs, similar to the translation equivalent strategy found in the judgment task. These findings differ from those found in naturalistic speech in other New Mexican communities, which display a preference for a masculine default strategy. We suggest that the nature of participants’ bilingual profile and the community norms (urban setting, heterogeneous and diverse language contact profiles) may play a key role in the observed code-switching patterns in mixed noun phrases.

Abstract Background The health system responsiveness is a concept developed by the World Health Organization that measures patients’ expectations for the non-medical care they receive. The aim of this study is to assess primary care responsiveness as seen by people with mental illness and to analyse the factors associated with poor responsiveness. Methods Cross-sectional descriptive study on 426 people with mental illness who had attended primary care consultations at least once in the previous 12 months. The responsiveness of the health system was determined through the short questionnaire “Multi-country Survey Study on Health and Health Systems Responsiveness”. Differences in responsiveness by sociodemographic characteristics were compared through the Chi-squared test. Logistic regression identified the factors associated with poor responsiveness. Results Overall responsiveness was measured as good by 77.4% of patients, being this probability higher in the domains: dignity, confidentiality, and communication. The most valued domains by people with mental illness were prompt attention (42.4%), dignity (30.1%), and communication (17%). Only prompt attention scored high importance and poor responsiveness. In patients with an income lower than 900 euros per month and low level of studies, the probability of poor confidentiality responsiveness was multiplied by 3 and 2.7 respectively. Conclusions People with mental illness perceive good responsiveness from primary care in terms of dignity, confidentiality, and communication. Prompt attention, as the domain of greatest importance and worst valuation, should be prioritised through the implementation of organisational measures in health centres to reduce waiting times, especially in urban areas.

Según la literatura, el incremento de la complejidad en la gestión de los centros educativos podría explicar la escasez de candidaturas para la dirección escolar y el agotamiento y deserción de directivos. No obstante, los directivos se muestran satisfechos y apasionados por su trabajo. El objetivo de este estudio es analizar los niveles de pasión en profesionales de la dirección escolar mediante la escala de la Pasión (Marsh et al., 2013) y su relación con variables profesionales. Los resultados muestran altos niveles de pasión y pasión armoniosa entre los sujetos de la muestra, tanto en Chile como en Cataluña.

Abstract Background The objective of this study is to deepen our understanding of perceptions towards Primary Health Care Response Capacity by specifically using patients with and without mental disorders, as well as family doctors and a manager, in order to compare and endorse perspectives. For it, a qualitative study was performed. In-depth interviews were conducted with 28 patients with and without mental health disorders and focus groups were held with 21 professionals and a manager. An inductive thematic content analysis was performed in order to explore, develop and define the emergent categories of analysis. Results The fundamental domains for patients are dignity, communication, and rapid service. People with mental health problems also highlight the domain of confidentiality as relevant, while patients who do not have a mental health problem prioritize the domain of autonomy. Patients with mental health disorders report a greater number of negative experiences in relation to the domain of dignity. Patients do not consider their negative experiences to be a structural problem of the system. These findings are also endorsed by health care professionals. Conclusions It is necessary to take these results into account as responsive systems can improve service uptake, ensure adherence to treatment, and ultimately enhance patient welfare.

Garlic is cultivated worldwide for the value of its bulbs, but its cultivation is challenged by the infertility of commercial cultivars and the accumulation of pathogens over time, which occurs as a consequence of vegetative (clonal) propagation. In this review, we summarize the state of the art of garlic genetics and genomics, highlighting recent developments that will lead to its development as a modern crop, including the restoration of sexual reproduction in some garlic strains. The set of tools available to the breeder currently includes a chromosome-scale assembly of the garlic genome and multiple transcriptome assemblies that are furthering our understanding of the molecular processes underlying important traits like the infertility, the induction of flowering and bulbing, the organoleptic properties and resistance to various pathogens.

We report the molecular characterization of an ethyl methanesulfonate (EMS)-induced mutation that causes albinism and lethality at the seedling stage in Arabidopsis thaliana. We identified the mutation using a mapping-by-sequencing approach that uses Fisher’s exact tests to detect changes in allele frequencies among the seedlings of an F2 mapping population, which had been pooled according to their phenotypes (wild-type or mutant). After purifying genomic DNA from the plants of both pools, the two samples were sequenced using the Illumina HiSeq 2500 next-generation sequencing platform. The bioinformatic analysis allowed us to identify a point mutation that damages a conserved residue at the acceptor site of an intron of the At2g04030 gene, which encodes the chloroplast-localized AtHsp90.5 protein, a member of the HSP90 family of heat shock proteins. Our RNA-seq analysis demonstrates that the new allele alters the splicing of At2g04030 transcripts in multiple ways, leading to massive deregulation of genes encoding plastid-localized proteins. A search for protein–protein interactions using the yeast two-hybrid method allowed us to identify two members of the GrpE superfamily as potential interactors of AtHsp90.5, as has previously been reported for green algae.

Background: Enteral nutrition interruptions (ENI) are prevalent in the pediatric intensive care unit (PICU), but there is little evidence of their characteristics. Methods: This is a cross-sectional multicenter study including critically ill children on enteral nutrition. ENIs were classified as PICU procedures, procedures performed outside the PICU (PPOP), feeding intolerance and other criteria. The number and features of ENIs were collected. Results: A total of 75 children were enrolled. There were 41 interruptions affecting 37.3% of the patients with a median duration of 5 ± 9.4 h. The most common reason for ENI was PPOP (41.5%), followed by other criteria. Interruptions were considered preventable in 24.4% of the cases, but only eight were compensated. ENIs were more prevalent among children with cardiac disease (p = 0.047), higher PRISM (p = 0.047) and longer PICU stay (p = 0.035). There was association between PRISM and total interruption time (p = 0.02) and lower caloric intake (p = 0.035). Patients with respiratory illness (p = 0.022) and on noninvasive ventilation (p = 0,028) had fewer ENIs. ENI total time was associated with lower caloric (p = 0.001) and protein (p = 0.02) intake. Conclusions: ENIs are prevalent in PICU, especially in children with higher PRISM, longer PICU stays and cardiac disease, and result in lower caloric and protein intake.

Recent advances in hardware and information technology have accelerated the proliferation of smart and interconnected devices facilitating the rapid development of the Internet of Things (IoT). IoT applications and services are widely adopted in environments such as smart cities, smart industry, autonomous vehicles, and eHealth. As such, IoT devices are ubiquitously connected, transferring sensitive and personal data without requiring human interaction. Consequently, it is crucial to preserve data privacy. This paper presents a comprehensive survey of recent Machine Learning (ML)- and Deep Learning (DL)-based solutions for privacy in IoT. First, we present an in depth analysis of current privacy threats and attacks. Then, for each ML architecture proposed, we present the implementations, details, and the published results. Finally, we identify the most effective solutions for the different threats and attacks.

Resumen: Objetivo: Describir la utilización y percepción de la necesidad de productos de apoyo e identificar su asociación con instrumentos para medir la dependencia y la sobrecarga de cuidados. Diseño: Estudio transversal. Emplazamiento: Atención Primaria del área sur de Pontevedra. Participantes: Un total de 112 personas cuidadoras informales que atienden a 125 personas dependientes. Mediciones principales: Mediante entrevista personal se recogen datos sobre la utilización y percepción de necesidad de estos productos. Además, se obtienen datos sociodemográficos e información referida al tiempo dedicado al cuidado diario, la sobrecarga del cuidador, el Baremo de Valoración de Dependencia y el indicador de dependencia DEP-6D. Se ha utilizado un contraste de medias para identificar si existen diferencias significativas en dichos indicadores en función de si utilizan o no los productos facilitadores. Se ha calculado una regresión logística para identificar aquellas variables asociadas con tener una necesidad no cubierta. Resultados: Un 60% (IC 95%: 50,9-68,7) de las personas dependientes tienen necesidades no cubiertas de estos dispositivos. Estas necesidades se asocian, principalmente, con una renta familiar baja, un mayor grado de dependencia y una mejor salud del cuidador, aunque en situaciones de gran dependencia y muy mala salud del cuidador estas carencias se suavizan. Conclusiones: Existe una fuerte asociación entre grado de dependencia y utilización de estos productos. Sin embargo, también hay un elevado porcentaje de la muestra con necesidades no cubiertas de estos productos que están asociadas tanto con la situación socioeconómica del hogar como con las características del dependiente y de la persona cuidadora. Abstract: Objective: To describe the use and perception of the need for assistive devices and identify their relationship with instruments to measure dependence and caregiver burden. Design: Cross-sectional study. Location: Primary Health Care in the southern area of Pontevedra. Participants: A total of 112 informal caregivers providing care to 125 dependent persons. Key measurements: Through a personal interview, data was collected on the use and perception of the need for these devices. In addition, sociodemographic data and information on the time dedicated to daily care, caregiver burden, the Dependency Rating Scale, and the DEP-6D dependency indicator were obtained. A contrast mean was used to identify whether there are significant differences in these indicators, depending on whether or not they use the facilitator devices. A logistic regression was performed to identify those variables most associated with not having a need covered. Results: More than half (60%: 95% CI; 50.9-68.7) of dependent persons have unmet needs for these devices. These needs are mainly associated with a low family income, a high degree of dependence, and better health of the caregiver; although in extreme situations of greater dependence and worse health of the caregiver these deficiencies are smoothed out. Conclusions: There is a strong association between the level of dependence and the use of these devices. However, there is a high percentage of the sample that do not have the products that they need, which are associated with the socioeconomic situation of the household as well as with the characteristics of the dependent person and the caregiver.

Introduction: Chronic kidney disease (CKD) increases the risk of mortality during coronavirus disease 2019 (COVID-19) episodes, and some reports have underlined the high incidence and severity of this infection in dialysis patients. Information on COVID-19 in nondialysis CKD patients is not available yet. Case Reports: Here we present 7 patients with grade 4–5 CKD who developed symptomatic COVID-19; they comprise 2.6% of our 267 advanced CKD patients. The estimated GFR was between 12 and 20 mL/min during the month prior to COVID-19. The 3 major symptoms were fever, cough, and dyspnea, and 5 patients showed bilateral pneumonia. Hydroxychloroquine, azithromycin, ceftriaxone, and steroids were the most frequently prescribed drugs. Two patients needed noninvasive mechanical ventilation. All patients showed minimal to moderate kidney function deterioration during admission, with an eGFR decline below 5 mL/min in 6 cases. No patient required acute dialysis. Six patients were discharged alive and remained dialysis free athe t the time of reporting, and one 76-year-old patient died. Conclusions: COVID-19 affects grade 4–5 CKD patients, but prognosis may be acceptable if prompt supportive measures are applied. These findings should be confirmed in larger cohorts, and further observations will be needed to understand the full spectrum of clinical features and the optimal approach to COVID-19 in patients with advanced CKD.

Introduction: Percutaneous renal biopsy (PRB) of native kidneys is an important tool for diagnosis and management of renal disease. In this study, we analyzed the success, safety, and risk complications of PRB in our center. Methods: A retrospective review of ultrasound-guided PRB done at our institution from January 1998 to December 2017 was performed. Clinical and laboratory data were collected for 661 PRBs. Statistical analysis was performed using the Mann-Whitney U test for continuous variable and chi-square test for categorical variables. Multivariate analysis using logistic regression was performed to assess factors associated with increased risk of complications after PRB. Results: The median age was 56 (42–68) years old, the majority were male (64%) and white (82%). Ten glomeruli were present in 63.5% of PRBs. Overall, the rate of complications was 16.6%, where 15.1% of them were minor complications and 1.5% were major complications. Perinephritic hematoma accounted for the minor complication that occurred most frequently, whereas the need of a blood transfusion was the prevalent for major complications. By multivariate analysis, increased activated partial thromboplastin time (aPTT; OR 1.11, 95% CI 1.035–1.180) and prebiopsy lower hemoglobin (Hgb; OR 1.61, 95% CI 1.086–2.304) were identified as independent risk factors for major complications. In addition, older patients (OR 1.057, 95% CI 1.001–1.117) were identified as an independent risk factor for blood transfusion requirement. Conclusion: The current risk of complications after native PRB is low. Major complications are most common in case of increased aPTT and decreased Hgb baseline level.

Cyberattacks in the Internet of Things (IoT) are growing exponentially, especially zero-day attacks mostly driven by security weaknesses on IoT networks. Traditional intrusion detection systems (IDSs) adopted machine learning (ML), especially deep Learning (DL), to improve the detection of cyberattacks. DL-based IDSs require balanced datasets with large amounts of labeled data; however, there is a lack of such large collections in IoT networks. This paper proposes an efficient intrusion detection framework based on transfer learning (TL), knowledge transfer, and model refinement, for the effective detection of zero-day attacks. The framework is tailored to 5G IoT scenarios with unbalanced and scarce labeled datasets. The TL model is based on convolutional neural networks (CNNs). The framework was evaluated to detect a wide range of zero-day attacks. To this end, three specialized datasets were created. Experimental results show that the proposed TL-based framework achieves high accuracy and low false prediction rate (FPR). The proposed solution has better detection rates for the different families of known and zero-day attacks than any previous DL-based IDS. These results demonstrate that TL is effective in the detection of cyberattacks in IoT environments.

Background and Aims: Complement system seems to play an important role in the pathogenesis of Acute Kidney Injury (AKI). The aim of this study was to investigate the role of complement system in the pathogenesis of human AKI. For this purpose, we studied Membrane Attack Complex (MAC) and factor H in plasma and kidney tissue in AKI. Methods: Plasmatic concentrations of MAC and Factor H were studied in patients with hospital-acquired AKI and their respective controls. MAC and Factor H expression and localization within the kidney were studied by immunohistochemistry in kidney tissue samples from autopsies. Demographical, past medical, and laboratory data in patients on admission and 3 years after discharge were recorded. Results: Plasmatic MAC concentrations were significantly higher in AKI-patients (5848±3604 vs 3703±1483 mAU/mL, p< 0.01), mainly in the severe cases, as measured by the need of renal replacement therapy, non-recovery of renal function, RIFLE classification and CKD development. MAC deposition was observed in tubular epithelial cell basal membranes, showing a larger number of tubules with MAC deposition, larger perimeter of affected tubules and greater intensity of MAC immunostaining in AKI patients. Factor H concentrations were higher in AKI patients (0.86±0.05 vs 0.60±0.04 mg/mL, p=0.007), showing a strong positive association with plasmatic MAC (r=0.7, p< 0.01)). Factor H immunostaining showed a tubular cytoplasmic pattern, with significant variations in the staining intensity, associated with the severity of histologic damage. Conclusion: Our data confirm that complement system is involved in human AKI, through the lytic action of MAC in tubular epithelial cells. These results suggest that complement system activation in AKI could be related with CKD development.

Introducción: El cáncer de mama es considerado una enfermedad crónica que impacta de modo importante en la calidad de vida (QoL) de las pacientes. En la actualidad no se dispone de un sistema de evaluación totalmente satisfactorio que refleje la complejidad del cáncer de mama. Además, la mayoría de instrumentos están más orientados a la enfermedad en estadios iniciales que a la enfermedad avanzada. Objetivo: Describir la metodología utilizada para evaluar la calidad de vida y las estrategias de afrontamiento en un grupo representativo de cáncer de mama que incluye tanto pacientes metastásicas como no metastásicas. Método: Estudio prospectivo en pacientes con cáncer de mama del Servicio de Oncología Médica del Institut Català d’Oncologia (ICO) en diferentes estadios de la enfermedad, previo consentimiento informado. Se les aplica un protocolo de aproximadamente una hora de entrevista presencial donde se recoge su información sociodemográfica, y se contestan varios cuestionarios de calidad de vida como el cuestionario QLCA-AFex Font, QLQ-C30, QLQ-BR23 HADS, DME, BRCS, MINI-MAC, LOT-R y OE, que se completan con una entrevista semiestructurada. Resultados: De junio de 2017 a marzo de 2020, 257 pacientes han sido incluidas en el estudio. La media de edad es de 57,9 años (SD 10,1), la mayoría son mujeres (98,8%), con hijos (87,9%) y casadas (65,4%). Respecto al estadiaje clínico 75,5% son no-metastásicas y 24,5% metastásicas. El cumplimiento del protocolo se consiguió en más del 90% de los cuestionarios sin diferencias entre pacientes metastásicas y no metastásicas. Conclusión: Este protocolo multidimensional permite hacer una evaluación integral de la calidad de vida, así como reflejar las necesidades no atendidas y las preocupaciones que muestran las pacientes tanto en estadio precoz como avanzado, complementando los sistemas de valoración actualmente disponibles. Después del análisis de los resultados de este estudio sería interesante poder obtener un cuestionario único con las preguntas más relevantes que pudiera ser aplicado a la práctica clínica.

The GenoDiabMar registry is a prospective study that aims to provide data on demographic, biochemical, and clinical changes in type 2 diabetic (T2D) patients attending real medical outpatient consultations. This registry is also used to find new biomarkers related to the micro- and macrovascular complications of T2D, with a particular focus on diabetic nephropathy. With this purpose, longitudinal serum and urine samples, DNA banking, and data on 227 metabolomics profiles, 77 immunoglobulin G glycomics traits, and other emerging biomarkers were recorded in this cohort. In this study, we show a detailed longitudinal description of the clinical and analytical parameters of this registry, with a special focus on the progress of renal function and cardiovascular events. The main objective is to analyze whether there are differential risk factors for renal function deterioration between sexes, as well as to analyze cardiovascular events and mortality in this population. In total, 650 patients with a median age of 69 (14) with different grades of chronic kidney disease—G1–G2 (eGFR > 90–60 mL/min/1.73 m2) 50.3%, G3 (eGFR; 59–30 mL/min/1.73 m2) 31.4%, G4 (eGFR; 29–15 mL/min/1.73 m2) 10.8%, and G5 (eGFR < 15 mL/min/1.73 m2) 7.5%—were followed up for 4.7 (0.65) years. Regardless of albuminuria, women lost 0.93 (0.40–1.46) fewer glomerular filtration units per year than men. A total of 17% of the participants experienced rapid deterioration of renal function, 75.2% of whom were men, with differential risk factors between sexes—severe macroalbuminuria > 300 mg/g for men OR [IQ] 2.40 [1.29:4.44] and concomitant peripheral vascular disease 3.32 [1.10:9.57] for women. Overall mortality of 23% was detected (38% of which was due to cardiovascular etiology). We showed that kidney function declined faster in men, with different risk factors compared to women. Patients with T2D and kidney involvement have very high mortality and an important cardiovascular burden. This cohort is proposed as a great tool for scientific collaboration for studies, whether they are focused on T2D, or whether they are interested in comparing differential markers between diabetic and non-diabetic populations.

Here, we present the characterization of three iPSC lines derived from dermal fibroblasts of old healthy subjects. Fibroblasts were reprogrammed using Sendai viral vectors encoding OCT4, SOX2, KLF4 and c-MYC. The iPSCs expressed endogenous pluripotency markers, could generate the three germ layers (ectoderm, mesoderm and endoderm), maintained a stable karyotype, and were free from Sendai vectors and reprogramming factors. These integration-free iPSCs can serve for establishing control cell cultures in studies searching for phenotypes and mechanisms that could potentially be dysregulated in degenerative diseases.

The epsilon4 (ε4) allele of the APOE gene, which encodes the apolipoprotein E4 (ApoE4), is the strongest genetic risk factor known for late-onset Alzheimer´s disease (LOAD). The familial form of Alzheimer's disease (FAD), which is caused by mutations in PRESENILIN 1 (PSEN1) and amyloid precursor protein (APP) genes, represents less than 5% of all AD cases and has an early-onset. We report the generation and characterization of an iPSC line derived from a FAD patient carrying the PSEN1-G206D mutation. The iPSC line maintained the original genotype, a normal karyotype, was free from Sendai viral vectors and reprogramming factors (OCT4, SOX2, KLF4 and c-MYC), presented a typical morphology, expressed endogenous pluripotency markers, and could be differentiated into ectodermal, mesodermal and endodermal cells, confirming its pluripotency.

The L444P mutation in the GBA1 gene which encodes β-glucocerebrosidase-1, is a major risk factor for developing Parkinson's disease (PD) and dementia with Lewy bodies (DLB). We report the generation and characterization of an induced pluripotent stem cell (iPSC) line derived from a female PD patient carrying the L444P/wt mutation. The iPSC line presented a normal morphology, expressed endogenous pluripotency markers, could be differentiated into endodermal, mesodermal and ectodermal cells, was free from Sendai vectors and reprogramming factors, had a normal karyotype and maintained the original GBA1 genotype. Thus, this iPSC line can serve to establish cellular models of PD.

Antecedentes y objetivos: La enfermedad renal diabética es la primera causa de enfermedad renal crónica terminal en nuestro medio. El bloqueo del sistema renina-angiotensina aldosterona (BSRAA) enlentece la progresión de la enfermedad renal diabética. Nuestros objetivos son: estudiar pacientes tratados con BSRAA, comparando su evolución según el filtrado glomerular, su perfil de seguridad y si se asocia a una progresión mayor de la enfermedad renal crónica. Materiales y métodos: Estudio retrospectivo de 197 pacientes con enfermedad renal diabética. Se dividieron en tres grupos según tratamiento: pacientes que no recibieron BSRAA (no BSRAA), pacientes que en algún momento lo recibieron (BSRAA discontinuado) y pacientes que recibieron BSRAA (BSRAA mantenido). Se estudiaron características clínicas y analíticas: función renal, ionograma, hemoglobina glicosilada, filtrado glomerular según CKD-EPI y MDRD. Analizamos su evolución (basal, año y 3 años) en relación con el grupo de tratamiento, supervivencia, factores de riesgo y pronóstico renal. Background/Aims: Recurrent acute kidney injury (AKI) is common among patients after a first hospitalized AKI. However, little is known about the prognosis of recurrent AKI episodes in chronic kidney disease (CKD) development, cardiovascular events and mortality. Methods: A retrospective study included patients admitted to our Hospital from 2000 to 2010. AKI was defined according to the Acute Dialysis Quality Initiative criteria. In the follow-up period after the first AKI episode, clinical, laboratory data and the number of repeated AKI episodes, etiology and severity were recorded. Results: Among the 359 AKI survivor patients included, 250 new AKI episodes were observed in 122 patients (34%). Variables independently associated to new episodes were: type 2 DM [OR 1.2, 95%CI 1.2-3.8, p=0.001], ischemic heart disease [OR 1.9; 95%CI 1.1-3.6, p=0.012], and SCr at the first AKI event>2,6 mg/dl [OR 1.2; 95%CI 1.03-1.42, p=0.02]. Development of CKD during four years follow-up was more frequent in patients with recurrent AKI, HR [2.2 (95% CI: 1.09-4.3, p=0.003)] and 44% of recurrent AKI patients who developed CKD occurred during the first 6 months after the initial event. Cardiovascular events were more frequent among patients with recurrent AKI patients than in those with one AKI episode (47.2% vs 24%, p=0.001). Acute and chronic kidney lesions induce an increase in A Disintegrin And Metalloproteinase domain 17 (ADAM17) that cleaves several transmembrane proteins related to inflammatory and fibrotic pathways. Our group has demonstrated that renal ADAM17 is upregulated in diabetic mice and its inhibition decreases renal inflammation and fibrosis. The purpose of the present study was to analyze how Adam17 deletion in proximal tubules affects different renal structures in an obese mice model. Tubular Adam17 knockout male mice and their controls were fed a high-fat diet (HFD) for 22 weeks. Glucose tolerance, urinary albumin-to-creatinine ratio, renal histology, and pro-inflammatory and pro-fibrotic markers were evaluated. Results showed that wild-type mice fed an HFD became obese with glucose intolerance and renal histological alterations mimicking a pre-diabetic condition; consequently, greater glomerular size and mesangial expansion were observed. Adam17 tubular deletion improved glucose tolerance and protected animals against glomerular injury and prevented podocyte loss in HFD mice. In addition, HFD mice showed more glomerular macrophages and collagen accumulation, which was prevented by Adam17 deletion. Galectin-3 expression increased in the proximal tubules and glomeruli of HFD mice and ameliorated with Adam17 deletion. In conclusion, Adam17 in proximal tubules influences glucose tolerance and participates in the kidney injury in an obese pre-diabetic murine model. The role of ADAM17 in the tubule impacts on glomerular inflammation and fibrosis.

Disintegrin and metalloproteinase domain 17 (ADAM17) activates inflammatory and fibrotic processes through the shedding of various molecules such as Tumor Necrosis Factor-α (TNF-α) or Transforming Growht Factor-α (TGF-α). There is a well-recognised link between TNF-α, obesity, inflammation, and diabetes. In physiological situations, ADAM17 is expressed mainly in the distal tubular cell while, in renal damage, its expression increases throughout the kidney including the endothelium. The aim of this study was to characterize, for the first time, an experimental mouse model fed a high-fat diet (HFD) with a specific deletion of Adam17 in endothelial cells and to analyse the effects on different renal structures. Endothelial Adam17 knockout male mice and their controls were fed a high-fat diet, to induce obesity, or standard rodent chow, for 22 weeks. Glucose tolerance, urinary albumin-to-creatinine ratio, renal histology, macrophage infiltration, and galectin-3 levels were evaluated. Results showed that obese mice presented higher blood glucose levels, dysregulated glucose homeostasis, and higher body weight compared to control mice. In addition, obese wild-type mice presented an increased albumin-to-creatinine ratio; greater glomerular size and mesangial matrix expansion; and tubular fibrosis with increased galectin-3 expression. Adam17 deletion decreased the albumin-to-creatinine ratio, glomerular mesangial index, and tubular galectin-3 expression. Moreover, macrophage infiltration in the glomeruli of obese Adam17 knockout mice was reduced as compared to obese wild-type mice. In conclusion, the expression of ADAM17 in endothelial cells impacted renal inflammation, modulating the renal function and histology in an obese pre-diabetic mouse model.

Objective: To estimate the intangible effects of alcohol misuse on the drinker's quality of life, based on general population preferences Methods: The most important effects (dimensions) were identified by means of two focus groups conducted with patients and specialists. The levels of these dimensions were combined to yield different scenarios. A sample of 300 people taken from the general Spanish population evaluated a subset of these scenarios, selected by using a fractional factorial design. We used the probability lottery equivalent method to derive the utility score for the evaluated scenarios, and the random-effects regression model to estimate the relative importance of each dimension and to derive the utility score for the rest of scenarios not directly evaluated. Results: Four main dimensions were identified (family, physical health, psychological health and social) and divided into three levels of intensity. We found a wide variation in the utilities associated with the scenarios directly evaluated (ranging from 0.09 to 0.78). The dimensions with the greatest relative importance were physical health (36.4%) and family consequences (31.3%), followed by psychological (20.5%) and social consequences (11.8%). Conclusions: Our findings confirm the benefits of adopting a heterogeneous approach to measure the effects of alcohol misuse. The estimated utilities could have both clinical and economic applications.

Objetivo: Describir la sobrecarga de las cuidadoras informales de personas dependientes e identificar las variables relacionadas. Diseño: Estudio transversal descriptivo observacional. Emplazamiento: Atención Primaria del área sur de Pontevedra. Participantes: Un total de 97 cuidadoras/es de dependientes. Mediciones principales: Mediante una entrevista personal se recogen datos socioeconómicos y del estado de salud de la cuidadora y del dependiente, el tiempo dedicado al cuidado y la carga del cuidador (Zarit abreviada). Además de una descripción de la muestra —incluyendo su nivel de sobrecarga—, se ha utilizado un contraste de medias para identificar aquellas características que inciden en la puntuación de la escala Zarit y una regresión logística para analizar aquellas que inciden en la probabilidad de experimentar sobrecarga. Resultados: Un 61,9% de las cuidadoras están sometidas a sobrecarga intensa. El ítem de la escala que más contribuye a la sobrecarga es la falta de tiempo para uno mismo, seguida de los efectos negativos en las relaciones interpersonales. El contraste de medias muestra que el grado de parentesco, el número de horas de cuidado, la salud de la cuidadora y la agresividad del dependiente producen diferencias significativas en la escala Zarit. La salud física y psicológica de la cuidadora, y la agresividad del dependiente, se asocian a la probabilidad de padecer sobrecarga. Conclusiones: Las cuidadoras informales de personas dependientes presentan un alto nivel de sobrecarga, que se relaciona tanto con sus características, como con las del dependiente. La sobrecarga replantea la necesidad de que las políticas públicas enfocadas en la dependencia adopten una visión integradora cuidadora-dependiente.

Mutations in the GBA1 gene, which encodes the lysosomal enzyme Glucocerebrosidase1 are major risk factors for Parkinson's disease (PD) and dementia with Lewy bodies (DLB). We have generated induced pluripotent stem cells (iPSCs) from fibroblasts of four PD patients carrying the N370S/wt or the L444P/wt heterozygous mutations in GBA1. The iPSCs presented a normal morphology, expressed endogenous pluripotency markers and could be differentiated into endodermal, mesodermal and ectodermal cells. These iPSCs were free from Sendai viral vectors and reprogramming factors, had a normal karyotype and maintained the original GBA1 genotype.

Background and objectives: Diabetic renal lesions can only be diagnosed by kidney biopsy. These biopsies have a high prevalence of non-diabetic lesions. The aims of the study were to determine the predictability of non-diabetic nephropathy (NDN) in diabetics and study differences in survival and renal prognosis. In addition, we evaluated histological lesions and the effect of proteinuria on survival and renal prognosis in patients with diabetic nephropathy (DN). Material and methods: A descriptive, retrospective study of kidney biopsies of diabetics between 1990 and 2013 in our centre. Results: 110 patients were included in the study: 87 men (79%), mean age 62 years (50–74), mean serum creatinine 2.6 mg/dl (0.9–4.3) and proteinuria 3.5 g/24 hours (0.5–6.5). 61.8% showed NDN, 34.5% showed DN and 3.6% showed DN + NDN. The most common NDN was IgA nephropathy (13.2%). In the multivariate analysis, creatinine (OR: 1.48, 1.011–2.172, p = 0.044), proteinuria/24 hours (OR: 0.813, 0.679–0.974, p = 0.025), duration of diabetes (OR: 0.992, 0.987–0.998, p = 0.004), age (OR: 1.068, 95% CI: 1.010–1.129, p = 0.022), and diabetic retinopathy (OR: 0.23, 0.066–0.808, p = 0.022) were independently associated with NDN. We did not find any differences in survival or renal prognosis. Concerning patients with DN, increased nodular mesangial expansion (p = 0.02) and worse renal prognosis (p = 0.004) were observed in nephrotic proteinuria as compared to non-nephrotic proteinuria. We did not find differences in patient survival. Conclusions: The most common cause of NDN was IgA nephropathy. Higher creatinine levels, shorter duration of diabetes, absence of diabetic retinopathy, lower proteinuria, and older age were risk factors for NDN. Patients with DN and nephrotic-range proteinuria had worse renal prognosis.

Antecedentes y objetivos: La afectación renal del diabético solo puede determinarse mediante biopsia renal, que presenta una elevada prevalencia de lesiones no diabéticas. Los objetivos del estudio fueron determinar la predictibilidad de nefropatía no diabética (NND) en diabéticos, estudiar diferencias de supervivencia y pronóstico renal, evaluar las lesiones histológicas en nefropatía diabética (ND) y el efecto de la proteinuria en la supervivencia y pronóstico renal en esta población. Material y métodos: Estudio descriptivo y retrospectivo de las biopsias renales de diabéticos entre 1990 y 2013 en nuestro centro. Resultados: Incluimos a 110 pacientes: 87 hombres (79%), con una edad media de 62 años (50-74), creatinina sérica media de 2,6 mg/dl (0,9-4,3) y proteinuria de 3,5 g/24 h (0,5-6,5). El 34,5% presentaban ND, el 61,8% NND y el 3,6% ND + NND. La NND más frecuente fue nefropatía IgA (13,2%). En el análisis multivariado, se asociaron de forma independiente a NND: edad (OR 1,068; IC 95%: 1,010-1,129; p = 0,022), años de diabetes (OR: 0,992; 0,987-0,998; p = 0,004), creatinina (OR: 1,48; 1,011-2,172; p = 0,044), proteinuria de 24 h (OR: 0.813; 0,679-0,974; p = 0,025) y retinopatía diabética (OR: 0,23; 0,066-0,808; p = 0,022). No hallamos diferencias de supervivencia ni de pronóstico renal. Entre los pacientes con ND, presentaban mayor expansión nodular mesangial los que tenían proteinuria nefrótica (p = 0,02), así como peor pronóstico renal (p = 0,004) comparado con proteinuria no nefrótica. No evidenciamos diferencias en la supervivencia del paciente. Conclusiones: La causa más frecuente de NND fue nefropatía IgA. Los pacientes con mayor edad, creatinina, menor duración de diabetes, ausencia de retinopatía diabética y menor proteinuria presentan mayor riesgo de NND. Los pacientes con ND y proteinuria en rango nefrótico tuvieron peor pronóstico renal.

The COVID-19 pandemic has led to frequent referrals to the emergency department on suspicion of this infection in maintenance hemodialysis (MHD) and kidney transplant (KT) patients. We aimed to describe their clinical features comparing confirmed and suspected non-confirmed COVID-19 cases during the Spanish epidemic peak. Confirmed COVID-19 ((+)COVID-19) corresponds to patient with positive RT-PCR SARS-CoV-2 assay. Non-confirmed COVID-19 ((−)COVID-19) corresponds to patients with negative RT-PCR. COVID-19 was suspected in 61 patients (40/803 KT (4.9%), 21/220 MHD (9.5%)). Prevalence of (+)COVID-19 was 3.2% in KT and 3.6% in MHD patients. Thirty-four (26 KT and 8 MHD) were (+)COVID-19 and 27 (14 KT and 13 MHD) (−)COVID-19. In comparison with (−)COVID-19 patients, (+)COVID-19 showed higher frequency of typical viral symptoms (cough, dyspnea, asthenia and myalgias), pneumonia (88.2% vs. 14.3%) and LDH and CRP while lower phosphate levels, need of hospital admission (100% vs. 63%), use of non-invasive mechanical ventilation (36% vs. 11%) and mortality (38% vs. 0%) (p < 0.001). Time from symptoms onset to admission was longer in patients who finally died than in survivors (8.5 vs. 3.8, p = 0.007). In KT and MHD patients, (+)COVID-19 shows more clinical severity than suspected non-confirmed cases. Prompt RT-PCR is mandatory to confirm COVID-19 diagnosis.

A health system’s responsiveness is the result of patient expectations for the non-medical care they receive. The objective of this study was to assess mental patients’ responsiveness to the health system in primary care, as related to the domains of dignity, autonomy, confidentiality, and communication. Data were collected from 215 people over the age of 18 with mental disorders, using the Multi-Country Survey Study (MCSS) developed by the World Health Organization. Of them, 95% reported a good experience regarding the dignity, confidentiality, communication, and autonomy domains. Regarding responsiveness, patients valued the dignity domain as the most important one (25.1%). Among the patients who experienced poor confidentiality, five out of seven earned less than 900 euros per month (Χ2 = 10.8, p = 0.004). Among those who experienced good autonomy, 85 out of 156 belonged to the working social class (90.4%), and among those who valued it as poor (16.1%), the highest proportion was for middle class people (Χ2 = 13.1, p = 0.028). The two students and 87.5% of retirees experienced this dimension as good, and most patients who valued it as poor were unemployed (43.5%) (Χ2 = 13.0, p = 0.011). Patients with a household income higher than 900 euros more frequently valued responsiveness as good, regarding those domains related to communication, with OR = 3.84, 95% CI = 1.05–14.09, and confidentiality, with OR = 10.48, 95% CI = 1.94–56.59. To conclude, as regards responsiveness in primary care, the dignity domain always obtained the best scores by people with mental disorders. Low economic income is related to a poor assessment of confidentiality. Working class patients, students, and retirees value autonomy as good.

This paper proposes several activities that encourage self-learning in engineering courses. For each activity, the context and the pedagogical issues addressed are described emphasizing strengths and weaknesses. Specifically, this work describes and implements five activities, which are: questionnaires, conceptual maps, videos, jigsaw and projects. These activities are applied in seven different knowledge fields and are conducted individually or in group depending on the nature of the subject and of the activity. Furthermore, this paper shows how the same activity can be applied in subjects of different years and how the implementation level changes, depending on the course in which it is conducted. The activities proposed have been introduced in engineering courses, but they can also be applied in any other knowledge field. Finally, the paper proposes four rubrics to assess three of the proposed activities (videos, jigsaw and project), being two of them for the project activity.

Metastasic breast cancer patients live longer thanks to the lasts improvements in their treatment. Nonetheless, there exist few studies focused on the quality of live (QoL) of those patients. The aim of our study is to improve the knowledge of metastatic breast cancer patient’s quality of life, for this purpose we have elaborated an open questionnaire as a complement to the QoL tests available. Material and methods: From June 2011 to February 2012 we conducted 24 interviews with a questionnaire based on medical and psychosocial items. Results: 58% of patients considered that treatment improved their QoL, an 83.3% expressed that the benefit of the treatment compensated for the secondary effects, complementary tests give them relieve, control over the illness and information. Only two thirds of those patients expressed to have social support, and one third said to share their grief with nobody. The main concerns expressed by the patients were in order of value: their family, loss of self-caring, fear about the future and to the illness progression. Half of the patients think that the disease has brought some value to their lives such as improving their scale of values and to improve empathy to somebody else’s problems. Conclusion: Metastatic breast cancer patient’s attitude is admirable in the way they are adapted to such a wearing situation. Nonetheless it’s imperative to offer them the help and the medical information they need to compensate for their solitude in front of the illness.

El estado emocional de las mujeres con cáncer de mama puede variar a lo largo del tiempo, en función de las dificultades físicas y psicosociales que se pueden encontrar a lo largo de la enfermedad, y que pueden continuar una vez finalizado el proceso de los tratamientos oncológicos. Cuando la paciente espera volver a recuperar su vida cotidiana, con frecuencia se mantienen emociones negativas como el miedo a las recaídas o sentirse deprimido. El objetivo del presente estudio longitudinal es conocer como evolucionan estas emociones desadaptativas, cuando se participa en una terapia de grupo orientada a mejorar la calidad de vida. La muestra final se compone de 38 mujeres con cáncer de mama que se encuentran en intervalo libre de enfermedad. Los resultados obtenidos indican que el denominado “malestar emocional” mejora a lo largo de la terapia, si bien en algunas pacientes afloran más las emociones negativas durante la terapia que al inicio de la misma. Colistin is administered as its inactive prodrug colistimethate (CMS). Selection of an individualized CMS dose for each patient is difficult due to its narrow therapeutic window, especially in patients with chronic kidney disease (CKD). Our aim was to analyze CMS use in patients with CKD. Secondary objectives were to assess the safety and efficacy of CMS in this special population. In this prospective observational cohort study of CMS-treated CKD patients, CKD was defined as the presence of a glomerular filtration rate (GFR) < 60 mL/min/m2 for more than 3 months. The administered doses of CMS were compared with those recently published in the literature. Worsened CKD at the end of treatment (EOT) was evaluated with the RIFLE (Risk, Injury, Failure, Loss of kidney function, and End-stage kidney disease) criteria. Colistin plasma concentrations (Css) were measured using high-performance liquid chromatography. Fifty-nine patients were included. Thirty-six (61.2%) were male. The median age was 76 (45⁻95) years and baseline GFR was 36.6 ± 13.6. The daily mean CMS dosage used was compared with recently recommended doses (3.36 vs. 6.07; p < 0.001). Mean Css was 0.9 (0.2⁻2.9) mg/L, and Css was ss were under the recommended target of 2 mg/dL. Despite this, clinical cure rate was high. In this patient cohort, the incidence of nephrotoxicity was similar to those found in other recent studies performed in the general population and was reversible in 52.6%. These results suggest that CMS is safe and effective in patients with CKD and may encourage physicians to adjust dosage regimens to recent recommendations in order to optimize CMS treatments.
