The detection of paroxysmal arrhythmia in patients suffering from intermittent palpitations remains an important challenge in modern cardiology.

Catheter Ablation of Stable Ventricular TachycardiaBackground In the Ventricular Tachycardia Ablation in Coronary Heart Disease (VTACH) study, an intention-to-treat approach was used and may have diminished the observed degree of treatment effect. We present a subanalysis of the VTACH study by treatment actually received. Methods and Results The VTACH study was a prospective, open, randomized controlled trial, undertaken in 16 European centers, comparing defibrillator implantation with and without ventricular tachycardia (VT) ablation in patients with stable VT, previous myocardial infarction, and reduced left-ventricular ejection fraction. Of the 52 patients in the ablation group, 7 (13%) did not receive VT ablation and 19% of patients assigned to implantable cardioverter defibrillator (ICD) only treatment group crossed over and had an ablation. The primary endpoint (first recurrence of any documented VT or ventricular fibrillation [VF]) was reached after a median of 19.5 months in the ablation group and 5.9 months in the ICD only group (P = 0.01). Overall, 685 VT/VF events occurred per year of follow-up in 22 patients of the ablation group and 4,986 events in 43 patients of the control group (P = 0.024). In the ICD only group, median numbers of VT/VF episodes were 25 (IQR 5.8–45.3) and 1.5 (IQR 0–24.8) per patient and year before and after crossover (n = 12), respectively. Conclusion On-treatment analysis of the VTACH study emphasizes the effectiveness of VT ablation in patients receiving ICD treatment because of monomorphic VT post myocardial infarction. VT ablation clearly prolonged time to recurrence of VT/VF episodes and markedly decreased VT/VF burden.

AV Conduction After TAVI and SAVR. Introduction: Atrioventricular conduction abnormalities (AVCA) may complicate transcatheter aortic valve implantation (TAVI) and surgical aortic valve replacement (SAVR). The aim of this study was to prospectively evaluate AVCA after TAVI and SAVR. Methods and Results: Among 50 patients undergoing TAVI and 25 patients undergoing SAVR a continuous 7 day Holter electrocardiogram (ECG) was recorded after the procedure. ECGs during TAVI and 12 lead ECGs before and 1 and 7 days after TAVI and SAVR were analyzed. At baseline TAVI patients were older (mean 82.1 vs 75.4 P < 0.001) had a longer PR interval (median 200 milliseconds vs 175 milliseconds P = 0.004) and broader QRS width (median 100 milliseconds vs 80 milliseconds P = 0.007) than SAVR patients. New AVCA were observed among 29 TAVI patients (58) mostly new left bundle branch block (76). Predilatation induced new AVCA in 14 TAVI patients (28). New AVCA resolved within 24 hours in 15 TAVI patients (30) and persisted in 14 TAVI (28) and 3 SAVR patients (12 P = 0.12). Among patients with persistent QRS width

Heart failure is a serious condition and equivalent to malignant disease in terms of symptom burden and mortality. At this moment only a comparatively small number of heart failure patients receive specialist palliative care. Heart failure patients may have generic palliative care needs, such as refractory multifaceted symptoms, communication and decision making issues and the requirement for family support. The Advanced Heart Failure Study Group of the Heart Failure Association of the European Society of Cardiology organized a workshop to address the issue of palliative care in heart failure to increase awareness of the need for palliative care. Additional objectives included improving the accessibility and quality of palliative care for heart failure patients and promoting the development of heart failure-orientated palliative care services across Europe. This document represents a synthesis of the presentations and discussion during the workshop and describes recommendations in the area of delivery of quality care to patients and families, education, treatment coordination, research and policy.

Implantable cardioverter-defibrillators (ICD) are increasingly used for prevention of sudden cardiac death (SCD). Although mortality risk reduction is about the same in primary and secondary prevention trials (~30%), we hypothesised that the incidence and the nature of ventricular arrhythmias is different in high risk ICD recipients without prior arrhythmias compared to patients who presented with life threatening arrhythmias.A hundred consecutive ICD recipients were allocated to 2 groups: 1) secondary prevention: an ICD was implanted for secondary prevention of episodes of ventricular tachycardia (VT) or ventricular fibrillation (VF). 2) primary prevention: patients at high risk of SCD without prior arrhythmias. They were prospectively followed and the incidence of appropriate ICD therapies was determined by reviewing stored electrograms.During a mean follow-up of 20 (10) months, the overall mortality was 5% and 5% of the patients underwent heart transplantation. Of the 67 secondary prevention patients, 40% (n = 27) had VT/VF triggering ICD therapy, whereas only 15% (n = 5) of the 33 primary prevention patients had VT/VF triggering ICD therapy (p0.05). The adjusted hazard ratio for arrhythmias triggering ICD interventions in the primary prevention group was 0.345 (95% confidence interval 0.132 to 0.902, p = 0.03).The risk of developing arrhythmias triggering appropriate ICD intervention was 65% lower among the primary prevention patients than in secondary prevention patients. Importantly, ICD therapies are not correlated with lives saved, and efficacy of ICD therapy in primary and secondary prevention cannot be drawn from these data. However, the low incidence of ICD use in primary prevention patients emphasises that efforts should be made to develop better instruments for stratification.

OBJECTIVES We sought to characterize re-entry circuits causing intra-atrial re-entrant tachycardias (IARTs) late after the repair of congenital heart disease (CHD) and to define an approach for mapping and ablation, combining anatomy, activation sequence data and entrainment mapping. BACKGROUND The development of IARTs after repair of CHD is difficult to manage and ablate due to complex anatomy, variable re-entry circuit locations and the frequent co-existence of multiple circuits. METHODS Forty-seven re-entry circuits were mapped in 20 patients with recurrent IARTs refractory to medical therapy. In the first group (n = 7), ablation was guided by entrainment mapping. In the second group (n = 13), entrainment mapping was combined with a three-dimensional electroanatomic mapping system to precisely localize the scar-related boundaries of re-entry circuits and to reconstruct the activation pattern. RESULTS Three types of right atrial macro–re-entrant circuits were identified: those related to a lateral right atriotomy scar (19 IARTs), the Eustachian isthmus (18 IARTs) or an atrial septal patch (8 IARTs). Two IARTs originated in the left atrium. Radiofrequency (RF) lesions were applied to transect critical isthmuses in the right atrium. In three patients, the combined mapping approach identified a narrow isthmuses in the lateral atrium, where the first RF lesion interrupted the circuit; the remaining circuits were interrupted by a series of RF lesions across a broader path. Overall, 38 (81%) of 47 IARTs were successfully ablated. During follow-up ranging from 3 to 46 months, 16 (80%) of 20 patients remained free of recurrence. Success was similar in the first 7 (group 1) and last 13 patients (group 2), but fluoroscopy time decreased from 60 ± 30 to 24 ± 9 min/procedure, probably related to the increasing experience and ability to monitor catheter position non-fluoroscopically. CONCLUSIONS Entrainment mapping combined with three-dimensional electroanatomic mapping allows delineation of complex re-entry circuits and critical isthmuses as targets for ablation. Radiofrequency catheter ablation is a reasonable option for treatment of IARTs related to repair of CHD.
