Abstract

Worldwide, sewer networks are designed to transport wastewater to a centralized treatment plant to be treated and returned to the environment. This process is critical for the current society, preventing waterborne illnesses, providing safe drinking water and enhancing general sanitation. To keep a sewer network perfectly operational, sampling inspections are performed constantly to identify obstructions. Typically, a Closed-Circuit Television system is used to record the inside of pipes and report the obstruction level, which may trigger a cleaning operative. Currently, the obstruction level assessment is done manually, which is time-consuming and inconsistent. In this work, we design a methodology to train a Convolutional Neural Network for identifying the level of obstruction in pipes, thus reducing the human effort required on such a frequent and repetitive task. We gathered a database of videos that are explored and adapted to generate useful frames to fed into the model. Our resulting classifier obtains deployment ready performances. To validate the consistency of the approach and its industrial applicability, we integrate the Layer-wise Relevance Propagation explainability technique, which enables us to further understand the behavior of the neural network for this task. In the end, the proposed system can provide higher speed, accuracy, and consistency in the process of sewer examination. Our analysis also uncovers some guidelines on how to further improve the quality of the data gathering methodology.

Abstract

Summary: Background: The WHO has set a goal to decrease viral hepatitis‐related fatalities by 65% by 2030. Aims: To locate and retrieve to care all individuals diagnosed with hepatitis B, D or C, and investigate why they were not linked to appropriate medical management. Methods: We conducted a retrospective‐prospective search for patients with hepatitis B, D or C virus (HBV, HDV and HCV) infection in the central laboratory database of the Barcelona northern health area (catchment population, 450,000). We reviewed records and contacted candidates for retrieval who were offered a specialist medical visit. Results: We reviewed records of 3731 patients with viral hepatitis (January 2019–December 2022): 1763 HBsAg+, 69 anti‐HDV+ and 1899 HCV‐RNA+. Among these, 644 (37%) HBV, 20 (29%) HDV and 1116 (56%) HCV patients were not currently linked to care. The proportion of patients receiving appropriate care was higher in HBV and HDV (p < 0.0001), and a higher percentage of unlinked hepatitis C patients had low life expectancy/comorbidities (39%; p < 0.0001). After implementing the linkage strategy, 254 HBV, 16 HDV and 54 HCV patients were successfully reintegrated into care. Among 1780 patients requiring linkage, 638 (35.8%) had moved to another health area or were missing essential contact data. Conclusions: Among patients with viral hepatitis who required appropriate specialist care and were eligible for contact, 64% with HBV, 100% with HDV and 27% with HCV were successfully reintegrated into care. Abstract

Background & Aims: Hepatitis B infection is the most frequent cause of chronic hepatitis and liver cancer worldwide. Active searching for individuals with chronic hepatitis B has been proposed as a strategy to achieve the elimination of this virus. The primary aim of this study was to link to specialists HBsAg‐positive individuals detected in a laboratory database and to characterize individuals who were not linked to care. Methods: We performed a retrospective–prospective evaluation of all HBsAg‐positive serum samples identified in the central laboratory of the Northern Barcelona area between January 2018 and June 2022. After reviewing the patients' clinical charts, all those not linked to care were given an appointment with a specialist. Results: Medical records of 2765 different HBsAg‐positive serum samples were reviewed and 2590 individuals were identified: 844 (32.6%) were not linked to a specialist, 653 were candidates for linkage, and 344 attended the specialist visit. The two main reasons why they were not under specialist care were administrative issues, such as living in another region (12.1%) and lacking contact details (4.1%), and low life expectancy (2.8%). Individuals who did not attend their scheduled visit were mainly young [38.1 ± 12.9 vs. 44.0 ± 14.0 (p <.001)], non‐White European [75.3% vs. 58.1% (p <.001)] and men [70.7% vs. 56.4% (p <.001)]. Conclusions: One in every three HBsAg‐positive individuals in our setting was not currently under specialist care. Of particular note, half of them had never attended a specialist consultation, an essential step for evaluating the disease and starting therapy in some countries. [ABSTRACT FROM AUTHOR]

Abstract

Artificial intelligence (AI) generative models driven by the integration of AI and natural language processing technologies, such as OpenAI’s chatbot generative pre-trained transformer large language model (LLM), are receiving much public attention and have the potential to transform personalized medicine. Dialysis patients are highly dependent on technology and their treatment generates a challenging large volume of data that has to be analyzed for knowledge extraction. We argue that, by integrating the data acquired from hemodialysis treatments with the powerful conversational capabilities of LLMs, nephrologists could personalize treatments adapted to patients’ lifestyles and preferences. We also argue that this new conversational AI integrated with a personalized patient-computer interface will enhance patients’ engagement and self-care by providing them with a more personalized experience. However, generative AI models require continuous and accurate updates of data, and expert supervision and must address potential biases and limitations. Dialysis patients can also benefit from other new emerging technologies such as Digital Twins with which patients’ care can also be addressed from a personalized medicine perspective. In this paper, we will revise LLMs potential strengths in terms of their contribution to personalized medicine, and, in particular, their potential impact, and limitations in nephrology. Nephrologists’ collaboration with AI academia and companies, to develop algorithms and models that are more transparent, understandable, and trustworthy, will be crucial for the next generation of dialysis patients. Abstract

In 2016, the Hepatitis B and C Public Policy Association (HepBCPPA), gathered all the main stakeholders in the field of hepatitis C virus (HCV) to launch the now landmark HCV Elimination Manifesto, calling for the elimination of HCV in the EU by 2030. Since then, many European countries have made progress towards HCV elimination. Multiple programmes—from the municipality level to the EU level—were launched, resulting in an overall decrease in viremic HCV infections and liver-related mortality. However, as of 2021, most countries are not on track to reach the 2030 HCV elimination targets set by the WHO. Moreover, the COVID-19 pandemic has resulted in a decrease in HCV diagnoses and fewer direct-acting antiviral treatment initiations in 2020. Diagnostic and therapeutic tools to easily diagnose and treat chronic HCV infection are now well established. Treating all patients with chronic HCV infection is more cost-saving than treating and caring for patients with liver-related complications, decompensated cirrhosis or hepatocellular carcinoma. It is more important than ever to reinforce and scale-up action towards HCV elimination. Yet, efforts urgently need the dedicated commitment of policymakers at all governmental and policy levels. Therefore, the third EU Policy Summit, held in March 2021, featured EU parliamentarians and other key decision makers to promote dialogue and take strides towards securing wider EU commitment to advance and achieve HCV elimination by 2030. We have summarized the key action points and reported the ‘Call-to-Action’ statement supported by all the major relevant European associations in the field.

50. Modelos de machine learning para la optimización en tiempo real de la dosificación de coagulante en la ETAP de Sant Joan Despí

Abstract

The goal of this work is to optimize the sedimentation stage of a water purification plant, by real-time adjusting the use of coagulant in relation to the quality of the incoming water, in order to reduce costs and prevent the consequences of unexpected changes. Coagulants are used to aggregate particles and suspended impurities in the water, allowing them to settle. To achieve this goal, a predictive model has been build using the Extra Trees Regressor algorithm, which is capable of calculating the coagulant setpoint based on the quality parameters of the river, the dosage of dioxide in the water, and the turbidity in the sedimentation stage. The model has been developed using a set of 57,481 samples collected at the ETAP of Sant Joan Despí (data cleaning was carried out and 15,878 samples were eliminated, 10% has been used for validation). Finally, a validation of the model was performed which gave an R2 of 97.8% and a MAPE of 0.05%, which demonstrates its viability as a useful tool for the improve, efficiency and quality of the water treatment process., El presente trabajo tiene como objetivo optimizar la etapa de decantación de una planta de potabilización de agua, ajustando a tiempo real el uso del coagulante en relación a la calidad del agua de entrada, con el fin de reducir costos y prevenir las consecuencias de cambios inesperados. Los coagulantes son utilizados para aglomerar partículas e impurezas suspendidas en el agua, permitiendo que estas sedimenten. Para llevar a cabo el objetivo se ha construido un modelo predictivo mediante el algoritmo Extra Trees Regressor, capaz de calcular la consigna de coagulante en función de los parámetros de calidad del río, la dosificación de dióxido en el agua y la turbidez en la etapa de decantación. El modelo ha sido desarrollado mediante un conjunto de 57.481 muestras recolectadas en la ETAP de Sant Joan Despí (se ha realizado una limpieza de datos con la que se han eliminado 15.878 muestras y se ha usado un 10% para la validación). Finalmente se ha realizado una validación del modelo que ha dado un R2 del 97.8% y un MAPE del 0.05%, que demuestran su viabilidad como herramienta útil para la mejora, la eficiencia y calidad del proceso de tratamiento del agua., Aquest treball té com a objectiu optimitzar l'etapa de decantació d'una planta de potabilització d'aigua, ajustant en temps real l'ús del coagulant en relació a la qualitat de l'aigua d'entrada, amb la finalitat de reduir costos i prevenir les conseqüències dels canvis inesperats. Els coagulants s'utilitzen per aglomerar partícules i impureses suspeses en l'aigua, permetent que aquestes sedimentin. Per portar a terme l'objectiu s'ha construït un model predictiu mitjançant l'algoritme Extra Trees Regressor, capaç de calcular la consigna de coagulant en funció dels paràmetres de qualitat del riu, la dosificació de diòxid en l'aigua i la terbolesa en l'etapa de decantació. El model s'ha desenvolupat mitjançant un conjunt de 57.481 mostres recollides en l'ETAP de Sant Joan Despí (s'ha realitzat una neteja de dades amb la qual s'han eliminat 15.878 mostres i s'ha utilitzat un 10% per a la validació). Finalment s'ha realitzat una validació del model que ha donat un R2 del 97.8% i un MAPE del 0.05%, que demostren la seva viabilitat com a eina útil per a la millora, l'eficiència i qualitat del procés de tractament de l'aigua.
