Abstract

This systematic review and meta-analysis compare the incidences of complications, reoperations and hardware removal between anterior and dorsal plating after corrective osteotomy for dorsally angulated distal radial malunions. A total of 403 patients were included; 253 patients underwent anterior corrective osteotomy and 150 underwent dorsal corrective osteotomy. Mean follow-up was 30 months. Anterior plating was associated with fewer reoperations (9% vs. 28%), less hardware removal (3% vs. 18%) and comparable major complications (5% vs. 6%) compared to dorsal plating. The adjusted model showed a significant reduction (approximately 85%) in the odds of reoperation and hardware removal in the anterior group. There was no difference in major or minor complications between the groups. Surgeons should be fully aware of the increased risks when using dorsal plate fixation after corrective osteotomy for dorsally angulated distal radial malunions. [ABSTRACT FROM AUTHOR]

18. Influence of health insurance on withdrawal of life sustaining treatment for patients with isolated traumatic brain injury: a retrospective multi-center observational cohort study.

Abstract

Background: Healthcare inequities for patients with traumatic brain injury (TBI) represent a major priority area for trauma quality improvement. We hypothesized a relationship between health insurance status and timing of withdrawal of life sustaining treatment (WLST) for adults with severe TBI. Methods: This multicenter retrospective observational cohort study utilized data collected between 2017 and 2020. We identified adult (age ≥ 16) patients with isolated severe TBI admitted participating Trauma Quality Improvement Program centers. We determined the relationship between insurance status (public, private, and uninsured) and the timing of WLST using a competing risk survival analysis framework adjusting for baseline, clinical, injury and trauma center characteristics. Multivariable cause-specific Cox regressions were used to compute adjusted hazard ratios (HR) reflecting timing of WLST, accounting for mortality events. We also quantified the between-center residual variability in WLST using the median odds ratio (MOR) and measured insurance status association with access to rehabilitation at discharge. Results: We identified 42,111 adults with isolated severe TBI treated across 509 trauma centers across North America. There were 10,771 (25.6%) WLST events in the cohort and a higher unadjusted incidence of WLST events was evident in public insurance patients compared to private or uninsured groups. After adjustment, WLST occurred earlier for publicly insured (HR 1.07, 95% CI 1.02–1.12) and uninsured patients (HR 1.29, 95% CI 1.18–1.41) compared to privately insured patients. Access to rehabilitation was lower for both publicly insured and uninsured patients compared to patients with private insurance. Accounting for case-mix, the MOR was 1.49 (95% CI 1.43–1.55), reflecting significant residual between-center variation in WLST decision-making. Conclusions: Our findings highlight the presence of disparate WLST practices independently associated with health insurance status. Additionally, these results emphasize between-center variability in WLST, persisting despite adjustments for measurable patient and trauma center characteristics. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: Obesity has been recognized as a major public health problem worldwide that requires preventive action. Prevention is best targeted at children, there is lack of quantitative and qualitative research on obesity prevention in children and most have been conducted in western countries. The aim of this study is to describe the prevalence of obesity among 6-7 years old school children, investigate contributing factors and identify potential components for an intervention programme to prevent obesity amongst children in the State of Qatar. Methods The study consisted of two distinct parts: cross sectional survey and focus groups with a range of stakeholders. Results: Whilst there was a high prevalence of overweight and obesity 16%, underweight was also prevalent (21.7%). The results of the focus group discussions indicate that causes of childhood obesity are multifactorial, and a multi-sector approach to prevention would be acceptable. Some of the important barriers that need to be considered in developing interventions were highlighted. Conclusion: In Qatar there is coexistence of underweight and obesity in primary school children. Qualitative results suggest that a multi-sector approach to prevention would be acceptable toward dietary and physical activity, and suggested potential components for an intervention programme in preventing obesity amongst children in the State of Qatar.

Abstract

This study analyzes Qays ibn Al-Khateem's poem "anna sarabti" to show the complexities of dream and its impact on the artistic structures and literary aesthetics of the poem. The study adopted the literary psychoanalytic approach as a theoretical framework. The analysis of the study revealed that the poet was hunted by his dreams that served as an outlet allowing him to evade his bitter life. Also, it showed that the poet succeeded in employing dream in his poem as an appropriate medium through which he was able to achieve his daydreams, desires and endeavor for the redemption of the reality of his bitter life. Abstract

This study aims at identifying the Effect of Using Technology and Modern Techniques (Ayat) that is based on the connectivism theory, in developing Quran recitation skills among curriculum's students of secondary school in Saudi Arabia. In order to achieve the study's objectives, the researcher uses the semi-experimental method. The study examines a sample of (71) students of secondary school curriculum system in the first semester of the academic year 1438H - 1439H in the government schools in Riyadh. They were divided into two equal groups (experimental and control). Students of the experimental group were taught according to the software program based on connectivism theory, and the others were taught in the traditional way. An oral test is used as a study tool to measure the existence of recitation skills (correct reading, smooth reading, recitation, and chanting), and to measure the Effect of Using Technology and Modern Technique in developing these skills. After analyzing the statistical data, the results of the study indicated the Effect of Using of the Modern Techniques, based on the connectivism theory, in the development of recitation skills (correct reading, smooth reading, recitation, and chanting) among curriculum's students of secondary school in Saudi Arabia. The study also showed a statistically significant difference of (0.05) between the average scores of the two groups in the post-measurement of the recitation test in the recitation skills such as (correct reading, smooth reading, recitation, and chanting) after adjusting the pre-measurement.

Carbon monoxide (CO) poisoning is a frequent cause of accidental and intentional injuries in the world. Many researchers suggest that the use of hyperbaric oxygen (HBO) in CO poisoning, prevents the development of neurological sequelae. This led to the widespread use of HBO in the treatment of patients with carbon monoxide poisoning. The National Health Fund (NHF) reimburses patients treatment in a multi-person hyperbaric chamber. In the Hospital Emergency Department (HED) in Bydgoszcz there is a one-person hyperbaric chamber, which is mainly used to treat patients poisoned with CO. Treatment is also refunded by the NHF. The cost of treating the patient in the monoplace chamber is included in the contract - (lump sum) from NHF for HED.

Abstract

The obligations and rights of the parties involved are allocated in the agreed terms of the contract, and the parties’ actions are generally administered by the governing law. Therefore, legal controls and ample statutory regulations are expected to be provided by the governing law to regulate the terms, in addition to offering adequate guidance to courts in interpreting these terms. This dissertation aims to investigate the notion of unforeseen physical conditions, the different standard form of contracts provisions relating to unforeseen physical conditions, and the other shared common notions of force majeure, imprevision, and misrepresentation. This study is intended to assess United Arab Emirates (UAE) legislation in comparison with its common law and civil law counterparts. The research findings provide a need for statutory intervention in the UAE in matters of unforeseen physical conditions for the construction industry.

47. Perioperative outcomes after minimally invasive and open surgery for treatment of spine metastases: a systematic review and meta-analysis.

Abstract

Objective: This systematic review and meta-analysis compared minimally invasive surgery (MIS) to open surgery for treatment of spinal metastases with respect to perioperative outcomes. Few studies have systemically assessed the body of evidence on this topic., Methods: A systematic review of EMBASE and PubMed from database inception to December 2023 was performed to identify studies comparing MIS with open surgery for the treatment of spine metastases. Nine outcomes were collected: estimated blood loss (EBL), operative time, hospital length of stay (LOS), risk of revision, risk of neurological deterioration, likelihood of receiving postoperative radiation therapy, time to radiation therapy, time to chemotherapy, and treatment of pain measured through patient-reported visual analog scale (VAS) scores. Meta regression was used to estimate adjusted mean differences (aMDs) and adjusted odds ratios (aORs) for outcomes. Certainty of evidence was appraised using the Grading of Recommendations, Assessment, Development and Evaluations approach., Results: There were 34 eligible studies including 1656 patients with spinal metastases; 904 (54.6%) patients were treated with MIS and 752 (45.4%) were treated with open surgery. MIS was associated with significantly less blood loss (aMD -602 mL, 95% CI -1204 to -0.2 mL; I2 = 97%) with a moderate certainty of evidence. MIS was found to be noninferior with respect to operative time (aMD -2.6 minutes, 95% CI -53.3 to 48.1 minutes; I2 = 88%), risk of revision (aOR 0.9, 95% CI 0.8-1.1; I2 < 0.01), risk of neurological deterioration (aOR 0.9, 95% CI 0.8-1.0; I2 < 0.01), likelihood of postoperative radiation therapy (aOR 0.9, 95% CI 0.7-1.4; I2 < 0.01), and postoperative VAS score (aMD -0.6, 95% CI -1.5 to 0.4; I2 = 52%) with low certainty of evidence. MIS was associated with significantly shorter time to chemotherapy (MD -0.9 weeks, 95% CI -1.9 to -0.01 weeks; I2 = 22%), with very low certainty of evidence. Inferences for LOS and time to radiation were indeterminate; however, we found a trend toward earlier radiation therapy with MIS that was significant in the subgroup of patients treated with decompression and fusion., Conclusions: Treatment with MIS compared with open surgery was associated with reduced EBL, shorter time to chemotherapy, similar operative time, and similar reductions in postoperative pain. Limitations were largely due to heterogeneity across studies. Future research among subgroups is very likely to improve certainty in the comparative effect estimates.

48. Timing of surgery for children and adolescents sustaining complete traumatic spinal cord injury.

Abstract

Objective: Spinal cord injury (SCI) trials have historically underrepresented pediatric patients. There are limited pediatric data examining the influence of surgical timing on complications and mortality for children and adolescents who have sustained complete traumatic SCI., Methods: The following multicenter cohort study used Trauma Quality Improvement Program data from 2010 to 2020. The authors identified pediatric patients (aged < 18 years) who sustained complete traumatic SCI and underwent surgical intervention within 7 days of admission. Propensity score matching was performed between patients who underwent surgery within 24 hours versus ≥ 24 hours. The authors then assessed differences for the following outcomes: major in-hospital complications, immobility-related complications, length of stay (LOS), and mortality., Results: There were 837 patients with complete traumatic SCI managed across 297 trauma centers identified for study inclusion (70% underwent early surgery). After matching, 494 patients were available for analysis. Patients undergoing delayed surgery experienced longer ICU LOS (mean difference 3.74 days, 95% CI 0.91-6.57 days) and more major in-hospital complications (OR 1.77, 95% CI 1.16-2.73) and immobility-related complications (OR 2.09, 95% CI 1.25-3.56). There were no differences in mortality between groups. Younger age, non-White race, penetrating injuries, lower Glasgow Coma Scale score at admission, severe concomitant abdominal injuries, and motor vehicle collision injury mechanisms were associated with increased time to surgery., Conclusions: The authors demonstrated an association between early surgery and shorter ICU LOS and reduced in-hospital complications. Future work is needed to quantify the impact of surgical timing on functional neurological outcomes and to explore upstream social determinants of health influencing timing of surgery.

Objective: To determine the association between residence in racialized neighborhoods with direct healthcare expenditure and days at home (DAH) after moderate to severe traumatic brain injury (TBI)., Summary Background Data: Differences in ethno-racial background have been associated with health outcome disparities. Much of this prior research was conducted in settings without universal healthcare coverage. The influence of ethno-racial background on health outcomes after TBI in universal healthcare settings remains unclear., Methods: This retrospective multicenter cohort study utilized linked administrative health data to identify adults sustaining moderate to severe TBI between 2009-2021. The primary exposure was an area-level index corresponding to the degree of racialized and immigrant populations within neighborhoods of residence (Q1-least racialized; Q5-most racialized). Co-primary outcomes were direct healthcare expenditure and DAH365days after injury. Secondary outcomes included discharge to rehabilitation and functional independence measure (FIM) scores at rehabilitation discharge., Results: 6,188 patients met inclusion criteria. Patients in the most racialized neighborhoods incurred higher crude and adjusted direct healthcare costs compared to those in the least racialized neighborhoods. This effect was driven predominantly by physician claims and acute care costs. There were no significant differences in crude or adjusted DAH across quintiles. Access to rehabilitation and discharge FIM scores were comparable for patients residing different racialized neighborhood quintiles., Conclusions: Despite differences in healthcare expenditure, this study found similar home time, access to rehab and discharge FIM scores for TBI patients according to racialized neighborhood residence.
