We used bipedicle V-Y 'cup' flaps to reconstruct 13 consecutive volar oblique fingertip amputations encountered in a nearly 2-year period. All flaps survived completely with no bone protuberance, scarring or neuroma evident at the end of follow-up (mean, 7 months; range 4–13 months). Two of the 13 patients had moderate or severe tenderness, three had moderate or severe cold intolerance and two had noticeable hook nails. There were no significant (≥10°) flexion contracture in the interphalangeal joints. The mean static two-point discrimination score was 4 mm (range 3–6) and the modal Semmes–Weinstein monofilament test score was 3.6 g (range 3.2–4.2). All patients were satisfied with their surgical outcomes. The bipedicle V-Y 'cup' flap provides good results in terms of fingertip contour, padding and sensation, and has a low complication rate when used to treat volar oblique fingertip amputations. 29. Ayak İkinci Parmak Transplantasyonu İle Üç Nokta Tutmasının (Tripod Pinch) Rekonstrüksiyonu

- Abstract
Turizm Fakültesi öğrencisi olan 20 yaşındaki bir hasta (dominanat) elini kıyma makinesine kaptırdıktan 6 saat sonra ameliyata alındı. Başparmak dışındaki tüm parmakları roksimal falanks seviyesinden ampute olup (metacarpal el) sadece 4. Parmağın ampute bölümü replante edilebilir durmdaydı. İlk aşamada bu parmak 2. parmak güdüğüne transfer edildi. 1 ay sonra sol ayak 2. parmağı, elde 3. parmak güdüğüne transplante edildi. Post-operatif 6. ayda her iki parmakta reinnervasyonun sağlandığı, ayaktan transfer edilen parmakta daha güçlü olmak üzere pinch ve tutma fonksiyonlarının başarıyla gerçekleştirebildiği tespit edildi. Multiple parmak amputasyonlarında üç noktadan tutma fonksiyonunu kazandırmak için yan yana en az iki parmağın rekonstrükte edilmesi gerekmektedir. Üç noktadan tutma, iki parmak pulpasıyla tutma (pulp-to-pulpe pinch)'ya göre daha sağlam olmakla birlikte,parmakların lateral stabilitesinde, asarak kaldırma kuvvetinde ve ince el hareketleri sırasında manipulasyon kabiliyetinde artışa yol açmaktadır.

İZOLE MENISKUS YIRTIKLARINDA ARTROSKOPIK MENİSEKTOMİ ÖZET: Çukurova Üniversitesi Tıp Fakültesi, Ortopedi ve Travmatoloji Anabilim Dalında Kasım 1990 ile Mayıs 1996 tarihleri arasında, izole meniskus yırtığı nedeniyle artroskopik menisektomi uygulanan ve en az 6 aylık takipleri bulunan 218 hastanın 232 dizinin fonksiyonel skorları Lysholm, aktivite düzeyleri ise Tegner kriterlerine göre değerlendirildi. Ayrıca postoperatif 1 yıldan fazla takip edilen olguların diz grafileri çekilerek Fairbank radyolojik dejenerasyon bulguları açısından, preoperatif grafileri ile kıyaslanarak değerlendirildi. Bu çalışma ile hastaların yaşı, preoperatif semptomatik sürenin uzunluğu, yırtığın tipi ve eklem kıkırdak dejenerasyon düzeyinin fonksiyonel sonuçlar üzerinde etkili olduğu, radyolojik dejenerasyon bulguların ilerlemesi ise çıkarılan meniskus miktarı ile ilgili olduğu sonucuna varıldı. Artroskopik menisektominin tatminkar fonksiyonel sonuçları ve düşük morbidite ile komplikasyon oranı, yöntemin tercih edilen bir prosedür olmasındaki en önemli faktörlerdir. Meniskus yırtıklarının tedavisinde, önemli fonksiyonları nedeniyle, parsiyel menisektomi gibi meniskusları korumaya yönelik girişimlerin tercih edilmesi gerektiği kanısındayız. ARTHROSCOPIC MENISCECTOMY IN ISOLATED MENISCAL TEARS SUMMARY: Between November 1990 - May 1996, 232 knees of 218 patients underwent arthroscopic meniscectomy for isolated meniscal tear in the Department of Orthopaedics and Traumatology of Çukurova University, Faculty of Medicine. Patients who followed minimum of 6 months were evaluated by Lysholm's functional scores and Tegner criterions of activity level. Preoperative and the latest postoperative knee roentgenograms of patients followed more than 1 year were also compared according to Fairbanks degenerative X-ray findings. As a result of this study, it is concluded that functional scores were all affected by age, preoperative symptomatic duration, tear type and degeneration level of cartilage. Moreover derangement of radiological findings of degeneration was directly proportional to the amount of meniscus removed. Satisfactory functional results, low morbidity and complication rates are the main factors that popularized arthroscopic meniscectomy. Since the functions of the meniscii are very important, procedures like partial meniscectomy should be prefered in meniscal tear treatment. Amaç: El bileği ve parmakların ekstensör mekanizmasındaki kayıplar, günlük hayatta kavrama-tutma-bırakma eylemlerin gerçekleştirilmesinde ve bu eylemlerin kuvvetinde zaafiyete ve ayrıca ciddi kozmetik problemlere yol açan ve bu nedenle ciddi morbidite yaratan klinik tablolardır. Bu durumlarda, etiyolojileri ne olursa olsun, tedavi seçenekleri arasındaki son tercih tendon transferleridir. Bunun temel sebebi tendon transferinin bir feda ediş girişimi olmasıdır. El ekstensör mekanizmasındaki problemler için uygulanan tendon transferlerinin diğer transferlere göre daha öngörülebilir sonuçlara sahip olduğu öne sürülmektedir. Bununla birlikte transfer gerginliğinin nasıl ayarlanması gerektiği ve sonuçlar üzerindeki etkileri konusu halen belirsizdir. Bu yüzden klinikten kliniğe, cerrahiyi gerçekleştiren kişiden kişiye ve hastadan hastaya değişen sonuçlar bildirilmektedir. Bu çalışmada el bileği ve parmaklarda ekstensör mekanizma restorasyonu için tendon transferi yapılan hastalarda ayarlanan gerginliğin tüm yönlerdeki etkilerini ve gelişebilen fonksiyonel yetersizliklerin hastalardaki subjektif ve objektif bulgularla ilişkisini araştırdık.Yöntem: 01.09.2007 ile 01.02.2019 tarihleri arasında el bileği ve parmakların ekstensiyon mekanizmasındaki yetmezlik nedeniyle polikliniğe başvuran hastalara ait bilgiler dijital arşiv sisteminden retrospektif olarak derlendi. Çalışma planı içerisinde yer alan hastaların değerlendirilmesi geldikleri son poliklinik kontrollerinde yapıldı. Çalışmaya toplamda 65 (46 erkek, 19 kadın) hasta dahil edildi. Ortalama yaş 30,8±17,8 yıl, ortalama takip süresi ise 47,4±34,4 ay idi. Hastalar etiyolojik olarak periferik sinir sistemi hasarı (n: 35), kas-tendon ünitesi hasarı (n: 22), ve santral sinir sistemi hasarı (n: 8) olarak 3 ana gruba ayrıldı. Hastalarda toplam 124 ekstensiyon fonksiyon restorasyonu (51 başparmak ekstansiyonu, 39 diğer parmak/lar ekstensiyonu ve 34 el bilek ekstansiyonu) yapılmıştı. Tüm yönlerde eklem hareket açıklıkları, kavrama, sıkıştırma (pinç) güçleri, QDASH ve Mayo El Bileği skorları ve sübjektif memnuniyet skorları değerlendirildi. Etiyoloji, restore edilen fonksiyon, kullanılan tendon, zaman, yaş gibi farklı değişkenler temelinde istatistiksel analizler yapıldı. 38. The anterior center edge angle: a study of 102 volunteers

- Abstract
Objectives. – To measure the anterior center edge (VCE) angle that reflects anterior coverage of the femoral head.Patients and methods. – False profile views of both hips of 102 volunteers, 23 male and 79 female, were taken between october 2000 and october 2001. Radiographs with evidence of degenerative hip disease and those with poor image quality were excluded from the study. This left 181 radiographs.Results. – An orthopedic surgeon and a radiologist used a standard protractor to examine each of the 181 radiographs twice, at an interval of 24 h. The mean VCE angle was 49.27 ± 7.77° (range, 24.75–68.75), a value different from those found in previous studies. No significant intra-observer or interobserver differences were found.Conclusions. – Our findings may contribute to the determination of a new parameter for evaluating anterior femoral head covering. In patients with acetabular dysplasia and deficient anterior coverage, this parameter may prove useful for selecting patients for surgery, planning the procedure, and evaluating postoperative results. Humerus suprakondiler kırıkları çocukluk döneminin sık görülen ve ciddikomplikasyonlara yol açan kırıklardır. Bu kırıklardan sonra geç dönemdebelirginleşebilen dirsek bölgesi deformiteleri kozmetik açıdan kötü görünümün yanısıra fonksiyonel kayıplar ve ek komplikasyonlara yol açabilirler. Bu deformitelerin ensık karşılaşılanı kubitus varustur. Kubitus varus, isminin uyandırdığı zihinsel algıdanfarklı olarak, sadece tek ve koronal planlı bir deformite değildir. Aksiyel planda içrotasyon ve sagital planda hiperekstensiyon, eşlik eden deformite bileşenleridir.Literatürde dirsek bölgesi deformitelerinin rotasyonel plan bileşenlerinin öneminesıkça vurgu yapılmasına rağmen ölçülmeleri ve değerlendirilmeleri yönünde sınırlısayıda çalışma vardır. Bu çalışmaların çoğu iyonizan ışın maruziyeti gerektirenbilgisayarlı tomografi temellidir. Fizik muayeneye dayanan tek çalışma Yamamototarafından tanımlanmış ancak yaygınlık kazanmamıştır. Bu çalışmada güncel dijitalteknolojilerden yararlanılarak çocuk suprakondiler kırıkları sonrası distal fragmanınrotasyonel pozisyonunun ölçülmesi için ışın maruziyeti gerektirmeyen tekrarlanabilirbir ölçüm tekniği geliştirmeyi ve elde edilen verilerin klinik sonuçlarla ilişkisiniaraştırmayı amaçladık.YöntemBu çalışma Ocak 2009 – Aralık 2015 tarihleri arasında Gartland tip 2 ve 3deplase humerus suprakondiler kırığı için ameliyat edilmiş, ameliyat yaşları 5,6±2,85,son kontrol esnasında yaşları 10,4±3,3, ortalama takip süreleri 59,5±21,7 ay olan 28erkek ve 17 kız olmak üzere 45 hastada yapıldı. Son kontrolde olgular eklemhareketleri, kas güçleri ve nörovasküler durum açısından muayene edildi vekozmetik-fonksiyonel statüye dayalı Flynn kriterlerine göre skorlama yapıldı.Standardize klinik görüntü almak için tasarlanan düzenek yardımıyla dirsekekstensiyon ve önkol supinasyondayken taşıma açısı (f-TA); dirsek fleksiyonda veönkol supinasyondayken kol-önkol açısı (f-KÖA); omuz maksimum pasif dışrotasyonda, dirsek 90 derece fleksiyonda ve önkol supinasyondaykentransepikondiler-yer düzlemi açısı (f90°-RDA) ve omuz maksimum pasif dışrotasyonda, dirsek tam fleksiyonda ve önkol supinasyondayken transepikondiler-yerdüzlemi açısı (fTF-RDA) ölçülebilecek şekilde dijital fotğraflar elde edildi. AyrıcaYamamoto'nun rotasyonel deformite tayini için tanımladığı pozisyonda da fotoğraf6(fY-RDA) elde edildi. Yukarıda belirtilen açılar ile birlikte olguların son kontrollerindekiradyografik humeroulnar (rHUA), Baumann(rBA), metafizodiyafizer (rMDA) ve lateralhumerokapitellar (rLHKA) açılar ile trokleokapitellar indeks (rTCI) üç ayrı gözlemcitarafından birbirinden bağımsız olarak ölçüldü. Ölçümler, gözlemciler arası uyumaçısından analiz edildi. Daha sonra ölçümlerin birbirileri, olguların Flynn kliniksonuçları, tespitte kullanılan K teli sayısı ve olguların ameliyat yaşı-takip sürelerikombinasyonu ile ilişkileri analiz edildi.BulgularFotoğrafik açı ölçümlerinin hepsinde istatistiksel açıdan anlamlı (p0,80 ve 0,800,05).Olguların son kontroldeki Flynn klinik sonuçları ile rBA hariç diğer tüm koronalplan ölçümleri (fTA, rHUA, rMDA ve rTCI) anlamlı korelasyon ilişkisi (p60 ay (Grup B); yaş>5,5 yıl,takip süresi5,5 yıl, takip süresi>60 ay (Grup D)olmak üzere 4 gruba ayrıldı. Bu gruplar tüm ölçümler açısından birbirleriylekıyaslandığında Grup A ile Grup D arasında rHUA açısından istatistiksel olarak7anlamlı fark saptandı (p=0,043). Literatür ile yapılan karşılaştırmada çalışmagrubumuzun yaş, cinsiyet, taraf, kırık tipi ve tespit şekli itibarıyla humerussuprakondiler kırık geçiren çocuk popülasyonunu temsil ettiği anlaşılmaktadır.Fotoğrafik ve radyografik ölçümlerin tamamında gözlemciler arası uyum bulunmasıkullanılan teknikler ile bu ölçümlerin güvenilir, tekrarlanabilir ve normalden küçükfarkları gösterebilecek hassasiyette yapılabildiğini göstermiştir.Farklı parametreler arası ilişki analizleri temelinde bu çalışmadatanımladığımız aksiyel plan ölçüm tekniklerinin literatürde bulunan tek klinikrotasyonel değerlendirme yöntemi olan Yamamoto yöntemine göre daha etkin vebasit olduğu sonucuna vardık. Koronal plana ait fotoğrafik ve radyografik tümparametreler arasında anlamlı korelasyon ilişkisi bulunması nedeniyle rezidüdeformite açısından subklinik seyreden olgularımızda temel açısal değişimin koronalplanda olduğuna kanaat getirdik. Kullanılan yöntemlerin bu plandaki küçük açısaldeğişimleri dahil gösterebileceğini düşünüyoruz.Çalışmamızda yapılan ölçümlerin klinik sonuçlar ile ilişkisinin analizi temelindeFlynn klinik değerlendirme kriterlerinin aksiyel plan deformitelerini yansıtamadığı,dolayısıyla bu yönden daha kapsayıcı kriterlerin tanımlanmasına gereksinimolabileceği sonucuna vardık.Ölçülen açıların zamana ilişkin parametrelerle ilişkisinin analizi temelindeçocukların ameliyat yaşı ve takip süresi büyüdükçe klinik açıdan belirgin olmazsa daradyografik humeroulnar açı bakımından daha iyi sonuçlar elde edilebileceği kanısınavardık.Sonuç olarak bu çalışmada tanımlanan dijital fotoğraflama yöntemi ile, dirseğinrotasyonel plan dahil olmak üzere, olası deformitelere dair açı ölçümlerinde8standardizasyon sorunlarının çözümlenebileceği, birden fazla gözlemci için görüntüsağlanabileceği, tekrarlanabilir ve güvenilir düzeyde dijital ölçümler ve arşivlemeyapılabileceği kanısına vardık. Humerus supracondylar fractures in children are common and may lead toserious complications. Deformity in the region of elbow as one of the latecomplications of these fractures causes cosmetic and functional problems andadditional secondary complications. The most frequent deformity after these fracturesis cubitus varus. Although its name resembles a single coronal plan deformity butinternal rotation and hyperextension are its components in axial and sagittal planes.In literature there are frequent emphases on the importance of axial planecomponent of the cubitus varus deformity. But the number of the studies dealing withits measurement and evaluation is few. The most of these studies use computertomography an ionizing irradiation technique for evaluation of axial plane deformities.The single physical examination technique has been described by Yamamoto,although it has not gain wide application. Using current digital technologies,development of a reproducible technique to measure the amount of malrotations ofthe distal fragment after paediatric humerus supracondylar fractures with no need ofionizing irradiation was the aim of this study.Materials and MethodsThis study was done in 28 boys and 17 girls who were operated for theirhumerus supracondylar fractures between January 2009 and December 2015. Allpatients had displaced Gartland type 2 or 3 fractures. Their mean ages duringoperation were 5.6±2.85 years and at their last control were 10.4±3.3 years. Themean follow-up period was 59.5±21.7 months. In the last control patients wereevaluated for their joints' range of motions, muscle strengths and neurovascularstatus. Cosmetic and functional results were evaluated due to Flynn criteria. Toobtain standardized clinical photographs a positioning stand was developed. With theaid of that stand digital photographs was obtained. These were photographic carryingangle (p-CA) while elbow in full extension and forearm in supination; arm-forearmangle (pAFA) while elbow in flexion and forearm in supination; transepicondylarhorizonangle while shoulder in full passive external rotation, elbow in 90° flexion andforearm in supination (p90°-RDA) and transepicondylar-horizon angle while shoulderin full passive external rotation, elbow in full flexion and forearm in supination (pFFRDA).A digital photograph was also obtained while patient was in Yamamoto's10position to evaluate Yamamoto's rotational deformity angle (pY-RDA). The angleslisted above along with radiographical angles of humeroulnar (rHUA), Baumann(rBA), metaphysodiaphyseal (rMDA), lateral humerocapitellar (rLHCA) angles andtrocleocapitellar index (rTCI) in latest radiographs of the patients were evaluatedindependently by 3 different observers. Results of the measurements were analysedregarding inter-observer correlations. Also correlations among differentmeasurements, results of the measurements with Flynn clinical results, the number ofK wires which were used for fracture fixation and combination of operation age-followup period were analysed.ResultsThere were statistically significant inter-observer correlation (p0.05).Clinical results according to Flynn criteria showed significant correlation(p60months (Group B); age>5.5 and follow up 5.5 and11follow up>60 months (Group D). These groups were compared regarding allmeasurements. Due to these analyses there were significant difference betweenGroups A and D regarding rHUA (p=0.043). There were no differences regardingother measurements among the groups (p>0.05).Discussion and OutcomeAs this study aimed to describe a new evaluation technique, its plan wasformed to reach high internal consistency and external validity. For these aims aspecial designed photography stand and digital angle measurement software wereused. For measurement of axial plane angles, position of the evaluated extremitieswas standardized and measurements in all patients were performed in both fracturedand normal extremities. Comparing with the literature, our study group can representchildren with humerus supracondylar fracture regarding age, sex, involved side,fracture type and undergone treatment method.There was statistically significant inter-observer correlation for allphotographical and radiographical measurements. This results show thatmeasurements of above mentioned parameters by the means described in this studyare reliable and reproducible with satisfying sensitivity to show even smalldifferences.Upon our results we can state that the measurement techniques of rotationaldeformity described in this study are more effective and simple than the singlepreviously described technique of Yamamoto. As there were significant correlationsamong all coronal plane measurements, either photographical or radiographical, weconclude that although the degree of deformity in our patients was sub-clinic but themain plane of residual deformity was coronal. Used techniques could be able toidentify even small amounts of angulation in this plane.As there were no relationships among the axial plane measurements withFlynn clinical results, we can state that Flynn criteria does not represent rotationaldeformities thus this criteria might need development so that it cover all planes'deformities.Analyses of the relationships among measured angles and time relatedparameters showed that as the operation age and follow up period of patients12increases better radiographical humeroulnar angles can be obtained. Betterradiographical results do not necessarily mean better clinically evident results. 151

Amaç:Radius alt uç kırıkları (RAUK) sık karşılaşılan kırıklar olup travma enerji düzeyleri ve yaş aralıkları birbirinden farklı hasta gruplarında görülmektedir. Erişkin RAUK genç-erkek grubunda sıklıkla yüksek, ileri yaş-kadın grubunda düşük enerjili travmalar sonucu görülür. Tedavi yönteminin seçiminde yaralanmaların enerji düzeyi, sınıflamalar ve LaFontaine'in tanımladığı instabilite kriterleri önemli yol göstericilerdir. Literatürde bahsi geçen yol göstericiler arası ve instabilite kriterleri ile tedavi sonuçları arasındaki ilişkilere dair yeterli bilgi bulunmamaktadır. İki aşamadan oluşan bu çalışmada bahsi geçen etkenler arasındaki ilişkilerin araştırılması amaçlanmıştır. Çalışmanın birinci aşamasında rutinde sık kullanılan Frykman, Fernandez ve AO sınıflamalarının maruz kalınan travma enerji düzeyini ve LaFontaine tarafından tanımlanan instabilite kriterlerini öngörme güçleri araştırıldı. Çalışmanın ikinci aşamasında ise farklı yöntemlerle tedavi edilen hastalarda instabilite kriterleri ve tedavi şekillerinin fonksiyonel ve radyolojik sonuçlar üzerindeki etkileri araştırıldı.Yöntem:Çalışmanın birinci aşamasında 01.01.2007 – 30.07.2015 tarihleri arasında kliniğimizde RAUK tanısı ile tedavisi yapılan 18 yaş üstü 164 olgu çalışmaya dahil edildi. Hastalar yaralanma mekanizmasına göre düşük ve yüksek enerjili yaralanmalar olmak üzere iki kategoride sınıflandırıldı. Basit düşme düşük enerjili yaralanma olarak kabul edilirken; trafik kazası, bisikletten ve yüksekten düşme ile ezici yaralanmalar yüksek enerjili yaralanma olarak kabul edildi. Ardından hastaların kırık ve tedavi sonrası grafileri değerlendirildi. Kırıklar, Frykman, Fernandez, AO ve Lafontaine'in instabilite kritelerine göre sınıflandırıldı. LaFontaine'nin beş instabilite kriterinden en az üçünü taşıyan kırıklar anstabil olarak kabul edildi. Barındırdıkları instabilite kriterleri temelinde Frykman sınıfları 3'e ve Fernandez sınıfları 2'ye indirgendi. Mevcut ve yeniden gruplandırılan sınıflamaların her biri stabilite durumu ve travma enerji düzeyini öngörme açısından istatistiksel analize tabi tutuldu.Çalışmanın ikinci aşamasına yapılan çağrı doğrultusunda son kontrollerine gelen hastalar dahil edildi. Toplam 52 hasta (54 kırık olgusu) son kontrollerine geldi. Bu hastaların 33'ü erkek, 19'u kadın ve yaş ortalaması 49,5 ± 16,3 idi. Ortalama takip süreleri 22,6 ±12,6 ay olan bu olguların 16'sı alçılama, 14'ü pinleme ve 24'ü plaklama ile tedavi edilmişti. Bu hastalarda fonksiyonel sonuç açısından Quick Disabilities of the Arm, Shoulder and Hand Questionnaire (QDASH) ve Mayo El Bileği (MEB) skorları, kavrama ve çimdikleme kuvvetleri ve eklem hareket açıklıkları değerlendirildi. Olguların çekilen son grafilerinde distal radyoulnar eklem (DRUE) instabilitesi veya artrozu ile radyokarpal eklem (RKE) artrozu açısından değerlendirme yapıldı. 41. First 10 days after the 6th of February 2023 earthquake disaster: experience of an orthopedic clinic on the border of the disaster zone.

Background: The earthquake disaster that occurred on February 06, 2023, caused serious destruction and loss of life in the south of Türkiye. The purpose of this article consisting of two interconnected parts is to report the experience of our orthopedics clinic, which is located just on the border of the disaster area. The subject of the first part of the study is the characteristics of ortho-pedic traumas in earthquake victims and the treatment methods applied. The subject of the second part is the disaster work plan of the clinic and its consequences in practice., Methods: For the first step, descriptive information, diagnoses, and treatment methods of 204 earthquake victims who were treated by our clinic in the first 10 days after the disaster were compiled from the archives. In terms of the second phase of the study, an in-clinic work plan was created on the morning of the 1st day of the disaster. The teams and working hours in the emergency department, inpatient service, and operating room were determined. Hospital management and other clinics were contacted and hospital facilities were organized for orthopedic trauma victims., Results: The mean age of the patients was 42.3 years. Among the age groups, the smallest group was children under 10 years old (6.4% of all patients). 132 of the patients had at least one fracture in 135 extremities. Most of the injuries involved the lower extrem-ity. 66 patients had crush injury with or without fracture. 181 orthopedic surgical procedures were performed in 144 patients. The most common operations were internal fracture fixation and debridement. The most used implant was the plate-screw combination. Thanks to the in-clinic work plan and the in-hospital assistance and allocation of facilities, we did not experience serious problems in the hospitalization, surgery, and post-operative follow-ups of the patients. Although we received numerous offers of assistance from external institutions, this was not possible due to bureaucratic obstacles. Among our most important problems were the shortage of sterilized powered surgical drills and the lack of a dynamic patient information database., Conclusion: Orthopedic clinics should prepare their in-clinic work plans for earthquake disasters and develop their facilities. In order not to encounter bureaucratic obstacles in emergency assistance, orthopedic clinics in different risk regions should be formally matched beforehand. A patient database table that clinical staff can access and revise using their smartphones facilitates the follow-up of large numbers of simultaneously hospitalized patients.

42. Outcomes of bipedicle V-Y 'cup' flap for repair of volar oblique fingertip amputations.

- Abstract
We used bipedicle V-Y 'cup' flaps to reconstruct 13 consecutive volar oblique fingertip amputations encountered in a nearly 2-year period. All flaps survived completely with no bone protuberance, scarring or neuroma evident at the end of follow-up (mean, 7 months; range 4-13 months). Two of the 13 patients had moderate or severe tenderness, three had moderate or severe cold intolerance and two had noticeable hook nails. There were no significant (≥10°) flexion contracture in the interphalangeal joints. The mean static two-point discrimination score was 4 mm (range 3-6) and the modal Semmes-Weinstein monofilament test score was 3.6 g (range 3.2-4.2). All patients were satisfied with their surgical outcomes. The bipedicle V-Y 'cup' flap provides good results in terms of fingertip contour, padding and sensation, and has a low complication rate when used to treat volar oblique fingertip amputations. - Abstract
Objective: The aim of this study was to evaluate the results of the innervated digital artery perforator (IDAP) flap and to analyze the relationship between patient satisfaction and outcome measures., Methods: A total of 17 fingertips of 15 patients (14 men and one woman; mean age: 47.2 (26-62) years) were included in this retrospective study. Patients' injured finger and defect type were recorded. At the last follow-up, the static two-point discrimination (s2PD) test, Semmes Weinstein monofilament (SWM) test, and range of motion of the affected finger were analyzed. We interviewed patients to determine hand dominance, cold intolerance, and their satisfaction with the result. We performed correlation and logistic regression analyses between patient satisfaction and outcome measures., Results: The mean follow-up period was 13.8 (7-18) months. The mean range of motion was 77.3±3.5 (70-80) degrees for the distal interphalangeal joints of affected fingers. The mean s2PD was 6.4 (3-10) mm, and the SWM records ranged from 2.83 to 4.93 monofilament markings. Cold intolerance was noted in seven fingers (41%). Patient satisfaction was negatively correlated with cold intolerance, and cold intolerance decreased as the follow-up period extended., Conclusion: IDAP flap satisfies both patient and surgeon, with the only significant problem being cold intolerance, regarding which patients must be informed. Although cold intolerance is hard to treat, fortunately, it generally improves with time., Level of Evidence: Level IV, Therapeutic study.

- Abstract
Vascular tumors in neonates are mostly benign; however, locally aggressive voluminous forms may destabilize the hemodynamics of a neonate. Herein, we present an unusual case of a neonatal giant vascular tumor in the right upper extremity, causing a consumption coagulopathy and acute deterioration of vital signs. The patient required mechanical ventilation, inotropic support, and administration of blood products by the seventh day. Vascular embolization attempts failed to improve the general condition of the patient. Due to the deteriorating and life-threatening general condition of the patient, amputation around the upper arm level occurred under emergency conditions on the twelfth day. The patient's hemodynamic parameters were regained immediately, with neither inotropic agents nor blood products required after the second postoperative day. Clinical and pathological diagnosis revealed kaposiform hemangioendothelioma. Patient monitoring proceeded until the age of 15 months, with no local recurrence around the stump or soft tissue coverage complications. Therefore, since other treatment options failed, the early amputation decision was life-saving.

45. Longitudinal extensibility of the dorsal skin of the hand.

- Abstract
We aimed to assess the longitudinal extensibility of the dorsal skin of the hand. Measurements were done in five hand positions on the dominant hands of 64 volunteers. The positions were full flexion of the wrist and fingers, drop hand, flat hand, hand with opposed thumb and full extension. Dorsal marks were made on the skin in full flexion. The marks were transferred to transparent parchment paper strips with the hand in the different positions. Skin extensibility was assessed by the change in the distance between two skin marks from the full extension position to the other positions. Opposition required the highest degree of skin extensibility in the dorsal radial region, followed orderly by ulnar and central regions. Generally, the extensibility increased from distal to proximal and from ulnar to radial. These findings may be useful in the management of dorsal hand soft tissue defects.

- Abstract
Background: Children have a curiosity for climbing walls, stairs, and railings and have an increased risk of falling. Here, we report our experience with railings causing injuries by penetrating the upper extremities, and aim to call attention to spiked railing injuries in children., Methods: We report on five children with a mean age of 8.8. All of the children were male. The penetrating railing parts were removed in a surgical room., Results: The injured structures repaired immediately were as follows: flexor digitorum profundus tendon, A4 pulley, volar plate of the distal interphalangeal joint, radial digital nerve, ulnar digital artery, and radial digital artery., Conclusion: Spiked railings can lead to significant injury that requires further exploration. Boys particularly are at risk, and parents should be alert regarding these type of injuries. We recommend a standard regulation for fence erection, and we wish to warn owners of this type of fence regarding probable legal sanctions.

- Abstract
The informative value of axial radiographs of the patellofemoral joint is highly dependent on application techniques and knee positioning. We developed a simple device that enables an appropriate and easy positioning. With the use of this device, patellofemoral axial radiographs can be obtained at 30 degrees of knee flexion.
