1. Role of the constriction angle on the clogging by bridging of suspensions of particles

Abstract

Confined flows of particles can lead to clogging, and therefore failure, of various fluidic systems across many applications. As a result, design guidelines need to be developed to ensure that clogging is prevented or at least delayed. In this Letter, we investigate the influence of the angle of reduction in the cross section of the channel on the bridging of semidilute and dense non-Brownian suspensions of spherical particles. We observe a decrease of the clogging probability with the reduction of the constriction angle. This effect is more pronounced for dense suspensions close to the maximum packing fraction where particles are in contact in contrast to semidilute suspensions. We rationalize this difference in terms of arch selection. We describe the role of the constriction angle and the flow profile, providing insights into the distinct behavior of semidilute and dense suspensions. Abstract

Pairwise Markov Models (PMMs) extend the wellknown Hidden Markov Models (HMMs). Being significantly more general, PMMs enable several types of processing, like Bayesian filtering or smoothing, similar to those used in HMMs. In this paper, we deal with Bayesian forecasting. The aim is to show analytically in the simple stationary Gaussian case that the extent results obtained with HMM can be improved. We complete contributions with a theoretical error study and two real examples we deal with. Experiments show that PMMs-based forecasting can significantly improve HMMs-based ones.

Abstract

The Argo array of profiling floats has considerably increased the observing capability of the three-dimensional global ocean and the knowledge of the ocean response to climate change. In particular, the Argo sampling has allowed observing relevant ocean indicators over the whole Mediterranean Sea especially during the last decade. In this study, the Mediterranean Argo network is comprehensively described from its spatio-temporal coverage to its capability to observe ocean monitoring indicators at sub-regional scale. For this purpose, the Argo array, as a non-interpolated product of profiles, is used to estimate the ocean heat and salt contents integrated within the upper, intermediate and deep layers over the period 2013-2022 in the different sub-regions of the basin. The same computational method is also applied to a model reanalysis product to estimate the impact of sampling of the sole Argo array. The sampling error is defined at sub-regional scale by comparing estimations from the whole model grid (full-sampled model) and from the Argo-like sampled model grid (sub-sampled model). Warming and salinification trends are well captured by the Argo array over the period of study, warming trends being the highest in the sub-regions of the western Mediterranean Sea from surface to depth and salinification trends being higher in the eastern sub-basin for the upper layer and in the western sub-basin for the deeper layers. This study also demonstrates the capability of the Argo array to capture local ocean structures and dynamics (e.g. anticyclonic and cyclonic gyres, intermediate and deep convection events and Atlantic Water inflows) and to account for their impact in the sub-regional variability of ocean heat and salt contents in the upper, intermediate and deep layers from seasonal to interannual scales. Considering these structures is fundamental for the understanding of the thermohaline circulation and changes observed in the Mediterranean Sea, and thus for future climate studies.

Abstract

Clogging can occur whenever a suspension of particles flows through a confined system. The formation of clogs is often correlated to a reduction in the cross-section of the channel. In this study, we consider the clogging by bridging, i.e., through the formation of a stable arch of particles at a constriction that hinders the transport of particles downstream of the clog. To characterize the role of the volume fraction of the suspension on the clogging dynamics, we study the flow of particulate suspensions through 3D-printed millifluidic devices. We systematically characterize the bridging of non-Brownian particles in a quasi-bidimensional system in which we directly visualize and track the particles as they flow and form arches at a constriction. We report the conditions for clogging by bridging when varying the constriction width to particle diameter ratio for different concentrations of the particles in suspension. We then discuss our results using a stochastic model to rationalize the influence of solid fraction on the probability of clogging. Understanding the mechanisms and conditions of clog formation is an important step for optimizing engineering design and developing more reliable dispensing systems.

Abstract Introduction The aim of this work is to evaluate the real-world outcomes of the reinforced treat-and-extend (RTE) protocol for the treatment of exudative age-related macular degeneration with intravitreal injections of aflibercept or ranibizumab (anti-vascular endothelial growth factor therapies). Methods This was a retrospective review of patients from two tertiary ophthalmology centers in France initiating the RTE protocol between February 2018 and June 2021. The primary outcome was change in best-corrected visual acuity (BCVA) after 24 months. Secondary outcomes were change in central retinal thickness (CRT), recurrence, and management-related factors (injection interval, number of injections/consultations). Outcomes were additionally evaluated after protocol changes (strict versus modified RTE protocol groups). Results Sixty-eight patients (72 eyes) were included (68% females; mean age 82.2 ± 7.8 years). La Garonne après sa confluence avec le Lot a connu une crue majeure en février 2021, sa plus haute crue depuis décembre 1981. À cette occasion, de nombreuses données d’observations ont été acquises par le réseau Vigicrues : jaugeages sur des niveaux de crue parfois plus que décennaux, relevés de laisses de crues et photographies aériennes. Ces données ont permis d’actualiser la connaissance des débits, notamment la courbe de tarage de Tonneins, de produire et de valider les cartographies des zones inondées potentielles sur la plaine marmandaise, ainsi que de réévaluer les niveaux de vigilance crues de façon harmonisée entre la Garonne marmandaise et la Garonne girondine. Les travaux se poursuivront par la mise en place d’un modèle hydraulique 2D en mesure de représenter la nonlinéarité qui apparaît au-delà des 8,50 m à Tonneins et dont la prévisibilité conditionne la bonne prévision sur Marmande et La Réole lors des crues majeures.

Background Immunotherapy can be associated with prolonged disease control even after cessation of treatment without the need for further cancer-directed therapy. Treatment-related adverse events (TRAEs) can also persist after discontinuation of therapy. Treatment-free survival (TFS) with and without toxicity as a component of a partitioned survival model can characterize patient survival time, which is not captured by standard outcome measures.Methods Data from 1096 patients with advanced renal cell carcinoma treated with first-line nivolumab plus ipilimumab (NIVO+IPI) versus sunitinib (SUN) in the CheckMate 214 trial were analyzed. TFS was defined as the area between two Kaplan-Meier curves for time from randomization to protocol therapy discontinuation and time from randomization to subsequent systemic therapy initiation or death, estimated as the difference in 60-month restricted mean times with confidence intervals (CIs) obtained using bootstrap sampling. Time on protocol therapy and TFS were further characterized as time with and without grade 2+ and 3+TRAEs. Survival functions were estimated in subgroups including International Metastatic Renal Cell Carcinoma Database Consortium risk groups using the Kaplan-Meier method.Results At 5 years from randomization, 48% of patients treated with NIVO+IPI and 37% of patients treated with SUN were alive. In the intent-to-treat population, 18% of the NIVO+IPI-treated and 5% of SUN-treated patients are surviving treatment-free. For favorable-risk patients, the 60-month mean TFS was 14.4 months for NIVO+IPI versus 5.5 months for SUN (difference 8.9 months (95% CI 4.9 to 12.8)). TFS for NIVO+IPI versus SUN with grade 2+TRAEs was 5.0 and 2.1 months, respectively, and with grade 3+TRAEs was 1.2 and 0.3 months, respectively. For intermediate/poor-risk patients, the 60-month mean TFS was 10.1 months for NIVO+IPI versus 4.1 months for SUN (difference 6.1 months (95% CI 4.2 to 7.9)). TFS for NIVO+IPI versus SUN with grade 2+TRAEs was 4.0 versus 2.0 months, respectively, and 0.6 versus 0.3 months with grade 3+TRAEs.Conclusions Although overall survival was similar, favorable-risk patients treated with NIVO+IPI spent more time surviving treatment-free with and without toxicity versus SUN after 60 months of follow-up. Intermediate/poor-risk patients treated with NIVO+IPI had longer survival and longer TFS without toxicity versus SUN.Trial registration number NCT02231749.

Abstract

Amyotrophic lateral sclerosis (ALS) is the most common motor neuron disease worldwide and is characterized by progressive muscle atrophy. There are currently two approved treatments, but they only relieve symptoms briefly and do not cure the disease. The main hindrance to research is the complex cause of ALS, with its pathogenesis not yet fully elucidated. Retinoids (vitamin A derivatives) appear to be essential in neuronal cells and have been implicated in ALS pathogenesis. This study explores 4-[2-(5,5,8,8-tetramethyl-5,6,7,8-tetrahydroquinoxalin-2-yl)ethylnyl]benzoic acid (Ellorarxine, or DC645 or NVG0645), a leading synthetic retinoic acid, discussing its pharmacological mechanisms, neuroprotective properties, and relevance to ALS. The potential therapeutic effect of Ellorarxine was analyzed in vitro using the WT and SOD1G93A NSC-34 cell model of ALS at an administered concentration of 0.3–30 nM. Histological, functional, and biochemical analyses were performed. Elorarxine significantly increased MAP2 expression and neurite length, increased AMPA receptor GluA2 expression and raised intracellular Ca2+ baseline, increased level of excitability, and reduced Ca2+ spike during depolarization in neurites. Ellorarxine also displayed both antioxidant and anti-inflammatory effects. Overall, these results suggest Ellorarxine shows relevance and promise as a novel therapeutic strategy for treatment of ALS.

Abstract

The State flood warning and anticipation tools in France have evolve a lot since the years 2000. This paper first presents an overview of the progress made since then. A description of the existing flood vigilance system (www.vigicrues.gouv.fr) is given, with a focus on two of the main on-going improvements: the particular case of river sections under sea influence and the specific organization dedicated to some of the French overseas territories. The next step is to underline the most recent developments carried out to extend the services offered to the public: better communication on occurring floods, forecasts and uncertainties; anticipation of flooded areas; enhanced flash flood warnings dedicated to municipalities; experiments and advice to local authorities on more direct on-site warning systems. The targeted objective is to develop tools and methods to deliver more relevant information, as responsive as possible to the different types of flooding which people are exposed to, over the whole French territory.

Abstract

Flooded areas are key information to crisis managers, both at local and national level. They help sizing human and material resources to be deployed before and during a flood. These data already exist on several French rivers as they are been produced in a legislative framework but mainly addressing the hundred-year flood. The purpose of the Flood Forecasting Services is to complete water levels forecasting published on www.vigicrues.gouv.fr with forecasting information on spatial flooded areas. State services are developing new flooded areas ranging from first overflows to the hundred-year floods. A national database called VIGInond provides secure web services to access these new data. It was developed in 2015. A key step in the standardization of these new data is the collaboration between various state services. The access to VIGInond is initially opened to the Ministry of Interior services. This first step allows the operators to test and to understand the impacts of forthcoming flooding. Once tested, it will be made available to other crisis managers through the forthcoming Vigicrues® portal version 2. The new data will be gradually included in Vigicrues® watercourses, while coastal areas and new watercourses will be subsequently added. These new information will increase awareness of flooding risks among stakeholders and citizens in the near future.

Abstract

Immunotherapy profoundly changed the landscape of cancer therapy by providing long-lasting responses in subsets of patients and is now the standard of care in several solid tumor types. However, immunotherapy activity beyond conventional immune checkpoint inhibition is plateauing, and biomarkers are overall lacking to guide treatment selection. Most studies have focused on T cell engagement and response, but there is a growing evidence that B cells may be key players in the establishment of an organized immune response, notably through tertiary lymphoid structures. Mechanisms of B cell response include antibody-dependent cellular cytotoxicity and phagocytosis, promotion of CD4+ and CD8+ T cell activation, maintenance of antitumor immune memory. In several solid tumor types, higher levels of B cells, specific B cell subpopulations, or the presence of tertiary lymphoid structures have been associated with improved outcomes on immune checkpoint inhibitors. The fate of B cell subpopulations may be widely influenced by the cytokine milieu, with versatile roles for B-specific cytokines B cell activating factor and B cell attracting chemokine-1/CXCL13, and a master regulatory role for IL-10. Roles of B cell-specific immune checkpoints such as TIM-1 are emerging and could represent potential therapeutic targets. Overall, the expanding field of B cells in solid tumors of holds promise for the improvement of current immunotherapy strategies and patient selection.

RÉSUMÉLes jeux sérieux sont depuis les années 2000 considérés comme des outils de formation à part entière : ils proposent un modèle pédagogique différent des formations classiques transmissives, avec la possibilité de mettre en œuvre des situations complexes et de proposer aux apprenants de constituer leur propre processus d’apprentissage. Mis au point par le Schapi et les référents régionaux inondation (RRI) dans le cadre de l’animation nationale des missions de référent départemental inondation (RDI), Consiliari est un jeu de plateau qui reprend les codes des jeux de société traditionnels, accordant une part importante au volet ludique et à l’attractivité visuelle. Le jeu met en scène une équipe d’interprètes de l’inondation, chargée de prodiguer au gouverneur les bons conseils pour gérer une situation d’inondation et sauver la population. Depuis son lancement début 2022, plusieurs dizaines de missions RDI ont pu jouer à Consiliari. Les premiers retours montrent que le jeu remplit ses objectifs principaux, à savoir mobiliser les compétences, savoir-faire et savoir-faire nécessaires pour l’exercice de la mission RDI, sans oublier le côté ludique. Des pistes d’amélioration sont toutefois d’ores et déjà identifiées pour améliorer et prolonger l’expérience de jeu.

Abstract

Background Cardiogenic shock (CS) is a significant complication of Takotsubo syndrome (TTS), contributing to heightened mortality and morbidity. Despite this, the Society for Cardiovascular Angiography and Interventions (SCAI) staging system for CS severity lacks validation in patients with TTS and CS. This study aimed to characterize a patient cohort with TTS using the SCAI staging system and assess its utility in cases of TTS complicated by CS. Methods and Results From a TTS national registry, 1591 consecutive patients were initially enrolled and stratified into 5 SCAI stages (A through E). Primary outcome was all‐cause in‐hospital mortality; secondary end points were TTS‐related in‐hospital complications and 1‐year all‐cause mortality. After exclusions, the final cohort comprised 1163 patients, mean age 71.0±11.8 years, and 87% were female. Patients were categorized across SCAI shock stages as follows: A 72.1%, B 12.2%, C 11.2%, D 2.7%, and E 1.8%. Significant variations in baseline demographics, comorbidities, clinical presentations, and in‐hospital courses were observed across SCAI shock stages. After multivariable adjustment, each higher SCAI shock stage showed a significant association with increased in‐hospital mortality (adjusted odds ratio: 1.77–29.31) compared with SCAI shock stage A. Higher SCAI shock stages were also associated with increased 1‐year mortality. Conclusions In a large multicenter patient cohort with TTS, the functional SCAI shock stage classification effectively stratified mortality risk, revealing a continuum of escalating shock severity with higher stages correlating with increased in‐hospital mortality. This study highlights the applicability and prognostic value of the SCAI staging system in TTS‐related CS.

Abstract

Patients, life science industry and regulatory authorities are united in their goal to reduce the disease burden of patients by closing remaining unmet needs. Patients have, however, not always been systematically and consistently involved in the drug development process. Recognizing this gap, regulatory bodies worldwide have initiated patient-focused drug development (PFDD) initiatives to foster a more systematic involvement of patients in the drug development process and to ensure that outcomes measured in clinical trials are truly relevant to patients and represent significant improvements to their quality of life. As a source of real-world evidence (RWE), social media has been consistently shown to capture the first-hand, spontaneous and unfiltered disease and treatment experience of patients and is acknowledged as a valid method for generating patient experience data by the Food and Drug Administration (FDA). While social media listening (SML) methods are increasingly applied to many diseases and use cases, a significant piece of uncertainty remains on how evidence derived from social media can be used in the drug development process and how it can impact regulatory decision making, including legal and ethical aspects. In this policy paper, we review the perspectives of three key stakeholder groups on the role of SML in drug development, namely patients, life science companies and regulators. We also carry out a systematic review of current practices and use cases for SML and, in particular, highlight benefits and drawbacks for the use of SML as a way to identify unmet needs of patients. While we find that the stakeholders are strongly aligned regarding the potential of social media for PFDD, we identify key areas in which regulatory guidance is needed to reduce uncertainty regarding the impact of SML as a source of patient experience data that has impact on regulatory decision making.

Abstract

A 46-year-old woman presented for a routine echocardiogram, which revealed a giant atrial septal aneurysm (ASA) with an atrial septal defect (ASD). We surgically excised the ASA and closed the ASD with a pericardial patch. ASA, although rare, poses embolic risks, thus necessitating closure when associated with shunts. Optimal stroke prevention remains uncertain.

Abstract

Abstract Background Cabozantinib, a standard of care metastatic renal cell carcinoma (mRCC), may be associated with weight and muscle loss. These effects of new generation VEGFR tyrosine kinase inhibitor on muscle mass loss are poorly described. Methods All cabozantinib‐treated mRCC patients from January 2014 to February 2019 in our institution were included. Clinical data including weight were collected during therapy. Computed tomography images were centrally reviewed for response assessment, and axial sections at the third lumbar vertebrae were used to measure the total muscle area. Toxicities and cabozantinib outcomes were evaluated. Co‐primary endpoints included skeletal muscle loss and weight loss (WL), longitudinally evaluated during treatment. WL has been classified according to CTCAEv5.0: Grade 1 (loss of 5 to 20% of baseline body weight). Results Patients were mostly men (70.3%), median age was 59.2 (range: 22.0–78.0) years, and median baseline body mass index was 25.0 (range: 16.4–49.3) kg/cm2. Prognosis according to International Metastatic RCC Database Consortium score was good, intermediate, and poor for 13 (13.0%), 63 (63.0%), and 24 (24.0%) patients, respectively. Out of a total of 120 patients, 101 patients with a median follow‐up of 22.3 months (range: 4.5–62.2) were eligible for analysis; 85 experienced muscle loss and muscle loss >10% increased during cabozantinib exposition, especially after 6 months of treatment. At cabozantinib baseline, 71 patients (70.3%) had sarcopenia, and 16/30 (53.3%) non‐sarcopenic patients developed sarcopenia during treatment. Baseline sarcopenia was associated with lower response rates (P = 0.031) and higher grades 3–4 toxicities (P = 0.001). Out of 92 patients included in the WL analysis, 44 (47.8%) and 12 (13.0%) experienced grades 2 and 3 WL, respectively. Conclusions We report a high incidence of grades 3–4 WL, fourth times higher than reported in prior pivotal trials, and half of the patients developed sarcopenia while on cabozantinib treatment. Weight and muscle mass loss with cabozantinib are underreported and may require further investigations and early management.
