Abstract

Abstract Chiari disease (or malformation) is in general a congenital condition characterized by an anatomic defect of the base of the skull, in which the cerebellum and brain stem herniate through the foramen magnum into the cervical spinal canal. The onset of Chiari syndrome symptoms usually occurs in the second or third decade (age 25 to 45 years). Symptoms may vary between periods of exacerbation and remission. The diagnosis of Chiari type I malformation in patients with or without symptoms is established with neuroimaging techniques. The most effective therapy for patients with Chiari type I malformation/syringomyelia is surgical decompression of the foramen magnum, however there are non-surgical therapy to relieve neurophatic pain: either pharmacological and non-pharmacological. Pharmacological therapy use drugs that act on different components of pain. Non-pharmacological therapies are primarly based on spinal or peripheral electrical stimulation. It is important to determine the needs of the patients in terms of health-care, social, educational, occupational, and relationship issues, in addition to those derived from information aspects, particularly at onset of symptoms. Currently, there is no consensus among the specialists regarding the etiology of the disease or how to approach, monitor, follow-up, and treat the condition. It is necessary that the physicians involved in the care of people with this condition comprehensively approach the management and follow-up of the patients, and that they organize interdisciplinary teams including all the professionals that can help to increase the quality of life of patients.

[Resumen] En este trabajo se pretende describir las alteraciones posturales más frecuentes en la parálisis cerebral infantil (PCI), traumatismo craneoencefálico (TCE), y en la hemiplejia; y analizar cómo influyen éstas en las alteraciones orofaciales, presentes en la mayoría de estos pacientes. Se presenta una valoración de fisioterapia analítica de la disfunción orofacial, y se muestra un posible tratamiento de fisioterapia,adaptable a las diferentes alteraciones presentes en estos pacientes. [Abstract] The aim of this article is to describe the postural changes in the children cerebral palsy, brain traumatic injury and stroke. Another one is also to analyze the relationchips between the postural changes and the oral and facial disturbances, which are very common in those kinds of patients. Once these changes were analyzed, we show a physiotherapist assesment, evaluating the oral and facial disturbances in an analytic way, and showing at the same time, a possible physiotherapist treatment for these disturbances in those patients.

6. La biotecnología y los medios de comunicación en España

Abstract

In general terms, Spaniards show more optimistic and favourable attitudes towards biotechnology than the average of European citizens, although their attitude is of greater distrust and disagreement, when dealing with specific applications. In any case, the level of information and implication of Spaniards in the public debate on scientific and technological subjects seems to be traditionally low. Nevertheless, analysing the presence of biotechnology in the electronic archives of two national newspapers (El Mundo and El País) and of the state news agency EFE, in the last eight years, we have verified that many of the most relevant biotechnological applications have not gone unnoticed in the media agenda. The presence of biotechnology in the media has tripled in eight years and biotechnology is now a daily interest subject. This allows us to foresee an undergoing deep maturation of Spaniards' biotechnological culture and that this process will probably lead to a greater polarization of their opinions and attitudes on biotechnological controversial issues., En general, los españoles muestran actitudes más optimistas y favorables a la biotecnología que la media de los ciudadanos europeos, aunque cuando se trata de aplicaciones específicas, sus actitudes son de mayor desconfianza y desconcierto. En todo caso, el nivel de información y de implicación de los españoles en el debate público sobre temas científicos y tecnológicos ha sido tradicionalmente escaso. Sin embargo, analizando la presencia de la biotecnología en los archivos electrónicos de dos periódicos nacionales (El Mundo y El País) y de la agencia estatal de noticias EFE en los últimos ocho años, comprobamos que muchas de Ias aplicaciones biotecnológicas más relevantes no han pasado inadvertidas en la agenda mediática. La presencia de la biotecnología en los medios se ha triplicado en ocho años y actualmente es un área de interés informativo diario. Esto nos permite suponer que se está produciendo un proceso de maduración profunda de la cultura biotecnológica de los españoles, lo que probablemente conducirá a una mayor polarización de sus opiniones y actitudes sobre temas biotecnológicos sometidos a debate.

8. Evidencia del método Pilates en el dolor lumbar crónico

Abstract

[Resumen] Introducción: el dolor lumbar inespecífico representa un importante problema de salud a nivel mundial. Las intervenciones basadas en ejercicios terapéuticos son los tratamientos más utilizados para los pacientes con esta afección. En la última década en nuestro país, el método Pilates, ha sido uno de los ejercicios más populares utilizados en la práctica clínica. Objetivo: el objetivo de esta revisión es conocer la efectividad del método Pilates como tratamiento para la disminución del dolor y la discapacidad en pacientes con dolor lumbar crónico inespecífico. Metodología: se realizó una búsqueda en las bases de datos Pubmed, Scopus y PEDro, con las palabras clave low back pain, back pain, Pilates y Pilates method, obteniendo 7 artículos. Resultados: se identificaron 71 artículos, que tras haber realizado una revisión en profundidad, se encontraron 7 que cumplían todos los criterios de selección. Los artículos muestran que los pacientes que realizaron ejercicio terapéutico mediante el método Pilates obtuvieron una disminución del dolor y la discapacidad funcional a corto plazo; del mismo modo que los participantes que habían recibido otro tipo de ejercicios terapéuticos dirigidos por fisioterapeutas. Discusión y conclusiones: Existe correlación entre el método Pilates y la disminución del dolor y el aumento de la funcionalidad, sin que quede completamente demostrada una relación causal. Pilates obtuvo mejores resultados a corto plazo en la reducción del dolor y la discapacidad que una mínima o nula intervención. El método Pilates obtuvo mejores resultados en la disminución de la discapacidad en relación a otros ejercicios terapéutico a medio-largo plazo; pero no obtuvo mejores resultados en la reducción del dolor a medio-largo plazo en comparación con otros ejercicios terapéuticos.
