ÖzetAmaç : Covid-19 pandemisi sürecinde psoriasisli hastaların tedavilerine düzenli devam edip etmediklerini ve kullandıkları tedavilerde aksamalar olup olmadığını araştırdık.Yöntem : Deri ve Zührevi Hastalıklar “Kronik Hastalıklar Polikliniği”'nde izlenen psoriasis ve psoriatik artiritli 57 hastanın tedavi ve takiplerinde pandemik sürecin etkilerini 13 sorudan oluşan anket formu ile değerlendirdik. Bulgular : Hastaların yaş ortalaması 46,3 ± 13,6 yıl idi. Hastaların pandeminin başlangıcına yakın dönemlerden itibaren polikliniğe enson gelmelerinden başlangıç alan ortanca tedavi süreleri 1,0 (0,0 – 12,0) ay idi. Tedavisiz geçen sürenin uzunluğu erkek ve kadınlar arasında istatistiksel olarak benzerdi. Pandemi döneminde hastaların % 42,1’i (n=24) tedaviye devam edebildiğini, % 57,9 u (n=33) ise edemediğini belirtti. Pandemi öncesinde kaydedilmiş olan PASI değerleri ortancası 0,0 (0,0 – 22,0) idi. Introduction: The COVID-19 pandemic dramatically disrupts health care for patients with chronic diseases including chronic spontaneous urticaria (CSU). As of now, it is unknown if the effects of the pandemic in CSU are different than in other chronic diseases. We also do not know, if different groups of CSU patients, for example female and male patients, are affected differently. Aim: To understand how CSU patients and subgroups are affected by the COVID-19 pandemic in their disease activity and control and treatment, using psoriasis as control. Patients and Methods: We analyzed 399 patients (450 visits) with CSU or psoriasis assessed during August 2019, i.e. before the pandemic, or August 2020, i.e. during the pandemic, for changes in disease activity, disease control, and the treatment they used, and how these changes are linked to age, gender, and disease duration. Results: Male but not female patients with CSU had markedly increased disease activity during the pandemic. CSU patients' age or disease duration were not linked to changes. Male and female patients with psoriasis showed similar increases in disease activity and decreases in disease control. The rate of omalizumab treatment, during the pandemic, was unchanged in male patients and increased in female patients with CSU. The efficacy of omalizumab treatment, during the pandemic, was reduced in male patients but not female patients with CSU. Conclusion: Male but not female CSU patients, during the COVID-19 pandemic, show loss of disease control linked to loss of omalizumab efficacy. The reasons for this need to be investigated.

Amaç: Deri prick testi (DPT) başlıca atopik dermatit (AD), kronik ürtiker (KÜ), alerjik astma (AA), ve alerjik rinit (AR) gibi hastalıkların tanı ve takibinde kullanılmaktadır. Bu çalışmanın amacı, Dermatoloji Kliniği Alerji Ünitesi (DKAÜ)'nde yapılan DPT sonuçlarının geriye dönük olarak incelenerek, endikasyonları ve pozitiflik oranlarının araştırılmasıdır.Yöntem: DKAÜ arşivi kullanılarak 2014-2016 yıllarında yapılan DPT sonuçları incelenmiştir. DPT sonuçları, her hasta için ayrıca arşivlenen DPT formları okunarak yapılmıştır.Bulgular: DKAÜ'nde 2014-2016 yılları arasında 1916 hastaya DPT yapılmıştır. Bu hastaların 941'inde AA, 133'ünde AR, 842'sinde dermatolojik hastalık olduğu görülmüştür. En az bir ve birden fazla alerjen madde ile DPT pozitiflik oranı, sırasıyla AA, AR ve dermatolojik hastalıklarda; %92.1, %71.4 ve %50 olarak saptanmıştır. Dermatolojik hastalıklar incelendiğinde, 69 kronik ürtiker (KÜ), 55 atopik dermatit (AD) hastası haricinde, geriye kalan 718 hastada, başlıca dermatit, idiopatik generalize pruritus (İGP) olmak üzere farklı dermatolojik hastalıkların olduğu görülmüştür. DPT pozitifliği KÜ’de %55.1, AD’te %52.7 ve diğer dermatolojik hastalıklarda %48.6 olarak saptanmıştır.Sonuç: Bu çalışmanın sonuçlarına göre DPT pozitiflik oranı, AR ve AA hastalarında hem KÜ ve AD hem de diğer dermatolojik hastalıklara göre daha yüksektir. Bunun nedeni, KÜ ve AD etyopatogenezinde tip 1 aşırı duyarlılık reaksiyonlarının rolünün daha az olması ve/veya farklı dermatolojik tanılarla yönlendirilen hastalarda doğru olmayan DPT endikasyonları olabilir.

Mikozis fungoides (MF) en sık görülen kutane T hücreli lenfoma (KTHL) formudur. Hastalık yavaş seyirliden agresif hızlı seyirli formlara kadar değişken klinik seyri vardır. MF tedavisinde birçok alternatif mevcuttur ve deriyi hedefleyen tedavilerden kemoterapiye kadar değişen farklı yöntemler tedavide kullanılır. Retinoidler hücre proliferasyonu, diferansiyasyonu ve apoptozisi düzenleyen ve immün yanıtı etkileyen A vitamini türevleridir. Retinoidler MF tedavisinde uzun yıllardır kullanılmaktadır ancak etki mekanizması belirsizliğini korumaktadır. Retinoidler uygulama kolaylığı ve nispeten düşük toksisite profili nedeniyle MF’in tüm evrelerinde tedavi seçeneklerinin önemli bir bileşenidir. Beksaroten KTHL tedavisinde Gıda ve İlaç İdaresi (FDA) tarafından onaylanan ilk retinoiddir. MF tedavisinde retinoidlerin diğer ajanlarla kombinasyonu iyi tolere edilir ve nükseden, dirençli hastalarda daha yüksek tedavi cevabı ile ilişkilidir. Bu derlemede, MF hastalarında retinoidlerin kullanımı ile ilgili litetarür bilgileri gözden geçirlmiştir.

Trichofolliculoma is arare tumour, presenting as a facial papule with central hairs on the scalp,face, neck and vulva of adults. Recently, dermoscopic examination has provenvaluable in a great number of dermatological diseases. Easy-to-guess dermoscopicpatterns differ according to the variability and duration of the lesion incases of trichofolliculoma. Recently, two dermoscopic demonstrations oftrichofolliculoma were published. One was defined as a “firework pattern”,histopathologically resembling nests of cells radiating from a follicularepithelium. The other was depicted as a “bluish nodule with a centralwhite-pink area, shiny white structures and dotted vessels”. This paperpresents a new dermoscopic pattern, termed “rosary bead with tassel”. Thenodule resembled a “rosary bead” and the tufted hairs, the “tassel”. In two cases presented herein, the lesionswere in late stages with one and three-year histories. Literatür bilgilerine dayalı olarak Türkiye’de psoriasise özel HKS olmadığı bilinmektedir. Burada psoriasise özel olarak ilk kez kullanılan HKS’nin sunulması amaçlanmıştır. Yöntemler: Psoriasis Takip Sistemi (PSORTAKSİS), Kayseri Sağlık Uygulama ve Araştırma Merkezi (KSUAM) Dermatoloji Kliniği tarafından hazırlanmış ve KSUAM Bilgi İşlem Merkezi tarafından hastane otomasyon sistemine entegre edilmiştir. PSORTAKSİS, 01 Ekim 2016 tarihinden itibaren KSUAM Dermatoloji Kliniği Kronik Deri Hastalıkları Polikliniği’nde, hastalık şiddeti ya da tedavi farklılıkları gözetilmeksizin tüm psoriasis hastalarının takibinde kullanılmaktadır. Bulgular: PSORTAKSİS hastaların demografik verileri, klinik bulguları, laboratuvar çıktıları ve tedavi bilgilerinin kaydedilmesini sağlayan ana ve alt sekmelerinden oluşmaktadır. Sonuç: HKS, tedavi seçenekleri oldukça çok olan ve ayrıntılı takip gerektiren psoriasis için önemlidir. İlaçların güvenli ve etkin kullanılmasının yanı sıra HKS epidemiyolojik ve klinik çalışmalar için bilgiler sağlar. KSUAM Dermatoloji Kliniği’nde bir yıla yakın zamandır PSORTAKSİS kullanılmaktadır. Türkiye’de ilk uygulama olan PSORTAKSİS’in geliştirilmesi ve ülke çapında yaygınlaştırılması önemli ve gerekli bir katkı sağlayacaktır. Objective: In many countries, information about patients receiving systemic and biologic agent treatment has been included in patient registration systems (PRS) to enable effective follow-up of medications. Based on literature info, it is known there is not PRS a specific to psoriasis in Turkey. In this study, it was aimed to represent the first used PRS, specifically for psoriasis. Methods: Psoriasis Follow-up System (PSORTAKSIS, abbreviation of "Psoriasis Takip Sistemi" in Turkish) was designed by Kayseri Health Training Research Center (KHTRC) Dermatology Clinic and integrated in to the hospital automation system by KHTRC Data Processing Center. PSORTAKSIS is used for follow-up of patients with psoriasis regardless of disease severity and treatment differences at Chronic Skin Disease Polyclinic of KHTRC Dermatology Clinic since 01.10.2016. Results: PSORTAKSIS consists of main and sub tabs which provide recording of patients' demographic data, clinical findings, laboratory output, and treatment informations. Conclusion: PRS is important for psoriasis which has a lot of treatment options and requires detailed follow-up. Besides the safe and effective use of drugs, PRS provides informations for epidemiological and clinical studies. PSORTAKSIS has been used in KHTRC Dermatology Clinic nearly a year. The development and nationwide dissemination of PSORTAKSIS, as a first application in Turkey, will provide a significant and necessary contribution.

Objectives Low hair zinc level has been shown to be a good indicator of mild tomoderate zinc deficiency. Ultraviolet (UV)radiation has mutating effects on cell DNA and induces inflammatory factors incells. The aim of thisstudy was to observe the in vitro effect of UV exposure, on zinc concentrationof hair samples.Material and MethodHair samples of 72 patients were included into the study of whom theclinician suspected of zinc deficiency. Hairsamples were divided into three group and treated with a phototherapy device (DavlinUSA model 3 series PC/SP 311/48) and exposed toeither 16,5 j/L UVA, or 4910 mj UVB or both UVA and UVB together, for 8 times within 24 hour intervals. Zincmeasurements of hair sample pairs (UV exposed or not) were analyzedconcurrently by atomic absorption spectrophotometry (AAS).ResultsHair samples of 72 patients (female 45, male 27) were included into thestudy. Mean age of patients was 6.1±3.6 years (min-max 1.0-15 years). Zinc concentrationof hair was 120.9±82.0 µg/gr before UV exposure and 109.2±71.0 µg/gr after UVexposure (p=0.002). Decrease in hair zinc was not associated with gender andwas significant in group both UVA and UVB exposed (p=0.01).ConclusionHair zinc level was significantly reduced in UV exposed samples inconcurrently analyzed before and after UV exposed hair samples. The mostdecrease was with UVA+UVB exposure which is also present in the sunlight. Saçörnekleri 24 saatlik aralıklarla 8 kere, 16,5 j/L UVA, 4910 mj UVBve UVA+UVB ye maruz bırakıldı. Saç örneği çiftlerinde (UV’ye maruz kalan vekalmayan) çinko ölçümleri eş zamanlı olarak atomik absorpsiyonspektrofotometresi (AAS) ile yapıldı.Bulgular72 hastaya ait saç örneği (45 kadın, 27 erkek) çalışmayadahil edildi. Hastaların yaş ortalaması 6.1±3.6 (min-max 1.0-15yaş) idi. Saç çinko konsantrasyonları UV maruziyetinden önce 120.9±82.0 µg/gr,UV maruziyetinden sonra 109.2±71.0 µg/gr (p=0.002) idi. Acne vulgaris is a chronic inflammatory disease that primarily affects the pilosebase unit. Acne vulgaris remains one of the commonest diseases to afflict humanity, with prevalence up to 90% during adolescence. Relatively little is known about its epidemiology despite the fact that acne is a universal condition in younger population. This disease is more common in developed countries than in less industrialized regions of the world. The etiopathogenesis of acne is multifactorial. Sebum production, follicular epithelial keratinization, bacterial proliferation, and inflammation play significant role in etiopathogenesis. Recent studies shown that toll like receptors and antimicrobial peptides play a role in the pathogenesis of acne vulgaris. The relationship between diet and acne is highly controversial. Strong evidence shows that high glycemic loads diet may exacerbate acne. In addition, the factors contributing to the formation of acne also include genetic predispositions, hormonal abnormalities, psychological, environmental and iatrogenic factors. The disease is mainly observed face, chest, back and shoulders. When acne is untreated, it may cause social isolation, difficulty in finding jobs, depression and suicide attempt as a result of emotional and physical scar formation. In this article, the epidemiology and etiopathogenesis of acne are overviewed., Akne vulgaris başlıca pilosebase üniteyi etkileyen kronik inflamatuvar bir hastalıktır. Akne vulgaris adolesanlarda %90'a varan prevalansıyla, insanlığı etkileyen en sık görülen hastalıklardan biridir. Hastalığın epidemiyolojisi ile ilgili bilinenler nispeten az olmasına karşın; akne genç popülasyonda görülen evrensel bir durumdur. Bu hastalık gelişmiş ülkelerde dünyanın daha az endüstrileşmiş bölgelerinden daha yaygındır. Aknenin etiyopatogenezi multifaktöriyeldir. Etiyopatogenezinde sebum üretimi, foliküler epitelyal keratinizasyon, bakteriyel proliferasyon ve inflamasyon önemli rol oynar. Güncel çalışmalar akne vulgaris etiyopatogenezinde toll benzeri reseptörler ve antimikrobiyal peptidlerin de rol oynadığını göstermektedir. Akne ve diyet arasındaki ilişki ise son derece tartışmalıdır. Yüksek glisemik indeksli gıdaların akne vulgarisi şiddetlendirdiğine yönelik güçlü kanıtlar mevcuttur. Ayrıca genetik yatkınlık, hormonal bozukluklar, psikolojik, çevresel ve iyatrojenik faktörler de akne gelişimine katkıda bulunur. Hastalık sıklıkla yüz, sırt, göğüs ve omuz bölgesini tutar. Tedavi edilmediği zaman emosyonel ve fiziksel bozukluklara neden olarak sosyal izolasyon, iş bulma güçlüğü, depresyon ve intihar girişimine yol açabilmektedir. Bu makalede akne vulgarisin epidemiyolojisi ve etiyopatogenezi gözden geçirilmiştir.

The crucial position of IgE within the pathogenesis of allergic diseases made it a key target for therapy. The inhibition of the allergic inflammatory cascade by anti-immunoglobulin E (IgE) therapy is a new and promising concept in the treatment of these diseases. Currently available anti-IgE agent omalizumab has been started to be used in past 3 years in the cases of chronic spontaneous urticaria (CSU) and chronic inducible urticaria (CINDU), resistant to the first-, second-, and some third-line treatments. The use of omalizumab as an effective and safe biological therapy for inadequately controlled severe, persistent patients with CSU and CINDU provided a valuable new treatment option for these patients. However, the data about possible mechanisms of anti-IgE therapy in these patients, treatment strategies and dose regimens of anti-IgE therapy are different, and special patient groups and possible side effects are still insufficient. Also, studies about possible future anti-IgE treatment options are ongoing in CSU.

AMAÇ: Son yıllarda dermatoloji polikliniğine başvuran gebe sayısı artmıştır. Bu da beraberinde bu hastalara özel bir yaklaşım zorunlulu- ğunu getirmiştir. Gebe bir hastaya dermatolojik olarak yaklaşabilmek için klinisyenlerin gebelikte görülen dermatolojik hastalıkları iyi bilmesi gerekmektedir. Gebeler ve gebelik düşüncesi olanlar ayrıcalıklı olarak gözden geçirilmelidir. Bu çalışmadaki amacımız gebelerde görülen deri hastalıklarını incelemek ve şehrimizdeki dermatologların bu hasta grubunda tercih ettiği tedavileri belirlemekti. YÖNTEM: Kliniğimize başvuran 697 hasta retrospektif olarak de- ğerlendirmeye alındı. Bu hastalara verilen tanılar ve reçetelendirilen ilaçlar analiz edildi. BULGULAR: Gebe hastalarda en sık görülen hastalıklar kaşıntı, ürtiker, akne ve kontakt dermatit olarak bulundu. Bu hastalara ço- ğunlukla topikal tedaviler reçete edilmişti. Sistemik tedavilerden en sık tercih edilenler antihistaminikler, steroidler ve antibakteriyellerdi. Bu sistemik tedaviler sırası ile 195, 148 ve 87’şer defa reçete edilmişti ve gebe hastalarda yazılan sistemik tedavilerin yaklaşık % 96’sını oluşturuyorlardı. SONUÇ: Gebelik çok sayıda dermatolojik hastalığın görülmesi ile kendine özgü bir dönemdir. Aynı zamanda gerek anne gerek de fetüs açısından çok hassas bir dönemdir. Bu nedenlerden dolayı doktorlar bu hasta grubunda genellikle topikal tedavileri tercih etmektedirler. Bu çalışma bölgemizdeki dermatologlarının gebe hastalarda tedavi tercihlerini göstermesi açısından yol göstericidir., AIM: The number of pregnant women applying to dermatology outpatient clinics has increased. This has brought to notice the need for a specialized approach. In order to deal with pregnant women and their diseases, one must have a good knowledge of the skin disorders of pregnancy. Pregnant women and women who are considering pregnancy should be treated exclusively. METHODS: Our aim was to evaluate the skin disorders of the pregnant women and to establish the treatment choices of dermatologists in our city when their patients were pregnant women. RESULTS: Six hundred and ninety seven pregnant women who applied to the outpatient clinics were included in the study. Their diagnoses and the medications which are prescribed to them are retrospectively analyzed. Results: Pruritus, urticaria, acne and contact dermatitis were the most common diagnoses. Mostly topical medications were prescribed by the dermatologists. Among the systemic therapies antihistamines, steroids and antibacterials were prescribed 195, 148 and 87 times respectively which account for almost 95.98% of all systemic medications. CONCLUSION: Pregnancy is a unique period with a different range of dermatological diseases. It is also a vulnerable stage in terms of both the mother and the fetus. That’s why the physicians generally choose topical therapies. Our study is an instructive one, showing the therapy preferences of dermatologists in our city when their patients are pregnant women.
