Amaç: Meckel divertikülü (MD) gastrointestinal kanalın en sık görülen konjenital anomalisidir. Çalışmamızın amacı MD hastalarımızın epidemiyolojik ve karakteristik özelliklerini araştırmak ve konuyla ilgili tecrübelerimizi paylaşmaktır.Gereç ve Yöntemler: MD hastalarının dosyaları retrospektif olarak incelendi. Hastaların demografik ve klinik verileri, uygulanan tedaviler, divertikülün görünümü, lokasyonu ve histopatolojik inceleme sonuçları araştırıldı. Hastalar önce cinsiyetlerine sonra semptomatik olup olmadığına göre gruplandırılarak karşılaştırıldı. İstatistiksel analizler SPSS version 21 ile yapıldı. Epilepsi HastalarındaRutin, Bir Saatlik Ve Tüm Gece EEEG’nin İED Saptamaya KatkısıAmaç: Rutinelektroensefalogram (EEG) incelemelerinde interiktal epileptiform İED saptamaoranı %12-53 arasında değişmektedir. İnteriktal epileptiform deşarj yakalamakiçin tekrarlayan rutin EEG, daha uzun süreli uyku aktivasyonlu veya tüm geceEEG’ler kulanılabilir. Yöntem: Buçalışmada Ocak 2002-Haziran 2022 tarihleri arasında epilepsi tanısı ileizlenen, hem rutin EEG’si, hem bir saatten uzun vEEG’si, hem de tüm gece vEEG’siolan 73 hastayı retrospektif olarak inceledik. EEG’lerinde İED olup olmadığınıkontrol ettik. İED varsa fokal veya jenaralize oluşu, fokalse İED’ninlokalizasyonunu not ettik. Bulgular: İnteriktalepileptiform deşarj rutin EEG’de 30 (%41,1), bir saatten uzun vEEG’de 38(%52,1), tüm gece EEG’de 41 (%56,2) hastada izlendi. Tüm gece EEG ile birsaatten uzun vEEG, İED yakalamada rutin EEG’den istatiksel olarak anlamlıderecede üstün bulundu (p=0,047,p=0,010). Tüm gece vEEG ile bir saatten uzun vEEG karşılaştırıldığında, tümgece vEEG’de İED daha fazla saptanmasına rağmen istatiksel anlamlı farksaptanmadı (p=0,084).Sonuç:Çalışmamızda İED yakalamada hem tüm gece hem de bir saatten uzun vEEG’nin,rutin EEG çekimlerine göre daha üstün olduğunu bulduk. Tüm gece ile bir saatlik vEEG ise İED saptamaaçısından anlamlı fark izlenmedi. Rutin EEG incelemelerine göre daha rahatortamda çekilen 1-3 saatlik vEEG, hastanın uyku evrelerini içerdiği için İEDyakalamada tüm gece EEG ile benzer bulunmuştur. VEEG laboratuvarlarının işyükünü azaltmak için tüm gece yerine gündüz 1-3 saatlik çekimlerdenyararlanılabilir.&nbsp

Objective: In our study, we aimed to analyze the use of Therapeutic plasma exchange (TPE) in the manage-ment of septic and Thrombocytopenia-associated multiple-organ failure (TAMOF) in the burn in-tensive care unit of a children’s hospital retrospectively. Material and Methods: Demographic, clinical, and laboratory data of the pediatric burn patients who were applied TPE between 1 January 2016 and 1 January 2021 were obtained from the hospital information system and medical records and analyzed. The patients were divided into two groups those who died du-ring follow-up and those who recovered. Results: TPE was performed on 14 burned children (Boy: Girl 5:9). The median age of the patients was 6,6 years (range 1-18 years). The mean TBSA of the patients was 47.76% (20-75). The most common cause of burns was flame burn. The mean hospital stay of the patients was 18.4±12.6 (7-94) days. 4 patients in group 1 recovered and 10 patients in group 2 died during follow-up. There was no statistical difference between the groups in terms of age, gender, and TBSA (p=0.590, 0.890, 0.990). We determined that patients in group 2 were statistically higher in terms of MODS (p=0.030), Pelod score (p=0.001), and expected death rate according to Pelod score (p=0.003). It was observed that the application of TPE in the first 24 hours after the occurrence of TAMOF significantly reduced mortality (p=0.010).Conclusion: TPE should be used as an additional treatment method to conventional therapy in critically ill pati-ents in pediatric burn intensive care units. Hastalar takipte ölenler ve iyileşenler olarak iki gruba ayrıldı.Bulgular: 14 yanık çocuk hastaya (Erkek:Kadın5:9) TPE yapıldı. Hastaların ortalama yaşı 6.6 yıl (1-18 yıl)’di. Hastaların ortalama TBSA’sı %47.76 (20-75)’di. En sık yanık nedeni alev yanığıydı. Hasta-ların ortalama hastanede kalış süreleri 18.4±12.6 (7-94) gündü.Grup 1’de iyileşen 4 hasta ve grup 2’de takip sırasında ölen 10 hasta vardı. Gruplar arasında yaş, cinsiyet ve TBSA açısından istatistiksel fark yoktu (p=0.590, 0.890, 0.990). Grup 2’deki hastaların MODS (p=0.030), Pelod skoru (p=0.001) ve Pelod skoruna göre beklenen ölüm oranı (p=0.003) açısından istatistiksel olarak daha yüksek olduğunu belirledik. TAMOF oluştuktan sonraki ilk 24 saat içinde TPE uygulamasının mortaliteyi anlamlı derecede azalttığı gözlendi (p=0.010).Sonuç: Çocuk yanık yoğun bakım ünitelerindeki kritik hastalarda TPE konvansiyonel tedaviye ek bir teda-vi yöntemi olarak kullanılabilir. TAMOF oluştuktan sonraki ilk 24 saat içinde TPE uygulaması mortaliteyi azaltmaktadır.

Objectives This study aims to investigate whether there was a difference between epinephrine (EPN) and tranexamic acid (TXA) in providing visual clarity during arthroscopic rotator cuff tear repair. Patients and methods This double-blind, prospective, randomized-controlled clinical study included a total of 90 patients (42 males, 48 females; mean age: 55.6±8.3 years; range, 18 to 69 years) in whom either EPN (EPN group, n=47) or TXA-diluted irrigation solutions (TXA group, n=43) were used during rotator cuff tear arthroscopy between December 2017 and November 2019. Arthroscopy was performed using irrigation fluid containing 0.33 mg of EPN per 1 L of saline in the EPN group and 0.42 mg of TXA per 1 L of saline in the TXA group. All procedures were performed by two specialized shoulder surgeons. Visual clarity (primary endpoint) was rated by the operating surgeon using the Visual Analog Scale (VAS) in the immediate postoperative period. Secondary endpoints included total operating time (TOT), potential thrombotic or thromboembolic side effects, mean arterial pressure (MAP), and total amount of irrigation fluid used. Results There was no significant difference in the surgeon rated- VAS scores between the groups. The mean VAS score was 7.6±1.62 (range, 4 to 10) in the EPN group and 7.1±1.74 (range, 3 to 10) in the TXA group (p=0.59). No cardiac, thrombotic, or thromboembolic complications were observed in any of the groups. Conclusion Adding TXA to the irrigation fluid during the arthroscopic rotator cuff repair may provide similar visual quality to the EPN, as measured by VAS.

The term "Crisis Europe" simply defines the current state of the EU affairs. Within the last ten years, the financial crisis, the entrance of refugees, the pandemic, and lately, the Russian invasion have deeply damaged the actorness of the bloc. Under the shadow of war, European decision-makers have once again turned to their endless search for solidarity and more integration as a response. Yet, it seems that the EU has a mountain to climb to convince its inner circle and the outer world. This short analysis tries to understand the dynamics that pushed the EU into a crisis stage and issues that consistently pulled the rug out from under the idea of unity. Moreover, it also offers a more flexible approach as an alternative plan to unblock the ongoing impasse.

Amaç: 3. basamak bir çocuk hastanesinde çocuk cerrahisi polikliniğine başvuran veya konsülte edilen labial füzyonlu hastaları retrospektif olarak değerlendirmeyi amaçladık. Gereç ve Yöntem: 01/01/2005-31/12/2020 tarihleri arasında çocuk cerrahisi kliniğine labial füzyon nedeni ile başvuran veya konsülte edilen 889 hasta tanı anındaki yaşları, başvuru şikâyetleri, hangi kliniklerden çocuk cerrahisine konsülte edildiği, tedavileri, nüks ve komplikasyonlar açısından retrospektif olarak değerlendirildi.. Bulgular: Hastaların yaş ortalaması 2,21(0,1-11) yıl idi. Hastaların çoğu asemptomatik idi (%82,5). Hastaların %72,3’ü ebeveynlerin fark etmesi ile direk çocuk cerrahisi polikliniğine getirildi. Bunun dışında pediatri poliklinikleri, pediatrik endokrinoloji polikliniği, pediatrik nefroloji polikliniği ve çocuk acilden hastalar kliniğimize konsülte edildi. İlk muayenede, 885 hastaya manuel seperasyon ve 4 hastaya cerrahi uygulandı. Tüm hastalara işlemden sonra 15 gün boyunca ılık suya oturma banyosu ve topikal östrojen tedavisi uygulandı. Manuel seperasyon yapılan 80 (%9) hastada nüks tespit edildi. Nüks olan hastaların 78’ine yeniden manuel seperasyon yapılırken, sert fibrotik füzyonu olan iki hastaya (%2,5) ise cerrahi seperasyon yapıldı.Sonuç: Labial füzyon tedavisinde, manuel seperasyon ve topikal östrojenli krem tedavi kombinasyonunu, poliklinik ortamında güvenilir şekilde uygulanabilir olması ve nüks oranın düşük olmasından dolayı önermekteyiz. Sert, kalın ve fibrotik labial füzyonlarda ise cerrahi seperasyon tercih edilmelidir., Aim: We aimed to retrospectively evaluate the patients with labial fusion who presented to or were referred to the pediatric surgery clinic in a tertiary pediatric hospital.Material and Method: Between 01/01/2005 and 31/12/2020, 889 patients admitted or consulted to the pediatric surgery clinic due to labial fusion, age at the time of diagnosis, complaints on admission, which clinic referred the patient to the pediatric surgery clinic, treatments, recurrence, and complications were evaluated retrospectively.Results: The mean age of the patients was 2.21 (0.1–11) years. Most of the patients (82.5%) were asymptomatic. Parents noticed the condition and brought their child directly to the pediatric surgery clinic in 72.3% of the cases. Otherwise, patients were consulted to the pediatric surgery clinic from pediatric clinics, the pediatric endocrinology clinic, the pediatric nephrology clinic, or the pediatric emergency department. During the initial examination, manual separation was performed in 885 patients, and surgery was required for four patients. After the procedures, hydrotherapy with warm water and topical estrogen therapy were applied to all patients for 15 days. Recurrence was detected in 80 (9.0%) patients who were treated by manual separation. Manual separation was performed again in 78 of the patients, while surgical separation was performed in two (2.5%) patients who had severe fibrotic fusions.Conclusion: In the treatment of labial fusion, we recommend the combination of manual separation and topical estrogen cream treatment because it can be applied safely in the clinic, and the recurrence rate is low. Surgical separation is preferred in severe, thick, and fibrotic labial fusions.

Amaç: Çocukluklarda COVID-19 sıklıkla asemptomatik ya da daha selim bir seyre sahip olsa da temas riskinden dolayı sağlık çalışanlarının güvenliği asıl hedef olmalıdır. Çalışmamızda hem hasta hem ameliyathane hem de sağlık çalışanlarının iş güvenliğine ışık tutacak acil ve ertelenemeyen cerrahi olgularında asemptomatik COVID-19 PCR pozitifliğini araştırmayı amaçladık.Gereç ve Yöntemler: Çalışmamız pandemi hastanesine dahil çocuk hastanesinin çocuk cerrahisi kliniğine acil ve ertelenemeyen cerrahi nedenler ile başvuran 18 yaş altı olgular için tasarlanmış olup ileriye dönük yapılmış tek merkezli bir çalışmadır. Çalışmaya Mayıs 2020 -Ekim 2020 tarihleri arası altı aylık izlemde acil ve ertelenemeyen cerrahi yapılan olgular dahil edildi. Belirtilen tarihler arasında başvuran 458 hasta değerlendirildi ve kriterleri karşılayan 433 hasta çalışma kapsamında değerlendirildi. olgulardan preoperatif muayene ve tetkik esnasında COVID-19 için nazofaringeal sürüntü (PCR) örneği alındı.Bulgular: Çalışmaya dahil edilen 433 olgunun 155’i kız (%33.5), 278’i (%66.5) erkekti. Yaş ortalaması 9.5 yıl olarak hesaplandı. Acil vakaların tüm olguların 389’unu (%89.8) oluşturduğu saptandı. Yaş gruplarına göre en sık acil patolojilerin; yenidoğan döneminde intestinal obstrüksiyon veya atreziler, okul öncesi dönemde solunum yoluna yabancı cisim aspirasyonlar, okul çağında akut apandisit olduğu görüldü. Nazal sürüntü PCR sonuçları sadece dört hastada pozitif bulundu, bu oranın tüm çalışma grubunda %0.9 olduğu görüldü, bu olguların hiçbirinde postoperatif komplikasyon ve sağlık çalışanına bulaş saptanmadı.Sonuç: Bu prospektif çalışma ile acil veya ertelenemeyen cerrahi planlanan asemptomatik çocuklarda düşük COVID-19 PCR pozitifliği (%0.9 prevalans) tespit edilmiş olup güvenli prosedür uygulamalarının planlanmasına ışık tutacağı kanaatindeyiz., Objective: Although COVID-19 in childhood is often asymptomatic or has a more benign course, the safety of healthcare workers should be the primary goal because of the risk of contamination. In our study, we aimed to investigate the asymptomatic COVID-19 PCR positivity and its effect on the development of complications in emergency and non-deferred surgery cases that will help both the patient, the operating room and the health care workers.Material and Methods: This is a prospective study that was performed in patients under 18 years of age who were operated on due to emergency and urgent cases in the Ankara City Children’s Hospital, which was serving as an anti-pandemic hospital. The patients were evaluated from May 1 to October 1, 2020. Of the 458 patients, 433 patients who met the criteria were included in the study. COVID-19 Nasopharyngeal swab (PCR) samples were taken from all patients along with preoperative examinations.Results: Of the 433 cases included in the study, 155 (33.5%) were girls and 278 (66.5%) were boys. It was determined that emergency cases constituted 389 (89.8%) of all cases. The most common emergency pathologies according to age groups were intestinal obstruction or atresia in the neonatal period, foreign body aspirations of the respiratory tract in the preschool period, and acute appendicitis in the school age. Nasal swab PCR results were positive in only four patients, this rate was found to be 0.9% in the entire study group, and postoperative complications and transmission to healthcare workers were detected at none.Conclusion: In this prospective study, low COVID-19 PCR positivity (0.9% prevalence) was detected in asymptomatic children scheduled for emergency or non-deferred surgery, and we believe that it will shed light on the planning of safe procedure practices.
