- Abstract
Introduction: there is a tendency toward nonoperative management of appendicitis resulting in an increasing need for preoperative diagnosis and classification. For medical purposes, simple conceptual decision-making models that can learn are widely used. Decision trees are reliable and effective techniques which provide high classification accuracy. We tested if we could detect appendicitis and differentiate uncomplicated from complicated cases using machine learning algorithms. Materials and methods: we analyzed all cases admitted between 2010 and 2016 that fell into the following categories: healthy controls (Group 1); sham controls (Group 2); sham disease (Group 3), and acute abdomen (Group 4). The latter group was further divided into four groups: false laparotomy; uncomplicated appendicitis; complicated appendicitis without abscess, and complicated appendicitis with abscess. Patients with comorbidities and whose complete blood count and/or pathology results were lacking were excluded. Data were collected for demographics, preoperative blood analysis, and postoperative diagnosis. Various machine learning algorithms were applied to detect appendicitis patients. Results: there were 7244 patients with a mean age of 6.84 +/- 5.31 years, of whom 82.3% (5960/7244) were male. Most algorithms tested, especially linear methods, provided similar performance measures. We preferred the decision tree model due to its easier interpretability. With this algorithm, we detected appendicitis patients with 93.97% area under the curve (AUC), 94.69% accuracy, 93.55% sensitivity, and 96.55% specificity, and uncomplicated appendicitis with 79.47% AUC, 70.83% accuracy, 66.81% sensitivity, and 81.88% specificity. Çalışmamızda, fundoplikasyon sonrası başarısızlık mekanizmalarını analiz etmeyi amaçladık. Yöntem: Çalışmaya, Ocak 2011 ile Şubat 2014 arasında fundoplikasyon yapılan GÖRH’li çocuklar dahil edildi. Ameliyat sonrası devam eden reflü semptomları, nüks gastroözofageal reflü hastalığı (nGÖRH) olarak kabul edildi. Cinsiyet, yaş, gelişme geriliği, nörolojik durum, hiatus hernisi, skolyoz, ameliyat yöntemleri, gastrostomi ve orofaringeal semptomlar ile nüks oluşumu arasındaki ilişki araştırıldı. Değişkenler lojistik regresyon analizi ile incelendi. Bulgular: Çalışmaya 67 çocuk (erkek 40, kız 27) dahil edildi. Ortalama ameliyat yaşı 57,10±52,22 ay, takip süresi 19,64±7,75 ay idi. On altı (%23,8) çocukta nüks reflü saptandı. Nüks olguların baryumlu calışmalarında yalnızca 1 olguda (%6,2) fundoplikasyon manşonu bozulmuş idi. Manşonu sağlam olan 15 hastadan 10’una endoskopik inceleme yapılabildi. Nüks nedeni olarak, 7 olguda gevşek manşon, 3 olguda aralıklı manşon gevşemesi aktiviteleri gözlendi. Gevşek manşon ve aralıklı gevşemenin, sırasıyla ciddi yutma bozukluğu ve öğürme gibi orofarengeal semptomlar ile ilişkili oldugu saptandı. Sonuç: Nörolojik engeli olan bir kısım hastada anatomik olarak sağlam fundoplikasyon yapıya rağmen, alt özofagus sfinkter relaksasyonlarının devam etmesinin veya gevşek manşon yapısının, hastalarda nüks nedeni olabilecegini varsaymaktayız. Ayrıca bu endoskopik bulguların farklı orofarengeal semp- tomlar ile ilişkisi altta yatan baska bir kompleks nüks mekanizmasını düşündürmektedir. Orofarengeal semptomlar ameliyat öncesinde nüks gelişim riskini öngörmede yararlı olabilir., Objective: Nissen fundoplication is the most frequently performed surgery for the gastroesophageal reflux disease (GERD), however recurrence is frequent in neurologically impaired patients. Anatomic deformation of the fundoplication wrap is the classically defined presentation. The putative risk factors exist in most of cases and but they are unable to define the pathology of failure. We aimed to analyze failure mechanisms after fundoplication. Method: Children with GERD who had undergone fundoplication between January 2011 and February 2014 were included in the study. Ongoing reflux symptoms after surgery were considered as recurrence of gastroesophageal reflux disease (rGERD). Correlation between development of disease recurrence and gender, age, failure to thrive, neurological status, hiatus hernia, scoliosis, surgical methods, gastrostomy and oropharyngeal symptoms was investigated. - Abstract
Introduction: delayed presentation of posterior urethral valves (PUVs) is a rare condition. Presentation and diagnosis of the patients with late PUVs are challenging. Voiding cystourethrogram (VCUG) is mainly practiced. In this study, we aimed to evaluate the children with late-presented PUVs, and the reliability of VCUG in this group. Materials and methods: between January 2003 and December 2017 records of patients who were diagnosed with late-presented PUVs were analyzed. Delayed presentation of PUV was defined as patients who were diagnosed and treated after infancy. Cases were examined in terms of age at diagnosis, presenting symptoms, urinalysis, urinary ultrasound, urodynamic studies, VCUG, and dimercaptosuccinic acid scintigraphy findings. Postoperative follow-up conditions were also assessed. Results: seventeen boys were diagnosed with late-presented PUVs (mean age was 7.35 years). The most common symptoms at presentation were frequency (58.8%), day and nighttime incontinence (47%), and febrile urinary infection (41%). PUV was noted by VCUG in 10 patients alone. The classical sign of dilated posterior urethra was detected in 9 patients. The 10th patient had posterior urethral irregularity. Urethra could not be evaluated due to unsuccessful voiding in one patient. Six patients had normally appearing urethra on VCUG. Reflux was detected in nine (52.9%) patients. Conclusion: late-presented PUVs may be missed on VCUG. Whether a PUV might be present is crucial in boys with a history of recurrent urinary infection, persistent reflux, and repetitive daytime incontinence. Objectives: Lower urinary tract symptoms (LUTS), particularly urgency, incontinence and intermittency are common in children and it is suggested that the specific symptoms may be used for definite diagnosis for LUT dysfunction (LUTD). This study was performed to investigate the relationship between each LUTD and its associated symptoms, using uroflowmetry/electromyography (UF/EMG) as a diagnostic tool. Methods: Each patient was categorized into one of four LUT conditions which were overactive bladder (OAB), dysfunctional voiding (DV), underactive bladder and primary bladder neck dysfunction (PBND), according to UF/EMG results. Patients' complaints and findings were documented by using voiding dysfunction symptom score, bladder diary, urine analysis and ultrasonography. In addition, a detailed history of bowel habits (including use of Rome III criteria) was obtained. Results: There were 189 children of which 106 were female. The OAB was the largest group including 91 patients, followed by the DV group which had 61 patients. The symptoms specific to any LUTD group were constipation and hesitancy (P <.05). Hesitancy was present in 89.4% with PBND and constipation was present in 78.6% of patients with DV. None of other symptoms were able to differentiate any LUTD group from the other. Conclusions: While certain symptoms are often presumed by clinicians to imply specific diagnoses, the main outcome of this study is that there is a generally weak correlation between the specificity of symptoms and LUTD. Symptoms‐based approach may lead to misdiagnosis in LUTD. Thus, it may be essential to focus on the underlying pathologies and UF/EMG test may help this. [ABSTRACT FROM AUTHOR]

Introduction: Meatal stenosis (MS) is a known complication of newborn circumcision. Symptoms are usually masked in young kids with a diaper. Deflation of urinary stream, dysuria, urinary frequency, and incontinence are the most common reported symptoms. Diagnosis of MS is still controversial. A narrow pinpoint urethral meatus is the current accepted diagnostic criteria. Objective: In our practice, we observed that our cases do not overlap to the classic definition and presentation of MS. In this study, we presented our cases and aimed to share our observations. Materials and methods: Records of children who had meatoplasty between 2014 with 2016 were analyzed retrospectively. Only children with MS who had had newborn circumcision performed in our clinic were included in the study. Over 500 newborn circumcisions are performed per year in our clinic using the Gomco Clamp technique. Age at diagnosis, presenting complaints, penile anomalies including urethral meatus configuration, urinary symptoms, operation methods, and postoperative follow-up were gathered. Results: A total of 13 boys who were circumcised in the newborn period had undergone meatoplasty with the diagnosis of MS. The mean age at the time of meatoplasty was 52.5 +/- 17.9 (range 37e93 months). All children were examined before and during the circumcision. There were no associated penile anomalies, including MS. The only presenting complaint was upward deflation of urinary stream instead of other lower urinary tract symptoms. Although the appearance of the meatus was pinpoint in all cases (Figure); it was easy to calibrate with an 8-10 Fr feeding tube. There was no stenosis, but a ventral web was detected. Meato-plasty with removal of this web was performed under general anesthesia. Two sutures were applied at 5 and 7 o'clock with 7/ 0 PDS. Mean postoperative follow-up time was 16.9 +/- 7.5 months. - Abstract
Introduction and Aim: Previous studies have investigated the association between several perinatal factors and the risk of newborn neuroblastoma, but none of these studies have investigated the effect of maternal emotional experiences. The aim of this study is to attract attention to the possible relation between the fear induced maternal stress and newborn neuroblastoma. Material and Methods: The files of three newborn neuroblastoma patients were analyzed and the mothers were questioned to investigate the presence of emotional stress during the pregnancy period. Results: Diagnosis of all babies was discovered in the first two weeks of life. The babies were normal during the prenatal sonographic examinations. Just one month prior to delivery, all mothers experienced severe stress. Two of the mothers were involved in a car accident, and the other experienced severe distress because she was involved in an accident where her son was hurt. In two cases the tumor was surgically removed. These infants are disease free two and three years after the surgery. Complete removal was not successful in the third case, and chemotherapy had to be performed. Four years after chemotherapy, the patient was diagnosed with myelodysplastic syndrome and then secondary leukemia and is now undergoing treatment. Conclusion: Because of the rarity of neonatal neuroblastomas, it is clear that multicenter studies are necessary to understand the epidemiologic background and to develop optimal treatment protocols., Giriş ve Amaç: Daha önce yapılan çalışmalarda çeşitli perinatal faktörlerle yenidoğan nöroblastom riski arasındaki bağlantı araştırılmış, ancak bu çalışmaların hiçbirisi annenin duygusal deneyimlerinin etkisini araştırmamıştır. Bu çalışmanın amacı annenin çok korkmasına bağlı oluşan stresle yenidoğan nöroblastomu arasındaki olası bağlantıya dikkat çekmektir. Gereç ve yöntem: Yenidoğan nöroblastomlu üç olgunun dosyaları incelenmiş, annelerinin perinatolojik dönemde yaşadıkları duygusal deneyimler sorgulanmıştır. Bulgular: Bebeklerin hepsinin tanısı ilk iki hafta içerisinde konulmuştur. Perinatolojik takipleri tamamen normal olan hastaların anneleri son gebelik ayında ciddi bir korkma anı yaşayarak strese maruz kalmışlardır, iki anne trafik kazasına uğramış, diğer annenin ise çocuğu bisikletle annenin gözü önünde kaza geçirmiştir. İki hastada tümör tamamen çıkarılabilmiştir. Bu iki hasta ameliyattan sonra ikinci ve dördüncü yıllarında sorunsuz takip edilmektedir. Üçüncü hastada cerrahi olarak tam çıkarlamamış, ve kemoterapi uygulanmak zorunda kalınmıştır. Kemoterapiden dört yıl sonra myelodisplastik sendrome ve ikincil lösemi tanıları konulmuştur. Hasta halen tedavi altındadır. Sonuç: Çok nadir tümörler olduklarından etkin kemoterapi protokollerini oluşturmak ve epidemiyolojik tabanı saptayabilmek için daha çok merkezli çalışmalara ihtiyaç duyulmaktadır.

29. Kortikosteroidli Kremlerin Yenidoğan Sünneti Sonrası Penil Yapışıklıkların Tedavisinde Yeri Var Mıdır?

- Abstract
Obejective and Aim: The objective ofthis study is to analyze the effectiveness of corticosteroid creams in treatment ofmucosal preputial adhesions after neonatal circumcision. Material and Methods: The treatment modalities ofpreputial adhesions in children, who have been circumcised in the newbom period in our hospital by the Gomco Clamp technique, were reviewed and the results after using corticosteroid creams were analysed. The patients with skin bridges were not included in the study. Results: 181 patients with preputial adhesions were admitted in the last three years. Topical steroid was used for three weeks in 41 of them. Steroid creams were not effective in any of the patients, and the adhesions were simply lysed after EMLA erearn anesthesia. .. Conclusion: Although commonly seen after circumcision, it is stil/ not clear how to approach and how to treat penile adehsions when necessary. Topical steroid application does not have a role in treatment ofmucosal adhesions after circumcision., Giriş ve Amaç: Bu çalışmanın amacı, kortikosteroidli kremlerin yenidoğan sünnetinden sonra gelişebilen mukoza/ prepusyum yapışık/ık/arındaki etkinliğini araştırmaktır. Gereç ve yöntem: Hastanemize yenidoğan sünneti sonrası penil yapışıklıkla başvuran olguların dosyaları incelenmiş, kortikosteroidli krem kullanılanlarda etkinlik araştırılmıştır. Cilt köprüsü olan çocuklar çalışmaya dahil edilmemiştir. Bulgular: Son üç yıl içerisinde 181 hasta prepusyal yapışıklıka başvurmuştur. 41 tanesinde üç hafta boyunca topikal steroid uygulanmıştır. Steroidli kremler hiçbir yapışıklıkta etkili olamamıştır, yapışıklık/ar EMLA krem ile uygulanan fokal anestezi yardımıyla elle açılmıştır. Tartışma: Sünnetten sonra oldukça sık görülmelerine rağmen, gerektiği taktirde penil yapışıklık/ara nasıl yak/aşılacağı ve nasıl tedavi edilmesi gerektiği net değildir. Topikal steroid kullanımının sünnet sonrası mukoza! yapışıklık/arın tedavisinde rolü yoktur.

ÖZET Amaç: Bu çalışmadaki amacımız mesane düz kasının parsiyel obstrüksiyon karşısındaki fizyolojik değişikliklerini incelemek, sistometrik çalışmalarla obstrüksiyon öncesi ve sonrası farklılıkları değerlendirmek ve NO donörü nitroprussid'in detrusor üzerindeki etkisini araştırmaktır. Gereç ve Yöntem: Çalışmada toplam 12 adet erişkin tavşan kullanılmış, 6 tanesi 3 hafta süreyle parsiyel mesane çıkışı obstrüksiyonuna maruz bırakılmış, 6 tanesi ise kontrol grubunu oluşturmuştur. Obstrüksiyonlu tavşanlara, obstrüksiyonlu oldukları 3 hafta öncesinde ve sonrasında sistometri yapılarak değerleri karşılaştırılmıştır. Üç hafta sonunda mesaneden alınan dokuların bir kısmı histolojik değerlendirilmiş, bir kısmı ise deneysel çalışmada kullanılmıştır. Deneysel çalışma için hazırlanan detrusor şeritlerinin elektriksel uyarı ve asetilkolin uyarısıyla kasılmaları kayıt edilmiş, asetilkolin ve neostigmin ile kasılmaları sağlanmış dokuların, NO vericisi olan nitroprussid karşısındaki gevşeme değerleri ölçülerek, kontrol grubundaki değerlerle karşılaştırılmıştır. Detrusor şeritlerinin ayrıca artan ağırlık uygulamaları karşısında boy uzamaları ölçülmüş ve elastik limitleri saptanmıştır. Sonuçlar: Dokuların histolojik değerlendirilmesinde obstrüksiyonlu grupta detrusor kasında hipertrofı, duvar içindeki damarlarda hiperemi ve seyrek mononükleer hücre infiltrasyonu bulunmuştur. Sistometrik değerlendirme sonucunda obstrüksiyonlu tavşanların mesane kapasitelerinde %32.5 kadar azalma saptanmış, ortalama kompliyans değerleri kontrol grubunda 4.3±1.8 iken, obstrüksiyonlu grupta 1.6±0.3 hesaplanmıştır. Detrusor şerit çalışmalarında, kasılma için gerekli eşik voltaj değerleri obstrüksiyon grubunda 57.5'dan 93.3V değerine çıkmış, elektriksel uyarıya yanıt artmış, asetilkolin uyarısında ise belirgin bir fark gözlenmemiştir. Kasılma sağlanmış detrusor 32üzerine nitroprussidin herhangi bir etkisi olmadığı saptanmıştır. Artan ağırlık uygulamaları ile saptanan elastik limit değerinin, obstrüksiyonlu grupta daha yüksek olduğu bulunmuştur. Tartışma: Obsrüksiyona maruz kalan detrusor kendisini obstrüksiyona karşı adapte etmeye çalışmakta, bunun için de hipertrofiye uğramaktadır. Objective: The time of passage of the newborn’s first meconium/stool is an indicator of health and is used to screen for normal gastrointestinal tract function. Most newborns urinate after birth, and this is an indication of normal renal function. The aim of this study is to investigate the meconium/stool and urinary patterns of healthy neonates, and to determine whether they correlate with delivery mode, birth weight, feeding method and its frequency. Patients and Methods: Newborns with a gestational age of ≥ 34 week delivered by normal vaginal delivery (NVD) or cesarean section (CS) were included. The newborns were fed either breast milk exclusively or a combination of breast milk and formula. The frequency of meconium/stool and urine passage and the delivery type, birth weight, and feeding method and its frequency were recorded throughout their hospital stay. Results: A total of 1095 newborns were included. By the first 24 h after birth, 986 (90%) newborns had passed their first meconium or stool and 1084 (99%) newborns had passed their first urine. The mean number of meconium/stool and urine passages was higher in neonates delivered by CS. The mean number of meconium/stools within the first 24 h was higher in newborns fed breast milk exclusively. Combination-fed newborns and newborns with a lower birth weight had a higher mean number of urine passages. The number of meconium passages in the first 24 h was significantly lower in newborns weighing < 2500 g. Conclusion: The mode of delivery, birth weight, and feeding method and its frequency may influence meconium/stool and urinary patterns in newborns. [ABSTRACT FROM AUTHOR]

Introduction: there is a tendency toward nonoperative management of appendicitis resulting in an increasing need for preoperative diagnosis and classification. For medical purposes, simple conceptual decision-making models that can learn are widely used. Decision trees are reliable and effective techniques which provide high classification accuracy. We tested if we could detect appendicitis and differentiate uncomplicated from complicated cases using machine learning algorithms. Materials and methods: we analyzed all cases admitted between 2010 and 2016 that fell into the following categories: healthy controls (Group 1); sham controls (Group 2); sham disease (Group 3), and acute abdomen (Group 4). The latter group was further divided into four groups: false laparotomy; uncomplicated appendicitis; complicated appendicitis without abscess, and complicated appendicitis with abscess. Patients with comorbidities and whose complete blood count and/or pathology results were lacking were excluded. Data were collected for demographics, preoperative blood analysis, and postoperative diagnosis. Various machine learning algorithms were applied to detect appendicitis patients. Results: there were 7244 patients with a mean age of 6.84 +/- 5.31 years, of whom 82.3% (5960/7244) were male. Most algorithms tested, especially linear methods, provided similar performance measures. We preferred the decision tree model due to its easier interpretability. With this algorithm, we detected appendicitis patients with 93.97% area under the curve (AUC), 94.69% accuracy, 93.55% sensitivity, and 96.55% specificity, and uncomplicated appendicitis with 79.47% AUC, 70.83% accuracy, 66.81% sensitivity, and 81.88% specificity. Introduction: Meatal stenosis (MS) is a known complication of newborn circumcision. Symptoms are usually masked in young kids with a diaper. Deflation of urinary stream, dysuria, urinary frequency, and incontinence are the most common reported symptoms. Diagnosis of MS is still controversial. A narrow pinpoint urethral meatus is the current accepted diagnostic criteria. Objective: In our practice, we observed that our cases do not overlap to the classic definition and presentation of MS. In this study, we presented our cases and aimed to share our observations. Materials and methods: Records of children who had meatoplasty between 2014 with 2016 were analyzed retrospectively. Only children with MS who had had newborn circumcision performed in our clinic were included in the study. Over 500 newborn circumcisions are performed per year in our clinic using the Gomco Clamp technique. Age at diagnosis, presenting complaints, penile anomalies including urethral meatus configuration, urinary symptoms, operation methods, and postoperative follow-up were gathered. Results: A total of 13 boys who were circumcised in the newborn period had undergone meatoplasty with the diagnosis of MS. The mean age at the time of meatoplasty was 52.5 +/- 17.9 (range 37e93 months). All children were examined before and during the circumcision. There were no associated penile anomalies, including MS. The only presenting complaint was upward deflation of urinary stream instead of other lower urinary tract symptoms. Although the appearance of the meatus was pinpoint in all cases (Figure); it was easy to calibrate with an 8-10 Fr feeding tube. There was no stenosis, but a ventral web was detected. Meato-plasty with removal of this web was performed under general anesthesia. Two sutures were applied at 5 and 7 o'clock with 7/ 0 PDS. Mean postoperative follow-up time was 16.9 +/- 7.5 months. Children were pleased with direction of urinary stream. Discussion: Both the diagnosis and morphological definition of MS have some controversies. To our knowledge, there is no similar reported study with pinpoint appearance and normal calibration of the meatus. There was no stenosis in any of them. Unlike classical MS presentations, upward deflation of urinary stream resulting from ventral web was a single presenting complaint. Conclusions: Upward deflation of urinary stream can develop after neonatal circumcision because of a meatal web without stenosis, and can be cured easily with meatoplasty. Instead of 'meatal stenosis', 'meatal web' may be a more appropriate term to explain the pathology in these patients., NA

- Abstract
The development of intra- and extrahepatic bile duct stones has been reported as one of the most serious complications after choledochal cyst excision with biliary-enteric reconstruction through Roux-en-Y hepaticojejunostomy (HJ). Here, we report our experience with extracorporeal shock wave lithotripsy (ESWL) in a case of giant intrahepatic stones developing after choledochal cyst surgery. ESWL is an excellent therapeutic modality for large intrahepatic biliary calculi, and after dilating the HJ anastomosis percutaneously, it can be offered as first-line therapy to these patients.
