Cuidar de una persona dependiente, supone tener que hacer frente a las demandas que desbordan las posibilidades del cuidador, si es familiar las consecuencias suelen ser severas. La enfermedad de Alzheimer por su evolución y carácter irreversible puede hacer del enfermo una persona dependiente total. El cuidado en estos casos se considera una situación de estrés, constituyendo uno de los paradigmas para el estudio del estrés crónico. Muchos son los estudios que abordan el malestar del cuidador desde la perspectiva individual sin la precisión en un encuadre teórico. Lo expuesto condujo a la necesidad de fundamentar un diseño metodológico para la evaluación del cuidado informal en enfermos de Alzheimer, a partir del modelo de estrés y afrontamiento adaptado a la situación del cuidado. Lo anterior parte de considerar que los mecanismos que contribuyen a las consecuencias negativas sobre la salud de los cuidadores pueden entenderse mejor desde los modelos relacionales o transaccionales del estrés. Para el cumplimiento del objetivo fueron utilizados métodos teóricos y empíricos, privilegiándose la revisión de la literatura científica en cuanto al tratamiento de los estudios sobre situaciones de cuidado y la figura del cuidador. Se propuso una definición de cuidado informal que facilitó la operacionalización de la variable. Se elaboró un diseño mixto en paralelo. Se eligieron técnicas, algunas de las cuales habían sido elaboradas y/o validadas en estudios sobre cuidadores de enfermos de Alzheimer o situaciones de cuidado. Se obtuvo un diseño viable fundamentado en un modelo teórico coherente con el problema objeto de investigación.Taking care of a dependent person, implies facing the demands that overwhelm the possibilities of the care giver, if the person is a family member, the consequences have a tendency to be severe. In Alzheimer’s disease, due to its natural history and irreversible character, the patient becomes into a total dependent individual. The care of these cases is considered a stress condition, making it one of the paradigms to the study of chronic stress. Numerous studies deal with the discomfort of the care givers from an individual perspective without a theoretical frame. According to the analysis carried out it was necessary to create a methodological design to the assessment for the informal care of Alzheimer’s patients, from the stress and face-up models regarding the situation of the care-giver. The previous, started from the consideration of the mechanisms which contribute to the negative consequences for the health of care givers, overseeing relational or transactional models of stress. To achieve this objective, theoretical and empiric methods were used, granting privilege to the review of scientific literature with respect to the management of the studies about situations; where the care and the figure of the care givers are involved. A definition of the informal care was conducted to provide the implementation of the variable. A mixed design was created in parallel where some techniques were chosen; some of them had been prepared and validated in studies about Alzheimer’s patients or situations of health care. A feasible design supported on a theoretical method consistent with the problem of investigation.

La enfermedad de Alzheimer, dado su curso progresivo, obliga al acompañamiento de estas personas. Quienes fungen como cuidadores informales presentan quejas por malestares asociados a la situación de cuidar. Para la comprensión de esta problemática se realizó un estudio descriptivo y transversal cuyo objetivo general fue caracterizar el cuidado informal de personas con demencia en la enfermedad de Alzheimer, a partir de las dimensiones comunes de los modelos de estrés y afrontamiento adaptado a las situaciones de cuidado. Se seleccionó una muestra intencional de quince cuidadores informales e igual número de familiares enfermos, pertenecientes a la ciudad de Pinar del Río. Para el cumplimiento del objetivo se utilizaron las técnicas: entrevista semiestructurada, el Inventario de ansiedad de Beck, la Escala de Depresión del Centro para Estudios Epidemiológicos, el Cuestionario de carga del cuidador de Zarit, el test Listado de problemas de memoria y de conducta, el Cuestionario de apoyo social y la Escala de modos de afrontamientos. Entre los principales resultados estuvo la presencia de estresores relativos a la cantidad de horas y actividades de cuidado. Predominaron los comportamientos problemáticos con presentación muy frecuente en la mayoría de los enfermos, refiriéndose en los cuidadores sentimientos negativos asociados a los mismos. Predominó la percepción de sobrecarga intensa, así como ansiedad y depresión. La estrategia de afrontamiento más utilizada fue la aceptación de la responsabilidad, seguida del autocontrol. La mayoría de los cuidadores perciben un bajo apoyo social. Se declara repercusión del cuidado en las áreas laboral, socioeconómica y de descanso.Alzheimer's disease, due to its gradual progression, compels accompanying persons for these patients. Caregivers complain of discomforts associated with the care of Alzheimer's patients. To be aware of this problem a descriptive and cross-sectional study was conducted, aimed at characterizing caregivers of Alzheimer's patients with dementia from the common dimensions of stress and coping-psychosocial adaptation models with the situation of care giving. An intentional sample was comprised of fifteen (15) caregivers and the same number Alzheimer's disease patients belonging to Pinar del Rio city. Semi-structured interviews, Beck anxiety inventory test (BAI), depression inventory to the Center of Epidemiological Studies, questionnaire Zarit caregiver burden, conduct and memory test, social support questionnaire, coping mode scale were applied. The presence of stress due to the quantity of hours in care giving activities was among the main results. Very frequent presentation of problematical behaviors prevailed in the majority of patients, and caregivers expressed negative feelings associated with this entity; the perception of extreme burden together with anxiety and depression prevailed. Coping-adaptation strategy with their responsibility was the most used, followed by self-control. The majority of caregivers perceived low-social support. The repercussion of care giving activity is confirmed in working, socioeconomic and leisure time areas.

Fidel Castro ha sido el artífice de muchísimas proezas y logros de la salud Pública Cubana. En el presente trabajo se pretende exaltar el papel protagónico de su pensamiento y para ello se realiza análisis de documentos que avalan el quehacer del mandatario en los diferentes momentos por los que ha decursado la Salud Pública Cubana. Los resultados muestran que Fidel ha sido el actor social principal en el proceso de Formación de Recursos Humanos en Salud y todas las transformaciones que se han producido, incluida la creación de las Escuelas Latinoamericanas de Ciencias Médicas, fue además quien ideó la existencia en Cuba del Programa del Médico de la Familia. Se ilustra el pensamiento internacionalista del líder cubano como rector del quehacer del personal de la Salud en el país y los resultados del trabajo de la Misión Milagros y el Contingente Henry Reeve. Los éxitos en el Sector Salud Cubano tienen como principal referente teórico el pensamiento del Comandante en Jefe. Se recomienda la necesidad de profundizar, durante el proceso de formación, el estudio y aplicación del pensamiento de Fidel.Fidel Castro has been the architect of many deeds and achievements in the Cuban Public Health System. The present paper was intended to praise the leading role of his thoughts. An analysis of the documents that back up his works during the different moments of the Cuban Public Health System was carried out. Results showed that Fidel Castro has been the main social actor in the process of forming human resources in health assistance and all the transformations of the Cuban Health System. He was the founder of the Latin-American Medical Schools and devised the existence in Cuba of the Family Doctor Program. The thought of the Cuban leader as a rector of the tasks of the Health Personnel into the country and the results of the internationalism with the creation of " Mision Milagro" and "Henry Reeve" Brigade- both his ideas- were also analyzed. The success of the Cuban Health Sector has as a main theoretical referent, the thoughts of the Commander in Chief. It is recommended the need to deepen, during the process of formation, the study and application of Fidel's thoughts.

La Edad es un factor determinante en el sueño y son los adultos mayores una población muy afectada por trastornos de la cantidad y la calidad del sueño. Fueron objetivos de esta investigación determinar la Prevalencia de Trastornos de sueño en 116 adultos mayores en régimen ambulatorio, pertenecientes a dos consultorios médicos del Policlínico Hermanos Cruz, así como la asociación de estos trastornos a algunos factores. Los Ancianos fueron encuestados sobre si tenían o no trastornos del sueño, sus tipos, si ingerían café y horario de la ingesta evaluándose su estado psicológico según Escala de depresión geriátrica. Los resultados muestran que un 52,6 % de los ancianos tenían trastornos de sueño, de ellos, el sexo femenino fue el más afectado triplicando el riesgo de padecerlo. Además, se comprobó una asociación significativa de los trastornos de sueño con el estado psicológico. El tipo más frecuente de Trastorno de sueño resultó ser el insomnio y entre ellos el mixto. Concluyendo que alrededor de la mitad de la población anciana en régimen ambulatorio padece de trastornos de sueño muy en relación a trastornos psicoafectivos y al sexo de los pacientes.Age is a significant factor in sleeping and the aged is a very affected population due to the quantity and quality of sleeping.The purpose of this research is to determine the prevalence of the sleeping disturbances in 116 adults in ambulatory setting attending to two Family Medical Offices in "Hermanos Cruz "Policlinic, as well as the association of these disturbances to some factors. The aged were surveyed for knowing if they had or not sleeping disturbances, its types , if they drink coffe and time of the drinking and it is assessed the psychological condition according to the geriatric depression scale.Results show that 52,6% of them had sleeping disturbances being female the most affected sex (3 times over), likewise it was proved a significant association between the sleeping disturbances and the psychological condition being insomnia the most frequent type of sleeping disturbance and among them ,the mixed one, it is concluded that almost half of the aged population in ambulatory setting suffers from sleeping disturbances being close related to psychoaffective disturbances and to the sex of the patients.

La respuesta cortical del Potencial Somatosensorial del nervio tibial posterior se registra en la mayor parte de los laboratorios utilizando una sola derivación precisándose cada vez más el diseño de nuevos protocolos que abarquen el registro de mayor número de áreas corticales. Esta investigación se propuso diseñar e implementar el registro del Potencial Evocado Somatosensorial con 19 derivaciones del Sistema Internacional 10/20 en el equipo MEDICID-4 lo que permite el mapeo de la respuesta cortical de la P40 del nervio tibial posterior. Inicialmente se buscó el valor del corte a las altas más bajo, que permitiera obtener el registro sin modificar sus características. Para ello se registró el potencial en 10 sujetos sanos con el electromiógrafo NEURONICA-4 utilizando diferentes filtros a las altas frecuencias. Posteriormente se diseñó un protocolo con 19 derivaciones en el electroencefalógrafo MEDICID-4, utilizando un ancho de banda entre 0.5 y 300 Hz. Fue necesario modificar el software TrackWalker para que permitiera una frecuencia de muestreo de 1 kHz y para lograr que las marcas de sincronismo del estimulador externo fueran reconocidas por el software EP Workstation. Los resultados arrojaron que los cortes a las altas inferiores a 300 Hz modificaron marcadamente la latencia del componente P40. Con este corte se logró obtener respuestas sin diferencias significativas (p> 0.05) en latencia, amplitud y frecuencia a la obtenida con el corte a las altas frecuencias más recomendado en la literatura. Se concluyó que con este protocolo diseñado, es posible registrar la respuesta cortical del Potencial Evocado Somatosensorial del nervio tibial posterior en más de una derivación lo que permitirá contribuir a la caracterización del patrón de anormalidad en ciertos estados patológicos.ABSTRACT The cortical response of the somatosensory potential of the posterior tibial nerve is recorded in the most of laboratories using only one derivation and requiring even more the design of the new protocols involving the record with the most cortical areas; the present research tries to design and implement the Somatosensory Evoked Potential with 19 derivations from the International System 10/20 in the 4-MEDICID-equipment, allowing a mapping of the cortical response in P40 of the posterior tibial nerve. Initially, we look for the lower cutting value in the high frequencies which allowed the record with any change in the characteristics. For this purpose the potential in 10 healthy subjects is recorded using the 4- NEURONICA- electromyography, with the application of different filters in high frequencies. Later, a protocol with 19 derivations is designed in the 4-MEDICI electromyography using a band weigh between 0.5 and 300 Hz. It was necessary to modify the Trackwalker software for allowing a 1 kHz sample frequency and for obtaining that the synchronization traces of the external stimulator were recognized by EP Workstation software. Findings yielded that cuts at high frequencies less than 300 Hz modified significatively the latency of component P40.With this cut ,responses with no significant differences were obtained ( p>0.05) according to latency, weigh and frequency in comparison to those obtained with the cut at high frequencies which is more recommended in literature. It is concluded that with this protocol is possible to record the cortical response of the somatosensory evoked potential of the posterior tibial nerve in more than a derivation, contributing to the characterization of abnormal pattern in some pathologic conditions.

Introducción: Se dispone de escasos indicadores predictores del rendimiento académico en la asignatura Fisiología. Objetivos: Evaluar la capacidad predictiva del corte de la décima semana respecto a los resultados de examen final ordinario en la asignatura de Fisiología de la carrera de Medicina. Materiales y Métodos: Se estudiaron los resultados del corte y la nota del examen final ordinario de 1250 estudiantes que recibieron la asignatura en los últimos 5 semestres que se han impartido hasta este curso 2006-2007 en la Facultad de Ciencias Médicas de Pinar del Río "Dr. Ernesto Guevara de la Serna". Para variables cualitativas se utilizaron el cálculo de porcentajes y números absolutos. La concordancia entre las evaluaciones se evaluó mediante el índice Kappa al 95 % de certeza. Además se realiza Chi cuadrado para evaluar capacidad predictiva. Resultados: Existió correspondencia de los resultados del corte con los resultados del examen final ordinario con significación leve en cuanto a la asociación cuando se usa como predictor de éxito-fracaso. Se constataron diferencias en el comportamiento por semestres y cursos escolares. Conclusiones: El corte de la décima semana constituye un indicador que aproximadamente es capaz de predecir el rendimiento académico de los alumnos en la asignatura Fisiología y que es mejor predictor cuando existen mayor número de estudiantes evaluados dentro de cada categoría.Introduction: Predictive indicators of the academic profit in Physiology subject are scarce. Objectives: To assess the predictive capacity of the academic cutting in the 10th week in relation to the results of the final examination of Physiology subject in medical major. Material and methods: Results of the academic cutting and the final examination marks of 1250 students receiving the subject in the last five semesters till course 2006-2007 were collected at "Ernesto Guevara del Serna" Medical School in Pinar del Rio. Qualitative variates were used to calculate the percentages and absolute numbers. The coincidence in evaluations was assessed using Kappa index with a 95% of certainty. Results: A correspondence with the results of the academic cutting and the results of the final examination was observed having a slight significance regarding the association when using failure-success as a predictor. Differences in the behavior per semesters and the academic courses were noted. Conclusions: The academic cutting in the tenth week constitutes an indicator to predict the academic profit of the students in Physiology subject and this was the best predictor to be used when having a greater number of students evaluated in each category.

La respuesta cortical del Potencial Evocado Somatosensorial del nervio tibial posterior ha mostrado en diferentes investigaciones una gran variabilidad en cuanto a los resultados obtenidos por diferentes autores. Los objetivos de este trabajo fueron evaluar en una muestra de sujetos adultos sanos la distribución topográfica de la respuesta cortical del potencial evocado somatosensorial del nervio tibial posterior empleando un protocolo de registro con 19 derivaciones del Sistema Internacional 10/20 en el equipo MEDICID-4. Se estudiaron 18 sujetos adultos sanos. En todos los sujetos se obtuvo la respuesta cortical P40, manteniéndose los 19 canales de registro, modificándose los filtros a valores entre 0.5 y 300 Hz, y la frecuencia de muestreo se modificó a 1000 Hz. Los resultados muestran una máxima amplitud de la P 40 en línea media. Además, simultáneamente se obtiene dicha respuesta en la propia región centro-parietal ipsilateral, sin existir gran variabilidad intra e interindividuos. De esto se concluye que la estimulación del nervio tibial el pico P40 mostró una distribución cortical centro-parietal tanto en línea media como ipsilateral, con máxima amplitud en esta última, es decir, la P40 se obtuvo paradójicamente con mayor amplitud hacia la misma región del lado estimulado.The cortical response of the somatosensorial evoked potential of the posterior tibial nerve has shown, in different research works, a great variability in terms of the results obtained by different authors. The aims of this paper were to evaluate in a sample of healthy adult individuals the topographical distribution of the cortical response of the somatosensorial evoked potential of the posterior tibial nerve by using a protocol registration with 19 derivations from the International System 10/20 in the MEDICID-4 team. Eighteen adult individuals were studied. In each individual the cortical response was P40, remaining the 19 registration canals, modifying the filter between 0.5 and 300 Hz, and the sampling frequency was modified to 1000 Hz. Results show a maximum amplitude of the P40 in the middle line. In addition, simultaneously, such response is registered in the central parietal ipsilateral region, with no great intraindividual and interindividual variability. As a conclusion, the stimulation of the tibial nerve and peak P40 showed a cortical central parietal distribution in the middle line, as well as ipsilaterally, with maximum amplitude in the latter. P40 was obtained paradoxically with a greater amplitude towards the same region of the stimulated site.
