Summary: Tumor immunotherapy has been convincingly demonstrated as a feasible approach for treating cancers. Although promising, the immunosuppressive tumor microenvironment (TME) has been recognized as a major obstacle in tumor immunotherapy. It is highly desirable to release an immunosuppressive “brake” for improving cancer immunotherapy. Among tumor-infiltrated immune cells, tumor-associated macrophages (TAMs) play an important role in the growth, invasion, and metastasis of tumors. The polarization of TAMs (M2) into the M1 type can alleviate the immunosuppression of the TME and enhance the effect of immunotherapy. Inspired by this, we constructed a therapeutic exosomal vaccine from antigen-stimulated M1-type macrophages (M1OVA-Exos). M1OVA-Exos are capable of polarizing TAMs into M1 type through downregulation of the Wnt signaling pathway. Mediating the TME further activates the immune response and inhibits tumor growth and metastasis via the exosomal vaccine. Our study provides a new strategy for the polarization of TAMs, which augments cancer vaccine therapy efficacy.

Bu çalışmada, AISI 1.2738 malzemenin farklı işleme parametrelerinde elektro erozyon tezgahında bakır ve grafit elektrot ile aşındırma işlemi gerçekleştirilmiştir. Kullanılan aşındırma parametreleri yapılan ön deneyler sonrasında üç farklı vurum süresi (100, 200 ve 300 µs), bekleme süresi (10, 20 ve 30 µs) ve boşalım akım (10, 20 ve 30 amper) olarak belirlenmiştir. Sinyal/Gürültü oranları ile belirlenen yüzey pürüzlülük değerinin optimum değerleri, vurum süresi için Seviye 1 (100 µs), bekleme süresi için Seviye 2 (30 µs), amper için Seviye 1 (10 A) ve elektrot için Seviye 2 (Grafit), anova sonuçlarına göre ise yüzey pürüzlülüğü üzerine en etkili parametrelerin sırası ile amper, vurum süresi, bekleme süresi ve elektrot, gri ilişki analizine göre her iki elektrot malzemesi için ideal yüzey pürüzlülük değerinin kullanılabileceği parametreler 100 µs vurum süresi, 30 µs bekleme süresi ve 20 amper akım olarak tespit edilmiştir.

Background & objectives: Crimean-Congo hemorrhagic fever (CCHF) is a fatal disease, caused by a tick-borne virus (Nairovirus), having a high mortality rate. The study was aimed to evaluate the risk factors, the presenting symptoms and findings of the patients with prediagnosis of CCHF disease, and to compare these variables between the CCHF-positive and CCHF-negative patients. It was also aimed to develop a scoring formula for the diagnosis of CCHF. Methods: In total, 281 patients who were admitted to the Sabuncuoglu Serafeddin Training and Research Hospital, Amasya, Turkey between 2011 and 2015 and were prediagnosed with CCHF based on the clinical symptoms, laboratory findings and risk factors were included in the study. The definitive laboratory diagnosis of patients with prediagnosis of CCHF was ensured via molecular and serological methods. Aim:Depression and hopelessness, cause impairment in the quality of life and leads to the loss of one's efficiency are important problems. The purpose of this study is to search the frequency of depression and hopelessness and its relations with socio-demographic characteristics among medical students.Materials and Methods:This is a cross sectional analytical study. It was conducted 2-4 March 2009 between. 1st, 2nd, and 3rd year students from the Faculty of Medicine, Suleyman Demirel University were included in the study. 236 of a total 330 students (68.5 %) participated in the study. A questionnaire which aims to determine the students’ socio-demographic characteristics, Beck depression inventory and Beck hopelessness scale were applied under observation to the participants. According to the Beck depression inventory, those who scored 17 and over were classified as possible depressive. Data were evaluated statistically.Results:The average score of the students according to Beck hopelessness scale was found 5.3±4.6. According to Beck depression inventory 64 of the students (28.3%) were found to be possible depressive. The average Beck hopelessness scale score was higher for possible depressive students compared to the others (p=0.00). Depression scores was higher among the first year students compared to the others (p=0.03), higher among the females compared the males (p=0.02) and higher among dwelling in dorm compared to not dwelling in dorm (p=0.001). When the age of students, monthly income of family, the educational status of the parents were compared with hopelessness and depression rates there was no statistically significant relation.Conclusion:As a result, depression rates were high in basic medical students, furthermore hopelessness scores were found high among possible depressive students. Medical students should be given guidance and counseling service. Effective monitoring and prevention programs should be developed in order to lower the possible effects of depression and hopelessness on their future professional lives.

Experimental acute sinusitis was induced in 21 New Zealand hybrid rabbits by occluding the ostium and inoculating them with Streptococcus pneumonia. While a group of rabbits with sinusitis was left untreated, two other groups were administered parenteral sodium nitroprussid (SNP) and oral levofloxacin for ten days. While staphylococci species, non-hemolytic streptococcus and contaminated flora were isolated from the sinuses of controls, Streptococcus pneumonia was re-isolated in two of six untreated rabbits, in one of six SNP administered rabbits and none of the levofloxacin treated rabbits. Serum and maxillary sinus mucosal nitric oxide (NO) levels were correlated. While the mean maxillary sinus NO level of controls was significantly higher than that of untreated rabbits, the mean maxillary sinus and serum NO levels were significantly higher in SNP administered rabbits than in the others. Although goblet cell hyperplasia and squamous cell metaplasia were detected in some slides, edema and neutrophil infiltration were the prominent findings. The most severe inflammatory changes were found in the untreated sinusitis group on the third and fifth days. The earliest improvement was observed in the levofloxacin treated rabbits. It was concluded that NO level is decreased during acute sinusitis and that SNP administration hastens the bacteriological and histological recovery.

The resistance to erythropoietin, which is used to treat normochromic, normocytic anemia in chronic renal failure, can develop in patients with conditions such as iron deficiency, aluminum toxicity, hyperparathyroidism, chronic inflammatory diseases, and primary hematological disorders. We found amyloidosis in the bone marrow of a woman without any other etiology for erythropoietin resistance who was undergoing chronic hemodialysis. Her anemia did not improve, despite 6 months of erythropoietin therapy. Bone marrow amyloidosis was found to be the reason for erythropoietin-resistant anemia in our patient with chronic renal failure and renal anemia. We present the case of bone marrow amyloidosis because it is a very rare cause of erythropoietin resistance.

Tuberous sclerosis (TS) is an autosomal dominant disorder. It is characterized by hamartomatous lesions in various organs such as the brain, skin, kidneys and heart. Subependymal and parenchymal nodules occur in the cerebrum. Although giant cell astrocytoma may develop from these subependymal nodules, tumor formation is very rare in the cortical nodules. Epstein-Barr virus (EBV) is implicated as an etiologic factor in Hodgkin's disease (HD). The proportion of HD patients with EBV is high in developing countries but low in developed countries. In the present study, the EBV association with HD in Northeast Anatolia was investigated. Thirty-six formalin-fixed paraffin-embedded cases of HD were analysed for the presence of EBV and, for the latent membrane protein (LPM-1) by immunohistochemistry. There were 26 males and 10 females; age distribution ranged from 12 to 73 years (mean ± standard deviation, 34.1 ± 15.2 years). Overall, LPM-1 was detected in 27 of the 36 cases (75%). LPM-1 expression varied according to the istological subtype of HD (9/55 cases of lymphocyte predominance subtype, 3/4 cases of nodular sclerosis, 18/18 cases of mixed cellularity subtype, and 1/5 cases of lymphocyte depletion subtype). In this study, the EBV-positivity ratio in HD was found extremely high in Northeast Anatolia.

A case of a large dentigerous cyst containing a canine tooth in the maxillary antrum is presented. This case is of interest due because of its extensiveness and the presence of a canine tooth in the roof of the maxillary sinus. In addition, this caused a right-sided epiphora. Enucleation of the cyst containing the ectopic tooth was made.
