Amaç: Coronavirus 2019 (COVID-19)’a bağlı olarak yaşanan kapanma süreçleri Alzheimer Hastalarının nöropsikiyatrik durumu üzerinde olumsuz bir etkiye sahip olabilir. Bu çalışmada, Alzeimer Hastalarının tam ve kısmi karantina dönemlerindeki nöropikiyatrik durumları değerlendirilerek geleceğe yönelik çıkarımların bulunması amaçlanmıştır. Gereç ve Yöntemler: Çalışmamız prospektif, kesitsel ve çok merkezli bir çalışma olup; Türkiye'nin farklı bölgelerindeki polikliniklerce en az bir yıldır takip edilen Alzheimer Hastalarıyla yapılmıştır. Sosyo-demografik bilgiler, komorbidite, mobilite ile tam kapanma ve kısmi kapanma sürecindeki sosyal etkileşim düzeyi, Klinik Demans Değerlendirme (CDR) Ölçeği ve Nöropsikiyatrik Envanter’e (NPI) ilişkin bilgiler bakımverenler ile dolduruldu. Bulgular: Çalışmaya toplam 302 Alzheimer Hastası (ortalama yaş: 78±8 yıl, ortalama eğitim süresi: 5,8±9 yıl) dahil edildi. En sık eşlik eden hastalığın hipertansiyon olmak üzere, toplam komorbidite oranı %84 olarak bulundu. NPI skorlarının ortalaması tam kapanmada 22,9±21 ve kısmi kapanmada 17,7±15 olup gruplar arasındaki farkın istatistiksel olarak anlamlı olduğu bulundu. Kapanma süreleri ve cinsiyete göre NPI puanlarının toplam puanları ile karşılaştırıldığında, tam kapanmada sosyal etkileşimlerin varlığı, mobilite ve komorbiditelerin etkisi kısmi kapanmaya göre daha yüksek bulundu. Tam kapanmadan kısmi sokağa çıkma yasağına geçildiğinde, komorbiditeler, mobilite ve CDR'ın NPI puanları üzerinde anlamlı bir etkisi olduğu bulundu. Regresyon analizinde, her iki kapanma süresi için de Alzheimer Hastalarının nöropsikiyatrik durumu üzerinde en etkili parametrenin CDR puanı olduğu bulundu. Sonuç: Kapanma ile ilgili kısıtlamalar azaltıldığında, Alzheimer Hastalarının nöropsikolojik durumları önemli ölçüde iyileşti. Kapanma kuralları bu veriler göz önünde bulundurularak düşünülmelidir. The mean NPI score was 22.9±21 in total lockdown and 17.7±15 in partial lockdown, which is statistically significantly different. When lockdown periods were compared with the total scores of NPI scores according to gender, existence of social interactions, mobility, and comorbidities were found higher in the total lockdown than the partial lockdown. When switching from total lockdown to partial lockdown, the presence of comorbidities, mobility, and CDR were found to be factors that had a significant effect on NPI scores. In regression analysis, CDR score was found as the most effective parameter on the neuropsychiatric status of AD cases for both lockdown periods. Conclusion: When lockdown-related restrictions were reduced, the neuropsychological conditions of AD cases were significantly improved. Lockdown rules should be considered with these data in mind.

Objective: Interest in the concept of palliative care is increasing in patients with advanced dementia; however, the effects of palliative care on the natural course of the disease remain unknown. Therefore, this study aimed to present the 1-year experience of an institutional palliative care unit for patients with dementia. Materials and Methods: This is a retrospective cross-sectional before-after archive study, which included 35 patients who received palliative care for at least 6 months in the Palliative Care Unit of the Dementia-Alzheimer Disabled Care Center, operated by the Turkish Alzheimer Association's Mersin Branch. Patient data during the palliative care unit admission and the end of 6 months of care were compared. Results: The mean age of 35 patients (14 females and 21 males) was 80.00±8.47 years. The average length of stay in the palliative care unit was 10.03±2.13 months. Patient weights increased with palliative care (p<0.001). Patients with pain experienced a decrease in their pain levels (p=0.014). Pressure ulcers in 10 (28.57%) patients during admission had improved pressure sore stage and pressure ulcer healing scale (p=0.007 and p=0.005, respectively). No new pressure ulcers occurred in any patient. There was a decrease in patients with behavioral symptoms, and no patients developed new behavioral symptoms and/or sleep disorders. Conclusion: Our results indicate that the quality of life of patients who received multidisciplinary care in an institutional palliative care unit improved. [ABSTRACT FROM AUTHOR]

ÖZETMERSİN İLİ MUT İLÇESİ'NDEKİ DEĞİŞİK YAŞ GRUPLARINDA LEISHMANIAANTİKOR DÜZEYLERİLeishmaniasis, Leishmania cinsi parazitik kamçılı protozoonların neden olduğuhastalığa verilen genel bir isim olup, klinik olarak Visseral Leishmaniasis, KutanözLeishmaniasis ve Mukokutanöz Leishmaniasis olmak üzere üç grup altındaincelenmektedir.Bu çalışmada Mersin İli Mut ilçesinde yaşayan 2 yaşın üzerinde, değişik yaşgruplarında Leishmaniasis lezyonu bulunmayan, 212 erkek ve 196 kadın olmaküzere toplam 408 kişiden alınan serum örneklerinde IFA yöntemi ile Leishmania IgGantikor varlığı araştırılmıştır. Rastlantısal olarak toplanan Mut ilçesi ve köylerindeikamet eden bu kişilerden alınan örneklerin incelenmesi sonucunda, çalışmaörneklerinin 5'inde (% 1.2) Leishmania IgG tipi antikor varlığı tespit edilmiştir.Örneklerin 4 ünde (% 0.98) 1/32 titre düzeyinde, 1'inde (% 0.24) ise 1/64 titredepozitiflik saptanmıştır.Endemik bölgelerde parazitle mücadelede hastaların tespiti ve tedavi edilmesi,bölgede yaşayan insanların eğitimi ve bu kişilere koruyucu önlemlerin aktarılması,bölgemizde düşük endemi gösteren hastalığın yok edilebilmesi için gerekligörülmektedir.Anahtar sözcükler: Leishmaniasis, IFA, IgG antikor ABSTRACTANTI-LEISHMANIAL ANTIBODY LEVELS IN MUT TOWN OF MERSİN CITYThe cause of Leishmaniasis is a flagelled protozoon known as Leishmania. The study group was composed of 408 people ( 212 male,196 female). The samples were collected from the region of Mut randomly. We detectedIgG antibody of Leishmania in 5 of the collected samples (1.2 %). We also detected 1/32titer of Ig G antibody in 4 of the samples (0.98 %) and 1/64 titer of IgG antibody level in1 of the samples ( 0.24 %).Detection and treatment of the patients, education and protective applications tothe people living in endemic regions, seems to be needed, in order to eradication of thislow endemic disease.Key words: Leishmaniasis, IFA, IgG antibody 43
