Purpose: Inappropriate dosing of direct oral anticoagulants is associated with an increased risk of stroke, systemic embolism, major bleeding, cardiovascular hospitalization, and death in patients with atrial fibrillation. The main goal of the study was to determine the prevalence and associated factors of inappropriate dosing of direct oral anticoagulants in real-life settings. Methods: This study was a multicenter, cross-sectional, observational study that included 2004 patients with atrial fibrillation. The study population was recruited from 41 cardiology outpatient clinics between January and May 2021. The main criteria for inappropriate direct oral anticoagulant dosing were defined according to the recommendations of the European Heart Rhythm Association. Results: The median age of the study population was 72 years and 58% were women. Nine-hundred and eighty-seven patients were prescribed rivaroxaban, 658 apixaban, 239 edoxaban, and 120 dabigatran. A total of 498 patients (24.9%) did not receive the appropriate dose of direct oral anticoagulants. In a logistic regression model, advanced age, presence of chronic kidney disease and permanent atrial fibrillation, prescription of reduced doses of direct oral anticoagulants or edoxaban treatment, concomitant use of amiodarone treatment, and non-use of statin treatment were significantly associated with potentially inappropriate dosing of direct oral anticoagulants. Conclusion: The study demonstrated that the prevalence of inappropriate direct oral anticoagulant dosing according to the European Heart Rhythm Association recommendations was 24.9% in patients with atrial fibrillation. Several demographic and clinical factors were associated with the inappropriate prescription of direct oral anticoagulants. © 2022, The Author(s), under exclusive licence to Springer Science+Business Media, LLC, part of Springer Nature.

- Abstract
Extracellular nanovesicles, particularly exosomes, can deliver their diverse bioactive biomolecular content, including miRNAs, proteins, and lipids, thus providing a context for investigating the capability of exosomes to induce stem cells toward lineage-specific cells and tissue regeneration. In this study, it is demonstrated that rat subventricular zone neural stem cell-derived exosomes (rSVZ-NSCExo) can control neural-lineage specification of human mesenchymal stem cells (hMSCs). Microarray analysis shows that the miRNA content of rSVZ-NSCExo is a faithful representation of rSVZ tissue. Through immunocytochemistry, gene expression, and multi-omics analyses, the capability to use rSVZ-NSCExo to induce hMSCs into a neuroglial or neural stem cell phenotype and genotype in a temporal and dose-dependent manner via multiple signaling pathways is demonstrated. The current study presents a new and innovative strategy to modulate hMSCs fate by harnessing the molecular content of exosomes, thus suggesting future opportunities for rSVZ-NSCExo in nerve tissue regeneration.

- Abstract
In this research, the effects of rubidium (Rb) substitution on the microstructural, physical, and superconducting properties of Bi2Sr2Ca1-xRbxCu2O8 + y with x = 0.0, 0.025, 0.05, 0.075, 0.10, and 0.125 superconductor have, in detail, been studied. For this purpose, samples were prepared through the classical solid state reaction route, followed by directional grown using the laser floating zone technique and annealing. XRD measurements showed that all samples are composed of Bi-2212 phase as the dominant one, together with less amount of secondary phases. The crystal symmetries of samples were determined as pseudo tetragonal. SEM-EDX confirmed that Bi-2212 phase is the major one, with minor amounts of Bi2 + x(Sr,Ca)2O6 + d, (Sr,Ca)CuO2, CuO2, and CaO secondary phases. In magnetization versus temperature measurements, very sharp diamagnetic transition is observed. The critical onset temperatures, TC, for all samples are obtained around 90 K. From M-H measurement, it has been found that all samples have very large loop areas. By using the data from M-H measurements in the Bean model, the intragranular critical field, JC, has been determined. It has been found that the x = 0.05Rb sample shows the highest superconducting properties and the largest JC values (2.2 105 A/cm2 at 10 K). Moreover, the maximum pinning force, FPmax, at this temperature has also been obtained in x = 0.05Rb sample (8.5 108 Oe A/cm2).

In this work, the effect of cesium substitution on the structural and superconducting properties of Bi2Sr2Ca1-xCsxCu2O+y (where x = 0.0, 0.025, 0.050, 0.075, 0.10, 0.125, and 0.15) samples prepared by the solid-state reaction route and by directional growth using the laser floating zone (LFZ) technique has been studied. X-ray diffraction has shown that both kinds of samples present the 2212 phase as the major one, with pseudo tetragonal structure, accompanied by minor amounts of (Sr,Ca)CuO2 and Bi2(Sr,Ca)4O6 + d secondary phases. SEM-EDX results indicate that Bi-2212 phase is the major one, with small amounts of secondary phases. Cs substitution diminishes the size and content of secondary phases. Magnetization measurements reflect different behavior for sintered and LFZ grown samples. Although the diamagnetic transition is very wide for the former samples, it is very sharp for the last ones. The critical onset temperatures, TC, are around 80 and 90 K, for sintered and LFZ grown samples, respectively. Moreover, M–H measurements indicate that LFZ grown samples exhibit larger areas than the sintered ones. The intragranular critical current, JC, calculated from the M–H loops and Bean’s formula, is maximum for x = 0.025Cs sintered and x = 0.05Cs LFZ grown samples. The maximum pinning forces, FPmax, determined in these samples are higher than those obtained in undoped ones, reaching 9 × 107 Oe*A/cm2 and 10 × 108 Oe*A/cm2, at 10 K, for sintered and LFZ grown samples, respectively.

- Abstract
In this research, the effect of carbon nanotube (CNT) addition on the structural and superconducting properties of Bi2Sr2CaCu2Oy + x wt% CNT materials with x = 0, 0.5, 1, and 3 prepared by solid state method, followed by directional grown using the laser floating zone (LFZ) technique has been investigated. XRD measurements showed that all samples are composed of almost single Bi-2212 phase. SEM images proved that Bi-2212 phase is the major one, with minor amounts of (Sr,Ca)3BiOx, and Bi-2201 secondary phases. According to the magnetic results, carbon nanotubes decrease Tc values and significantly reduce the magnetic hysteresis loops area and the magnetic critical current density.

19. Clinicopathological evaluation of clear cell hidradenoma (Acrospiroma) within multiple tumor complex in a dog

Background: Clear cell hidradenoma (acrospiroma) is adnexal tumors that arise from the distal excretory duct of eccrine sweat glands. It is generally defined in humanbeings. It presents solitary structure in firmness nodule. Most frequently are encountered in head, face, and upper extremities in humans. Hidradenomas are called generally by definition benign. Their malignant transformations and metastasis are seen very rarely. In veterinary literature database, the malignant form of tumor has been defined as an unique report in four dogs. They are extremely rare tumors in dogs. It is not documented within multiple tumor complex up to now. Case: In the case, three tumors were detected in a 9-year-old neutered female German shepherd dog. After clinical examination, a complete surgical resection of all masses was performed and masses were sent to Pathology for diagnosis. A mass on the right last rib was a 8x6 cm in diameters with severe ulceration. The mass had a hard texture, immobility and irregular borders. Other masses were localized on the back and left caudo-abdominal mammary lobe. Tumors on the back and the mammary lobe did not invade muscle layer, but the tumor on the last rib aggressively invaded surrounding tissue and were very difficult to remove surgically After macroscopy, all the masses were stained with hematoxylin-eosin (H&E), Mayer's mucicarmine and Periodic Acid Shiff (PAS) stainings. In the first mass, there were multilobular epithelial islands between prominent fibrous septa from the upper to deep of dermis. The neoplastic cells were generally round or polyhedral in shape. Some of cytoplasms were eosinophilic at different degree. But cytoplasms were generally finely granular and vesicular or clear in appearance. Some cells had possible glycogen deposits. Nucleus was oval to round and had fine reticular chromatin and a distinct nucleolus. The tumor was diagnosed as clear cell hidradenoma. Others were was belonged to hemangioma on the back and malignant mixed tumor in the mammary gland. Discussion: In our case, two cell types having benign characteristics were also noted. Some clear cells contained PAS (+) material showing glycogen deposits. Additionally, there were some fibrous septa separating those cell islands. Thus, it is thought that the histopathology shows parallelism to many reports described in human beigns. Clear cell hidradenoma described in this case is first documentation as benign counterpart on the basis of veterinary literatures even though a previous report including clear cell hidradenocarcinoma in four dogs. And also, it has been reported to be very rarely seen tumor in human beings. However, any hormonies in terms of its predilection site can not be found among the cases with clear cell hidradenoma in human counterparts, because the tumor is encountered at skin of last right rib. This situation has shown us the tumor does not select any predilection site as described in this case. It has been understood on the basis of literatures that this is the first case report of describing benign tumor of ecrine sweat duct in dogs. Also, there has been no any documentation regarding in this multiple tumor complex. © 2018 Universidade Federal do Rio Grande do Sul. All rights reserved. Background: Mooren's ulcer is a chronic and painful ulceration of the cornea. It begins progressively in the periphery and spread centrally in cornea. In human, it is seen uniaterally in most of cases. Mooren ulcer has not been reported in any kind of animals up to now. Although its aetiology is not completely enlighted, it has been suspected of the inflammatory reaction against injuries-microbiological and immun mediated effects. Immun response in presence of accumulation of immune complexes into the limbal vessels.As a result of the deficit in the regulatory mechanism because the number of suppressor cells control over B and T lymphocytes, These situations can result in a progressive tendancy to inflammation because the production of autoantibodies and/or lymphokine from cytotoxic T-lymhocytes creates an immune-mediated vasculitis. Numerous immigrant inflammatory cells and proteins are evaded from vessels. After triggering inflammatory cells and releasing of meditors, corneal vascularization, scar tissue and re-epithelization develop. This regenerative-reperative process plays an important role during post-inflammatory process. Case: In this case, it was aimed to detect pathomorphological structure and immunologic relations in progressive Mooren's ulcer (MU). A 1 year-old mix breed cat was submitted to clinic with complaints of progressive painful and eyesight loss in left eye. There were 1 cm-ulceration, opacification and old haemorrhagic areas at peripheral cornea. Histopathologically, there was wide ulceration including all layers of corneal epithelium and particularly vacuolar degeneration at suprabasal cells. In corneal stroma, numerous neutrophiles and mononuclear cells were infiltrated. Neovascularisation and fibrosis beginning from limbus were also present. This fibrotic progress was confirmed by Masson's trichrome staining method. Immunohistochemically, Cytokeratin 3 (CK3) and cytokeratin 12 (CK12) positivities showing regenerative activity of suprabasal and basal cells were not widespread. Epidermal Growth Factor (EGF) positivities were generally weak in epithelial cells. In stroma, moderate vimentin positivities were detected proliferated in fibrocytes originating from limbus. ?1-antichymotrypsin (A1AC) was mildly reacted in neutrophiles. CD3 and CD4 confirmed the presence of regulatory and helper T lymphocytes. CD3 and CD8 marked cytotoxic T lymphocytes and CD20 marked B lymphocytes in inflammatory areas. CD34 were also positive in peripheral corneal stem cells derived from limbal basal epitheliums in partly regenerated area. CD57 positivity in T lymphocytes and NK cells and CD68 positivity in macrophages were attended to the area. Discussion: CD1a positivity in T lymphocytes proved mediating lipid and microbial origin glycolipid antigens. TUNEL reactions showing DNA in situ fragmentation were present in the destructive and aging epithelial cells at periphery. In conclusion, the case has been found as unique in terms of its immunohistochemical characterization. The markers show that CD1a and CD68 expressions follow different progress in animals unlike in humanbeings even though the ulcer of pathogenetic mechanism is found identical to humanbeings.. The roles of CD20 and CD57 markers have potential roles in this ulcer. It is also concluded that insufficient epithelial regeneration, fibrosis, inflammation and apoptosis showed progressive Mooren's ulcers having possibly microbial origin. © 2018 Universidade Federal do Rio Grande do Sul. Chartered Inst. of Building Services Engineers, The aim of this study was to evaluate coxofemoral joint luxation in cats clinically and radiographically, to treat with double-sided modified toggle pin technique and to share the early-term outcomes. The study material consisted cases of 17 cats which were brought with complaints of hind limb lameness and diagnosed with coxofemoral luxation by radiography and orthopaedic examination. Radiograms revealed that the luxations of all cases either at right or left sides were formed craniodorsally. Fractures of femur (n=2), sacroiliac luxation (n=1), and os pubis and os ischii fractures (n=1) accompanied coxofemoral luxation in some cats. Toggle pins of different sizes were prepared using Kirschner wires and they were sterilized a day before surgery. Approaching the hip joint craniolaterally, luxated coxofemoral joint was allowed to set to its anatomic position, using an apparatus prepared with toggle pin and nylon sutures (USP: 0). Double-sided modified toggle pin technique was performed for the treatment. Cats, which only had luxations were able to use their legs without any problems one day after surgery; although, this time was prolonged in cases having additional orthopaedic problems. However, postoperative clinical and radiographical findings were satisfactory in all cases. As a result, double-sided modified toggle pin technique was concluded to be an appropriate method for surgical treatment of coxofemoral luxations in cats, with its satisfactory joint stabilization, quick gain of extremity functions and its compatibility with body tissues. © 2016, Chartered Inst. of Building Services Engineers. Osteoid osteoma and osteoblastoma are uncommon benign bone tumors which originate from osteoblasts in animals. Both tumors have similar characteristics. Osteoid osteoma is well described in humans, but there known to be only two cases in domestic animal species. Differentiating the diagnosis of osteoid osteoma from osteoblastoma by using immunohistochemistry was aimed in this study. A 12-year-old mix breed female dog was brought to the clinic with complaints of swelling in proximal forelimb and progressive loss of motion. Animal was died during surgery. In necropsy, tissue samples were taken from the mass and fixed in %10 formalin. Samples were stained with hematoxylin and eosin, Masson’s trichrome and Alizarin Red S. For revealing of cell differentiation, ABC-P method was applicated to adhesive section by using BMP6, S100, vimentin ve p53 markers. Macroscopically, proximal humerus was swollen. Cut section was generally lytic within bone residues. Soft tissue was haemorrhagic and edematouse. Microscopically, osteoblasts and hypervascular nidus were surrounded by large sclerotic and mineralized bone tissue. Large lytic areas were also observed. Masson’s trichrome revealed sclerosis and Alizarin Red S revealed osteoid matrix. Immunohistochemically, BMP6 was moderately reacted in osteoid matrix and S100 was moderately reacted with nerve fibers in nidus. Vimentin had strictly positive reaction in fibrocytes. P53 was negative in osteoblasts. In conclusion, this case is the third reported in domestic animals and the second in dogs. Also osteoid osteoma is evaluated for the first time by immunohistochemistry in animals.

PubMed ID: 26419526, Background: In Turkey, large regional inequalities were found in maternal and child health. Yet, evidence on regional inequalities in adult health in Turkey remains fragmentary. This study aims to assess regional and rural/urban inequalities in the prevalence of poor self-rated health and in disability among adult populations in Turkey, and to measure the contribution of education and wealth of individual residents. The central hypothesis was that geographical inequalities in adult health exist even when the effect of education and wealth were taken into account. Methods: We analyzed data of the 2002 World Health Survey for Turkey on 10791 adults aged 20 years and over. We measured respondents' rating of their own general health and the prevalence of five types of physical disability. Logistic regression was used to estimate how much these two health outcomes varied according to urban/rural place of residence, region, education level and household wealth. We stratified the analyses by gender and age (50 and 50 years). Results: Both health outcomes were strongly associated with educational level (especially for older age group) and with household wealth (especially for younger age group). Both health outcomes also varied according to region and rural/urban place of residence. Higher prevalence rates were observed in the East region (compared to West) with odd ratios varying between 1.40-2.76. After controlling for education and wealth, urban/rural differences in health disappeared, while regional differences were observed only among older women. The prevalence of poor self-rated health was higher for older women in the Middle (OR?=?1.69), Black Sea (OR?=?1.53) and East (OR?=?2.06) regions. Conclusion: In Turkey, substantial geographical inequalities in self-reported adult health do exist, but can mostly be explained by differences in socioeconomic characteristics of residents. The regional disadvantage of older women in the East, Middle and Black Sea may have resulted from life-long exposure to gender discrimination under a patriarchal ideology. Yet, not geographic inequalities, but the more fundamental socioeconomic inequalities, are of key public health concern, also in Turkey. © 2015 Ergin and Kunst.

Pseudochylotorax is usually associated with chronic inflammatory disorders like tuberculosis or rheumatoid arthritis [RA]. Pseudochylotorax due to RA is much more rare and there were only 21 cases in the English literature until 2009. Long standing pleural effusion [over 5 years] could result empyema and pseudochylothorax but our patients had a history of 2 years and 3 years. At this paper we report two cases of RA with pseudochylotorax and empyema for the supplement of the literature and to emphasize time is not a criterion for the diagnosis of pseudochylotorax.

Aim The aim of this study was to investigate the medical and forensic importance of thorax computed tomography in stable patients with blunt chest trauma. Material and Methods Fifty patients with blunt chest injury were retrospectively evaluated with chest radiography and thorax computed tomography in the first 24 hours after trauma. Patient demographics, thoracic lesions, management options, and forensic assessment were rewieved. Results The most common lesion of the study was rib fracture. Thorax computed tomography was significantly superior to chest radiography in detecting thoracic lesions. Sixty eight (33%) occult lesions were detected. Forty six (18%) of these were life threatening that not detected with plain chest radiography. Associated injuries were present in 33 (66%) patients. Pelvic and extremity fractures were the most common injury. Thorax computed tomography was significantly superior to chest radiography in detecting pneumothorax, hemothorax and lung contusion. Eightyone life threatening lesions were detected and 50 (61%; pneumothorax 13, hemothorax 24, lung contusion 9, and pneumomediastinum 4) of these lesions could not be detected with plain chest radiography. The clinical management [in 15 patients (30%)], and the forensic assesment was changed [in 14 (28%)] patients were changed. Conclusion We concluded that using Computed Tomography of the thorax in thoracic travmas prive meticulous assesment in management of patients and forensic issues.

This article reviews the surgical and other invasive procedures in the management of pleural effusions. Pleural effusion is frequent in clinical practice. Pleural procedures vary from aspiration to more challenging procedures such as thoracoscopy. However it is difficult to make diagnosis by thoracentesis or closed pleural biopsy. Thoracoscopy is gold standart in the investigation and management of pleural disease. Pleural biopsy by thoracoscopy should be actively carried out in patients with pleurisy, because the technique has a high diagnostic rate and can be easily and safely performed. Flexible or rigid thoracoscope, bronchoscope, endoscope and mediastinoscope can be used for operation.

A gamma spectrometric method is described for the direct “absolute” determination of uranium contents in solid samples. To obtain the exact information about the measurement of 238U in samples by gamma-ray spectroscopy, it is essential that any one of the daughters of 238U should exist in radioactive equilibrium with 238U. This condition is mainly fulfilled by one of the first two daughters, namely, 234Th (24.1 d) and 234mPa (1.17 min) because these daughters are very short-lived compared with 238U. Therefore, the present analytical method employs direct determination of U via the 63.3 keV gamma emission from Th and via the 1001 keV gamma emission from 234mPa. The absolute photopeak efficiency data in the range of 40 keV to 2000 keV for the reverseelectrode Germanium(REGe) detector used is measured using a powder radioactive standard source. The certified uranium samples were prepared in thin thicknesses to minimize the selfabsorption of gamma rays. They are counted for the the time predetermined between 75,000 s and 125,000 s. The measured activities of 238U of the samples via the 63.3 keV (234Th ) and via the 1001 keV (234mPa) are corrected for self-absorption effects due to changes in composition of standard reference materials in a given sample-detector geometry. The self-absorption factors applied to the 46.5 keV(210Pb) peak and the 63.3 keV(234Th) peak results ranged from 10.5% to 33.1% and from 4.9% to 18.4%, respectively. For the 1001 keV(234mPa) gamma emission, the necessary self-absorption factors varied between 0.1% and 3.9%. The results obtained from the 46.5 keV of Pb being far distant member of U and those of the 63.3 keV from 234Th are used to check whether the equilibrium exist in the uranium samples. The results of U via the 63.3 keV peak of Th for the uranium standards agreed to within 0.1 to 6.9% with the certified values of 238U in the samples with uranium concentration ranging from 0.022±001 to 1.02±0.01 wt%. The results of 238U via the 1001 keV peak of 234mPa agreed well to within 0.01 to 4.7% with the certified activity values of 238U of the same samples. When the measurements of the uranium are carried out in a REGe detector system, the results indicate that the uranium concentrations in the samples can be determined to the average 7% error by using the 63.3 keV(234Th) gamma emission with appropriate corrections for self-absorption effects. The 1001 keV peak of 234mPa gives also accurate results for the determination of 238U in the same samples and the need for self absorption correction for the 1001 keV emission seems to be negligible, except in case of thicker samples than that of 1.5 cm thick.

The repair of eyelid full-thickness tissue defects could be solved rather simply by flaps derived from the frontal or temporal region, or rotated from the opposite lid or by free full-thickness skin grafts of eyelid. In this study, full thickness tissue loss of lower eyelid was described in two cats. In both cats, after en bloc resection of lesion, the full-thickness defect was reconstructed with a transposition flap from the temporal region. Histopathological examinations revealed squamous cell carcinoma. Transposition flap technique was found useful for reconstruction of wide full thickness eyelid defects resulting from squamous cell carcinoma excision. Diagnosis of vaginal prolapse and retroflexion of the urinary bladder were carried out. Foetal stress was detected with the help of ultrasonography. The article discusses vaginal prolapse concurrent with cystocele in bitches.

- Abstract
The article describes the case of a 5 year old, male, tabby cat presented with complaints of loss of appetite, fatigue and weight loss that was previously with antibiotics for 1 week. Surgery was decided following clinical, radiographical and ultrasonographical examinations of the cat and a vetral midline laparotomy was performed. It suggests that bilobed gallbladder can be diagnosed by combining various imaging techniques in cats and extrahepatic biliary obstruction can be treated surgically.

40. Effects of social determinants on food choice and skipping meals among Turkish adolescents.

Purpose: To present data that contributes to understanding factors that influence food choice and skipping meals in adolescents.Methods: A cross sectional study is carried in selected high schools in Bornova. Study sample compromises of 527 students chosen randomly by class from a population of 2410 first year in high school students. Self-administered questionnaires containing sociodemographic determinants, self reported weight and height, food choices and meal patterns were used.Findings: A psychosocial factor that affects almost all of the students is the "taste and sensory perception of food". The second noticable factor is the "health and nutritious value of food". The time conserved and the convenience in the preparation of food is one of the lifestyle factors that affect more than half of the students. The cost of the food was also found to have an effect. Among the third group of factors categorized as "media", the leading factor is advertisement, effective in one third of the students. Among boys and girls, there was no statistical difference in the type of meal skipped. Living in Izmir for more than 10 years compared to less than ten years, being in a nuclear family to extended family, and belonging to the "owner" social class to "wage laborer" class also do not statistically differ with regard to skipping meals. However, the mother's and father's education level and having a working mother are associated with skipping meals.Conclusions: These results provide important evidence to support opportunities to positively influence the adoption of healthful eating. Background: Thesis is an important part of specialisation and doctorate education and requires intense work. The aim of this study was to investigate the publication rates of Turkish Public Health Doctorate Theses (PHDT) and Public Health Specialization (PHST) theses in international and Turkish national peer-review journals and to analyze the distribution of research areas. Methods: List of all theses upto 30 September 2009 were retrieved from theses database of the Council of Higher Education of the Republic of Turkey. The publication rates of these theses were found by searching PubMed, Science Citation Index-Expanded, Turkish Academic Network and Information Center (ULAKBIM) Turkish Medical Database, and Turkish Medline databases for the names of thesis author and mentor. The theses which were published in journals indexed either in PubMed or SCI-E were considered as international publications. Results: Our search yielded a total of 538 theses (243 PHDT, 295 PHST). It was found that the overall publication rate in Turkish national journals was 18%. The overall publication rate in international journals was 11.9%. Overall the most common research area was occupational health. Conclusion: Publication rates of Turkish PHDT and PHST are low. A better understanding of factors affecting this publication rate is important for public health issues where national data is vital for better intervention programs and develop better public health policies. We present the case of an asymptomatic 36-year-old female with a solid, round, mass 4.0 cm in diameter in the lower lobe of right lung. The urinary bladder was replaced in the normal anatomic position and cystopexy was performed via a midline laparotomy in each dog, and an ovariohysterectomy (OHE) was performed to prevent recurrence of vaginal prolapse. Surgical amputation of the prolapsed vagina was performed after the cystopexy and OHE. This case report demonstrates the possibility of severe vaginal prolapse concurrent with cystocele in bitches.

- Abstract
Aims: We aimed to determine the use of sodium-glucose cotransporter 2 inhibitors (SGLT2is) and to identify clinical factors associated with their use in patients with heart failure (HF) in a real-life setting., Methods: Real-world data on Empagliflozin and Dapagliflozin use in patients with HEART failure: The RED-HEART study is a multicentre, cross-sectional and observational study that included HF patients in the outpatient setting regardless of ejection fraction from 19 cardiology centres between August 2023 and December 2023., Results: The study population consisted of 1923 patients with HF, predominantly men (61.2%), with a median age of 66 (range: 19-101) years. Overall, 925 patients (48.1%) were receiving SGLT2is. Among the study population, 22.1% had HF with preserved ejection fraction, 21.5% had HF with mildly reduced ejection fraction, 56.4% had HF with reduced ejection fraction and the use of SGLT2is was 42.0%, 47.9% and 50.6% in each group, respectively (P = 0.012). The use of SGLT2is was 76.6% in patients with HF and diabetes, 19.8% in patients with HF and chronic kidney disease and 26.8% in patients without diabetes and chronic kidney disease (P < 0.001). Higher education level [odds ratio (OR): 1.80; 95% confidence interval (CI): 1.06-3.05; P = 0.027], higher household income (OR: 3.46; 95% CI: 1.27-9.42; P = 0.015), New York Heart Association functional class IV (OR: 2.72; 95% CI: 1.16-6.35; P = 0.021), diabetes (OR: 9.42; 95% CI: 6.72-13.20; P < 0.001), the use of angiotensin receptor-neprilysin inhibitors (ARNis) (OR: 4.09; 95% CI: 2.39-7.01; P < 0.001), the use of mineralocorticoid receptor antagonists (MRAs) (OR: 2.02; 95% CI: 1.49-2.75; P < 0.001), the use of loop diuretics (OR: 1.62; 95% CI: 1.18-2.22; P = 0.003) and the use of thiazide diuretics (OR: 1.72; 95% CI: 1.30-2.29; P < 0.001) were independently associated with the use of SGLT2is. Conversely, atrial fibrillation (OR: 0.63; 95% CI: 0.45-0.88; P = 0.008), chronic kidney disease (OR: 0.53; 95% CI: 0.37-0.76; P = 0.001), the use of dihydropyridine calcium channel blockers (OR: 0.68; 95% CI: 0.48-0.98; P = 0.042) and the use of statins (OR: 0.67; 95% CI: 0.49-0.91; P = 0.010) were independently associated with the non-use of SGLT2is., Conclusions: The RED-HEART study provided comprehensive real-world data about implementing SGLT2is in patients with HF. - Abstract
Objective: The main objective of this study is to determine the incidence and predictors of clinical outcomes in patients with AF treated with factor Xa inhibitors in a real-world setting., Methods: The present study was a multicentre and observational study that included patients with AF who were treated with factor Xa inhibitors. The primary outcome was the composite of ischemic stroke, TIA, systemic embolism, major bleeding, and all-cause mortality., Results: A total of 1162 patients from 26 cardiology centers were included in this study, with a median age of 72 years. During the median 12-month follow-up period, the primary outcome occurred in 195 patients (16.8%). Treatment with rivaroxaban compared with apixaban and edoxaban showed a lower rate of ischemic stroke, TIA, and/or systemic embolism (2.2% vs. 4.7% vs. 6.5%, respectively, p = 0.014). The major bleeding rate was similar between all three factor Xa inhibitors. The all-cause mortality rate in the rivaroxaban group was lower compared with the apixaban and edoxaban groups (9.8% vs. 15.1% vs. 12.4%, respectively, p = 0.042). Overall, the frequency of primary outcome was 13.8%, 19.6%, and 20.6% for patients treated with rivaroxaban, apixaban, and edoxaban, respectively (p = 0.019). Older age, male sex, low body weight, high bleeding risk, heart failure, hypertension, liver failure, and treatment with apixaban 2.5 mg b.i.d. were independently associated with the development of primary outcome., Conclusion: The follow-up data from the ANATOLIA-AF study provides detailed data about the incidence and independent predictors of adverse clinical outcomes in patients with AF treated with factor Xa inhibitor treatment., (© 2025 The Author(s). - Abstract
While previous reviews found a positive association between pre-existing cancer diagnosis and COVID-19-related death, most early studies did not distinguish long-term cancer survivors from those recently diagnosed/treated, nor adjust for important confounders including age. We aimed to consolidate higher-quality evidence on risk of COVID-19-related death for people with recent/active cancer (compared to people without) in the pre-COVID-19-vaccination period. We searched the WHO COVID-19 Global Research Database (20 December 2021), and Medline and Embase (10 May 2023). We included studies adjusting for age and sex, and providing details of cancer status. Risk-of-bias assessment was based on the Newcastle-Ottawa Scale. Pooled adjusted odds or risk ratios (aORs, aRRs) or hazard ratios (aHRs) and 95% confidence intervals (95% CIs) were calculated using generic inverse-variance random-effects models. Random-effects meta-regressions were used to assess associations between effect estimates and time since cancer diagnosis/treatment. Of 23 773 unique title/abstract records, 39 studies were eligible for inclusion (2 low, 17 moderate, 20 high risk of bias). Risk of COVID-19-related death was higher for people with active or recently diagnosed/treated cancer (general population: aOR = 1.48, 95% CI: 1.36-1.61, I
2 = 0; people with COVID-19: aOR = 1.58, 95% CI: 1.41-1.77, I2 = 0.58; inpatients with COVID-19: aOR = 1.66, 95% CI: 1.34-2.06, I2 = 0.98). Risks were more elevated for lung (general population: aOR = 3.4, 95% CI: 2.4-4.7) and hematological cancers (general population: aOR = 2.13, 95% CI: 1.68-2.68, I2 = 0.43), and for metastatic cancers. Meta-regression suggested risk of COVID-19-related death decreased with time since diagnosis/treatment, for example, for any/solid cancers, fitted aOR = 1.55 (95% CI: 1.37-1.75) at 1 year and aOR = 0.98 (95% CI: 0.80-1.20) at 5 years post-cancer diagnosis/treatment. In conclusion, before COVID-19-vaccination, risk of COVID-19-related death was higher for people with recent cancer, with risk depending on cancer type and time since diagnosis/treatment., (© 2023 The Authors. - Abstract
Aims: The use of guideline-directed medical therapy (GDMT) among patients with heart failure (HF) with reduced ejection fraction (HFrEF) remains suboptimal. The SMYRNA study aims to identify the clinical factors for the non-use of GDMT and to determine the prognostic significance of GDMT in patients with HFrEF in a real-life setting., Methods and Results: The SMYRNA study is a prospective, multicentre, and observational study that included outpatients with HFrEF. Patients were divided into three groups according to the status of GDMT at the time of enrolment: (i) patients receiving all classes of HF medications including renin-angiotensin system (RAS) inhibitors, beta-blockers, and mineralocorticoid receptor antagonists (MRAs); (ii) patients receiving any two classes of HF medications (RAS inhibitors and beta-blockers, or RAS inhibitors and MRAs, or beta-blockers and MRAs); and (iii) either patients receiving class of HF medications (only one therapy) or patients not receiving any class of HF medications. The primary outcome was a composite of hospitalization for HF or cardiovascular death. The study population consisted of 1062 patients with HFrEF, predominantly men (69.1%), with a median age of 68 (range: 20-96) years. RAS inhibitors, beta-blockers, and MRAs were prescribed in 76.0%, 89.4%, and 55.1% of the patients, respectively. The proportions of patients receiving target doses of guideline-directed medications were 24.4% for RAS inhibitors, 11.0% for beta-blockers, and 11.1% for MRAs. Overall, 491 patients (46.2%) were treated with triple therapy, 353 patients (33.2%) were treated with any two classes of HF medications, and 218 patients (20.6%) were receiving only one class of HF medication or not receiving any HF medication. Patient-related factors comprising older age, New York Heart Association functional class, rural living, presence of hypertension, and history of myocardial infarction were independently associated with the use or non-use of GDMT. During the median 24-month period, the primary composite endpoint occurred in 362 patients (34.1%), and 177 of 1062 (16.7%) patients died. Patients treated with two or three classes of HF medications had a decreased risk of hospitalization for HF or cardiovascular death compared with those patients receiving ≤1 class of HF medication [hazard ratio (HR): 0.65; 95% confidence interval (CI): 0.49-0.85; P = 0.002, and HR: 0.61; 95% CI: 0.47-0.79; P < 0.001, respectively]., Conclusions: The real-life SMYRNA study provided comprehensive data about the clinical factors associated with the non-use of GDMT and showed that suboptimal GDMT is associated with an increased risk of hospitalization for HF or cardiovascular death in patients with HFrEF., (© 2023 The Authors. - Abstract
Aim: This study aims to determine health-related quality of life (QoL) and the related factors from the perspective of social determinants of health among children., Background: Childhood is the most intense period of life, and environmental factors surrounding children, as well as individual lifestyle factors, are related to the child's physical and mental well-being. To our knowledge, there is a lack of studies evaluating the relationship between determinants of health and the QoL of healthy children in general., Methods: This cross-sectional study was executed in the Bayrakli district of Izmir city. Stratified clustered sampling was used including 24 schools and 3367 7th-grade children, and 1284 students were targeted (50% prevalence, 95% CI, %5 margins of error, 2.25 design effect, and 20% replacement). The response rate was 84.9% ( n = 1090). The Turkish KID-KINDL Health-Related Quality of Life Questionnaire for Children was used to assess QoL. Independent variables were examined in four layers using Dahlgren's Determinants of Health Model: basic characteristics, lifestyle factors, family characteristics, and life conditions., Results: The mean QoL score was 71.3 ± 12.6. Our study explained 31.7% of the variance in QoL. Higher QoL scores were associated with better health status, perceived academic achievement, normal/thin body perception, physical activity (PA), and adequate sleep duration. Living with both parents and having fewer siblings positively influenced QoL. Moreover, the presence of structural problems in the household and poorer health perceptions were associated with lower QoL scores ( P < 0.05) This study highlighted the multifaceted nature of QoL in Turkish children, revealing the importance of various determinants of health. The results show that in order to improve the general well-being of this population, interventions and policies are required that concentrate on elements including health status, academic accomplishment, body perception, physical activity, family structure, and living situations.

- Abstract
Tumor-draining lymph nodes (TDLNs) are important for tumor antigen-specific T cell generation and effective anticancer immune responses. However, TDLNs are often the primary site of metastasis, causing immune suppression and worse outcomes. Through cross-species single-cell RNA-Seq analysis, we identified features defining cancer cell heterogeneity, plasticity, and immune evasion during breast cancer progression and lymph node metastasis (LNM). A subset of cancer cells in the lymph nodes exhibited elevated MHC class II (MHC-II) gene expression in both mice and humans. MHC-II+ cancer cells lacked costimulatory molecule expression, leading to regulatory T cell (Treg) expansion and fewer CD4+ effector T cells in TDLNs. Genetic knockout of MHC-II reduced LNM and Treg expansion, while overexpression of the MHC-II transactivator, Ciita, worsened LNM and caused excessive Treg expansion.
