Aim: The aim of this study was to determine whether there were abnormalities in thyroid hormone levels and anti-thyroid peroxidaseantibodies in pregnant women with and without gestational diabetes mellitus.Material and Methods: We analyzed the medical records of 107 pregnant women. Clinical data related to maternal age, gestationalweek, body mass index, thyroid function tests, and anti-thyroid peroxidase antibodies, fasting blood glucose tests and HbA1c levels wereobtained. Aim: In this study, we aimed define a new technique to visualize the aortic arch in order to diagnose aortic arch anomalies with a dorsal approach when fetal spine is in anterior position within the uterus. Materials and Methods: 115 patients who were referred to the prenatal screening unit between gestational weeks 20-40 were admitted to the study. 110 patients with normal cardiac findings and 5 fetuses with postnatally confirmed diagnosis of coarctation of aorta were evaluated with the new technique. Results: Aortic arch of 115 fetuses were successfully visualized with the technique defined in detail. Conclusion: The dorsal approach for the visualization of the aortic arch seems as a durable and a convenient technique for the fetal cardiac evaluation, especially in terms of better diagnostic capability for aortic arch pathologies under circumstances where the ventral approach is not feasible., Amaç: Bu çalışmada, fetus dorso-anterior pozisyondayken aort kemerinin görüntülenmesi ve anomalilerinin tespiti için yeni bir teknik tanımlamayı amaçladık. Gereç ve Yöntem: 20. ile 40. gebelik haftalarında prenatal değerlendirme birimine yönlendirilen 115 gebe çalışmaya dahil edildi. Normal bulgulara sahip 115 fetus ile doğum sonrası konfirme edilmiş beş aort koarktasyonlu fetus tanımlanan yeni teknik ile değerlendirildi. Bulgular: 115 dorso anterior duruşlu fetusta aort kemeri tanımlanan yeni teknikle başarıyla gösterilmiştir. Sonuç: Aort kemerinin izlenmesinde dorsal yaklaşım, özellikle ventral yaklaşımın mümkün olmadığı durumlarda aort kemerinin patolojileri için daha iyi tanı kabiliyeti ile uygun bir teknik olarak kullanılabilir.

İniensefali, boynun aşırı hiperekstansiyonu ve çeşitli seviyelerde vertebral defektlerle birlikte görülen nadir ve letal bir anomalidir. Nadiren doğan bebekler ise kısa sürede kaybedilirler. Literatürde çok az sayıda uzun dönem yaşayan iniensefali hastası olgu sunumları şeklinde yer almaktadır. Fetal seyirden dolayı ilk trimesterde tanı alan olgular için gebelik terminasyonu önerilmelidir. Bu yazıda takipleri olmayan ve geç tanı alan bir iniensefali olgusu sunulmaktadır., Iniencephaly is a rare, lethal anomaly with spinal retroflexion and multiple defects. Fetuses with iniencephaly, birth uncommonly and die in a short time. In literature, there are few case reports about long live iniencephalic infants. Because of lethal course, termination should be offered. In this report, out of follow-up and late diagnosis case is presented.

Amaç: Uterin septum kadınların büyük bir kısmını, özellikle tekrarlayan düşüğü olan hastaları etkilemektedir. Histeroskopik septoplasti tedavi için yararlı ve minimal invaziv bir tekniktir. Histeroskopi sonrası gebelik oranları değişik çalışmalarda değişkenlik göstermektedir. Çalışmamızda histeroskopik septoplasti sonrası gebelik sonuçları ve farklı histeroskopik tekniklerin yararlı etkilerini karşılaştırmayı amaçladık.Gereç ve Yöntem: Ege Üniversitesinde 2007-2013 tarihleri arasında histeroskopik septoplasti gerçekleştirilen hastalar retrospektif olarak tarandı. Hastaların gebelik sonuçları üniversite bilgi sistemi ve telefon görüşmeleri ile takip edildi. Canlı doğum oranları ve düşükler değerlendirilmeye alındı. Farklı cerrahi teknikler (rezektoskop, soğuk makas) canlı doğum oranları baz alınarak karşılaştırıldı.Bulgular: Yüz yirmi iki hasta çalışmaya dahil edildi. Doksan dört gebelik %77 oran ile gerçekleşti. Yetmiş dokuz canlı doğum ve 15 düşük saptandı. Altmış dört (%54) kadın en az bir canlı doğum gerçekleştirmişti. Histeroskopik septoplasti; 72 (%59) hastada rezektoskop, 50 (%41) hastada 5 mm histeroskop ve soğuk makas kullanılarak gerçekleştirildi. Rezektoskop ile işlem gören hastalardan 42’sinde (%58) ve soğuk makas ile gerçekleştirilen hastalardan 24’ünde (%48) canlı doğum gerçekleşti. Cerrahi tekniklere göre canlı doğum oranları karşılaştırıldığında istatistiksel anlamlı fark saptanmadı.Sonuç: Histeroskopik septoplasti; uterin septum tedavisinde güvenli, yararlı ve minimal invaziv bir yöntemdir. Rezektoskop ve soğuk makas kullanımına göre canlı doğum oranları arasında istatistiksel anlamlı fark saptanmamıştır. Uterin septuma sahip infertil hastalara histeroskopi seçeneği düşük komplikasyon oranları ve yüksek gebelik sonuçları ile önerilebilir., Aim: Uterine septum affects a large proportion of women, especially patients with recurrent abortion. Hysteroscopic septoplasty is a novel and minimally invasive method for its treatment. Pregnancy rates after hysteroscopy vary in different studies. We aimed to evaluate pregnancy outcomes after hysteroscopic septoplasty and to compare the beneficial effects of different hysteroscopic techniques.Materials and Methods: Patients undergoing hysteroscopic septoplasty between 2007 and 2013 in Ege University were retrospectively reviewed. Patients’ pregnancy outcomes were followed-up by using the university’s registry system and telephone interviews. Live birth rates and miscarriages were determined. Different surgical techniques (resectoscope, cold scissors) were compared regarding live birth ratios.Results: One hundred twenty two patients were included in this study. Ninety-four pregnancies occurred with an overall pregnancy rate of 77%. Seventy nine live births and 15 abortions had occurred. Sixty-six women (54%) gave at least one live birth. Hysteroscopic septoplasty was performed in 72 (59%) and 50 (41%) patients using resectoscope and cold scissors with 5mm hysteroscope, respectively. Fourty two (58%) of patients undergoing septoplasty by resectoscope and 24 patients (48%) of patients undergoing septoplasty by cold scissors group had a live birth. There was no statistically significant difference among the pregnancy outcomes of the two surgical techniques. Conclusion: Hysteroscopic septoplasty is a safe, effective and minimally invasive method for uterine septum treatment. There is no significant difference in the live birth rates according to whether the resectoscope or cold scissors were used. Infertile patients with uterine septum should be offered the option of hysteroscopy which is feasible with low complication rates and high pregnancy rates.

Amaç: Gebeliğinde apendektomi yapılan hastaların gebelik sonuçlarının değerlendirilmesi.Gereç ve Yöntemler: 2010-2014 yılları arasında apendektomi yapılan hastalar retrospektif olarak incelendi. Tüm hastaların gebelik sonuçları üniversite kayıt sistemi ve telefon görüşmeleri ile takip edildi. Hastalar yaş, gebelik yaşı, klinik ve laboratuvar muayeneleri, görüntüleme çalışmaları, acil servis ve operasyon arasındaki ortalama zaman, ortalama operasyon süresi, gebelik sonucu ve apandiksin patoloji sonucuna göre değerlendirildi. Bulgular: Otuz dokuz hasta çalışmaya dahil edildi. Otuz dokuz hastanın 16’sı ilk, 15’i ikinci ve 8’i gebeliğin üçüncü trimesterindeydi. Üç hastaya laparoskopik apendektomi kalanlarına laparatomi yapıldı. Apendiksin patolojik değerlendirilmesinde, yedi hasta (17%) normal apendiks, dört hasta perfore apendiks, bir hasta nöro-endokrin tümor ve kalanları apandisit saptandı. Operasyon sonrası iki missed abortus meydana gelirken kalanları canlı doğum yaptı. Bunlardan altısı preterm ve otuz biri term doğum yaptı. On iki hasta vajinal yolla ve kalanları sezaryen yoluyla doğurtuldu. Yirmi hasta gebeliğin birinci yarısında (grup 1) ve 19 hasta gebeliğin ikinci yarısındaydı (grup 2). Gruplar arasında operasyon zamanı ve acile kabul ile operasyon arasındaki ortalama zaman aralığı açısıdan anlamlı farklılık yoktu. Sonuç: Gecikmiş operasyon ve negatif apendektomi olumsuz gebelik sonuçlarına neden olabilir. Şüpheli olgulardaki izlem tedavisi negatif apendektomi oranlarını düşürebilir; ancak aynı zamanda perforasyona neden olabilir. Bilgisayarlı tomografi ve manyetik rezonans görüntüleme, ultrasonografinin net olmadığı zaman düşünülmelidir. Erken doğum tehditini önlemek için tokolitik rejimler uygulanabilir. Doğum şekli için obstetrik endikasyonlar geçerlidir., Objective: To evaluate the pregnancy outcomes of patients who underwent appendectomy during pregnancy. Materials and Methods: Patients who underwent appendectomy between years 2010 and 2014 were retrospectively evaluated. All patients’ pregnancy outcomes were followed-up by using university registry system and telephone interview. Patients were evaluated regarding age, gestational age, clinical and laboratory examinations, imaging studies, mean time interval between emergency department and operation, mean operative time, pregnancy outcome and pathologic results of the appendix. Results: Thirty-nine patients were included in the study. Sixteen of 39 patients were in the first, 15 of them in the second and 8 of them were in the third trimester of the pregnancy. Three patients underwent laparoscopic appendectomy and the rest underwent laparotomy. In pathologic evaluation of the appendix, seven patients (17%) had normal appendix, 4 patients had perforated appendix, one patient had neuro-endocrine tumor and rest of the patients had appendicitis. Two missed abortion occurred after operation, rest of the patients had live birth. Six of them were preterm and 31 had term birth. Twelve patients delivered through vaginal birth and the rest via caesarean section. Twenty patients were in the first half of the pregnancy (group 1) and 19 patients were in the second half of the pregnancy (group 2). There were no significant differences between the groups in operation time and mean time interval between emergency administration and operation. Conclusion: Delayed operation and negative appendectomy can cause adverse pregnancy outcomes. Expectant management in suspected cases may decrease negative appendectomy rates but can also lead to perforation. Computed tomography and MRI ought to be considered if ultrasonography is inconclusive. Tocolytic regimens can be administered to prevent threatened preterm labor. Obstetric indications were valid for delivery mode.

Isolated tubal torsion containing an ectopic pregnancy is a very rare event. Ectopic pregnancy may also cause these symptoms and may impede the diagnosis of tubal torsion when these two coexist. We present a case with isolated fallopian tube torsion containing an ectopic pregnancy. A 25-year-old gravida 2, parity 0 woman presented with left lower abdominal pain and vaginal bleeding. Sonography revealed a 2 cm adnexal mass adjacent to the left ovary. β-hCG level was 157 mIU/ml. Laparoscopy depicted torsion of the left fallopian tube together with an ectopic pregnancy. Left salpingectomy was performed. The clinician should also think about tubal torsion when the pain is intermittent and if he suspicious about the ectopic pregnancy. Granüler hücreli tümörler, deride Schwann hücrelerinden geliştiği düşünülen ve oldukça ender görülen lezyonlardır. En sık başboyun bölgesinde görülmekle birlikte, %5.3 oranında vulvar bölgede saptanabilirler. Olguların büyük çoğunluğu benign olmasına rağmen, makroskopik olarak bu tümörlerin sınırları belirgin değildir ve çevre dokulara yapışıklık gösterebilirler. Nadir olarak malign transformasyon bildirilmiştir. Ayrıca bu hastalarda nüks de oldukça sık görülür. Bu olgu sunumunda, 59 yaşında daha önce meme karsinomu nedeniyle unilateral mastektomi operasyonu geçirmiş olan hastada gelişen vulvar granüler hücreli tümör güncel tedavi yaklaşımları ile birlikte tartışılacaktır., Granular cell tumors are unusual neoplasms presumably derived from Schwann cells. These tumors are mostly seen in head and neck area and 5.3%of cases are localized in the vulva. Although most of the cases are benign, macroscopically they are poorly circumscribed and show adherence to the surrounding connective tissue. Malignant transformation was also reported and recurrence is common in these patients. In this case report, a vulvar granular cell tumor is presented and discussed in a 59-yearold woman with a history of unilateral mastectomy due to breast cancer.

Rudimenter horn, müllerian kanal anomalilerinden biri olup rudimenter hornda gebelik sıklığı 1/76000 ile 1/150,000 arasında bildirilmiştir. Tanısı genellikle rüptüre olan gebelikten sonra konulabilmektedir. Vaka sunumumuzda 31 yaşında, öyküsünde sezeryan ile sağlıklı doğumu olan olgunun, rudimenter horn gebeliğinin sekizinci gebelik haftasında ultrasonografi ile erken tanısının konulması ve bu rudimenter horn gebeliğinin laparoskopik olarak eksizyonu bildirilmiştir., Rudimentary horn is one of the mullerian canal anomalies, the probability of rudimentary horn pregnancy is between 1/76000 and 1/150000. Usually, it is diagnosed after the rupture. in this case report, a patient who had a cesarean section before and who had an early diagnosis of rudimentary horn pregnancy in the eighth pregnancy week is mentioned. the rudimentary horn is excised by laparoscopy in this patient.

İmmatür over teratomu nadir görülüp erken yaşlarda ortaya çıkan ve erken metastaz yapan germ hücreli tümördür. Saf olarak yada miks olarak germ hücrelerle birlikte bulunabilirler. İmmatüriteyi belirleyen nöral doku komponenti olup bu tümörlerin malign yayılımı direkt komşu organlara intraperitoneal yayılım yoluyla olmaktadır. Burada, sol overde saf immatür teratom ve karaciğer metatazı olan kemoterapi ve cerrahi ile tedavi edilen bir olgu sunulmuştur., Immature ovarian teratoma is a rare germ cell tumors occuring in early ages with early metastasis. It is found either in pure form or a component of mixed germ cell tumor. Immaturity is determined by neural tissue and spread of this tumors directly to the adjacent tissue by peritoneal spread. We present a case with left ovarian immature teratoma and hepatic metastasis and treated with combination of chemotherapy and surgery.
