Objective: The aim of this study is to determine efficacy, safety, and feasibility of video-assisted thoracoscopic surgery (VATS) in childhood empyema with two port technique. Material and Methods: 34 patients under 17 years of age were included to the study. Demographic and clinical data of the patients were recorded retrospectively Results: 34 patients under 17 years of age were included in the study. The first time the complaints started was 12 days. VATS was performed on mean 12 (2-46) days after the complaints of the patients started. The empyemas were in right hemithorax in 21 children and left in 11. Two patients underwent bilateral decortication Mean of chest tube removal time was 9.70 (2-26) days. While the postoperative stay was 23.50 (4-120) days, the total hospital stay was 32.50 (7-142) days. Emphysema developed in 7 patients in the following period. Chest tube revision was performed in a patient. Emphysema spontaneously regressed at follow-up in 6 patients. Two children died from non-thoracoscopy reasons after respiratory arrest and liver transplantation. Conclusion: VATS is a feasible method with good results pleural empyema in children. This article presents a case study of a 10-month-old patient who developed severe tracheal stenosis after heart surgery. The patient received extracorporeal cardiopulmonary resuscitation (ECPR) and was treated with venoarterial extracorporeal membrane oxygenation (VA ECMO) and a tracheostomy. The article emphasizes the importance of ECPR in managing severe cardiac and airway complications after heart surgery and highlights the need for awareness of upper airway anomalies in patients with congenital heart disease. Necrotizing enterocolitis (NEC) is one of the most important life- threatening diseases in neonates. Recurrent episodes of supraventricular tachycardia (SVT) can lead to gut ischemia, with subsequent reperfusion injury resulting in NEC. Mesenchymal stem cells (MSCs) are multi-potential cells which can differentiate into multiple cell types. Case report: A 22-day old male baby delivered to 34 yr old mother at 37-weeks' gestation via cesarean section, birth weight 3550 g, developed NEC 14 hours after being treated with cardioversion for SVT. At laparotomy, there was pan-NEC, and 60 cm of necrotic and perforated bowel was resected. 1 × 107 umbilical cord origin allogeneic MSCs were given intravenously, with subsequent improvement of the remaining bowel. Conclusion: MSC may be a promising treatment for adjacent ischemic bowel in NEC helping prevent short bowel syndrome. [ABSTRACT FROM AUTHOR]

Objective: The purpose of this study was to discuss minimal invasive surgical options for surgical thoracic pathologies in the pediatric age group. Material and Methods: A retrospective analysis was performed for 45 patients who had undergone thoracoscopic surgery by a single surgeon. The underlying pathologies, surgical procedures and anesthetic aspects and postoperative outcomes were analyzed. Results: Forty-five children underwent thoracoscopic surgery in a ten-year period. The conditions requiring thoracoscopic surgery were congenital diaphragmatic hernia (CDH), esophageal atresia (EA), congenital malformations, pulmonary and mediastinal masses, empyema, spontaneous pneumothorax, and interstitial lung diseases. There were 21 girls and 24 boys. The mean age of the children except the CDH and EA cases was 4.5 years (n=33, 2 months to 17 years). The mean age of the children with CDH and EA was 5.5 days (n=12, 1 day to 8 months). There were 6 conversions in total. The mean chest tube removal time was 6 days (1-24 days). The mean hospital stay was 9 days (3-34 days). Conclusion: Thoracoscopy in the pediatric age group is safe and feasible in experienced hands. Objective: Robotic surgery is the newest of minimal invasive surgery developments. The opinions of surgeons are different about this topic, though robotic surgery is in a widespread period. This period is going to take some time. The advantages and disadvantages of robotic surgery is going to play a role in this period. The aim of this study to reveal the generality of robotic surgery in Turkey and the factors effect that. Material and Methods: A questionnaire on the advantages and disadvantages of robotic surgery was prepared. It was sent to 104 actively working robotic surgeons via the SurveyMonkey System. The answers of the 41 responders (39.4%) were evaluated. Results: Of the 41 responders, 32 had been working as a specialist for more than 10 years. Eighty percent of them (n=32) stated that they had advanced endoscopic surgery experience. Thirty-one stated that endoscopic experience was an advantage for performing robotic surgery. Forty percent of responders (n=15) had received robotic surgery training abroad and sixty percent (n=25) in Turkey. Even though there were some attendees who thought training abroad was important, 80% of the responders believed that training in Turkey was necessary. We found that 22% (n=9) of the responders had performed more than 100 robotic procedures while 60% (n=25) had performed less than 25. These numbers were less than 10% of the routine operations for 73% (n=30) of the participants. Ninety percent (n=37) of the surgeons stated that not touching the patient was not a limitation. Seventy-six percent reported that robotic surgery training should take place after the residency period. When the superiority of robotic to endoscopic surgery was queried, 83% (n=34) stated that robotic surgery was superior. The advantages of robotic surgery were listed as follows; 98% (n=40) three dimensional - magnified imaging, 88% (n=36) surgeon-controlled camera, 85% (n=35) wrist motion instruments, 75% easier suturing, 73% no camera and instrument tremor, 68% advanced imaging techniques and ergonomy. When the limitations of robotic surgery were queried, 63% (n=25) of the participants reported that robotic surgery had some limitations when compared to endoscopic surgery, and 65% stated that robotic surgery was more disadvantageous when it comes to conversion to open surgery. Ninety percent (n=37) of the surgeons thought that robotic surgery would become widespread in Turkey. The obstacles in front of robotic surgery were determined as cost (100%), insufficiency of health insurance paybacks (66%), long preparation time (20%), and the difficulty of assuring the equipment and a team. Conclusion: Robotic surgery is considered to have a wide range of usage in Turkey. Arrangements regarding the cost should be done and national training facilities should be increased to accelerate this process. [ABSTRACT FROM AUTHOR]
