Amaç: Bu çalışmada ağız solunumunun kraniyofasiyal gelişimüzerine etkisini belirlemek için nazal veya ağız solunumu yapançocukların sefalometrik grafileri karşılaştırıldı.Hastalar ve Yöntemler: Çalışmaya maloklüzyon nedeniyleortodontik tedavi gören 132 pediatrik hasta (65 erkek, 67 kız; ort.yaş 12.1 yıl; dağılım, 10-14 yıl) dahil edildi. Solunum şekli birkulak burun boğaz hastalıkları uzmanı tarafından aile öyküsü,fizik muayene, anterior rinoskopi ve fleksibl nazofarengoskopikmuayene ile değerlendirildi. Hastalar bu muayene sonucundanazal solunum yapanlar (n60) ve ağız solunumu yapanlar(n72) olarak sınıflandırıldı. Hastaların ortodontik tanı vetedavileri için alınan sefalometrik radyografileri kraniyofasiyalanalizler için kullanıldı.Bulgular: Nazal solunum yapan hastalara kıyasla ağızsolunumu yapan hastalarda retrognatik mandibula, artmışmandibüler düzlem açısı ve okluzal düzlem açısı vardı (p0.05).Nazal solunum yapan hastalara kıyasla ağız solunumu yapanhastaların hyoid kemikleri daha yüksekte yerleşimli venazofarengeal hava yolları daha dar idi (p0.05).Aktif büyüme döneminde ağız solunumu yapançocuklarda yüksek yerleşimli hyoid kemiği, retrognatikmandibula ve artmış vertikal yüz yüksekliği gözlemlendi. Ağızsolunumu yapan çocuklarda multidisipliner bir yaklaşım ileerken tanı sağlanarak kraniyofasiyal ve dental anomalileringelişimi önlenebilir., Objectives: This study aims to compare the cephalometricradiographs of nasal or mouth-breathing children to determinethe effect of mouth-breathing on craniofacial development. Patients and Methods: The study included 132 pediatric patients(65 males, 67 females; mean age 12.1 years; range, 10 to 14 years)having orthodontic treatment for malocclusion. Mode ofbreathing was assessed by an otorhinolaryngologist with thefamily history, physical examination, anterior rhinoscopy, andflexible nasopharyngoscopy. As a result of this examination,patients were classified as nasal-breathing patients (n60)or mouth-breathing patients (n72). Patients’ cephalometricradiographs, which were obtained for their orthodonticdiagnosis and treatment, were used for the analyses. Results: Compared to nasal-breathing patients, mouth-breathing patients had retrognathic mandible, increasedmandibular plane angle, and occlusal plane angle (p0.05).Compared to nasal-breathing patients, mouth-breathingpatients’ hyoid bones were located more superiorly andnasopharyngeal air spaces were narrower (p0.05). Conclusion: We observed superiorly located hyoid bone,retrognatic mandible, and increased vertical face height inmouth-breathing children during the active growing period.Early diagnosis may be established with a multidisciplinaryapproach in mouth-breathing children, thus preventing thedevelopment of craniofacial and dental anomalies.

Amaç: Bu çalışmada retroauriküler yaklaşımla yapılan otolojik cerrahilerden sonra aurikulada lateralizasyon olup olmadığı araştırıldı.Hastalar ve Yöntemler: Eylül 2014 - Ocak 2017 tarihleri arasında kliniğimizde retroauriküler yaklaşımla otolojik cerrahi yapılan toplam 83 hasta (35 erkek, 48 kadın; ort. yaş 37.614.3 yıl; dağılım 14-69 yıl) değerlendirildi. Aurikulanın serbest helikal kenarı ile mastoid cilt arasındaki mesafe üç yerden ölçüldü: A mesafesi (kulağın üst serbest helikal noktası ile mastoid cilt arası), C mesafesi (kulak lobülünün bittiği nokta ile mastoid cilt arası) ve B mesafesi (A ve C arasındaki mesafe). Meatoplasti yapılmadan inside to out mastoidektomi ve timpanoplasti yapılan hastalar grup 1 (n62), meatoplasti yapılarak açık kavite timpanoplasti ve radikal mastoidektomi yapılan hastalar grup 2 (n21) olarak belirlendi. Üç mesafe ameliyat öncesi, ameliyat sonrası birinci ay ve altıncı ayda ölçüldü ve grup içi ve gruplar arası karşılaştırmalarla değerlendirildi.Bulgular: Her iki grupta da ameliyat öncesi ölçümlere göre ameliyat sonrası birinci ayda istatistiksel olarak anlamlı farklılık görülürken (p0.05), ameliyat sonrası altıncı ayda istatistiksel olarak anlamlı farklılık görülmedi (p0.05). Ancak grup 1'de beş hastada (%8.1), grup 2'de ise üç hastada (%14.3) ameliyat sonrası altıncı ayda aurikulada asimetri saptandı. Gruplar arası karşılaştırmada; ameliyat sonrası birinci ay ile ameliyat öncesi değerler ve ameliyat sonrası altıncı ay ile ameliyat öncesi değerler arasında istatistiksel olarak anlamlı bir farklılık görülmedi (p0.05).Sonuç: Çalışma sonuçlarımıza göre retroauriküler yaklaşımda ameliyat sonrası erken dönemde aurikulada belirgin kepçelik görülürken ameliyat sonrası geç dönemde bu sorun büyük ölçüde düzeldi. Ancak hastaların %9.6'sında asimetri devam etti. Bu nedenle retroauriküler yaklaşım planlanan hastaların cerrahi öncesi onamlarını alırken böyle bir riskin olabileceğini hastalara anlatmakta fayda olacağını düşünmekteyiz., Objectives: This study aims to investigate whether auricular lateralization occurs after otologic surgeries performed with retroauricular approach.Patients and Methods: A total of 83 patients (35 males, 48 females; mean age 37.6±14.3 years; range 14 to 69 years) who were performed otologic surgeries with retroauricular approach were evaluated between September 2014 and January 2017. Distance between free helical rim of auricula and mastoid skin was measured from three localizations: distance A (between upper free helical point of ear and mastoid skin), distance C (between ear lobule ending point and mastoid skin), and distance B (distance between A and C). Patients who were performed inside to out mastoidectomy and tympanoplasty without meatoplasty were designated as group 1 (n62), while patients who were performed open cavity tympanoplasty and radical mastoidectomy with meatoplasty were designated as group 2 (n21). The three distances were measured preoperatively and at postoperative first and sixth months, and assessed by intra- and intergroup comparisons.Results: While statistically significant difference was seen in both groups at postoperative first month compared to preoperative measurements (p0.05). However, auricular asymmetry was detected in five patients (8.1%) in group 1 and three patients (14.3%) in group 2 at postoperative sixth month. In intergroup comparison; no statistically significant difference was seen between postoperative first month and preoperative values, and postoperative sixth month and preoperative values (p>0.05).Conclusion: According to our study results, while significant auricular protruding was seen in early postoperative period in retroauricular approach, this issue was recovered considerably in late postoperative period. However, asymmetry persisted in 9.6% of patients. So, we think that it will be helpful to explain this kind of risk to patients who are planned retroauricular approach when taking their informed consent before surgery.

Nazal kavitenin lobüler kapiller hemanjiomları (Piyojenik granülom) benign bir tümör olup etyolojisi tam olarak bilinmemektedir. En çok üzerinde durulan nedenler nazal travma ve hormonal faktörlerdir. Genellikle bir pedinkül ile anterior septum mukozasından kaynaklanırlar. Tek taraflı burun tıkanıklığı ve burun kanaması en önde gelen semptomlardır. Hızlı büyümeleri ve ülseratif görünümlerinden dolayı bazen nazal maligniteler ile karıştırılabilmektedir. Bu yazıda 46 yaşında bir kadın hastada daha önceki tedavilerinden sonra iki kez nüks eden piyojenik granülom olgusunun endoskopik olarak tüm sınırları görülerek tutunduğu mukoza ve perikondrium ile birlikte çıkarılması sunuldu. Nazal piyojenik granülom olgularının cerrahi tedavilerinin endoskopik yöntemle mukoza ve perikondrium ile birlikte çıkarılmasının gerekliliği literatür eşliğinde tartışıldı., Lobular capillary hemangiomas of the nasal cavity (Pyogenic granuloma) are benign tumors and their etiology is not known exactly. Most emphasized reasons are nasal trauma and hormonal factors. They are usually pedunculated and originated from the anterior septal mucosa. Unilateral nasal obstruction and epistaxis are the most common symptoms. Due to rapid growth and ulcerative appearance, they can sometimes be confused with nasal malignancies. In this article we report the second recurrence of pyogenic granuloma after previous treatments of a 46-year-old female patient that was removed subperichondrially and subperiostially with the endoscopic visualization of all of its boundaries. In the light of the literature, we discussed the necessity of endoscopic method in surgical treatments of the nasal pyogenic granulomas in subperichondrial and subperiosteal excision of the tumor.

Sinonazal bölgenin en sık görülen anatomik varyasyonu olan konka bülloza, orta konkanın pnömatizasyonu olarak tanımlanır. Konka bülloza genellikle asemptomatik olsa da boyutuna bağlı olarak nazal septum ve osteomeatal bölge ile ilişkisine göre semptomatik hale gelebilir. En sık karşılaşılan semptomlar burun tıkanıklığı, baş ve yüz bölgesinde ağrı, koku problemleri, sinüzit, postnazal akıntı ve hipozmidir. Tanı; anterior rinoskopi, endoskopik muayene ve paranazal sinüs tomografisiyle konulur. Tedavide yeterli burun açıklığı sağlamak amacıyla endoskopik yöntemle orta konkanın parsiyel rezeksiyonu yapılır. Bu yazıda kronik baş ağrısı, burun tıkanıklığı ve koku alma bozukluğuna neden olan tek taraflı dev konka büllozası olan 34 yaşında bir kadın hasta literatür eşliğinde sunuldu., Concha bullosa, which is the most frequent anatomic variation of the sinonasal region, is defined as the pneumatization of the middle turbinate. Although concha bullosa is usually asymptomatic, it may become symptomatic according to its relationship with the nasal septum and osteomeatal region based on its size. The most frequent symptoms are nasal obstruction, pain in head and face region, olfactory problems, sinusitis, postnasal discharge, and hyposmia. Diagnosis is established by anterior rhinoscopy, endoscopic examination, and paranasal sinus tomography. To ensure sufficient nasal patency for treatment, partial resection of the middle turbinate is performed by endoscopic method. In this article, we report a 34-year-old female patient with unilateral giant concha bullosa causing chronic headache, nasal obstruction, and olfactory disorder in the light of the literature.

Amaç: Bu çalışmada, otoskleroz ameliyatı olan hastaların ameliyat yöntemi, işitme sonuçları ve komplikasyonları literatür eşliğinde değerlendirildi.Hastalar ve Yöntemler: Haziran 2010 - Haziran 2015 tarihleri arasında kliniğimizde otoskleroz tanısı konularak ameliyat edilen 50 hastanın (21 erkek, 33 kadın ort. yaş 3815 yıl; dağılım 18-67 yıl) dosyaları retrospektif olarak incelendi. Hastaların muayene bulguları, uygulanan ameliyat yöntemleri, kullanılan protezler, işitmedeki kazanç ve değişiklikler ile karşılaşılan komplikasyonlar değerlendirildi. Ameliyat öncesi ve sonrası hava-kemik yolu eşikleri karşılaştırılarak fonksiyonel kazanç oranları hesaplandı.Bulgular: Elli otoskleroz hastasının 54 kulağına stapedotomi ameliyatı uygulandı. Ameliyat sonrasında hastaların hava yolu işitme eşikleri ve hava-kemik aralıklarında istatistiksel olarak anlamlı oranda azalma saptandı. Ameliyat sonrası 46 hastada (%85.1) fonksiyonel başarı elde edildi. Hastaların birinde inkus dislokasyonu, üçünde korda timpani hasarı, birinde yüzen taban ve iki hastada da total sensörinöral işitme kaybı gözlendi. Beş hastada da geçici vertigo şikayeti gözlendi.Sonuç: Klasik yöntemle uygulanan stapedotomi, yüksek başarı oranı ve düşük komplikasyon oranları ile başarılı bir tedavi yöntemidir. Ameliyatın nadir de olsa ciddi morbiditeye neden olan komplikasyonlarının olabileceği akılda tutulmalı ve medikolegal sorunlar ile karşılaşmamak için hastalar ameliyat öncesi bilgilendirilmelidir., Objectives: This study aims to evaluate the surgical method, hearing results and complications of patients undergoing surgery for otosclerosis, in the light of literature.Patients and Methods: We retrospectively examined the records of 50 patients (21 males, 33 females; mean age 38±15 years; range 18 to 67 years) who were diagnosed with otosclerosis and underwent surgery in our clinic between June 2010 and June 2015. We evaluated the examination findings of the patients, surgical procedures applied, prostheses that were used, gains and changes in hearing, and complications. The rates of functional gain were calculated by comparing pre- and postoperative air-bone conduction thresholds.Results: Stapedotomy surgery was applied to 54 ears of 50 otosclerosis patients. Statistically significant reduction was detected in airway hearing thresholds and in air-bone gaps of patients following the surgery. After surgery, functional success was accomplished in 46 patients (85.1%). We observed incus dislocation in one patient, chorda tympani injury in three, floating base in one, and total sensorineural hearing loss in two patients. We also observed temporary vertigo complaint in five patients.Conclusion: Stapedotomy applied with the conventional method is a successful method of treatment with high success rate and low complication rates. It should be kept in mind that there may be rare complications of the surgery causing serious morbidity, and patients should be informed prior to surgery to avoid medicolegal problems.

Timpanik membrana ventilasyon tüpü tatbiki en sık uygulanan kulak cerrahilerinden bir tanesidir. Kronik seröz otit medianın cerrahi tedavisinde sıkça uygulanan bu yöntem hastada orta kulağın ventilasyonunu sağlayıp orta kulak basıncını düzenler. Ventilasyon tüpü tatbikine bağlı en sık karşılaşılan komplikasyonlar miringoskleroz, atrofi ve otoredir. Tüpün atılım sürecinde mediale doğru yönlenip orta kulağa atılmasıyla sağlam zarın medialinde görülmesi haline oldukça nadir rastlanan bir komplikasyondur. Ventilasyon tüpü tatbiki sonrası komplikasyon çok nadir gözlense de erken dönemde tanınıp tedavi edilmesi durumun daha kötüye gitmesini engelleyecektir.Bu vaka sunumunda rutin takiplerini bırakan geç dönem muayenesinde tüpü orta kulağa atılan ve çevresinde granülasyon dokusu oluşan hastanın literatür eşliğinde sunulması amaçlandı., The application of ventilation tubes (VT) is one of the most frequently performed surgical ear. VT frequently applied in chronic serous otitis media surgery. VT, edits the middle ear pressure and provides ventilation of the midle ear. Altough the VT complications are rare, the most common complications associated with ventilation tube are myringosclerosis, atrophy and otorrhea. Thrown tube into the middle ear is an extremely rare complication. It is diagnosed and treated at an early stage of complications will prevent the situation from getting worse. In this study; the patient was presented ,whose ventilation tube migrate to the middle ear and granulation tissue formed around.

Objectives: In this study, we aimed to investigate whether the spine of Henle could a new landmark for the localization of the superior semicircular canal SCC . Patients and Methods: Between March 2014 and March 2015, a total of 30 adult cadaveric temporal bones were used in this study. All temporal bones were positioned and, then, canal-wall up mastoidectomy and facial recess approach were performed. The landmarks such as sigmoid sinus SS , middle cranial fossa dura MCFD , posterior cranial fossa dura PCFD , SCCs, short process of the incus, facial nerve FN , and chorda tympani nerve were identified. The shortest distances between the spine of Henle and the following structures were measured: i superior SCC; ii lateral SCC; iii posterior SCC; iv MCFD; v PCFD; vi SS; and vii FN. All measurements were performed using a digital caliper. Results: After excluding three temporal bones with absent spine of Henle, 27 temporal bones were studied. The mean distances between the spine of Henle and the superior, lateral and posterior SCCs, MCFD, PCFD, SS, and FN were 20.3±2.4, 15.9±2.1, 17.8±2.0, 13.7±2.9, 19.0±2.7, 14.9±3.6, and 15.8±1.7 mm, respectively. The distances between the spine of Henle and superior SCC were higher in the temporal bones, while the distances between the spine of Henle and MCFD were higher than the mean values. Similar differences were found for the distances between the spine of Henle and the lateral and posterior SCCs and PCFD. Conclusion: Based on our study results, the spine of Henle can be used as a landmark for the localization of the superior SCC during mastoidectomy and superior SCC surgeries.

Ischemia-Reperfusion Injury (IRI) is an important contributor to the viability of flaps after transferring. The goal of this research was for assess the probable useful impacts of ozone on flap survival in a rat muscle-flap design. Materials and Methods: We examined the effects of postconditioning ozone administration on viability of pedicled composite flaps. Twenty-eight Wistar rats were randomized into four groups: sham-operated (S), ischemia-reperfusion (IR), sham-operated + ozone (O), IR + ozone (IR + O), respectively. The animals were sacrificed on the eighth day. In a general histological evaluation, flap tissues were examined with a light microscope, and apoptotic cells were counted. The Apoptotic Index (AI) was then calculated. Flap-tissue samples were sent for analyses of malondialdehyde (MDA), catalase (CAT), glutathione peroxidase (GSH-Px), superoxide dismutase (SOD), and protein carbonyl (PCO), and blood samples were sent for analyses of Total Oxidant Score (TOS), and Total Antioxidant Capacity (TAC). Data were evaluated statistically using the Kruskal–Wallis test. Results: The histomorphometric score was remarkably greater in O (p =.002). The AI was greater in IR (p =.002). Aim: Sudden sensorineural hearing loss (SSHL) is usually diagnosed in a single ear. The main objective of this study is to assess the dominance of affected ear by comparing right versus left-ear cases.Materials and Methods: We retrospectively examined the records of 105 patients (63 males, 42 females; mean age 41.5±15 years; range 18 to 72 years) between January 2013 and January 2017.Results: Sixty-three of the included cases were male and 42 were female patients. SSHL was diagnosed on the left ear in 51 (48.6%) patients and on the right ear in 49 (45.7%) patients. No statistically significant dominance on the affected ear side was detected whereas bilateral involvement was significantly low. In male patients, 33 (52.3%) had hearing loss on the right and 29 (46%) had hearing loss in the left ear. Sixteen (32.7%) female patients had right ear and 22 (43.1%) had left ear involvement. There was a slight dominance on the right ear in male and on the left ear in female patientsConclusion: There is no significant difference between left and right ear involvement in SSHL. The results of our study which showed right ear dominance in males and left ear dominance in female patients are thought to be purely coincidental.Keywords: Ear, sudden hearing loss, laterality

Amaç: Disfaji pediatric popülasyonda insidansı giderek artmakta olan bir semptomdur. Disfaji tanısında çeşitli yöntemler kullanılmaktadır. Disfaji tedavisinde fizyoterapik rehabilitasyon uygulanmaktadır. Bu çalışmada amacımız disfaji şikayetiyle kliniğimize başvuran çocuk hastalarda terapi etkinliğinin ultrason ile değerlendirmesidir.Gereç ve Yöntem: Disfaji şikayetiyle başvuran 25 çocuk hasta çalışmaya alındı. Hastaların geniş bir tıbbi anamnezi alındı ve sistemik muayenesi yapıldı. Hastalara uygun yutma rehabilitasyonu uygulandı. Hastalara MBYÇ ve FEYÇ testleri yapıldı. Muayene bulguları; PAS, YGS, FOIS, KÇS ölçekleri ile değerlendirildi. Hastaların aileleri tarafından pEAT-10 anketi doldurdu. Hastalar USG ile muayene edildi. USG ile dil kökü kesitsel alanı, submental kas (digastrik,mylohyoid,geniohyoid) kesitsel kalınlıkları, hyoid elevasyonu incelendi. Terapi sonrası tüm muayeneler ve değerlendirme ölçekleri ve USG incelemeleri tekrarlandı. Veriler istatiktiksel olarak karşılaştırıldı.Bulgular: Hastaların ortalama yaşları 4.26 yaştı. WOS: 000351489300020, PubMed: 25759784, Background: Pleomorphic adenomas (PAs) are the most common type of benign salivary gland tumours and usually originate from the major salivary glands. PA originating from the nasopharynx has rarely been reported in the literature. Case Report: A 62 year-old male presented with right aural fullness and subjective hearing loss. Otoscopic examination revealed an opaque and bulging tympanic membrane. Nasal endoscopic examination revealed a smooth mass arising from the right wall of the nasopharynx, obstructing the opening of the right Eustachian tube. The MRI showed a 1.3x2x1.3 cm sharply outlined and lobar mass in the right lateral wall of the nasopharynx. The tumour was meticulously detached from its attachment site and excised via the transnasal route under endoscopic visualisation. Conclusion: We reported pleomorphic adenoma in the nasopharynx in a patient who presented with serous otitis media that successfully resected with transnasal endoscopic surgery and discussed the treatment methods.
