Background Supratip depression is a common complication after preservation rhinoplasty. Objectives This aim of this paper was to present a simple surgical maneuver to prevent supratip depression. Methods Thirty-six patients who underwent closed-approach, low-septal-resection dorsal preservation rhinoplasty between January and June 2021 were included in this retrospective study. Depending on the operation performed on Pitanguy's midline ligament, the patients were divided into 2 groups as follows: (1) a group in which Pitanguy's midline ligament was transected (transection group); and (2) a group in which Pitanguy's midline ligament was preserved (preservation group). Standardized postoperative 6-month lateral-view photographs were scanned for the presence of supratip depression or pollybeak deformity. The Rhinoplasty Outcome Evaluation (ROE) scale was applied at 6 months. Results Supratip depression was observed in 4 patients in the preservation group (n = 16), but was not observed in any of the patients in the transection group (n = 20; P  < 0.05). There was no pollybeak deformity in either group. With respect to the Rhinoplasty Outcome Evaluation scores and number of satisfied patients, no statistically significant difference was found between the transection and preservation groups (P  > 0.05). Conclusions Transecting Pitanguy's midline ligament reduces the likelihood of supratip depression and does not affect the likelihood of pollybeak deformity in closed-approach, low-septal-resection dorsal preservation rhinoplasty. Objective: The aim of this study was to determine the medication adherence and affecting factors in elderly patients age 65 and older. Material and Method: A descriptive study was conducted with 396 elderly patients age 65 and older applied to some clinics of a university hospital. Data was collected through face to face interviews. In the study irregular drug use (defined as; skipping the dosage of the drugs used daily more than once in a week with any reason) and polypharmacy (five or more drug use) were analyzed. Data were evaluated by chi-square test, fisher's exact test and t-tests. The ethical approval was taken. Results: In the study, 52.5% (208) of the participants was woman and the mean age was 70.1±5.1. Most of the participants had at least one chronic disease (94.9%) and mean number of having chronic disease was 2.6±1.4. 94.2% of the participants use drug continuously and mean number of drug use was 3.5±2.3 (in women 3.9, in men 3.1). Women use drug more irregularly than men. In the study, 27.0% of the participants used five or more drugs. Medication adherence was 71.3%. There is no relation between medication adherence and polypharmacy (p>0.05). Chronic obstructive lung disease-Asthma drugs (62.5%), gastrointestinal system drugs (63.2%) and central nervous system drugs (54.8%) were the medications with lowest adherence at both sex. The most important factor that affected medical adherence was "to forget to take the pills" and it was also seen that the patients didn't take the pills when they felt good. It was found that the drug usage was very high and as the number of the drug increased, medication adherence was decreased. Conclusion: It is important that the number of prescribed drugs must be minimum, in order to organize drug usage in elderly people. It is thought that prescribing less drugs and preventing unnecessary drug use would increase medication adherence. Better communication between health stuff and patients and informing the patients about their chronic diseases would help to prevent drug misuse. [ABSTRACT FROM AUTHOR]

Long-term stability and recurrent ptosis are among the most debated topics in endoscopic brow lifts. Although there are many publications on endoscopic brow lifts, more research is needed on long-term brow elevation and stability. This systematic review aims to evaluate the amount of elevation and stability achieved by endoscopic brow lifts in the long term. To evaluate the long-term outcomes of endoscopic brow lifts, the databases 'Pubmed', 'Web of Science', 'Scopus', and 'Google Scholar' were searched using the keywords 'endoscopic brow lift', 'endoscopic forehead lift', 'forehead lift', 'foreheadplasty', 'brow lift', 'endoscopic brow fixation', and 'brow fixation'. Studies published between September 1994 and May 2024, including isolated or combined endoscopic brow lift surgeries, were included. 5324 articles were screened, and 85 full texts were reviewed. Of these studies, 12 (14.1%) were found suitable for systematic review and meta-analysis. Brow elevation values were evaluated separately for medial, central, and lateral parts. The pooled effect sizes for medial, central, and lateral brow elevations were found to be 3.25 mm (2.44-4.06), 3.86 mm (2.93-4.8), and 4.35 mm (3.06-5.64), respectively. This study is the first meta-analysis to present the average elevation values that can be achieved in the long term in endoscopic brow lifts. These data guide a better understanding of the patient candidates and the technique for endoscopic brow lifts. Sharing more objective data over the long term with different fixation methods will contribute to a better understanding of the criteria related to the indication of this surgery., (© The Author(s) 2024. A normal alar-columellar relationship is crucial for lower third finesse in rhinoplasty. Hanging columella is a common complaint in the alar-columellar relationship and membranous septum excision is frequently recommended for its correction. This paper focuses on the role of the membranous septum in achieving a normal alar-columellar relationship. It was aimed to present an algorithm that could be used for aberrancies in the alar-columellar relationship.Ninety-seven patients who underwent closed-approach rhinoplasty between January and December 2022 were included in this retrospective study. Routine tip plasty maneuvers were performed on all patients. Membrane septum excision was not performed in any of the cases. Standardized preoperative and postoperative 12-month lateral view photographs were scanned for hanging or retracted columella.Hanging columella was observed in seven patients preoperatively ( n  = 97). No hanging or retracted columella was observed postoperatively.Hanging columella can be successfully corrected with alternative surgical maneuvers without excision of the membranous septum in closed-approach rhinoplasty in appropriate cases.
