Aim: The aim of this case report was to examine the effect of mycozis fungoides on pregnancy, the pattern of mycozis fungoides during pregnancy and the literature review. Case: The patient was 30 and gravida 1 and parity 1. She was 7 weeks pregnant when she applied to our clinic and her treatment was interfered because of the pregnancy. Psoralen and ultraviolet-A light (PUVA) therapy was applied to the patient. Conclusion: The effect of MF on pregnancy is not known well due to the limited number of cases and there were not important side effects of the MF observed in our case. However, more studies are needed considering the effect of MF on pregnancy and MF therapy during pregnancy. Amaç: Bu vaka sunumunda mycozis fungoides’in (MF) gebelik üzerine etkisini ve gebelikte mycozis fungoides paternini ve literatürün gözden geçirilmesi amaçlanmıştır. Vaka: Hasta 30 yaşında, gravidası 1, paritesi 1 idi. Kliniğimize 7 haftalık gebelikle başvurdu ve tedavisine gebelik nedeniyle ara verildi. Hastaya Psoralen ve ultraviyole -A light (PUVA) tedavisi uygulandı. Sonuç: Gebeliğe MF’in etkisi, sınırlı vaka sayısı nedeni ile tam bilinmemektedir ve vakamızda MF’nin önemli yan etkisi gözlenmemiştir. Fakat gebelikte MF‘in etkisi ve gebelik süresince MF tedavisi ile ilgili daha geniş çalışmalara ihtiyaç vardır.

Jinekolojik kanserler; ovaryan, servikal, endometriyal, vulvar kanserler olup kadınlardaki yıllık 5,1 milyon yeni kanserin %19’unu oluşturmaktadır. Serum tümör belirteçleri hastalığın taramasında, tespit edilmesinde, tedavinin monitorizasyonunda ve takip programında, özellikle rekürrensin prediksiyonunda önemli rol oynamaktadır. Bu derlemede jinekolojik kanserlerde tümör belirteçlerinin rolü tartışılmıştır.

The most common causes of Carbonmonoxide (CO) poisoning in developing countries are pooand low-burnt coal (stove and water heater poisoning), motor vehicle exhausts (poisonings occurring in closed garages), spray paints, and fires. The presence of low concentration of 0.1% in inhaled air and detection of over 15% COHb in the blood of pregnant woman are causes ofeto-maternal morbidity and mortality. When pregnant women acutely exposed to CO gas; toxic effects such as fetal death, abortion, intrauterine growth retardation (IUGR), anatomical malformations and functional changes may occur. In oustudy, we discuss a case of CO positioning in the week of 32 of the pregnancy and aim to emphasize the approach to CO poisoning in the pregnancy., Karbonmonoksit (CO) zehirlenmesinin gelişmekte olan ülkelerde en sık nedeni; kötü ve az yanmış kömür (soba-şofben zehirlenmeleri), motorlu taşıt egzozları (kapalı garajlarda meydana gelen ze- hirlenmeler), sprey boyalar ve yangınlardır. Nefes alınan havada %0.1 gibi düşük konsantrasyon- larda bulunması, gebe kanında %15 ve üzerinde COHb tespit edilmesi, maternal-fetal mortalite ve morbidite nedenidir. CO gazına akut olarak maruz kalan gebelerde fetal ölüm, abortus, intra- uterin gelişme geriliği (IUGG), anatomik malfor- masyonlar ve fonksiyonel değişimler gibi toksik etkiler meydana gelebilmektedir. Çalışmamızda, 32. gebelik haftasında CO zehirlenmesine maruz kalan vakayı tartışarak, gebelikte CO zehirlenme- sine yaklaşımı vurgulamayı amaçladık.

Amaç: Gonadotropin salgılatıcı hormon (GnRH) agonist ile downregüle edilmiş, başarısız bir in-vitro fertilizasyon (İVF) siklusunu takip eden tedavi siklusunda protokol tercihinin agonist veya antagonist olmasının gebelik sonuçları üzerine etkisinin değerlendirilmesi. Gereç ve Yöntemler: Bu bir retrospektif çalışmadır. Bir İVF ünitesinde 2002-2012 yılları arasında GnRH agonist protokol kullanılan ve başarısız bir İVF siklusu sonrasında bir yıl içinde tekrar İVF uygulanan 269 hasta çalışmaya dahil edilmiştir. Hastaların yaşı, FSH değerleri, antral follikül sayıları, indüksiyon süreleri, toplanılan oosit sayısı, transfer edilen embriyo sayısı ve transfer günleri, klinik ve devam eden gebelik oranları her iki tedavi siklusu için değerlendirilmiştir. Bulgular: Normoresponder hastalar ikinci siklusta agonist ya da antagonist protokol seçimine göre ikiye ayrılmış ve ardışık iki İVF sikluslarının sonuçları karşılaştırılmıştır. Uygulanan gonadotropin dozu, indüksiyon süresi, elde edilen oosit sayısı, embriyo transfer günü ve sayısı açısından her iki grup arasında istatistiksel olarak anlamlı bir fark saptanmamıştır (p>0,05). Ayrıca fertilizasyon, klinik ve devam eden gebelik oranları açısından her iki grup benzer oranlara sahiptir. J Turk Soc Obstet Gynecol 2014;4:198-202 Objective: Evaluation of the impact of agonist or antagonist protocol selection on pregnancy outcomes after failure of in-vitro fertilization (IVF) treatment cycles which were down regulated with Gonadotropin Releasing Hormone (GnRH) agonist. Materials and Methods: This was a retrospective study. Two hundred and sixty nine patients who were treated with GnRH agonist protocol between years 2002-2012 at an IVF unit and underwent a second attempt following one year period after failure of IVF enrolled in the study. Age, basal FSH levels, antral follicle counts, duration of induction, the number of yielded oocytes, the number of transferred embryos and the transfer days, clinical and ongoing pregnancy rates were evaluated for each treatment cycle. Results: Normoresponder patients were separated into two groups according to the agonist or antagonist protocol selection at the second attempt and the results of two consequent IVF cycles were compared. There were no statistically significant difference between the groups for the dosage of administered gonadotropin, duration of induction, the count of yielded oocytes, the day and the number of transferred embryos (p>0.05). Furthermore the fertilization rate, clinical and ongoing pregnancy rates were similar in two groups. Lichen sclerosus (LS) is a lymphocyte-mediated, chronic, inflammatory dermatitis which most commonly involves the anogenital area. Treatment options include topical corticosteroids, tacrolimus, carbon dioxide laser ablation and, as a last resort vulvectomy. The objective of this study is to report a refractory anogenital LS case and describe the use of a V-Y advancement flap for reconstruction of the perineal defect after vulvectomy. A case of persistent LS, treated succesfully with surgery is presented and the literature is reviewed. The patient tolerated the surgical procedure well and a satisfactory cosmetic result was obtained. A multidisciplinary team is ideal in the management of patients with persistent LS. Surgical management of LS should be reserved as a last resort for patients who are refractory to other medical treatment options and, in the case of a surgical attempt, techniques for reconstruction of the vulva should be well known. Borderline ovarian tumours are usually seen in younger aged women and the issue of conservative surgery is increasingly important in today’s fertility-preserving concept. We present three borderline ovarian tumour cases who underwent conservative surgery in our instutition between 2006-2008 and their long-term follow-ups for their reproductive capacity after treatment. No recurrence after conservative treatment in our cases had been noted. Fertility-preserving surgery may be the treatment of choice with careful follow-up in women with Stage I borderline ovarian tumours, who desire further childbearing. Assisted Reproductive Techniques should be taken into consideration after the initial treatment and second-look surgery may be performed in the course of a caesarean section.Key words: Tumors of low malignant potential, borderline ovarian tumors, conservative surgery

Tubal sterilization is a widely used permanent contraceptive method for women who had completed their desired childbearing. If a pregnancy occurs after tubal sterilization, it is expected to be ectopic in most of the cases. However encountering an intrauterine pregnancy after Pomeroy tubal sterilization is extremely rare. A second time intrauterine pregnancy occured in a 34-year-old woman who had undergone tubal sterilization by Pomeroy technique in her last caesarean section. We are pointing out the failure risk of tubal sterilization with its possible mechanisms which might result in a viable intrauterine pregnancy.

In-vitro fertilizasyon (IVF) tedavisi ana hatları ile eksojen gonadotropinler kullanılarak yapılan kontrollü ovaryan hiperstimulasyon, ardından transvajinal ultrason eşliğinde overlerden oositlerin toplanması (OPU), embryoloji laboratuarında gerçekleştirilen fertilizasyon ve embryola- rın uterusa transservikal olarak transferi aşamalarından oluşmaktadır. OPU işlemi bu sürecin kısa bir dilimini oluştursa da, işlem sonrasında yeterli sayıda oosit elde edilebilmesi direkt olarak tedavi başarısı ile ilişkilidir. İşlemin uygun teknikle yapılması toplanan oosit sayılarının arttırılması ve hayatı tehdit edebilecek komplikasyonların önlenmesinde büyük öneme sahiptir. Biz bu derlememizde uygulayıcılar tarafından OPU tekniğinin optimize edilebilmesi için güncel literatürü gözden geçirerek en doğru tekniğin belirlenmesini hedefledik. Objective: The aim of this study was to assess tumor markers and clinicopathological findings of patients with serous and mucinous borderline ovarian tumor (BOT) features. Methods: The study consisted of 50 patients that were diagnosed with and treated for BOT between 2005- 2010 in three centers. CA125, CA19-9, and CA125+CA19-9 levels and clinicopathological features were compared in serous and mucinous histotypes. In serous and mucinous BOTs, correlations between tumor markers and demographics such as age, menopausal status, parity, clinical findings (stage, relapse, adjuvant chemotherapy, cytology, lymph node involvement and tumoral morphology (cystic-solid content, papilla, septation) were evaluated. Results: There were no significant differences between serous and mucinous tumors in the clinicopathological features such as stage, tumor markers, age, menopausal status, or cytology. Bu derlemede jinekolojik kanserlerde tümör belirteçlerinin rolü tartışılmıştır. 5,1 million women getting diagnosed as cancer every year and gynecological cancers (including ovarian, cervical, endometrial and vulvar cancers) constitutes 19% of all new cancers. Serum tumor markers have an important role in scanning and determining of disease, monitoring and follow up program of therapy and particularly prediction of recurrence. In this review, role of the tumor markers in gynecological cancers were discussed.

Amaç: Sezaryen sonrası vajinal doğum yapan olguda gelişen, uterin rüptür ve atoniye yaklaşımın sunulması. Olgu: Geçirilmiş sezaryen öyküsü bulunan hastada vaginal doğum sonrası eski insizyon skarında rüptür ile birlikte postpartum uterin atoni gelişmiştir. Laparotomi ile rüptüre alan belirlenerek primer sütüre edilmiş ve uterin atoni için aynı seansda B-Lynch sütürü uygulanmıştır. Sonuç: Sezaryen sonrası vajinal doğumlarda, postpartum gelişebilecek abondan vajinal kanamalarda eski uterin skar rüptürü ve bunun atoni ile birliktelik gösterebileceği akılda tutulmalıdır. Objective: To present a case with a history of previous cesarean delivery in which uterine rupture and atony occurred after vaginal birth. Case: After vaginal birth in a patient with a history of previous cesarean delivery, uterine rupture was diagnosed in the old incision scar concomitantly with postpartum uterine atony. Uterine rupture was repaired with laparotomy by applying primary sutures and B-Lynch suturing was performed for uterine atony. Conclusion: In patients who had deliver vaginally after caessarean section, uterine rupture in the old incision scar and the togetherness of this conditon with uterine atony should always be kept in mind.
