Background Environmental disinfection is essential for reducing spread of healthcare-associated infections (HAIs). Previous studies report conflicting results regarding the effects of ultraviolet (UV) light in reducing infections. This trial evaluated the impact of adding pulsed-xenon UV (PX-UV) to standard terminal cleaning in reducing environmentally implicated HAIs (eiHAIs). Methods The Lowering the Acquisition of MDROs with Pulsed-xenon (LAMP) trial was conducted in 2 hospitals (15 inpatient wards) utilizing a cluster-randomized, controlled, double-blinded, interventional crossover trial comparing standard terminal cleaning followed by either PX-UV (intervention arm) or sham (control arm) disinfection. The primary outcome was incidence of eiHAIs from clinical microbiology tests on the fourth day of stay or later or within 3 days after discharge from the study unit. EiHAIs included clinical cultures positive for vancomycin-resistant enterococci, extended-spectrum beta-lactamase–producing Escherichia coli or Klebsiella pneumonia , methicillin-resistant Staphylococcus aureus , and Acinetobacter baumannii , and stool polymerase chain reaction (PCR) positive for Clostridiodes difficile. Results Between 18 May 2017 and 7 January 2020, 25 732 patients were included, with an incidence of 601 eiHAIs and 180 954 patient-days. There was no difference in the rate of eiHAIs in the intervention and sham arms (3.49 vs 3.17 infections/1000 patient-days, respectively; RR, 1.10; 95% CI,.94–1.29; P =.23). Study results were similar when stratified by eiHAI type, hospital, and unit type. Conclusions The LAMP study failed to demonstrate an effect of the addition of UV light disinfection following terminal cleaning on reductions in rates of eiHAIs. The possible future emergence of new SARS-CoV-2 virus variants pushes the development of new chemoprophylaxis protocols complementary to the unspecific and specific immune-prophylaxis measures currently used. The SARS-CoV-2 virus is particularly sensitive to oxidation, due to the relevant positive electrical charge of its spike protein used as a ligand for target cells. The present study evaluated the safety and efficacy of a new oxidant preparation, liquid hyperoxygen (IOL), to neutralize the SARS-CoV-2 virus. IOL was incubated with throat swabs containing a human-type virus. The samples were then incubated with cells expressing the ACE2 receptor and, therefore, very sensitive to SARS-CoV-2 infection. The ability to neutralize SARS-CoV-2 was determined by assessing the amount of viral nucleic acid inside cells by PCR. The results obtained indicate that IOL, even at considerable dilutions, is capable, after incubation times of less than 30 min and even equal to 5 min, of completely inhibiting SARS-CoV-2 infection. This inhibitory effect has been shown to be due to the oxidizing capacity of the IOL. This oxidizing capacity is exerted towards the virus but does not damage eukaryotic cells either in the in vitro or in vivo skin models. Obtained results indicate that the use of IOL, a hydrophilic liquid mixture saturated with highly reactive oxygen and nitrogen species, is a new powerful, safe, and effective tool for preventing possible future outbreaks of the COVID-19 disease. In facing the outbreak of the pandemic, robots are highly appealing for their non-contact nature. Among them, we have selected the mobile robot manipulator to develop an autonomous system for pandemic prevention, as it possesses both mobility and manipulability. The robot was used as a platform for performing autonomous fever detection, medicine delivery, and environmental disinfection system for the fever station and isolation ward, which are the two primary units that deal with the pandemic in a hospital. The proposed novel algorithms aim to ensure both human safety and comfort by automating fever detection and recognizing medicine taking. Additionally, they address environmental disinfection by effectively covering blind spots. We conducted a series of experiments to evaluate their performance in a hospital-like setting, which was designed specifically for the testing of intelligent medical systems developed in our university. Quantitative assessment was administered to analyze how the introduction of the proposed autonomous system reduced the risk of infection, and feedback was also collected from participants through questionnaires. Environmental infection transmission is a perennial problem in long-term care facilities (LTCFs), exacerbated by shared living arrangements, residents with cognitive deficits, staffing shortages, and suboptimal cleaning and disinfection. This study evaluates the impact of dry hydrogen peroxide (DHP), as a supplement to manual decontamination, on bioburden within an LTCF neurobehavioral unit. In this prospective environmental cohort study utilizing DHP in an LTCF's 15-bed neurobehavioral unit, 264 surface microbial samples (44 per time point) were collected in 8 patient rooms, 2 communal areas on 3 consecutive days pre-DHP deployment and on days 14, 28, and 55 post-DHP deployments. The microbial reduction was evaluated by characterizing bioburden as total colony-forming units in each sampling site pre- and post-DHP deployment. Volatile organic compound levels were also measured in each patient area on all sampling dates. Multivariate regression was used to analyze microbial reductions associated with DHP exposure, controlling for sample and treatment sites. A statistically significant relationship was detected between exposure to DHP and surface microbial load (P ≤.00001). Additionally, the average volatile organic compound level postintervention was significantly lower than baseline levels (P =.0031). Surfaces in highly anthropized environments are frequently contaminated by both harmless and pathogenic bacteria. Accidental contact between these contaminated surfaces and people could contribute to uncontrolled or even dangerous microbial diffusion. Among all possible solutions useful to achieve effective disinfection, ultraviolet irradiations (UV) emerge as one of the most "Green" technologies since they can inactivate microorganisms via the formation of DNA/RNA dimers, avoiding the environmental pollution associated with the use of chemical sanitizers. To date, mainly UV-C irradiation has been used for decontamination purposes, but in this study, we investigated the cytotoxic potential on contaminated surfaces of combined UV radiations spanning the UV-A, UV-B, and UV-C spectrums, obtained with an innovative UV lamp never conceived so far by analyzing its effect on a large panel of collection and environmental strains, further examining any possible adverse effects on eukaryotic cells. We found that this novel device shows a significant efficacy on different planktonic and sessile bacteria, and, in addition, it is compatible with eukaryotic skin cells for short exposure times. The collected data strongly suggest this new lamp as a useful device for fast and routine decontamination of different environments to ensure appropriate sterilization procedures. It is well known that viruses cannot replicate on their own but only inside the cells of target tissues in the organism, resulting in the destruction of the cells or, in some cases, their transformation into cancer cells. While viruses have relatively low resistance in the environment, their ability to survive longer is based on environmental conditions and the type of substrate on which they are deposited. Recently, the potential for safe and efficient viral inactivation by photocatalysis has garnered increasing attention. In this study, the Phenyl carbon nitride/TiO2 heterojunction system, a hybrid organic–inorganic photocatalyst, was utilized to investigate its effectiveness in degrading the flu virus (H1N1). The system was activated by a white-LED lamp, and the process was tested on MDCK cells infected with the flu virus. The results of the study demonstrate the hybrid photocatalyst's ability to cause the virus to degrade, highlighting its effectiveness for safe and efficient viral inactivation in the visible light range. Additionally, the study underscores the advantages of using this hybrid photocatalyst over traditional inorganic photocatalysts, which typically only work in the ultraviolet range. Environmental cleaning and disinfection practices have been shown to reduce microorganism bioburden in the healthcare environment. This study was performed in four intensive care units in Thailand. Five high-touch surfaces were sampled before and after terminal manual cleaning and disinfection, and after pulsed xenon UV (PX-UV). Five nursing station sites were collected on a weekly basis before and after terminal manual cleaning. There were 100 patient rooms—50 rooms in the intervention arm and 50 rooms in the control arm—plus 32 nursing station sites. In the intervention arm, rooms with positive Gram-negative microorganisms were reduced by 50% after terminal manual cleaning and disinfection (p = 0.04) and 100% after PX-UV disinfection (p < 0.001). On five nursing station sites, colony counts of Gram-negative contamination decreased by 100% (p < 0.001) in the intervention arm while decreasing by 65.2% (p = 0.03) in the control arm after terminal manual cleaning and disinfection. The in-room time use was 15.6 min per room. A PX-UV device significantly reduced the level of Gram-negative microorganisms on high-touch surfaces in intensive care units. Chlorinated disinfectants are widely used in hospitals, COVID-19 quarantine facilities, households, institutes, and public areas to combat the spread of the novel coronavirus as they are effective against viruses on various surfaces. Medical facilities have enhanced their routine disinfection of indoors, premises, and in-house sewage. Besides questioning the efficiency of these compounds in combating coronavirus, the impacts of these excessive disinfection efforts have not been discussed anywhere. The impacts of chlorine-based disinfectants on both environment and human health are reviewed in this paper. Chlorine in molecular and in compound forms is known to pose many health hazards. Hypochlorite addition to soil can increase chlorine/chloride concentration, which can be fatal to plant species if exposed. When chlorine compounds reach the sewer/drainage system and are exposed to aqueous media such as wastewater, many disinfection by-products (DBPs) can be formed depending on the concentrations of natural organic matter, inorganics, and anthropogenic pollutants present. Chlorination of hospital wastewater can also produce toxic drug-derived disinfection by-products. Many DBPs are carcinogenic to humans, and some of them are cytotoxic, genotoxic, and mutagenic. DBPs can be harmful to the flora and fauna of the receiving water body and may have adverse effects on microorganisms and plankton present in these ecosystems. [ABSTRACT FROM AUTHOR]

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is the causative agent of coronavirus disease 2019, which has been a global pandemic. Since SARS-CoV-2 is transmitted through contaminated surfaces and aerosols, environmental disinfection is important to block the spread of the virus. Photocatalysts are attractive tools for virus inactivation and are widely used as air purifiers and coating materials. However, photocatalysts are inactive in the dark, and some of them need to be excited with light of a specific wavelength. Therefore, photocatalysts that can effectively inactivate SARS-CoV-2 in indoor environments are needed. Here, we show that a WO3 photocatalyst containing copper inactivated the SARS-CoV-2 WK-521 strain (Pango lineage A) upon irradiation with white light in a time- and concentration-dependent manner. Additionally, this photocatalyst also inactivated SARS-CoV-2 in dark conditions due to the antiviral effect of copper. Furthermore, this photocatalyst inactivated not only the WK-521 strain but also the Omicron variant BA.2. These results indicate that the WO3 photocatalyst containing copper can inactivate indoor SARS-CoV-2 regardless of the variant, in visible light or darkness, making it an effective tool for controlling the spread of SARS-CoV-2. Abstract Background No-touch environmental disinfection using ultraviolet devices has been highlighted in the past several years to control the transmission of multidrug-resistant organisms (MDROs). However, its effectiveness in non-US healthcare settings is yet to be examined. This study aimed to evaluate the effectiveness of disinfection by portable pulsed xenon ultraviolet (PX-UV) devices in controlling transmission of MDROs in a non-US healthcare setting. Methods All patients admitted in the intensive care unit in a 629-bed tertiary referral hospital in Japan from August 2016 to February 2019 were enrolled. During the study period, PX-UV disinfection was added to manual terminal cleaning after every patient transfer/discharge. For microbiological evaluation, surfaces were selected for sampling by contact plates before/after manual cleaning and after PX-UV. After overnight incubation, colonies on the plates were counted. Evidence has shown that the state of the patient care environment has a direct impact on heightening the risks of hospital-acquired infections among patients admitted in hospitals. Moreover in view of the suboptimal standard of cleanings by housekeeping staff, there has been the quest for a better approach to reliably disinfect environmental surfaces in health-care facilities. The ultraviolet light has been known for its antimicrobial property and has been used in water treatment, food processing, and in-duct cleaning of ventilations. A recent introduction of its use for surface decontamination has raised interest among health-care facilities. However, studies have shown that, in spite of its relative success in other applications, there is doubt in its efficacy in decontaminating shadowed areas of the room, and therefore, may not be seen as justifying its capital intensiveness.

This paper provides a novel and applicable work that builds a real system for disinfecting the air and surfaces of the environment in a hospital room, with a non-contact measurement system for supporting contagious disease treatments in hospitals. The system is built on an intelligent mobile robot system that operates autonomously in a simulated real treatment room. The research team uses a new positioning algorithm. It is a combination of data from the Lidar sensor, encoder, and Extended Kalman filter. The program that applies segmentation and image feature extraction algorithms is developed to meet requirements of real-time environment mapping in the room. Control algorithms for moving and avoiding obstacles are also proposed. Next, techniques for collecting health data including patient identification, body temperature, and blood oxygen index via wireless sensor network are also mentioned in the article. Analysis and experimental results show qualified outcomes and promise. The main contribution of the paper can be listed as follows.•Design and build a new CEE-IMR, an intelligent mobile robot that can regconize patients, guide and lead them walking in hospitals, especially keep a safe distance avoiding contagious deseases.•A novel framework for controlling the robot is proposed. Health care-associated infections can be reduced through better environmental disinfection practices. Central to effective disinfection is the environmental service worker (ESW). The aim of this systematic literature review was to identify factors associated with the disinfection practices of ESWs in health care settings. A systematic literature review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses principles. Ten online databases and search engines were selected for the review of English-language studies published between 2000 and 2019. Thirty-one (31) eligible studies were identified. Environmental disinfection was associated with many factors (eg, knowledge, perceptions) as well as ESW exposure to education/training sessions. When education/training was combined with performance evaluation/feedback, significant improvement in ESW disinfection practices was reported. To sustain improvements in disinfection practices, education/training sessions need to be continuous. The long-term commitment of an organization is also essential for continuous improvement in disinfection outcomes. While knowledge transfer is critical for behavior change, education/training also needs to focus on the skills and perceptions of ESWs. The coronavirus disease 2019 (COVID-19) originated in bats and human-to-human transmission through respiratory droplets and contact with surfaces of infected aerosol are the main ways of transmitting this virus. Until now, there is no effective pharmaceutical treatment; conclusively it is important to evaluate the types of applied disinfectants in different areas against Severe Acute Respiratory Syndrome Corona Virus 2 (SARS-CoV-2), the virus that causes COVID-19. The aim of this review was to summarize the information about environmental disinfection for preventing of COVID-19. We performed a review of the science direct database to find articles providing information on disinfection used for SARS-CoV-2. The extracted results were given the original data on inactivation coronaviruses by disinfectants in different places of health care facilities. The final search recognized that five groups of disinfectants include: chlorine containing disinfectants, alcohol, UV irradiation, Hydrogen peroxide, and other disinfectant were used against SARS-CoV-2 in different environments. Among these groups, bleach (chlorine containing disinfectants) has the most applicability. Also, in many studies by using disinfectants with 62–71% ethanol can reduce coronavirus in contaminated areas. Furthermore, after the using of operating room for COVID-19 patients, hydrogen peroxide and UV irradiation should be used for 24 h before it can be used again. The povidone-iodine or the chlorhexidine, could be recommended when there is a risk of SARS-CoV-2 contamination especially for open wounds. According to the different studies on SARS-CoV-2 disinfection, because of the SARS-CoV-2 can remain in the air and on surfaces, as well as observing individual disinfection guidelines in different hospital areas, disinfection of surfaces is necessary to decrease SARS-CoV-2 spreading. As an important reservoir for hospital-acquired infections, environmental surfaces have long been targeted by interventions to improve cleaning and disinfection. Differences in disinfection practices across US hospitals, however, are still unclear. We conducted a nationwide survey of environmental services (EVS) personnel in the United States regarding their environmental surface disinfection practices from January 2019 to June 2019. We developed and pilot tested the survey in conjunction with EVS specialists. Survey questions inquired about choices of disinfectants and cleaning equipment during daily and terminal disinfection of both contact isolation and non-contact isolation rooms. We contacted 273 EVS personnel by phone and email to participate in the survey. Fourty-seven EVS personnel representing different hospitals from 26 US states were included in the analysis. Hypochlorite (bleach) and quaternary ammonium compounds were the most frequently used disinfectants. Most respondents reported using microfiber-based cloths and mops to carry out disinfection. High-touch surfaces in contact isolation rooms were frequently disinfected using bleach (81%, n = 38); floors, however, were not disinfected as frequently in patient rooms. The vast majority of respondents reported use of sporicidal disinfectants for contact isolation rooms but not regular rooms. While frequently used to disinfectant contact isolation rooms, sporicidal agents are rarely used to disinfect regular rooms. Abstract Background Environmental disinfection with continuously antimicrobial surfaces could offer superior control of surface bioburden. We sought to decide the efficacy of photocatalyst antimicrobial coating in reducing methicillin-resistant Staphylococcus aureus (MRSA) acquisition in high incidence setting. Methods We performed prospective cohort study involving patients hospitalized in medical intensive care unit. A titanium dioxide-based photocatalyst was coated on high touch surfaces and walls. Five months of pre-intervention data were compared with five months of post-intervention data. The incidence rates of multidrug-resistant organism acquisition and the rates of hospital-acquired blood stream infection, pneumonia, urinary tract infection, and Clostridium difficile–associated diseases were compared using Cox proportional hazards regression analysis. Results In total, 621 patients were included. There was significant decrease in MRSA acquisition rate after photocatalyst coating (hazard ratio, 0.37; 95% confidence interval, 0.14–0.99; p = 0.04). However, clinical identification of vancomycin-resistant Enterococcus spp. and multidrug-resistant Acinetobacter baumannii did not decrease significantly. The hazard of contracting hospital-acquired pneumonia during the intervention period compared to baseline period was 0.46 (95% confidence interval, 0.23–0.94; p = 0.03). Conclusions In conclusion, MRSA rate was significantly reduced after photocatalyst coating. We provide evidence that photocatalyst disinfection can be an adjunctive measure to control MRSA acquisition in high-incidence settings. Trial registration ISRCTN Registry (ISRCTN31972004). This study aimed to evaluate the effectiveness of disinfection by portable pulsed xenon ultraviolet (PX-UV) devices in controlling transmission of MDROs in a non-US healthcare setting.Methods: All patients admitted in the intensive care unit in a 629-bed tertiary referral hospital in Japan from August 2016 to February 2019 were enrolled. During the study period, PX-UV disinfection was added to manual terminal cleaning after every patient transfer/discharge. For microbiological evaluation, surfaces were selected for sampling by contact plates before/after manual cleaning and after PX-UV. After overnight incubation, colonies on the plates were counted.Results: The incidence of newly acquired methicillin-resistant Staphylococcus aureus (MRSA) declined significantly (13.8 to 9.9 per 10,000 patient days, incidence rate ratio 0.71, p = 0.002), as well as that of newly acquired drug-resistant Acinetobacter (48.5 to 18.1, 0.37, p < 0.001). The percent reduction of the microbiological burden by manual cleaning was 81%, but a further 59% reduction was achieved by PX-UV.Conclusions: PX-UV is effective in further reducing the microbial burden and controlling MDROs in a non-US healthcare setting. Photocatalysis, as a sustainable and environmentally friendly green technology, has garnered widespread recognition and application across various fields. Especially its potential in environmental disinfection has been highly valued by researchers. This study commences with foundational research on photocatalytic disinfection technology and provides a comprehensive overview of its current developmental status. It elucidates the complexity of the interface reaction mechanism between photocatalysts and microorganisms, providing valuable insights from the perspectives of materials and microorganisms. This study reviews the latest design and modification strategies (Build heterojunction, defect engineering, and heteroatom doping) for photocatalysts in environmental disinfection. Moreover, this study investigates the research focuses and links in constructing photocatalytic disinfection systems, including photochemical reactors, light sources, and material immobilization technologies. It studies the complex challenges and influencing factors generated by different environmental media during the disinfection process. Simultaneously, a comprehensive review extensively covers the research status of photocatalytic disinfection concerning bacteria, fungi, and viruses. It reveals the observable efficiency differences caused by the microstructure of microorganisms during photocatalytic reactions. Based on these influencing factors, the economy and effectiveness of photocatalytic disinfection systems are analyzed and discussed. Finally, this study summarizes the current application status of photocatalytic disinfection products. The challenges faced by the synthesis and application of future photocatalysts are proposed, and the future development in this field is discussed. The potential for research and innovation has been further emphasized, with the core on improving efficiency, reducing costs, and strengthening the practical application of photocatalysis in environmental disinfection. A growing body of research has demonstrated that manual cleaning and disinfection of the operating room (OR) is suboptimal. Residual environmental contamination may pose an infection risk to the surgical wound. This study evaluates the impact of a visible-light continuous environmental disinfection (CED) system on microbial surface contamination and surgical site infections (SSI) in an OR. Samples from 25 surfaces within 2 contiguous ORs sharing an air supply were obtained after manual cleaning on multiple days before and after a visible-light CED system installation in 1 of the ORs. Samples were incubated and enumerated as total colony-forming units. SSIs in both ORs, and a distant OR, were tracked for 1 year prior to and 1 year after the visible-light CED system installation. There was an 81% (P =.017) and 49% (P =.015) reduction in total colony-forming units after the visible-light CED system installation in the OR in which the system was installed, and in the contiguous OR, respectively. In the OR with the visible-light CED system, SSIs decreased from 1.4% in the year prior to installation to 0.4% following installation (P =.029). A visible-light CED system, used in conjunction with manual cleaning, resulted in significant reductions in both microbial surface contamination and SSIs in the OR. [ABSTRACT FROM AUTHOR]

Background. Recently, several advanced technologies have been considered to reduce the microbial load in hospital environments and control Healthcare Associated Infections (HAIs) incidence. New strategies for preventing HAIs have continuously evolved, including enforcement of hygiene procedures by novel liquid biocides or no-touch technologies, self-disinfecting surfaces coated by heavy metals or light-activated photosensitizers such as Titanium Dioxide (TiO2) nanoparticles. Study design. Review publications concerning the use of photocatalytic systems in hospital setting, focusing on products based on TiO2. Methods. Specific keywords combinations were analitically searched in PubMed and Scopus databases. Results. Starting 80s-90s, over 2000 papers report "in vitro" studies on antimicrobial activity of TiO2 photocatalysis on several microorganisms including bacteria, viruses, fungi, yeasts, and antibiotic resistant strains. Besides, at least 4 selected papers addressed the potentials of this approach by "in field" studies, showing a widespread pool of applications in hospital and healthcare settings. However, the low number of available experiences and their heterogeneity represent major limitations to achieve a comprehensive final overview on effectiveness and feasibility of these technologies. Conclusions. Photocatalytic systems based on TiO2 represent a promising strategy for hospital hygiene and HAI prevention. Background The aim of this study was to evaluate the performance of a focused multivector ultraviolet (FMUV) system employing shadowless delivery with a 90-second disinfection cycle for patient care equipment inside and outside the operating room (OR) suite without manual-chemical disinfection. Methods A 5-point multisided sampling protocol was utilized to measure the microbial burden on objects inside and outside the OR environment in a 3-phase nonrandomized observational study. Surface sampling was performed pre- and postdisinfection in between cases (IBCs) to assess the performance of manual-chemical disinfection. FMUV system performance was separately assessed pre- and postdisinfection before the first case and IBCs. Additionally, visibly clean high-touch objects were sampled outside the OR, and the microbial burden reductions after FMUV disinfection were quantified without manual-chemical disinfection. Results Manual-chemical disinfection reduced the active microbial burden on sampled objects IBCs by 52.8%-90.9% (P <.05). FMUV reduced the active microbial burden by 92%-97.7% (P <.0001) before the firstcase and IBCs combined, and 96.3%-99.6% (P <.0001) on objects outside the OR without chemical disinfection. Conclusions Five-point multisided sampling proved effective for assessing disinfection performance on all exterior sides of equipment. FMUV produced significant overall reductions of the microbial burden on patient care equipment in all study phases and independent of manual cleaning and chemical disinfection. Wet surface biofilms are a potential reservoir for multidrug-resistant Gram-negative organisms, including carbapenemase-producing Enterobacteriaceae (CPE). Recognition of environmental sources is important in reducing secondary patient transmission. We report the increased detection of blaIMP-4+ CPE in environmental samples from floor drains in burns unit shower rooms, when collected following cleaning as compared to pre-cleaning. We propose that disruption of biofilms during cleaning may account for the increased detection of multi-resistant organisms. The results highlight the role of the wet environment as an under-recognized potential source of CPE transmission. There are many sources of contamination in the perioperative environment. Patient experience can be negatively affected by the presence of environmental contamination, especially if it is the cause of a surgical site infection. Perioperative and environmental services staff members and leaders are tasked with ensuring a clean and safe environment for their patients while maintaining an awareness of time and budgetary constraints. In addition, leaders are responsible for the competency of their staff members and must address performance issues when needed. New technological advances designed to streamline monitoring and reporting processes related to OR cleanliness are available for use. This article describes the quality improvement project that one multifacility organization completed related to the use of remote video auditing and the positive effect it had on the organization's environmental contamination. Environmental disinfection has become the new frontier in the ongoing battle to reduce the risk of health care–associated infections. Evidence demonstrating the persistent contamination of environmental surfaces despite traditional cleaning and disinfection methods has led to the widespread acceptance that there is both a need for reassessing traditional cleaning protocols and for using secondary disinfection technologies. Ultraviolet-C (UV-C) disinfection is one type of no-touch technology shown to be a successful adjunct to manual cleaning in reducing environmental bioburden. The dilemma for the infection preventionist, however, is how to choose the system best suited for their facility among the many UV-C surface disinfection delivery systems available and how to build a case for acquisition to present to the hospital administration/C-suite. This study evaluated specific infection control practices in community veterinary practices in southern Ontario. Environmental disinfection, management of infectious patients and antimicrobial use in clean surgical procedures were investigated. Community companion animal veterinary practices ( n = 101) in Southern Ontario were recruited, and a questionnaire was administered to one veterinarian and one veterinary technician from each practice. The veterinarian questionnaire gathered data on clinic demographics, management of infectious patients, infectious diseases of concern, environmental disinfection and antimicrobial use in surgical procedures. The veterinary technician questionnaire gathered data on environmental disinfection. None of the veterinary practices had a formal infection control programme. Sixty-five per cent ( n = 66) of the veterinary practices did not have an isolation area and 61% ( n = 40) of these practices did not employ any specific infection control measures for infectious cases. The products most frequently used for environmental disinfection were hydrogen peroxide based or quaternary ammonium compounds. Bleach was the agent most commonly used for environmental disinfection of infectious body fluids; however 60% of the veterinarians and 40% of the veterinary technicians did not identify a product for environmental disinfection of infectious body fluids. Twenty-four per cent of the veterinarians reported using antimicrobials in animals undergoing elective sterilization surgeries and 60% reported using antimicrobials in other clean surgical procedures. There is a need for community veterinary practices to develop infection control programmes specific to their individual practice. Background: Between June 1, 2004, and March 14, 2005, 16 patients in the surgical/medical intensive care unit (ICU) were infected and another 2 were colonized with multidrug-resistant (MDR) Acinetobacter baumannii. We describe the systematic investigation initiated to discover an environmental reservoir and a novel measure taken to terminate the outbreak. Methods: Cultures were taken from moist areas in the ICU, including sink traps, sink and counter surfaces, drains, and faucets. Strains were characterized using restriction endonuclease analysis. A weekly full drainpipe chase cleansing protocol with sodium hypochlorite (bleach) solution for all 24 ICU and waiting room area sinks connected by common plumbing was initiated in March 2005. Results: Eleven of 16 infected patients (69%) had a clonal MDR strain, 1 patient (6%) was infected with an unrelated strain, and in 4 patients (25%) strains were not available for typing. The reservoir for the A baumannii clone was detected in a sink trap within one of the ICU patient rooms that likely represented contamination of the entire horizontal drainage system. The bleaching protocol initiated in March 2005 successfully decontaminated the reservoir and eliminated the MDR A baumannii infections. Conclusion: A systematic search for an environmental reservoir followed by decontamination significantly reduced (P < .01) the incidence of MDR A baumannii infection. [Copyright &y& Elsevier]

The doffing of personal protective equipment (PPE) after contamination with pathogens such as Ebola poses a risk to health care workers. Pulsed xenon ultraviolet (PX-UV) disinfection has been used to disinfect surfaces in hospital settings. This study examined the impact of PX-UV disinfection on an Ebola surrogate virus on glass carriers and PPE material to examine the potential benefits of using PX-UV to decontaminate PPE while worn, thereby reducing the pathogen load prior to doffing. Ultraviolet (UV) safety and coverage tests were also conducted. PX-UV exposure resulted in a significant reduction in viral load on glass carriers and PPE materials. Occupational Safety and Health Administration–defined UV exposure limits were not exceeded during PPE disinfection. Predoffing disinfection with PX-UV has potential as an additive measure to the doffing practice guidelines. The PX-UV disinfection should not be considered sterilization; all PPE should still be considered contaminated and doffed and disposed of according to established protocols. [ABSTRACT FROM AUTHOR]

The authors reflect on the study that addresses the impact a simple intervention to improve hospital cleanliness. The authors suggest the need for hospitals to ensure that the basic components of environmental cleaning are followed by using surrogate markers of bacterial contamination to assist with education and feedback for environmental services staff. They argue that the study is an important reminder of the need to return to the roots of infection control.

The coronavirus disease 2019 (COVID-19) originated in bats and human-to-human transmission through respiratory droplets and contact with surfaces of infected aerosol are the main ways of transmitting this virus. Until now, there is no effective pharmaceutical treatment; conclusively it is important to evaluate the types of applied disinfectants in different areas against Severe Acute Respiratory Syndrome Corona Virus 2 (SARS-CoV-2), the virus that causes COVID-19. The aim of this review was to summarize the information about environmental disinfection for preventing of COVID-19. We performed a review of the science direct database to find articles providing information on disinfection used for SARS-CoV-2. The extracted results were given the original data on inactivation coronaviruses by disinfectants in different places of health care facilities. The final search recognized that five groups of disinfectants include: chlorine containing disinfectants, alcohol, UV irradiation, Hydrogen peroxide, and other disinfectant were used against SARS-CoV-2 in different environments. Among these groups, bleach (chlorine containing disinfectants) has the most applicability. Also, in many studies by using disinfectants with 62-71% ethanol can reduce coronavirus in contaminated areas. Furthermore, after the using of operating room for COVID-19 patients, hydrogen peroxide and UV irradiation should be used for 24 h before it can be used again. The povidone-iodine or the chlorhexidine, could be recommended when there is a risk of SARS-CoV-2 contamination especially for open wounds. According to the different studies on SARS-CoV-2 disinfection, because of the SARS-CoV-2 can remain in the air and on surfaces, as well as observing individual disinfection guidelines in different hospital areas, disinfection of surfaces is necessary to decrease SARS-CoV-2 spreading. Environmental disinfection greatly reduces the occurrence of nosocomial or healthcare associated infections (HCAIs) which are the major healthcare problems worldwide. In India, Ayurvedic traditional fumigation with natural plant products is used to disinfect environment. In the present study, environmental disinfection efficiency of traditional fumigation practice has been evaluated by using natural plant products such as garlic (Allium sativum) peel, turmeric (Curcuma longa) powder, Carom (Trachyspermum ammi) seeds (Ajwain) and Loban (resin of Styrax benzoin and Boswellia species). The efficiency of traditional fumigation using these natural products to disinfect air and surface was evaluated. The effect of traditional fumigation on the microbiological quality of air was revealed by active air sampling. In addition, the ability of the traditional fumigation using garlic peel to disinfect inanimate surface was evaluated using three strains of methicillin resistant Staphylococcus aureus (MRSA). Glass slide was artificially contaminated with the bacteria and fumigated whereas non-fumigated slide served as control. The control and fumigated slides were analyzed for surviving bacteria and subjected to scanning electron microscopy (SEM) analysis. Traditional fumigation performed separately with three grams of garlic peel, turmeric, carom seeds and loban powder reduced the average air borne bacterial colony forming units (cfu)/m 3 compared to non-fumigated control. The SEM analysis showed reduced number of bacteria in garlic peel fumigated surface samples. The results of the study strongly suggested that the traditional Ayurvedic fumigation with natural plant products is effective in reducing air-borne bacteria and in disinfecting inanimate surfaces. In a simultaneous assessment of 250 environmental surfaces after terminal cleaning using aerobic cultures as a gold standard, both fluorescent marker and an adenosine triphosphate bioluminescence assay system demonstrated better diagnosticity compared with subjective visual inspection. Background: Hospital equipment made from copper alloys can play an important role in complementing traditional methods of disinfection. Aims of the study: The aim of this study was to assess the dynamics of the antimicrobial properties of selected copper alloys in different simulations of environmental conditions (with organic contamination vs. without organic contamination), and to test alternatives to the currently used testing methods. Materials and Methods: A modification of Japanese standard JIS Z 2801 as well as Staphylococcus aureus (SA) and Escherichia coli (EC) suspended in NaCl vs. tryptic soy broth (TSB) were used in tests performed on seven commonly used copper alloys, copper, and stainless steel. Results: A much faster reduction of the bacterial suspension was observed for the inoculum prepared in NaCl than in TSB. A faster reduction for EC than for SA was observed in the inoculum prepared in NaCl. The opposite results were found for the inoculum based on TSB. A significant correlation between the copper concentration in the copper alloys and the time and degree of bacterial suspension reduction was only observed in the case of EC. Conclusions: This study confirmed the antimicrobial properties of copper alloys, and additionally showed that Staphylococcus aureus was more resistant than Escherichia coli in the variant of the experiment without organic contamination. However, even for SA, a total reduction of the bacterial inoculum's density took no longer than 2 h. Under conditions simulating organic contamination, all of the tested alloys were shown to have bactericidal or bacteriostatic properties, which was contrary to the results from stainless steel., Competing Interests: The authors declare no conflict of interest.
