Objetivo: evaluar los cambios en la tasa anual de sangrados (TAS) y el comportamiento del título de inhibidores en una serie de casos de pacientes pediátricos con hemofilia A severa e inhibidores de alta respuesta, quienes recibieron profilaxis con agentes bypass y posteriormente fueron cambiados a Emicizumab. Materiales y Métodos: estudio retrospectivo observacional crossover (antes y después) que evaluó el comportamiento de la TAS en una cohorte de pacientes que recibió durante 52 semanas agentes bypass y, posteriormente, esos mismos pacientes fueron cambiados a Emicizumab. Se revisaron de forma retrospectiva las historias clínicas de 4 pacientes con hemofilia A severa e inhibidores de alta respuesta en una institución de salud de Antioquia entre 2018 y 2020. La medición del título de inhibidores, se llevó a cabo por el método cromogénico bovino y la valoración radiológica fue realizada por medio de un equipo GE Logic E7 con dópler a color. La dosis de carga de Emicizumab fue 3 mg/kg semanal durante cuatro semanas y la dosis de mantenimiento fue 3 mg/kg cada dos semanas para tres pacientes y 6 mg/kg cada cuatro semanas en un paciente. Resultados: los 4 pacientes tenían una media de edad de 9 años (Ver Tabla 1). Previo al inicio de Emicizumab, tres pacientes estaban en profilaxis con Factor VII recombinante activado (rFVIIa) y uno con concentrado de complejo protrombínico activado (APCC), dos de los pacientes presentan genotipo F8 de inversión del intrón 22 y en dos no se realizó el estudio genético. Previo a Emicizumab, a los 4 pacientes se les realizó inducción a la inmunotolerancia (ITI), las cuales fueron fallidas. La cohorte tuvo una media del pico histórico de inhibidor de 275 UB/ml y una media del título de inhibidor de 200 UB/ml, previo al inicio con Emicizumab (Figura 1).

Resumen Objetivo: Comparar la composición corporal por antropometría clásica e impedancia bioeléctrica en estudiantado universitario de una universidad peruana. Método: Estudio correlacional, transversal, conformado por 152 estudiantes (77 varones y 75 mujeres) matriculados en el ciclo 2018-II con edad promedio de 20.45 ± 3.63 años. Se calculó para el método de antropometría el índice de masa corporal, mediciones antropométricas y se utilizó la ecuación de Yuhasz (1974) para determinar el porcentaje de grasa corporal, y para el de bioimpedancia eléctrica se utilizó el analizador tetrapolar multifrecuencia de medición segmental directa Inbody 120. Para establecer la correlación se utilizó el coeficiente de Pearson y para la concordancia se utilizó el método gráfico de Bland Altman e índice de estabilidad. Introducción: la esquizofrenia es una patología mental crónica que afecta a alrededor de 24 millones de personas en el mundo, dentro de esta enfermedad es común encontrar el consumo de sustancias psicoactivas por parte de los pacientes en las que el cannabis encabeza esta lista, esta droga tiene relaciones particulares con la esquizofrenia por medio del efecto de los cannabinoides propios de la planta. Objetivo: determinar cuál es la relación entre el consumo de cannabis y la esquizofrenia. Metodología: se realizó una revisión bibliográfica narrativa; para la búsqueda de información utilizándose bases de datos como Scopus y PubMed con descriptores MESH/DECS mediante palabras clave, teniendo en cuenta artículos desde el 2009 hasta el 2023, en idiomas inglés y español y con criterios de inclusión y exclusión de los artículos a analizar. Resultados: se determinó que el consumo de cannabis podía desencadenar un episodio psicótico en individuos vulnerables; se encontró que la transición a la psicosis se produjo entre quienes empezaron a consumir cannabis antes de los 15 años y continuaron con su consumo de manera frecuente. Los consumidores de cannabis no fueron más propensos a desarrollar psicosis que aquellos que nunca habían consumido cannabis. Conclusiones: el consumo de cannabis es uno de los factores de riesgo modificables con mayor importancia en el desarrollo de psicosis ya que este actúa como una causa componencial para el desarrollo de un evento psicótico que se enmarque dentro del espectro de la esquizofrenia.

Introduction: the role of magnetic resonance imaging (MRI) in heart failure (HF) is much discussed, both in the etiological and prognostic evaluation of this disease. Therefore, a description of the usefulness of MRI would show us its role in the evaluation of prognosis in patients with HF of various etiologies. Aim: to describe the usefulness of MRI for the evaluation of prognosis in patients with HF of various etiologies. Methodology: a narrative literature review was carried out by means of a search in databases such as PubMed, Scopus, Elsevier and Springer (BMC). Results: cardiac magnetic resonance (CMR) is superior to echocardiography in assessing left ventricular volumes and function with better reproducibility, also when analyzing systolic abnormalities and characterizing viable myocardial tissue, as well as that with the presence of myocardial fibrosis. Precisely, the detection of myocardial fibrosis has a predictive value for adverse events in patients with HF. Conclusion: CMR is useful to identify previously undetected pathology and pathologies that are difficult to diagnose. Thus, its use is associated with better diagnostic performance and, therefore, a better predictor of adverse events in patients with HF.

Introducción: Los ríos andinos en Colombia están fuertemente influenciados por diferentes actividades antrópicas. Los macroinvertebrados han sido utilizados para evaluar la calidad del agua en estos ecosistemas. Objetivo: Este estudio analizó el comportamiento de diferentes índices basados en macroinvertebrados para evaluar la calidad del agua del tramo medio-bajo del río Jordán en Jamundí (Colombia). Materiales y métodos: Se ubicaron tres estaciones de muestreo: E1 (1173 m.s.n.m.), E2 (1069 m.s.n.m.) y E3 (1019 m.s.n.m.). Se colectaron individuos en grava, bolos y macrófitas (n=15) y se midieron las concentraciones de variables fisicoquímicas y microbiológicas (n=15). Se evaluaron los índices de calidad de agua ICA-NSF y bióticos (BMWP/Univalle, ASPT y EPT) y los índices de Shannon-Weaver, Margalef y Simpson. Se realizó una correlación de Spearman entre las matrices de similitud biótica y ambiental. Resultados: El ICA-NSF presentó variaciones espacio temporales no significativas entre las estaciones de muestreo, con calidades de agua entre “regular” y “excelente”. Los índices de diversidad mostraron que la equidad y riqueza son mayores en la estación E1. El BMWP/Univalle evidenció la influencia de las diferentes actividades sobre la subcuenca del río Jordán, clasificando la estación E1 como “Buena”, E2 “Aceptable” y E3 “Dudosa”. Las variables que mejor explicaron el patrón de diversidad fueron las asociadas a la contaminación orgánica (DBO5 y Coliformes totales). Conclusiones: Integralmente, los índices muestran que la mejor calidad del agua se presenta en la estación E1 donde la influencia de las actividades antrópicas es menor.

A técnica de dopplerfluxometria é importante para aferir a hemodinâmica renal, auxiliando no diagnóstico e prognóstico de problemas renais, todavia estudos com animais silvestres ainda são escassos. Assim sendo, é de extrema importância o desenvolvimento de estudos com o intuito de estabelecer valores de referências normais nas diferentes espécies, uma vez que esses dados servirão como base para a compreensão da anatomia e fisiologia, bem como, diagnóstico de doenças na clínica veterinária. Neste sentido, este trabalho teve como objetivo obter dados dos aspectos ultrassonográficos renais normais em cutias (Dasyprocta prymnolopha, Wagler 1831). Para avaliação da morfometria ultrassonográfica utilizou-se oito cutias, nas quais foram avaliados o comprimento e largura dos rins direito e esquerdo. Para avaliação da dopplerfluxometria foram utilizadas 11 cutias, sendo analisadas as velocidades do pico sistólico máximo (VPS) e diastólico final (VDF) do fluxo sanguíneo (cm/s) e do índice de resistividade (IR) das artérias arqueadas dos rins direito e esquerdo. Os valores médios do comprimento e largura em centímetros do rim direito e esquerdo variaram entre 3,05 ± 0,13 a 1,84 ± 0,12 e 3,04 ± 0,19 a 1,91 ± 0,10, respectivamente. O valor médio observado para o índice de resistividade na artéria arqueada dos rins direito e esquerdo foram 0,41 ± 0,06 e 0,39 ± 0,05 cm/s, respectivamente. O índice de resistividade foi inferior aos observados em outras espécies de animais domésticos e até mesmo de humanos.

Hemophilia A is an X-linked bleeding disorder caused by mutations in the FVIII gene. Genetic factors have been shown to be a risk factor for the development of inhibitors. We aimed to identify the specific variations of the FVIII gene of patients with hemophilia A with inhibitors and their association with the inhibitor titer. Methods: Cross-sectional descriptive study. We included 12 Colombian patients from a health care provider, “Integral Solutions SD”, who underwent analysis of genetic material (DNA), which was reported by the Molecular Hemostasis Laboratory in Bonn, Germany. Results: All of these patients were diagnosed with severe hemophilia A with inhibitors; ages ranged between 6 and 48 years, with a median age of 13.5 years. Molecular analysis showed the inversion of intron 22 in six patients (50.0%), a small duplication in two patients (16.7%), the inversion of intron 1 in one patient (8.3%), a large deletion (8.3%), a nonsense mutation (8.3%) and a splice-site (8.3%), findings similar to those of other studies. A total of 58.3% of the patients presented inversion mutations with a high risk of developing inhibitors A total of 83.3% of the evaluated patients presented null mutations; however the presence of high inhibitor titers was 66.7%. The most frequent mutation was the inversion intron 22. Knowing the type of mutation and its association as a risk factor for generating inhibitors invites us to delve into other outcomes such as residual values of coagulation FVIII as well as its impact on the half-life of the exogenous factor applied in prophylaxis.

[Abstract] Waves with periods between 25 s and some minutes can amplify the motions of moored ships, which may result in terminal downtimes and compromise safety. The purpose of this work is twofold: (i) studying infragravity waves and their influence on moored ship motions, including the definition of operational thresholds and (ii) developing a novel and integrated approach to identify and assess resonance situations using wavelet transform analysis. The resonant modes of the harbor basin were identified using a numerical model validated with full-scale data. The motions of five similar LPG vessels moored at two adjacent jetties were analyzed, both in the frequency and frequency–time domains. It was concluded that surge is the most important motion at the berth that has greater operational problems. Moreover, the infragravity motion periods vary with the mooring line pretension and are proportional to the ratio of water depth to vessel draft. Three episodes of mooring line breaking were attributed to large infragravity surge oscillations. In addition, operational thresholds for basin-vessel resonance situations were defined based on the port tide gauge data. A significant wave height of 0.075 and 0.010 m for the 30–65 s period band was established for LPGs and oil tankers, respectively. The infection by the SARS-CoV-2 virus and the disease generated by COVID-19 that began in 2019 in Wuhan China has had a great impact on global morbidity and mortality, which in turn led to the creation in record time of the first vaccines used in humans against coronavirus. Vaccines such as Sinovac, Janssen, AstraZeneca, Pfizer and Moderna, of which have been authorized and administered in multiple countries with really favorable results. However, late post-vaccination adverse effects have been reported in patients vaccinated especially by Moderna and Pfizer, observing a compromise at the level of the skin causing the appearance in most cases of urticaria, edema, erythema, reactivation of herpes zoster, morbilliform eruptions, among others that are undoubtedly related to the mechanisms of action of the aforementioned vaccines.

Financiado para publicación en acceso aberto: Universidade da Coruña/CISUG [Abstract:] The coupling of low-frequency oscillatory modes of a harbor with horizontal motions of moored ships can amplify the latter, causing operational and safety problems. The objectives of this study were to analyze infragravity waves and their effect on moored ship motions; to validate the Wavelet Transform Analysis (WTA) method for studying port-ship resonance in the Outer Port of Punta Langosteira, Spain; and to study the climatic forcing of downtimes. The excited resonant modes of the basin (102 and 132 s) were determined using a numerical model and high-frequency water level measurements at different points of the harbor. Yaw and sway motions of three bulk carriers were analyzed in the frequency-time domain using WTA. The results showed that the oscillation periods of the studied vessel motions are directly related to the water depth to vessel draft ratio. In addition, when the wind speeds exceeds 20 km/h, the ship moves away from the fenders, which lead to a lengthening of these periods. Based on our findings, yaw may induce oscillations of the same period in sway. Additionally, operational thresholds were proposed based on outer and inner waves data that will lead to optimize ship stays, avoiding downtimes, and to advance the safety of loading and unloading operations. The dispersion of surface jets in crossflows such as rivers or channels can cause critical environmental problems in the form of chemical or thermal pollution of these water bodies. The turbulent flow structures occurring in such crossflows play an important role in the mixing of surface jets with the surrounding water bodies. In this study, experimental measurements of the time history of the 3-D velocity field were conducted to better understand the flow structure of surface jets in crossflow conditions. Stereoscopic Particle Image Velocimetry was used to measure the instantaneous spatial and temporal velocity distribution downstream of the jet’s discharge point. In addition to the mean velocity distribution, turbulent flow characteristics such as the turbulent kinetic energy ( $$k$$ ), turbulent kinetic energy dissipation rate ( $$\epsilon$$ ), and turbulent eddy viscosity ( $$\nu_{t}$$ ) were calculated. The formation and evolution of a vortex in the surface jet’s flow structure was detected over the measurement zone. The vortex in the surface jets in crossflow resembled to half of the vortices in a counter-rotating vortex pair (CVP) of submerged jets in crossflows. It can be inferred that the water surface performed like a plane of symmetry.

[Resumen] La metodología docente de las asignaturas de Puertos y Costas emplea las visitas a obra y al laboratorio CITEEC como una parte de sus herramientas fundamentales. El confinamiento generado por la pandemia del COVID-19 obligó a adaptar dicha metodología, sustituyendo dichas visitas por la exposición de videos de ensayos y de los fenómenos físicos explicados en clase. Por otro lado, fue necesaria una adaptación de la evaluación. Se aumentó el peso de la evaluación continua y se disminuyó el del examen final. Los resultados alcanzados mostraron que la exposición de videos no permitía que el alumnado comprendiera los fenómenos físicos de una forma tan clara como antes, y, además, las clases virtuales generaban una menor participación. Por último, aunque la modificación en la evaluación ha permitido aumentar la tasa de aprobados, esta generó una mayor dificultad para evaluar de una forma individual los conocimientos del alumnado. [Abstract] The teaching methodology of the Ports and Coasts subjects uses site visits and the CITEEC laboratory as some of their fundamental tools. The lockdown generated by the COVID-19 pandemic made to adapt this methodology necessary, replacing these visits with the exhibition of videos about experinmental models and physical phenomena explained in class. On the other hand, the evaluation method also needed to be adapted. The weight of the continuous evaluation was increased and the weight of the final exam was decreased. The results showed that the presentation of videos did not allow the students to understand the physical phenomena as clearly as before. In addition, participation was lower due to the virtual lessons. Finally, although the modification in the evaluation has allowed an increase in the pass rate, it made it more difficult to evaluate the knowledge of each student individually. http://hdl.handle.net/2183/28796

[Abstract] Several port authorities are involved in the R+D+i projects for developing port management decision-making tools. We recorded the movements of 46 ships in the Outer Port of Punta Langosteira (A Coruña, Spain) from 2015 until 2020. Using this data, we created neural networks and gradient boosting models that predict the six degrees of freedom of a moored vessel from ocean-meteorological data and ship characteristics. The best models achieve, for the surge, sway, heave, roll, pitch and yaw movements, a 0.99, 0.99, 0.95, 0.99, 0.98 and 0.98 R2 in training and have a 0.10 m, 0.11 m, 0.09 m, 0.9°, 0.11° and 0.15° RMSE in testing, all below 10% of the corresponding movement range. Using these models with forecast data for the weather conditions and sea state and the ship characteristics and berthing location, we can predict the ship movements several days in advance. These results are good enough to reliably compare the models’ predictions with the limiting motion criteria for safe working conditions of ship (un) loading operations, helping us decide the best location for operation and when to stop operations more precisely, thus minimizing the economic impact of cargo ships unable to operate. [Abstract] Minimizing the stay of a vessel in port can lead to improvements in harbor efficiency. Currently, downtimes of cargo operations or their performance reduction because of excessive vessel motion are especially relevant. This work aims to evaluate the operational conditions of moored vessels in the inner port of A Coruña (Spain), comparing them with motion thresholds established by international standards for cargo operations. To this end, motions of 19 ships were monitored. Data analysis revealed large angular motions, particularly roll and yaw, including amplitudes that exceeded the limiting criteria of the standards in most of the analyzed vessels, with no registered downtime. Regarding linear movements, heave and surge recorded lower amplitudes compared to the values of standard thresholds. The specific behavior of each vessel was analyzed in terms of its size, maritime conditions, and mooring location. Field campaigns such as those performed in this work are an effective way of analyzing the operational conditions of ports, which could help in identifying problems in the mooring zone. The aim of this study was to test equine semen cryopreservation techniques for the conservation of donkey germplasm. Ejaculates of three male donkeys were used (n = 18; six ejaculates per donkey; six repetitions), collected by the artificial vagina method. To remove the seminal plasma, the ejaculates were split and submitted to filtration or centrifugation methods. To assess the freezing method, each fraction was submitted to the automated system or the conventional system, and groups were formed: automated centrifuge, automated filtrate, conventional centrifuge and conventional filtrate. After thawing (37°C/30 seconds), were analyzed the sperm kinetic parameters, integrity and functionality of the plasma membrane and mitochondrial membrane potential. Highest sperm concentration (P.05) was observed in the filtrate groups; the conventional filtrate group presented lower (P.05) progressive motility and curvilinear velocity compared to the other groups; no difference was observed (P.05) among the groups for the membrane integrity and functionality, and mitochondrial membrane potential. Thus, centrifugation is the most indicated technique to remove donkey seminal plasma and the automated and conventional freezing methods can be used in donkey semen conservation.

This paper presents an analysis of the stability of breakwater roundheads protected with a Cubipod single-layer armor. This model was then subjected to a battery of 22 physical model tests, and the most significant variables were observed to be wave steepness (spr), the radius divided by the nominal diameter (Rn), and the relationship between the radius and wavelength at the breakwater toe (R/Lrp). RESUMEN El análisis de comunidades fitoplanctónicas utilizando el enfoque de grupos funcionales (GF), es empleado para predecir la respuesta de la composición de especies fitoplanctónicas a la variación de las condiciones ambientales. El objetivo de este estudio fue evaluar por primera vez bajo condiciones de trópico americano la dinámica de los grupos funcionales de fitoplancton asociados a un sistema de laguna algal de alta tasa fotosintética (LALAT), empleada para la biorremediación de lixiviados de rellenos sanitarios, utilizando la propuesta de grupos funcionales basada en la morfología (MBFG). Se tomaron muestras de agua para análisis biológico y fisicoquímico durante siete meses. Adicionalmente, se calculó el biovolumen del fitoplancton y se realizó un análisis de correlación canónica para explicar la relación entre las variables físicoquímicas y los grupos funcionales. La LALAT trató un lixiviado de edad intermedia caracterizado por altas concentraciones de nutrientes. Se identificaron cuatro MBFG (IV, V, VI y VII). El grupo V, conformado por fitoflagelados, fue dominante durante todo el periodo de estudio. Este grupo puede sobrevivir y proliferar en ambientes con altas concentraciones de materia orgánica. Además, contribuye a la transformación de los contaminantes presentes en el lixiviado. El aumento del biovolumen de los grupos V y VII se relacionó con los periodos en los cuales la laguna presentó los menores valores de carbono orgánico disuelto. Finalmente, se encontró que el enfoque de los MBGF puede ser útil para comprender la dinámica de la comunidad de fitoplancton en la LALAT utilizado para el tratamiento de lixiviados en rellenos sanitarios. ABSTRACT The functional group (FG) approach is used to predict the response of the composition of phytoplankton species to changes in the environmental conditions. We assessed for the first time the dynamics of phytoplankton communities in a High-Rate Algal Pond (HRAP), implemented for the bioremediation of landfill leachate, and using morphology-based functional groups (MBFG), under the American tropics conditions. These groups were later correlated with the physical and chemical variables of the system. Water samples were taken for biological, physical and chemical analysis for seven months. In addition, phytoplankton biovolumes were estimated and a canonical correlation analysis was performed to explain the relation between physical and chemical variables and FGs. Subsequently, the HRAP treated a medium-age leachate characterized by high concentrations of nutrients. Four MBFGs were identified (IV, V, VI and VII). Group V, conformed by phytoflagellates, was found to be dominant during the entire study period. This group proliferate in environments with high concentrations of organic matter, while contributing to the transformation of the contaminants present in the leachate. The largest biovolumes of Groups V and VII occurred during periods of low water levels and dissolved organic carbon. Finally, we show that the MBFG technique may be useful in understanding the dynamics of the phytoplankton community in the HRAP used for the treatment of landfill leachate.
