The study attempts to evaluate the efficacy and safety of contraindicated medications like enoxaparin and eptifibatide among dialysis patients undergoing percutaneous coronary intervention (PCI). The results indicate that in many cases use of such drugs led to an increased risk of in-hospital major bleeding.

A study was conducted to examine the phase 3 trials, which compares the efficacy and safety of rivaroxaban, an oral direct inhibitor of factor Xa with enoxaparin in patients undergoing total hip arthroplasty. Findings observed that 10-mg oral dose of rivaroxaban are more effective for extended thromboprophylaxis than 40-mg subcutaneous dose of enoxaparin in patients undergoing elective total hip arthroplasty.

Objective: To investigate efficacy and safety of enoxaparin for catheter-related arterial thrombosis in infants with congenital heart disease. Design: Prospective observational study. Setting: Pediatric Intensive Care and Cardiology Unit at the University Children's Hospital of Zurich. Patients: A cohort of 32 infants aged 0-12 months treated with enoxaparin for catheter-related arterial thrombosis from 2002 to 2005. Measurements: Dose requirements of enoxaparin, resolution of thrombosis by Doppler ultrasound, and bleeding complications. Results: Catheter-related arterial thrombosis was located in the iliac/femoral arteries in 31 (97%) infants and aorta in 1 infant, and was related to indwelling catheters and cardiac catheterization in 17 (53%) and 15 (47%) cases, respectively. Newborns required increased doses of enoxaparin to achieve therapeutic anti-FXa levels (mean 1.62 mg/kg per dose) compared with infants aged 2-12 months (mean 1.12 mg/kg per dose; p = 0.0002). Complete resolution of arterial thrombosis occurred in 29 (91%) infants at a mean of 23 days after initiation of enoxaparin therapy. Partial or no resolution was observed in 1 (3%) and 2 (6%) infants, respectively, at a mean follow-up time of 4.3 months. Bleeding complications occurred in 1 (3%) infant. Conclusion: Enoxaparin is efficient and safe for infants with congenital heart disease and catheter-related arterial thrombosis, possibly representing a valid alternative to the currently recommended unfractionated heparin. Unstable angina combined with non-ST-segment elevation myocardial infarction (UA/NSTEMI) acute coronary syndromes (ACS) is the most common manifestations of coronary heart disease that lead to death. More than twenty thousand patients were studied across the ACS spectrum, and it was found that enoxaparin is more effective than unfractionated heparin in preventing combined end point of death or myocardial infarction.

Tests were conducted to check the efficacy of the combination of enoxaparin and tirofiban in comparison with unfractionated heparin and tirofiban in patients with non-ST-elevation acute coronary syndromes (ACS). It was found that in patients receiving tirofiban and aspirin enoxaparin is a good alternative when compared to unfractionated heparin.

Enoxaparin has shown better results over unfractionated heparin in low-to-moderate risk patients with non-ST-segment elevation acute coronary syndromes (ACS) and tests were conducted to compare the outcomes of patients treated with either of the drugs. Enoxaparin was not found to be superior to unfractionated heparin, but it is safe and an effective alternative.

Design: Case report. Setting: The medical and surgical intensive care units of a tertiary care university teaching hospital. Patients: Two adult patients receiving enoxaparin developed retroperitoneal hematoma and abdominal compartment syndrome. Both patients became anuric and required high-dose intravenous fluids and vasopressors to maintain blood pressure. Intervention: Discontinuation of enoxaparin, followed by exploratory laparotomy and evacuation of the hematoma. Measurements and results: Immediate clinical improvement following evacuation of hematoma. Conclusions: High-risk patients receiving low-molecular-weight heparin should be identified and closely monitored to prevent serious bleeding complications. Heparin is known to reduce ischemic occurrences beyond that of aspirin in acute coronary setting and unfractionated heparin is known to have several limitations and inability to inhibit clot-bound thrombin. Trials show that enoxaparin is a good alternative to unfractionated heparin in regular acute coronary syndrome (ACS) treatment.
