1. Australian rental housing standards: institutional shifts to reprioritize the housing–health nexus

Abstract: ABSTRACTA large proportion of Australia’s housing stock offered for private rental or socially let is of poor quality, which has implications for residents’ health and well-being. This problem has arisen from historically weak regulation of housing standards and under-investment in public housing services, both features of Australia’s neoliberal housing regime. In this paper, we reviewed the institutional contexts of two policy settings used to address problems of housing quality: the Homes Act 2018 (UK) and the Healthy Homes Guarantee Act 2017 (Aotearoa–New Zealand). From these two case studies, five institutional shifts required to reprioritize the housing–health nexus in Australia were synthesized: (1) policy objectives should explicitly link housing conditions and health outcomes; (2) community awareness and sector advocacy should be harnessed to overcome complex structures of government; (3) policy approaches should foster collective social responsibility; (4) mandatory requirements should be made transparent and objective; and (5) robust protocols for tracking progress should be developed and applied.

In 2022, a collaboration of Australian universities and researchers commissioned a large survey of 22,550 private rental, social rental and homeowner households to build a data infrastructure on the household and demographic characteristics, housing quality and conditions in the Australian housing stock. This is the third and largest instalment in a national series of housing conditions data infrastructures.

Abstract: ObjectiveThe association of vitamin D status with osteoarthritis (OA) has been demonstrated previously. The current study was performed to examine the association of vitamin D status with oxidative stress markers and matrix metalloproteinases (MMPs) in patients with knee OA.MethodsThis case–control study was conducted on 124 subjects with mild to moderate knee OA and 65 healthy controls. Demographic data was collected from all participants at baseline. Serum levels of vitamin D as well as markers of oxidative stress including malondialdehyde (MDA), total oxidant status (TOS), superoxide dismutase (SOD), oxidative stress index (OSI), paraoxonase-1 (PON-1), glutathione peroxidase (GPX), catalase (CAT), and total antioxidant capacity (TAC) were evaluated for each participant. Furthermore, serum concentrations of MMP-1, MMP-3, MMP-13, and cartilage oligomeric matrix protein (COMP) were measured.ResultsThe results of the present study indicated that individuals with vitamin D insufficiency had higher levels of MDA, TOS, SOD, and OSI as well as lower levels of PON-1 and TAC. Abstract: Trunnionosis is emerging as an early mode of failure in conventional metal-on-polyethylene total hip arthroplasty. It is defined as wear or corrosion at the trunnion, the taper at the femoral head-neck interface. Trunnion wear can result in a variety of negative sequelae and, in severe cases, necessitate revision arthroplasty. We describe a 64-year-old man with a metal-on-polyethylene total hip arthroplasty who presented with a sensation of clunking in the hip. Initial imaging and laboratory studies were inconclusive, and the decision was made to monitor. Two years later, trunnion wear was detected on radiographs, presenting as an abnormal alignment of the femoral neck relative to the femoral head. Several case reports and series describe catastrophic total hip arthroplasty failure due to trunnionosis. However, few describe the radiographic signs of wear at the trunnion before gross failure. This early presentation is important to recognize to minimize patient morbidity and aid surgical planning.

9. A cross-sectional analysis of podiatrist-initiated review processes after issuing prescribed foot orthoses

Abstract: Background Foot orthoses are widely used in clinical practice to treat foot, lower limb and back pathology. As published information guiding the clinical use of foot orthoses is scarce, the aim of this study is to profile the review processes used by practicing podiatrists after issuing an orthotic device. Methods A cross-sectional observational study design formed the basis for a self-administered online questionnaire. The questionnaire was distributed through podiatry networks based in Australia. Results Two-hundred and thirty-eight practicing podiatrists participated in this study. Ninety-seven percent of respondents indicated that they would recommend a review appointment after the initial fitting of an orthotic device. Forty percent (n = 84) of respondents scheduled the first review appointment four weeks after the initial fitting, while 33% (n = 69) preferred a two-week review period. A second review consultation was standard practice for 32% (n = 68) or respondents, and were typically scheduled either two (23%, n = 12) or four (38%, n = 20) weeks after the initial review consultation. Annual review of orthotic devices was recommended by 64% (n = 123) of participants in the study, while 19% (n = 37) would suggest that yearly reviews were scheduled only if required. Conclusions Variation was identified in the orthotic review processes used by practicing podiatrists, although most respondents recommend a routine short-term review appointment for foot orthoses. It is not clear why practitioners adopt such varied approaches. In the absence of any clear evidence on this topic, it may be that the differing approaches to patient review reflect different philosophical perspectives regarding patient management.

10. Impact of a progressive stepped care approach in an improving access to psychological therapies service: An observational study.

Abstract: England's national Improving Access to Psychological Therapies (IAPT) programme advocates stepped care as its organizational delivery of psychological therapies to common mental health problems. There is limited evidence regarding the efficacy of stepped care as a service delivery model, heterogeneity of definition and differences in model implementation in both research and routine practice, hence outcome comparison in terms of effectiveness of model is difficult. Despite sound evidence of the efficacy of low intensity interventions there appears to be a perpetuation of the notion that severity and complexity should only be treated by a high intensity intervention through the continuation of a stratified care model. Yet no psychotherapy treatment is found to be more superior to another, and not enough is known about what works for whom to aid the matching of treatment decision. In the absence of understanding precise treatment factors optimal for recovery, it may be useful to better understand the impact of a service delivery model, and whether different models achieve different outcomes. This study aims to contribute to the discussion regarding the stepped care definition and delivery, and explores the impact on clinical outcomes where different types of stepped care have been implemented within the same service. An observational cohort study analysed retrospective data (n = 16,723) over a 4 year period, in a single IAPT service, where delivery changed from one type of stepped care model to another. We compared the outcomes of treatment completers with a stratified care model and a progression care model. We also explored the assumption that patients who score severe on psychological measures, and therefore are potentially complex, would achieve better outcomes in a stratified model. Outcomes in each model type were compared, alongside baseline factor variables. A significant association was observed between a recovery outcome and model type, with patients 1.5 times more likely to recover in the progression delivery model. The potential implications are that with a progression stepped care model of service delivery, more patients can be treated with a lower intensity intervention, even with initial severe presentations, ensuring that only those that need high intensity CBT or equivalent are stepped up. This could provide services with an effective clinical model that is efficient and potentially more cost effective.

11. Does employment security modify the effect of housing affordability on mental health?

Abstract: This paper uses longitudinal data to examine the interrelationship between two central social determinants of mental health – employment security and housing affordability.Data from ten annual waves of the longitudinal Household, Income and Labour Dynamics in Australia (HILDA) survey (which commenced in 2000/1 and is ongoing) were analysed using fixed-effects longitudinal linear regression. Change in the SF-36 Mental Component Summary (MCS) score of working age individuals (25–64 years) (51,885 observations of 10,776 people), associated with changes in housing affordability was examined. Models were adjusted for income, age, survey year, experience of serious injury/illness and separation/divorce. We tested for an additive interaction between the security of a household's employment arrangements and housing affordability.People in insecurely employed households appear more vulnerable than people in securely employed households to negative mental health effects of housing becoming unaffordable. In adjusted models, people in insecurely employed households whose housing became unaffordable experienced a decline in mental health (B=−1.06, 95% CI −1.75 to −0.38) while people in securely employed households experienced no difference on average.To progress our understanding of the Social Determinants of Health this analysis provides evidence of the need to bridge the (largely artificial) separation of social determinants, and understand how they are related.

Abstract: Autism spectrum disorder (ASD) is a perplexing and pervasive developmental disorder characterized by social difficulties, communicative deficits, and repetitive behavior. The increased rate of ASD diagnosis has raised questions concerning the genetic and environmental factors contributing to the development of this disorder; meanwhile, the cause of ASD remains unknown. This study surveyed mothers of ASD and non-ASD children to determine possible effects of labor and delivery (L&D) drugs on the development of ASD. The survey was administered to mothers; however, the results were analyzed by child, as the study focused on the development of autism. Furthermore, an independent ASD dataset from the Southwest Autism Research and Resource Center was analyzed and compared. Indeed, L&D drugs are associated with ASD ( p = .039). Moreover, the Southwest Autism Research and Resource Center dataset shows that the labor induction drug, Pitocin, is significantly associated with ASD ( p = .004). We also observed a synergistic effect between administrations of L&D drugs and experiencing a birth complication, in which both obstetrics factors occurring together increased the likelihood of the fetus developing ASD later in life ( p = .0003). The present study shows the possible effects of L&D drugs, such as Pitocin labor-inducing and analgesic drugs, on children and ASD.

Abstract: In Australia, an increasing number of households face problems of access to suitable housing in the private market. In response, the Federal and State Governments share responsibility for providing housing assistance to these, mainly low-income, households. A broad range of policy instruments are used to provide and maintain housing assistance across all housing tenures, for example, assisting entry into homeownership, providing affordability assistance in the private rental market, and the provision of socially owned and managed housing options. Underlying each of these interventions is the premise that secure, affordable, and appropriate housing provides not only shelter but also a number of nonshelter benefits to individuals and their households. Although the nonshelter outcomes of housing are well acknowledged in Australia, the understanding of the nonshelter outcomes of housing assistance is less clear. This paper explores nonshelter outcomes of three of the major forms of housing assistance provided by Australian governments—low-income mortgage assistance, social housing, and private rent assistance. It is based upon analysis of a survey of 1,353 low-income recipients of housing assistance, and specifically measures the formulation of health and well-being, financial stress, and housing satisfaction outcomes across these three assistance types. We find clear evidence that health, finance, and housing satisfaction outcomes are associated with quite different factors for individuals in these three major housing assistance types.

Abstract: Housing inequality is far more than a housing matter. To discover how housing inequality has been used across disciplines, and how this may inform future housing research, we performed a systematic scoping review. We found that housing inequality provides multiple understandings as well as a variety of uses, for example, as a measurement tool, a conceptual device, or as subject matter. To draw together useful lessons from this conceptually diverse body of work, we identify four principle uses of ‘housing inequality’ in the literature – an outcome, an experience, a product, and a construct. These four framings offer a level of conceptual clarity for thinking about, and researching, the different expressions of housing inequality. 20. Social Provisioning in Crisis: A Feminist Economic Analysis of the Federal Policy Response to the 2008 Economic Crisis in Canada

Abstract: The 2008 economic crisis and subsequent recession in Canada was among the worst since the Great Depression. Mainstream economists have credited Canada’s policy response to the crisis in avoiding the worst of its effects. However, many Canadians continued to experience the aftershocks of the crisis long after economists considered it over. Feminist economics challenges the foundational assumptions and values of mainstream economics and defines economics as social provisioning – how we meet our human needs for sustenance. Using social provisioning as an analytical framework, this paper demonstrates that Canada’s federal policy response to the 2008 economic crisis was firmly entrenched within a capitalist, neoliberal political economy that reproduces relations of inequality, and that the determination of what constitutes an economic crisis is informed by that inequality. This analysis reveals both an alternative policy response to crisis, and a caution for future crises if past mistakes are repeated.

21. Sustainable social housing retrofit? Circular economy and tenant trade-offs

Abstract: This research examines the preferences and trade-offs of tenants during social housing retrofit programs, particularly in regard to implementing circular economy (CE) practices. The study looks beyond the relatively narrow consideration of energy efficiency, to respond to the broader requirements of the social housing sector—to incorporate and balance tenant needs with provider mandates, budgetary limitations, and wider social policy.Retrofitting, or upgrading, existing social housing stock has been proposed as a cost-efficient solution to concerns around energy efficiency, thermal performance, and quality issues.The research found that households’ preferences for housing retrofit and upgrade options did not necessarily align with evidence of optimal retrofit priorities and do not align with the typical activities which receive government funding. For example, commonly provided retrofit measures (such as draft sealing) were not widely valued, while less common interventions that were less focussed on energy efficiency (such as a deep clean) were highly regarded by consumer households.The objectives underlying retrofit programs are rarely explicit and vary greatly between stakeholders: social housing providers may be largely motivated to assist their tenants to avoid energy poverty; industry groups seem principally focussed on sustainability outcomes; and many tenants’ main motivation is wanting homes that are more liveable, efficient, clean and warm. These different, and often competing, objectives obviously limit successful outcomes. Retrofit and quality improvements that are undertaken with a short term focus, based on whatever funding or opportunities are available at that point in time, constrains all stakeholders from longer term planning or strategic coordination, but also reduces the opportunity to use CE principles in retrofit activities.

Abstract: Refereed/Peer-reviewed The social housing sector provides housing to some of society's most vulnerable people, disproportionately housing people with disabilities and chronic health conditions, the aged and people unable to work. These groups are often more susceptible to health impacts from poor temperature conditions within their home. In this paper, we examine temperature conditions in Australian social housing, explore tenant experiences and reflect on possible remediation responses. Using a novel contact-free delivery protocol for data collection, temperature was measured in 36 social housing dwellings over a 3-month springtime period. Semistructured interviews were conducted with occupants to better understand their experience of (adverse) indoor temperature conditions. On average, participants spent 35 per cent of time across the study period in temperatures outside the WHO guidelines (18–24°C). Most participants perceived their homes to be cold or very cold during periods of cold weather, and many considered energy unaffordable. Building conditions, such as poor sealing around windows and doors, lack of insulation and inadequacy of space heating appliances, were of greatest concern to participants. Participants' preferences for remediation work suggest that considerable benefit could be gained from making homes more energy efficient through draft sealing and insulation.

25. IMPROVING CARDIOVASCULAR DISEASE RISK PREDICTION WITH MACHINE LEARNING USING MENTAL HEALTH DATA: A PROSPECTIVE UK BIOBANK STUDY

Abstract: BackgroundRobust and accurate prediction of cardiovascular disease (CVD) risk facilitates early intervention to benefit patients. It is well-known that mental disorders and CVD are interrelated. Nevertheless, psychological factors are not considered in existing models, which use either a limited number of clinical and lifestyle factors, or have been developed on restricted population subsets.ObjectivesTo assess whether inclusion of psychological data could improve CVD risk prediction in a new machine learning (ML) approach.MethodsUsing a comprehensive, long-term UK Biobank dataset (n=375,145), we examined the correlation between CVD diagnoses and traditional and psychological risk factors. An ensemble ML model containing five constituent algorithms [decision tree, random forest, XGBoost, support vector machine (SVM), and deep neural network (DNN)] was tested for its ability to predict CVD risk based on two training datasets: one using traditional CVD risk factors alone, or a combination of traditional and psychological risk factors.ResultsOur ensemble ML model could predict CVD with 71.31% accuracy using traditional CVD risk factors alone. However, by adding psychological factors to the training data, accuracy dramatically increased to 85.13%. The accuracy and robustness of our ensemble ML model outperformed all five constituent learning algorithms. 26. Housing affordability and mental health: an analysis of generational change

Abstract: Unaffordable housing has many dimensions, not least its far-reaching implications for mental health. Although the psycho-social effects of housing affordability stress are well documented there is a lack of research on their variation within or between cohorts who have shared experiences of housing (social generations). This article fills that gap by following 14,000 Australians in the national Household, Income and Labour Dynamics survey for 16-years as they enter and exit unaffordable housing. We model when cohorts seem most vulnerable to mental health effects of unaffordable housing. We find contemporaneously that while people born in the 1980s have a high likelihood of falling below the affordability threshold, older people have a lower likelihood of recovering. These trends create a ‘pinch point’ for this older generation with negative mental health consequences. We position housing affordability stress as an indicator of precarity whose mental health effects may vary both within cohorts and between generations as a product of their shared experiences of housing.

Abstract: The FMR1 premutation (PM:55-199 CGG) is associated with fragile X-associated tremor/ataxia syndrome (FXTAS) and when maternally transmitted is at risk of expansion to a hypermethylated full mutation (FM: ≥ 200 CGG) that causes fragile X syndrome (FXS). We describe a maternally transmitted PM (77 CGG) that was passed to a son (103 CGG), and to a daughter (220-1822 CGG), who were affected with FXTAS and FXS, respectively. The male with the PM showed low-level mosaicism for normal size of 30 and 37 CGG. This male had two offspring: one female mosaic for PM and FM (56, 157, >200 CGG) and another with only a 37 CGG allele detected in multiple tissues, neither with a clinical phenotype. The female with the 37 CGG allele showed normal levels of FMR1 methylation and mRNA and passed this 37 CGG allele to one of her daughters, who was also unaffected. These findings show that post-zygotic paternal retraction can lead to low-level mosaicism for normal size alleles, with these normal alleles being functional when passed over two generations.

Abstract: The study characterised differences in costs associated with raising a child between four rare disorders and examined the associations between these costs with clinical severity. Caregivers of 108 individuals with Prader-Willi, Angelman (AS), Chromosome 15q Duplication and fragile X (FXS) syndromes completed a modified Client Services Receipt Inventory and participants completed intellectual/developmental functioning and autism assessments. AS incurred the highest yearly costs per individual ($AUD96,994), while FXS had the lowest costs ($AUD33,221). Intellectual functioning negatively predicted total costs, after controlling for diagnosis. The effect of intellectual functioning on total costs for those with AS was significantly different to the other syndromes. The study highlights the significant costs associated with these syndromes, particularly AS, linked with severity of intellectual functioning.

Abstract: Background: Many patients with COVID-19 have been treated with plasma containing anti-SARS-CoV-2 antibodies. We aimed to evaluate the safety and efficacy of convalescent plasma therapy in patients admitted to hospital with COVID-19. Methods: This randomised, controlled, open-label, platform trial (Randomised Evaluation of COVID-19 Therapy [RECOVERY]) is assessing several possible treatments in patients hospitalised with COVID-19 in the UK. The trial is underway at 177 NHS hospitals from across the UK. Eligible and consenting patients were randomly assigned (1:1) to receive either usual care alone (usual care group) or usual care plus high-titre convalescent plasma (convalescent plasma group). The primary outcome was 28-day mortality, analysed on an intention-to-treat basis. The trial is registered with ISRCTN, 50189673, and ClinicalTrials.gov, NCT04381936. Findings: Between May 28, 2020, and Jan 15, 2021, 11558 (71%) of 16287 patients enrolled in RECOVERY were eligible to receive convalescent plasma and were assigned to either the convalescent plasma group or the usual care group. There was no significant difference in 28-day mortality between the two groups: 1399 (24%) of 5795 patients in the convalescent plasma group and 1408 (24%) of 5763 patients in the usual care group died within 28 days (rate ratio 1·00, 95% CI 0·93–1·07; p=0·95). The 28-day mortality rate ratio was similar in all prespecified subgroups of patients, including in those patients without detectable SARS-CoV-2 antibodies at randomisation. Allocation to convalescent plasma had no significant effect on the proportion of patients discharged from hospital within 28 days (3832 [66%] patients in the convalescent plasma group vs 3822 [66%] patients in the usual care group; rate ratio 0·99, 95% CI 0·94–1·03; p=0·57). Among those not on invasive mechanical ventilation at randomisation, there was no significant difference in the proportion of patients meeting the composite endpoint of progression to invasive mechanical ventilation or death (1568 [29%] of 5493 patients in the convalescent plasma group vs 1568 [29%] of 5448 patients in the usual care group; rate ratio 0·99, 95% CI 0·93–1·05; p=0·79). Interpretation: In patients hospitalised with COVID-19, high-titre convalescent plasma did not improve survival or other prespecified clinical outcomes. Abstract: Refereed/Peer-reviewed Australia is considered by many to be a warm climate country and hence winter cold and its health effects are often overlooked. The majority of the Australian population live in temperate climate regions, which are heating-dominated and experience cold wintertime conditions. The prevalence of cold in Australian homes has to date been rarely measured or estimated, and the few studies that do are based on proxy data that estimate a low prevalence (around 5 %). This paper tests these proxy measures, using high resolution temperature data on indoor temperature from 100 homes across temperate Australia. The data were assessed using the World Health Organization's Housing and Health guideline for minimum indoor temperature (18 °C), which provides an internationally recognised benchmark for defining cold in homes. Across the sampled homes, 81 % were below 18 °C on average across the whole of winter (June–August 2022). Average winter indoor temperatures were 16.5 ± 2.7 °C across all homes, with no significant difference between locations. These early findings suggest that the problem of unhealthily cold homes in Australia is likely to be significantly more prevalent than previously estimated. Far from affecting 5 % of Australian households, the affected population may be 10 times this value. These initial findings have important implications for how we model health impacts and develop policy. This early release data from a recently commenced large environmental monitoring project is of timely importance. It challenges our understanding of the prevalence of cold housing in Australia context, flagging the pressing need to increase policy attention in advance of winter.

Abstract: Hematopoiesis is extrinsically controlled by cells of the bone marrow microenvironment, including skeletal lineage cells. The identification and subsequent studies of distinct subpopulations of maturing skeletal cells is currently limited because of a lack of methods to isolate these cells. We found that murine Lin–CD31–Sca-1–CD51+ cells can be divided into 4 subpopulations by using flow cytometry based on their expression of the platelet-derived growth factor receptors ⍺ and β (PDGFR⍺ and PDGFRβ). The use of different skeletal lineage reporters confirmed the skeletal origin of the 4 populations. Multiplex immunohistochemistry studies revealed that all 4 populations were localized near the growth plate and trabecular bone and were rarely found near cortical bone regions or in central bone marrow. Functional studies revealed differences in their abundance, colony-forming unit–fibroblast capacity, and potential to differentiate into mineralized osteoblasts or adipocytes in vitro. Furthermore, the 4 populations had distinct gene expression profiles and differential cell surface expression of leptin receptor (LEPR) and vascular cell adhesion molecule 1 (VCAM-1). Interestingly, we discovered that 1 of these 4 different skeletal populations showed the highest expression of genes involved in the extrinsic regulation of B lymphopoiesis. This cell population varied in abundance between distinct hematopoietically active skeletal sites, and significant differences in the proportions of B-lymphocyte precursors were also observed in these distinct skeletal sites. This cell population also supported pre-B lymphopoiesis in culture. Our method of isolating 4 distinct maturing skeletal populations will help elucidate the roles of distinct skeletal niche cells in regulating hematopoiesis and bone.

Abstract: Fragile X syndrome (FXS) is caused by CGG expansions of ≥200 repeats (full mutation: FM). Typically, FM causes abnormal methylation of the FMR1 promoter and silencing of FMR1, leading to reduction of FMRP, a protein essential for normal neurodevelopment. However, if unmethylated, these alleles cause over-expression of FMR1 mRNA which has been associated with Fragile X Tremor and Ataxia Syndrome (FXTAS), a late onset disorder. This report details the molecular and clinical profile of an asymptomatic male (29 years) identified as a result of cascade testing who was found to have a rare unmethylated FM (UFM) allele, as well as premutation (PM: 55-199 CGG) size alleles in multiple tissues. Full-scale IQ was within the normal range and minimal features of autism were observed. Southern blot analysis identified FM smears in blood (220-380 CGG) and saliva (212-378 CGG). A PM of 159 CGG was identified in blood and saliva. FMR1 promoter methylation analysis showed all alleles to be unmethylated. FMR1 mRNA levels were greater than fivefold of median levels in typically developing controls and males with FXS mosaic for PM and FM alleles. Issues raised during genetic counseling related to risk for FXTAS associated with UFM and elevated FMR1 mRNA levels, as well as, reproductive options, with implications for future practice.

Abstract: Since the COVID-19 pandemic began, there has been a reported surge of ageism toward older adults. Research demonstrates that events perpetuating negative attitudes towards older adults can increase ageism and associated negative outcomes. The purpose of this phenomenological qualitative study was to explore how older adults navigated experiences of ageism and their social relationships during the COVID-19 pandemic. Semi-structured interviews with adults ages 60 and older were conducted between February and April of 2021 over Zoom. Data were coded using an iterative, inductive approach and thematic analysis was performed to draw themes from the data. A total of 24 participants ages 61-80 (mean = 70.6) were interviewed. Most participants identified as white (n = 19) female (n = 14), retired (n = 21) and had at least a bachelor’s degree (n = 22). Findings showed that participants experienced ongoing ageism but did not report ageist experiences associated specifically with COVID-19. Ageist experiences, unrelated to COVID-19, as shared by participants included assumptions about older adults’ (in)ability to use technology, ageism in professional settings, and feelings that ageism is an inevitable part of growing older. Future research should examine the impact of intersectionality on this topic within more diverse populations and explore potential differences that may have occurred throughout different stages of the pandemic.

Abstract: Often included within ‘high-risk sibling’ studies, the Autism Observation Scale for Infants (AOSI) has only one independent replication study and no evaluation with community-ascertained cohorts. We administered the AOSI and established clinical measures with 103 infants (68% male) at ‘high autism likelihood’ on the Social Attention and Communication Surveillance - Revised (SACS-R) tool, at 9–14 months of age and again 6 months later. AOSI Total scores showed adequate internal consistency and strong inter-rater agreement (live- or video-coded) and were approximately normally distributed at each visit. Modest significant associations presented between Time 1 AOSI scores and concurrent developmental/adaptive skills measures. Concurrent associations were stronger at Time 2, particularly between AOSI Total and Autism Diagnostic Observation Schedule (ADOS) Social Affect scores. AOSI scores were only moderately associated across Time 1 and 2 assessments, as were Time 1 AOSI with Time 2 ADOS scores. These data from a clinically indicated cohort broadly replicate previous AOSI validity accounts from ‘high-risk sibling’ studies, particularly beyond the first year. Strong inter-rater agreement indicates viable AOSI inclusion within protocols necessitating blinded evaluation (e.g. intervention trials). Moderate within-participant stability suggests that, like ‘high-risk siblings,’ community-ascertained infants experience variable early trajectories. Lay Abstract We investigated whether a commonly used research assessment – the Autism Observation Scale for Infants (AOSI) – accurately measures autism behaviours among infants showing early signs of autism identified within the community. The AOSI is often included in studies tracking the development of infants at increased likelihood of autism, such as the infant siblings of diagnosed children. However, the suitability of this measure has not previously been tested with community-referred infants. We administered the AOSI with infants when aged 9 to 14 months and again 6 months later. Our researchers – independent of the AOSI development team and newly trained on this measure – were able to administer the brief interactive assessment and score it accurately. The infants’ AOSI scores were linked to their scores on other established and validated clinical assessments, particularly at the second visit when average age was 18 months. Stronger correspondence of AOSI and other scores at this second visit suggests early autism behaviours are better established and more consistent by 18 months of age, even though these infants showed clear enough signs of possible autism to prompt referral to our study around 12 months of age. However, the moderate association of AOSI scores over time suggests that, like infant siblings – who mostly do not develop autism – community-identified infants showing early signs may also have variable developmental pathways in early life.

Abstract: When housing is insufficient, or poor quality, or unaffordable there are well established health effects. Despite the pervasiveness of housing affordability problems (widely referred to as Housing Affordability Stress—HAS), little quantitative work has analysed long-term mental health effects. We examine the mental health effects of (prolonged and intermittent) patterns of exposure to housing affordability problems. We analysed a large, nationally representative longitudinal population sample of individuals, following them over five-year periods to assess the relative mental health effects of different patterns of exposure to housing affordability problems. To maximise the number of observations and the robustness of findings, we used 15 years (2002–2016) of data, across three pooled exposure windows. Longitudinal regression analysis with Mundlak adjustment was used to estimate the association between prolonged (constant over a 5-year period) and intermittent exposure to HAS, and mental health (as measured using the SF-36 MCS). We found that, on average, both prolonged and intermittent exposure were associated with lower mental health (Beta = − 1.338 (95% CI − 2.178–0.488) and Beta = − 0.516 (95% CI − 0.868–0.164), respectively). When we additionally adjusted for baseline mental health, thereby accounting for initial mental health status, coefficients were attenuated but remained significant. Both prolonged and intermittent exposure to HAS negatively impact mental health, irrespective of baseline mental health. Interventions that target affordable housing would benefit population mental health. Mental health interventions should be designed with people’s housing context in mind.

Abstract: Refereed/Peer-reviewed Recent crises have underscored the importance that housing has in sustaining good health and, equally, its potential to harm health. Considering this and building on Howden-Chapman's early glossary of housing and health and the WHO Housing and Health Guidelines, this paper introduces a range of housing and health-related terms, reflecting almost 20 years of development in the field. It defines key concepts currently used in research, policy and practice to describe housing in relation to health and health inequalities. Definitions are organised by three overarching aspects of housing: affordability (including housing affordability stress (HAS) and fuel poverty), suitability (including condition, accessibility and sustainable housing) and security (including precarious housing and homelessness). Each of these inter-related aspects of housing can be either protective of, or detrimental to, health. This glossary broadens our understanding of the relationship between housing and health to further promote interdisciplinarity and strengthen the nexus between these fields.
