Interstitial lung abnormalities (ILA) can be incidentally detected in patients undergoing low-dose CT screening for lung cancer. In this retrospective study, we explore the downstream impact of ILA detection on interstitial lung disease (ILD) diagnosis and treatment. Using a targeted approach in a lung cancer screening programme, the rate of de novo ILD diagnosis was 1.5%. The extent of abnormality on CT and severity of lung function impairment, but not symptoms were the most important factors in differentiating ILA from ILD. Disease modifying therapies were commenced in 39% of ILD cases, the majority being antifibrotic therapy for idiopathic pulmonary fibrosis.

Objectives An increase in lung nodule volume on serial CT may represent true growth or measurement variation. In nodule guidelines, a 25% increase in nodule volume is frequently used to determine that growth has occurred; this is based on previous same-day, test–retest (coffee-break) studies examining metastatic nodules. Whether results from prior studies apply to small non-metastatic nodules is unknown. This study aimed to establish the interscan variability in the volumetric measurements of small-sized non-metastatic nodules. Methods Institutional review board approval was obtained for this study. Between March 2019 and January 2021, 45 adults (25 males; mean age 65 years, range 37–84 years) with previously identified pulmonary nodules (30–150 mm3) requiring surveillance, without a known primary tumour, underwent two same-day CT scans. Non-calcified solid nodules were measured using commercial volumetry software, and interscan variability of volume measurements was assessed using a Bland–Altman method and limits of agreement. Results One hundred nodules (range 28–170 mm3; mean 81.1 mm3) were analysed. The lower and upper limits of agreement for the absolute volume difference between the two scans were − 14.2 mm3 and 12.0 mm3 respectively (mean difference 1.09 mm3, range − 33–12 mm3). The lower and upper limits of agreement for relative volume difference were − 16.4% and 14.6% respectively (mean difference 0.90%, range − 24.1–32.8%). Conclusions The interscan volume variability in this cohort of small non-metastatic nodules was smaller than that in previous studies involving lung metastases of varying sizes. An increase of 15% in nodule volume on sequential CT may represent true growth, and closer surveillance of these nodules may be warranted. Identifying false-negative cases is an important quality metric in lung cancer screening, but it has been infrequently and variably reported in previous studies. Although as a proportion of all screening participants, false-negative cases are uncommon, such cases may constitute a substantial proportion of all lung cancers diagnosed (up to 15%) within a screening program. This article reviews the impact and causes of false-negative lung cancer screening tests, including those related to radiologic evaluation, nodule management protocols, and management decisions made by multidisciplinary teams. Following a review of data from international screening studies, this article discusses the controversies within the screening literature surrounding the definition and classification of a false-negative lung cancer screening test and how data on false-negative rates should be captured and recorded. Challenges, such as avoiding overly cautious surveillance of lung nodules while minimizing overdiagnosis and investigation of indolent or benign lesions, are considered. Finally, the advantages and disadvantages of different approaches to dealing with false-negative results in lung cancer screening are discussed.

BackgroundLung cancer screening programs provide an opportunity to support smokers to quit, but the most appropriate model for delivery remains to be determined. Immediate face to face smoking cessation support for people undergoing screening can increase quit rates, but it is not known whether remote delivery of immediate smoking cessation counselling and pharmacotherapy in this context is also effective.Materials and MethodsIn a single-blind randomised controlled trial, smokers aged 55-75 years attending a Targeted Lung Health Check (TLHC) were allocated by day of attendance to receive either immediate telephone smoking cessation support (TSI) (starting immediately and lasting for 6 weeks) with appropriate pharmacotherapy, or usual care (very brief advice to quit and signposting to smoking cessation services) (UC). The primary outcome was self-reported 7-day point prevalence smoking abstinence at three months. Differences between groups were assessed using logistic regression.Results315 current smokers taking part in the screening programme, mean (SD) age 63(5.4) years, 48% female, were randomised to telephone smoking cessation (n=152) or usual care (n=163). The two groups were well-matched at baseline. Self-reported quit rates were higher in the intervention arm, 21.1% vs 8.9% (odds ratio [OR]: 2.83, 95% CI 1.44-5.61, p=0.002). Controlling for participant demographics, baseline smoking characteristics or the discovery of abnormalities on low dose CT scanning did not modify the effect of the intervention.ConclusionImmediate provision of an intensive telephone-based smoking cessation intervention, delivered within a targeted lung screening context, is associated with increased smoking abstinence at three months.Trial registrationThis study is registered online: ISRCTN12455871.Take home messageProviding immediate, telephone smoking cessation support with pharmacotherapy to smokers enrolled in a TLHC program increases quit rates in this population by almost a third.

Objectives The West London lung screening pilot aimed to identify early-stage lung cancer by targeting low-dose CT (LDCT) to high risk participants. Successful implementation of screening requires maximising participant uptake and identifying those at highest risk. As well as reporting pre-specified baseline screening metrics, additional objectives were to 1) compare participant uptake between a mobile and hospital-based CT scanner and 2) evaluate the impact on cancer detection using two lung cancer risk models. Methods From primary care records, ever-smokers aged 60–75 were invited to a lung health check at a hospital or mobile site. Participants with PLCOM2012 6-yr risk ≥1.51 % and/or LLPv2 5-yr risk ≥2.0 % were offered a LDCT. Lung cancer detection rate, stage, and recall rates are reported. Participant uptake was compared at both sites (chi-squared test). LDCT eligibility and cancer detection rate were compared between those recruited under each risk model. Results Of 8366 potential participants invited, 1047/5135 (20.4 %) invitees responded to an invitation to the hospital site, and 702/3231 (21.7 %) to the mobile site (p = 0.14). The median distance travelled to the hospital site was less than to the mobile site (3.3 km vs 6.4 km, p Conclusion Targeted screening is effective in detecting early-stage lung cancer. Similar levels of participant uptake at a mobile and fixed site scanner were demonstrated, indicating that uptake was driven by factors in addition to scanner location. The LLPv2 model was more permissive; recruitment with PLCOM2012 alone would have missed several cancers.

Respiratory symptoms were present in 18/39 (46.1%) and crackles were auscultated in 17 of 22 subjects (77.3%) undergoing physical examination. Past exposure to potential environmental triggers was noted in 21/39 (53.8%). Diagnostic bronchoalveolar lavage was performed in 7/39 (17.9%) and one patient underwent transbronchial lung cryobiopsy. After MDT discussion, ILD was concluded in 31/39 (79.5%) cases, of which 14/31 (45.2%) were diagnosed with IPF. In the IPF subgroup, antifibrotics were initiated in 7/14 (50%) of cases. In those diagnosed with other ILDs, immunomodulatory treatment was initiated in 2/25 (8%) subjects.ConclusionA large proportion of individuals with newly identified ILAs have an abnormal clinical examination and respiratory symptoms, consistent with the widely held suspicion that ILD is underdiagnosed in the community. Lung cancer screening in this demographic provides a unique opportunity to address this unmet health metric. Earlier identification of ILD, specifically IPF, allows institution of antifibrotic therapies proven to modify the natural history of the disease by preserving lung function and extending life. The cost-effectiveness of this approach for ILD screening warrants detailed evaluation.

Testicular macrocalcification is an uncommon finding when imaging the scrotum with ultrasonography (US). It is not normally a recognized risk factor for development of testicular malignancy, and patients are not usually offered follow-up US examinations or counseled for self-examination. This aspect is in distinction to patients with testicular microlithiasis (usually with an additional risk factor), who are offered follow-up on the assumption that microlithiasis is associated with malignancy. We report a series of 6 patients with predetermined testicular macrocalcification, with development of a malignancy on follow-up US. A 52-year-old man presented with progressive dyspnoea and productive cough over 4 months. He was in type 2 respiratory failure (pO2 7.99 kPa/60 mm Hg; pCO2 9.32 kPa/70 mm Hg) with fluid overload and was started on non-invasive ventilation and intravenous diuresis in intensive care. Physical examination was significant for his short neck, small jaw and body mass index of 50. Background Pilot lung cancer screening programmes in England have utilised a lung health check (LHC) model, comprising a nurse-led respiratory consultation, spirometry and a lung cancer risk calculation. We report the short-term outcomes of recommendations made to primary care for management of non-cancer incidental findings in a screening pilot. Methods 1542 participants from 17 general practice (GP) surgeries attended for a LHC between August 2018 and April 2019. Lung nodules, significant incidental lung findings on computed tomography (CT), unexplained respiratory symptoms, and suspected non-lung malignancies were managed within the screening programme. Participants with: i) ‘red-flag’ symptoms without lung cancer, ii) unexplained obstructive spirometry and respiratory symptoms, iii) significant coronary artery calcification (CAC) on CT, who were not known to have previously undergone cardiovascular risk stratification, and iv) significant, but non-urgent, non-lung incidental findings on CT, were referred to primary care. GP records were evaluated to establish outcomes. Results 165 primary care recommendations were made in 157/1542 (10.2%) individuals. Results below are from 16 GP practices and will be updated at the time of presentation. 49/1542 (3.2%) were referred to their GP for suspected undiagnosed chronic obstructive pulmonary disease (COPD), of whom 19/49 (38.8%) had a community-based respiratory review. 12/49 (24.5%) were newly diagnosed with COPD, and 5/49 (10.2%) commenced inhaler therapy. Of 52/1145 (4.5%) scanned participants with heavy CAC but without known ischaemic heart disease, 26/52 (50.0%) had a QRISK2 score (all >10%). Lipid-lowering therapy was commenced in 21/52 (40.4%). Echocardiography was recommended for 22/1542 (1.4%) participants with suspected cardiac disease, largely aortic valve calcification. 7/22 (31.8%) underwent echocardiography. Only 1/22 (4.5%) was deemed to require intervention for significant aortic stenosis. 7/1542 (0.5%) recommendations were made for other non-urgent/non-cardiothoracic incidental findings; none required further intervention. Conclusions A minority of participants required primary care management for incidental findings based on the West London lung screening study protocol. Although not all recommendations were implemented, incidental findings infrequently led to changes to patient management overall. Changes to patient management most commonly occurred as a result of recommendations for assessment for COPD and cardiovascular risk.

BackgroundThere are limited studies about testicular microlithiasis (TML) and background information such as health, lifestyle, and socioeconomic status.PurposeTo assess the prevalence of TML in relation to socioeconomic status and ethnicity.Material and MethodsFrom a database of scrotal ultrasound examinations in a single institution, all men who underwent routine ultrasound examinations for a variety of symptoms from 1998 to 2015 were included. Skilled observers performed all examinations, and presence of any form of intra-testicular calcification, including TML, was recorded on the examination report and a representative image obtained and stored. A total of 1105 cases with TML were reviewed and random sample of 1105 controls from the same database was also reviewed. Demographics were recorded including ethnicity (white, black, and others) and socioeconomic groups (IMD Quintile).ResultsBlack men had increased prevalence of TML (odds ratio [OR] = 2.17, 95% confidence interval [CI] = 1.72–2.75) compared with white men. Among the 1105 TML cases, 423 (38.3%) were white, 273 (24.7%) black, 152 (13.8%) had other ethnicities, and 257 (23.2%) had no ethnicity recorded. In the control group of 1105 men without TML, 560 (50.7%) were white, 171 (15.5%) black, 111 (10.0%) had other specified ethnicities, and 263 (23.8%) had no ethnicity recorded. Men from the most deprived socioeconomic groups had higher prevalence of TML than men in the most affluent groups, with a trend in OR from the least deprived to the most deprived group.ConclusionPathogenesis and clinical relevance of TML is unknown but our results point towards possible ethnic and socioeconomic variation in the underlying causes of TML.

A 22 year old woman in a new sexual relationship presented to her general practitioner with slightly painful intermenstrual bleeding for 10 days. She had pelvic tenderness but no cervical excitation on bimanual examination. The doctor suspected that the bleeding was a side effect of the oral contraceptive pill, started three months earlier. However, as he was unable to rule out pelvic inflammatory disease (PID), endocervical swab samples were taken to test for the sexually transmitted infections caused by Neisseria gonorrhoeae and Chlamydia trachomatis , and broad spectrum antibiotics prescribed, after which the symptoms resolved. The swab sample confirmed the presence of C trachomatis . PID is due to infection of the upper female genital tract resulting in a wide range of pelvic pathology, from mild endometritis to pelvic peritonitis. Causative organisms are either sexually transmitted (such as C trachomatis , N gonorrhoeae , Mycoplasma genitalium ) or endogenous vaginal organisms (for example, Bacteroides species) that ascend into the pelvic area from the lower genital tract through the cervix. #### How common is PID? PID may present with classic symptoms and signs or with mild and subtle symptoms that may be easy to miss, according to a review of the evidence in national guidelines written by the British Association for Sexual Health and HIV.2 The best recent data on easily missed PID come from a retrospective audit in a walk-in centre for sexual health.
