Abstract

Objective: Anterior cervical discectomy and fusion is a common procedure performed by spine surgeons with rare complications and high treatment success. Late presentation of retropharyngeal abscess in patients with a history of anterior cervical discectomy and fusion is rare but can have devastating consequences. There is a paucity of data to guide medical and surgical management of retropharyngeal abscess in these patients. Methods: We discuss 7 patients who presented to our institution with a late retropharyngeal abscess after having a history of anterior cervical discectomy and fusion. A review and description of the current literature regarding treatment and outcomes is described. Results: Seven patients presented to our institution with a retropharyngeal abscess ranging from 10 months to 7 years after undergoing anterior cervical discectomy and fusion. All patients received at least a 6-week course of appropriate intravenous antibiotics. Only one patient had their initial ACDF instrumentation removed at the time of presentation for the abscess. Four out of the 7 patients were treated with irrigation and debridement in addition to intravenous antibiotics, whereas 3 patients were treated with no surgery and intravenous antibiotics alone. All patients were asymptomatic at final follow up. Conclusions: Late retropharyngeal abscess after anterior cervical discectomy and fusion is a rare complication. Surgical management should be considered along with long term antibiotics. Removal of implants may not be necessary for infection resolution. Antibiotic treatment alone may be indicated for patients who are not septic, do not have airway compromise, or and can be considered for poor surgical candidates. [ABSTRACT FROM AUTHOR]

37. Upper-airway obstruction after multilevel cervical corpectomy for myelopathy

Abstract

Surgery on the cervical spine (area of the neck) may result in postoperative swelling with obstruction of the airway, requiring immediate intervention. A review was undertaken of seven patients who required reintubation for upper airway obstruction following operation on the cervical spine Over a 15-year period, 133 such operations had been performed. All patients underwent surgery for cervical spondylitic myelopathy (arthritis of the cervical vertebrae with spinal cord abnormality). The cause of the airway obstructions was believed to be swelling, rather than hematoma (a mass of blood), and two patients died due to complications of the airway obstruction. Several risk factors common to these patients were identified: all had moderate to severe myelopathy (spinal cord abnormality) due to compression. Therefore, these patients required a two- and three-level procedure suggesting the presence of severe disease behind, and not just in front of, the main portion of the neck vertebrae. The multilevel procedure requires longer anesthesia and operating time than the single-level procedure. Six of the seven patients had a history of heavy cigarette smoking, and the seventh patient had a 30-year history of asthma. It is suggested that extra caution should be utilized in the postoperative care of patients undergoing multilevel cervical spine surgery who have a history of these conditions. 48. Additive Effects of Exogenous IL-12 Supplementation and Antibiotic Treatment in Infection Prophylaxis#

Abstract

The increasing clinical incidence and host risk of open fracture-associated infections, as well as the reduced effectiveness of conventional antibiotics to treat such infections, have driven the development of new therapies for the prophylaxis of open fracture-associated infections. We investigated percutaneous supplementation of a natural cytokine (i.e. interleukin 12p70 or IL-12) at an open fracture site to reduce open fracture-associated infections. We also determined the efficacy of the combination therapy of IL-12 and conventional antibiotic therapy in the prophylaxis of open fracture-associated infections. An open femur fracture infection model was produced by direct inoculation of a clinical isolate of Staphylococcus aureus after creating a femur fracture using rats. The animals were assigned to one of four groups: no drug administration, percutaneous supplementation of IL-12, intraperitoneal administration of the antibiotic ampicillin, or percutaneous IL-12 in combination with intraperitoneal ampicillin. Animals were euthanized at post-operative days 6, 10, 14, and 21. Percutaneous IL-12 led to a reduction in infection at post-operative days 6 and 10. For the first time, exogenous IL-12 was found to have additive effects in the prevention of infection when combined with conventional treatment (i.e. antibiotic therapy). Combination therapy of ampicillin and IL-12 substantially reduced the infection rate at post-operative day 6 and also decreased the time needed for complete inhibition of infection. Therefore, exogenous IL-12, providing a mechanism of protection independent of antibiotic resistance, complements the routine use of antibiotics.
